Abstract:We study the persistence properties in a simple model of two coupled interfaces characterized by heights h(1) and h(2) , respectively, each growing over a d -dimensional substrate. The first interface evolves independently of the second and can correspond to any generic growing interface, e.g., of the Edwards-Wilkinson or of the Kardar-Parisi-Zhang variety. The evolution of h(2) , however, is coupled to h(1) via a quenched random velocity field. In the limit d-->0 , our model reduces to the Matheron-de Marsily model in two dimensions. For d=1, our model describes a Rouse polymer chain in two dimensions advected by a transverse velocity field. We show analytically that after a long waiting time t(0) -->infinity , the stochastic process h(2) , at a fixed point in space but as a function of time, becomes a fractional Brownian motion with a Hurst exponent, H2 =1- beta(1) /2 , where beta(1) is the growth exponent characterizing the first interface. The associated persistence exponent is shown to be theta(2)(s) =1- H2 = beta(1) /2 . These analytical results are verified by numerical simulations.

Abstract:The specificity of the synthetic substrate Gly-[L-Asp]4-L-Lys 2-naphthylamide originally developed for the assay of enteropeptidase (EC 3.4.21.9), was investigated with partially purified aminopeptidase. Our results indicate that, not only enteropeptidase, but also the concerted action of the aminopeptidases of the rat small intestine, can rapidly release 2-naphthylamine from the substrate. A previously undescribed, highly active, dipeptidylaminopeptidase, which hydrolyses a Gly-Asp dipeptide from the N-terminus of the substrate, was detected in rat small intestine. The resulting [L-Asp]3-L-Lys 2-naphthylamide fragment is then degraded by a combination of aminopeptidase A and N to yield free 2-naphthylamine. Thus the present substrate cannot be regarded as being specific for enteropeptidase, and its use leads to an over-estimation of enteropeptidase activity in homogenates and extracts of intestinal tissue. In order to prevent this non-specific hydrolysis by aminopeptidases, stereoisomeric substrates with the sequence L-Ala-D-Asp-[L-Asp]3-L-Lys methyl ester, D-Ala-[L-Asp]4-L-Lys methyl ester and L-Ala-[Asp]4-L-Lys methyl ester were synthesized and tested as alternative substrates by their ability to inhibit the enteropeptidase-catalysed activation of trypsinogen.

Abstract:Intro: The assessment of scar outcomes is important to both patient care and research focused on understanding the results of medical and surgical interventions. The Vancouver Scar Scale and Patient and Observer Scar Assessment Scale are validated and simple instruments to assess scars. However, these subjective scales have shortcomings. The VSS fails to capture patient perception and has indeterminate validity and reliability. The POSAS captures patient perception, but the observer scale has been shown to have moderate amounts of inter-rater variability. Studies highlighting the ability of objective scar assessment tools to produce reliable and reproducible results are needed. In this study, we aimed to validate the use of the Fibrometer ® , Elastimeter ® , and SkinColorCatch ® as an objective adjunct in the assessment of hypertrophic scar and keloid outcomes. Methods: This was a prospective single-center study which assessed patient scars using the Vancouver Scar Scale, the Patient and Observer Scar Assessment scale, and the aforementioned objective study tools. Correlations between the different methods of scar assessment were measured. Results: The Fibrometer ® and SkinColorCatch ® showed significant correlations with the VSS total and the Observer POSAS total. The Elastimeter ® showed significant correlations with both the Patient and Observer POSAS totals. Unexpected correlations between Elastimeter ® measurements and the vascularity/pigmentation of scars indicate that scoring of these categories may be influenced by how severe the scar looks to the observer subjectively, further necessitating the need for reliable objective scar assessment tools. Conclusion: These results highlight the ability of these devices to assess scars and demonstrate their potential in serving as an important adjunct to previously validated scar assessment scales.

