Abstract:The objectives of this study were to assess the adequacy of physical parameters/factors (temperature, relative humidity, noise, and illuminance levels) of the work environment in PHC facilities, to evaluate the association between the adequacy of these measured physical parameters and the physical characteristics of the PHC facilities and their surroundings and to assess the association between health professionals' perceptions about exposure to physical risks in the PHC work environment and the adequacy of physical parameters measured in the same facilities. The study monitored 23 PHC facilities in southern Brazil and interviewed 210 health professionals. Data analysis involved Pearson's chi-square, Fisher's exact test, Spearman's correlation, and multivariate linear regression analysis was used to control for confounding factors. The significance level was set at 5% (p ≤ 0.05). The combination of temperature and relative humidity presented thermal comfort levels outside the adopted criteria for adequacy in consultation (outdoor relative humidity, p = 0.013) and procedure rooms (front door open, p = 0.034). Inadequate sound comfort (noise) levels in the morning shift were found in the vaccination (front door open, p = 0.021) and consultation rooms (movement of people, p = 0.016). In PHC facilities where reception rooms had insufficient lighting, internal curtains were opened less frequently (p = 0.047). The analysis of health professionals' perceptions of physical factors demonstrated that physicians more frequently perceive the physical risk of temperature and humidity (p = 0.044). The higher the number of nurses (p = 0.004) and oral health technicians in the PHC facilities (p = 0.031), the greater the general percentage of adequacy of monitored physical parameters. It was also confirmed that the higher the perception of moderate or severe physical risk among health professionals, the lower the general percentage of the adequacy of the physical parameters of the work environment of the PHC facilities evaluated (rs = -0.450, p = 0.031). This study's evidence contributes to a better understanding of physical conditions and future occupational interventions to ensure the comfort, safety, and well-being of PHC workers.

Abstract:INTRODUCTION: The increased prevalence of mental health symptoms in primary care workers in recent years is a major cause of concern, and highlights the need to identify modifiable risk factors for mental health disorders in this population.OBJECTIVES: To analyze the prevalence of common mental disorders and associated factors in primary care workers in the city of Diamantina, state of Minas Gerais.METHODS: This was a cross-sectional, exploratory study involving 203 workers in different sectors of primary health care. Participants completed the Self-Report Questionnaire (20 item version), the Job Stress Scale (JSS), and a form with questions regarding sociodemographic factors, occupational characteristics, lifestyle and habits.RESULTS: The prevalence of common mental disorders in the sample was 20.2%. These disorders were significantly associated (p ≤ 0.05) with younger age, poor self-assessed health, poor sleep quality and physically demanding work.CONCLUSIONS: Poor working conditions may directly and indirectly affect the occurrence of mental health disorders by influencing lifestyle and habits. The restructuring of organizational norms, together with worker support policies, may positively contribute to the mental health of workers.

Abstract:Introduction: Community health workers play a prominent role in the primary care context in Brazil. Burnout syndrome is an important work-related condition whose consequences affect job satisfaction. Objectives: To evaluate the extent of burnout syndrome and job satisfaction among community health workers in a city in southern Santa Catarina. Methods: This analytical, individual, cross-sectional study was conducted in Family Health Units. The participants were selected through random probability sampling, with a sampling error of 10%. After sociodemographic data collection, the Maslach Burnout Inventory and a job satisfaction scale were applied. Results: All 66 included workers were women. According to the analyses, there was a moderate positive correlation between satisfaction with colleagues and satisfaction with supervisors. There were weak positive correlations between emotional exhaustion and depersonalization and between professional fulfillment and satisfaction with promotions. However, there was a weak negative correlation between emotional exhaustion and both professional fulfillment and satisfaction with salary. Conclusions: The results indicate that the conditions of these workers are sufficiently satisfactory to deal with the demands of the job. Nevertheless, there was substantial dissatisfaction with salary, which can be a demotivator and trigger work-related depression.
Social determinants and work-related musculoskeletal disorders in Brazil

