Abstract:Intravitreal injection of Bevacizumab for various retinal vascular disorders has become the mainstay of treatment. The injection of Bevacizumab is usually dispensed through local pharmacies after formulating from a larger vial under sterile conditions. We report an interesting finding of silicone oil bubble in vitreal cavity after intravitreal Bevacizumab injection. This is attributed to the fact that the syringes used for dispensing bevacizumab are coated with silicone oil that acts as a lubricant. These silicone oil droplets can lead to floaters and other possible complications. We report two cases of such occurrence and propose strategies to avoid this complication. Abstract:Silicone oil is used on a regular basis during vitrectomy for the surgical management of complex or recurrent retinal detachment. Incomplete removal of particles can cause postoperative complications. A 68-year-old pseudophakic patient with previous retinal detachment surgery presented with reduced vision. On examination, silicone oil could be visualized lodged between the hydrophobic intraocular lens (IOL) and intact posterior capsule. YAG laser capsulotomy was performed to clear the visual axis and disperse the oil droplets into the vitreous cavity. Corrected distance visual acuity improved to 6/6 and symptoms resolved. Silicone oil can accumulate in the capsular bag after vitrectomy even in patients with an intact posterior capsule. It is important to ascertain the type of IOL material used during cataract surgery and to properly identify the positioning of the oil droplets to safely treat the patient and avoid unnecessary complications.

Abstract:Background and objective: To report cases with intraocular lens (IOL) opacification following silicone oil (SO) endotamponade. Patients and methods: Medical charts of 32 eyes with IOL opacification were evaluated retrospectively. All eyes had rhegmatogenous retinal detachment and had a history of previous hydrophilic acrylic IOL implantation. All patients underwent vitrectomy with SO endotamponade. Clinical features of all cases and the results of histochemical evaluation of explanted IOLs were reported. Results: The mean duration of SO endotamponade was 4.6 ± 2.0 months. The mean follow-up was 67.0 ± 23.5 months. The interval between phacoemulsification surgery and IOL opacification was 27.4 ± 18.3 months. With the exception of two eyes, all IOL opacification was detected during the follow-up period after SO removal. IOL exchange was performed in 12 eyes (37.5%). Histochemical analysis revealed significant calcification mostly on the surface of explanted IOL optics. Conclusion: Vitreoretinal surgeons should be aware of that some hydrophilic IOLs may have the potential of opacification following SO endotamponade. Phacoemulsification and foldable intraocular lens implantation in eyes filled with silicone oil

Abstract:Background and objective: To evaluate the visual outcome of phacoemulsification and foldable intraocular lens (IOL) implantation in eyes already filled with silicone oil. Patients and methods: Following vitrectomy and silicone oil injection, 16 eyes of 16 consecutive patients undergoing phacoemulsification and IOL implantation were included in the study. Results: The mean age of patients was 58.3 +/- 9.2 years (range, 38 to 71 years). The time interval between the cataract surgery and silicone oil injection was 5.8 +/- 1.7 months (range, 4 to 9 months). The mean number of previous surgeries was 2.0 +/- 0.8 (range, 1 to 4). Trypan blue (0.4%) was used for anterior capsule staining in 3 eyes with white cataract. Dispersive viscoelastic material was used during all steps of phacoemulsification. A capsular tension ring was inserted after capsulorhexis and hydrodissection in all cases. An IOL with a 6.0-mm optic was implanted in all eyes. The preoperative best-corrected visual acuity ranged from hand motions to 0.3. The mean postoperative best-corrected visual acuity was 0.3 +/- 0.1 (range, 0.1 to 0.6); best-corrected visual acuity increased in all eyes. A small amount of silicone oil moved into the anterior chamber during phacoemulsification in 1 eye and was evacuated. In all eyes, no silicone oil was present in the anterior chamber postoperatively. Conclusion: The use of dispersive viscoelastic and a capsular tension ring provides safer phacoemulsification and foldable IOL implantation in eyes already filled with silicone oil.
Prevalence of silicone oil droplets in eyes treated with intravitreal injection

