Abstract:The diagnosis of preoperative lymph node (LN) metastasis is crucial to evaluate possible therapy options for T1 lung adenocarcinoma patients. Radiologists preoperatively diagnose LN metastasis by evaluating signs related to LN metastasis, like spiculation or lobulation of pulmonary nodules in CT images. However, this type of evaluation is subjective and time-consuming, which may result in poor consistency and low efficiency of diagnoses. In this study, a 3D Multi-scale, Multi-task, and Multi-label classification network (3M-CN) was proposed to predict LN metastasis, as well as evaluate multiple related signs of pulmonary nodules in order to improve the accuracy of LN metastasis prediction. The following key approaches were adapted for this method. First, a multi-scale feature fusion module was proposed to aggregate the features from different levels for which different labels be best modeled at different levels; second, an auxiliary segmentation task was applied to force the model to focus more on the nodule region and less on surrounding unrelated structures; and third, a cross-modal integration module called the refine layer was designed to integrate the related risk factors into the model to further improve its confidence level. The 3M-CN was trained using data from 401 cases and then validated on both internal and external datasets, which consisted of 100 cases and 53 cases, respectively. The proposed 3M-CN model was then compared with existing state-of-the-art methods for prediction of LN metastasis. The proposed model outperformed other methods, achieving the best performance with AUCs of 0.945 and 0.948 in the internal and external test datasets, respectively. The proposed model not only obtain strong generalization, but greatly enhance the interpretability of the deep learning model, increase doctors' confidence in the model results, conform to doctors' diagnostic process, and may also be transferable to the diagnosis of other diseases.

Deep learning model based on primary tumor to predict lymph node status in clinical stage IA lung adenocarcinoma: a multicenter study

Abstract:Objective: To develop a deep learning model to predict lymph node (LN) status in clinical stage IA lung adenocarcinoma patients. Methods: This diagnostic study included 1,009 patients with pathologically confirmed clinical stage T1N0M0 lung adenocarcinoma from two independent datasets (699 from Cancer Hospital of Chinese Academy of Medical Sciences and 310 from PLA General Hospital) between January 2005 and December 2019. The Cancer Hospital dataset was randomly split into a training cohort (559 patients) and a validation cohort (140 patients) to train and tune a deep learning model based on a deep residual network (ResNet). The PLA Hospital dataset was used as a testing cohort to evaluate the generalization ability of the model. Thoracic radiologists manually segmented tumors and interpreted high-resolution computed tomography (HRCT) features for the model. The predictive performance was assessed by area under the curves (AUCs), accuracy, precision, recall, and F1 score. Subgroup analysis was performed to evaluate the potential bias of the study population. Results: A total of 1,009 patients were included in this study; 409 (40.5%) were male and 600 (59.5%) were female. The median age was 57.0 years (inter-quartile range, IQR: 50.0-64.0). The deep learning model achieved AUCs of 0.906 (95% CI: 0.873-0.938) and 0.893 (95% CI: 0.857-0.930) for predicting pN0 disease in the testing cohort and a non-pure ground glass nodule (non-pGGN) testing cohort, respectively. No significant difference was detected between the testing cohort and the non-pGGN testing cohort (P = 0.622). The precisions of this model for predicting pN0 disease were 0.979 (95% CI: 0.963-0.995) and 0.983 (95% CI: 0.967-0.998) in the testing cohort and the non-pGGN testing cohort, respectively. The deep learning model achieved AUCs of 0.848 (95% CI: 0.798-0.898) and 0.831 (95% CI: 0.776-0.887) for predicting pN2 disease in the testing cohort and the non-pGGN testing cohort, respectively. No significant difference was detected between the testing cohort and the non-pGGN testing cohort (P = 0.657). The recalls of this model for predicting pN2 disease were 0.903 (95% CI: 0.870-0.936) and 0.931 (95% CI: 0.901-0.961) in the testing cohort and the non-pGGN testing cohort, respectively. Conclusions: The superior performance of the deep learning model will help to target the extension of lymph node dissection and reduce the ineffective lymph node dissection in early-stage lung adenocarcinoma patients.

