Abstract:Background The novel coronavirus disease (COVID-19) has been declared a global pandemic, with the cumulative number of confirmed cases and deaths exceeding 150 million and 3 million, respectively. Here, we examined the dynamic changes in the immune and clinical features of patients with COVID-19. Methods Clinical data of 98 patients with confirmed COVID-19 diagnosis were acquired from electronic medical records and curated. The data were analyzed based on the stage of the admission, deterioration, and convalescence, which included age, sex, severity, disease stages, biochemical indicators, immune cells, inflammatory cytokines, and immunoglobulins. Additionally, temporal changes in the immune response in patients undergoing continuous renal replacement therapy (CRRT) were also examined. Results Compared to mild stage patients, severe stage patients with COVID-19 exhibited a significant reduction in lymphocyte [23.10 (17.58–33.55) vs. 4.80 (2.95–6.50), P<0.001], monocyte [8.65 (7.28–10.00) vs. 3.45 (2.53–4.58), P<0.001], and NK cell levels [244.00 (150.50–335.00) vs. 59.00 (40.00–101.00), P<0.001] but showed elevated levels of neutrophils [64.90 (56.30–73.70) vs. 90.95 (87.60–93.68), P<0.001], inflammatory cytokines [Interleukin-10, 3.05 (1.37–3.86) vs. 5.94 (3.84–8.35), P=0.001; and tumor necrosis factor-α, 11.50 (6.55–26.45) vs. 12.96 (12.22–36.80), P=0.029], which improved during convalescence. Besides, the number of immune cells—T lymphocytes, B lymphocytes, helper T cells, suppressor T cells, NK cells, and monocytes, except neutrophils—slowly increased in critically ill patients receiving CRRT from 0 to 3 weeks. Conclusions Our results indicate that the surveillance of immune cells may contribute to monitoring COVID-19 disease progression, and CRRT is a potential therapeutic strategy to regulate the immune balance in critically ill patients.

Abstract:Abstract Objectives The clinical implications of different blood indices in patients with coronavirus disease-2019 (COVID-19) were analyzed at different stages. Methods We compared blood test results of 17 COVID-19 patients treated in Jinhua Central Hospital between January 1 and March 5, 2020 at different stages. We also compared the initial blood results of 17 COVID-19 patients with 115 influenza virus A/B (Flu A/B)-positive patients, 19 Mycoplasma pneumonia (MP)-positive patients and 50 healthy subjects (HSs). Results (1) The white blood cell count (WBC) and absolute neutrophil count (NEU#) were lower in the SARS-CoV-2 group than in the MP and Flu A/B groups; the eosinophil percentage (EO%) and absolute eosinophil count (EO#) were lower in the severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) group than in the MP group (p<0.05). (2) Aspartate aminotransferase (AST) levels were significantly lower when patients were discharged from the hospital (p<0.05), EO% and EO# recovered at discharge, and returned to normal levels during follow-up (p<0.05). (3) When the throat swab was nucleic acid-negative but the stool was still positive, lymphocyte percentage (LY%) and absolute lymphocyte count (LY#) decreased (p<0.05). (4) As the cycle threshold (Ct) value of the nucleic acid increased or decreased, EO# showed a consistent trend. Conclusions Blood cell count indices upon hospital admission could be helpful to give some tips of diagnosis of SARS-CoV-2-infection, Flu A/B-infection and MP-infection; AST and EO# could be used to predict the outcome of patients. Feces turned negative for nucleic acid more slowly than throat swabs; LY# was lower during the fecal-positive period and low Ct values of fecal nucleic acid were negatively associated with the patient’s recovery level.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Introduction</h3><p>Coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has been widely spread. We aim to investigate the dynamic changes of peripheral blood lymphocytes subsets in adult patients with COVID-19.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Electronic medical records including demographics, clinical manifestation, comorbidities, laboratory data, and radiological materials of 435 hospitalized COVID-19 patients, with confirmed result of SARS-CoV-2 viral infection, were extracted and retrospectively analyzed. The counts of lymphocyte subsets at each week after the onset of the illness were compared with other weeks of illness and with those from normal individuals.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The various lymphocyte subsets (CD3 +, CD4 +, CD8 +, CD19 +and CD16/56 +) were below the normal ranges at the first week after the onset of the illness, reaching a nadir during the second week, which increased gradually in the third week and returned to normal levels in the fifth week, but was still lower than the healthy controls. The CD3+, CD4+ and CD8+ counts were significantly lower in those with severe disease compared to those with non-severe disease, and in those patients that died compared to those who recovered.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Discussion</h3><p>our research indicates that the levels of peripheral blood lymphocyte subsets (CD3 +, CD4 +, and CD8 +) are associated with disease progression, severity, and prognosis in patients with COVID-19. Dynamic monitoring of human immune function is one of the indicators for evaluating the severity and prognosis of COVID-19 patients, and it is helpful to formulate appropriate treatment strategies.</p>

