Abstract:In the last ten years, multimorbidity in children under the age of five years has become an emerging health issue in developing countries. The study of multimorbidity of anaemia, malaria, and malnutrition (MAMM) among children in Nigeria has not received significant attention. This study aims to investigate what risk factors are associated with the prevalence of multimorbidity among children aged 6 to 59 months in Nigeria. This study used two nationally representative cross-sectional surveys, the 2018 Nigeria Demographic and Health Survey and the 2018 National Human Development Report. A series of multilevel mixed-effect ordered logistic regression models were used to investigate the associations between child/parent/household variables (at level 1), community-related variables (at level 2) and area-related variables (at level 3), and the multimorbidity outcome (no disease, one disease only, two or more diseases). The results show that 48.3% (4917/10,184) of the sample of children aged 6–59 months display two or more of the disease outcomes. Being a female child, the maternal parent having completed higher education, the mother being anaemic, the household wealth quintile being in the richest category, the proportion of community wealth status being high, the region being in the south, and place of residence being rural were among the significant predictors of MAMM (p < 0.05). The prevalence of MAMM found in this study is unacceptably high. If suitable actions are not urgently taken, Nigeria's ability to actualise SDG-3 will be in grave danger. Therefore, suitable policies are necessary to pave the way for the creation/development of integrated care models to ameliorate this problem.

Abstract:Severe acute malnutrition (SAM) poses a significant threat to child health globally, particularly in low- and middle-income countries. Zambia, like many Sub-Saharan African nations, faces high rates of child malnutrition, with SAM contributing significantly to under-five mortality. Therefore, this study aimed to determine the prevalence and factors associated with SAM. This retrospective cross-sectional study was conducted at Livingstone University Teaching Hospital in Zambia (LUTH). SAM was defined according to the World Health Organization (WHO) criteria as either weight-for-height less than -3 standard deviations, mid-upper arm circumference (MUAC) less than 115 mm, or presence of bilateral pitting edema in children between 6 months and 5 years old who were attended to between 2020 and 2022. Data abstraction from pediatric patient records was conducted between August 2023 and January 2024. The records without the age and outcome variable were excluded. A total of 429 participants between 6 months and 5 years old were included, with demographic, clinical, and hematological parameters analyzed. Univariable and multivariable logistic regression were employed to investigate factors associated with SAM. Overall, 429 medical records were included in the study and the prevalence of SAM was 27.0% (n = 116). Age group 6–24 months (Adjusted Odds Ratio [AOR]: 11.60; 95% Confidence Interval [CI]: 3.34–40.89, p<0.001), living with HIV (AOR:3.90; 95% CI: 1.14–13.70, p = 0.034), Tuberculosis (TB) (AOR:22.30, 95% CI: 4.53, 110.3, p < 0.001), comorbidities (AOR: 2.50; 95% CI 1.13, 5.88, p = 0.024) and platelet count (AOR: 1.00; 95% CI 1.00, 1.00, p = 0.027) were positively associated with SAM. This study found a high prevalence of SAM, exceeding the WHO target of reducing SAM to 5% by 2025. SAM was associated with younger age (6–24 months), HIV infection, TB, comorbidities and platelet count. Therefore, there is need to enhance strategies aimed at reducing SAM among young children, children living with HIV, TB and comorbidities, particularly by intensive treatment, continuing and strengthening nutrition services.

