Abstract:During the reign of Louis XIV, anatomical demonstrations became a public attraction in Paris. At the Jardin du Roi, the star performer was Joseph-Guichard Duverney, who attracted hundreds to his anatomy lectures. Simultaneously, Duverney also instructed the Dauphin and his courtiers, lectured to medical students at the Hôtel-Dieu hospital (making off with corpses in the process) and dissected before the Paris Academy of Sciences. Duverney's dramatic, rhetorical and anatomical skills made him the best-known man of science in Louis XIV's Paris.
Human cadaveric dissection: a historical account from ancient Greece to the modern era

Abstract:The review article attempts to focus on the practice of human cadaveric dissection during its inception in ancient Greece in 3rd century BC, revival in medieval Italy at the beginning of 14th century and subsequent evolution in Europe and the United States of America over the centuries. The article highlights on the gradual change in attitude of religious authorities towards human dissection, the shift in the practice of human dissection being performed by barber surgeons to the anatomist himself dissecting the human body and the enactment of prominent legislations which proved to be crucial milestones during the course of the history of human cadaveric dissection. It particularly emphasizes on the different means of procuring human bodies which changed over the centuries in accordance with the increasing demand due to the rise in popularity of human dissection as a tool for teaching anatomy. Finally, it documents the rise of body donation programs as the source of human cadavers for anatomical dissection from the second half of the 20th century. Presently innovative measures are being introduced within the body donation programs by medical schools across the world to sensitize medical students such that they maintain a respectful, compassionate and empathetic attitude towards the human cadaver while dissecting the same. Human dissection is indispensable for a sound knowledge in anatomy which can ensure safe as well as efficient clinical practice and the human dissection lab could possibly be the ideal place to cultivate humanistic qualities among future physicians in the 21st century.

Abstract:www.PRSJournal.com 466e T authors should be congratulated on this thoughtful and organized approach to robotic surgical innovation. They effectively make the argument for transoral robotic surgery in cleft surgery and, in the process, create a model to methodically learn, teach, and study this technique. I happened to be a plastic surgery resident at Penn in 2003 when Neil Hockstein, then an ear, nose, and throat resident, first conceived of transoral robotic surgery.1 I watched as professors Weinstein and O’Malley developed these techniques and brought them into clinical practice, receiving U.S. Food and Drug Administration approval in 2009.2,3 Witnessing this process sparked my interest in robotic surgery and led me to investigate the indications for robotics in plastic surgery.4 This simple coincidence was a seminal moment in my career. I have since gone on to develop techniques and indications for robotics across a range of adult reconstructive plastic surgery.5–9 My work in transoral robotic surgery has continued in adult cancer reconstruction, where many of the principles discussed in this article apply. However, having done my share of cleft surgery in training, I always felt that the best fit for transoral robotic surgery was not in cancer, but in pediatric cleft and velopharyngeal repair. This article and the work it represents does long overdue justice to this indication. The benefit of transoral robotic surgery is that oropharyngeal anatomy is confined in threedimensional space. The small, slender arms with wrist articulation at the instrument tips; tremor elimination; motion scaling; and high fidelity, three-dimensional visualization make the robot a very attractive platform for use in confined spaces with small surgical targets. There are few places where precision, visualization, and space restriction play a bigger role than in cleft surgery, and for this reason, robotics is a natural fit. The use of the operating microscope in cleft surgery was a half-measure in this direction, but it only addresses visualization, and arguably further compromises space constraints by getting in the way. The elegance of the robotic solution is that it addresses visualization, space constraints, and demands for precision all at once. The authors also deserve credit for surgical innovation in the pediatric population. The world is especially (appropriately) sensitive to patient safety issues in children. Children are small and fragile, and the robot is large and scary—it is no wonder cleft surgeons have been slow to proceed. The authors have gone a long way in overcoming this hurdle by developing a high-fidelity simulator. Not only is this good for teaching conventional techniques, but it allows the steep part of the learning curve to take place outside of the patient. This is a critical element in the success of the safe stewardship of surgical innovation. One important aspect of simulation is its permissive effect on the development of a novel technique. This was the missing piece in the transition of transoral robotic surgery into cleft surgery, and the authors have supplied it. In addition, this article is a step toward competency-based training in both cleft and robotic surgery. Competency-based training is the idea that learners advance based on the quality of the training, rather than the quantity of the training. This is the future of all education, including surgical education.10 A critical component of competencybased training is accurate and valid assessment tools—you cannot measure without a good ruler. Not only do the authors have a high-fidelity model, which is important for instructional learning, but they have deconstructed cleft palate surgery to a series of steps, conducted evaluations, and tested interrater reliability. Art and anatomy in the renaissance: are the lessons still relevant today

