Abstract:To investigate the efficacy of sequential butylphthalide therapy combined with urinary kallidinogenase in the treatment of acute cerebral infarction complicated with cognitive dysfunction. Sixty-two patients with acute cerebral infarction complicated with cognitive impairment admitted to our Hospital from January 2022 to May 2022 were recruited by convenience sampling method. They were randomly divided into two groups using a random number table: the treatment group ( n = 31) and the control group (n = 31). Patients in the treatment group received sequential butylphthalein therapy combined with urinary kallidinogenase, while patients in the control group were treated with traditional therapy. The efficacy of the two groups was compared. Different treatment methods showed different effects on the NIHSS score, MMSE score, MoCA score and mRS score of the two groups ( F NIHSS treatment = 142.414, P < 0.001; F MMSE treatment = 87.418, P < 0.001; F MoCA treatment = 122.724, P < 0.001; F mRS treatment = 91.341, P < 0.001), and the scores of each scale in the treatment group were better than those in the control group at each time point, with statistically significant differences ( P < 0.05). In terms of efficacy comparison, the response rate of the treatment group was higher than that of the control group (83.87% VS 61.29%, χ 2 = 3.971, P = 0.046). No statistically significant difference was observed in the incidence of adverse reactions between the two groups (19.35% VS 16.13%, χ 2 = 0.111, P = 0.740). Sequential butylphthalide therapy combined with urinary kallidinogenase boasts improved efficacy in patients with acute minor stroke, improving their neurological deficits and promoting neurological recovery. Meanwhile, urinary kallidinogenase combined with butylphthalide can ensure better efficacy at the same incidence of complications.

Safety of Early Antiplatelet Therapy for Non-Cardioembolic Mild Stroke Patients with Thrombocytopenia.

Abstract:OBJECTIVES:To investigate the safety of early antiplatelet therapy for non-cardioembolic mild stroke patients with thrombocytopenia. METHODS:Data of acute ischemic stroke patients with baseline National Institutes of Health Stroke Scale (NIHSS) score ≤3 and a platelet count <100×109/L were obtained from a multicenter register. Those who required anticoagulation or had other contraindications to antiplatelet therapy were excluded. Short-term safety outcomes were in-hospital bleeding events, while the long-term safety outcome was a 1-year all-cause death. The short-term neurological outcomes were evaluated by modified Rankin scale (mRS) score at discharge. RESULTS:A total of 1868 non-cardioembolic mild stroke patients with thrombocytopenia were enrolled. Multivariate regression analyses showed that mono-antiplatelet therapy significantly increased the proportion of mRS score of 0-1 at discharge (OR=1.657, 95%CI: 1.253-2.192, P<0.01) and did not increase the risk of intracranial hemorrhage (OR=2.359, 95%CI: 0.301-18.503, P>0.05), compared with those without antiplatelet therapy. However, dual-antiplatelet therapy did not bring more neurological benefits (OR=0.923, 95%CI: 0.690-1.234, P>0.05), but increased the risk of gastrointestinal bleeding (OR=2.837, 95%CI: 1.311-6.136, P<0.01) compared with those with mono-antiplatelet therapy. For patients with platelet counts ≤75×109/L and >90×109/L, antiplatelet therapy significantly improved neurological functional outcomes (both P<0.05). For those with platelet counts (>75-90)×109/L, antiplatelet therapy resulted in a significant improvement of 1-year survival (P<0.05). For patients even with concurrent coagulation abnormalities, mono-antiplatelet therapy did not increase the risk of various types of bleeding (all P>0.05) but improved neurological functional outcomes (all P<0.01). There was no significant difference in the occurrence of bleeding events, 1-year all-cause mortality risk, and neurological functional outcomes between aspirin and clopidogrel (all P>0.05). CONCLUSIONS:For non-cardioembolic mild stroke patients with thrombocytopenia, antiplatelet therapy remains a reasonable choice. Mono-antiplatelet therapy has the same efficiency as dual-antiplatelet therapy in neurological outcome improvement with lower risk of gastrointestinal bleeding.
Effect of butylphthalide combined with edaravone d borneol on efficacy in elderly patients with acute cerebral infarction

