Abstract:The aim of this study was to retrospectively interpret body composition in various wheelchair athletes. In total, 69 athletes (mean ± standard deviation; age 33 ± 11 years; body mass 65.1 ± 14.8 kg; height 169.9 ± 14.9 cm and time since injury 19 ± 11 years) from different national teams in wheelchair sports underwent a dual X-ray absorptiometry (DXA) measurement during the yearly medical check-up. The data showed a significant difference between total fat and total fat-free mass between male (fat mass: 15.1 ± 7.6 kg; fat-free mass: 51.8 ± 9.3 kg) and female (fat mass: 19.4 ± 7.8 kg; fat-free mass: 36.8 ± 7.6 kg) athletes (p = 0.032, p < 0.001). In contrast, no significant difference (p = 0.16, p = 0.07) in fat and fat-free mass between paraplegic, tetraplegic and non-SCI athletes was found. Comparing different sports, the lowest fat mass was found in paracycling athletes whereas curling game players showed the highest total fat mass. Basketball game players showed the highest fat-free mass (fat-free mass: 54.8 ± 10.1 kg). In tetraplegic athletes, difference in fat-free mass between left and right arms correlated with the upper extremity motor score. For the interpretation of the data it seems to be crucial, that many different parameters (i.e., gender, motor level of the injury) are taken into consideration in wheelchair athletes.

Body composition and disability in people with multiple sclerosis: A dual-energy x-ray absorptiometry study

Abstract:Introduction: Body composition refers to the relative distribution of different tissue types within the body, including fat, lean, and bone tissues. There is evidence for associations between body composition and the development of multiple sclerosis (MS). The relationship between body composition and disease progression and disability accumulation, however, is unclear. Objectives: To examine: a) differences in overall and regional body composition by disability status in persons with multiple sclerosis (MS); and b) the relationship between body composition and other outcomes reflecting impairments and limitations due to MS. Methods: Cross-sectional investigation of 47 ambulatory persons with relapsing remitting MS who were grouped by Expanded Disability Status Scale (EDSS) scores as having mild (1.0-4.0; n = 26) or moderate (4.5-6.5; n = 21) disability. Main outcome measures were whole-body and regional soft tissue composition (%body fat (BF), fat mass (FM), and fat-free soft tissue mass (FFM)), bone mineral content (BMC), and bone mineral density (BMD) determined from dual-energy X-ray absorptiometry (DXA). Other outcomes included physical fitness, mobility, cognitive processing speed, symptoms, and health-related quality of life (HRQOL). Results: Whole-body and regional %BF and FM were significantly higher, and whole-body and appendicular BMC and BMD were significantly lower in participants with moderate disability than those with mild disability (all p < .05). There were no significant differences in whole-body or regional FFM by disability status. In the overall sample, body fat correlated significantly with cardiorespiratory fitness (prs = -.52 to -.56), pain symptoms (prs = .32), and psychological HRQOL (prs = .34). FFM (prs = .38-.48) and BMC (prs = .53-.69) correlated primarily with measures of muscular strength. Conclusions: Persons with MS who have greater disability present with higher body fat and lower bone tissue content and density than those with mild disability. These findings highlight the need for strategies that address potential changes in body composition with disability accumulation.
Physique and performance in male sitting volleyball players: implications for classification and training

Abstract:Background: This study assessed whether anthropometry, physical fitness and sport-specific sprint performance vary across the three groups of sitting volleyball (SV) athletes (athletes with a disability (VS1), athletes with a minimal disability (VS2) and able-bodied SV athletes (AB)) in order to explore the validity of the current system of classification. This study also investigated how the anthropometric and physical fitness characteristics of athletes relate to their sprint performance. Methods: Thirty-five SV male athletes aged 37.4 ± 10.8 years and practicing SV at a national/international level volunteered for this study. Testing consisted in the evaluation of linear anthropometry, physical fitness (body composition by-means of dual-energy X-ray absorptiometry and upper-body strength) and sprint performance (5-meter sprint tests, agility test and speed and endurance test). Results: Athletes in the three groups differed in fat mass percentage (%FM) which was higher in VS1 versus AB at the sub-total level (+9%), in the arms (+15%) and in the non-impaired leg (+8%) regions. Greater hand span, greater length of the impaired lower leg, lower %FM at both the sub-total and regional level and a higher level of strength in the upper body are all associated with better performances in the considered sprint tests (P < 0.05 for all). These results do not confirm the validity of the current system of classification of athletes adopted in SV. Professionals dealing with SV athletes should include specific exercises aimed at improving whole-body and regional body composition and the strength of the trunk and upper limbs in their training programs.

