Abstract:Hypothermia has been reported in many clinical states and sometimes it is life threatening. However, there are rare cases reported about hypothermia related with Parkinson's disease (PD) 1-3. We now present a patient with PD, who developed an episode of severe hypothermia. The unique changes both in electrocardiogram (ECG) and in electroencephalogram (EEG) during hypothermia were presented and discussed. An 89-year-old woman was presented to our hospital with conscious impairment for 1 day. She was diagnosed with Parkinson's disease about 10 years ago and has been taking carbidopa-levodopa at 500–750 mg/day since then. Her symptoms, such as slowness of movement did not significantly deteriorate in nearly 10 years, with Hoehn-Yahr Stage II. During last 2 years, her slow movement worsened, but she did not add the dose of carbidopa-levodopa. In recent 2 months, she had prominent deterioration of her motor slowness (Hoehn-Yahr Stage IV). Her caregiver and son lived with her in the house with air-conditioner, keeping room temperature around 20°C. One day before the admission, she was found by her caregiver in a new onset state of mutism in the afternoon, with her eyes opening spontaneously. She sat in a warm room and showed mere response to outside stimuli and no voluntary movement or facial expression, accompanied with urinary and fecal incontinence. But no static tremor or shivering was noticed. No obvious daytime sleepiness or disturbed nocturnal sleep was observed. Her son and caregiver stated that she was carefully looked after and slept with electric blanket covered. They denied any exposure to cold environment before and after sudden onset of mutism and they did not measure her body temperature. On examination, her aural canal temperature was 30.9°C, blood pressure 112/67 mmHg, heart rate 50 beats/min, respiratory rate 20 /min, with an oxygen saturation of 96.7% while breathing room air. Glasgow coma scale was 8. She was noted to have obvious conjunctival edema. On auscultation and percussion, the lungs were clear and no cardiac murmur was present. Abdomen was flat and bowel sound was audible. No abnormal mass was palpable. Neurological examination revealed generalized muscular rigidity with masked face and neck stiffness. She was in a stupor and exhibited minor myoclonia in bilateral upper extremities which could be intensified by passive extension of the fingers. Deep tendon reflexes were brisk and bilateral, Babinski sign was negative. Results of routine laboratory tests, including complete blood count, electrolytes, fasting serum glucose, hepatic and renal function, C-reactive protein, Troponin-I, and antinuclear antibody, were all normal. Arterial blood gas showed metabolic acidosis with pH of 7.297, pCO2 49.5 mmHg, pO2 99.5 mmHg, BE 3.1 mM, SB 21.7 mM, AG 15.9 mM. Her thyroid function showed free T4 of 13.23 pM, free T3 of 2.62 pM, and thyroid stimulating hormone of 2.18 mIU/L. Her thyroid peroxidase antibody (TPO-Ab) and thyroglobulin antibody (TG-Ab) were normal. Serum adrenocorticotropic hormone (ACTH) was also normal, 22.52 pg/mL in 8:00 a.m. and 27.13 pg/mL in 4:00 p.m. Brain CT scan and magnetic resonance images were generally normal. Her first electrocardiography (ECG) taken immediately after admission (30.9°C, Figure 1A) demonstrated bradycardia and Osborn waves. The next day after admission (35.4°C, Figure 2), 16-channel electroencephalography (EEG) with scalp electrodes placed according to international 10–20 system was firstly recorded. At that time, she was still in a state of stupor. EEG demonstrated no alpha rhythm, but widespread theta waves of 80 μV (4–6c/s) and delta waves of 120 μV (1.5–3c/s) throughout both hemispheres. The frequent triphasic discharges of up to 100 μV with sharp contour were conspicuous. These waves mainly occurred synchronously in left hemisphere channels, with intervals of 1.5–3 s. After several days' rewarming by warm environment in hospital and blanket, her body temperature was gradually getting back to normal (37.0°C) on the 4th day in hospital. She became alert and talked by simple phrases. Conjunctival edema and myoclonus also disappeared. The second ECG (Figure 1B) revealed no Osborn