Abstract:Purpose: To investigate the efficacy of butylphthalide combined with edaravone d-borneol in elderly patients with acute cerebral infarction. Methods: From October 2020 - October 2021, 166 elderly patients suffering from acute cerebral infarction in Affiliated Hospital of Youjiang Medical College for Nationalities, Baise City, China were selected as the study subjects, and randomly divided into study and control groups, respectively, with 83 patients each. Control group was treated with edaravone d-borneol injection (15 mL of edaravone d-borneol in 100 mL of saline), while the study group was treated with butylphthalide and sodium chloride injection, combined with edaravone d-borneol injection (15 mL of edaravone d-borneol and 30 mL of butylphthalide and sodium chloride in 100 mL of saline). Clinical efficacy, as well as the levels of cytokines, viz, tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and IL-8, vascular endothelial function indicators, namely, nitric oxide (NO) and vascular endothelial growth factor (VEGF) as well as oxidative stress indicators, viz, malondialdehyde (MDA), glutathione peroxidase (GSH-Px) and superoxide dismutase (SOD) of the two groups were evaluated. Results: Total efficacy/effectiveness in the study group was 93.98 %, which was significantly higher than 73.49 % in the control group (p < 0.05). Post-treatment, the levels of cytokines, vascular endothelial function and oxidative stress in the two groups were significantly improved compared with the values before treatment (p < 0.05). Furthermore, patients in the study group showed better treatment outcomes when compared with control group (p < 0.05). Conclusion: The combination of butylphthalide and sodium chloride injection with edaravone d-borneol injection (15 mL of edaravone d-borneol and 30 mL of butylphthalide and sodium chloride in 100 mL of saline) is more effective than edaravone d-borneol injection (15 mL of edaravone d-borneol in 100 mL of saline) alone in the treatment of elderly patients with acute cerebral infarction, as it regulates microcirculation and improves cellular inflammatory state and oxidative stress reaction. However, further clinical trials should be conducted prior to application in clinical practice.

Influences of Butylphthalide Injection in Combination with Edaravone on Clinical Efficacy and Cytokines in Elderly Acute Cerebral Infarction Patients

Abstract:Background: Acute cerebral infarction threatens human health and life safety. The edaravone is a new antioxidant and hydroxyl radical scavenger, which is used in the treatment of cardiovascular diseases. However, long-term use of edaravone is prone to drug resistance, resulting in limited therapeutic effect. Butylphthalein can improve collateral circulation, protect the integrity of vascular endothelial cells, improve vasospasm, increase blood perfusion in the ischemic area of brain tissue, and promote the recovery of cerebral nerve function. Objective: To measure the influences of butylphthalide injection in combination with edaravone on clinical efficacy and cytokines in elderly acute cerebral infarction patients, aiming to provide reference for the treatment of acute cerebral infarction. Design: This was a retrospective study. Setting: This study was performed in Yantaishan Hospital. Participants: A total of 168 elderly acute cerebral infarction patients who accepted therapy in our hospital from February 2022 to February 2023 were chosen and allocated into a combination group and a control group. Interventions: The control group accepted the edaravone injection treatment. The combination group was treated with edaravone injection combined with butylphthalide injection. Primary outcome measures: (1) Therapeutic effect (2) Cytokines (3) Vascular endothelial function (4) Oxidative stress (5) Degree of neurological impairment (6) Living ability. Results: The total effective rate in the combination group presented elevation when in contrast to the control group (P < .05). After therapy, the levels of cytokines in the combination group presented reduction relative to the control group (P < .05), nitric oxide level in the combination group presented elevation when in contrast to the control group (P < .05), vascular endothelial growth factor level in the combination group presented lessened relative to the control group (P < .05), glutathione peroxidase along with superoxide dismutase levels in the combination group presented higher relative to the control group (P < .05) and malondialdehyde level in the combination group presented lower relative to the control group (P < .05). After therapy, the National Institute of Stroke Scale score in the combination group presented reduced when compared with the control group (P < .05). In contrast, the activity of daily living score in the combination group presented higher relative to the control group (P < .05). Conclusion: Butylphthalide injection in combination with edaravone can effectively inhibit the inflammatory response and oxidative stress, promote vascular endothelial function, improve daily behavior ability as well as promote the neurological function of elderly acute cerebral infarction patients.