Abstract:Burn scars are frequently accompanied with sensory deficits often remaining present months or even years after injury. Clinimetric properties of assessment tools remain understudied within burn literature. Tactile sense of touch can be examined with the touch pressure threshold (TPT) method using the Semmes Weinstein monofilament test (SWMT). There is in recent research no consensus on the exact measurement procedure when using the SWMT. The aim of this paper was to determine the interrater and intrarater reliability of TPT within burn scars and healthy controls using the 'ascending descending' measurement procedure. We used the newly developed guidelines for reporting reliability and agreement studies (GRRAS) as a basis to report this reliability study. In total 36 individuals were tested; a healthy control group and a scar group. The interrater reliability was excellent in the scar group (ICC=0.908/SEM=0.21) and fair to good in the control group (ICC=0.731/SEM=0.12). In the scar group intrarater ICC value was excellent (ICC=0.822/SEM=0.33). Within the control group also an excellent intrarater reliability (ICC=0.807/SEM=0.27) was found. In conclusion this study shows that the SWMT with the 'ascending descending' measurement procedure is a feasible and reliable objective measure to evaluate TPT in (older) upper extremities burn scars as well as in healthy skin.

Abstract:Objective: The aim was to determine the efficacy of shear wave elastography (SWE) in assessing skin stiffness and aiding in the diagnosis of patients with systemic sclerosis (SSc). Methods: A total of 66 patients with SSc, 100 healthy individuals and 27 patients with SSc-like disorders were included. SWE was performed at 17 modified Rodnan skin score (mRSS) measurement sites. The correlation between SWE and clinical profiles was assessed, and the diagnostic value of SSc was explored. Results: The SWE values at all 17 mRSS sites were significantly higher in SSc than in the healthy group [54.95 (45.95, 66.55) vs 41.10 (39.18, 45.45) m/s, P < 0.001]. For clinically uninvolved sites (mRSS = 0) of patients with SSc, 11 of 17 sites showed significantly higher SWE values compared with healthy controls. SWE was positively correlated with total mRSS (r = 0.783, P < 0.001), the European Scleroderma Study Group disease activity index (r = 0.707, P < 0.001) and histological collagen deposition (r = 0.749, P = 0.013). SWE effectively distinguished patients with SSc from patients with SSc-like disorders (area under the curve, AUC = 0.819). Use of SWE-detected skin sclerosis showed a significantly higher sensitivity compared with 1980 ACR criteria [0.818 (95% CI 0.709, 0.893) vs 0.727 (95% CI 0.610, 0.820), P = 0.031]. Conclusion: SWE correlates well with disease activity and collagen deposition in the skin, provides greater reliability than mRSS and aids in the diagnosis of SSc. SWE could be considered as a convenient and reliable quantitative tool for assessing skin sclerosis and disease progression in SSc.

Abstract:SignificancePost-burn scars and scar contractures present significant challenges in burn injury management, necessitating accurate evaluation of the wound healing process to prevent or minimize complications. Non-invasive and accurate assessment of burn scar vascularity can offer valuable insights for evaluations of wound healing. Optical coherence tomography (OCT) and OCT angiography (OCTA) are promising imaging techniques that may enhance patient-centered care and satisfaction by providing detailed analyses of the healing process.AimOur study investigates the capabilities of OCT and OCTA for acquiring information on blood vessels in burn scars and evaluates the feasibility of utilizing this information to assess burn scars.ApproachHealthy skin and neighboring scar data from nine burn patients were obtained using OCT and processed with speckle decorrelation, Doppler OCT, and an enhanced technique based on joint spectral and time domain OCT. These methods facilitated the assessment of vascular structure and blood flow velocity in both healthy skin and scar tissues. Analyzing these parameters allowed for objective comparisons between normal skin and burn scars.ResultsOur study found that blood vessel distribution in burn scars significantly differs from that in healthy skin. Burn scars exhibit increased vascularization, featuring less uniformity and lacking the intricate branching network found in healthy tissue. Specifically, the density of the vessels in burn scars is 67% higher than in healthy tissue, while axial flow velocity in burn scar vessels is 25% faster than in healthy tissue.ConclusionsOur research demonstrates the feasibility of OCT and OCTA as burn scar assessment tools. By implementing these technologies, we can distinguish between scar and healthy tissue based on its vascular structure, providing evidence of their practicality in evaluating burn scar severity and progression.