Abstract:Background: Malnutrition is one of the most critical health challenges confronting public health agencies in developing nations. This study aimed to determine the scope and underlying factors contributing to malnutrition in West African countries, specifically Gabon, Gambia, Liberia, Mauritania, and Nigeria. Method: For this secondary data analysis, this study drew upon the demographic and health surveys (DHS) conducted within these West African nations. These surveys employed a complex sampling design involving a combination of stratification and cluster sampling in two stages, with varying probabilities of selection leading to weighted samples that effectively represented different components of the population. Given the intricacies of this sampling design, it is paramount to account for them when analyzing the survey data. To address this concern, this study applied a survey logistic regression model, which accommodates factors such as stratification, clustering, and sampling weights and departs from the assumption of independence inherent in the ordinary logistic regression model. Results: The outcomes of this model revealed several variables that emerged as statistically significant (p < 0.05) determinants of malnutrition. These influential factors encompass the region of the respondent, the current age of the mother, the highest level of education attained by the mother, the source of drinking water, the type of toilet facility, the household's wealth status, the age and gender of the child, and whether the child experienced a fever in the preceding two weeks. Conclusion: These findings demonstrate with poignant clarity the importance of primary health care interventions in the recognition and management of malnutrition. The countries of interest should invest in public health care interventions including community workshops and outreach programs. Workshops may occur at primary health care facilities during queue waits, or health workers may work with community leaders to perform workshops in areas of high foot traffic, such as places of worship, shopping hubs and collection points for financial aid or grants.
The Intersection of a Child's Demographics and Household Socioeconomic Status in the Multimorbidity of Malaria, Anaemia, and Malnutrition among Children Aged 6-59 Months in Nigeria

Abstract:Multimorbidity of malaria, anemia, and malnutrition (MAMM) is a condition in which an individual has two or more of these health conditions, and is becoming an emergent public health concern in sub-Saharan African countries. The independent associations of a child's demographic variables and household socioeconomic (HSE) disparities with a child's health outcomes have been established in the literature. However, the effects of the intersection of these factors on MAMM, while accounting for other covariates, have not been studied. Therefore, this study aimed to determine how children's sex, age, and household socioeconomic status interact to explain the variations in MAMM among children aged 6-59 months in Nigeria. Data from the 2018 Nigeria Demographic and Health Survey and the 2018 National Human Development Report (NHDR) were used. This study included weighted samples of 10,184 children aged 6-59 months in Nigeria. A three-level multilevel mixed effect ordinal logistic regression model was used, such that individual characteristics at level 1 were nested in communities at level 2 and nested in states at level 3. Subsequently, predictive probability charts and average adjusted probability tables were used to interpret the intersectional effects. Five models were created in this scenario. Model 1 is the interaction between the child's sex and household wealth status; model 2 is the interaction between the child's sex and age; model 3 is the interaction between the child's age and household wealth status; model 4 has the three two-way interactions of the child's sex, age, and household wealth status; and model 5 includes model 4 and the three-way interactions between a child's sex, age, and household wealth quintiles; while accounting for other covariates in each of the models. The prevalence of children with a 'none of the three diseases' outcome was 17.3% (1767/10,184), while 34.4% (3499/10,184) had 'only one of the diseases', and 48.3% (4918/10,184) had 'two or more' MAMMs. However, in the multivariate analyses, model 3 was the best fit compared with other models, so the two-way interaction effects of a child's age and household wealth status are significant predictors in the model. Children aged 36-47 months living in the poorest households had a probability of 0.11, 0.18, and 0.32 of existing with MAMM above the probability of children of the same age who live in the middle class, more prosperous, and richest households, respectively, while all other covariates were held constant. Thus, the variation in the prevalence of MAMM in children of different ages differs depending on the household wealth quintile. In other words, in older children, the variations in MAMM become more evident between the richer and the poorer household quintiles. Therefore, it is recommended that policies that are geared toward economic redistribution will help bridge the disparities observed in the prevalence of multiple diseases among children aged 6-59 months in Nigeria.