Abstract:A total of 64 patients with acute lacunar infarction were enrolled within 24 hours of onset. The patients received conventional therapy (antiplatelet drugs and hypolipidemic drugs) alone or conventional therapy plus 450 mg Xueshuantong once a day. The main ingredient of the Xueshuantong lyophilized powder used for injection was Panax notoginseng saponins. Assessments were made at admission and at discharge using the National Institutes of Health Stroke Scale, the Activity of Daily Living and the Mini-Mental State Examination. Additionally, the relative cerebral blood flow, relative cerebral blood volume and relative mean transit time in the region of interest were calculated within 24 hours after the onset of lacunar infarction, using dynamic susceptibility contrast magnetic resonance perfusion imaging technology. Patients underwent a follow-up MRI scan after 4 weeks of treatment. There was an improvement in the Activity of Daily Living scores and a greater reduction in the scores on the National Institutes of Health Stroke Scale in the treatment group than in the control group. However, the Mini-Mental State Examination scores showed no significant differences after 4 weeks of treatment. Compared with the control group, the relative cerebral blood flow at discharge had increased and showed a greater improvement in the treatment group. Furthermore, there was a reduction in the relative mean transit time at discharge and the value was lower in the treatment group than in the control group. The experimental findings indicate that Xueshuantong treatment improves neurological deficits in elderly patients with lacunar infarction, and the mechanism may be related to increased cerebral perfusion.

Abstract:Objective: To explore the clinical efficacy of butylphthalide in treating patients with stroke attributed to intracranial artery stenosis (ICAS). Methods: In this retrospective study, records of 163 patients with stroke attributed to ICAS admitted to Jiaxing Second Hospital from January 2021 to January 2023 were retrospectively analyzed. Patients were divided into two groups based on the treatment received: control group (patients received routine treatment, n=55) and observation group (patients treated with butylphthalide on a routine basis, n=58). Changes in levels of cerebrovascular reactivity (CVR), breath-holding index (BHI), pulsatility index (PI), and middle cerebral artery mean flow velocity (Vm) between the two groups before and after treatment were compared. In addition, cognitive function, neurological function, and living ability were compared between the two groups before and after treatment, as well as the overall clinical efficacy of the treatment. Results: The baseline data was comparable between the two groups (P>0.05). After the treatment, CVR, BHI, and Vm indicators in the observation group were significantly higher than those in the control group, while the levels of PI indexes were significantly lower than those in the control group (P<0.05). Montreal Cognitive Assessment (MoCA) and Barthel scale scores of the observation group were significantly higher compared to the control group, while the scores of National Institutes of Health Stroke Scale (NIHSS) were significantly lower (P<0.05). Abstract:Objective To investigate the effect of butylphthalide and sodium chloride injection on patients who received endovascular treatment for acute anterior circulation large vessel occlusive stroke.Methods A total of 173 patients were identified from February 2015 to December 2017 in the Department of Neurology of Jingling Hospital in this retrospective observational study.Propensity score-matching analysis was performed to balance differences in baseline characteristics between patients who received butylphthalide injection (butylphthalide group) and those who did not (control group).The modified Rankin Scale scores at 90 days were compared between the butylphthalide and control groups.Results A total of 144 patients who received endovascular treatment for acute anterior circulation large vessel occlusive stroke were finally analyzed,54 cases in the butylphthalide group and 90 cases in the control group.The proportion of good functional outcome at 90 days in the butylphthalide group was higher than that in the control group (63.0％ (34/54) vs 44.4％ (40/90);x2=4.633,P=0.031).Thirty-six pairs were matched successfully by the propensity score matching,36 patients in the butylphthalide group and 36 in the control group.There was no statistically significant difference in the 90-day functional outcome between the two groups (66.7％ (24/36) vs 44.4％ (16/36);x2=3.600,P=0.058).One hundred and fifteen patients were recanalized,47 cases in the butylphthalide group and 68 cases in the control group,and after the propensity score matching,30 pairs were analyzed.The proportion of good functional outcome at 90 days in the butylphthalide group was higher than that in the control group (73.3％ (22/30) vs 46.7％ (14/30);x2=4.444,P=0.035).Conclusion After propensity score-matching,butylphthalide and sodium chloride injection could improve 90-day functional outcome in patients with acute anterior circulation large vessel occlusive stroke and obtained recanalization by endovascular treatment while could not before propensity score-matching.