Abstract:The study of body composition can be used to verify one’s state of fitness and the effectiveness of a training program. The aim of the study is to present the integration of this method for total and segmental assessment of body composition. 59 elite soccer players (aged 22.47 ± 5.58 years, height 1.81 ± 0.08 m) were enrolled for the study. Before the season and after 50 days of training, a body composition assessment was carried out. For the measurement of fat mass, folds of skin in 8 different regions were used. To evaluate cell mass and hydration status, bioimpedance with both whole-body and vector methods was used. The localized evaluation was performed by assessing bioelectrical values of the quadriceps, hamstrings and calf. Data before and after 50 days of soccer training were compared (t test for paired data). The values of body composition were within the normal range for the sample. After 50 days of training, there was a significant increase in BMI mainly dependent on extracellular water. The fat did not undergo quantitative changes but only a change in distribution occurs. Lower limb muscles showed an increase in the amount of water more significant than the total body assessment. An integration of fat and fat-free mass in athletes allows us to determine different adaptations to training methods. The assessment of body segments under more stress by physical training provides more specific information than the total body assessment in athletes. This method could evaluate a peripheral status of hydration and cellular mass.
Validation of assessments to accurately analyze the body composition of highly trained sitting volleyball players: A pilot study

Abstract:Body composition is a fundamental component of physical fitness related to the performance of Sitting volleyball (SV) players. Also, establishing the best method for evaluating the body composition of these para-athletes would be highly necessary for this field. The purpose of this study was (1) to describe the body composition of male and female highly trained SV players, (2) to compare the values obtained from this population by two different methods and (3) to establish validity on one of these methods. Thirteen Brazilian SV national team players (five males and eight females) participated in this study. The air-displacement plethysmography (ADP) method as the criterion assessment and the skinfolds (SF) method were conducted for each player. Results showed that there were no significant differences between the values of all players, which ADP and SF measured for body fat percentage (BF%) and body density (BD) (p > 0.05). We found significantly different values between male and female players for BF% by SF (p = 0.04) and BD by SF (p = 0.04). A high degree of reliability was found between ADP and SF measures for BF% and BD. There were statistically significant positive correlations between BF% and BD in all values for both methods (p < 0.01). This pilot study suggests that considering the magnitude of space, expense, and other limitations related to the ADP method against the SF method, we recommend using the SF method, which is a valid, viable and reliable method for measuring body composition in elite SV players.
Associations of Body Composition Trajectories with Bone Mineral Density, Muscle Function, Falls and Fractures in Older Men: The Concord Health and Ageing in Men Project.

Abstract:BACKGROUND Weight loss increases fracture risk in older adults. We aimed to determine associations of two-year body composition trajectories with subsequent falls and fractures in older men. METHODS We measured appendicular lean mass (ALM) and total fat mass (FM) by dual-energy X-ray absorptiometry at baseline and Year 2 in 1,326 community-dwelling men aged ≥70 years. Body composition trajectories were determined from residuals of a linear regression of change in ALM on change in FM (higher values indicate maintenance of ALM over FM), and a categorical variable for change in ALM and FM (did not lose [≥-5% change] vs lost [<-5% change]). Bone mineral density (BMD), hand grip strength and gait speed were assessed at Years 2 and 5. After Year 2, incident fractures (confirmed by radiographic reports) and falls were recorded for 6.8 years. RESULTS Compared with men who did not lose ALM or FM, men who did not lose ALM but lost FM, and men who lost both ALM and FM, had reduced falls (-24 and -34%, respectively; both P<0.05). Men who lost ALM but did not lose FM had increased falls (incidence rate ratio=1.73; 95%CI 1.37-2.18). ALM/FM change residuals were associated with improved lumbar spine BMD (B=0.007; 95%CI 0.002-0.012 g/cm2 per SD increase) and gait speed (0.015; 0.001-0.029 m/s), and reduced hip fractures (hazard ratio=0.68; 95%CI 0.47-0.99). CONCLUSIONS Fracture risk may be increased in older men who lose higher lean relative to fat mass. Weight loss interventions for obese older men should target maintenance of lean mass.