Reliability and Validity Testing of the Patient and Observer Scar Assessment Scale in Evaluating Linear Scars after Breast Cancer Surgery

Abstract:BACKGROUND: The Patient and Observer Scar Assessment Scale is a promising new method incorporating observer and patient ratings in evaluating burn scars. The authors compared this tool to the Vancouver Scar Scale in a cohort of women with linear scars from breast cancer surgery.METHODS: Twenty women with newly diagnosed breast cancer were prospectively accrued. Thirty-one scars were evaluated. The median time from surgery to scar assessment was 8 weeks (range, 3 to 25 weeks). Observer assessment was performed by three independent raters using the Vancouver scale and the observer component of the new tool. Patient self-assessment was performed using the patient component of the tool. Internal consistency, interobserver reliability, and convergent validity were examined.RESULTS: Internal consistency was acceptable for the Vancouver scale and both components of the new tool (Cronbach's alpha, 0.71, 0.74, and 0.77, respectively). Interobserver reliability was substantial with both the Vancouver scale and the observer tool (average measure intraclass coefficient correlation, 0.78 and 0.60, respectively). The observer tool and Vancouver scale correlated significantly with each other (p < 0.001), but only the observer tool correlated well with patients' ratings (p = 0.04).CONCLUSIONS: In surgical scar assessment, the new Patient and Observer Scar Assessment Scale and Vancouver Scar Scale were both associated with acceptable internal consistency and interobserver reliability. The new tool is more comprehensive and has higher correlation with patients' ratings. These findings support the use of the new tool as a reliable, valid, and comprehensive approach to assess linear surgical scars.

The effects of shock wave therapy applied on hypertrophic burn scars: a randomised controlled trial

Abstract:Introduction: A wide variety of non-invasive treatments has been proposed for the management of hypertrophic burn scars. Unfortunately, the reported efficacy has not been consistent, and especially in the first three months after wound closure, fragility of the scarred skin limits the treatment options. Extracorporeal shock wave therapy (ESWT) is a new non-invasive type of mechanotherapy to treat wounds and scars. The aim of the present study was to examine the objective and subjective scar-related effects of ESWT on burn scars in the early remodelling phase. Material and methods: Evaluations included the Patient and Observer Scar Assessment Scale (POSAS) for scar quality, tri-stimulus colorimetry for redness, tewametry for trans-epidermal water loss (TEWL) and cutometry for elasticity. Patients were randomly assigned to one of two groups, the low-energy intervention group or the placebo control group, and were tested at baseline, after one, three and six months. All patients were treated with pressure garments, silicone and moisturisers. Both groups received the ESWT treatment (real or placebo) once a week for 10 weeks. Results: Results for 20 patients in each group after six months are presented. The objective assessments showed a statistically significant effect of ESWT compared with placebo on elasticity ( P = 0.011, η2 P=0.107) but revealed no significant effects on redness and TEWL. Results of the clinical assessments showed no significant interactions between intervention and time for the POSAS Patient and Observer scores. Conclusion: ESWT can give added value to the non-invasive treatment of hypertrophic scars, more specifically to improve elasticity when the treatment was already started in the first three months after wound closure. Abstract:Background: Current scientific evidence on the effect of Extracorporeal Shock Wave Therapy (ESWT) as adjunctive treatment for burn scars is scarce. However preliminary evidence, indicates it might prove a useful tool. Materials and methods: A prospective, randomized, controlled study was conducted from February 2017 to February 2019. Patients with burn scars were divided into two groups with twenty patients per group. The control group received the standard treatment for burn scars. The ESWT group received the standard treatment and treatment of burn scars with ESWT 512 impulses of 0.15mJ/mm2 in each session, twice per week for 4 weeks. We assessed the appearance of scar with the Vancouver Scar Scale (VSS), pruritus and pain with Visual Analog Scale (VAS) before the start of the treatment and at 2 weeks and 5 months after the treatment. Results: Both groups showed improvements in all variables through the study. However, these improvements were only statistically significant for the VSS at the 6th month for the control group and VSS and VAS pain and pruritus for the ESWT group. Nonetheless the results failed to show statistically significant differences between the ESWT and the control group neither at two weeks after treatment nor at 5 months after treatment. Conclusion: Our study questions the relevance of ESWT as adjunctive treatment for burn scars as far as outward appearance, pain and pruritus as end-results are concern. Nonetheless, further studies are required to accurately assess the potential benefits of ESWT as an adjunctive treatment for burn scars.
Intra- and inter-evaluator reliability of the MyotonPRO for the assessment of the viscoelastic properties of caesarean section scar and unscarred skin