Abstract:Introduction: Lymph node (LN) metastases are an important determinant of survival in patients with colon cancer, but remain difficult to accurately diagnose on preoperative imaging. This study aimed to develop and evaluate a deep learning model to predict LN status on preoperative staging CT.Methods: In this ambispective diagnostic study, a deep learning model using a ResNet-50 framework was developed to predict LN status based on preoperative staging CT. Patients with a preoperative staging abdominopelvic CT who underwent surgical resection for colon cancer were enrolled. Data were retrospectively collected from February 2007 to October 2019 and randomly separated into training, validation, and testing cohort 1. To prospectively test the deep learning model, data for testing cohort 2 was collected from October 2019 to July 2021. Diagnostic performance measures were assessed by the AUROC.Results: A total of 1,201 patients (median [range] age, 72 [28-98 years]; 653 [54.4%] male) fulfilled the eligibility criteria and were included in the training (n = 401), validation (n = 100), testing cohort 1 (n = 500) and testing cohort 2 (n = 200). The deep learning model achieved an AUROC of 0.619 (95% CI 0.507-0.731) in the validation cohort. In testing cohort 1 and testing cohort 2, the AUROC was 0.542 (95% CI 0.489-0.595) and 0.486 (95% CI 0.403-0.568), respectively.Conclusion: A deep learning model based on a ResNet-50 framework does not predict LN status on preoperative staging CT in patients with colon cancer.

Abstract:In the clinic, it is difficult to distinguish the malignancy and aggressiveness of solid pulmonary nodules (PNs). Incorrect assessments may lead to delayed diagnosis and an increased risk of complications. We developed and validated a deep learning-based model for the prediction of malignancy as well as local or distant metastasis in solid PNs based on CT images of primary lesions during initial diagnosis. In this study, we reviewed the data from multiple patients with solid PNs at our institution from 1 January 2019 to 30 April 2022. The patients were divided into three groups: benign, Ia-stage lung cancer, and T1-stage lung cancer with metastasis. Each cohort was further split into training and testing groups. The deep learning system predicted the malignancy and metastasis status of solid PNs based on CT images, and then we compared the malignancy prediction results among four different levels of clinicians. Experiments confirmed that human–computer collaboration can further enhance diagnostic accuracy. We made a held-out testing set of 134 cases, with 689 cases in total. Our convolutional neural network model reached an area under the ROC (AUC) of 80.37% for malignancy prediction and an AUC of 86.44% for metastasis prediction. In observer studies involving four clinicians, the proposed deep learning method outperformed a junior respiratory clinician and a 5-year respiratory clinician by considerable margins; it was on par with a senior respiratory clinician and was only slightly inferior to a senior radiologist. Our human–computer collaboration experiment showed that by simply adding binary human diagnosis into model prediction probabilities, model AUC scores improved to 81.80–88.70% when combined with three out of four clinicians. In summary, the deep learning method can accurately diagnose the malignancy of solid PNs, improve its performance when collaborating with human experts, predict local or distant metastasis in patients with T1-stage lung cancer, and facilitate the application of precision medicine.

Abstract:Lymph node (LN) metastasis status is one of the most critical prognostic and cancer staging factors for patients with resectable pancreatic ductal adenocarcinoma (PDAC), or in general, for any types of solid malignant tumors. Preoperative prediction of LN metastasis from non-invasive CT imaging is highly desired, as it might be straightforwardly used to guide the following neoadjuvant treatment decision and surgical planning. Most studies only capture the tumor characteristics in CT imaging to implicitly infer LN metastasis and very few work exploit direct LN's CT imaging information. To the best of our knowledge, this is the first work to propose a fully-automated LN segmentation and identification network to directly facilitate the LN metastasis status prediction task. Nevertheless LN segmentation/detection is very challenging since LN can be easily confused with other hard negative anatomic structures (e.g., vessels) from radiological images. We explore the anatomical spatial context priors of pancreatic LN locations by generating a guiding attention map from related organs and vessels to assist segmentation and infer LN status. As such, LN segmentation is impelled to focus on regions that are anatomically adjacent or plausible with respect to the specific organs and vessels. The metastasized LN identification network is trained to classify the segmented LN instances into positives or negatives by reusing the segmentation network as a pre-trained backbone and padding a new classification head. More importantly, we develop a LN metastasis status prediction network that combines the patient-wise aggregation results of LN segmentation/identification and deep imaging features extracted from the tumor region. Extensive quantitative nested five-fold cross-validation is conducted on a discovery dataset of 749 patients with PDAC.