waves. The repeated EEG (37°C, Figure 2) demonstrated alpha rhythm, instead of triphasic waves, with theta waves of 50 μV (4–6c/s) and delta waves of 50 μV (2–3c/s) widespread throughout both hemispheres. Her caregiver was asked to check her body temperature regularly after she was discharged. In the current report, we describe one patient with PD who developed severe hypothermia, during which both EEG and ECG demonstrated striking changes. After the body temperature returned to normal, her consciousness recovered and the abnormalities of ECG and EEG were also reversible. The remarkably reversible triphasic waves in EEG are of interest. The coexistence of myoclonus and triphasic waves was reported in patients with Creutzfeldt-Jakob disease and serotonin syndrome 4. But these reasons were ruled out in the current case because of the reversible characteristic and no history of taking serotonin reuptake inhibitor. Previous report also showed periodic triphasic activity following levodopa treatment 5. This chance also seems to be small in our case because she kept taking carbidopa-levodopa in the hospital. Myoclonus was reported to develop during brain hypothermia and disappeared after rewarming 6. The illustrated hypothermia and concomitant reversible triphasic waves in PD were also reported in two cases 1. We thus speculate that the reversible triphasic waves and myoclonus in the present case were related to accidental hypothermia. The metabolic derangements, such as metabolic acidosis and low ATP level during hypothermia, may explain the underlying mechanisms. Osborn waves on ECG, also known as J waves, was reported when the body temperature drops and resolute gradually with rewarming. It is found that the amplitude of Osborn wave was inversely correlated with body temperature, thus the amplitude may be used as an indicator and monitored during the rewarming process. Osborn wave is best observed in the inferior and lateral precordial leads and occur due to transient electrophysiological changes during cooling. During lower temperature, heterogeneous distribution of a transient action potential with notch in epicardium but not in endocardium across the ventricular wall leads to the electrocardiographic Osborn wave. Specific Osborn wave was reported in one case of accidental hypothermia in PD 3. We suggest that hypothermia is related to PD, not just an accidental event. The common causes of hypothermia, such as endocrinologic (hypopituitarism, hypoadrenalism, hypothyroidism, hypoglycemia), environmental and dermatologic diseases have been ruled out in this case. Interestingly, the absence of shivering during hypothermia leads to the suspicion of the thermoregulation dysfunction or even the pathology of hypothalamus. Animal experiment has shown the change of body temperature and thermal preference owing to the lesion of median preoptic area of hypothalamus 7. In patients with PD, the histopathological and functional changes in hypothalamic nuclei have been reported. PET study in PD showed the declines of 18F-dopa uptake in hypothalamus 8. Notably, the selective loss of hypothalamic hypocretin cell and the deficiency of cerebrospinal fluid hypocretin (orexin)-1 have been shown in patients with PD associated with disturbances of metabolism and thermoregulation 9. Hypocretin was thought to promote heat production 10. However, we did not notice the disturbance of sleepiness in this case, which was reported to be related with low levels of hypocretin. The relationship between hypocretin and hypothermia needs to be further clarified. We suggest that PD could be a predisposing cause of severe hypothermia, and the reversible EEG and ECG changes may be characteristic in hypothermia. Although it is rare to see a patient with PD who presented symptoms caused by hypothermia, the recognition of clinical entity of hypothermia and the related EEG and ECG changes is important to prevent misdiagnosis. Future study may be needed to find the possible mechanism, such as the reasons of transient dysfunction in diffuse cerebral areas and hypocretin in hypothermia in PD. COVID-19 or Clozapine? A Case of Likely Clozapine-Induced Systemic Inflammatory Response Syndrome

