Abstract:BACKGROUND: The anesthetic side effects of propofol still occur in clinical practice because no reliable monitoring techniques are available. In this regard, continuous monitoring of propofol in breath is a promising method, yet it remains infeasible because there is large variation in the blood/exhaled gas partial pressure ratio (R-BE) in humans. Further evaluations of the influences of breathing-related factors on R-BE would mitigate this variation. METHODS: Correlations were analyzed between breathing-related factors (tidal volume [TV], breath frequency [BF], and minute ventilation [V-M]) and R-BE in 46 patients. Furthermore, a subset of 10 patients underwent pulmonary function tests (PFTs), and the parameters of the PFTs were then compared with the R-BE. We employed a 1-phase exponential decay model to characterize the influence of V-M on R-BE. We also proposed a modified R-BE (R-DCM) that was not affected by the different breathing patterns of the patients. The blood concentration of propofol was predicted from breath monitoring using R-BEM and R-BE. RESULTS: We found a significant negative correlation (R = -0.572; P < .001) between V-M and R-BE (N = 46). No significant correlation was shown between PFTs and R-BE in the subset (N = 10). R-BEM demonstrated a standard Gaussian distribution (mean, 1.000; standard deviation [SD], 0.308). Moreover, the predicted propofol concentrations based on breath monitoring matched well with the measured blood concentrations. The 90% prediction band was limited to within +/- 1 mu g.mL(-1). CONCLUSIONS: The prediction of propofol concentration in blood was more accurate using R-BEM than when using R-BE and could provide reference information for anesthesiologists. Moreover, the present study provided a general approach for assessing the influence of relevant physiological factors and will inform noninvasive and accurate breath assessment of volatile drugs or metabolites in blood.
The Accuracy of Target-Controlled Infusion of Propofol in Chinese

Abstract:Objective To evaluate the accuracy of target-controlled infusion (TCI) of propofol using pharmacokinetic parameters reported by Marsh to predict plasma propofol concentration in Chinese. MethodsTwenty-two ASA I - II patients were divided into two groups: group Y aged 65 yr (n = 11) and group E ages 65 yr ( n = 11). Patients with liver, kidney or cardiovascular diseases were excluded. The patients were premedicated with pethidine 50mg and phenobarhital 0.1 g im. Radial artery and internal jugular vein(IJV) were cannulated. The pharmacokinetic parameters incorporated in the Graseby 3500 pump we used were: V1=228 ml-kg-1 , K10 =0.119 min-1 ,K12=0.112 min-1, K2l=0.055min-1 , K13 =0.0419 min-1 ,K31 =0.0033 min-1. Target concentration was started with 2ug-ml-1 and increased at increment of 1ugml-1 until loss of consciousness. The patient was then intubated. When target concentration of propofol was increased, the concentration of inhalation anesthetic was reduced to maintain hemodynamic stability. When target concentration of propofol was increased, arterial blood sample was taken 1-3 times for determination of plasma propofol concentration measured by HPLC (Agilent 1100) . Then blood samples every 10-15 min. For each sample prediction error(PE) and constancy error(CE) were calculated. For each patient median prediction error(MDPE), median absolute prediction error(MDAPE) . Median absolute constancy error (MDACE) and median constancy error (MDCE) were calculated.ResultsThere was remarkable initial overshot. PE and absolute PE were 63.3 % and 66.2 % in group E and 62.1 % and 62.7% in group Y. CE and absolute CE were -0.3% and 12.7% in group E and 0.6% and 13.5% in group Y. The median value of MDPE ( = the median value of MDAPE) was 78.1 % in group E and 66.1% in group Y. The median value of MDCE was 0.2% (group E) and 0.8% (group Y) and MDACE was 12.5% (group E) and 13.5% (group Y) . The measured concentrations were significantly linearly correlated with the premedicated concentrations. Conclusion TCI system with propofolpharmacokinetic parameters reported by Marsh can lead to initial overshot and underestimate the measured plasma propofol concentration but it can maintain a stable plasma concentration

