Abstract:Large-scale epidemics of arboviruses, such as dengue, have heightened societal awareness regarding the necessity of combating the primary transmission vectors. Equally critical is the identification of environmental conditions and variables that influence vector population dynamics. Aedes aegypti, the primary vector of arboviruses such as dengue and Zika in Brazil, is closely associated with the climatic and geographical conditions of urban environments. This study examines the relationship between precipitation and confirmed dengue cases in Recife (Brazil), employing regression and quantile analyses to evaluate the influence of meteorological conditions on the disease's spread. The findings reveal a direct correlation between monthly averages of precipitation and confirmed cases, although this is apparent only when excluding years of epidemic peaks. The highest number of cases generally aligns with the rainy season, and the lowest with the dry season, with weak, moderate and strong precipitation events being closely linked to increased dengue incidence. However, notable discrepancies were identified: four out of six major outbreaks occurred in drier months, challenging the assumption of a straightforward relationship between rainfall and dengue incidence. These findings underscore the multifaceted nature of dengue dynamics, suggesting that while precipitation plays a significant role, other factors, including serotype circulation and broader climatic phenomena, are equally critical in driving outbreaks. This complexity highlights the need for a more comprehensive understanding of the mechanisms influencing dengue epidemics.

Abstract:Background: Dengue is a major problem in Brazil. Epidemiological and clinical aspects were characterized in patients from two epidemics which occurred in Mato Grosso do Sul, Brazil. Methods: Dengue cases were classified according to the 2009 WHO criteria, tested by serological and molecular biology tests and analysed for nonstructural protein 1 (NS1) antigenemia. Results: Dengue was confirmed in 78.7% (48/61) and 75.6% (118/156) of the cases studied in 2010 and 2013, respectively. DENV-1 and DENV-2 were the serotypes involved in the 2010 epidemic and DENV-4 in the 2013 one. Most of the cases were classified as dengue without warning; however, severe dengue was observed in 18.7% (9/48) of the cases in 2010 and less observed in DENV-4 cases. NS1 levels were higher in patients with dengue with warning signs and severe dengue in 2010. Circulating aspartate aminotransferase (AST) and alanine transferase (ALT) were altered in all groups, independently of the infecting serotype or epidemic. Patients with DENV-1 and DENV-2 presented significant lower monocyte counts when compared to patients with DENV-4. An inverse correlation was found between platelet count, leucocytes, monocytes and NS1 levels. Conclusions: Epidemics caused by the prevalence of distinct DENV serotypes had different impacts and clinical characteristics in a same scenario and, despite the occurrence of secondary infections, the DENV-4 emergence was not associated with severe cases.

Abstract:This study aims to perform the first molecular and clinical-epidemiological analysis of dengue cases in Divinopolis, MG, Brazil. Data from 4,110 cases of dengue were accessed and 190 clinical samples were collected for molecular analyses. In this study, 2.7% of the men and 3.0% of the women were admitted to hospital. There was no association between gender and hospital admission. The symptoms observed in this study are according to the Health Ministry, but fever was present in 82.2% and not in 100% of cases. The chance of hospital admission was 1.55 higher in patients with any kind of bleeding (334) and 2.4% of individuals without bleeding were also hospitalized due to other warning signs. In the molecular analyses, 23% of the samples were positive for DENV. DENV-2 and DENV-3 were identified in 2010, DENV-3 in 2011, DENV-1 in 2012, and DENV-1 and DENV-4 in 2013. DENV detection was possible in samples with only one day of symptoms. This first report of dengue data in Divinópolis provided more insight into the viral types and effects of disease in the city, confirming the need for caution in assessing cases of suspected dengue and for revision of the criteria proposed by the Health Ministry to classify cases of the disease.

Abstract:A literature survey and analysis was conducted to describe the epidemiology of dengue disease in Brazil reported between 2000 and 2010. The protocol was registered on PROSPERO (CRD42011001826: http://www.crd.york.ac.uk/prospero/display_record.asp?ID=CRD42011001826). Between 31 July and 4 August 2011, the published literature was searched for epidemiological studies of dengue disease, using specific search strategies for each electronic database. A total of 714 relevant citations were identified, 51 of which fulfilled the inclusion criteria. The epidemiology of dengue disease in Brazil, in this period, was characterized by increases in the geographical spread and incidence of reported cases. The overall increase in dengue disease was accompanied by a rise in the proportion of severe cases. The epidemiological pattern of dengue disease in Brazil is complex and the changes observed during this review period are likely to have been influenced by multiple factors. Several gaps in epidemiological knowledge regarding dengue disease in Brazil were identified that provide avenues for future research, in particular, studies of regional differences, genotype evolution, and age-stratified seroprevalence.SYSTEMATIC REVIEW REGISTRATION: PROSPERO registration number: CRD42011001826.