Abstract:Physical activity offers numerous physical and mental health benefits for individuals with disabilities, while nutrition plays a crucial role in maintaining bodily homeostasis. This study aimed to assess the relationship between body composition and dietary habits among physically active people with disabilities. Fifty-five participants aged 16 to 61, including 28 with disabilities and 27 without, were included in the study. The FFQ-6 questionnaire, Tanita body composition analyzer, and Stadiometer were utilized. No significant differences in BMI were observed between the two groups. However, individuals with disabilities showed higher body fat, metabolic age, or pulse values, whereas the control group exhibited higher muscle mass, muscle quality, body type, or bone mass. Participants with disabilities were more likely to consume vegetables (p = 0.004) and animal fats (p = 0.027), while those without disabilities were more inclined to consume fast food, instant products (p = 0.006), sweetened beverages (p < 0.001), and alcohol (p < 0.001). People with disabilities often have a higher percentage of body fat, cautioning against the consumption of processed fruits, dried fruits, fast food, and red meat. Conversely, in non-disabled individuals, frequent consumption of eggs, animal fats, sugar, and sweets is not recommended due to the potential for increased body fat, visceral fat, and higher BMI.

Abstract:The volume of literature on field-based physiological testing of wheelchair sports, such as basketball, rugby and tennis, is considerably smaller when compared with that available for individuals and team athletes in able-bodied (AB) sports. In analogy to the AB literature, it is recognized that performance in wheelchair sports not only relies on fitness, but also sport-specific skills, experience and technical proficiency. However, in contrast to AB sports, two major components contribute towards ‘wheeled sports’ performance, which are the athlete and the wheelchair. It is the interaction of these two that enable wheelchair propulsion and the sporting movements required within a given sport. Like any other athlete, participants of wheelchair sports are looking for efficient ways to train and/or analyse their technique and fitness to improve their performance. Consequently, laboratory and/or field-based physiological monitoring tools used at regular intervals at key time points throughout the year must be considered to help with training evaluation. The present review examines methods available in the literature to assess wheelchair sports fitness in a field-based environment, with special attention on outcome variables, validity and reliability issues, and non-physiological influences on performance. It also lays out the context of field-based testing by providing details about the Paralympic court sports and the impacts of a disability on sporting performance. Due to the limited availability of specialized equipment for testing wheelchair-dependent participants in the laboratory, the adoption of field-based testing has become the preferred option by team coaches of wheelchair athletes. An obvious advantage of field-based testing is that large groups of athletes can be tested in less time. Furthermore, athletes are tested in their natural environment (using their normal sports wheelchair set-up and floor surface), potentially making the results of such testing more relevant than laboratory testing. However, given that many tests, such as the multistage fitness test and the Yo-Yo intermittent test, have originally been developed for AB games players, the assumption that these can also be used for wheelchair athletes may be erroneous. With the array of AB aerobic and anaerobic field tests available, it is difficult to ascertain which ones may be best suited for wheelchair athletes. Therefore, new, wheelchair sport-specific tests have been proposed and validated. Careful selection of tests to enable coaches to distinguish between disability classifications, wheelchair proficiency and actual performance improvements is paramount as this will not only enhance the value of field-based testing, but also help with the development of meaningful normative data.