Abstract:Introduction Clozapine has known utility for treatment of psychosis in patients with Lewy Body Dementia and Parkinson's disease. Clozapine's benefits, including improvement in psychotic symptoms and reduced risk of extrapyramidal symptoms, are counterbalanced by its potentially life-threatening side effects, including agranulocytosis, myocarditis, and seizures. The risk of adverse effects may be elevated in older adults, at least in part due to increased clozapine levels in the setting of reduced albumin and possible reduced activity of the CYP1A2 and 3A4 enzymes. In this report, we present a case of a patient who developed fever and laboratory evidence of inflammation shortly after initiation of clozapine during the COVID-19 pandemic. Methods The patient was a woman in her 70s with psychiatric history of treatment-refractory major depressive disorder with psychotic features, who was admitted to an inpatient psychiatry unit with severe depressive symptoms, paranoia, and somatic delusions. She also had visual hallucinations, tremulousness, and DaTscan abnormalities raising concern for Lewy Body Dementia or Parkinson's disease dementia as potential contributors to her psychiatric symptoms, though MMSE score was within normal limits. Her depressive and psychotic symptoms were inadequately managed by ECT and first-line antipsychotic medications (quetiapine, olanzapine), so she was cross-tapered to clozapine. About halfway through her second week of taking clozapine, she developed a fever. She had sore throat, rhinorrhea, and mild chest tightness but no chest pain. Labs were notable for normal WBC count, normal ANC, elevated CRP, normal high-sensitivity troponin, and negative SARS-CoV-2 PCR. Clozapine was continued given unremarkable cardiac markers, but over the following days, ESR, CRP, LDH, ferritin, procalcitonin, and IL-6 were elevated while infectious workup was unrevealing. She underwent multiple SARS-CoV-2 tests given that her inflammatory marker profile was similar to what had been seen in hospitalized patients with COVID-19, but all tests were negative. On day 14, HS-troponin increased though she had no flu-like symptoms, no chest pain, and no eosinophilia. Clozapine was initially continued, but the next day, there was a rapid increase in NT-proBNP. This altered the risk-benefit analysis of continuing clozapine, so it was stopped. Metoprolol was started due to possibility that prolonged tachycardia could explain some of the elevation in NT-proBNP. Echocardiogram showed unchanged, normal left ventricular ejection fraction. During this period, differential diagnosis per medicine consult team included unknown viral illness (more likely) versus clozapine-induced myocarditis (less likely). In the days following cessation of clozapine, HS-troponin normalized, NT-proBNP significantly decreased, and fever resolved. Results Given the severity and persistence of her psychiatric symptoms and uncertainty as to whether previous constellation of findings was viral or clozapine induced, approximately one week after its initial discontinuation, clozapine rechallenge was initiated with ongoing guidance from the internal medicine consult team. She had a couple of transient instances of slightly elevated NT-proBNP and HS-troponin measurements during re-titration, as well as elevations in ESR and CRP. Three weeks into repeat titration, she developed a fever, elevated procalcitonin, and mildly elevated NT-proBNP, but infectious workup was negative. Fever resolved and HS-troponin remained normal, so clozapine was continued and NT-proBNP had normalized by two days later. She was discharged shortly thereafter, and final EKG prior to discharge was normal. She continued clozapine for about two months after discharge before it was tapered off in the setting of unsteady gait and ultimately discontinued. Conclusions This case highlights the inflammatory response that can be seen in patients taking clozapine and how difficult it can be to distinguish from the laboratory abnormalities caused by COVID-19 or other viral illnesses. Of note, a previously proposed case definition for clozapine-related myocarditis included the absence of confirmed viral infection as part of the diagnostic criteria. Given difficulty of making definitive diagnosis of clozapine-induced myocarditis, it would be beneficial for a clear case definition to be widely disseminated and implemented. It is currently unknown whether older patients, particularly those with Parkinson-plus syndromes or recent viral illness, are more likely to develop inflammatory responses to clozapine. It is possible that older patients, particularly those with significant medical comorbidity, may benefit from a slower clozapine up-titration. Further research may help clinicians counsel older patients on the risks and benefits of a clozapine retrial following an episode of possible myocarditis.