Abstract:This review discusses how renaissance artists such as Leonardo and Michelangelo had to undertake anatomical studies of human cadavers in order to understand the anatomy that then informed their artworks, whether they were drawings, paintings or sculpture. Around this time, anatomists, such as Vesalius and Estienne, had to in part become artists or engage with artists and artisans to illustrate their many discoveries. This review tries to portray how this was occurring in a period in history not only when there was a shift-taking place in philosophical and theological thinking about the human condition but also when there was a concurrent revolution in the visual language with the advent of print reproduction. This allowed the creation of essentially the first medical texts, and the wide dissemination of newly acquired knowledge for the advancement of surgery and medicine henceforth. A classic example of where this did not align is Leonardo de Vinci many of whose original works were hidden for 150–400 years. This review also describes how learning anatomy and artistic endeavours still have a mutually beneficial relationship in the modern world—a second 'Renaissance'. Examples are given such as body painting, exposure of art students human cadavers resources and lastly there is consideration of how modern anatomy relies on many new technologies that allow students and practitioners to 'dissect' in a virtual sense and with the advent of a new visual language, that is, 3D printing, to create novel artforms of educational significance.

From the most important and time-consuming component of the preclinical program, anatomy education has reduced in size and status, and yielded in curricular space to accommodate other disciplines and topics. Meanwhile, radiology has become more prominent, as a means to visualize anatomy, not only in clinical care but also in education. For this perspective paper, the authors, all with backgrounds in anatomy, radiology and/or medical education, conducted structured conversations with several academic colleagues with similar backgrounds, reviewed pertinent literature and analyzed the causes of the historical decline of a knowledge domain of medical education, that nevertheless is widely considered essential for medical students and graduates. After this analysis, the authors propose four ways forward. These directions include systematic peer teaching and development of anatomy education as a scholarly domain, further vertical integration with postgraduate medical education, full integration with radiology education, and capitalizing on educational technology. Schools in several industrialized countries have made steps in these directions, which can be further strengthened. In less affluent countries, and in countries with curricula strongly determined by tradition, these steps are less easy to make. To respond to changes in global health and health care, combined with the inevitable technological progress, and international mobility, we believe all schools will move in these directions, slower or faster. Keywords: anatomy education, radiology education, vertical integration, technology, future Since Vesalius (1514–1564), the founder of accurate anatomy texts and drawings, and arguably the father of current anatomy teaching, medical education in the universities has had a strong emphasis on anatomy, if not being its core pillar for centuries. Around the turn of the previous century, two to three years of the preclinical curriculum devoted to morphology (anatomy, histology, and cytology) was considered time well spent, and around 1960, Dutch curricula still contained 225 to 572 hours of anatomy teaching. 1 Likewise, in 1901 medical schools in the United States had about 549 hours of anatomy teaching, and by 1955 approximately 330 hours. 2 The latter half of the 20th century showed a continuous decrease in the absolute and relative time devoted to teaching and learning anatomy in terms of curricular hours and in time students spend studying anatomy. While the same century has witnessed many changes in medical curricula, often aligning with Harden's SPICES model (more student-centered, problem-based, integrated, community-based, and systematic), 3 the decrease in time and effort to acquire anatomical content knowledge stands out. In one Dutch curriculum, the absolute time devoted to anatomy (and other basic sciences) decreased by 60% to about 100 hours in the period between 1990 and 2010, 4 in which year Australian/New Zealand medical schools ranged in total hours of anatomy teaching from as low as 56 to as high as 560. 5 A similar process happened in less affluent countries. In Nicaragua, by 2019, the medical curriculum contained 102 hours of anatomy teaching, 6 a reduction of 68% compared to 320 hours 20 years prior. Not all knowledge that, in the past, was considered relevant for medicine and patient care is still current. Some knowledge has become irrelevant, outdated, or discovered as incorrect. Regular adaptations in education are thus needed and justified. While new insights and advances in knowledge of basic and clinical sciences have been continuously added, and clinical training has become more prominent, existing components of the medical curriculum must decrease to make space. The Sun Also Rises: Tracing the evolution of humanistic values in anatomy pedagogy and research, including cadaveric acquisition practices