Socio-demographic inequalities in the clinical characteristics of dengue haemorrhagic fever in the city of Rio de Janeiro, Brazil, 2007-2008

Abstract:In 2007-2008, the city of Rio de Janeiro underwent an epidemiological change, with increases in the incidence in children and in severe forms of dengue. To describe the clinical profile and spatial distribution of dengue we performed an ecological study based on dengue surveillance data using the Brazilian classification (2005): dengue fever, dengue haemorrhagic fever (DHF) and dengue with complications. χ 2 test was used to describe the clinical and socio-demographic variables (P < 0.05). Spatial distribution of incidence and case-fatality was explored with thematic maps, Moran and Geary indices (P < 0.05). Of the total of 151 527 dengue cases, 38 808 met the inclusion criteria; 42.4% <18 years; 22.9% dengue with complications and 2.7% DHF. Case-fatality was higher in infants (1.4%) and in DHF (7.7%). Bleeding was more frequent in adolescents and adults while plasma leakage was more common in preschoolers and schoolchildren. The highest incidence was found in the West Zone of the city, in a different area from that of the worst case-fatality (P < 0.05). Although the incidence of DHF was higher in schoolchildren, infants showed higher case-fatality. The area with the highest case-fatality did not present the highest incidence, which suggests problems in the organization of health services.

Abstract:Dengue virus, the etiological agent of dengue fever (DF) occurs in four genetically distinct serotypes (DENV1-4), being transmitted by female Aedes mosquitoes. DF incidence is increasing in Brazil, following vector dispersal, proliferation and DENV serotypes introduction, co-circulation and substitution. Medium- and small-sized cities in Sao Paulo State, such as Marilia (Midwest region), have been affected by huge epidemics. To understand the evolution of DENV epidemics in medium-sized cities, in this study a historical data on DENV incidence (2000-2015) in Marilia, was evaluated. Previous studies disclosed regional and specific DF outcomes associated with 2007 outbreak in that city, when co-circulating DENV1 and DENV3 presented different hematological profiles. In this study, characteristics of 2007 DENV epidemics were compared to the epidemiological, hematological and demographic outlines of the major outbreak of DENV1 in Marilia in 2015. DENV1 genetic diversity was assessed through capsid and pre-membrane junction encoding gene (CprM) sequencing. The results revealed circulation of DENV1 serotype from 2007 to 2015, with epidemics occurring every three-years until 2013 and then, increasing yearly. There were significant differences in hematological profiles of DENV1 patients between 2015 and 2007. CprM showed DENV1 genetic variability in 2015, contrasting with the unique sequence pattern in 2007. These results reinforce the regional and temporal characteristics of DENV epidemics that need local public health research to improve care for people and to limit the spread of new serotypes/genotypes to uninfected areas.

Abstract:We conducted a spatial case-control study nested in a dengue incidence cohort to explore the role of the spatial and socioeconomic factors in the proportion of symptomatic (cases) and inapparent primary dengue virus infections (controls). Cohort participants were children and adolescents (2 to 16 years of age) at the beginning of the follow-up. Case definitions were, for symptomatic cases, fever plus a positive lab result for acute dengue (NS1, RT-PCR, ELISA IgM/IgG), and for inapparent infection a positive result for dengue IgG (ELISA) in subjects without symptoms and with a previously negative result at baseline. The covariates included sociodemographic factors, residential location, and socioeconomic context variables of the census tracts of residence of cases and controls. We used principal component analysis to reduce the contextual covariates, with the component values assigned to each one based on their residences. The data were modeled in a Bayesian context, considering the spatial dependence. The final sample consisted of 692 children, 274 cases and 418 controls, from the first year of follow-up (2014-2015). Being male, older age, higher educational level of the head of the family and having a larger number of rooms in the household were associated with a greater chance of presenting dengue symptomatic infection at the individual level. The contextual covariates were not associated with the outcome. Inapparent dengue infection has extensive epidemiological consequences. Relying solely on notifications of symptomatic dengue infections underestimates the number of cases, preserves a silent source of the disease, potentially spreading the virus to unaffected areas.

Abstract:In this study, we propose an indoor navigation system that utilizes visible light communication technology, which employs LED lights and a geomagnetic correction method, aimed at supporting visually impaired people who travel indoors. To verify the effectiveness of this system, we conducted an experiment targeting visually impaired people. Although acquiring accurate positional information and detecting directions indoors is difficult, we confirmed that using this system, accurate positional information and travel direction can be obtained utilizing visible light communication technology, which employs LED lights, and correcting the values of the geomagnetic sensor integrated in a smartphone.