Abstract:This study aimed to analyze the prevalence of work-related musculoskeletal disorders (WMSD) and their association with individual and contextual factors in the Brazilian population. This quantitative cross-sectional study used secondary data from the Brazilian National Health Survey from August 2013 to February 2014. The dependent variable included WMSD, and independent variables were analyzed as individual and contextual factors. WMSD was mostly prevalent in females, individuals aged 43 to 59 years, with chronic physical or mental disorders, reporting frequent sleep disorders, and performing integrative and complementary health practices, physical exercise or sports, and heavy physical activity or housework. Regarding contextual factors, high social classes and proportion of individuals with formal work were associated with a high prevalence of WMSD, whereas a high Gini index was associated with a low prevalence. Thus, a high prevalence of WMSD in the Brazilian population was associated with individual and contextual factors, which should be the target of health professionals for actions of promotion, prevention, and intervention at individual or collective care levels.

Abstract:The working environment is a crucial aspect to considerfor guaranteeing a sustainable life. However, workers are exposed to various health risks daily, namely, psychological risks. These risks can be due to imbalances on the part of the workers themselves or to organisational and inter-functional risk factors arising from interactions within companies and the challenges of professional responsibilities. Over the past 20 years, the Copenhagen Psychosocial Questionnaire (COPSOQ) has become one of the most prominent tools for assessing psychological and social factors at work. This study aimed to present, discuss, and evaluate aspects of the cultural adaptation and preliminary psychometric validation of the short version of COPSOQ III for a Portuguese sample. For this purpose, we used data from 361 participants employed in the industrial (30.7%) and services (69.3%) sectors across various regions of Portugal. A third-order confirmatory factor analysis (CFA) was performed using AMOS, revealing an acceptable fit. The results also demonstrate the robustness of the measurement model, confirming its reliability and validity. In light of some limitations of this preliminary study, directions for future research are proposed. Thus, a tool for assessing psychosocial risks is disseminated, making it possible to achieve more sustainable working environments where the operator's health and well-being are prioritised.

Abstract:The self-perception of the individual in relation to their health, work, and lifestyle is considered as work ability, a concept of various dimensions, characterizing a multidimensional and versatile construct, because it encompasses physical, mental, and social prerequisites. This study aims to identify the factors associated with work capacity among primary health care professionals. An integrative review was conducted from March to June 2020, following this eligibility criteria: studies in Portuguese, Spanish, and English; from 1996, available on PubMed, SciELO, LILACS, and Cochrane databases; longitudinal studies, clinical trials, and crosssectional studies; among primary health care professionals; which used the Work Ability Index. Three articles were found, two in English and one in Portuguese. It was concluded that further studies should be conducted, with primary health care professionals, in order to propose measures that can reduce inadequate capacity rates and better understand the variables associated with work ability.

Abstract:Background The Work Ability Index (WAI) assesses work demands, health status, and physical and mental resources. Objectives To evaluate the measurement properties of the WAI, single-item work ability score (WAS), and short version for online applications. Methods One hundred three workers completed the 3 versions of the WAI and the Perceived Stress Scale (PSS-10) questionnaire. The reproducibility and construct validity of the 3 versions of the WAI and their concurrent validity with the PSS-10 were tested. Statistical tests were performed with IBM® SPSS 28.0. Results The test-retest reliability of the WAI, WAS, and WAI short version ranged from good to excellent. The WAI and WAI short version showed excellent internal consistency. The construct validity of the WAS was moderate and positive and it was excellent and positive for the WAI short version. Concurrent validity of the WAI, WAS, and WAI short version was moderate and negative, and item mental resources were strong and negative. Factor analysis reached an acceptable level for all indexes (≥ 0.90). The analysis with two factors reached an acceptable level for all indexes (≥ 0.90) and was the best model. Conclusion The reliability and construct and concurrent validity of the various versions of the WAI were strong to excellent in online applications. Confirmatory factor analysis demonstrated a new grouping of items for the WAI Brazilian version. However, it is necessary to be careful in the choice due to the distribution model of items in health domains and aspects of work.