Abstract:8 ml h rate was begun for postoperative analgesia. The surgery was uneventful with the patient awake, using standard monitoring. The first dose of enoxaparin 4000 UI was given 10 h after perineural catheter placement. On the first postoperative day (POD), a Doppler study showed thrombosis of the right tibial posterior vein. Elastic-compressive stockings had been worn by the patient and enoxaparin 4000 UI was given twice a day. The perineural catheter was removed on the second POD, 12 h after the last enoxaparin dose, and the subsequent dose was administered 3 h after the catheter removal. On the fourth POD, the patient complained of left iliac fossa pain without sensory-motor deficit. The patient’s condition was stable and a computed tomography (CT) scan diagnosed a 15 12 cm haematoma of the left psoas muscle (Fig. 1). Laboratory exam: leucocyte count 14.3 10, erythrocyte 2.9 10, Hb 8.6 g dl, and Ht 26%. All the other biochemical measurements were within the normal range. The patient was continuously monitored, enoxaparin was discontinued, and antibiotic therapy was prescribed. By the fifth POD, the general condition was stable, the CT with contrast scan of abdomen and pelvis re-confirmed the haematoma, and a full blood count reported erythrocyte 2.15 10, Hb 6.4 g dl, and Ht 19%. Four units of concentrate red blood cells were transfused. Over the following days, the patient’s general condition was improving and psoas haematoma was progressively reducing. Enoxaparin 4000 UI once a day was recommenced on the 18th POD for treatment of the DVT. The patient was discharged on the 25th POD and a CT scan performed 2 weeks after hospital discharge showed almost complete resorption of the haematoma. Other authors reported renal subcapsular or psoas haematoma 2 after several attempts at placing a PCB block. Both Winnie and colleagues and Chayen and colleagues described needle insertion at fixed distance from iliac crest line and the vertebral column. Capdevila and colleagues proposed a point ‘1 cm cephalad from the point at the junction of the lateral third and medial two-thirds of a line between the spinous process of L4 and a line parallel to the spinal column passing through the posterior superior iliac spine’. Since these landmarks take in consideration the anatomical dimension of the individual patient, they may be more precise and thus avoid multiple attempts at placement, but this hypothesis has not been studied. Although CPCB offers a more complete block of the thigh and knee, no clinical evidence on better analgesia is reported in comparison with continuous femoral nerve block after major knee surgery. We suggest that it be a more reasonable and less risky analgesia technique than CPCB after major knee surgery.

Abstract:Abstract Background Despite consistent efforts to enhance child nutrition, poor nutritional status of children continues to be a major public health problem in Nepal. This study identified the predictors of severe acute malnutrition (SAM) among children aged 6 to 59 months in the two districts of Nepal. Methods We used data from a cross-sectional study conducted among 6 to 59 months children admitted to the Outpatient Therapeutic Care Centers (OTCC). The nutritional status of children was assessed using mid-upper arm circumference (MUAC) measurement. To determine which variables predict the occurrence of SAM, adjusted odds ratio was computed using multivariate logistic regression and p -value < 0.05 was considered as significant. Results Out of 398 children, 5.8% were severely malnourished and the higher percentage of female children were malnourished. Multivariate analysis showed that severe acute malnutrition was significantly associated with family size (five or more members) (Adjusted Odds Ratio [AOR]: 3.96; 95% Confidence Interval [CI]: 1.23–12.71). Children from severely food insecure households (AOR: 4.04; 95% CI: 1.88–10.53) were four times more likely to be severely malnourished. Higher odds of SAM were found among younger age-group (AOR: 12.10; 95% CI: 2.06–71.09) children (0–12 vs. 24–59 months). Conclusions The findings of this study indicated that household size, household food access, and the child’s age were the major predictors of severe acute malnutrition. Engaging poor families in kitchen gardening to ensure household food access and nutritious diet to the children, along with health education and promotion to the mothers of young children are therefore recommended to reduce child undernutrition.

Abstract:Background . The nutritional status is the best indicator of the well-being of the child. Inadequate feeding practices are the main factors that affect physical growth and mental development. The aim of this study was to develop a predictive score of severe acute malnutrition (SAM) in children under 5 years of age. Methods . It was a case-control study. The case group ( n = 263) consisted of children aged 6 to 59 months admitted to hospital for SAM that was defined by a z -score weight/height < −3 SD or presence of edema of malnutrition. We performed a univariate and multivariate analysis. Discrimination score was assessed using the ROC curve and the calibration of the score by Hosmer–Lemeshow test. Results . Low birth weight, history of recurrent or chronic diarrhea, daily meal’s number less than 3, age of breastfeeding’s cessation less than 6 months, age of introduction of complementary diets less than 6 months, maternal age below 25 years, parity less than 5, family history of malnutrition, and number of children under 5 over 2 were predictive factors of SAM. Presence of these nine criteria affects a certain number of points; a score 8 points presents a high risk of SAM. The area under ROC curve of this score was 0.9685, its sensitivity was 93.5%, and its specificity was 93.1%. Conclusion . We propose a simple and efficient prediction model for the risk of occurrence of SAM in children under 5 years of age in developing countries. This predictive model of SAM would be a useful and simple clinical tool to identify people at risk, limit high rates of malnutrition, and reduce disease and child mortality registered in developing countries.