Abstract:Fifty‐one cases of non‐pulmonary surgical tuberculosis are reviewed, 29 of the cases being immigrants. Of the 44 ‘new’ cases, tuberculosis was not among the preoperative differential diagnoses in 15. Lymphnodes were the site of the disease in more than half the cases. In only 29 cases was an operative specimen sent for bacteriological culture, and the necessity for culturing operative specimens and the reasons for the failure of isolation of tubercle bacilli in 50 per cent of the cultures are discussed. The impression that tuberculosis is not sufficiently considered by doctors in the preoperative differential diagnosis of several surgical presentations, especially in English patients, is confirmed.
Synergistic effects and molecular mechanisms of DL-3-n-butylphthalide combined with dual antiplatelet therapy in acute ischemic stroke

Abstract:DL-3-n-butylphthalide (NBP) is isolated from the seeds of Apium graveolens L., and has been recently used as a neuroprotective agent for acute ischemic stroke. The present study aimed to determine the efficacy and safety of the combined use of dual antiplatelet therapy (DAPT) and NBP for treating of acute ischemic stroke in rats and to explore the synergistic mechanism of this treatment strategy in rat middle cerebral artery occlusion models. The efficacy of DAPT combined with NBP was evaluated by determining neurological deficits, infarction status, and histological changes. Changes in body weight, blood glucose level, blood count, and serum biochemical parameters were detected to evaluate the safety. To explore the synergistic pharmacological mechanism, the mRNA expression and protein levels of key proteins in the pyroptosis–inflammatory pathway, and the pyroptosis ratio of microglias were examined. Compared with the administration of NBP or DAPT alone, combination of them significantly improved neurological deficits, reduced infarct area, and repaired tissue injury and inflammation after cerebral ischemia. No hepatorenal toxicity was observed. The mRNA expression and protein levels of key proteins in the pyroptosis–inflammation pathway, and the pyroptosis ratio of microglias were significantly downregulated in the combined administration group than in the monotherapy group. We demonstrated that the combined use of NBP and DAPT exhibits better efficacy and high safety and plays a synergistic role by inhibiting the pyroptosis–inflammation pathway in the brain tissues, particularly in microglial cells.

Cerebral hemodynamic changes assessment by transcranial doppler ultrasound in patients with acute cerebral infarction before and after treatment with butylphthalide

Abstract:To assess cerebral hemodynamic changes by transcranial doppler ultrasound in patients with acute cerebral infarction before and after treatment with butylphthalide, A total of 90 patients with acute cerebral infarction admitted to our hospital from January 2019 to January 2020 were selected and equally divided into the control group and the experimental group according to the order of admission. The control group was treated with conventional treatment, while the experimental group was additionally given butylphthalide drug treatment. The experimental group obtained better hemodynamic indexes as compared with the control group (P<0.05). The experimental group yielded a notably higher total clinical effective rate after treatment in contrast with the control group (P<0.05). After treatment, the serum indexes of the experimental group were evidently lower than those of the control group (P<0.05). After treatment, a remarkably lower NIHSS score of the experimental group than the control group was observed (P<0.05). The BI index score of the experimental group after treatment was considerably higher than that of the control group (P<0.05). After treatment, the MMSE score in the experimental group was significantly higher than it was in the control group (P<0.05). The treatment of butylphthalide in patients with acute cerebral infarction can effectively improve the clinical symptoms of the patients and the cerebral hemodynamics of the patients tested by TCD found that this treatment yields an excellent therapeutic effect and is worthy of promotion and application.
Efficacy of Fuyuan Xingshen Decoction Combined with Butylphthalide Sodium Chloride Injection in the Treatment of Acute Cerebral Infarction and Its Effect on Hemodynamics