Abstract:This cross-sectional study analyses differences in dietary habits, taste preferences, variety of protein sources and body composition (BC) profiles among individuals following omnivorous, flexitarian, lacto-ovo-vegetarian and pescatarian diets. Furthermore, it assesses the correlations between these dietary patterns and various sports, classified by exercise intensity, in relation to BC parameters. The study analysed the eating habits and BC data of 1342 participants aged 18–65 years, classified into four diet groups based on their 7-day food diaries and questionnaire responses. Our analysis revealed gender- and age-related differences in weekly food consumption and protein source variety, with men generally consuming more meat, processed meat and fish than women, especially in younger age groups. Differences in dairy and soy consumption were also noted between age groups, while legume and soy preferences showed no gender disparity across all ages. Among non-sporting individuals, vegetarians exhibited lower fat mass (FM%) compared to other diets, while among athletes, vegetarians and pescatarians in in endurance and strength sports, respectively, displayed lower FM%, with flexitarians and omnivores in endurance sports showing higher FM%. Non-athletic omnivores and vegetarians demonstrated a greater proportion of body protein, while among athletes, those engaged in strength training exhibited a higher body protein content across all dietary groups compared to those in endurance training. Among non-athletic groups, vegetarians exhibited the lowest FM/FFM (fat mass/fat-free mass) ratio, while among athletes, vegetarians in endurance sports and participants in strength training across other diets showed lower FM/FFM ratios. The results emphasise the complex interaction between diet, BC and lifestyle choices, revealing how different combinations of diet and sport are associated with optimised BC.

Do Paralympic athletes suffer from brittle bones? Prevalence and risk factors of low bone mineral density in Paralympic athletes

Abstract:Background: Bone health may be a concern in Paralympic athletes, given the presence of multiple risk factors predisposing these athletes to low bone mineral density (BMD). Objective: We aimed to assess the prevalence of low BMD among Paralympic athletes participating in various sport disciplines, and to identify potential risk factors for low BMD. Methods: Seventy Paralympic athletes, of whom 51 % were wheelchair-dependent, were included in this cross-sectional study. BMD of the whole-body, lumbar spine, total hip, and femoral neck were assessed by dual-energy x-ray absorptiometry. Comparisons between groups were conducted by one-way ANOVA, and regression analyses were conducted to identify potential risk factors for low BMD. Results: The prevalence of low BMD (Z-score < -1.0) was highest at femoral neck (34 %), followed by total hip (31 %), whole-body (21 %), and lumbar spine (18 %). Wheelchair-dependent athletes had significantly lower BMD Z-scores compared to the non-wheelchair-dependent athletes at whole-body level (-0.5 ± 1.4 vs 0.2 ± 1.3; P = 0.04), total hip (-1.1 ± 1.2 vs 0.0 ± 1.1; P < 0.01), and femoral neck (-1.0 ± 1.3 vs -0.1 ± 1.2; P < 0.01). At the lumbar spine, low BMD was completely absent in wheelchair basketball and tennis players. Regression analyses identified body mass, wheelchair dependence, and type of sport, as the main risk factors for low BMD. Conclusions: In this cohort of Paralympic athletes, low BMD is mainly present at the hip, and to a lesser extent at the whole-body and lumbar spine. The most prominent risk factors for low BMD in Paralympic athletes are related to mechanical loading patterns, including wheelchair use, the type of sport, and body mass.
Bone Health, Body Composition and Physiological Demands in 70-85-Year-Old Lifelong Male Football Players

Abstract:The effects of lifelong football training on bone health, body composition and physiological demands were evaluated. A total of 20 veteran football players (VPG; 73.4 ± 3.7 years) and 18 untrained age-matched men (CG; 75.6 ± 4.2 years) were enrolled. Whole-body and regional dual-energy X-ray absorptiometry scans of arms, legs, proximal femur and lower spine (L1-L4) were recorded in all participants. We observerd higher bone mineral density (BMD) in the whole-body, arms and femoral regions and higher bone mineral content (BMC) in the legs and lower spine compared to the CG (p < 0.05), also higher total lean body mass (p < 0.05) and lower total body fat percentage (p < 0.05), were found. No differences in food habits were evidenced between the VPG and the CG, as evaluated using 3-day food records. Resting heart rate (RHR), blood pressure (BP) and activity profile during a football match were recorded using a global positioning system only in the VPG. The mean heart rate (HR)of theoretical maximal HR (ThHRmax), and peak of ThHRmax were 83.9 ± 8.6% and 98.6 ± 10.2%, respectively; the mean of total distance covered was 3666 ± 721 m, and the means of accelerations and decelerations were 419 ± 61 and 428 ± 65, respectively. Lifelong participation in football training improves regional BMD and BMC in legs, femur and lumbar spine compared to the CG. A high number of intense actions in term of HR and accelerations and decelerations suggests an elevated energy expenditure that in turn correlates to the healthier body composition observed in the VPG compared to the CG.
Body Composition of Elite Female Players in Five Different Sports Games