Abstract:The Cave of the Hands (Cueva de las Manos) in Argentina, where the paintings of multiple human hands are visible. Considered an iconic symbol throughout the planet, it depicts a message from an unknown lost generation to us while symbolising the existence of humanity. This article traces the trajectory of humanism that has shaped anatomy teaching and learning over millennia, and the cave painting provides philosophical strength to the depth and scope of its discourse. Anatomy has always been at the intersection of the socio‐cultural and political landscape, where new ideas constantly replace older wisdom. From ancient Egyptians through the Greeks, and then the Romans, finally culminating into the European Renaissance—all the significant eras of human civilisation have left their insignia and distinct marks on the evolution of anatomical practices. Despite its utility as a tool for anatomy pedagogy and research that has proven its worth over millennia, cadaveric dissection has particularly been subject to political and social vicissitudes. A major debate about anatomical dissection lay with the ethical considerations, or its lack thereof, while acquiring corpses for demonstration in the dissection halls. From antiquity, anatomical dissection—often synonymous with medical studies—had typically been carried out on the dead bodies of executed criminals with certain laws, such as the Murder Act of 1752, facilitating such uses. Gradually, the uses of unclaimed bodies, resourced primarily from the impoverished sections of society, were also introduced. However, these body acquisition protocols often missed the crucial element of humanism and ethical considerations, while knowledge augmentation was taken as sufficient reasoning. Unfortunately, a gross disregard towards humanistic values promulgated heinous and illegal practices in acquiring corpses, including grave robbery and even murders like in the case of Burke and Hare murders of 1828. Follow‐up legislation, such as the Anatomy Act of 1832, and comparable laws in other European nations were passed to curb the vile. What distils from such a historical discourse on humane values in anatomy dissection, or medical science in general, is that the growth and integration of humanism in anatomy have never been linear, but there were intermittent and, yet, significant disruptions in its timeline. For example, there were serious human rights violations in anatomical practices during the Third Reich in Germany that perpetrated the holocaust. The medical community has kept evolving and introducing new moral values and principles while using such egregious events as lessons, ultimately resulting in the Declaration of Helsinki in 1964. This article revisits the heterogeneous journey of integrating humanistic values in anatomy practice. Such humanistic traits that, like medical science, have also developed over centuries through the inputs of physicians, researchers, and philosophers—from Greece to modernity with an important stopgap at the Renaissance—are a fascinating lore that deserves to be re‐envisioned through the lens of contemporary values and ethos. In parallel to human medicine, humanistic values continue to influence veterinary medicine, a welcome development, as our society condemns animal cruelty in any form. There are lessons to be learned from this historical journey of how humanism shaped many of the concepts that anatomists use now. Finally, and most importantly, it might prevent the medical community from repeating the same mistakes by cautioning against the traps that are there, and in a convoluted world where morality as such is eroding from our social fabric, will always be there. Such historical account acts as a righteous, ethical, and contextual compass to guide the existing and upcoming anatomists in discerning between light and dark, right and wrong, and roads—to be or not to be—taken.