Abstract:OBJECTIVE To analyze population pharmacodynamics of propofol in neurosurgical patients and to discuss the influences of the age, weight or gender on the propofol pharmacodynamics. METHODS Twenty-seven ASA Ⅰ～Ⅱ elective neurosurgical patients were treated with propofol 2 mg·kg~ -1 for 5 min and maintained the infusion of propofol 10 mg·kg~ -1 ·h~ -1 for 5 min. CSF and arterial blood were collected simultaneously before infusion and at different time points after infusion. The concentrations of propofol were measured with HPLC with fluorescence detection. At the same time of sample collection,BIS index(BIS)was recorded. Model simulation was performed and the influences of age, weight, and gender were investigated by using NONMEM program.RESULTS The parameters of a and b for linear addictive model were 1.11 and 95.4; the parameters of a and b for linear exponential model were 1.05 and 92.7; the parameters of E_ max , EC_ 50 and N for Sigmoid E_ max model were 119,53.6 and 1.51, respectively. The predicted concentrations of CSF showed a good correlation with the measured concentrations(r~2 were 0.911 2, 0.9146, and 0.926, respectively). CONCLUSION Linear additive, linear exponential and Sigmoid E_ max models can describe the pharmacodynamics of propofol.
A comparison between target-controlled and manually controlled propofol infusions in patients undergoing routine surgical procedures.

Abstract:Background and objective Previous studies have shown that target-controlled infusion (TCI) was associated with increased propofol consumption as compared with manually controlled infusion (MCI). However, most studies lacked an objective measurement of anaesthetic depth. We compared TCI and MCI with regard to propofol dose requirement while monitoring depth of anaesthesia with bispectral index (BIS) and midlatency auditory-evoked potentials (MLAEPs). Methods Fifty ASA 1 or 2 patients undergoing elective ENT surgery were randomly allocated to the TO group (n = 25) or the MCI group (n = 25). BIS, MLAEPs, haemodynamic parameters and propofol plasma concentration were determined at various stages before, during and after anaesthesia. During anaesthesia, BIS values were always maintained between 40 and 60. Results Anaesthesia significantly increased latency of Pa and Nb compared with when patients were awake (P<0.01). After extubation, the latency of Pa and Nb significantly decreased and basically returned to baseline level. BIS and latency of both Pa and Nb did not differ between groups. Also, the propofol induction dose (mean +/- SD: TCI, 2.09 +/- 0.36 mg kg(-1); MCI, 2.06 +/- 0.16 mg kg(-1)) and total dose (TCI, 5.57 +/- 0.96 mg kg(-1) h(-1); MC, 5.79 +/- 0.87 mg kg(-1) h(-1)) and measured plasma propofol concentration during anaesthesia did not differ between groups. At 15 min after intubation, the mean predicted plasma concentration (2.2 +/- 0.2 mu g ml(-1)) differed significantly from the measured plasma concentration (2.8 +/- 1.2 mu g ml(-1)). No correlation was observed between predicted and measured propofol plasma concentrations. Conclusion Titration of propofol to achieve a comparable depth of anaesthesia results in comparable propofol consumption and recovery variables regardless of the type of propofol administration. Predictive pharmacodynamic performance of the Eleveld pharmacokinetic-pharmacodynamic model for propofol: comparison of predicted and measured bispectral index