Abstract:Objective. This study aims to determine the curative effect of Fuyuan Xingshen Decoction combined with butylphthalide sodium chloride injection in acute cerebral infarction (ACI) treatment and its effect on hemodynamics. Methods. In our hospital, a total of 84 ACI patients from May 2020 to February 2022 were randomly divided into two groups: observation and control (n = 42 each). Fuyuan Xingshen Decoction in combination with butylphthalide sodium chloride injection was provided to the observation group, while the control group received butylphthalide sodium chloride injection alone. Both groups' clinical efficacy was observed. Before and after treatment, the neurological function of the two groups was evaluated using the National Institutes of Health Stroke Scale (NIHSS), while the daily living ability of both groups was determined using the Barthel index rating scale. Both groups were investigated for their cognitive function, before and after treatment, by using the Mini-Cog scale and Montreal Cognitive Assessment (MoCA) scale. The pulsatility index (PI), peak velocity (Vp), and mean velocity (Vm) of intracranial arteries were measured by transcranial Doppler ultrasonography before and after treatment. The levels of ubiquitin carboxy-terminal hydrolase-1 (UCH-L1), Fibulin-5, and visinin-like protein-1 (VILIP-1) in serum and the expression levels of miR-34c, miR-25, and miR-182 in peripheral blood of the two groups were compared. Both groups were observed for the incidence of adverse reactions. Results. Compared with the control (71.43%), the observation group exhibited a significantly higher effective rate of around 90.48%. In the observation group, the improvement in NIHSS, Barthel index, Mini-cog, and MoCA scores; hemodynamic indexes including Vp, PI, and Vm; serum UCH-L1, Fibulin-5, and VILIP-1 levels; and the miR-34c, miR-25, and miR-182 expression levels in peripheral blood was better than the control group, with significant difference (all P < 0.05 ). The incidence of adverse reactions between the groups demonstrated no significant differences ( P > 0.05 ). Conclusions. Fuyuan Xingshen Decoction combined with butylphthalide sodium chloride injection can effectively improve patients' daily living ability, neurological function, cognitive function, and cerebral hemodynamics in the treatment of ACI, with good effect and safety.

Abstract:Purpose: To investigate the clinical effects and safety of ginkgo biloba diterpene lactone glucosamine injection, combined with dual antibiotic therapy for acute ischemic stroke in elderly individuals within a hyper thrombolytic time window. Methods: 100 elderly patients with acute ischemic stroke at Changsha First Hospital, Changsha, China from March 2022 to March 2023 were randomly and equally assigned to study and control groups (50 patients each). Study group received aspirin and clopidogrel, while control group received ginkgo biloba diterpene lactone glucosamine injection along with dual antibodies. Clinical efficacy, National Institutes of Health Stroke Scale (NIHSS) score, Instrumental Activities of Daily Living (IADL) score, cerebral hemodynamics, and safety were compared between the two groups. Results: There was no significant difference in effective rate between the two groups (p > 0.05). However, following treatment, study group showed superior NIHSS and IADL scores compared to control group, with greater statistical significance as the disease progressed. Doppler ultrasound revealed that study group had smaller Pulsatility index (PI) values and larger mean cerebral artery blood flow velocity (Vm), and systolic value (Vs) compared to control group (p < 0.05). In study group, three adverse reactions were reported (gastrointestinal discomfort, abnormal liver/kidney function, and skin itching), whereas the control group experienced five adverse reactions (gastrointestinal discomfort, abnormal liver/kidney function, dizziness, and skin itching). Conclusion: The combination of Ginkgo biloba diterpene lactone glucosamine injection and dual antibodies in elderly individuals with acute ischemic stroke within a super thrombolytic time window enhances intracerebral blood flow levels, effectively promotes neurological recovery, improves daily living abilities, and safety. Further research is needed to elucidate the exact pathways involved.