Abstract:Introduction: Anaemia among children is an issue of significant concern within the realm of public health due to its potential to result in significant adverse outcomes, including cognitive deficits and growth deficiencies. Its prevalence exhibits regional variations, wherein larger frequencies are observed in regions characterized by lower levels of socioeconomic development. This study sought to identify the individual, household, and contextual factors associated with anaemia in Nigeria. Methods: Data on a weighted sample of 11,223 children under five years of age from 42,000 households was obtained from the Nigeria Demographic and Health Survey (NDHS) 2018. The study utilized the household members recode file, a data base which allows for the estimation of prevalence of childhood anaemia together with selected household variables and the kids recode file, a data base that allows for estimation of other household variables. Relevant variables were then generated, and the two data files were merged. Descriptive statistics were generated, and the multi-level logistic regression was computed to identify individual, household and contextual factors associated with anaemia. Results: The study found that the overall prevalence of anaemia was 67.9%. It was noted that being a: child of Muslim parents; big baby at birth; wasted, stunted; underweight, child resident in Southeast and Southwest Nigeria and a child resident in rural area were significantly associated with increased odds of anaemia . While having a mother with secondary education, being a child in the second, third, or fourth birth order belonging to richest households, living in households with tiled floors, living in households with cemented floors, living in households with carpeted floors and improved sanitation/water source and living in Northeast Nigeria were significantly associated with decreased odds of anaemia. About 8.5% of individual, household and contextual factors determine the average prevalence of anaemia in children in Nigeria. Conclusion: The findings from this study reveal that individual, household and contextual factors are important determinants of anaemia in children in Nigeria.

Abstract:Background: Household-level food safety practices may have a long-term outcome on the nutrition and health status of under-five children. This study explores the relationships between caregivers' self-reported food safety knowledge, behavior, perception of food safety control, and their young child's (< 5 years) nutrition status. Methods: In a cross-sectional study design, 664 caregivers from five Local Government Areas (LGAs) in Ibadan, Nigeria were surveyed using an interviewer-administered questionnaire on their food safety knowledge, behavior and perceived food safety control. Anthropometric measurements of the household's index child (aged 6-59 months) were also taken. Variables for food safety knowledge and behavior score were selected using principal component analyses. The nutritional status of the children (Z-scores for weight-for-height (WAZ), height-for-age (HAZ), and weight-for-age (WHZ)) were calculated. Description statistics were run on all variables and logistic regression models examined associations between the three food safety constructs and the children's nutritional status. Covariates such as LGAs, caregiver's age, household size, wealth index, child's gender, and age were adjusted. Results: High-level of food safety knowledge, behavior and lots of perceived control on food safety issues were reported by 77.7%, 76.7%, and 81.1% of the caregivers. Using WAZ, HAZ and WHZ that are ≤ -2, acute malnutrition (6.0%), chronic malnutrition (42.5%), and underweight (22.0%), respectively were prevalent among the children. Perceived food safety control was the only food safety construct associated with acute malnutrition. Wealth index, caregivers' age, number of under 5 at home, child age, and gender were associated with acute malnutrition. Caregiver's age, child's age, and gender were associated with chronic malnutrition. Furthermore, caregiver's age, education and child's age were associated with a child being underweight. The significance level was at P < 0.05. Conclusion: Factors associated with food safety and children's nutritional outcomes are multi-faceted in nature. Future studies should explore access to resources and other intermediate factors that may explain the linkage between childhood malnutrition and caregivers' food safety awareness, knowledge, and behavior. Public health and food safety initiatives to bridge the gap between perceived food safety control and actual food safety behavior/practices.
Investigating a severe acute malnutrition outbreak in Dubti District, Awsiresu Zone, Afar Region, Northeast Ethiopia (2022)