Abstract:Background: The clinical evaluation of viscoelastic properties of the Caesarean section (C-section) scar, such as stiffness and elasticity, is usually carried out using subjective scales and palpation techniques. There is currently no reliable and valid tool that objectively quantifies these properties. The MyotonPRO could fill this gap. Materials and methods: Nineteen healthy women aged between 21 and 40 years with C-section scars participated in this reliability study. Two points, one on the scar and one on unscarred skin, were measured four times successively with the MyotonPRO by three independent evaluators on the same day. The intra-class correlation (ICC) coefficients were estimated using a two-factor ANOVA to determine the inter- and intra-rater reliability. The capacity of the MyotonPRO to discriminate the viscoelastic properties of the C-Section scar against unscarred skin was assessed using the Wilcoxon signed rank test. Results: The intra- and inter-rater reliability of the viscoelastic property measurements was good to excellent (ICC 0.99-1.00 and 0.87-0.98, respectively). There was no significant difference between C-section scar and unscarred skin in terms of elasticity (P = .737). Significant differences between C-section scars and unscarred skin tissue were observed for tone (P < .001), stiffness (P < .001), creep (P < .001), and mechanical stress relaxation time (P < .001). Conclusion: The MyotonPRO is a reliable tool for an objective measurement of the viscoelastic properties of the C-section scar and unscarred skin. The MyotonPRO can discriminate the viscoelastic properties of the C-section scar against the unscarred skin, for tone, stiffness, creep and relaxation times, but not for elasticity.
Biomechanical alterations in normal skin and hypertrophic scar after thermal injury

Abstract:Functional recovery after burn injury can be significantly limited by scar contraction and contracture. Mechanisms to explain these problems are not well described. Surface (shear) waves were generated in normal skin and hypertrophic burn scar with a newly described device. The velocity of propagation was used to examine biomechanical changes after burn injury. Shear wave velocity was markedly higher in scar and slightly higher in injured tissue without obvious scarring, verifying increased skin stiffness. Normal skin anisotrophy was exaggerated in scar tissue and not seen in the uninjured skin adjacent to hypertrophic scar. This result suggests the reorganization of normal tissue attempts to compensate for scar contraction. Measurements of shear wave velocity objectively document changes in skin properties that affect the recovery from thermal injury. Use of this or alternate technologies may increase understanding of postburn skin dysfunction, improving our ability to treat such patients.