Abstract:Background: The Eleveld pharmacokinetic-pharmacodynamic model for propofol predicts bispectral index (BIS) processed electroencephalogram values from estimated effect-site concentrations. We investigated agreement between measured and predicted BIS values during total intravenous anaesthesia (TIVA). Methods: Forty participants undergoing lower limb surgery received TIVA using remifentanil target-controlled infusions and propofol by manually controlled, target-guided infusions based upon the Eleveld model and directed by two pharmacokinetic computer simulation applications: PKPD Tools and StelSim. We evaluated the predictive performance of the Eleveld model by calculating median prediction errors (BIS units) and by Bland-Altman analyses. We also performed |Bland-Altman analysis of supplementary data provided by the authors of the Eleveld model. Results: Whereas median prediction errors were small (MDPE -1.9, MDAPE 10), the ranges were wide (-18.5 to 24.3 and 1.7 to 24.3). The proportion of MDAPE >10 BIS units was 47.8%. Bland-Altman analysis showed a small mean bias (-0.52 BIS units) with wide limits of agreement (-27.7 to 26.2). Each participant's limits of agreement did not meet the requirements for declaring interchangeability between the two measurements. The measurement differences depended on the BIS values, as indicated by the positive slopes of the differences vs BIS values. Bland-Altman analysis of the Eleveld model supplementary data revealed similar results. Conclusion: BIS predictions by the Eleveld model should be interpreted with caution. In spite of the acceptable MDPE and MDAPE, there are unacceptable degrees of both within-subject and between-subject variation during propofol target-controlled infusions. This limits the use of adjusting targeted concentrations to achieve desired simulated BIS values with confidence.
Concentrations of Propofol in Cerebral Spinal Fluid: Target-Controlled Infusion.

Abstract:Background Although the performance of target-controlled infusion (TCI) have been studied extensively, the accuracy and safety of a TCI system that targets the effect site remains to be demonstrated. This study was to investigate the relations of TCl of propofol to its concentrations in cerebral spinal fluid (CSF), the effect-site concentrations and bispectral index (BIS). Methods Twelve mongrel dogs were used for investigations. The target effect-site concentration was set at 3 μg/ml and the infusion was lasted for 15 minutes. CSF and blood samples were then collected and propofol concentrations were determined by using high performance liquid chromatography with fluorescence detection. BIS and hemodynamic data were monitored continuously. Results The predicted plasma concentrations were generally overestimated. Median performance error (MDPE) and absolute median performance error (MDAPE) were -10.0% and 29.9% respectively. Propofol CSF concentrations were much lower than its effect-site concentrations. Changes in BIS were consistent with propofol concentrations in CSF, both of which changed direction at 5 minutes while the effect-site concentrations relatively lagged behind. Better correlation (r2=0.9195) was found between BIS and CSF concentrations, when compared with that between BIS and effect-site concentrations (r2=O.554). Conclusion With 1% enflurane inhaled, the inconsistency of drug effect to the effect-site concentrations may result from inaccuracy of pharmacokinetic parameters. Abstract:Propofol's pharmacokinetics have been extensively studied using human blood samples and applied to target-controlled infusion systems; however, information on its concentration in the brain remains scarce. Therefore, this study aimed to simultaneously measure propofol plasma and brain concentrations in patients who underwent awake craniotomy and establish new pharmacokinetic model. Fifty-seven patients with brain tumors or brain lesions who underwent awake craniotomy were sequentially assigned to model-building and validating groups. Plasma and brain (lobectomy or uncapping margins) samples were collected at five time-points. The concentration of propofol was measured using high-performance liquid chromatography. Population pharmacokinetic analysis was conducted through a nonlinear mixed-effects modeling program using a first-order conditional estimation method with interactions. Propofol's brain concentrations were higher than its plasma concentrations. The measured brain concentrations were higher than the effect site concentrations using the previous models. Extended models were constructed based on measured concentrations by incorporating the brain/plasma partition coefficient (K p value). Extended models showed good predictive accuracy for brain concentrations in the validating group. The K p value functioned as a factor explaining retention in the brain. Our new pharmacokinetic models and Kp value can predict propofol's brain and plasma concentrations, contributing to safer and more stable anesthesia.

