Abstract:Background: The purpose of this study was to evaluate the clinical symptoms, surgical management, and outcomes of pregnant women with adnexal torsion due to assisted reproductive technology. Methods: It was a retrospective study that include 17 pregnant women with adnexal torsion, in which the maternal age, type of fertilization, gestational age, clinical symptoms, ultrasonic findings, side affected by the disease, surgical method, and pregnancy outcomes were evaluated. Results: A total of 17 patients with adnexal torsion were included in this study, of which 8 patients conceived by in vitro fertilization-embryo transfer (IVF-ET), 1 by artificial insemination (AIH), and the other 8 conceived naturally after ovulation induction. About 14 were reported to have occurred in the first trimester of pregnancy, 1 case in the second trimester, and the other 2 in the third trimester. Clinical symptoms were abdominal pain with or without nausea and vomiting. 14 cases occurred in the right adnexa and the other 3 in the left. 5 of the patients underwent laparoscopy, and the other 12 underwent laparotomy. 8 cases were of full-term delivery, 6 twins gave birth prematurely, and 3 patients had inevitable abortion. Conclusions: Adnexal torsion is an acute onset of lower abdominal pain in women, which seldom occurs during pregnancy. However, because of the wide application of assisted reproductive technology (ART), its incidence has increased. Early diagnosis and treatment can lead to better results. (C) 2019 Elsevier Inc. Successful pregnancy-preserving laparoscopic adnexectomy for complicated ovarian torsion during late pregnancy: A case report and review of the literature

Abstract:Introduction and importance: A rare disorder called ovarian torsion (OT) during pregnancy can harm both the mother and the fetus. Predisposing variables for the condition include enlarged ovaries, free mobility, and a long pedicle, despite the fact that its genesis is not entirely understood. When ovarian stimulation is used to treat infertility, the disease's incidence rises. Magnetic resonance imaging and ultrasound are examples of diagnostic imaging modalities (MRI). Case presentation: A 26-year-old woman with a 33-week pregnancy presented to our emergency department with acute, severe left groin pain. Laboratory evaluation was unremarkable except for leukocytosis (18.800/μL) with neutrophil shift. A radiologist used ultrasound to examine the abdomen and pelvis, and the results revealed a bulk enlargement of the left adnexa. The patient underwent a non-enhanced MRI in order to obtain a conclusive diagnosis, which revealed a massive enlargement and torsion of the left ovary with large areas of necrosis. The patient underwent a successful laparoscopic adnexectomy with preservation of the pregnancy. She delivered a healthy baby and had an uneventful follow up period. Discussion: The etiology of OT is largely unknown. Any tendency to rotate the infundibulopelvic and utero-ovarian ligaments should be considered as a possible etiology. The prevalence of OT among pregnant women is underreported and determined by small limited studies. Conclusion: Ovarian torsion should be included in the differential diagnosis of patients with suspected acute abdomen in advanced stages of pregnancy. In addition, MRI should be used as an alternative diagnostic modality in patients with normal sonographic findings.
Torsion of normal‐sized ovary during late pregnancy: A case report and review of the literature

Abstract:Adnexal torsion is one of the most common gynecologic surgical emergencies. All age groups can be affected, but torsion of normal-sized ovary that happens during late pregnancy is rare and challenging to be diagnosed. The objective of this article is to present a case of adnexal torsion in a normal-sized ovary suspected by magnetic resonance imaging (MRI) in the third trimester of pregnancy. A 36-year-old woman at 32 + 5 weeks gestational age was admitted to hospital due to recurrent severe left lower abdominal pain. Doppler ultrasound failed to demonstrate the ovarian diseases, while MRI scan suspected the diagnosis of adnexal torsion. The patient received emergent exploratory laparotomy, and the left adnexa with a necrotic ovary was removed. Tocolytic therapy was used before and after surgery. Finally, she delivered a healthy full-term infant via cesarean section. Adnexal torsion occurring in a normal-sized ovary was quite rare in the third trimester pregnancy. MRI might be better than ultrasound in the early diagnosis of ovarian torsion.

Abstract:Objective The study was conducted to understand the causes and clinical characteristics of acute abdomen resulting from ovarian hyperstimulation syndrome(OHSS) after controlled ovarian hyperstimulation(COH),and therefore develop a scientific method for early diagnosis and effective treatment of the disease.Method Fifteen cases of acute abdomen in 617 cases of OHSS from 1999 to 2009 were retrospectively reviewed.The etiology,occurrence rate,clinical characters,relationship with pregnancy,relationship with the severity of OHSS and treatment were analyzed.Findings The total occurrence rate of acute abdomen in 617 OHSS was 2.43%(15/617).The major etiology of acute abdomen is ovarian torsion(12/15),including 3 complete ovarian torsions and 9 incomplete ovarian torsions.The others were as follows: ovarian incarceration(1/15),oothecorrhexis(1/15) and retention of urine(1/15).Three patients with complete ovarian torsions were treated by unilateral adnexectomys.Other patients with incomplete ovarian torsions had conservative treatment,except that one patient underwent luteinized cyst puncture operation.Syndrome was not relieved after 36 hours conservative treatment in that patient.In the case of ovarian incarceration,the left ovary increased,displacemented and incarcerated in the left iliac back of knee.The patient underwent bilateral ovarian follicle puncture and left ovary reposition.Eight cases were confirmed to have become pregnant of the 15 cases of acute abdomen.Of the 12 cases of ovarian torsion,7 cases became pregnant.Except for 1 abortion,all other 7 cases had good pregnancy outcome.Conclusion Ovarian torsion is the major etiology of acute abdomen due to OHSS after COH.OHSS accompanied by pregnancy was a high-risk factor of ovarian torsion.B-ultrasomotonography is an effective method for early diagnosis.Timely diagnosis and treatment does not affect the outcome of pregnancy.