Abstract:Abstract The goal of this study was to identify and compare body composition (BC) variables in elite female athletes (age ± years): volleyball (27.4 ± 4.1), softball (23.6 ± 4.9), basketball (25.9 ± 4.2), soccer (23.2 ± 4.2) and handball (24.0 ± 3.5) players. Fat-free mass (FFM), fat mass, percentage of fat mass (FMP), body cell mass (BCM), extracellular mass (ECM), their ratio, the percentage of BCM in FFM, the phase angle (α), and total body water, with a distinction between extracellular (ECW) and intracellular water, were measured using bioimpedance analysis. MANOVA showed significant differences in BC variables for athletes in different sports (F60.256 = 2.93, p 0.05). Significant changes in other BC variables were found in analyses when sport was used as an independent variable. Soccer players exhibited the most distinct BC, differing from players of other sports in 8 out of 10 variables. In contrast, the athletes with the most similar BC were volleyball and basketball players, who did not differ in any of the compared variables. Discriminant analysis revealed two significant functions (p < 0.01). The first discriminant function primarily represented differences based on the FFM proportion (volleyball, basketball vs. softball, soccer). The second discriminant function represented differences based on the ECW proportion (softball vs. soccer). Although all of the members of the studied groups competed at elite professional levels, significant differences in the selected BC variables were found. The results of the present study may serve as normative values for comparison or target values for training purposes.

Impact of body composition on clinical outcomes in patients with active radiographic axial spondyloarthritis under biological therapy

Abstract:Abstract Objective To assess the association of body composition (BC), evaluated by bioimpedance analysis (BIA), with disease activity, physical function and mobility in patients with axSpA undergoing bDMARD treatment for one year. Methods Patients with AS (radiographic axSpA) were enrolled in an extension of the German Spondyloarthritis Inception Cohort (GESPIC). Patients were required to be candidates for bDMARD therapy at baseline presenting high disease activity despite previous treatment with nonsteroidal anti-inflammatory drugs. Outcomes (disease activity, function and mobility) and BC parameters were assessed at baseline and every 6 months thereafter. The association between BC parameters and outcomes over 1 year was analysed using longitudinal generalized estimating equations. Results Seventy-four patients with radiographic axSpA were included in current analysis with a mean age of 36.5 years, disease duration of 6.2 years and Axial Spondyloarthritis Disease Activity Score (ASDAS) of 3.4 at baseline. Fat mass value and fat mass index were positively associated with disease activity (ASDAS: ß = 0.012, 95% CI [−0.003, 0.027] and ß = 0.035, 95% CI [−0.010, 0.081], respectively) and functional disability (BASFI: ß = 0.037, 95% CI [0.004, 0.070] and ß = 0.125, 95% CI [0.031, 0.219], respectively). Visceral adipose tissue (VAT) was associated with reduced spine mobility (BASMI: ß = 0.20, 95% CI [0.07, 0.33]). Additionally, increase in VAT and fat mass parameters was linked to worse disease activity and functional disability in women, while they were strongly associated with reduced spinal mobility in men. Conclusion Higher levels of body fat and VAT were positively associated with increased disease activity, functional disability, and reduced spinal mobility in patients with radiographic axSpA treated with bDMARDs.