Historical roots of modern neurosurgical cadaveric research practices: dissection, preservation, and vascular injection techniques

Abstract:Because of the complexity of the brain and its structures, anatomical knowledge is fundamental in neurosurgery. Anatomical dissection, body preservation, and vascular injection remain essential for training, teaching, and refining surgical techniques. This article explores the historical development of these practices and provides the contextual background of modern neurosurgical cadaveric brain models. Body preservation has ancient beginnings, evident in the Chinchorro mummifications and Egyptian embalming. However, brain preservation techniques for education were scarce until the beginning of the Renaissance in Europe. At the University of Bologna in the 13th century, occasional dissections were performed only in winter because of the lack of preservation techniques. Pope Sixtus IV's 1482 papal bull (official decree) formalized and expanded the use of dissection in medical education, leading to an explosion in anatomical studies. This surge brought advances in body preservation, such as soaking bodies in vinegar and distilled liquors. In subsequent centuries, Andreas Vesalius and Charles Bell advanced brain anatomical techniques and knowledge, combining novel illustrations and instruction. To better understand brain vasculature, Richard Lower developed vascular injection techniques using india ink and spirits of wine, leading to the 1664 description of the circle of Willis by Thomas Willis. In 1868, August Hofmann synthesized formaldehyde, markedly improving tissue preservation. Later, William Kruse introduced latex in 1939, and Sidney Sobin introduced silicone in 1965 for vascular studies. These advancements laid the foundation for modern neurosurgical cadaveric studies, many remaining relevant today.
[On the dawn of human dissections in early modern Japan]

Abstract:The commencement of human dissections in Japan during the Edo period is often depicted as the individual achievement of Yamawaki Tōyō. In 1754, this renowned scholar of the "School of Ancient Prescriptions, " feeling unable to resolve discrepancies in the Chinese classical literature, succeeded in obtaining permission for a dissection and was allocated a corpse at the execution place in Kyoto. However, a closer look at this issue reveals that the paradigm change to gaining knowledge through anatomical studies did not occur suddenly after centuries of stagnation. Beginning with the arrival of western medicine in the mid-17th century, this paper demonstrates how foreign and indigenous medical, social, political and religious stimuli gradually led to a new attitude toward human dissections and a rising awareness of the merits of anatomical observations. Decades before Yamawaki counted the number of human viscera, an ophthalmologist had discovered the faculty of the eye as a means for new insights and bone-setters had begun to revise textual knowledge by observing and manipulating the skeletons of rotting corpses. Yamawaki's accomplishment does not lie in the nature of his dissection or the (quickly outdated) results, but in the sheer fact that he carried out the dissection with the permission of the shogunal authorities and managed to publish his findings. Furthermore, among those physicians who quickly followed his example, we find Kawaguchi Shinnin, whose intellectual and mental breakthrough has not been recognized sufficiently yet. In Kawaguchi's case, there was no searching through the classical literature, no detached reflection and no awe resulting from the knife and the body fluids. The dissection that he conducted in 1770 was an unemotional "clinical" search for new insights by measuring sizes, determining positions, colors and consistencies, and by manipulating and investigating. The paper finishes with a comparison of the basic traits of anatomical illustrations in Japan and Europe.
Cadaveric dissection as an educational tool for anatomical sciences in the 21st century

Abstract:Anatomical education has been undergoing reforms in line with the demands of medical profession. The aim of the present study is to assess the impact of a traditional method like cadaveric dissection in teaching/learning anatomy at present times when medical schools are inclining towards student-centered, integrated, clinical application models. The article undertakes a review of literature and analyzes the observations made therein reflecting on the relevance of cadaveric dissection in anatomical education of 21st century. Despite the advent of modern technology and evolved teaching methods, dissection continues to remain a cornerstone of anatomy curriculum. Medical professionals of all levels believe that dissection enables learning anatomy with relevant clinical correlates. Moreover dissection helps to build discipline independent skills which are essential requirements of modern health care setup. It has been supplemented by other teaching/learning methods due to limited availability of cadavers in some countries. However, in the developing world due to good access to cadavers, dissection based teaching is central to anatomy education till date. Its utility is also reflected in the perception of students who are of the opinion that dissection provides them with a foundation critical to development of clinical skills. Researchers have even suggested that time has come to reinstate dissection as the core method of teaching gross anatomy to ensure safe medical practice. Nevertheless, as dissection alone cannot provide uniform learning experience hence needs to be complemented with other innovative learning methods in the future education model of anatomy. The History of Surgical Education in the United States: Past, Present, and Future