Abstract:Objective This study aimed to compare sexual function before and 1 year after laparoscopic sacrocolpopexy using a porcine dermis or a polypropylene mesh material. Methods This was a secondary analysis of sexual function measured before and 1 year after laparoscopic sacrocolpopexy in a group of 81 sexually active women participating in a randomized controlled trial comparing porcine dermis and polypropylene mesh. Sexual function was assessed using the short form of the Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire (PISQ-12). Responses to individual questions from the physical domain of the PISQ-12 were also analyzed. Additional information included the type of mesh material used and whether a concomitant suburethral sling or perineorrhaphy was performed. Results There was a significant postoperative improvement in total PISQ-12 scores for the entire cohort (33.2 vs 38.3, P < 0.01). Similarly, PISQ-12 scores were significantly improved in both groups (33.2 preoperative vs 37.4 one year postoperative in the porcine dermis, P < 0.01 and 33.2 vs 39.2 in the polypropylene mesh, P < 0.01). There were no differences between the 2 graft material groups. Preoperatively, 63.0% (48/76) of women reported avoiding sexual intercourse because of bulging in vagina (PISQ12-question #8), at 1 year postoperatively only 4% (3/76) had a positive response (P < 0.01). We observed a significant decrease in the number of women who reported pain during intercourse at 12 months as evidenced by the responses to the PISQ12-question #5, 47.4% (36/76) versus 26.3% (20/76) (P < 0.01). The addition of a suburethral sling or a perineorrhaphy did not negatively impact sexual function at 1 year. Conclusions Laparoscopic sacrocolpopexy had a positive impact on sexual function at 1 year regardless of whether a porcine dermis or a polypropylene mesh material was used.
Immune Characteristics of Patients with Coronavirus Disease 2019 (COVID-19)

Abstract:Up to now, little is known about the detailed immune profiles of COVID-19 patients from admission to discharge. In this study we retrospectively reviewed the clinical and laboratory characteristics of 18 COVID-19 patients from January 30, 2020 to February 21, 2020. These patients were divided into two groups; group 1 had a severe acute respiratory syndrome coronavirus 2 nucleic acid-positive duration for more than 15 days (n = 6) and group 2 had a nucleic acid-positive duration for less than 15 days (n = 12). Group 1 patients had lower level of peripheral blood lymphocytes (0.40 vs. 0.78 ×109/L, p = 0.024) and serum potassium (3.36 vs. 3.79 mmol/L, p = 0.043) on admission but longer hospitalization days (23.17 vs. 15.75 days, p < 0.001) compared to Group 2 patients. Moreover, baseline level of lymphocytes (r = -0.62, p = 0.006) was negatively correlated with the nucleic acid-positive duration. Additionally, lymphocytes (420.83 vs. 1100.56 /μL), T cells (232.50 vs. 706.78 /μL), CD4+ T cells (114.67 vs. 410.44 /μL), and CD8+ T cells (94.83 vs. 257.44 /μL) in the peripheral blood analyzed by flow cytometry were significantly different between Group 1and Group 2. Furthermore, there was also a negative correlation between lymphocytes (r = -0.54, p = 0.038) or T cells (r = -0.55, p = 0.034) at diagnosis and the nucleic acid-positive duration, separately. In conclusion, the patients with nucleic acid-positive ≥ 15 days had significantly decreased lymphocytes, T cell and its subsets compared to those who remained positive for less than 15 days.