Clozapine-Induced Severe Toxicity: Exploring the Pharmacokinetic Profile of Clozapine and Its Significance in Hemodynamic Instability - A Case Report

Abstract:Hemodynamic instability in patients with clozapine intoxication can indirectly reflect the serum concentration of clozapine.We have described a case of a 32-year-old pregnant woman who developed life-threatening clozapine toxicity at 28 weeks of gestation. The levels of clozapine and norclozapine in the serum were high. We initiated hemoperfusion(HP) and other detoxification therapies to remove the drug. The patient had severely dilated peripheral blood vessels, which led to cardiac symptoms such as fatal hypotension and uncontrollable tachycardia, resulting in very high cardiac output and elevated Central venous oxygen saturation (ScvO2). Pharmacological intervention significantly improved the hemodynamics.In light of our observations in the ongoing case, we posit that evaluating hemodynamic parameters before and after blood detoxification could serve as a valuable means to gauge effectiveness and provide guidance for treatment.
Evaluation of major adverse events of clozapine based on accordance to an international titration guideline

Abstract:Introduction: Clozapine is the only antipsychotic approved for treatment-resistant schizophrenia, but without appropriate monitoring, it can be associated with potentially fatal outcomes. An International Adult Clozapine Titration Guideline categorizes patients into normal or slow metabolizers. Categorization provides clozapine titration schedules and recommends regular c-reactive protein (CRP) and clozapine concentration monitoring to reduce the risk of adverse drug reactions (ADRs). The impact of the guideline on clozapine ADRs has not been evaluated. Methods: A retrospective chart review assessed clozapine titrations, laboratory monitoring, ADRs, and discontinuations for clozapine-naive adult inpatients at a single center from January 1, 2013, to June 1, 2022. Each patient's cumulative weekly clozapine dosage was compared with their guideline recommended dosage to create a percent accordance. Linear logistic regression evaluated the relationship between titration speed and the presence of an ADR, while descriptive statistics analyzed laboratory monitoring. Results: Forty-three patients were included, with the majority being White males with schizophrenia. An inverse relationship existed between the last inpatient week clozapine dose percent accordance and the probability of an ADR. Nonobese patients were less likely than obese patients to experience an ADR (odds ratio = 0.17; 95% CI, 0.03-0.99). CRP and clozapine concentration monitoring was suboptimal. Discussion: Based on our small retrospective review of primarily White males, more aggressive clozapine titrations did not increase ADRs. Future studies with more diverse samples are needed and should focus on specific ADRs, which may have increased occurrence with rapid titrations. Obese patients were at higher risk of ADRs, correlating with the guideline-recommended slower titrations for these patients.

Abstract:In this paper, we propose and evaluate a Finite State Machine (FSM) approach for Krudanta processing for Marathi; an agglutinative and highly inflectional language belonging to the Indo-European family with semblances of Dravidian languages. Krudanta is the phenomenon of participial construction that needs adroit handling for high quality output in machine translation (MT). We contextualize this work in a transfer based Marathi to Hindi Machine Translation system to observe the importance of Krudanta processing. Results with and without Krudanta processing establish the need for this work.
Clozapine-induced cholinergic urticaria: a case report

Abstract:Clozapine, renowned for its efficacy in treatment-resistant schizophrenia, is associated with rare yet potentially severe side effects, including hematological disorders, myocarditis, seizures and gastrointestinal obstruction. Dermatological adverse effects, though less serious, can profoundly impact patients' quality of life. We present the first reported case of cholinergic urticaria induced by clozapine, in a 25-year-old male with treatment-resistant schizophrenia. Four months into clozapine therapy, the patient developed intensely pruritic erythematous lesions triggered by sweating, significantly impairing his daily activities. Despite attempts at management, including dose reduction and antihistamine therapy, the urticaria persisted. However, a favorable outcome was achieved upon switching to quetiapine. This case underscores the importance of recognizing and managing treatment-related adverse effects, even when they arise late in treatment, and highlights the need for individualized therapeutic approaches.We discuss potential mechanisms underlying clozapine-induced cholinergic urticaria and emphasize the significance of patient-centered care in optimizing treatment outcomes in schizophrenia.