Effect of Remifentanil on Propofol Pharmacokinetics with Target Controlled Infusion in Elderly Patients

Abstract:OBJECTIVE:To investigate the effects of remifentanil administered by target-controlled infusion (TCI) with different plasma concentration and time on pharmacokinetics of propofol in elderly patients.METHODS:Thirty-two ASA I-II patients (65-82 years old) undergoing elective lower abdominal operations were divided into 4 groups randomly: TCI propofol combined with epidural block (group A, n=8); TCI remifentanil with plasma concentration 4 μg/L and propofol (group B, n=8); TCI remifentanil with plasma concentration 7 μg/L and propofol (group C, n=8); and TCI propofol and remifentanil (plasma concentration 4 μg/L) with infusion time longer than 4 hours (group D, n=8). Propofol was infused by target-controlled infusion with plasma concentration 3 mg/L in the 4 groups. bispetral index (BIS), heart rate (HR), blood pressure (BP) were recorded during operation. Blood samples were collection from radial arterial catheter. Samples of 2 mL of arterial blood were taken at 0, 1, 3, 5, 10, 15, 30 min and then every 30 min after the start of the infusion of propofol, and at 0, 2, 4, 6, 8, 10, 15, 30, 45, 60, 120, 240, 360, 480, 720, 1440 min after the termination of the infusion of propofol. Propofol concentrations in blood were measured by reversed-phase high-performance liquid chromatography (HPLC). The pharmacokinetics analyses were performed using the nonlinear mixed-effects model approach implemented in NONMEM computer program. After obtaining the best NONMEM model with covariates, the influence of coadministered remifentanil on the model was examined.RESULTS:In all the patients, the depth of anesthesia was enough (BIS value=40-60), and the circulatory system function was stable during operation. The final model of propofol pharmacokinetics in the three groups (A+B, A+C, and A+D groups) was best described by a three-compartment mammillary model. The values of objective function (OFV) were -810.1, -714.4, and -896.4. Addition of remifentanil covariate effects to the final model resulted in no improvement in the objective function.CONCLUSION:target-controlled infusion of propofol combined with different plasma concentration of remifentanil or remifentanil infusing more than 4 hours had no effect on pharmacokinetics of propofol in elderly patients.
Implementation of a Bayesian based advisory tool for target-controlled infusion of propofol using qCON as control variable

Abstract:This single blinded randomized controlled trial aims to assess whether the application of a Bayesian-adjusted CePROP (effect-site of propofol) advisory tool leads towards a more stringent control of the cerebral drug effect during anaesthesia, using qCON as control variable. 100 patients scheduled for elective surgery were included and randomized into a control or intervention group (1:1 ratio). In the intervention group the advisory screen was made available to the clinician, whereas it was blinded in the control group. The settings of the target-controlled infusion pumps could be adjusted at any time by the clinician. Cerebral drug effect was quantified using processed EEG (CONOX monitor, Fresenius Kabi, Bad Homburg, Germany). The time of qCON between the desired range (35-55) during anaesthesia maintenance was defined as our primary end point. Induction parameters and recovery times were considered secondary end points and coefficient of variance of qCON and CePROP was calculated in order to survey the extent of control towards the mean of the population. The desired range of qCON between 35 and 55 was maintained in 84% vs. 90% (p = 0.15) of the case time in the control versus intervention group, respectively. Secondary endpoints showed similar results in both groups. The coefficient of variation for CePROP was higher in the intervention group. The application of the Bayesian-based CePROP advisory system in this trial did not result in a different time of qCON between 35 and 55 (84 [21] vs. 90 [18] percent of the case time). Significant differences between groups were hard to establish, most likely due to a very high performance level in the control group. More extensive control efforts were found in the intervention group. We believe that this advisory tool could be a useful educational tool for novices to titrate propofol effect-site concentrations.
Data-driven identification and comparison of full multivariable models for propofol–remifentanil induced general anesthesia