Work overload and associated factors in healthcare professionals during the COVID-19 pandemic

Abstract:Introduction: The COVID-19 pandemic changed the work routine of professionals at the family healthcare center (Núcleo de Atenção à Saúde da Família - NASF-AB), providing new conditions and work overload. Objective: The purpose of this study was to explore factors associated with work overload in NASF-AB professionals during the COVID-19 pandemic. Methods: A cross-sectional study was carried out with NASF-AB workers in the city of Salvador, Bahia, Brazil, from May to August 2021. Working conditions and overload were assessed using the National Program for Improving Access and the Quality of Primary Care questionnaire (Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica - PMAQ) and the scale measuring burden of professionals in mental health services (Escala de Avaliação da Sobrecarga de Profissionais em Serviços de Saúde Mental - IMPACTO-BR), respectively. A multivariate linear regression model was used. A total of 68 health professionals participated in the study, including 19 physiotherapists, 13 occupational therapists, 10 social workers, 10 nutritionists, 9 psychologists and 7 physical education professionals. Results: There was a significant association between overall overload and being female (p=<0.005), having spaces for reflection on the work process (p=0.027), and having difficulty moving around to conduct activities in the territory (p=0.002) for increasing the chance of work overload. Conclusions: Our findings encourage workers' health policies and closer ties and negotiation with local management, as well as the return of the institutional support figure for the effectiveness and resolution of actions in primary healthcare units.
WORK ABILITY PREDICTS OCCUPATIONAL HEALTH-RELATED ABSENTEEISM IN PROFESSIONAL DRIVERS: A 1-YEAR LONGITUDINAL STUDY

Abstract:Background Work ability (WA) is considered the result of the interaction of personal, social, and other factors related to the work environment. To what extent WA can predict absenteeism at work due to occupational health in professional drivers remains poorly investigated. Objectives To analyze the association between work ability and occupational health-related absenteeism at work in professional drivers. Methods From 2020 to 2021, 449 professional drivers were assessed for sociodemographic information, lifestyle (Baecke questionnaire, Work Stress Scale), and work ability (Work Ability Index, WAI) in Curitiba, Paraná (Brazil). Follow-up assessments were conducted at 6 and 12 months by telephone to answer 3 questions that aimed at information about professional performance, situations of occupational absenteeism, and accidents at work. Results After 12 months of the initial interview, 270/449 drivers (60%) remained as research participants and the others did not remain active in the profession (n=29/449, 7%) or did not respond to telephone contact (n= 53/449, 12%). WAI was inversely associated (β=-0.119 CI 95% -0.233 to -0.006, P=0.039) with general absenteeism at 12 months, explaining better WAI variability and showing a better fit of the latter model (AIC=137, R²= 0.028, P=0.074 vs. R² = 0.050, AIC = 92, P=0.039). Separating the reasons for absenteeism between occupational health and accidents at work, we concluded that WAI was inversely associated with absenteeism due to occupational health at 6 months ((β=-0.096 CI95% -0.187 to -0.006, P=0.037) and accidents at work at 12 months (β=-0.189 95% CI -0.331 to -0.047, P=0.009). Conclusion Work ability can predict 1-year absenteeism due to occupational health in professional drivers. Implications Absenteeism due to occupational health can be detected using current work ability information in professional drivers. Further studies are required to test whether ergonomic interventions can promote occupational health and decrease absenteeism in this population.