Abstract:Introduction Clozapine is a second-generation atypical antipsychotic medication used in patients with treatment-refractory schizophrenia. Its use is limited due to its associated adverse effects, including myocarditis. These adverse effects may have variable presentations, such as myocarditis transient or persistent, and a generalized inflammatory process. Thus, clinical monitoring to inform accurate diagnosis is essential to avoid unnecessary discontinuation of clozapine, leading to psychiatric decompensation. Objectives To review clinical features of clozapine induced Myocarditis and accurately identify signs and symptoms attributed to be most specific for myocarditis and determine at what stage clozapine should be discontinued. Methods We conducted a literature review on PubMed, MeSH, google scholar and Mount Sinai's Levy Library using keywords, clozapine, drug related side effects, adverse reaction, myocarditis, treatment resistant schizophrenia. Review of two cases series was done. Results A review of 15 articles that addressed the cardiac complications of clozapine was performed. This review provides a base on variable clinical characteristics and outcomes of clozapine –induced Myocarditis. It showed patients who had myocarditis ruled out, demonstrated high prevalence of systemic signs of inflammation such as fever, malaise, tachycardia and elevated c-reactive protein. However, despite clozapine maintenance in most, this systemic response subsided without any intervention. A nonspecific inflammatory response is common when initiating clozapine, this inflammatory "clozapine storm' occurs within the first month of initiation and is not necessarily predictive of myocarditis. These patients were monitored closely. Those confirmed with clozapine- induced myocarditis using echocardiography and cardiac magnetic resonance imaging were managed with dose reduction, laboratory monitoring, vital signs check, with early initiation of beta-blockers without discontinuation of clozapine, with improvement in their laboratory results and vital signs. Those with progressive clinical signs of myocarditis required immediate cessation of clozapine. Conclusions We are proposing a critical need for a multidisciplinary team of psychiatrists, cardiologists and pharmacists collaborating to prevent premature termination of clozapine in cases of treatment-refractory schizophrenia. Our cases showed middle aged patients with treatment - refractory schizophrenia, presenting with symptoms suggestive of clozapine induced- myocarditis, few weeks after initiation. Clozapine was continued with close monitoring, as symptoms resolved. Though clozapine is associated with myocarditis, with proper knowledge on guidelines for monitoring patients, it can mitigate unnecessary discontinuation of clozapine in those patients. Abstract:Background: Cardiometabolic health significantly impacts on the mortality of people with severe mental illness. Clozapine has the greatest efficacy for Treatment Resistant Schizophrenia (TRS) but the greatest negative impact on cardiometabolic health. Balancing the risks and benefits of treatment, dignity, autonomy, liberty, mental and physical health can be challenging, particularly when imposing interventions with potentially life threatening adverse events, such as clozapine. We describe the successful administration of clozapine in the face of myocardial infarction, pulmonary embolism and hyperlipidaemia resulting in the termination of long-term seclusion for a gentleman with TRS in high secure psychiatric services. Case presentation: The impact of clozapine on a 44-year-old gentleman with TRS, extreme violence requiring physical restraint and long-term segregation, and numerous other significant physical health complications is described. He had metabolic syndrome; a poor diet, sedentary lifestyle, Body Mass Index (BMI) of 31.5, poorly controlled lipids and had smoked heavily since childhood. During preparations to initiate clozapine, he suffered a myocardial infarction and pulmonary embolism. His compliance with secondary prevention medications was poor due to paranoid persecutory and somatic delusions. Despite these concerns, nasogastric administration of clozapine was approved and prescribed within nine months of his myocardial infarction and a month from his pulmonary embolism but was ultimately not required. Accepting oral medication, his mental state made a rapid and dramatic improvement. After spending 1046 days in seclusion, this was terminated 94 days after clozapine initiation. He has been compliant with all medications for 24 months, had no incidents of violence or seclusion, and has been transferred to medium secure services. His physical health stabilised despite continuing to lead a sedentary lifestyle and remaining obese (BMI of 35). He developed hypertension, Type II Diabetes Mellitus and his triglycerides rose to 22.2 mmol/L in the same month after clozapine initiation. However, with pharmacological intervention, 24 months later these are controlled, and he has had no further thromboembolic events. Conclusions: We highlight that despite significant physical health concerns, clozapine can be successfully initiated and safely prescribed with a significantly positive effect on both the psychiatric and holistic care of patients with treatment resistant schizophrenia.

Abstract:Clozapine is a highly effective second-generation antipsychotic with few extrapyramidal reactions, making it a preferred choice among clinicians. However, instances of acute clozapine poisoning resulting from suicide attempts and misuse have been reported. Through our review of existing literature, we identified that we believe to be the highest recorded overdose of clozapine in elderly patients, resulting in a nonfatal outcome.

Clozapine in a patient with treatment-resistant schizophrenia and hypertrophic cardiomyopathy: a case report