Abstract:Sport and exercise participation are associated with small, albeit positive improvements in subjective well-being (SWB). Recent theorizing has emphasized the importance of distinguishing between performance aspects (i.e., frequency, intensity, time engaged) and the experiential aspects of sport and exercise participation among people with disabilities. This study assessed the relative contributions of time spent participating in sport and exercise (a performance measure) versus measures of participation experiences, in explaining variance in SWB. Participants were 535 adults with physical disabilities, recruited through a nation-wide survey, who participated in sport (n = 271; 62% male; 44 ± 14 years) or exercise (n = 264; 42% male; 57 ± 14 years). They completed measures of minutes/week of sport or exercise participation, experiential aspects of participation, and SWB measures (overall life satisfaction, satisfaction with physical, psychological and social life-domains, and positive/negative affect). Hierarchical multiple regression analyses showed minutes/week of sport did not explain significant variance in any SWB measures, but sport participation experiences explained significant variance (9.2%–20.9%) in all SWB measures (p < 0.05). Similarly, minutes/week of exercise explained significant variance only in physical life-domain satisfaction (ΔR 2 = 2.6%, p = 0.026), but exercise participation experiences explained significant variance (4.8%–10.7%) in all SWB measures (p < 0.05). Experiences of belonging and mastery were particularly strong, consistent predictors across SWB outcomes. These results suggest participation experiences better explain relationships between sport and exercise participation and SWB than time spent performing sport and exercise. Findings have implications for designing future studies to test the effects of sport and exercise on SWB, and developing theories and interventions to explain and maximize the use of sport and exercise to improve SWB in adults with disabilities.

Abstract:The purpose of this study was to review the various risks and benefits of wheelchair basketball (WB) and explore some of the research which outlines factors that influence WB player performance and conditioning. WB offers several physical and psychological advantages. Physically, it can improve muscle strength, endurance, and cardiovascular fitness while decreasing the prevalence of chronic physical disorders. From a psychological standpoint, WB has been shown to alleviate anxiety and feelings of depression while also creating and improving social relationships. Despite the many benefits, WB can cause injuries, particularly in the upper extremities, and preventative measures should be employed. WB necessitates intense intermittent efforts and athletes must maintain excellent cardiovascular fitness, strength, and muscular endurance. Healthy sleeping patterns have also been shown to improve performance in WB players. Wheelchair mobility and biomechanical variables as well as wheelchair size and weight appear to be critical success elements in WB. WB can be a powerful tool for coaches and therapists to boost the physical and emotional health of individuals with disabilities and motivate them to participate in team-based sport.
Change of physical activity parameters of hip and pelvic fracture patients during inpatient rehabilitation and after discharge: analysis of global and in-depth parameters

Abstract:Abstract Background A growing number of older adults suffer hip and pelvic fractures leading to hospital admission. They often result in reduced physical activity (PA) and impaired mobility. PA can be objectively measured with body-worn sensors. Usually, global cumulative PA parameters are analysed, such as walking duration, upright-time and number of steps. These traditional parameters mix different domains of PA, such as physical capacity (PC), behaviour and living environment. We examined the change of global cumulative PA measures during rehabilitation and after discharge in patients with hip or pelvic fracture and whether more ‘in-depth’ PA parameters, such as walking interval length, variability of interval length and sit-to-stand transitions and their changes during rehabilitation and 3 months after discharge might better reflect the above mentioned three clinically relevant domains of PA. Methods This study is a secondary data analysis of a randomised controlled trial to improve PA and fall-related self-efficacy in hip or pelvic fracture patients (≥60 years) with concerns about falling. Changes of accelerometer-measured global cumulative and in-depth PA parameters (activPAL3) were analysed in an observational design before and after discharge combining both groups. For comparison, the same analyses were applied to the traditional PC measures gait speed and 5-chair-rise. Results Seventy-five percent of the 111 study participants were female (mean age: 82.5 (SD = 6.76) years. Daily walking duration, upright time and number of steps as aspects of global PA increased during inpatient rehabilitation as well as afterwards. The in-depth PA parameters showed differing patterns. While the total number of walking bouts increased similarly, the number of longer walking bouts decreased by 50% after discharge. This pattern was also seen for the average walking interval length, which increased by 2.34 s (95% confidence interval (CI): 0.68; 4.00) during inpatient rehabilitation and decreased afterwards below baseline level (− 4.19 s (95% CI: − 5.56; − 2.82)). The traditional PC measures showed similar patterns to the in-depth PA parameters with improvements during rehabilitation, but not at home. Conclusion Our findings suggest that the in-depth PA parameters add further information to the global cumulative PA parameters. Whereas global cumulative PA parameters improved significantly during inpatient rehabilitation and after discharge, in-depth PA parameters as well as PC did not continuously improve at home. In contrast to global cumulative PA parameters the in-depth parameters seem to reflect contextual factors such as the build environment and aspects of PC, which are traditionally assessed by clinical PC measures. These in combination with digital mobility measures can help clinicians to assess the health status of fragility fracture patients, individually tailor therapy measures and monitor the rehabilitation process.