Abstract:Abstract Objective To assess the number of eyes with silicone oil in the vitreous after intravitreal injection. Methods This cross-sectional, comparative study was divided into 2 groups: (1) treatment—eyes subjected to antiangiogenic therapy; (2) control—no history of intravitreal injection. Subjects were assessed regarding age, gender, clinical diagnosis, lens status, visual acuity and number of previous intravitreal injections. All eyes underwent a meticulous slit-lamp and ultrasound examination for the identification of silicone oil. ImageJ software was used to quantify the index of silicone oil (IOS) by ultrasonography. Results Sixty-seven eyes (30 controls, 37 treated) were included. Slit-lamp examination found silicone oil droplets in 25 out of 37 (67.57%) treated eyes and in none of the control group. Ultrasonography identified silicone oil in 28 out of 37 (75.68%) treated eyes and in 1 out of 30 (3.33%) controls. An observed agreement of 85.07% and a Cohen’s Kappa coefficient of 69.10% (p < 0.0001) between ultrasonography and biomicroscopy were found. Wilcoxon test showed a statistically significant difference (p = 0.0006) in IOS between controls (0.41 ± 0.43%) and treated eyes (2.69 ± 2.55%). Spearman’s correlation test (0.61; p < 0.0001) showed that the greater the number of injections, the higher the IOS. Conclusions Silicone oil droplets were found in the majority of the eyes previously treated with antiangiogenic intravitreal injection. The greater the number of injections, the higher the likelihood of finding silicone oil. An improvement in the technique of injection and better-quality syringes post-injection silicone oil droplets.

Abstract:The paper is devoted to the interfacial aspects of the intraocular behavior, migration and distribution of commonly injected ophthalmic drugs in eyes filled with medical-grade 1300 cSt silicone oil used as a retinal tamponade agent. Novel in vitro and ex vivo models were created for studying the physical properties of the retinal surface and interfacial spreading of the ophthalmic drugs over retinas. In vitro model experiments showed that droplets of all tested drugs sank rapidly in the silicone oil to contact with the plasma-treated glass, and then rapidly spread over the glass surface. In the ex vivo model, the migration phase was followed by contact with, and rapid spread/absorption by the retinal interface. The wetting behavior of drugs under contact with the glass substrate and retinas was similar. Characteristic time scales of drugs spreading, controlled by the viscous dissipation, were close. All tested drugs migrated to the retinal surface and rapidly spread across the retinal surface. This suggests that intravitreal drugs might be used effectively in eyes filled with a silicone oil tamponade, as they rapidly migrate to, and spread over the retinal surface.

Silicone Oil-Associated Extensive Intraocular Ossification: A case report

Abstract:Background: Intraocular ossification is an uncommon calcium deposition process associated with trauma, chronic inflammation, tumor, and long-standing retinal detachment. This is the first reported extensive intraocular bone formation associated with silicone oil. Case presentation: A 30-year-old Han Chinese man came to us with complaint of red, painful blind right eye. He had a history of ocular trauma, retinal detachment, and two failed retinal reattachment surgeries with silicone oil left in the eye. On examination, conjunctiva congestion, band keratopathy, silicone oil emulsification, and limbus neovascularization were found. B-scan ultrasound and computed tomography scanning demonstrated retinal detachment and calcification of the eyeball wall. Histopathological analysis indicated ossification overlying the choroid. Evisceration was finally operated to relieve the pain. Conclusion: The retention of silicone oil in the eye probably accelerates the ossification. Timely silicone oil removal and evisceration should be recommended if necessary for phthisis bulbi.