Abstract:Background: There is currently limited experience in the initiation and maintenance of clozapine for treatment-resistant psychosis in adults with established structural heart disease. These complex patients require close supervision and liaison between colleagues. Here we present the successful experience of treating one such patient within our service and describe a monitoring plan to ensure that these treatments can be provided both safely and effectively. Case presentation: A 36-year-old man with treatment-resistant schizophrenia and known hypertrophic cardiomyopathy (HCM) was admitted to a specialist unit for a trial of clozapine. His psychiatric illness was characterised by multimodal hallucinations and delusions combined with low mood and poor motivation. The diagnosis of HCM was made 3 years previously following a routine electrocardiogram (ECG), and he had remained asymptomatic throughout this time; there were concerns about the risk of initiating clozapine given his pre-existing cardiac condition. Baseline investigations were performed as per local guidelines prior to commencing clozapine; these were within normal limits other than a mildly raised troponin level of 54 ng/L (normal <16 ng/L), which was attributed to the HCM. In addition, baseline transthoracic echocardiography (TTE) was performed which showed no change in the structural heart disease in comparison with previous TTEs. Clozapine was started at 12.5 mg daily and up-titrated to 150 mg twice daily over 14 days as per our institute's guidelines. The patient was monitored with regular testing of troponins, inflammatory markers and ECG. On day 18, the troponin level increased to 1371 ng/L. Creatine kinase and inflammatory markers remained stable. No changes in ECG or TTE were noted and the patient remained clinically asymptomatic. Cardiology opinion was sought and reported that the finding of an isolated elevated troponin was likely to reflect a 'troponin leak' in the context of increased cardiac muscle mass associated with HCM. In the absence of any clinical compromise, it was not felt to be of concern. Clozapine was continued with good effect on mental state. Troponin levels gradually reduced and the patient remained well. Conclusions: While multiple cases of clozapine-induced cardiotoxicity have been reported in the literature, its implications for pre-existing structural disease are unclear. This case report suggests that clozapine can be safely introduced in pre-existing HCM, explores strategies for monitoring and highlights the importance of liaising with experienced cardiologists. Abstract:Objectives: To determine if and when to continue clozapine in patients with clozapine-induced thrombocytopenia. Methods: A case report and literature review of a geriatric patient with Parkinson's disease (PD) psychosis, on longstanding treatment with clozapine with recent development of thrombocytopenia, is presented. Results: 85-year-old male with a history of PD complicated with sialorrhea, constipation, levodopa-induced dyskinesia, rapid eye movement sleep behavior disorder, neurocognitive impairment, and PD psychosis who follows up with the Emory Brain Health Center. Medications include carbidopa-levodopa, clozapine, donepezil, and rasagiline. He reports distressing delusional jealousy and egodystonic visual hallucinations. He has tried multiple antipsychotics with insufficient response. He has been on clozapine for nearly 10 years with moderate efficacy. His current dose is 62.5mg daily. Absolute neutrophil counts have been normal, but he developed thrombocytopenia. Though he had normal platelets at baseline, his platelets fell to 134,000/μL and have gradually decreased within a year, most recently to 82,000/μL. He has been asymptomatic without easy bleeding or bruising. His clozapine dose has remained the same with ongoing laboratory monitoring. He follows with his primary care doctor and was referred to hematology. Conclusions: The literature on clozapine-induced thrombocytopenia is limited. Incidence is variable and ranges from 0.083%-8.2% in larger samples. Most cases occur within the first 18 weeks of clozapine initiation. It is generally transient and self-resolving, usually lasting under 13 weeks, though a case report noted it to last up to 40 months. It is imperative to have hematologic baselines. The manufacturer recommends discontinuing the drug when platelet counts fall below 100,000/μL, resuming therapy when they return to normal, and permanently discontinuing if this reoccurs. UK guidelines recommend that if platelet counts fall below 50,000/μL, clozapine should be discontinued, with monitoring frequency increased. Therapy can be resumed once platelet counts normalize and if asymptomatic. Special caution must be taken in geriatric patients who are prone to fall and balance-related injuries.

Abstract:Background: Rebound cholinergic syndrome is a rare, but well known unwanted phenomenon occurring after abrupt clozapine discontinuation. There have been previous reported cases of cholinergic rebound in the literature; however, these reports described cholinergic rebound following cessation of high doses of clozapine in patients diagnosed with schizophrenia. Here, we report a case of rebound cholinergic syndrome and catatonia in a male patient three days after abrupt discontinuation of 50 mg of clozapine prescribed for type I bipolar affective disorder. Case presentation: A 66-year old male of Spanish origin, treated for type I bipolar affective disorder for 15 years and for Crohn disease, was brought to the emergency department because of a sudden onset of mutism, dysphagia and trismus. He was described catatonic and presented hypertension, tachycardia and tachypnea. His body temperature was normal and the laboratory tests were unremarkable at presentation. A head CT and an EEG were in the normal range. While reviewing his history, it appeared the he was on clozapine 50 mg a day, first introduced 2 months ago, during a previous hospitalization for a manic episode resistant to other mood stabilizers. For an unknown reason, the patient's psychiatrist stopped clozapine three days before the admission and replaced it by risperidone 5 mg and quetiapine 200 mg daily. A cholinergic rebound syndrome was then evoked. The patient's ability to speak recovered dramatically and fast after the intravenous administration of 2.5 mg of biperiden supporting the diagnosis. Risperidone and quetiapine were also stopped. The patient fully recovered in 20 days after the reintroduction of 50 mg of clozapine and 2.5 mg of biperiden daily. Conclusions: This case report underscores that cholinergic rebound syndrome may occur in patients suffering from bipolar affective disorders, being on clozapine as a mood stabilizer. The low dose clozapine does not preclude severe manifestations of the phenomenon. Progressive tapering should therefore be adopted in any case.