Abstract:Dengue is a viral infection transmitted by the Aedes aegypti mosquito. This study aimed to assess the distribution of cases and deaths from dengue and severe dengue, and its relationship with social vulnerability in Belo Horizonte, State of Minas Gerais, Brazil, from 2010 to 2018. The incidence and lethality rates of dengue and their relationship with sex, age, education, skin color, and social vulnerability were studied using chi-square tests, Ordinary Least Squares (OLS), and Geographically Weighted Regression (GWR) analyses. The number of cases of dengue in Belo Horizonte during the study period was 324,044 dengue cases, with 1,334 cases of severe dengue and 88 deaths. During the past few decades, the incidence rate of both dengue and severe cases varied, with an average incidence rate of respectively 1515.5 and 6.2/100,000 inhabitants. The increase in dengue cases was directly related to areas with higher social vulnerability areas and more working-age people. Also, the disease is more severe in people self-declared as black, elderly, and male. The findings of this study might provide relevant information for health services in the organization of control and prevention policies for this problem, emphasizing the most vulnerable urban areas and categories.

Abstract:This study considers the vulnerability of the urban area of the City of Belo Horizonte to dengue. A total number of 89,607 cases registered in the surveillance system from 1996 to 2002 were analyzed. Seven epidemic waves were identified during this period. Cases were grouped into 2,563 census areas, and three risk categories were proposed based on how many times each area reached a threshold established for each epidemic wave. The association between the risk categories and the socioeconomic, demographic and urban-infrastructure characteristics was evaluated. Analysis included Kruskal–Wallis test variance comparisons and multivariate regression using multinomial models. Incidence rates differed significantly among the three risk categories in most of the epidemic waves. The factors that best characterized the areas were low educational level (≤4 years of schooling), low income of the head of the family (≤2 minimum wages per household), household density, and proportion of children and elderly women. Information related to basic sanitation was not enough to discriminate levels of susceptibility to dengue, and study of population density and concentration of establishments considered vulnerable to vector infestation yielded questionable results. It is important to consider different levels of exposure of the population to explain the heterogeneous pattern of distribution of dengue cases in an urban setting. Understanding the dynamics of dengue fever is essential for surveillance purposes, to improve control measures and to avoid epidemics of this disease.
Spatio-Temporal Dynamics and Climate Change Scenarios Forecast of Dengue Incidence in Brazil

Abstract:Dengue is a neglected tropical vector-borne disease that is spreading rapidly and increasing worldwide. Climate change has been considered one of the primary factors contributing to this escalation. In Brazil, a vast and heterogeneous country, each epidemic has set new records, with the disease currently spreading widely across the country. In this study, we used predictive modeling techniques, grounded on a Bayesian hierarchical framework, to forecast the spatio-temporal dynamics of dengue in Brazil under future climate scenarios. We used monthly historical data on dengue incidence from 2000 to 2021, collected over 557 Brazilian microregions, to learn about its association with population, environmental, climatic, and socioeconomic conditions. By integrating projections of precipitation, minimum temperature and population based on different Shared Socioeconomic Pathway scenarios, we obtained spatially resolved projections of dengue incidence for 2021-2040 and 2041-2060. The model’s predictive performance was ensured via robust cross-validation. We observed an increase in dengue incidence with rising precipitation, urban infrastructure, and Normalized Difference Water Index, and a decrease with increas-ing elevation and deprivation index. Minimum temperature exhibited a nonlinear and positive association behavior. Our forecasts show great regional variability. In the south region, some traditionally non-endemic microregions are expected to see a clear increase in dengue incidence rate. Conversely, other regions show geographic variability of patterns, suggesting that in some cases elevated temperatures may exceed the viable threshold for dengue transmission. Investing in reducing greenhouse gas emissions and implementing measures to address the rising incidence of dengue due to climate change is crucial.