Abstract:In this paper, we present results with clinical data to enable a 2x2 input–output multivariable patient model for hypnosis and analgesia. Nonlinear multi-drug interaction models are identified from data recorded from 70 patients undergoing surgery during total intravenous anesthesia (TIVA) with several medical monitors for variables such as Bispectral Index, Nociception level (Medasense), skin conductance (Medstorm) and advanced spectral analysis conductance (AnspecPro). Bispectral index measures the depth of hypnosis (lack of consciousness), while nociception related indices from Medasense, Medstorm, and AnspecPro devices measure levels related to analgesia (lack of reaction to noxious stimuli). A comparison is given among three response surface model (RSM) structures – Minto, Greco, and Reduced Greco – for hypnotic and analgesic states during Propofol–Remifentanil interaction. The identified models capture the pharmacodynamic properties of dose–effect concentrations of Propofol/Remifentanil while the pharmacokinetic part of the patient model is calculated from patient's biometric values using Schnider/Minto (SM), and Eleveld/Eleveld (EE) models. In presence of strict clinical protocols delivering data under poor identifiability conditions, we propose two methods of identification: (i) based on steady-state gains, and (ii) using all available data which includes part of the dynamic transient. The model parameters are optimized with Genetic Algorithm based on a goodness of fit performance measure complemented with root mean square error. The results suggest that the EE model combination is advantageous for Bispectral index pharmacokinetic modeling at the cost of numerical complexity, therefore reducing the uncertainty left to be identified in the pharmacodynamic part of the patient model. By contrast, the SM model combination is less computationally demanding and provides some improvement in the RSM accuracy for nociception level indicators. The comparison of three devices for nociception levels evaluation suggests that clinical data captured with the Medasense monitor provides best fitted RSMs with the Reduced Greco RSM structure, despite having fewer parameters.

Remifentanil pharmacodynamics during conscious sedation using algometry: a more clinically relevant pharmacodynamical model

Abstract:Background: The Minto remifentanil pharmacokinetic/pharmacodynamic (PK/PD) model is used in target-controlled infusion (TCI) devices. The endpoint used to calculate the PD parameters, including the ke0, was the electroencephalogram (EEG), which only changes at high remifentanil concentrations. As the ke0 should adequately predict the time course of drug effects at clinically relevant concentrations, we evaluated the temporal agreement between effect-site concentrations estimated with the Minto model and pressure pain thresholds during conscious sedation. Methods: We enrolled 100 patients scheduled for gynaecological surgery. The first group (35 subjects) received an effect site targeted remifentanil infusion (target 1.5 ng ml-1); the second group (35 subjects) received the same infusion and a 1 mg bolus of midazolam to evaluate anxiolytic effects; the control group (30 subjects) received a saline infusion. Algometry and vital signs were measured at different time points. Results: The Minto model predicted stable effect-site concentrations within 1.5 min of starting the infusion. Haemodynamic variables stabilised within 5 min, whereas there was a significant increase in pressure pain threshold for up to 15 min in both remifentanil groups. Midazolam had no effect on pressure pain threshold. A PD model based on algometry and Minto PK model was developed. Conclusions: Our results demonstrate the limitation of the Minto PD model at low target remifentanil concentrations, with a discrepancy in the time course between EEG and pressure pain threshold changes. Clinicians should be aware that the time course of onset of analgesic effects is slower than the estimates of the Minto model. Investigators should consider using algometry data in future opioid PD modelling studies.