Abstract:In just over 100 years, surgical education in the United States has evolved from a disorganized practice to a refined system esteemed worldwide as one of the premier models for the training of physicians and surgeons. But in the changing environment of health care, new challenges have arisen that could warrant a reform. To design our future, we must understand our past. The present work is not intended to be a comprehensive account of the history of American surgery. Instead, it tells the abridged history of surgical education in our country: the evolution from apprenticeships to residencies; the birth of hospital-based teaching; the impact of key historical events on training; the marks left by some preeminent characters; the conception of regulatory entities that steer our education; and, finally, how our process of training surgeons might need to be refined for the continued progress of our profession. Told in chronological order in a manner that will be memorable to readers, this story weaves together the key events that explain how our current surgical training models came to be. We conclude with a timely invitation to draw from these past lessons to redesign the future of graduate medical education, making a case for the transition to time-variable, competency-based medical education for surgical residency programs in America.
Classic versus millennial medical lab anatomy

Abstract:This study investigated the integration, implementation, and use of cadaver dissection, hospital radiology modalities, surgical tools, and AV technology during a 12-week contemporary anatomy course suggesting a millennial laboratory. The teaching of anatomy has undergone the greatest fluctuation of any of the basic sciences during the past 100 years in order to make room for the meteoric rise in molecular sciences. Classically, anatomy consisted of a 2-year methodical, horizontal, anatomy course; anatomy has now morphed into a 12-week accelerated course in a vertical curriculum, at most institutions. Surface and radiological anatomy is the language for all clinicians regardless of specialty. The objective of this study was to investigate whether integration of full-body dissection anatomy and modern hospital technology, during the anatomy laboratory, could be accomplished in a 12-week anatomy course. Literature search was conducted on anatomy text, journals, and websites regarding contemporary hospital technology integrating multiple image mediums of 37 embalmed cadavers, surgical suite tools and technology, and audio/visual technology. Surgical and radiology professionals were contracted to teach during the anatomy laboratory. Literature search revealed no contemporary studies integrating full-body dissection with hospital technology and behavior. About 37 cadavers were successfully imaged with roentograms, CT, and MRI scans. Students were in favor of the dynamic laboratory consisting of multiple activity sessions occurring simultaneously. Objectively, examination scores proved to be a positive outcome and, subjectively, feedback from students was overwhelmingly positive. Despite the surging molecular based sciences consuming much of the curricula, full-body dissection anatomy is irreplaceable regarding both surface and architectural, radiological anatomy. Radiology should not be a small adjunct to understand full-body dissection, but rather, full-body dissection aids the understanding of radiology mediums. The millennial anatomy dissection laboratory should consist of, at least, 50% radiology integration during full-body dissection. This pilot study is an example of the most comprehensive integration of full-body dissection, radiology, and hospital technology.

Abstract:This article delves into the beginnings of the dissection course, a teaching practice which today is still in place in Vienna and continues to shape future medical practitioners. Based on a comparison of different historical sources the article shows that the Viennese tradition of a dissection course dates back to the 1780s and the initiative of the anatomist Joseph Barth to build a dissection institute and to implement a dissection course, two endeavors that coincided with Joseph II's reform ideas regarding a practically orientated medical and surgical education and a Europe-wide practice turn. Additionally, this paper shows the role of the Viennese dissection course as model for other Habsburg universities and, thus, explains the similarities of today's dissection courses in different former Habsburg universities.