Abstract:This study analyzed factors associated with the quality of life (QoL) of prison officers (POs) in the Midwest Region of Brazil. POs in five penitentiary units participated in this cross-sectional study. Sociodemographic data were obtained through face to face interview and a World Health Organization Quality of Life abbreviated version (WHOQOL-BREF) questionnaire was applied to assess QoL. Student’s t-test or ANOVA were used for bivariate analysis and multiple linear regression was applied for adjusted analysis. The domains used for outcomes were: physical, psychological, social relations, and the environment. The lowest score among WHOQOL-BREF domains was environment (59.9; 95%CI 58.0–61.5). After adjustment, the factors associated with the physical domain were ‘female sex’ and ‘no history of workplace PO-PO violence’; factors associated with the psychological domain were ‘female sex’, ‘without spouse’, and ‘no history of inmate-PO violence’; factors associated with the social relationships domain were ‘female sex’, ‘work experience in years’, ‘no higher education’, ‘no private health insurance’, and ‘no history of inmate-PO violence’; and factors associated with environment domain were ‘female sex’, ‘work experience in years’, ‘no private health insurance’, and ‘no history of PO-PO violence’. This study showed that female workers and those with a history of violence at work had worse QoL scores. This investigation highlights the importance of prison management in promoting QoL of POs, as well as support and development of strategies to prevent workplace violence.

An evaluation of the contribution of the Swedish International Development Authority (SIDA) to leprosy control in India based on the implementation of multiple drug therapy (MDT) 1981-1993.

Abstract:The Swedish International Development Authority (SIDA) first supported the National Leprosy Control Programme in India in 1978. In 1981/82 priority was given to the implementation of multiple drug therapy (MDT), starting in two high-endemic districts, and gradually extending to a total of 19 districts in the years by 1993. SIDA then decided to undertake a detailed evaluation of its 12-year contribution and this was carried out by an international team between November 1993 and April 1994. In terms of epidemiological and public health impact, the main results were impressive and clear-cut; 837,519 cases (old and newly arising) were successfully treated, with few complications and a low rate of relapse. The voluntary reporting rate had improved significantly. Data relating to new case detection, child and disability rates were, however, less clear and difficult to interpret. Deficiencies were also identified in the areas of health education, community participation, gender issues, disability prevention and management, rehabilitation, operational research and assessment of cost-effectiveness. These problems should not, however, detract from the contribution of SIDA, from 1981 onwards, in establishing the implementation of MDT in two 'pilot' districts at an early and important stage in the history of the MDT programme in India. SIDA also made significant contributions in other areas, namely pre-MDT 'screening' of registers in 45 endemic districts in 1990-1993, appointment of consultant leprologists at district level, group education activities, annual meetings of voluntary agencies and the development of a monitoring and information system, with computer facilities, at national level. This paper describes the design and methodology, main findings and conclusions of the evaluation, based on the final report and the appendices submitted to SIDA in Stockholm in April 1994.
Mental disorders and related factors in higher education professors working remotely: a correlational study

Abstract:Introduction: Recent changes in labor relations and the employment market in university education have posed significant challenges and increased workloads for faculty a situation that has been exacerbated by the global epidemiological status in 2020. Objectives: To investigate the presence of mental disorders and associated factors in faculty members at a private university in São Paulo, Brazil. Methods: This is a quantitative, descriptive, exploratory cross-sectional study conducted in June 2021, using online questionnaires to collect social and demographic and lifestyle data, namely, Self-Reporting Questionnaire (SRQ-20) and the Patient Health Questionnaire (PHQ-9). The presence of associations between variables was investigated using odds ratio and relative risk. Results: Participants included 49 professors, mostly women, aged between 29-78. Among the subjects, 61.2% had a body mass index above the recommended level and 40.8% were considered sedentary. A chronic illness was reported by 40.8% and 65.3% reported continued-use medication. A common mental disorder was reported by 28.5%, and the same prevalence was found for depression, with a positive association with being considered sedentary, overweight or obese, drinking alcohol at least once a week, and having worked as a professor for less than 5 years. Conclusions: The study showed a significant level of emotional distress among professors, associated with lifestyle habits that can compromise their quality of life and professional performance.
[Clinical value of 18F-FDG-PET and 11C-MET-PET in patients with plasma cell malignancy].