Abstract:In colorectal cancer (CRC), accurately predicting lymph node metastasis (LNM) contributes to developing appropriate treatment plans and serves as the key to long‐term survival of patients. In the clinical settings, preoperative LNM diagnosis in CRC predominantly depends on computed tomography (CT). Nevertheless, lymph nodes are small in size and difficult to identify on 3D CT scans, and CT‐based diagnosis of metastatic lymph nodes is prone to a significant misdiagnosis rate and lacks consistency across clinicians. Currently, there is no automatic system available for LNM prediction in CRC via 3D CT scans. In addition, existing deep learning‐ (DL‐) based lymph node detection models present low detection accuracy and high false‐positive rates, and most existing DL‐based lymph node metastasis prediction models mainly use tumor area characteristics but fail to adequately utilize lymph node information, thus not yielding satisfactory results. To tackle these issues, we propose an intelligent diagnosis system for this challenging task, mainly including a lymph node detection (LND) model and a lymph node metastasis prediction (LNMP) model. In detail, the LND model utilizes an encoder‐decoder network to detect lymph nodes, and the LNMP model employs an innovative attention‐based multiple instance learning (MIL) network. An instance‐level self‐attention feature enhancement module is designed to extract and augment lymph node features as a bag of instances. Furthermore, a bag‐level MIL prediction module is employed to extract instance features and create a bag representation for the ultimate LNM prediction. As far as we know, the proposed intelligent system represents the pioneering method for addressing this complex clinical challenge. In experiments, our proposed intelligent system achieves the AUC of 75.4% and the accuracy of 73.9%, showcasing a significant enhancement compared to physicians specialising in CRC and highlighting its strong clinical applicability. Abstract:: Identification of early-stage pulmonary adenocarcinomas before surgery, especially in cases of subcentimeter cancers, would be clinically important and could provide guidance to clinical decision making. In this study, we developed a deep learning system based on 3D convolutional neural networks and multitask learning, which automatically predicts tumor invasiveness, together with 3D nodule segmentation masks. The system processes a 3D nodule-centered patch of preprocessed CT and learns a deep representation of a given nodule without the need for any additional information. A dataset of 651 nodules with manually segmented voxel-wise masks and pathological labels of atypical adenomatous hyperplasia (AAH), adenocarcinomas in situ (AIS), minimally invasive adenocarcinoma (MIA), and invasive pulmonary adenocarcinoma (IA) was used in this study. We trained and validated our deep learning system on 523 nodules and tested its performance on 128 nodules. An observer study with 2 groups of radiologists, 2 senior and 2 junior, was also investigated. We merged AAH and AIS into one single category AAH-AIS, comprising a 3-category classification in our study. The proposed deep learning system achieved better classification performance than the radiologists; in terms of 3-class weighted average F1 score, the model achieved 63.3% while the radiologists achieved 55.6%, 56.6%, 54.3%, and 51.0%, respectively. These results suggest that deep learning methods improve the yield of discriminative results and hold promise in the CADx application domain, which could help doctors work efficiently and facilitate the application of precision medicine. SIGNIFICANCE: Machine learning tools are beginning to be implemented for clinical applications. This study represents an important milestone for this emerging technology, which could improve therapy selection for patients with lung cancer.
Computed tomography-based deep-learning prediction of lymph node metastasis risk in locally advanced gastric cancer