Attempts were made to condition instrumentally the smiling behavior of 13 infants, using their mothers and female strangers as dispensers of s6cial reinfrocement. The results indicated that the mothers were more effective than the strangers in obtaining reinforcement control of the smiling behavior.

Abstract:Objective: Heat stroke is a medical emergency. Psychiatric patients are particularly susceptible to heat stroke. Therefore, awareness and preventive measures of heat stroke are important for both clinicians and patients. Case description A 49-year-old man with schizophrenia, who was under maintenance treatment with olanzapine 20 mg/day, trihexyphenidyl 4 mg/day, and trazodone 50 mg/day, suffered from heat stroke in a heat wave and required intensive care. He recovered with the medical treatment provided. Discussion Several factors could have contributed to the impaired thermoregulation and the occurrence of heat stroke in this case: schizophrenia, the psychotropic regimen, and lack of preventive measures. Possible differential diagnoses of heat stroke in this case include infection, neuroleptic malignant syndrome, and serotonin syndrome. Conclusion: Heat stroke can occur during the maintenance treatment of olanzapine, trihexyphenidyl, and trazodone for schizophrenia. Clinicians should be proactive to reduce the risk of heat stroke in psychiatric patients.

Abstract:Clozapine has a unique efficacy in treatment-resistant schizophrenia. Its use is, however, associated with potential adverse events. Among those, clozapine induced rhabdomyolysis can compromise clozapine treatment. Recommendations surrounding the management of this rare adverse event are limited. We present a case of clozapine-induced rhabdomyolysis. A 20-year-old Caucasian male diagnosed with resistant schizophrenia developed, after a 5-month total exposition and a significant response to treatment, a marked creatine kinase (CK) elevation and important myalgia in the weeks following an increment from 175 to 200 mg of the daily dose of clozapine. This event also coincided with weight training as reported by the patient. The patient was hospitalized, and the clozapine was stopped following the diagnosis of rhabdomyolysis (CK 45,564 U/L). The cause of rhabdomyolysis was thoroughly investigated, and clozapine was held accountable for most. Clozapine cessation led to a severe psychotic relapse. Clozapine rechallenge while strictly monitoring CK was then performed allowing a significant clinical response. Clozapine was pursued despite two other episodes of mild CK elevations observed following weight training. Rhabdomyolysis comes as a rare adverse event of clozapine and its mechanism is poorly understood. Evidence on clozapine rechallenge following this adverse event is lacking and the innocuity of such practice is unknown. The unique aspect of our case report is that a shared decision with the medical team, patient and family led to a proactive clozapine rechallenge. More research is needed to provide robust guidelines and evidenced based approaches for clinicians in such a clinical dilemma.

Abstract:Introduction Electroconvulsive therapy (ECT) is a therapeutic method that induces artificial seizure by electrical stimulation to resolve various psychiatric symptoms. ECT is particularly effective in resistant schizophrenia and may improve response to medication despite the presence of potential adverse side effects. Post-ECT delirium and Headaches are some of the most frequent side effects presented in literature. Fever is yet another unexplained reaction, however there are a few case reports and retrospective studies that report on it. Objectives We aim to illustrate through a clinical case and a review of literature the prevalence of post ECT fever as well as the possible explanatory mechanisms. Methods In this study we report the case of a man with ultra-resistant schizophrenia who was treated successfully with ECT despite the development of transient febrile reaction and we present a review of literature on pubmed using the following key words : ECT, fever,resistant psychosis, mechanisms. Results Our patient is a 48-year-old man with a psychiatric history of schizophrenia evolving since the age of 34. He has a history of matricide in 2021 resulting in his hospitalisation in a forensic psychiatric ward. He underwent trials of classic and atypical antipsychotics that weren't efficacious thus he was diagnosed with resistant schizophrenia in 2022. He was treated initially with clozapine 500 mg per day and then with the association (clozapine + amisulpride) yet it wasn't effective on his persecutory delirium and fratricide ideas. Plus, there was no reduction in his PANSS (Positive and Negative Syndrome Scale) scores. The diagnosis of ultra-resistant schizophrenia was established. The staff indicated the adjunction of ECT to Clozapine. In the inpatient unit, hours after his fourth ECT session he developed a fever (40°C), his blood pressure (120/80 mm Hg), pulse(85 beats per minute),and respiratory rate(20 breaths per minute) were normal. Blood samples, including cultures, were drawn, which showed normal blood cell count and CPK(140U/L) but CRP was elevated (31 U/L), a chest x-ray showed no acute pulmonary disease, and his urinalysis result and Covid test were negative. His fever resolved then spontaneously after two hours. The same transient febrile reaction occurred again 3 times. It was postulated in literature that fever may be due to inadequate muscle reaction. Data also suggested the potential influence of ECT on the hypothalamus that is a key region in regulating body temperature. Conclusions Further studies are required in order to establish the real prevalence of this side effect and its possible causes. Disclosure of Interest None Declared