Abstract:Objective:To investigate the effects of butylphthalide injection on serum interleukin-6 (IL-6), interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) levels and the impact of prognosis in patients with acute cerebral infarction.Methods:80 patients with acute cerebral infarction who were hospitalized in Xuzhou Central Hospital from May 2016 to April 2018 were randomly divided into control group and treatment group, each with 40 cases. The control group was given conventional treatments such as blood pressure control, blood glucose control, lipid lowering, and anti-platelet aggregation. In addition of the conventional treatments, butylphthalide for injection was added for 14 days. The concentrations of IL-6, IL-1β, TNF-α and the National Institutes of Health Stroke Scate scores were measured before and 14 days after treatment. MOCA score were performed before and 3 months after treatment. The χ2 test was used to compare the differences in total effectiveness and gender, and the independent sample t test was used to compare the differences in serum IL-6, IL-1β, TNF-α concentration, NIHSS score, and MOCA score between the groups.Results:There were no significant differences in the concentrations of IL-6, IL-1β, TNF-α and NIHSS, MOCA scores between the two groups of patients before treatment (P＞0.05). After 14 days of treatment, the concentrations of IL-6, IL-1β, TNF-α in the treatment group decreased much more than that in the control group, and the difference was statistically significant. The NIHSS score of the treatment group was significantly lower than the control group [(6.29±1.72) pg/ml vs (7.78±1.97) pg/ml; (13.40±1.20) ng/L vs (18.12±1.30) ng/L; (1.28±0.35) pg/ml vs (1.43±0.32) pg/ml], and the difference was statistically significant. After 3 months, the MOCA score was higher than the control group [(26.38±1.60) vs (25.03±2.36)], and the difference was statistically significant.Conclusion:Compared with the control group, the butylphthalide injection group reduced IL-6, IL-1β, TNF-α secretion in patients with acute cerebral infarction, thereby probably improving the prognosis of patients with acute cerebral infarction.

Abstract:To investigate the clinical assessment of dual-enhanced antiplatelet therapy after cerebrovascular intervention to reduce the risk of cerebral infarction recurrence, and to provide a reference for the prevention and treatment of cerebral infarction recurrence risk. 202 patients with cerebral infarction who underwent cerebrovascular intervention in Tianjin Fifth Central Hospital from January 2018 to October 2022 were selected as study subjects. The patients were divided into a treatment group (n=104) based on randomized controlled single-blind method with 61 males and 43 females with a mean age of (62.33±2.57) years old and a control group (n=98) with 56 males and 42 females with a mean age of (62.49±2.36) years old. The control group was given aspirin mono-antiplatelet therapy, and the treatment group was given clopidogrel doublet augmented antiplatelet therapy on the basis of the control group, and both groups continued the treatment for 2 months. Platelet counts, coagulation indexes and inflammatory factors were compared between the two groups before and after treatment, and the America National Institutes of Health Stroke Scale (NIHSS) score was used to assess the neurological functions of the two groups before and after treatment, and the recurrence of cerebral infarction in the two groups was counted within 6 months after treatment. In addition, the patients in the treatment group were divided into the cerebral infarction recurrence group and the cerebral infarction non-recurrence group according to whether they had cerebral infarction recurrence within 6 months after treatment, and the clinical data of the patients in the treatment group were collected to analyze the influencing factors of the dual-enhancement antiplatelet therapy for the recurrence of cerebral infarction in patients with cerebral infarction after cerebral vascular intervention by multifactorial logistic regression. The results showed that after treatment, patients in the treatment group had an international normalized ratio (INR) of (1.76±0.38), a platelet activation rate of (39.52±4.79)%, a platelet aggregation rate of (48.54±5.21)%, a tumor necrosis factor-alpha (TNF-alpha) of (28.37±4.47)ng/L, an interleukin 6 (IL-6) of (24.77±3.52)ng/L, a high-sensitivity C-reactive protein (hs-CRP) of (7.39±1.53)mg/L and an NIHSS score of (6.11±1.39) were lower than those of the control group (2.32±0.41), (44.81±6.37)%, (51.39±5.58)%, (39.66±4.51) ng/L, (29.25±4.04) ng/L, (9.03±1.78) mg/L and (9.93±1.46) points (all P<0.05). At 6-month follow-up of all patients, cerebral infarction recurred in 16 (15.38%) patients in the treatment group and in 33 (33.67%) patients in the control group (χ2=9.185, P<0.05). Kaplan-Meier results showed a statistically significant difference in the rate of recurrence without cerebral infarction in the treatment group compared with the control group(LogRank χ2=4.595,P<0.05). Logistic regression analysis showed that smoking history, cervical vascular plaque, post-treatment NIHSS score, post-treatment stenosis score, post-treatment INR, post-treatment hs-CRP and CYP2C19 gene polymorphism were independent influences on the recurrence of cerebral infarction in cerebral infarction patients with cerebral vascular interventions followed by doublet augmentation of antiplatelet therapy (all P<0.05). In conclusion, dual-enhanced antiplatelet therapy may be an effective measure to reduce the risk of cerebral infarction recurrence after cerebrovascular intervention in patients with cerebral infarction, but it is still influenced by more factors.