Effect of Fatiguing Wheelchair Propulsion and Weight Relief Lifts on Subacromial Space in Wheelchair Users

Abstract:Objective This study aimed to identify targets of intervention for reducing shoulder pain in wheelchair users with spinal cord injury (SCI) by (1) examining changes in subacromial space [acromiohumeral distance (AHD) and occupation ratio (OccRatio)] with fatiguing wheelchair propulsion, and different loading conditions [unloaded position vs. weight relief lifts (WRL)]; (2) associating these changes with wheelchair user capacity, as well as (3) identifying subject characteristics associated with subacromial space, such as sex, lesion level, time since injury, body mass index and impaired shoulder range of motion. Methods Fifty manual wheelchair users with SCI [11 females, age = 50.5 (9.7) years, time since injury = 26.2 (11.4) years] participated in this quasi-experimental one-group pretest-posttest study. Ultrasound images were used to define AHD during an unloaded position, and during personal and instructed WRL before and after fatiguing wheelchair propulsion. Furthermore, supraspinatus and biceps thickness defined from ultrasound images were used to calculate OccRatios. Wheelchair user capacity was quantified as functional strength (maximum resultant force reached during maximum isometric forward push) and anaerobic work capacity (highest power output reached during 15-m sprint test). Multilevel mixed-effects linear regression analyses controlling for between subject variability and covariables were performed to address the research questions. Results AHD was significantly smaller during personal WRL ( p < 0.001) and instructed WRL ( p = 0.009, AHD both 11.5 mm) compared to the unloaded position (11.9 mm). A higher wheelchair user capacity (higher anaerobic work capacity) reduced the impact of WRL on AHD decrease. The fatiguing wheelchair propulsion had no effect on AHD ( p = 0.570) and on OccRatio of supraspinatus ( p = 0.404) and biceps ( p = 0.448). Subject characteristics related to a larger subacromial space were lower lesion level, shorter time since injury, impaired external rotation, a lower body mass index and a higher anaerobic work capacity. Conclusion This study showed a significant reduction in AHD during WRL with no effect of fatiguing wheelchair propulsion on the subacromial space in wheelchair users with SCI. A higher anaerobic work capacity was beneficial in stabilizing the shoulder during WRL. Our findings may assist clinicians in designing a shoulder injury prevention program.

Abstract:Soccer is a laterally dominant sport owing to the repetitive nature of unilateral kicking. The relationship between functional and body composition asymmetries related to limb dominance in soccer players has yet to be established. When present, asymmetries can increase the risk of injury and low back pain. Our study investigated whether lateral dominance is associated with limb asymmetries in a comprehensive body composition assessment among varsity soccer players. Twenty-seven varsity soccer players (age 20.4 ± 1.7 years old; BMI 22.6 ± 4.6 kg/m2) participated in this study. Body composition was assessed through dual-energy X-ray absorptiometry scans. Results showed low lower limb asymmetry indices in both males (3.82%) and females (3.36%) compared to normal ranges. However, upper limb lean mass exhibited high asymmetry, surpassing thresholds in males (7.3%) and females (4.39%). Significant differences were found in total bone mass among males and total lean body mass among females. Male players exhibited higher asymmetry indices in both arm and trunk mass compared to females. Despite these asymmetries, no significant correlations were found between asymmetry indices and occurrences of lower limb injury or low back pain. The study suggests that while evaluating body composition for injury prevention in soccer shows potential, lateral dominance may be influenced by factors extending beyond sport-specific adaptations.