Abstract:FDG-PET/CT is useful for the imaging of various malignancies including plasma cell malignancy. In multiple myeloma, FDG-PET has recently been used not only for staging but also for prediction of prognosis. FDG is a widely accepted tracer based on high glucose metabolism in malignant cells. However, myeloma cells are sometimes not avid for FDG. A radiolabelled tracer of 11C-methionine is abundantly incorporated into cells with high amino acid metabolism. Therefore, it is reasonable that 11C-methionine-PET is useful in myeloma, since myeloma cells usually produce a large amount of immunoglobulins. In this chapter, we explained of the features of 18F-FDG-PET and 11C-methionine-PET. Moreover, we demonstrated two case reports suggestive of the clinical advantage of 11C-methionine-PET in myeloma.
Quality of life and associated factors for community health workers in the context of the COVID-19 pandemic in northeastern Brazil

Abstract:In this study, we evaluated the quality of life (QoL) and associated factors of community health workers (CHWs) in different urban settings as a contributor to the preparedness of Brazilian primary care for future sanitary emergencies. The sample included 1935 CHWs from 4 state capitals and 4 inland municipalities in northeastern Brazil. Information was collected on QoL (WHOQOL-BREF), work routines, sociodemographics, direct and indirect exposure to violence, general self-efficacy, social support (MSPSS), mental health (SRQ-20) and coronavirus anxiety. The data were subjected to multiple linear regression analysis (α = 5%). In the state capitals, the factors associated with loss of QoL were poor mental health, lack of training, uncertainty about occupational biosafety, and lack of adaptation of services to tend to patients diagnosed with COVID-19. Among the inland municipalities, the main factors were coronavirus anxiety, poor mental health, lack of adaptation of services, lack of training, and lack of personal protective equipment (PPE). A high MSPSS (family and friends) score and self-efficacy had positive impacts on QoL in both urban settings. Our results highlight the need for investment in permanent education, PPE, social support, and mental health care for CHWs.

Association of poor perceived work ability and psychosocial work-related factors in health and social service worker age groups: a cross-sectional study

Abstract:Objective: Previous work ability studies have primarily focused on old workers and physical health. This study investigated how poor perceived work ability (PPWA) is associated with work-related factors in different health and social service (HSS) worker age groups. Design: Cross-sectional survey in 2020. Setting: HSS employees (general HSS and eldercare) in nine Finnish public sector organisations. Participants: All employees who were employed in the organisation completed self-reported questionnaires. Of the original sample (N=24 459, response rate 67%), 22 528 gave consent for research use. Primary and secondary outcome measures: Participants evaluated their psychosocial work environment and work ability. Lowest decile of work ability was categorised as poor. The association between psychosocial work-related factors and PPWA in different age-groups of HSS workers, adjusting for perceived health, was analysed with logistic regression. Results: The proportion of PPWA was highest in shift workers, eldercare employees, practical nurses and registered nurses. Considerable variation between age groups exists in the work-related psychosocial factors associated with PPWA. Among young employees engaging leadership and working time and work task autonomy were statistically significant, whereas in middle-aged and old employees procedural justice and ethical strain were highlighted. The strength of the association with perceived health also differs in age groups (young: OR=3.77, 95% CI 3.30 to 4.30; middle-aged: OR=4.66, 95% CI 4.22 to 5.14; old: OR=6.16, 95% CI 5.20 to 7.18). Conclusions: Young employees would benefit from engaging leadership and mentoring, and from more working time and work task autonomy. As employees get older they would benefit more from job modification and from ethical and just organisation culture.