Abstract:BACKGROUND: Although there are many COVID-19 case series studies, few studies report the relationship between variations in blood cell parameters and inflammatory factors and disease severity. This study aims to describe the dynamic trends in COVID-19 blood cell parameters and inflammatory factors.METHODS: Ninety-two patients with confirmed COVID-19 at Jingzhou Central Hospital, Hubei Province, China, between January 23, 2020, and April 10, 2020, were enrolled. Epidemiological data, clinical information, and laboratory test results were collected and analyzed.RESULTS: As patient age increased, disease severity increased (P<0.0001). The inflammatory factor C-reactive protein (CRP) showed a gradual increasing trend with disease aggravation. Based on dynamic change graphs, CRP in all patients with severe and critical COVID-19 initially increased and then decreased; however, CRP in patients with a good prognosis did not increase again after the initial decline (<20 mg/L). CRP in patients with a poor prognosis returned to a high level (>50 mg/L) 1 week after the initial decrease and continued to fluctuate at a high level. Lymphocyte count (LYM#) in patients with severe and critical disease was significantly lower (<1×10<sup>9</sup>/L) than that in patients with moderate disease; LYM# was significantly increased 3 weeks after disease onset in patients with a good prognosis (>1×10<sup>9</sup>/L), but patients with a poor prognosis continued to have a low LYM#.CONCLUSIONS: CRP and LYM# showed strong correlation with disease progression, suggesting that these parameters could be used to monitor changes in patient condition.

Hematologic changes predict clinical outcome in recovered patients with COVID-19

Abstract:2019 coronavirus disease (COVID-19) presents as a newly recognized pneumonia that has brought about a global pandemic and is increasingly considered as a systemic illness. We investigated the clinical and laboratory features of recovered COVID-19 patients without pre-existing hematologic diseases at Wuhan No. 1 Hospital. Fifty-nine male and 68 female Chinese patients were included with the median age at 64 years in the present study. Eosinopenia (37.80%), monocytosis (51.97%), lymphocytopenia (25.20%), and anemia (51.97%) were the most common hematologic findings in our cohort, particularly in severe or critically ill COVID-19. The levels of changes in leukocytes, neutrophils, lymphocytes, monocytes, eosinophils, basophils, platelets, hemoglobin levels, mean corpuscular volume (MCV), and mean cell hemoglobin concentration (MCHC) are overall associated with lung involvement, oxygen demand, and disease activity. However, changes of eosinophils (end hospitalization-baseline) (coefficients = 10.32; 95% CI = 1.03–19.60, <i>P</i> = 0.03) and basophils (Max − Min) (coefficients = 71.43; 95% CI = 8.55–134.31, <i>P</i> = 0.03) were independent predictors of delayed recovery in the hospital by the multivariate analysis in this recovered population. A variety of hematologic changes are associated with the severity and clinical outcome of recovered COVID-19 patients, which warrants further exploration of their underlying mechanisms.

Dynamic Changes in Clinical and CT Characteristics of COVID-19 Cases with Different Exposure Histories: a Retrospective Study