Abstract:Background Remifentanil may have a dose‐dependent haemodynamic effect during the induction of general anaesthesia combined with propofol. Our objective was to investigate whether systolic arterial blood pressure (SAP) was reduced to a greater extent when the remifentanil dose was increased. Methods This randomised, double‐blind, dose‐controlled study was conducted at the Day Surgery Unit of Haugesund Hospital, Norway. Ninety‐nine healthy women scheduled for gynaecological surgery were randomly allocated in a 1:1:1 ratio to receive remifentanil induction with a low, medium or high dose corresponding to maximum effect‐site concentrations (Ce) of 2, 4 and 8 ng/mL. The induction dose of propofol was 1.8 mg/kg, with a Ce of 2.9 μg/mL. Anaesthesia was induced using target‐controlled infusion. After 150 s of sedation, a bolus of remifentanil and propofol was administered. Baseline was defined as 55–5 s before the bolus dose, and the total observation time was 450 s. We used beat‐to‐beat haemodynamic monitoring with LiDCOplus. The primary outcome variable was the maximum decrease in SAP within 5 min after bolus administration of remifentanil and propofol. Absolute and relative changes from baseline to minimal values and the area under the curve (AUC) were used as effect measures. Comparisons of groups were performed using analysis of variance (ANOVA). Results Median remifentanil doses were 0.75, 1.5 and 3.0 μg/kg in the low‐, medium‐ and high‐dose groups, respectively. The absolute changes (mean ± standard deviation) in SAP in the low‐, medium‐ and high‐dose groups of remifentanil were −39 ± 9.6 versus −43 ± 9.1, and −41 ± 10 mmHg, respectively. No difference (95% confidence interval) in the absolute change in SAP was observed between the groups (ANOVA, p = .29); medium versus low dose 3.7 (−2.0, 9.4) mmHg, and high versus medium dose −2.2 (−8.0; 3.5) mmHg. The relative changes from baseline to minimum SAP values were −30% versus −32% versus −32% (p = .52). The between‐group differences in the AUC were not statistically significant. Relative changes in heart rate (−20% vs. −21% vs. −21%), stroke volume (−19% vs. −16% vs. −16%), cardiac output (−32% vs. −32% vs. −32%), systemic vascular resistance (−24% vs. −27% vs. −28%), and AUC were not statistically significant. Conclusion This trial demonstrated major haemodynamic changes during the induction of anaesthesia with remifentanil and propofol. However, we did not observe any statistically significant differences between low, medium or high doses of remifentanil when using continuous invasive high‐accuracy beat‐to‐beat monitoring.

Propofol–Bispectral Index (BIS) Electroencephalography (EEG) Pharmacokinetic-Pharmacodynamic Model in Patients With Post–Cerebral Hemorrhage Hydrocephalus

Abstract:Background. Titrating hypnotic agents for patients who suffer from a cerebral insult is a challenging task. To date there is no real gold standard to precisely quantify electroencephalography (EEG) response in a fashion that could be utilized for patients with post–cerebral hemorrhage hydrocephaly. While we must administer “as per usual” analgesics for noxious stimuli, we have to administer the hypnotic agents more “sparingly” due to lack of objective monitoring. Methods. We compared 15 adult post–cerebral hemorrhage hydrocephalus patients undergoing ventriculo-peritoneal shunt placement with 15 controls matched for gender and approximate age. We set propofol target controlled infusion estimated plasma concentrations (C p ) to gradually reach 4 μg/mL over 4 minutes. To precisely quantify post–cerebral hemorrhage mental dysfunction, we used electronically retrieved bispectral index (BIS) and propofol C p data points to create individual inhibitory monophasic mathematical model for each patient that incorporates an independent hysteresis “lag” function. Results. In post–cerebral hemorrhage patients C p -BIS curve, C 50 (propofol concentration associated with inhibitory 50% BIS response) cutoff point was significantly shifted to the left by 39%. Whereas before infusion and at stable propofol 4 μg/mL aneurismal surgical sides ipsilateral (75 ± 13, 25 ± 9) and contralateral (73 ± 15, 27 ± 9) mean ± SD BIS values were significantly lower than ipsilateral (95 ± 3, 46 ± 12) and contralateral (94 ± 3, 46 ± 12) matched controls. Conclusions. Using BIS as surrogate marker of propofol hypnotic effect, BIS monitoring in patients with post–cerebral hemorrhage hydrocephaly showed a pattern of change and trend that was similar albeit 39% significantly lower than subjects without.

Insights into the unusual alpha adrenoceptor subtype in dog saphenous vein using phenoxybenzamine.