Abstract:An endophytic fungus Aspergillus terreus, var. boedijnii (Blochwitz) was isolated from red marine alga Laurencia ceylanica, J. Agardh, cultured in large scale and extracted with EtOAc. The above-mentioned extract yielded a new butyrolactone, 3-hydroxy-4-(4-hydroxyphenyl)-5-methoxycarbonyl-5-(4-hydroxy-3-formylbenzyl)-2,5-dihydro-2-furanone (1), along with the previously reported nine compounds, butyrolactone-1 (2), 6-hydroxymellin (3), (3 R, 4 R)-6,7-dimethoxy-4-hydroxymellin (4), (+)-territonin (5), (+)-territonin-A (6), (+)-asterrelenin (7), (+)-terrein (8), oleic acid (9) and glucopyranosyl-β-sitosterol (10), on column and preparative thin-layer chromatography. Compounds 1–8 were subjected to β-glucuronidase inhibitory activity test, and 1 showed a remarkable activity, while 2 and 7 showed moderate activity.

Abstract:Abstract Introduction With limited dermal function at the recipient site, fibrosis is expected following split-thickness skin grafting. Additional hypertrophic scar may develop within the graft, along seams and margins or at the donor site. This study focuses on the mechanical changes in the skin following split-thickness skin grafting as assessed by a non-invasive negative pressure device and atomic force microscopy to describe gross and sub-cellular mechanical properties in the differing skin states post-grafting. Methods Participants with full-thickness burn injuries acutely managed with a split-thickness skin graft, closed donor and graft sites without any previous steroid injection or laser therapeutic interventions and a suitable control site were recruited. A negative pressure device was applied to grafted, donor, and uninjured contralateral control sites and response recorded to vacuum pressure and release to calculate Young’s elasticity modulus. A 4mm punch biopsy was harvested from each labelled site and immediately processed using atomic force microscopy using silicon cantilevers conjugated with a spherical glass tip (radius 2.5 μm;) in a custom 3D printed frame to determine Young’s elasticity based on the Hertz fit model (fig 1a-b). Results A total of 5 participants (3 male, mean age 32.4 range 21-54, 152-427 days post-injury) have been recruited for the study thus far. Measurements were taken on the thigh of 4 participants and on the abdomen of 1. Atomic force microscopy data demonstrates lowest Young’s elasticity modulus (mean 1.49 ± 0.12 kPa) at the control sites (fig 1b graft 3: x-axis frequency, y-axis YM) with highest Young’s elasticity modulus (mean 2.70 ± 0.10 kPa) at the grafted sites. The donor sites are in the middle of the graft and control, (mean 1.98 ± 0.88 kPa). This trend is in concordance with clinical observation and elasticity probe measurement, where the graft is the toughest and the donor and control are similar (fig 1c). Conclusions Initial elasticity measurement by a non-invasive vacuum device has been in agreement with atomic force microscopy assessment. Further sampling is required to verify preliminary results, simulate resting tension in the skin and assess change following intervention. Applicability of Research to Practice Valid and reliable non-invasive measures will be informative of treatment effectiveness. Sub-cellular assessment of mechanical changes in skin graft and scar may direct therapeutic interventions.