Abstract:Background To assess the dynamic changes in clinical and CT characteristics of COVID-19 patients with different epidemiology histories. Methods Fifty-three discharged COVID-19 patients were enrolled at Beijing YouAn Hospital, Capital Medical University, between January 21 and March 10, 2020. Spearman correlation analysis was performed between CT scores and laboratory indicators. Patients were divided into the Wuhan group (lived in or with travel to Wuhan, numbering 30 cases) and non-Wuhan group (close contacts or unknown exposure, totaling 23 cases). The CT and laboratory findings were compared between and within groups during the clinical process. Results Fever (88.7%), cough (64.2%), fatigue (34%), and abnormal laboratory indicators, including lymphopenia, reduced albumin, albumin/globulin (A/G), and elevated C-reactive protein (CRP), were mainly observed. Subpleural ground-glass opacities (86.8%) were usually detected at admission. The CT scores were highly correlated with lymphocytes, CRP, albumin, and A/G at initial and follow-ups (all p < 0.05). Four days after admission, most patients (66.7% Wuhan, 47.8% non-Wuhan) showed progression, and the CT scores of Wuhan significantly increased ( p = 0.015). Eight days after admission, the vast majority of patients (69.2% Wuhan, 100% non-Wuhan , p = 0.006) presented improvement, and the CT scores of non-Wuhan were significantly lower than Wuhan ( p = 0.006). Pneumonia was completely absorbed in most patients 2–4 weeks after discharge. Conclusions CT plays a crucial role in the early diagnosis and monitoring of changes in COVID-19. Lymphocytes, CRP, albumin, and A/G are expected to predict disease severity and prognosis. Viral pathogenicity in non-endemic areas may be weaker than core-infected areas. In most patients, lung lesions can disappear around 4 weeks after discharge.
Effect of blood analysis and immune function on the prognosis of patients with COVID-19

Abstract:This retrospective study investigated the implications of changes in blood parameters and cellular immune function in patients with Coronavirus Disease 2019 (COVID-19).Records were reviewed of 85 patients admitted with COVID-19 between February 4 and 16, 2020. The primary outcome was in-hospital death.Fourteen patients died. The baseline leukocyte count, neutrophil count and hemoglobin was significantly higher in non-survivors compared with survivors, while the reverse was true of lymphocyte count, platelet, PaO<sub>2</sub>/FiO<sub>2</sub>, CD3+ count and CD4+ count. The percentage of neutrophil count > 6.3×10<sup>9</sup>/L in death group was significantly higher than that in survival group, and multivariate logistic regression showed neutrophil count > 6.3×10<sup>9</sup>/L was independently associated with mortality. However, there were not significant difference in IgG, IgM, IgA, C3, C4 and the percentage of IgE > 100 IU/ml between the death group and survival group. Areas under the receiver operating characteristic curves of the following at baseline could significantly predict mortality: leukocyte, neutrophil, lymphocyte, CD3+ and CD4+ counts.For hospitalized patients with COVID-19, lymphocyte, CD3+ and CD4+ counts that marked decrease suggest a poor outcome. Admission neutrophil count > 6.3 ×10<sup>9</sup>/L is independently associated with mortality. At admission, leukocyte, neutrophil, lymphocyte, CD3+ and CD4+ counts should receive added attention.

Abstract:SARS-CoV-2, the pathogen of COVID-19, is spreading around the world. Different individuals infected with COVID-19 have different manifestations. It is urgent to determine the risk factors of disease progress of COVID-19. 364 patients diagnosed with COVID-19, who were admitted to Wuhan Pulmonary Hospital from February 3, 2020 to March 16, 2020, were divided into mild, ordinary, severe, and critical groups, according to Chinese novel coronavirus pneumonia diagnosis and treatment plan. Peripheral blood IL-6 and leukocyte characteristics were analyzed, to evaluate the correlation with the severity of COVID-19. The levels of peripheral blood IL-6 were 2.35 ± 0.46 pg/ml (mild), 6.48 ± 1.13 pg/ml (ordinary), 20.30 ± 5.15 pg/ml (severe), and 123.48 ± 44.31 pg/ml (critical). The leukocytes were 5.70 ± 0.41×10 9 /L (mild), 6.21 ± 0.14×10 9 /L (ordinary), 6.37 ± 0.26×10 9 /L (severe), and 10.03 ± 1.43×10 9 /L (critical). The lymphocytes were 1.46 ± 0.19×10 9 /L (mild), 1.89 ± 0.14×10 9 /L (ordinary), 1.26 ± 0.07×10 9 /L (severe), and 1.17 ± 0.23×10 9 /L (critical). The neutrophils were 3.63 ± 0.36×10 9 /L (mild), 3.78 ± 0.11×10 9 /L (ordinary), 4.47 ± 0.25×10 9 /L (severe), and 7.92 ± 1.19×10 9 /L (critical). The monocytes were 0.42 ± 0.05×10 9 /L (mild), 0.44 ± 0.01×10 9 /L (ordinary), 0.46 ± 0.02×10 9 /L (severe), and 0.78 ± 0.25×10 9 /L (critical). Conclusively, increase of peripheral blood IL-6 and decrease of lymphocytes can be used as the indicators of severe COVID-19. The increase of neutrophils and monocytes was noticed in critical cases of COVID-19, suggesting that the increase of neutrophils and monocytes should be considered as risk factors of critical cases of COVID-19. Peripheral blood IL-6 and leukocyte characteristics were also analyzed in different age groups. The increase of serum IL-6, decrease of lymphocytes, and increase of neutrophils were noticed in patients over 60 years old.