Abstract:Purpose Preoperative evaluation of lymph node metastasis (LNM) is the basis of personalized treatment of locally advanced gastric cancer (LAGC). We aim to develop and evaluate CT-based model using deep learning features to preoperatively predict LNM in LAGC. Methods A combined size of 523 patients who had pathologically confirmed LAGC were retrospectively collected between August 2012 and July 2019 from our hospital. Five pre-trained convolutional neural networks were exploited to extract deep learning features from pretreatment CT images. And the support vector machine (SVM) was employed as the classifier. We assessed the performance using the area under the receiver operating characteristics curve (AUC) and selected an optimal model, which was compared with a radiomics model developed from the training cohort. A clinical model was built with clinical factors only for baseline comparison. Results The optimal model with features extracted from ResNet yielded better performance with AUC of 0.796 [95% confidence interval (95% CI), 0.715-0.865] and accuracy of 75.2% (95% CI, 67.2%-81.5%) in the testing cohort, compared with 0.704 (0.625-0.783) and 61.8% (54.5%-69.9%) for the radiomics model. The predictive performance of all the radiological models were significantly better than the clinical model. Conclusion The novel and noninvasive deep learning approach could provide efficient and accurate prediction of lymph node metastasis in LAGC, and benefit clinical decision making of therapeutic strategy.

Abstract:Three Negro patients were evaluated for a diffuse dark pigmentation present from birth on all fingernails and toenails. Biopsies revealed numerous dopa-positive melanocytes of the nail matrix and pigmentation of the nail plate. The pigmentation was shown to be melanin, and it is postulated that this represents an extreme variant of the band-like pigmentation that is frequently seen in the nails of Negroes. Two hundred Negroes were evaluated for the presence of pigment anomalies, and pigmented bands were found to be as common as 77% in Negroes over the age of 20 years.
Development and Validation of a Deep Learning Radiomics Model Predicting Lymph Node Status in Operable Cervical Cancer

Abstract:Aim: To develop and validate a deep learning radiomics model, which could predict the lymph node metastases preoperatively in cervical cancer patients.Patients and methods: We included a cohort of 226 pathological proven operable cervical cancer patients in two academic medical institutions from December 2014 to November 2017. Then this dataset was split into training set (n = 176) and independent testing set (n = 50) randomly. Five radiomic features were selected and a radiomic signature was established. We then combined these five radiomic features with the preoperative tumor histology and grade of these patients together. Baseline logistic regression model (LRM) and support vector machine model (SVM) were established for the comparison. We then explored the performance of a deep neural network (DNN), which is a popular deep learning model nowadays. Finally, performance of this DNN was validated in another independent test set including 50 cases of operable cervical cancer patients.Results: One thousand forty-five radiomic features were extracted for each patient. Twenty-eight features were found to be significantly correlated with the lymph node status in these patients (P < 0.05). Five radiomic features were further selected for further study due to their higher predictive powers. Baseline LRM incorporating these five radiomic and two clinicopathological features was established, which had an area under receiver operating characteristic curve (ROC) of 0.7372 and an accuracy of 89.20%. The established DNN model had four neural layers, in which layer there were 10 neurons. Adagrad optimizer and 1,500 iterations were used in training. The trained DNN had an area under curve (AUC) of 0.99 and an accuracy of 97.16% in the internal validation. To exclude the overfitting, independent external validation was also performed. AUC and accuracy in test set could still retain 0.90 and 92.00% respectively.Conclusion: This study used deep learning method to provide a comprehensive predictive model using preoperative CT images, tumor histology, and grade in cervical cancer patients. This model showed an acceptable accuracy in the prediction of lymph node status in cervical cancer. Our model may help identifying those patients who could benefit a lot from radiation therapy rather than primary hysterectomy surgery if this model could resist strict testing of future randomized controlled trials (RCTs).