Abstract:Intraocular silicone oil tamponade is being used with increasing frequency as an adjunct to conventional vitrectomy techniques in the treatment of proliferative vitreoretinopathy. When used in patients with aphakia, corneal decompensation is a common long-term complication. In this study, we evaluated the clinical and morphologic changes that occurred in the corneas of 14 rabbits and seven cats when the anterior chamber was filled with silicone oil. Within six days, wide-field specular microscopy showed a 40% reduction in endothelial density in the area of the silicone oil bubble in both groups. Progressive stromal thinning occurred in the rabbit cornea, with gradual development of a retrocorneal membrane at the junction of silicone-endothelial cell contact. In contrast, persistent stromal edema, peripheral vascularization, irregular plaques on the endothelium, and eventual epithelial ulceration and corneal thinning occurred in cat eyes.

Abstract:Silicone oil has been used to fill the vitreous cavity for long-term or permanent internal tamponade in eyes with proliferative vitreoretinopathy or complicated retinal detachment due to ocular trauma, giant retinal tears, proliferative diabetic retinopathy, and cytomegalovirus retinitis. Reports from the Silicone Study confirmed its efficacy in the treatment of proliferative vitreoretinopathy and addressed outcome differences in vitrectomized and nonvitrectomized eyes, combined retinotomy, silicone oil removal, and complications associated with silicone oil tamponade, such as intraocular pressure abnormalities and corneal abnormalities. Because silicone oil is lighter than water and not adequate in supporting the inferior quadrants, several heavier-than-water materials have been introduced for intraocular tamponade. Silicone oil can be a potential vehicle for delivering antiproliferative agents to treat proliferative vitreoretinopathy.

Abstract:Intravitreal silicone oil achieves an effective endotamponade in patients with complex retinal detachments. Silicone oil displacement into the anterior chamber risks glaucoma and endothelial failure. We describe a 52-year-old patient with pseudophakia with silicone oil endotamponade presenting with visual loss and intraocular pressure of 60 mm Hg. Inferior YAG iridotomy was undertaken to repatriate silicone oil to the posterior segment. Despite normal intraocular pressure, acute corneal oedema occurred postiridotomy, resolving spontaneously over 2 weeks. Pupil block glaucoma secondary to silicone oil requires a management approach based on an understanding of silicone oil fluidics. Careful selection of inferior laser iridotomy site is critical to effectively reverse pupil block. Anterior migration of silicone oil in patients with pseudophakia is rare. We offer an hypothesis to explain unanticipated transient corneal oedema following silicone oil displacement from the anterior chamber. Clinicians must discuss the possibility of transient or permanent endothelial failure preoperatively in this patient group.

Abstract:To the Editor The study conducted by Bijon et al addresses a potentially important and underestimated issue pertaining to intravitreal injections (IVIs). Given the substantial number of IVI procedures performed globally, even seemingly minor concerns might merit attention. For instance, the 2018 American Society of Retina Specialists Preferences and Trends survey revealed that 60% of US retina specialists had encountered at least 1 patient with silicone oil (SO) droplets following IVI of bevacizumab, 5% had resorted to vitrectomy to manage symptomatic floaters secondary to SO, and 2% had witnessed patients pursuing legal action due to SO-related floaters. A recent publication has supported a much higher prevalence of SO in the vitreous than previously believed, affecting up to 80% of eyes undergoing routine IVI.

Recognizing intraventricular silicone

Abstract:Retinal detachment with subsequent silicone oil retinopexy is not uncommon. A known complication of silicone retinopexy is intraventricular migration of the intraocular silicone oil. While the oil itself does not result in direct pathology, misdiagnosis may lead to an unnecessary diagnostic workup and possibly predispose the patient to surgery intervention. Silicone oil typically appears hyperdense on computer tomography (CT) and hyperintense on T1-weighted magnetic resonance (MR). These imaging findings may mimic a mass or blood products. However, MR imaging of silicone results in chemical shift artifact which should help narrow the imaging differential. We present a patient with incidental CT and MRI findings which resulted in a prolonged hospital course following misidentification of intraventricular silicone oil. Although the imaging differential for an intraventricular lesion may include metastasis, lymphoma, hemorrhage, choroid plexus papilloma/carcinoma, meningioma, subependymoma, and ependymoma, secondary imaging findings should be noted to ensure an accurate diagnosis. In patients with evidence of prior silicone retinopexy, visualization of an intraventricular lesion with associated chemical shift artifact should raise the possibility of intraventricular silicone oil migration.