Abstract:Objectives The longitudinal and systematic evaluation of immunity in coronavirus disease 2019 (COVID-19) patients is rarely reported. Methods Parameters involved in innate, adaptive, and humoral immunity were continuously monitored in COVID-19 patients from onset of illness until 45 days after symptom onset. Results This study enrolled 27 mild, 47 severe, and 46 deceased COVID-19 patients. Generally, deceased patients demonstrated a gradual increase of neutrophils and IL-6 but a decrease of lymphocytes and platelets after the onset of illness. Specifically, sustained low numbers of CD8 + T cells, NK cells, and dendritic cells were noted in deceased patients, while these cells gradually restored in mild and severe patients. Furthermore, deceased patients displayed a rapid increase of HLA-DR expression on CD4 + T cells in the early phase, but with a low level of overall CD45RO and HLA-DR expressions on CD4 + and CD8 + T cells, respectively. Notably, in the early phase, deceased patients showed a lower level of plasma cells and antigen-specific IgG, but higher expansion of CD16 + CD14 + proinflammatory monocytes and HLA-DR − CD14 + monocytic-myeloid-derived suppressor cells (M-MDSCs) than mild or severe patients. Among these immunological parameters, M-MDSCs showed the best performance in predicting COVID-19 mortality, when using a cutoff value of ≥10%. Cluster analysis found a typical immunological pattern in deceased patients on day 9 after onset, which was characterized as the increase of inflammatory markers (M-MDSCs, neutrophils, CD16 + CD14 + monocytes, and IL-6) but a decrease of host immunity markers. Conclusions This study systemically characterizes the kinetics of immunity of COVID-19, highlighting the importance of immunity in patient prognosis.

Abstract:BACKGROUND: Millions of people have died of coronavirus disease 2019 (COVID-19) due to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, and retrospective studies of the disease in local regions are necessary.AIM: To characterize the epidemiological features and dynamic changes in blood biochemical indices for SARS-CoV-2-infected patients in Hebi, a representative city with a large floating population in North China.METHODS: From January 25 to February 10, 2020, the clinical data of patients who tested positive for SARS-CoV-2 by quantitative real-time polymerase chain reaction in Hebi city (China) were evaluated at admission, and laboratory data for hematologic parameters, inflammatory indices, coagulation function indices, liver function indices, blood lipid indices, renal function indices, myocardial enzyme activities and five blood biochemical markers of immunity were evaluated at admission, upon hospitalization and before discharge.RESULTS: Sixteen confirmed COVID-19 patients developed pneumonia but were cured after adequate treatment. Fever and fatigue were the common symptoms. The most common laboratory abnormalities of patients at admission were leukopenia, eosinopenia, decreased percentage of eosinophils, elevated high sensitivity C-reactive protein and fibrinogen levels, hypoalbuminemia, mildly increased aspartate transferase activity and levels of bilirubin, and increased levels of β2-microglobulin. Importantly, aggravated liver dysfunction was detected in most patients, which may be partially attributed to virus infection as well as medicinal treatment.CONCLUSION: This study provides several potential diagnostic markers and dynamic biochemical indices of disease progression to better prevent, diagnose and treat COVID-19 infection.