Evolution of illustrations in anatomy: a study from the classical period in Europe to modern times

Abstract:Illustrations constitute an essential element of learning anatomy in modern times. However it required a significant evolutionary process spread over centuries, for illustrations to achieve the present status in the subject of anatomy. This review article attempts to outline the evolutionary process by highlighting on the works of esteemed anatomists in a chronological manner. Available literature suggests that illustrations were not used in anatomy during the classical period when the subject was dominated by the descriptive text of Galen. Guido da Vigevano was first to use illustrations in anatomy during the Late Middle Ages and this concept developed further during the Renaissance period when Andreas Vesalius pioneered in illustrations becoming an indispensable tool in conveying anatomical details. Toward later stages of the Renaissance period, Fabricius ab Aquapendente endeavored to restrict dramatization of anatomical illustrations which was a prevalent trend in early Renaissance. During the 18th century, anatomical artwork was characterized by the individual styles of prominent anatomists leading to suppression of anatomical details. In the 19th century, Henry Gray used illustrations in his anatomical masterpiece that focused on depicting anatomical structures and were free from any artistic style. From early part of the 20th century medical images and photographs started to complement traditional handmade anatomical illustrations. Computer technology and advanced software systems played a key role in the evolution of anatomical illustrations during the late 20th century resulting in new generation 3D image datasets that are being used in the 21st century in innovative formats for teaching and learning anatomy.
Iontophoretically applied lidocaine and the prevention of pain associated with the injection of intravenous propofol: a comparison with intravenous lidocaine.

Abstract:BACKGROUND AND OBJECTIVE Pain following the injection of propofol is a common problem. It is often treated by the coadministration of intravenous lidocaine. Iontophoresis, which facilitates the delivery of charged molecules such as lidocaine to body tissues, may be a practical alternative to lessen the pain. METHODS The iontophoretic application of lidocaine was compared with intravenous lidocaine to assess the relative efficacy of reducing pain during propofol administration. RESULTS The incidence rate of moderate or severe pain was significantly greater in the iontophoresis group (26.7 compared with 6.9% in the intravenous lidocaine group) (P < 0.05). CONCLUSIONS The iontophoretic administration of lidocaine 60 mg does not compare favourably with lidocaine 40 mg mixed with propofol for the reduction of pain on administration of propofol.
Walkability is Only Part of the Story: Walking for Transportation in Stuttgart, Germany

Abstract:In modern Western societies people often lead inactive and sedentary lifestyles, even though there is no doubt that physical activity and health are related. From an urban planning point of view it would be highly desirable to develop built environments in a way that supports people in leading more active and healthy lifestyles. Within this context there are several methods, predominantly used in the US, to measure the suitability of built environments for walking and cycling. Empirical studies show that people living in highly walkable areas are more physically active (for example, walk more or cycle more). The question is, however, whether these results are also valid for European cities given their different urban planning characteristics and infrastructure standards. To answer this question we used the Walkability-Index and the Walk Score to empirically investigate the associations between walkability and active transportation in the city of Stuttgart, Germany. In a sample of household survey data (n = 1.871) we found a noticeable relationship between walkability and active transportation—the more walkable an area was, the more active residents were. Although the statistical effect is small, the health impact might be of relevance. Being physically active is multi-determined and not only affected by the walkability of an area. We highlight these points with an excursion into research that the health and exercise sciences contribute to the topic. We propose to strengthen interdisciplinary research between the disciplines and to specifically collect data that captures the influence of the environment on physical activity in the future.
Style and non-style in anatomical illustration: From Renaissance Humanism to Henry Gray

Abstract:Style is a familiar category for the analysis of art. It is less so in the history of anatomical illustration. The great Renaissance and Baroque picture books of anatomy illustrated with stylish woodcuts and engravings, such as those by Charles Estienne, Andreas Vesalius and Govard Bidloo, showed figures in dramatic action in keeping with philosophical and theological ideas about human nature. Parallels can be found in paintings of the period, such as those by Titian, Michelangelo and Hans Baldung Grien. The anatomists also claimed to portray the body in an objective manner, and showed themselves as heroes of the discovery of human knowledge. Rembrandt's painting of Dr Nicholas Tulp is the best-known image of the anatomist as hero. The British empirical tradition in the 18th century saw William Cheselden and William Hunter working with techniques of representation that were intended to guarantee detailed realism. The ambition to portray forms life-size led to massive volumes, such as those by Antonio Mascagni. John Bell, the Scottish anatomist, criticized the size and pretensions of the earlier books and argued for a plain style adapted to the needs of teaching and surgery. Henry Gray's famous Anatomy of 1858, illustrated by Henry Vandyke Carter, aspired to a simple descriptive mode of functional representation that avoided stylishness, resulting in a style of its own. Successive editions of Gray progressively saw the replacement of Gray's method and of all his illustrations. The 150th anniversary edition, edited by Susan Standring, radically re-thinks the role of Gray's book within the teaching of medicine.