Abstract:Introduction: Ethiopia is a global hotspot for child malnutrition, with an estimated 1.2 million children under five affected by severe acute malnutrition (SAM) in 2022. In response, the country has integrated SAM into its broader disease surveillance system. In January 2022, the Dubti District Health Office in the Afar Region detected an unusual surge in SAM cases through its surveillance system. This study aimed to assess the extent of the outbreak and identify the associated risk factors. Methods: We conducted an unmatched case–control study involving 258 mother–child dyads from five affected kebeles in the Dubti District of the Afar Region Ethiopia. The descriptive study included all 442 SAM cases from the line list, while 86 cases and 168 controls were selected using a simple random sampling method for the analytic study. The data were entered into EpiData software (version 3.1) and analyzed using SPSS software (version 25.0). Binary logistic regression (LR) analysis was performed to identify risk factors for SAM. Statistically, the results were summarized using an adjusted odds ratio (AOR), 95% confidence intervals (CIs), and a p -value of <0.05. Results: The median age of the cases was 22 months, with an interquartile range of 12–34 months. A total of 39 deaths were reported, with a case fatality rate (CFR) of 8.82%. The identified SAM risk factors included households with more than five members (AOR = 3.341, 95% CI: 1.475–7.563), more than five under-five children (AOR = 4.442, 95% CI: 2.000–9.866), lack of vaccination (AOR = 3.641, 95% CI: 1.618–8.198), pneumonia (AOR = 5.61, 95% CI: 2.488–12.651), diarrhea (AOR = 4.68, 95% CI: 2.169–10.097), lack of access to sanitation and hygiene (AOR = 3.18, 95% CI: 1.462–6.934), and household food insecurity (AOR = 9.46, 95% CI: 2.095–42.712). Conclusion: The study revealed a significant outbreak of SAM, with a CFR of 8.82%. The outbreak was associated with factors such as large family sizes, having multiple under-five children, a lack of vaccination, pneumonia, and diarrhea. These findings emphasize the urgent need to safeguard essential child health services, water supply, sanitation and hygiene, and household food security.

Abstract:Nutritional imbalance is an underlying cause of 2.6million death annually and a third of child's death globally. This study assessed and compared the nutritional status of primary school children and their caregiver's knowledge on malnutrition in rural and urban communities of Ekiti State. This is a cross-sectional comparative study carried out among 983 urban and rural primary school children in Ekiti State (495 in urban and 488 in rural) using interviewer-administered semi-structured questionnaire. A multi-stage sampling technique was used and data collected was analyzed using SPSS 23 with level of statistical significance set at p < 0.05. Underweight and stunting were relatively higher in rural (6.5% and 22.7% respectively) than in urban (6.3% and 19.4% respectively) and these differences are not statistically significant (p = 0.898, p = 0.197). However, wasting, overweight and obesity were higher in urban (12.7%, 6.1% and 7.7% respectively) than rural (11.5%, 3.7% and 7.5% respectively) but the difference is not statistically significant. (p = 0.242). Majority of the caregivers in both settings had good knowledge of malnutrition though higher in urban mothers (89.5%) with statistical significance than their rural counterparts (71.5%). However, there is no significant association between caregiver's knowledge and malnutrition in this study. Being in lower primary school class, relationship with caregiver, educational status of caregiver and occupation of caregiver were the common predictors of malnutrition among the school children in both community settings. Generally, the prevalence of malnutrition was high in both urban and rural primary school children in this study. However, while underweight and stunting were more prevalent among the children in the rural communities, wasting, overweight and obesity were more prevalent in the urban. The caregivers in both communities had good knowledge of malnutrition (better in the urban) but this is not good enough to bring a significant relationship with the occurrence of malnutrition in the children. Common predictors of malnutrition in both community settings are being in lower primary school class, relationship with caregiver, educational status of caregiver and occupation of caregiver. It is therefore recommended that regular continuous public enlightenment, nutritional education programmes and other programmes targeted at improving the economic power of the caregivers are measures that will improve the nutritional status of the primary school children.