Abstract:Abstract Background Coronavirus disease 2019 (COVID-19) is caused by severe acute respiratory syndrome coronavirus 2, and outbreaks have occurred worldwide. Laboratory test results are an important basis for clinicians to determine patient condition and formulate treatment plans. Methods Fifty-two thousand six hundred forty-four laboratory test results with continuous values of adult inpatients who were diagnosed with COVID-19 and hospitalized in the Fifth Hospital in Wuhan between 16 January 2020 and 18 March 2020 were compiled. The first and last test results were compared between survivors and non-survivors with variance test or Welch test. Laboratory test variables with significant differences were then included in the temporal change analysis. Results Among 94 laboratory test variables in 82 survivors and 25 non-survivors with COVID-19, white blood cell count, neutrophil count/percentage, mean platelet volume, platelet distribution width, platelet-large cell percentage, hypersensitive C-reactive protein, procalcitonin, D-dimer, fibrin (ogen) degradation product, middle fluorescent reticulocyte percentage, immature reticulocyte fraction, lactate dehydrogenase were significantly increased ( P < 0.05), and lymphocyte count/percentage, monocyte percentage, eosinophil percentage, prothrombin activity, low fluorescent reticulocyte percentage, plasma carbon dioxide, total calcium, prealbumin, total protein, albumin, albumin-globulin ratio, cholinesterase, total cholesterol, nonhigh-density/low-density/small-dense-low-density lipoprotein cholesterol were significantly decreased in non-survivors compared with survivors ( P < 0.05), in both first and last tests. Prothrombin time, prothrombin international normalized ratio, nucleated red blood cell count/percentage, high fluorescent reticulocyte percentage, plasma uric acid, plasma urea nitrogen, cystatin C, sodium, phosphorus, magnesium, myoglobin, creatine kinase (isoenzymes), aspartate aminotransferase, alkaline phosphatase, glucose, triglyceride were significantly increased ( P < 0.05), and eosinophil count, basophil percentage, platelet count, thrombocytocrit, antithrombin III, red blood cell count, haemoglobin, haematocrit, total carbon dioxide, acidity-basicity, actual bicarbonate radical, base excess in the extracellular fluid compartment, estimated glomerular filtration rate, high-density lipoprotein cholesterol, apolipoprotein A1/ B were significantly decreased in non-survivors compared with survivors ( P < 0.05), only in the last tests. Temporal changes in 26 variables, such as lymphocyte count/percentage, neutrophil count/percentage, and platelet count, were obviously different between survivors and non-survivors. Conclusions By the comprehensive usage of the laboratory markers with different temporal changes, patients with a high risk of COVID-19-associated death or progression from mild to severe disease might be identified, allowing for timely targeted treatment.