Abstract:For investigating an influence on butylphthalide sodium chloride injection combined with edaravone dexborneol on neurological function and serum inflammatory factor levels in sufferers having acute ischemic stroke, 120 sufferers having acute ischemic stroke from September 2020 to September 2021 are chosen for the study subjects. In line with the diverse therapies, they took part in a control group and the study group, with 60 examples in each group. The control group is treated with edaravone dexborneol, and the study group is treated with butylphthalide sodium chloride injection, based on the control group. The posttreatment curative efficacy on the two groups is recorded, and treatment of both the two groups is compared. Before and after neurological function indexes (NIHSS and mRS), inflammatory factor indexes (IL-6, CRP, and TNF-α), life quality index (Barthel index), hemorheological indexes (plasma-specific viscosity), and neurological levels of NSE are logged and contrasted between the two groups of adverse reactions during therapy. Postcure, the overall response rate and Barthel index of the study group obviously overtop those of the control group ( p < 0.05). IL-6, CRP, TNF-α, NSE, plasma specific viscosity, and NIHSS and mRS scores obviously hypodown those of the control group ( p < 0.05), and untoward effects on the two groups during curing are lower, and the discrepancy is not obvious( p > 0.05). Butylphthalide sodium chloride injection combined with edaravone dexborneol can enhance curative efficacy on sufferers having acute ischemic stroke, improve neurological function, blood rheology, and quality of life, and decrease the secretion of cytokine, having a better effect and high medication safety.

Clinical Study of Atorvastatin Combined with Clopidogrel in Patients with Acute Cerebral Infarction

Abstract:OBJECTIVE To investigate the therapeutic efficacy of atorvastatin combined with clopidogrel on acute cerebral infarction and the improvement on the prognosis of the disease.METHODS 120 cases with acute cerebral infarction were randomly divided into two groups:60 cases were treated with atorvastatin combined with clopidogrel;60 cases were given clopidogrel only.The scores of NIHSS,Barthel,mRS and MMSE were estimated before and 3 and 6 months after treatment.RESULTS In these two groups,all of the scores of NIHSS,Barthel and mRS were improved 3 and 6 months after the therapy(P0.05).The scores of NIHSS,Barthel and mRS were improved more in atorvastatin plus clopidogrel therapy group than those in clopidogrel therapy group.The significant statistical difference was found(P0.05).No alternation of MMSE estimation was found with no statistical difference(P0.05).CONCLUSION Atorvastatin plus clopidogrel therapy may be beneficial for restoring the nervous function and improving the ability of daily living and the prognosis.Atorvastatin plus clopidogrel can be used in acute cerebral infarction as a therapeutic approach.

Abstract:Objective To evaluate the long-term effect and safety of triple antiplatelet therapy(cilostazol,aspirin and clopidogrel)for the patients with diabetes and acute coronary syndromes(ACS)undergoing percutaneous coronary intervention(PCI).Methods Between December 2004 and February 2006,a randomized,single center trial was conducted in General Hospital of Shenyang Command for comparison of dual and triple antiplatelet therapy after PCI for the patients with ACS.Of 263 diabetic patients enrolled in present study,122 were randomly assigned to standard dual antiplatelet treatment with aspirin and clopidogrel,141 were assigned to triple antiplatelet therapy with aspirin,clopidogrel and cilostazol.Primary endpoint was a composite of cardiac death,non-fatal myocardial infarction(MI),stroke or target vessel revascularization(TVR)at 1 year.Results Baseline clinical and angiographic characteristics were comparable between the two groups.No definite stent thrombosis or MI occurred in either group during the period of follow-up,and no significant difference existed yet in the rates of 1-year cardiac death(4.1% vs 1.5%,P=0.255),stroke(3.3% vs 0.7%,P=0.186)and TVR(12.3% vs 7.8%,P=0.223)between the patients received dual or triple antiplatelet therapy.The rate of primary events was 9.9%(14/141)in triple group,which was significantly lower than that in dual group [18.9%(23/122),P=0.038].Patients receiving hypoglycemic medicine got more benefits from triple antiplatelet treatment.There was no significant difference between the two antiplatelet regimens regarding the risk of hemorrhagic events and premature discontinuation of aspirin or clopidogrel.Conclusion The regimen of cilostazol plus aspirin and clopidogrel for diabetic patients undergoing PCI is effective and safe in reducing long-term adverse cardiac and cerebral-vascular events,especially for the patients with high dangerous DM complicated with ACS.