Abstract:Purpose: To report five cases of post-operative sterile endophthalmitis in patients who underwent pars plana vitrectomy with silicone oil injection for retinal detachment. Methods: Review of the medical records of five patients who underwent pars plana vitrectomy with silicone oil injection at Southend University Hospital and the Royal Eye Infirmary, Derriford Hospital for rhegmatogenous or tractional retinal detachment. Results: All five patients presented with signs of anterior chamber inflammation exhibiting flare, fibrin and hypopyon in the early or late post-operative period following vitrectomy with silicone oil injection. Treatment with topical antibiotics and steroids was commenced, with rapid resolution of the anterior chamber inflammatory reaction and no further intervention required. Conclusion: The use of silicone oil as a tamponade agent for retinal detachment can rarely induce sterile anterior chamber inflammation with signs resembling infectious endophthalmitis. Onset of symptoms, absence of pain and good response to topical steroids can help differentiate between the two entities.

Abstract:BACKGROUND Endophthalmitis occurring in silicone oil-filled eyes is a very rare occurrence, with reported incidence rates ranging between 0.07% and 0.039%. Traditional methods of management of infectious endophthalmitis include the removal of silicone oil, washout of the vitreous cavity, administration of intravitreal antibiotics, and re-injection of silicone oil. CASE SUMMARY Herein, we report the case of a 39-year-old man with unilateral endophthalmitis after pars plana vitrectomy and silicone oil tamponade. Intravitreal injections of full-dose antibiotics and anterior chamber washout were used to treat the patient. No signs of retinal toxicity were observed during the follow-up period. CONCLUSION Intravitreal full-dose antibiotic injections and anterior chamber washout are promising alternatives to traditional therapies for endophthalmitis in silicone oil-filled eyes.

Abstract:Purpose: To evaluate light and electron microscopic changes of the anterior capsule and its epithelium after clear lens extraction of vitrectomized myopic eyes with silicone oil tamponade. Methods: This prospective, controlled, non-randomized, interventional study included 20 anterior lens capsular specimens that were excised during combined clear lens extraction and silicone oil removal from previously vitrectomized highly myopic patients with silicone oil tamponade for previous retinal detachment surgeries. The specimens were examined via light microscopy and electron microscopy and compared with 20 anterior capsule specimens removed during clear lens extraction of non-vitrectomized highly myopic eyes. Results: Light microscopic examination of clear lens anterior capsule specimens of vitrectomized myopic eyes filled with silicone oil showed relatively more flat cells with irregular outline of lens’ epithelial cells with wide intercellular spaces, deeply stained nuclei, and multiple intracytoplasmic vacuoles. Scanning electron microscopy revealed collagenous surfaces filled with multiple pits, depressions, and abnormal deposits. Transmission electron microscopy revealed lens epithelial cells with apoptotic changes, many cytoplasmic vacuoles, and filopodia-like protrusions between lens epithelial cells and the capsule. Epithelial proliferation and multilayering were also observed. Conclusion: silicone oil may play a role in the development of apoptotic and histopathological changes in clear lens epithelial cells. Clarity of the lens at the time of silicone oil removal does not indicate an absence of cataractous changes. We found justification of combined clear lens extraction and silicone oil removal or combined phacovitrectomy when silicone oil injection is planned, but further long-term studies with larger patient groups are required.