Abstract:Introduction Parkinson's disease is one of the most common neurodegenerative diseases, strongly affecting the well-being of patients and caregivers. Neuropsychiatric symptoms of Parkinson's disease include psychosis, dementia, depression, anxiety, REM sleep behavior disorder, and impulse control disorders. In patients with Parkinson's disease with distressing psychosis, pimavanserin, quetiapine, and clozapine can be used. Clozapine has fewer motor effects compared with other antipsychotic agents, though there are many side effects to consider. The most common side effects of clozapine are sialorrhea, constipation, and clozapine-induced gastrointestinal hypomotility. Some of the most dangerous are agranulocytosis, myocarditis, and cardiomyopathy. In addition to agranulocytosis, other hematologic side effects include neutropenia, leukocytosis, eosinophilia, thrombocytosis, and thrombocytopenia. Compared with data on clozapine induced leukopenia, the incidence, biological mechanism, and treatment options for clozapine induced thrombocytopenia are less well understood. We provide a case report on a patient in late life who has suffered from Parkinson's disease psychosis, on longstanding treatment with clozapine but recent development of thrombocytopenia. Methods A case report of the use of clozapine in treating psychosis in Parkinson's disease and potential thrombocytopenia along with a literature review of the subject will be presented. Results 85-year-old retired PhD male, living with his wife in a senior living community in Georgia, with a history of Parkinson's disease diagnosed 20 years ago, Parkinson's disease-related psychosis that started 10 years ago, dementia due to Parkinson's disease diagnosed 5 years ago, and REM sleep behavior disorder. Other past medical history includes prostate cancer, gradually worsening thrombocytopenia for the past 8 months, sialorrhea requiring Botox, constipation, and levodopa-induced dyskinesia. His medications are carbidopa-levodopa, clozapine, donepezil, desvenlafaxine succinate, rasagiline, and caffeine. He experiences bilateral tremor, bradykinesia, shuffling gait, masked facies, bradyphrenia, and hypophonia and is fully dependent for activities of daily living. His psychotic symptoms present as delusional jealousy and visual hallucinations of dogs and children, mostly at night. Initially, he tried multiple antipsychotics with insufficient response. He has been on clozapine for nearly 10 years with moderate efficacy, though he continues to have distressing delusions. His current dose is 62.5 mg daily. White blood cell and absolute neutrophil counts have been within normal limits. However, he has developed thrombocytopenia. Though his platelet counts were previously normal, they were 134,000/μL, 115, 000/μL, and 103,000/μL 8 months, 5 months, and 1 month ago respectively. He has had no symptoms of easy bleeding or bruising. He is continuing his current dose of clozapine with laboratory monitoring and referral to hematology to rule out other causes of thrombocytopenia. Conclusions The literature on clozapine induced thrombocytopenia is limited. There have been a handful of case reports on clozapine induced thrombocytopenia, including patients non-naïve to clozapine, along with a few studies and case series. The incidence of clozapine induced thrombocytopenia is variable but ranges from 0.083% to 8.2% in larger samples. Most cases occur within the first 18 weeks of clozapine initiation. Clozapine induced thrombocytopenia is usually transient and self-resolving, in most cases lasting under 13 weeks, though has been noted to last up to 40 months in one case report. One proposed biological mechanism is immune mediated due to a common side effect of eosinophilia. There is also a higher incidence of anemia in the first few years following clozapine initiation, demonstrating its potential to affect all three cell lines. It is important to document hematologic baselines before initiating clozapine treatment and to be aware of potential side effects in addition agranulocytosis. The manufacturer recommends discontinuing the drug when the platelet count falls below 100,000/μL, resuming therapy when the count returns to normal range, and permanently discontinuing if thrombocytopenia reoccurs. UK guidelines recommend that if the platelet count falls below 50,000/μL, clozapine should be discontinued, with monitoring frequency increased. If platelet counts remain over this limit and there are no side effects such as easy bleeding or bruising, it is reasonable to continue clozapine treatment and monitor for the period to pass.