Abstract:In the dog saphenous vein (DSV), phenylephrine (PE) responses through alpha-1 adrenoceptors receptors are antagonized by both alpha-1 and alpha-2 receptor antagonists. Furthermore, pretreatment with chloroethylclonidine (CEC) eliminates prazosin binding but reduces rauwolscine binding by half (). In new functional experiments, the effects of preincubation with phenoxybenzamine (PBZ), an irreversible alpha adrenoceptor antagonist, on responses to PE and two selective alpha-2 adrenoceptor agonists were evaluated. Also, the ability of prazosin or rauwolscine to prevent irreversible losses of responses to these agonists when coincubated with PBZ was determined. Preincubation in PBZ (10-300 nM) concentration dependently reduced PE Emax and the calculated fraction of residual receptors (q). Preincubation in PBZ (10-300 nM) increased KB values for prazosin (30 and 100 nM) but did not alter the KB value for rauwolscine (50 nM) acting at the residual receptors from control values. Coincubation of PBZ with prazosin partially prevented these PBZ actions (Emax partly restored) on responses to PE, but coincubation of rauwolscine (</=1 microM) with PBZ, did not. Rauwolscine competitively inhibited responses to two alpha-2 adrenoceptor agonists (Schild plot pA2 values near 9). Preincubation with PBZ concentrations of >/=300 nM caused >50% reduction in Emax values of responses but did not alter the EC50 values for either agonist. Coincubation of rauwolscine with PBZ protected responses to alpha-2 agonists against PBZ (1 microM) effects. This study shows that PE initiates contractions at atypical alpha-1 adrenoceptors represented by all sites of PE action. Rauwolscine antagonizes PE actions but does not protect against PBZ inactivation. Typical alpha-2 adrenoceptors are distinguished from the unusual alpha-1 adrenoceptors by their lesser sensitivity to PBZ and their protection by rauwolscine from PBZ.
Mechanisms of Propofol Attenuation of Ketamine-Induced Neonatal Brain Injury

Abstract:OBJECTIVE: We studied the mechanisms of protective effects of propofol on ketamine-induced damage to neonatal cognitive function.MATERIALS AND METHODS: We utilized a rat model of ketamine anaesthesia. Eighty neonatal rats (7 days after birth) were divided into four groups: normal saline group, ketamine group, and low-and high-dose propofol combined with ketamine groups. Six hours after anaesthesia, we obtained hippocampal tissue, and quantified apoptotic index and total protein concentration, and assessed global proteomics changes induced by two tested drugs. The latter changes were documented by two-dimensional electrophoresis and matrix-assisted laser desorption/ionization time of flight mass spectrometry. To evaluate cognitive functions, water maze test was applied after animals grew for 21 days. We further repeated proteomics studies at 21 days post-anaesthesia.RESULTS: Ketamine markedly up-regulated apoptotic index and decreased total protein concentration. Propofol dose-dependently reverted these adverse changes. Six hours post-anaesthesia, combined propofol and ketamine administration up-regulated the following proteins in the hippocampus: PD1A3, NDUFB10, HSPA8, ATP5JD, and PSMA1. Furthermore, the following proteins were down-regulated: PPIA, PKM2, GFAP, NSE, PPIA, PKM2, and GFAP. After 21 days, animals treated with ketamine showed marked disturbances in cognitive function as demonstrated by increased time of the water maze test, whereas propofol diminished these changes. In addition, expression of proteins largely normalized in propofol-treated animals, with only two up-regulated proteins (FUBP3 and PRDX5) and three down-regulated proteins (GAPDH, AKR1A1, and VCP).CONCLUSIONS: Adverse effects of ketamine on cognitive function are reverted by propofol, also through beneficial effects on protein expression in the hippocampus.