Abstract:Beddhu et al. [1] recently have reported that there were no positive associations of malnutrition with documented acute coronary syndromes requiring hospitalization in a large incident Medicare dialysis population. The authors state that their findings do not support our suggestion [2] of an association between malnutrition, inflammation and atherosclerosis (MIA hypothesis) in patients with end-stage renal disease (ESRD). No doubt the study by Beddhu et al. [1] is a well performed study conducted in a large group of dialysis patients. However, we feel that some fundamental issues regarding the nomenclature and definition of ‘malnutrition’ need to be discussed. First, as discussed by Mitch [3], the use of the word ‘malnutrition’ has often been used incorrectly in the renal literature, and, according toMitch, ‘malnutrition’ should be used to describe a deficient nutritional status caused by insufficient nutritient intake. However, literally, the word ‘malnutrition’ (derived from the latin word ‘malus’) means, ‘not correctly nourished’. Thus, both underand over-nourished (obese) patients could be considered to be ‘malnourished’. Secondly, whereas malnutrition is usually defined as a consequence of insufficient food intake and low serum protein levels, the loss of muscle mass (i.e. cachexia or wasting) in the ESRD patients is usually the consequence of a number of catabolic mechanisms stimulated by renal insufficiency. Indeed, the aetiology of loss of lean body mass in ESRD is very complex and may include numerous factors apart from poor food intake (i.e. true malnutrition), such as delayed gastric emptying, hormonal derangements, inadequate control of acidosis, co-morbidity and inflammation [3]. Whereas Beddhu et al. [1] used a low body mass index (BMI) and low creatinine excretion in urine as a marker of malnutrition, we used subjective global assessment (SGA). It is obvious that SGA and BMI are crude surrogate markers of nutritional status that cannot be readily compared. In our opinion, BMI is not a very precise parameter of nutritional status, especially in patients in whom gross imbalances in fluid homeostasis are commonly observed, such as in patients with ESRD, congestive heart failure and liver disease. Also, protein malnutrition with loss of muscle mass (sarcopoenia) is often associated with a relatively well-preserved fat mass in dialysis patients, resulting in small changes in BMI that may be obscured further by imbalances in fluid homeostasis. In fact, as BMI alone does not always capture adequately the joint relationship of body composition and body size to outcome [4], the waist circumference may have more potential in detecting overweight than BMI [5]. Indeed, although there may be a marked statistical difference in BMI levels comparing ESRD patients classified as well nourished (SGA1⁄4 1) or malnourished (SGA 2), there is a considerable overlap between the two groups (Figure 1). Thus, patients considered to be malnourished according to SGA may have a low as well as a high BMI (Table 1). Clearly, high-BMI patients could also have an ongoing catabolic wasting process, and low-BMI patients may show no signs of catabolism. To be able to compare the results presented by Beddhu et al. [1], we have analysed the independent predictive power of both BMI and SGA in 271 incident ESRD patients (173 males; mean 53±1 years) starting renal replacement therapy (Table 1). During the observation period (3.7±0.2 years; range 0.1–9.2 years), 94 patients died (66 of cardiovascular disease; CVD). Like Beddhu et al. [1], we divided the patients into three groups according to their BMI. Whereas no differences in the prevalence of diabetes mellitus, clinical overt CVD or inflammation [C-reactive protein (CRP) 10mg/l] were noted between the different BMI groups, the prevalence of malnutrition (according to SGA) differed significantly (Table 1).