Abstract:The aim of the study described here was to investigate the role of viscoelasticity in assessing muscle fibrosis and inflammation in a rat model of contusion using quantitative shear wave elastography (SWE). Unilateral gastrocnemius muscle contusion was induced in 32 male rats using an impactor apparatus. The contralateral muscles served as the control group. SWE was applied to the control group and rats 1, 3, 14 and 21 d after successful modeling (each time point group, n = 8). Histologic features were used as reference standards. The degree of fibrosis was moderately correlated with shear wave speed (r = 0.53), whereas the degree of inflammation was well correlated with shear wave dispersion (SWD) slope (r = 0.74). The area under the receiver operating characteristic curve (AUC) for the dispersion slope for muscle inflammation and fibrosis assessment was 0.87 (95% confidence interval: 0.705-0.963), which exceeded that of the shear wave speed (0.68, 95% confidence interval: 0.494-0.834). The larger decline in dispersion slope in the fibrotic stage than in the inflammation stage (1-d group vs. 14-d group or 21-d group, p < 0.05) indicated better predictive performance than the shear wave speed. Abstract:ABSTRACT OBJECTIVE To review the clinical and scientific literature on the subjective ways of assessing burn scars and describe their main characteristics. DATA SOURCES The Latin American, Caribbean Health Sciences Literature, Nursing Database, PubMed, CINAHL, and Scopus and Web of Science databases were used to search for studies published between 2014 and 2018 using descriptors in Portuguese, Spanish, and English. STUDY SELECTION After establishing the research question and the location and definition of the studies, as well as accounting for differences among databases and application of filters based on inclusion and exclusion criteria, 886 references remained. DATA EXTRACTION Investigators reviewed the titles and abstracts of the sample and selected 188 relevant studies for full review. DATA SYNTHESIS Twenty-six subjective forms of assessment were found; most research concerned the Patient and Observer Scar Assessment Scale and the Vancouver Scar Scale. CONCLUSIONS The Patient and Observer Scar Assessment Scale and the Vancouver Scar Scale are the most common scales for assessing burn scars and have similar evaluation points such as vascularization, pliability, pigmentation, and height, which are the main parameters that contribute to the general assessment and severity of a scar. There is a need to improve instructions for application of the scales to facilitate better understanding and improve agreement among evaluators.

Abstract:The effect of the "Patient and Observer Scar Assessment Scale" (POSAS) and "Vancouver Scar Scale" (VSS) on patients' quality of life and their correlation with objective scar assessment tools, such as the Cutometer®, is not fully elucidated. In addition, long-term results of the dermal substitute Matriderm® used in combination with split-thickness skin grafting (STSG) remain unclear. We evaluated burn scars of 45 patients at least 2 years postburn injury using the Cutometer® MPA 580, the VSS, and the POSAS with three additional questions regarding quality of life and correlated the results. Study groups were: 1) scars following conservative treatment, 2) scars following STSG, and 3) scars following STSG in combination with Matriderm®. Cutometer® measurements demonstrated better elastic qualities in the Matriderm® group compared with the STSG group. VSS and extended POSAS were rated best for the conservative group, followed by the STSG group and the Matriderm® group. There was a significant correlation between POSAS and VSS, quality of life and the objective Cutometer® measurements. Conservatively treated superficial dermal burns do not reach the elastic qualities of healthy skin, and the use of Matriderm® significantly improves the long-term elastic qualities of STSG in deep dermal and full-thickness burns 2 years post injury. Results from the VSS and the POSAS correlate with restrictions in the quality of life of patients and also with objective Cutometer® measurements and are therefore useful tools in scar evaluation following burn injury.

Abstract:Shear Wave Elastography (SWE) is an objective and non-invasive method widely used to quantify the tissue solidity. However, there are concerns about the accuracy of the skin SWE results due to the low signal-to-noise ratio (SNR) caused by subcutaneous fat, muscle and bone. This article analyzed the reproducibility of the result for skin SWE and therefore evaluated the availability of SME for skin elasticity involved diseases. Thirty volunteers (mean age: 37 +/- 12 years) were selected. SWE were taken on the skin of abdomen and the middle tibia in order to assess the impact of fat, muscle and bone on SWE results. Skin in the area of anterior and lateral tibia marked with seven parallel lines, and each line indicated an identical thickness of the subcutaneous fat from 1-7mm. Intra-class correlation coefficients (ICC) were used to evaluate the intra-observer and inter-observer reproducibility. The solidity of abdominal skin showed soft and small individual differences (12.4 +/- 2.7 kPa), whereas high shear moduli (25-48 kPa) were observed in the skin above tibia and tibialis anterior muscle. When the subcutaneous fat was thicker than 3mm(>= 3), we obtained excellent intra-observer reproducibility (ICC range 0.78-0.98) and inter-observer reproducibility (ICC range 0.75-0.98). The thickness of subcutaneous fat could affect the reproducibility of skin SWE. The further study on skin SWE standardization should be taken.