Abstract:Objective To observe the patients hemodynamic reactions to the different stimula-tions under different blood concentrations of propofol with remifentanil.Exerting the response surface methodology,the half of the maximum effective concentration of propofol and the interaction index of propofol with remifentanil were calculated.Methods A total of 120 patients for scheduled abdominal surgery,randomly divided into 12 groups(n = 10).TCI 12 different concentration combinations of propofol with remifentanil,also the the hemodynamic reactions to harmful stimulations of intubation and incising were observed.The data were analyzed by responding surface analysis.Results The in-teraction index of propofol and remifentanil under intubation was 0.523 0±0.223 7,that under inci-sing 0.361 3±0.148 4,half of the maximum effective concentration of propofol for intutation and in-cising was (6.878 7 ± 1.832 1 ) μg/ml and (4.41 1 6 ± 0.902 5 ) μg/ml respectively. Conclusion Propofol and remifentanil have a synergistic inhibitory effect on harmful cardiovascular reaction among Chinese.During the endotracheal intubation,the requirements of half of the maximum effective concentration of propofol and intensity of the synergy with remifentanil are greater than those during the incising.
Pharmacodynamic Analysis of the Influence of Propofol on Left Ventricular Long-Axis Systolic Performance in Cardiac Surgical Patients

Abstract:Background: Propofol induced a decline in the left ventricular (LV) systolic performance in non-cardiac surgery. We tested the hypothesis that propofol decreased the LV contractile function by dose dependent manner in cardiac surgery patients. Methods: Anesthesia was maintained with target-controlled infusions of propofol and remifentanil in cardiac surgery patients. With a fixed effect-site concentration (Ce) of remifentanil (20 ng/mL) after sternotomy, the Ce of propofol was adjusted to maintain a Bispectral index of 40-60 (Ce1). Mitral annular Doppler tissue image tracings and other echocardiographic variables, including end-diastolic and end-systolic volumes, stroke volume, and mitral inflow pulse wave Doppler profile at Ce1, were recorded using transesophageal echocardiography. Echocardiographic recordings were repeated after the Ce-values of propofol were doubled and tripled at 10-minute intervals (defined as Ce2 and Ce3, respectively). Serial changes in echocardiographic variables for each Ce of propofol were assessed using generalized linear mixed effect modeling. The pharmacodynamic relationship between the Ce of propofol and peak systolic mitral annular velocity (Sm) was analyzed by logistic regression using non-linear mixed effect modeling (NONMEM). Results: Means of Ce1, Ce2, and Ce3 were 0.8, 1.6, and 2.4 μg/mL, respectively, and their means of Sm (95% confidence interval) were 9.7 (9.3-10.2), 8.7 (8.2-9.1), and 7.5 cm/sec (7.0-8.0), respectively (P < 0.01). Ce values of propofol and Sm showed a significant inter-correlation and predictability (intercept, 10.8; slope-1.0 in generalized mixed linear modeling; P < 0.01). Ce values producing 10% and 20% decline of Sm with 50%-probability were 1.4 and 2.1 μ/mL, respectively. Conclusion: Propofol reduces LV systolic long-axis performance in a dose-dependent manner. Real-Time Personalised Pharmacokinetic-Pharmacodynamic Modelling in Propofol Anesthesia through Bayesian Inference

Abstract:Pharmacological models describe a patient's response to the administration of a medicinal drug based on parameters derived from population studies. However, considerable inter-patient variability exists, such that population models may underperform when used to predict the actual response of a specific individual. In applications which demand predictive accuracy-such as target-controlled infusion of anesthetic agents-modeling uncertainty may reduce system dependability and introduce clinical risk. Our work investigates the use of Bayesian inference, implemented through a particle filter algorithm, to refine a prior model of propofol pharmacokinetics-pharmacodynamics and estimate patient-specific parameters in real-time. We report here on an observational clinical study conducted on 40 adults undergoing general anesthesia, where we evaluated the performance of Bayesian inference-personalized models in forecasting forward trends of depth of anesthesia (Bispectral Index) measurements and compared it with that of a traditional population-based pharmacological model. Our results show a significant reduction in prediction error metrics for the patient-specific models. Our study demonstrates the viability and practical implementability of Bayesian inference as a tool for real-time intra-operative estimation of personalized pharmacological models in anesthesia applications.