Abstract:Background: Different pathological subtypes of lung adenocarcinoma lead to different treatment decisions and prognoses, and it is clinically important to distinguish invasive lung adenocarcinoma from preinvasive adenocarcinoma (adenocarcinoma in situ and minimally invasive adenocarcinoma). This study aims to investigate the performance of the deep learning approach based on high-resolution computed tomography (HRCT) images in the classification of tumor invasiveness and compare it with the performances of currently available approaches. Methods: In this study, we used a deep learning approach based on 3D conventional networks to automatically predict the invasiveness of pulmonary nodules. A total of 901 early-stage non-small cell lung cancer patients who underwent surgical treatment at Shanghai Chest Hospital between November 2015 and March 2017 were retrospectively included and randomly assigned to a training set (n=814) or testing set 1 (n=87). We subsequently included 116 patients who underwent surgical treatment and intraoperative frozen section between April 2019 and January 2020 to form testing set 2. We compared the performance of our deep learning approach in predicting tumor invasiveness with that of intraoperative frozen section analysis and human experts (radiologists and surgeons). Results: The deep learning approach yielded an area under the receiver operating characteristic curve (AUC) of 0.946 for distinguishing preinvasive adenocarcinoma from invasive lung adenocarcinoma in the testing set 1, which is significantly higher than the AUCs of human experts (P 0.05) and junior thoracic surgeons (0.768, P>0.05). Conclusions: We developed a deep learning model that achieved comparable performance to intraoperative frozen section analysis in determining tumor invasiveness. The proposed method may contribute to clinical decisions related to the extent of surgical resection.

Abstract:Background and Purpose The preoperative LN (lymph node) status of patients with LUAD (lung adenocarcinoma) is a key factor for determining if systemic nodal dissection is required, which is usually confirmed after surgery. This study aimed to develop and validate a nomogram for preoperative prediction of LN metastasis in LUAD based on a radiomics signature and deep learning signature. Materials and Methods This retrospective study included a training cohort of 200 patients, an internal validation cohort of 40 patients, and an external validation cohort of 60 patients. Radiomics features were extracted from conventional CT (computed tomography) images. T-test and Extra-trees were performed for feature selection, and the selected features were combined using logistic regression to build the radiomics signature. The features and weights of the last fully connected layer of a CNN (convolutional neural network) were combined to obtain a deep learning signature. By incorporating clinical risk factors, the prediction model was developed using a multivariable logistic regression analysis, based on which the nomogram was developed. The calibration, discrimination and clinical values of the nomogram were evaluated. Results Multivariate logistic regression analysis showed that the radiomics signature, deep learning signature, and CT-reported LN status were independent predictors. The prediction model developed by all the independent predictors showed good discrimination (C-index, 0.820; 95% CI, 0.762 to 0.879) and calibration (Hosmer-Lemeshow test, P =0.193) capabilities for the training cohort. Additionally, the model achieved satisfactory discrimination (C-index, 0.861; 95% CI, 0.769 to 0.954) and calibration (Hosmer-Lemeshow test, P =0.775) when applied to the external validation cohort. An analysis of the decision curve showed that the nomogram had potential for clinical application. Conclusions This study presents a prediction model based on radiomics signature, deep learning signature, and CT-reported LN status that can be used to predict preoperative LN metastasis in patients with LUAD.

Abstract:Background: Accurate detection of axillary lymph node (ALN) metastases in breast cancer is crucial for clinical staging and treatment planning. This study aims to develop a deep learning model using clinical implication-applied preprocessed computed tomography (CT) images to enhance the prediction of ALN metastasis in breast cancer patients. Methods: A total of 1128 axial CT images of ALN (538 malignant and 590 benign lymph nodes) were collected from 523 breast cancer patients who underwent preoperative CT scans between January 2012 and July 2022 at Hallym University Medical Center. To develop an optimal deep learning model for distinguishing metastatic ALN from benign ALN, a CT image preprocessing protocol with clinical implications and two different cropping methods (fixed size crop [FSC] method and adjustable square crop [ASC] method) were employed. The images were analyzed using three different convolutional neural network (CNN) architectures (ResNet, DenseNet, and EfficientNet). Ensemble methods involving and combining the selection of the two best-performing CNN architectures from each cropping method were applied to generate the final result. Results: For the two different cropping methods, DenseNet consistently outperformed ResNet and EfficientNet. The area under the receiver operating characteristic curve (AUROC) for DenseNet, using the FSC and ASC methods, was 0.934 and 0.939, respectively. The ensemble model, which combines the performance of the DenseNet121 architecture for both cropping methods, delivered outstanding results with an AUROC of 0.968, an accuracy of 0.938, a sensitivity of 0.980, and a specificity of 0.903. Furthermore, distinct trends observed in gradient-weighted class activation mapping images with the two cropping methods suggest that our deep learning model not only evaluates the lymph node itself, but also distinguishes subtler changes in lymph node margin and adjacent soft tissue, which often elude human interpretation. Conclusions: This research demonstrates the promising performance of a deep learning model in accurately detecting malignant ALNs in breast cancer patients using CT images. The integration of clinical considerations into image processing and the utilization of ensemble methods further improved diagnostic precision.
Deep learning assisted contrast-enhanced CT–based diagnosis of cervical lymph node metastasis of oral cancer: a retrospective study of 1466 cases