Abstract:Background: Coronavirus disease 2019 (COVID-19) is affecting the whole world and threatening human health. We aim to investigate the immunological characteristics of monocytes in critical patients with COVID-19. Methods: The number and immune status of monocytes were detected by flow cytometry in 32 COVID19 patients and 18 healthy individuals. Results: In critical patients with COVID-19, the absolute number of total monocytes and CD16mono cyteswas significantly decreased but CD16' pro-inflammatory monocytes was increased compared to healthy controls. Antigen presentation potential of monocytes, as measured by HLA-DR expression, was suppressed, while their inflammatory phenotype (CD38 expression) was enhanced. Cytokine levels showed sustained increases in critical patients. And the levels of IL-6 were positively correlated with CD16' monocytes number. IL-6 and IL-10 levels were negatively correlated with HLA-DR expression of monocytes. During the recovery of COVID-19 patients, the count and immune status of monocyte subsets were restored by degrees. HLA-DR' monocytes possessed good sensitivity and specificity for predicting the incidence of critical patients with COVID-19. Conclusions: In critical patients with COVID-19, decline in number and HLA-DR expression of monocytes might lead to decreased antigen presentation potential and thus immunosuppression, while increased CD16' pro-inflammatory monocytes might mediate hyperinflammation. HLA-DR' monocytes might be a meaningful assisted indicator to predict the incidence of critical patients with COVID-19. Abstract:<span>How the routine laboratory tests change in terms of coronavirus disease 2019 (COVID-19) was retrospectively analyzed in a large group of patients. Biochemical, hematological and inflammatory variables of a totaly 555 (<i>n</i> = 532 in non-intensive care unit (non-ICU), <i>n</i> = 23 in ICU) patients diagnosed with COVID-19 were analyzed accessing them through the laboratory information system. White blood cell (WBC), neutrophil (NEU), platelet large cell ratio, neutrophil to lymphocyte ratio (NLR), derived NLR (d-NLR), aspartate aminotransferase, urea, creatine kinase (CK) myocardial band (CK-MB), procalcitonin (PCT) values were high whereas lymphocyte (LYM), eosinophil, red blood cells (RBC), hemoglobin, lymphocyte to monocyte ratio, estimated glomerular filtration rate values were low in the ICU group when compared with non-ICU. WBC, NEU, red cell distribution width, alanine transaminase, creatinine, urea, CK-MB, CK, direct bilirubin, lactate dehydrogenase, glucose, C-reactive protein, erythrocyte sedimentation rate, ferritin, D-dimer, PCT and international normalized ratio values increased while RBC, hemoglobin, hematocrit, mean corpuscular volume and total bilirubin values decreased in a significant proportion of patients in both groups based on the normal reference ranges. LYM count was found to be low in a significant number of patients (57.5%) especially in the ICU group and as an important risk factor and diagnostic parameter on admission to ICU (OR: 125, AUC: 0.74). Routine laboratory tests provide important information in terms of both diagnosis and severity of COVID-19. Lymphopenia is a condition that should be monitored which indicates the severity of the disease.</span>

Abstract:<p>Cases of coronavirus disease 2019 (COVID-19) have been breaking out around the world recently. However, the dynamic changes in the clinical symptoms and prognosis of COVID-19 patients remain unknown. According to the onset time of clinical symptoms, 843 COVID-19 patients admitted between Jan 22 and Feb 14, 2020 were divided into three groups: group A (Jan 21 to Jan 25, n = 324), group B (Jan 26 to Jan 31, n = 358) and group C (Feb 1 to Feb 10, n = 161). Data on the demographics, symptoms, first laboratory results, treatments and outcomes (within 12 days of hospitalization) were collected. The results showed that the median duration from symptom onset to admission shortened over time (12.5, 10 and 5 days, respectively, p < 0.05). Fewer patients had fever symptoms and bilateral pneumonia in group C than in the group A and B. Laboratory results showed that white blood cell, neutrophil, and platelet counts, lactic acid and D-dimer levels were lower, while lymphocyte, CD3, and CD8 counts were higher in group C. In addition, group C had more mild-moderate cases and fewer severe cases than the other two groups. More importantly, the incidence of complications (18.5%, 14.2% and 11.2%, respectively, p < 0.05) and all-cause mortality (11.7%, 8.4%, and 5.6%, respectively, p < 0.05) decreased over time. The clinical characteristics and prognosis of COVID-19 patients changed over time. Improved prognosis was found at a later stage.</p>

Abstract:Introduction: Hematological parameters are crucial factors in disease severity and chronic condition pathogenesis. We aimed to evaluate the hematological factors in different severity stages of COVID-19 at different time intervals. Methods: Serum samples were collected from 470 patients (235 men and 235 women) with a confirmed RT-qPCR COVID-19 test exhibiting moderate, severe, and critical symptoms based on WHO criteria. Samples were collected at three-time intervals, including the first: the 1st days of infection, 2nd: the one month after, and 3rd: the three months after disease onset. Total WBC, neutrophil, lymphocyte, monocyte, eosinophil, RBC counting, Hb, HCT, MCV, MCH, MCHC, hsCRP levels, G6PD deficiency, and hemoglobinopathies were determined in all patients. Results: Total WBC, neutrophil, lymphocyte, platelet, RBC counting, Hb, HCT, MCV, MCH, and hsCRP levels were significantly changed with different disease severity (p<0.0001). Also, there were significant differences between different time intervals for WBC and RBC parameters (p<0.0001) except for monocytes and eosinophils. At all time intervals, there are significant changes in levels of hematological and hsCRP based on gender. Moreover, a significant correlation was observed between disease severity, age, and BMI (p<0.0001). Conclusion: Significant differences in hematological parameter and inflammatory parameter levels based on disease severity, time intervals, and gender revealed the importance of evaluating these factors in the management of infectious diseases, such as COVID-19, in patients during and post-disease times.