In healthcare services, this can also affect patients.OBJECTIVE: To assess the prevalence of and factors associated with depressive symptoms, burnout, job satisfaction and patient safety culture and the relationships between these constructs, among healthcare workers.DESIGN AND SETTING: Cross-sectional study in a university hospital in Manaus, Brazil.METHODS: Randomly selected workers were interviewed based on Brazilian-validated tools. We calculated the prevalence ratio (PR) and 95% confidence interval (CI) of depressive symptoms and burnout using Poisson regression with robust variance; and the β-coefficient of safety culture and job satisfaction using linear regression. Outcome relationships were assessed using partial least-squares structural equation modeling.RESULTS: 300 professionals were included; 67.3% were women. The prevalence of depressive symptom was 19.0% (95% CI: 14.5; 23.5%) and burnout, 8.7% (95% CI: 5.2; 12.3%). Lack of work stability increased depression (PR = 1.88; 95% CI: 1.17; 3.01) and burnout (PR = 2.17; 95% CI: 1.03; 4.57); and reduced job satisfaction (β = -11.93; 95% CI: -18.79; -5.07). Depressive symptoms and burnout were positively correlated, as also were job satisfaction and safety culture (P < 0.001); job satisfaction was negatively correlated with burnout (P < 0.001) and depression (P = 0.035).CONCLUSION: Impermanent employment contracts increased depression and burnout and reduced job satisfaction. Job satisfaction reduced poor mental health outcomes and increased safety culture. Job satisfaction and safety culture were directly proportional (one construct increased the other and vice versa), as also were depression and burnout. Better working conditions can provide a virtuous cycle of patient safety and occupational health.

Abstract:The Work Ability Index (WAI) is the most commonly used tool for evaluating work capacity. Self-assessments made by workers can be influenced by various occupational and emotional factors. We wanted to study the association of work-related factors, such as work annoyance, stress, overcommitment, job satisfaction, social support, and emotional factors, such as anxiety, depression, and happiness, with work ability, in a sample of 490 healthcare workers from an Italian public health company. A principal component analysis indicated the presence of two components of the WAI questionnaire; the first expresses "subjectively estimated work ability" (SEWA), and the second refers to "ill-health-related work ability" (IHRWA). Using stepwise multiple hierarchical linear regression, we identified the factors that best predicted the total score on the WAI and on the two components. The total score was negatively predicted by anxiety, depression, a lack of happiness, low job satisfaction, overcommitment, and work annoyance. Age, being female, anxiety, and occupational stress were associated with a reduction in the IHRWA component score, while overcommitment, work annoyance, a lack of social support, depression, and a lack of happiness were negatively associated with the SEWA component. These results can help interpret those of epidemiological studies and provide guidance on ways to improve work ability.

Abstract:Background: Being a victim support worker (VSW) involves exposure to victims' suffering, pain, and traumatic events, which may trigger the risk of VSWs developing mental health problems. Psychosocial risks (PSR) and work-related stress are considered the most challenging issues in occupational safety and health, considering they impact individuals, organizations, and economies. Methods: The purpose of the present study was to identify the PSR in a sample of 196 Portuguese victim support workers (VSW) (Mean age = 36.49; SD = 10.52). A questionnaire with socio-demographic characteristics, variables related to VSW's job, and the Portuguese medium version of the Copenhagen Psychosocial Questionnaire II (COPSOQ II) were used to assess these professionals' perception of PSR factors. Results: The results reveal that although VSW recognizes some psychosocial factors favourable to their health and well-being, they also identify some PSR that place them at intermediate and severe risk, i.e., emotional and cognitive demands, which are the main areas of risk to the VSW. VSW over 38 years old scored higher in job insecurity, burnout, and offensive behaviours. Conclusions: These findings give important insights into the areas that must be enhanced in this context involving VSW. Additionally, the results highlight the relevance of encouraging a healthy and supportive work environment, preventing and promoting the health and well-being of VSW, particularly when considering the coronavirus disease (COVID-19) pandemic.