Abstract:Abstract Objectives Lymph node (LN) metastasis is a common cause of recurrence in oral cancer; however, the accuracy of distinguishing positive and negative LNs is not ideal. Here, we aimed to develop a deep learning model that can identify, locate, and distinguish LNs in contrast-enhanced CT (CECT) images with a higher accuracy. Methods The preoperative CECT images and corresponding postoperative pathological diagnoses of 1466 patients with oral cancer from our hospital were retrospectively collected. In stage I, full-layer images (five common anatomical structures) were labeled; in stage II, negative and positive LNs were separately labeled. The stage I model was innovatively employed for stage II training to improve accuracy with the idea of transfer learning (TL). The Mask R-CNN instance segmentation framework was selected for model construction and training. The accuracy of the model was compared with that of human observers. Results A total of 5412 images and 5601 images were labeled in stage I and II, respectively. The stage I model achieved an excellent segmentation effect in the test set (AP 50 -0.7249). The positive LN accuracy of the stage II TL model was similar to that of the radiologist and much higher than that of the surgeons and students (0.7042 vs. 0.7647 ( p = 0.243), 0.4216 ( p < 0.001), and 0.3629 ( p < 0.001)). The clinical accuracy of the model was highest (0.8509 vs. 0.8000, 0.5500, 0.4500, and 0.6658 of the Radiology Department). Conclusions The model was constructed using a deep neural network and had high accuracy in LN localization and metastasis discrimination, which could contribute to accurate diagnosis and customized treatment planning. A Deep Multi-Task Network to Learn Tumor Pathological Representations for Lymph Node Metastasis Prediction.

Abstract:Lymph node metastasis is of paramount importance for patient treatment decision-making, prognosis evaluation, and clinical trial enrollment. However, accurate preoperative diagnosis remains challenging. In this study, we proposed a multi-task network to learn the primary tumor pathological features using the pT stage prediction task and leverage these features to facilitate lymph node metastasis prediction. We conducted experiments using electronic medical record data from 681 patients with non-small cell lung cancer. The proposed method achieved a 0.768 area under the receiver operating characteristic curve (AUC) value with a 0.073 standard deviation (SD) and a 0.448 average precision (AP) value with a 0.113 SD for lymph node metastasis prediction, which significantly outperformed the baseline models. Based on the results, we can conclude that the proposed multi-task method can effectively learn representations about tumor pathological conditions to support lymph node metastasis prediction.

Abstract:Objectives: Distinction of malignant pulmonary nodules from the benign ones based on computed tomography (CT) images can be time-consuming but significant in routine clinical management. The advent of artificial intelligence (AI) has provided an opportunity to improve the accuracy of cancer risk prediction. Methods: A total of 8950 detected pulmonary nodules with complete pathological results were retrospectively enrolled. The different radiological manifestations were identified mainly as various nodules densities and morphological features. Then, these nodules were classified into benign and malignant groups, both of which were subdivided into finer specific pathological types. Here, we proposed a deep convolutional neural network for the assessment of lung nodules named DeepLN to identify the radiological features and predict the pathologic subtypes of pulmonary nodules. Results: In terms of density, the area under the receiver operating characteristic curves (AUCs) of DeepLN were 0.9707 (95% confidence interval, CI: 0.9645-0.9765), 0.7789 (95%CI: 0.7569-0.7995), and 0.8950 (95%CI: 0.8822-0.9088) for the pure-ground glass opacity (pGGO), mixed-ground glass opacity (mGGO) and solid nodules. As for the morphological features, the AUCs were 0.8347 (95%CI: 0.8193-0.8499) and 0.9074 (95%CI: 0.8834-0.9314) for spiculation and lung cavity respectively. For the identification of malignant nodules, our DeepLN algorithm achieved an AUC of 0.8503 (95%CI: 0.8319-0.8681) in the test set. Pertaining to predicting the pathological subtypes in the test set, the multi-task AUCs were 0.8841 (95%CI: 0.8567-0.9083) for benign tumors, 0.8265 (95%CI: 0.8004-0.8499) for inflammation, and 0.8022 (95%CI: 0.7616-0.8445) for other benign ones, while AUCs were 0.8675 (95%CI: 0.8525-0.8813) for lung adenocarcinoma (LUAD), 0.8792 (95%CI: 0.8640-0.8950) for squamous cell carcinoma (LUSC), 0.7404 (95%CI: 0.7031-0.7782) for other malignant ones respectively in the malignant group. Conclusions: The DeepLN based on deep learning algorithm represented a competitive performance to predict the imaging characteristics, malignancy and pathologic subtypes on the basis of non-invasive CT images, and thus had great possibility to be utilized in the routine clinical workflow.