Abstract:Objective: Analyze the spatial distribution of classical dengue and severe dengue cases in the city of Rio de Janeiro. Methods: Exploratory study, considering cases of classical dengue and severe dengue with laboratory confirmation of the infection in the city of Rio de Janeiro during the years 2011/2012. The georeferencing technique was applied for the cases notified in the Notification Increase Information System in the period of 2011 and 2012. For this process, the fields "street" and "number" were used. The ArcGis10 program's Geocoding tool's automatic process was performed. The spatial analysis was done through the kernel density estimator. Results: Kernel density pointed out hotspots for classic dengue that did not coincide geographically with severe dengue and were in or near favelas. The kernel ratio did not show a notable change in the spatial distribution pattern observed in the kernel density analysis. The georeferencing process showed a loss of 41% of classic dengue registries and 17% of severe dengue registries due to the address in the Notification Increase Information System form. Conclusions: The hotspots near the favelas suggest that the social vulnerability of these localities can be an influencing factor for the occurrence of this aggravation since there is a deficiency of the supply and access to essential goods and services for the population. To reduce this vulnerability, interventions must be related to macroeconomic policies. Objetivo: Analisar a distribuição espacial dos casos de dengue clássico e dengue grave no município do Rio de Janeiro. Métodos: Estudo exploratório, considerando casos de dengue clássico e de dengue grave com comprovação laboratorial da infecção, ocorridos no município do Rio de Janeiro nos anos de 2011/2012. Foi aplicada a técnica de georreferenciamento dos casos notificados no Sistema de Informação de Agravos de Notificação, no período de 2011 e 2012. Para esse processo, utilizaram-se os campos "logradouro" e "número". Foi realizado o processo automático da ferramenta Geocoding do programa ArcGis10. A análise espacial foi feita a partir do estimador de densidade Kernel. Resultados: A densidade de Kernel apontou áreas quentes para dengue clássico não coincidente geograficamente a dengue grave, estando localizadas dentro ou próximas de favelas. O cálculo da razão de Kernel não apresentou modificação significativa no padrão de distribuição espacial observados na análise da densidade de Kernel. O processo de georreferenciamento mostrou perda de 41% dos registros de dengue clássico e 17% de dengue grave devido ao endereçamento da ficha do Sistema de Informação de Agravos de Notificação. Conclusões: As áreas quentes próximas às favelas sugerem que a vulnerabilidade social existente nessas localidades pode ser um fator de influência para a ocorrência desse agravo, uma vez que há deficiência da oferta e acesso a bens e serviços essenciais para a população. Para diminuir essa vulnerabilidade, as intervenções devem estar relacionadas a políticas macroeconômicas.
Resurgence of Dengue Virus Serotype 3 in Minas Gerais, Brazil: A Case Report

Abstract:This report provides a detailed overview of the resurgence of DENV-3 in the state of Minas Gerais, Brazil, which is a concerning scenario in the context of dengue, a mosquito-borne viral disease. Historically, Brazil has grappled with dengue epidemics caused primarily by the DENV-1 and DENV-2 serotypes. However, in 2023, a significant shift in this pattern was observed as DENV-3 made a notable resurgence. This resurgence was characterized by the increase in DENV-3 cases within the country and the region of the Americas. Given the absence of sustained DENV-3 circulation in Brazil in previous years, this situation poses a significant risk, making the population highly susceptible to a potential novel epidemic. In November 2023, a 31-year-old male patient in Belo Horizonte exhibited symptoms of acute febrile syndrome. Multiplex RT-qPCR using the Kit Molecular ZC D-Tipagem confirmed DENV-3 infection, suggesting a likely autochthonous case, as the patient reported no travel history. To promptly assess this resurgence, we applied the nanopore sequencing technology. This allowed for the rapid characterization of the initial DENV-3 case isolated in Minas Gerais in 2023, representing a 13-year interval since the serotype's previous documented circulation in that state. This case report underscores the critical importance of proactive monitoring and the swift implementation of targeted control strategies to address the evolving dynamics of dengue, with a specific emphasis on the resurgence of DENV-3 in the state.

[Atypical presentation of delirium: risk of misdiagnosis and undertreatment].

Abstract:An atypical presentation of delirium increases the risk of misdiagnosis and undertreatment. Three cases are presented in this article. A 65-year-old woman, admitted for malnutrition, has significant mood-related symptoms that resemble depression. A 50-year-old male, admitted with an abscess, necrotizing fasciitis and sepsis, appears to be suicidal. A 61-year-old male, admitted with pneumonia, has auditory hallucinations. All three patients turned out to have a delirium. The authors advise ruling out a delirium in all patients who have predisposing factors and who develop an acute psychiatric disorder while staying in the hospital, before considering other psychiatric diagnoses. Advice on how to improve the diagnostic process is given.