Exploring disparities in malnutrition among under-five children in Nigeria and potential solutions: a scoping review

Abstract:Introduction: Triple burden of malnutrition in children remains a significant public health issue. This scoping review aims to assess the information on undernutrition, micronutrient deficiencies and the quality of complementary feeding in various regions in Nigeria. Methods: A literature search was conducted using PubMed and Google Scholar databases from January 1, 2018 to January 31, 2023 to include studies focusing on 0 to 5 years old children in Nigeria, reporting data on nutritional status, nutrient deficiencies, and published in English. Results: 73 out of 1,545 articles were included. Stunting remained alarmingly high ranging from 7.2% (Osun, South West) to 61% (Kaduna, North Central), while wasting varied from 1% (Ibadan, South West) to 29% (FCT Abuja, Central) and underweight from 5.9% (Osun, South West) to 42.6% (Kano, North West) respectively. The overall prevalence of anemia and vitamin A deficiency ranged between 55.2 to 75.1 % and 5.3 to 67.6%, respectively. Low rates of achieving minimum dietary diversity and minimum meal frequency were reported across different states depicting the suboptimal quality of complementary feeding. The prevalence of overweight/obesity ranged from 1.5% (Rivers, South South) to 25.9% (Benue, North Central). Conclusion: Multiple early childhood malnutrition issues exist with a wide disparity across states in Nigeria, particularly in the Northern region. Targeted nutrition interventions must be implemented to improve the situation.
Nutritional status and morbidity patterns of the elderly in a Northwestern Nigerian hospital: A cross-sectional study

Abstract:Context: Nutrition is a significant factor in determining the health of older people because it affects almost all organs and systems, which could lead to varieties of diseases and premature death. Aim: To determine the nutritional status and its association with the morbidity patterns of elderly patients. Settings and design: A cross-sectional hospital-based descriptive study involving 348 patients aged 60 years and above who presented at the Family Medicine Clinic. Subjects and methods: Data of the socio-demographic profile, anthropometric measurements and clinical diagnosis were collected. The co-morbidities were classified based on the number, duration and affected organ or system. The nutritional status was assessed with the Mini-Nutritional Assessment tool. Statistical analysis: Chi-square test and logistic regression analysis were used to determine associations between nutritional status and morbidity patterns of the elderly. The level of significance was set at a P ≤ 0.05. Results: A total of 348 respondents were recruited with 60.9% of females and mean age of 67.83 (standard deviation ± 7.53) years. The prevalence of malnutrition was 25.3% and of risk of malnutrition 56.6%. Furthermore, the prevalence of multi-morbidity was 74.4%. Advanced age (odd ratio = 8.911, confidence interval [CI] = 1.992-39.872, P = 0.004), underweight (OR = 1.167, CI = 0.291-37.846, P < 0.001), lack of formal education, (OR = 1.569, CI = 0.357-0.908, P = 0.018), low monthly income (OR = 1.975, CI = 1.376-2.836, P < 0.001), chronic respiratory diseases (OR = 4.250, CI = 4.025-4.492, P < 0.001) and physical inactivity (OR = 2.466, CI = 1.063-5.722, P = 0.036) were the predictors of malnutrition. Furthermore, the duration of chronic disease for more than 10 years (OR = 1.632, CI = 0.408-0.979, P = 0.040) was significantly associated with at-risk of malnutrition. Conclusion: The study revealed advanced age, underweight, low educational status, chronic respiratory diseases and physical inactivity as independent risk factors for malnutrition among the elderly.
Predictors of mortality among severe acute malnourished children. A multi-center prospective follow-up study