Abstract:Purpose: To evaluate the morphological changes in the cornea by in vivo laser scanning confocal microscopy (LSCM) in a large case series with silicone oil endotamponade after vitreoretinal surgery and to explore the value of LSCM in the early detection of silicone keratopathy (SK). Methods: Ninety-nine patients (99 eyes) with silicone oil endotamponade after vitreoretinal surgery were included in the current study. Slit-lamp examination and measurement of intraocular pressure (IOP) were performed first. Then the central corneas of the subjects' eyes were examined by in vivo LSCM. The analysis of images of each corneal layer was performed and the endothelial cellular density (ECD), endothelial cellular area (ECA), coefficient of variation of cell size (CoV), and percentage of hexagonal cells (PHC) were measured. Moreover, the total size of stromal deposits was measured, and the correlation between the size of deposits and the parameters of endothelial cells was analyzed. Results: Clinically recognizable abnormalities involving the cornea were identified in only 12 eyes (12.1%) under slit-lamp biomicroscopy, whereas in vivo LSCM revealed morphological abnormalities in 40 eyes (40.4%). The manifestations of endothelial lesions varied from decreased cellular density, increased polymegathism and pleomorphism to hyperreflective silicone oil membrane or droplets adhering to the endothelium. Moreover, hyperreflective deposits with various shapes could be identified in both posterior and anterior stroma, along with the infiltration of Langerhans cells beneath the epithelium. The average ECD and PHC of eyes with corneal abnormalities were significantly lower than those of normal corneas, whereas the average ECA and CoV were significantly larger (all Ps < 0.001). The patients with corneal abnormalities were significantly older than those others (P = 0.003). The rate of pseudophakic and aphakic eyes having corneal abnormalities was significantly higher than that of phakic eyes (P = 0.045). Interestingly, the total size of stromal deposits had a significant negative correlation with ECD and PHC but a significant positive correlation with ECA and CoV (all Ps < 0.001). Further correlation analysis performed in groups divided according to the lens status showed similar results and even stronger correlations in aphakic and pseudophakic eyes, whereas no statistically significant correlations were found in phakic eyes. Conclusions: In vivo LSCM was a useful tool in the early detection of corneal abnormalities caused by silicone oil injection, including varying corneal endothelium lesions and stromal abnormalities. Pseudophakic or aphakic eyes, as well as those of older patients, should receive more attention because they are more inclined to develop silicone keratopathy.

Abstract:BACKGROUND We report a case of eye-penetrating injury in which a massive silicone oil migration into the patient’s subconjunctival space and orbit occurred after vitrectomy. CASE SUMMARY A 30-year-old male patient sought medical attention at Ganzhou People’s Hospital after experiencing pain and vision loss in his left eye due to a nail wound on December 9, 2023. Diagnosis of penetrating injury caused by magnetic foreign body retention in the left eye and hospitalization for treatment. On December 9, 2023, pars plana vitrectomy was performed on the left eye for intraocular foreign body removal, abnormal crystal extraction, retinal photocoagulation. Owing to the discovery of retinal detachment at the posterior pole during surgery, silicone oil was injected to fill the vitreous body, following which upper conjunctival bubble-like swelling was observed. Postoperative orbital computed tomography (CT) review indicated migration of silicone oil to the subconjunctival space and orbit through a self-permeable outlet. On December 18, 2023, the patient sought treatment at the First Affiliated Hospital of Nanchang University, China. The patient presented with a pronounced foreign body sensation following left eye surgery. On December 20, 2023, the foreign body was removed from the left eye frame and an intraocular examination was conducted. The posterior scleral tear had closed, leading to termination of the surgical procedure following supplementary laser treatment around the tear. The patient reported a significant reduction in ocular surface symptoms just one day after surgery. Furthermore, a notable decrease in the migration of silicone oil was observed in orbital CT scans. CONCLUSION The timing of silicone oil injection for an eye-penetrating injury should be carefully evaluated to avoid the possibility of silicone oil migration.