Abstract:Abstract Background Coronavirus disease 2019 (COVID-19) is a newly emerging infectious disease and rapidly escalating epidemic caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The pathogenesis of COVID-19 remains to be elucidated. We aimed to clarify correlation of systemic inflammation with disease severity and outcomes in COVID-19 patients. Methods In this retrospective study, baseline characteristics, laboratory findings, and treatments were compared among 317 laboratory-confirmed COVID-19 patients with moderate, severe, or critically ill form of the disease. Moreover, the longitudinal changes of serum cytokines, lactate dehydrogenase (LDH), high-sensitivity C-reactive protein (hsCRP), and hsCRP to lymphocyte count ratio (hsCRP/L) as well as their associations with disease severity and outcomes were investigated in 68 COVID-19 patients. Results Within 24 h of admission, the critically ill patients showed higher concentrations of inflammatory markers including serum soluble interleukin (IL)-2 receptor, IL-6, IL-8, IL-10, tumor necrosis factor alpha (TNF-α), ferritin, procalcitonin, LDH, hsCRP, and hsCRP/L than patients with severe or moderate disease. The severe cases displayed the similar response patterns when compared with moderate cases. The longitudinal assays showed the levels of pro-inflammatory cytokines, LDH, hsCRP, and hsCRP/L gradually declined within 10 days post admission in moderate, severe cases or those who survived. However, there was no significant reduction in cytokines, LDH, hsCRP, and hsCRP/L levels in critically ill or deceased patients throughout the course of illness. Compared with female patients, male cases showed higher serum concentrations of soluble IL-2R, IL-6, ferritin, procalcitonin, LDH, and hsCRP. Multivariate logistic regression analysis revealed that IL-6 > 50 pg/mL and LDH > 400 U/L on admission were independently associated with disease severity in patients with COVID-19. Conclusion Exuberant inflammatory responses within 24 h of admission in patients with COVID-19 may correlate with disease severity. SARS-CoV-2 infection appears to elicit a sex-based differential immune response. IL-6 and LDH were independent predictive parameters for assessing the severity of COVID-19. An early decline of these inflammation markers may be associated with better outcomes.

Abstract:Abstract Background Peripheral hematological changes in severe COVID-19 patients may reflect the immune response during SARS-CoV-2 infection. Characteristics of peripheral white blood cells as early signals were needed to be investigated for clarifying its associations with the fatal outcomes in COVID-19 patients. Methods A retrospective cohort study was performed and the hospitalized COVID-19 patients were recruited in wards of Sino-French New City Branch of Tongji Hospital in Wuhan, Hubei province, China. Characteristics of peripheral white blood cells in survivors and non-survivors were analyzed. Comparison among patients with different level of eosinophils was performed. Results Of 198 patients included in this study, 185 were discharged and 13 died. Levels of eosinophils, lymphocytes and basophils in non-survivors were significantly lower than those in survivors. Death rate in low eosinophils group was higher and no patient died in normal eosinophils group (16.7% vs 0, P < 0.001). The proportion of patients in low eosinophils group who used glucocorticoids was higher than in normal eosinophils group, but glucocorticoids usage was not an indicator for death in subgroup analysis in low eosinophils patients. Moreover, positive correlation was found between the counts of lymphocytes and eosinophils in patients with glucocorticoids use but not in patients without the treatment. Conclusions Hematological changes differed between survivors and non-survivors with COVID-19. Lymphopenia and eosinopenia could be predictors for poor prognosis of COVID-19 patients. Initial counts of eosinophils may guide us in usage of glucocorticoids for COVID-19 treatment.