Abstract:Background & aims: Severe acute malnutrition (SAM) is a public health problem in developing countries including Ethiopia, because of the high risk of death, most severely malnourished children are managed in hospitals. Hence, the aim of this study was to assess the predictors of mortality among severely malnourished children admitted in Gedeo zone hospitals, Southern Ethiopia. Methods: Multicenter institution-based prospective follow-up study was conducted among 568 severely malnourished children with age <5 years in Gedeo zone hospitals from December 2018 to April 2020. Survival analysis with Cox proportional hazard model was conducted to determine factors associated with mortality rate. Variables with a p-value <0.05 in multivariate regression were considered statistically significant. Results: From a total of 568 children admitted with SAM; 54(9.5%), 306(53.9%), 179(31.5%) and 29(5.1%) died, recovered, transferred out and defaulted respectively. Over the study period, the rate of mortality was eight per 1000 person-days. Comorbidity after admission (Adjusted hazard ratio (AHR) = 2.96; 95% CI 1.35, 6.47), being HIV reactive (AHR = 1.31; 95% CI 1.12, 1.72), hospital stay for more than one week (AHR = 1.78; 95% CI 1.03, 3.12), no formal education of the mother (AHR 2.25; 95% CI 1.24, 4.08) and Nasogastric (NG) tube (AHR = 1.87; 95% CI: 1.562-2.37) given were the significant predictors of mortality. Conclusions: Maternal/caregiver educational status, co-morbidity after admission, being HIV reactive, vaccination status, hospital stay for more than one week and NG tube given were found as significant determinant factors of mortality rate. Hence, the government of Ethiopia and stakeholders should implement strong interventions focusing on these predictors.

Abstract:Abstract Objective: The current study aims to determine household-, maternal- and child-related factors influencing nutritional status among children under five in Mali. Design: Quantitative cross-sectional study using secondary data extracted from Mali DHS-VI 2018. Setting: Urban and rural areas of Mali. Participants: A total of 8908 children participated, with 3999 in the younger age group (0–24 months) and 4909 in the older age group (25–59 months). Results: In the younger age group, the prevalence of stunting, wasting and underweight was 18·8 % (95 % CI%: 17·5, 20·0), 24·6 % (95 % CI: 23·2, 26·0) and 13·2 % (95 % CI: 12·1, 14·3), respectively, while in the older age group, it was 24·9 % (95 % CI: 23·7, 26·2), 22·7 % (95 % CI: 21·5, 24·0) and 5·7 % (95 % CI: 5·0, 6·5), respectively. Being average or large size at birth, having piped source of water, receiving Zn, deworming, high maternal BMI, receiving Fe during pregnancy, higher maternal education and being rich were associated with lower odds of one or more form of undernutrition in both groups. On the other hand, children who were anaemic, drank from a bottle, maternal anaemia, current pregnancy of mothers and living in rural areas were associated with higher odds of stunting, wasting or underweight. Interestingly, children who received Fe supplementation had a higher odds of wasting in the younger group but lower odds of all forms of undernutrition in the older group. Conclusions: This study emphasised the potential risk factors associated with undernutrition in children. Children who consume non-potable water, have mothers with lower levels of education and BMI and reside in rural areas are more likely to experience undernutrition.

Abstract:Background: Malnutrition is a key global health challenge and a major contributor to childhood morbidity and mortality. In recent times, the contrasting coexistence of undernutrition including micronutrient deficiencies and overweight/obesity called double burden of malnutrition has been noted at individual, household or population level and/or at different times in life. The objective of this study was to examine individual, neighborhood and country level factors that are associated with the double burden of childhood malnutrition. Methods: We conducted multivariable multilevel logistic regression analyses on the most recent demographic and health datasets from surveys conducted between 2015 and 2020 in low- and middle-income countries. We analyzed data of 138,782 children (level 1) living in 13,788 communities (level 2) from 27 countries (level 3). Results: The results of our analysis show variation in childhood malnutrition across the 27 countries from as low as 6.5% in Burundi to as high as 29.5% in Timor Leste. After adjusting for all level factors, we found that those who were wasted/overweight tended to have had an episode of diarrhea or fever in the last two weeks preceding the survey, were part of a multiple birth, were being breastfed at the time of the survey and born to mothers with more than one under 5-child resident in neighborhoods with high illiteracy and unemployment rates. The intra-neighbourhood and intra-country correlation coefficients were estimated using the intercept component variance; 44.3% and 21.0% of variance in odds of double burden of childhood malnutrition are consequent upon neighborhood and country level factors respectively. Conclusions: Evidence of geographical clustering in childhood malnutrition at community and country levels was found in our study with variability due to neighborhood level factors twice that of country level factors. Therefore, strategies in tackling the double burden of malnutrition must consider these shared drivers, contextual barriers and geographical clustering effects.