Abstract:OBJECTIVE: This study aims to explore the curative effect of dl-3-n-butylphthalide (NBP) on patients with acute cerebral infarction (ACI) and its effects on serum lipoprotein-associated phospholipase A2 (Lp-PLA2) and hypersensitive C-reactive protein (hs-CRP) levels.METHODS: A total of 136 ACI patients treated in our hospital, who met the criteria, were selected and randomly divided into two groups: control group (n = 60, including 28 males and 32 females) and treatment group (n = 76, including 32 males and 44 females). Patients in the control group were treated with routine drug therapy, while patients in the treatment group were treated with NBP on this basis. A dose of 100 ml was administered by intravenous injection for 2 times/day, for 14 days. The curative effect was evaluated using the National Institute of Health Stroke Scale (NIHSS) and Barthel index (BI) self-care ability. The levels of the two factors in serum were measured using enzyme-linked immunosorbent assay, and the changes in levels of these two factors in serum at different time points before and after treatment were compared between the two groups.RESULTS: (a) Lp-PLA2 and hs-CRP levels in the treatment group after treatment were significantly lower than those before treatment and those in the control group after treatment (p < .05). (b) The NIHSS and BI scores in the treatment group were significantly lower after treatment than before treatment and those in the control group after treatment (p < .05).CONCLUSION: Dl-3-n-butylphthalide can improve the expression of Lp-PLA2 and hs-CRP in serum in ACI patients. Furthermore, NBP has significant efficacy in inhibiting inflammation and improving neurological symptoms.

Abstract:Background Patients with symptomatic intracranial atherosclerosis are at high risk of early recurrent stroke. Dual antiplatelet therapy appears promising for early secondary prevention of stroke or TIA, yet the ideal patients to benefit from this regime have not been identified. Methods We investigated the efficacy and safety of early dual antiplatelets use in TIA and minor stroke patients (NIHSS 0-3) by performing a post-hoc analysis of CLAIR (Clopidogrel plus aspirin versus aspirin alone for reducing embolisation in patients with acute symptomatic cerebral or carotid artery stenosis) study - a randomized, open-label, blinded-endpoint study in patients who had symptomatic large artery atherosclerosis, in whom micro-embolic signals (MES) were present on transcranial doppler monitoring. MES was used as surrogate marker for recurrent stroke risk. Analysis was by modified intention to treat. Results Sixty-five of 100 patients recruited had TIA or minor stroke (NIHSS 0-3); 30 received dual therapy (aspirin 75-60mg daily and clopidogrel 300mg day 1 then 75mg daily) and 35 received aspirin alone (75-160mg daily) for 7 days. 93% patients had intracranial stenosis. Mean onset-to-randomization was 2.3 days in dual therapy group and 3.2 days in monotherapy group (p=0.03). At day 7, 9 of 29 patients in dual therapy group and 18 of 34 patients in monotherapy group for whom data were available had ≥ 1 MES (relative risk reduction 41.4%, 95% CI 29.8-51.1, p<0.001). The median number of MES on day 7 was 0 in dual therapy group and 1.0 in monotherapy group (p=0.02). Two patients in monotherapy group had recurrent stroke. No patients had intracranial or severe systemic hemorrhage, except two patients in dual therapy group had minor bleeding. Conclusion Early dual therapy with clopidogrel and aspirin reduces MES at day 7 among patients with predominant intracranial atherosclerosis who had minor ischemic stroke or TIA, without causing significant bleeding complications.