Abstract:Background Lymph node metastasis (LNM) plays a crucial role in the management of lung cancer; however, the ability of chest computed tomography (CT) imaging to detect LNM status is limited. Purpose This study aimed to develop and validate a vision transformer‐based deep transfer learning nomogram for predicting LNM in lung adenocarcinoma patients using preoperative unenhanced chest CT imaging. Methods This study included 528 patients with lung adenocarcinoma who were randomly divided into training and validation cohorts at a 7:3 ratio. The pretrained vision transformer (ViT) was utilized to extract deep transfer learning (DTL) feature, and logistic regression was employed to construct a ViT‐based DTL model. Subsequently, the model was compared with six classical convolutional neural network (CNN) models. Finally, the ViT‐based DTL signature was combined with independent clinical predictors to construct a ViT‐based deep transfer learning nomogram (DTLN). Results The ViT‐based DTL model showed good performance, with an area under the curve (AUC) of 0.821 (95% CI, 0.775–0.867) in the training cohort and 0.825 (95% CI, 0.758–0.891) in the validation cohort. The ViT‐based DTL model demonstrated comparable performance to classical CNN models in predicting LNM, and the ViT‐based DTL signature was then used to construct ViT‐based DTLN with independent clinical predictors such as tumor maximum diameter, location, and density. Abstract:Objectives: Lymph node metastasis (LNM) is a pivotal determinant that influences the treatment strategies and prognosis for oropharyngeal squamous cell carcinoma (OPSCC) patients. This study aims to establish and verify a deep learning (DL) radiomics model for the prediction of LNM in OPSCCs using contrast-enhanced computed tomography (CECT). Methods: A retrospective analysis included 279 OPSCC patients from three institutions. CECT images were used for handcrafted (HCR) and DL feature extraction. Dimensionality reduction for HCR features used recursive feature elimination and least absolute shrinkage and selection operator algorithms, whereas DL feature dimensionality reduction used variance-threshold and recursive feature elimination algorithms. Radiomics signatures were constructed using support vector machine, decision tree, random forest, k-nearest neighbor, gaussian naive bayes classifiers and light gradient boosting machine. A combined model was then constructed using the screened DL, HCR, and clinical features. The area under the receiver operating characteristic curve (AUC) served to quantify the model's performance, and calibration curves were utilized to assess its calibration. Results: The combined model exhibited robust performance, achieving AUC values of 0.909 (95% CI: 0.861-0.957) in the training cohort, 0.884 (95% CI: 0.800-0.968) in the internal validation cohort, and 0.865 (95% CI: 0.791-0.939) in the external validation cohort. It outperformed both the clinical model and best-performing radiomics model. Moreover, calibration was deemed satisfactory. Conclusions: The combined model based on CECT demonstrates the potential to predict LNM in OPSCCs preoperatively, offering a valuable tool for more precise and tailored treatment strategies. Advances in knowledge: This study presents a novel combined model integrating clinical factors with deep learning radiomics, significantly enhancing preoperative LNM prediction in OPSCC.