Abstract:Objective: We investigated the missed treatment opportunities affecting programs using Mid-Upper Arm Circumference (MUAC) as the sole anthropometric criterion for identification and monitoring of children suffering from Severe Acute Malnutrition (SAM). Design: Alongside MUAC, we assessed Weight-for-Height Z-score (WHZ) in children screened and treated according to the national MUAC only protocol in Pakistan. Besides, we collected parents' perceptions regarding the treatment received by their children through qualitative interviews. Setting: Data were collected from October to December 2021 in Tando Allah Yar District, Sindh. Subjects: All children screened in the health facilities (n=8818) and all those discharged as recovered (n=686), throughout the district, contributed to the study. All children screened in the community in the catchment areas of five selected health facilities also contributed (n=8459). Parents of 41 children randomly selected from these same facilities participated in the interviews. Results: Overall, 80.3% of the SAM cases identified during community screening and 64.1% of those identified in the health facilities presented a "WHZ-only" diagnosis. These figures reached 93.9% and 84.5% respectively in children aged over 24 months. Among children treated for SAM and discharged as recovered, 25.3% were still severely wasted according to WHZ. While parents positively appraised the treatment received by their children, they also recommended to extend eligibility to other malnourished children in their neighborhood. Conclusion: In this context, using MUAC as the sole anthropometric criterion for treatment decisions (referral, admission and discharge) resulted in a large number of missed opportunities for children in need of timely and adequate care.

Abstract:Background: In 2006, the Médecins sans Frontières nutritional program in the region of Maradi (Niger) included 68,001 children 6-59 months of age with either moderate or severe malnutrition, according to the NCHS reference (weight-for-height<80% of the NCHS median, and/or mid-upper arm circumference<110 mm for children taller than 65 cm and/or presence of bipedal edema). Our objective was to identify baseline risk factors for death among children diagnosed with severe malnutrition using the newly introduced WHO growth standards. As the release of WHO growth standards changed the definition of severe malnutrition, which now includes many children formerly identified as moderately malnourished with the NCHS reference, studying this new category of children is crucial. Methodology: Program monitoring data were collected from the medical records of all children admitted in the program. Data included age, sex, height, weight, MUAC, clinical signs on admission including edema, and type of discharge (recovery, death, and default/loss to follow up). Additional data included results of a malaria rapid diagnostic test due to Plasmodium falciparum (Paracheck) and whether the child was a resident of the region of Maradi or came from bordering Nigeria to seek treatment. Multivariate logistic regression was performed on a subset of 27,687 children meeting the new WHO growth standards criteria for severe malnutrition (weight-for-height<-3 Z score, mid-upper arm circumference<110 mm for children taller than 65 cm or presence of bipedal edema). We explored two different models: one with only basic anthropometric data and a second model that included perfunctory clinical signs. Principal findings: In the first model including only weight, height, sex and presence of edema, the risk factors retained were the weight/height(1.84) ratio (OR: 5,774; 95% CI: [2,284; 14,594]) and presence of edema (7.51 [5.12; 11.0]). A second model, taking into account supplementary data from perfunctory clinical examination, identified other risk factors for death: apathy (9.71 [6.92; 13.6]), pallor (2.25 [1.25; 4.05]), anorexia (1.89 [1.35; 2.66]), fever>38.5 degrees C (1.83 [1.25; 2.69]), and age below 1 year (1.42 [1.01; 1.99]). Conclusions: Although clinicians will continue to perform screening using clinical signs and anthropometry, these risk indicators may provide additional criteria for the assessment of absolute and relative risk of death. Better appraisal of the child's risk of death may help orientate the child towards either hospitalization or ambulatory care. As the transition from the NCHS growth reference to the WHO standards will increase the number of children classified as severely malnourished, further studies should explore means to identify children at highest risk of death within this group using simple and standardized indicators.