Abstract:Dengue is a global public health problem. The Dengue Virus (DENV) serotypes are transmitted by an Aedes aegypti mosquito. Vector control is among the primary methods to prevent the disease, especially in tropical countries. This study aimed to analyze the spatial distribution of dengue and its relationship with social inequalities using spatial modelling. An ecological study with temporal and spatial analysis was conducted in the state of Sergipe, Northeast Brazil, over a period of 18 years. Spatial modelling was used to determine the influence of space on dengue incidence and social inequalities. The epidemic rates in 2008, 2012, and 2015 were identified. Spatial modelling explained 40% of the influence of social inequalities on dengue incidence in the state. The main social inequalities related to the occurrence of dengue were the percentage of people living in extreme poverty and inadequate sanitation. The epidemic situation even increased the risk of dengue in the population of the state of Sergipe. These results demonstrate the potential of spatial modelling in determining the factors associated with dengue epidemics and are useful in planning the intersectoral public health policies.

Abstract:The aim of this research was to investigate the origin of imported cases of dengue in the city of Araraquara, Brazil and to describe the disease's main epidemiological characteristics. The study encompassed all confirmed cases of dengue recorded in the Information System for Notifiable Diseases (SINAN) [Sistema de Informação de Agravos de Notificação] from 1998 to 2013. Cases whose origin of infection was likely located outside Araraquara were considered imported. The epidemiological study entailed a descriptive analysis of the data, regarding the distribution of cases by sex, age, and classification of imported and autochthonous cases. A geographic information system was used to map flows and estimate distances. There were 6,913 confirmed cases, 419 of which were imported. In most cases, the origin of infection was located in the state of São Paulo as well as other Brazilian regions. The results indicate the relevance of imported cases and differences in the epidemiological profile with respect to age and sex. Conclusions indicate the need to increase epidemiological and environmental health surveillance at ports, airports, truck stops, and bus and train terminals.
Ambient Temperature and Dengue Hospitalisation in Brazil: A 10-year period two-stage case-design time series analysis

Abstract:Climate factors are known to influence seasonal patterns of dengue transmission. However, little is known about the effect of extremes of temperature on the severity of dengue infection, such as hospital admission. We aimed to quantify the effect of ambient temperature on dengue hospitalisation risk in Brazil. We retrieved daily dengue hospitalisation counts by each of 5,565 municipalities across the 27 states of Brazil from 1st of January 2010 to 31st of December 2019, from the Brazilian Public Hospital Admission System (“SIH”). We obtained average daily ambient temperature for each municipality from the ERA5-land product reanalysis. We combined distributed lag non-linear models with time stratified design model framework to pool a relative risk (RR) estimate for dose-response and lag-response structures for the association of temperature and Dengue hospitalisation. We estimate the overall dengue hospitalisation RR for the whole country as well for each of the five macro-regions by meta-analysing state level estimates. 579,703 hospital admissions due to dengue occurred over the 10 years’ period of 2010 to 2019. We observed a positive association between high temperatures and high risk of hospitalisation for Brazil. The overall RR for dengue hospitalisation was 2.185 (95% CI 1.457- 3.276) at the 50th percentile of temperature and 2.385 (95% CI 1.556-3.655) at 95th percentile of temperature for Brazil. We also found lag effects of heat on hospitalisation, particularly an immediate augmented risk to hospitalisation, both at the 50th percentile and 95th. High temperatures are associated with an increase in the risk of hospitalisation by dengue. These findings may guide preparedness and mitigation policies during dengue season outbreaks, particularly on the health-care demand.

Abstract:In this review, we discuss dengue surveillance, prevention, and control measures in Brazil. Data on dengue epidemics between 2000 and 2024 indicates an increase in the number of dengue cases and deaths. Global climate change is a key driver of this growth. Over the past 25 years, nearly 18 million Brazilians have been infected with the dengue virus, and the highest number of dengue cases in Brazil's history is projected to reach 2024. Dengue mortality in Brazil increased geographically over time. As of June, there were approximately 6 million probable cases and 4,000 confirmed deaths in Brazil, which represents the greatest dengue epidemic to date. Several technologies have been developed to control Aedes aegypti, including the deployment of Wolbachia-infected mosquitoes, indoor residual spraying, sterile insect techniques, and mosquito-disseminated insecticides. The Ministry of Health recommends integrating these technologies into health services. Brazil is the first country to incorporate the Takeda vaccine into its public health system, and the Butantan vaccine is currently undergoing Phase 3 clinical trials. Increasing the vaccination coverage and implementing novel Ae. aegypti control technologies could reduce the number of dengue cases in Brazil in the coming years. Community activities such as home cleaning and elimination of potential mosquito breeding sites, facilitated by social media and health education initiatives, must continue to achieve this reduction. Ultimately, a multisectoral approach encompassing sanitary improvements, mosquito control, vaccination, and community mobilization is crucial in the fight against dengue epidemics.