Case Report: A 32-year-old woman underwent a successful pregnancy after FET, but was hospitalized for observation because of a threatened abortion. During the 8th week of pregnancy, a transvaginal ultrasound examination was performed due to sudden lower abdominal pain, which displayed an 8.3× 5.2cm right ovary and a 4.3× 3.6 cm enhanced echogenic mass over the right ovary with striated structures. Consequently, a preliminary diagnosis of ovarian torsion was made, and urgent laparoscopic exploration was conducted. Intraoperatively, the right fallopian tube and right ovary were rotated counterclockwise by 720 degrees. After rotate and reposition the right ovary and fallopian tube, a 3 cm diameter cyst was punctured. After 1 hour of observation, a 2× 1× 1cm segment of ovarian tissue was excised. Pregnancy was maintained with aggressive postoperative luteal support and anti-infective treatment. Conclusion: The use of clomiphene citrate during endometrium preparation for frozen-thawed embryo transfer cycles may elevate the risk of ovarian torsion. Laparoscopic ovarian repositioning after ovarian torsion in pregnant women, even those with threatened abortion, is safe when combined with postoperative anti-infective treatment and intensive luteal support. Keywords: ovarian torsion, frozen-thawed embryo transfer, pregnancy, laparoscopy As is well documented, ovarian torsion is one of the most prevalent gynecologic emergencies among women of childbearing age. 1 It is caused by complete or partial rotation of the adnexal structures, leading to ischemic changes in the ovary. Its incidence in the general population typically ranges from 0.01% to 0.05%. 2 However, its incidence during pregnancy is uncertain, estimated to be between 0.2% and 3%. 3 Risk factors for ovarian torsion include ovarian masses, pregnancy, ovarian hyperstimulation syndrome (OHSS), polycystic ovarian syndrome, and multiple pregnancies. 4,5 Considering the increased risk of ovarian torsion in the presence of large ovarian cysts, in vitro fertilization (IVF) and pregnancy puts adnexal torsion at increased risk. A retrospective cohort study showed that GnRH agonist trigger in combination with frozen-thawed embryo transfer (FET) significantly reduced the incidence of ovarian torsion and OHSS. 6 In cases of suspected ovarian torsion, emergency laparoscopic surgery with ovarian repositioning or ovarian fixation and, if necessary, adnexectomy is reasonable. 7 Unlike in non-pregnancy, guidelines for the management of ovarian torsion during pregnancy are scarce. 8,9 Acute ovarian torsion with arterial obstruction, if left untreated, may result in ischemic necrosis of the ovary, 10 thereby impacting ovarian function. During early pregnancy, ovarian secretion of estrogen and progesterone is essential for the maintenance of pregnancy. This article presents the case of a patient hospitalized for threatened abortion after FET who developed ovarian torsion at 8 weeks of gestation and underwent laparoscopic exploratory surgery. Ovarian torsion: A retrospective case series at a tertiary care center emergency department

Abstract:Ovarian torsion (OT) is a gynecological emergency that can have diverse clinical presentations makings its diagnosis especially challenging. Few studies exist in the literature describing the clinical presentation as well as the management of OT in the emergency department (ED). This study aims to describe the clinical presentation, physical examination, emergency management, radiographic and intraoperative findings, histopathology reports and the time-to-intervention metrics of OT cases presenting to the emergency room of our tertiary care center. Data was collected by a retrospective chart review of all OT cases that presented to our ED over a period of 1 year. A total of 20 cases were included in the final analysis. The incidence of OT in the ED was 157.4 per 100 000 visits of women in the reproductive age group. All patients were women of reproductive age, with a mean age of 27.3 years. A total of 15 patients (78.9%) presented within 24 hours of symptom onset. The most common presenting symptom was abdominal pain (95%, 19/20) with most localizing to the right lower quadrant (60%, 12/20), followed by nausea and vomiting (90%, 18/20). Almost all patients (95%, 19/20) required opioids for pain management. The majority (80%, 16/20) were diagnosed by ultrasound prior to surgery, whereas (20%, 4/20) went straight to surgery based on a high index of clinical suspicion. Among those who underwent ultrasound, ovarian cyst was present in (75%, 12/16) while (68.9%, 11/16) showed ovarian enlargement and (50%, 8/16) showed abnormal ovarian blood flow. All patients except one patient underwent operative management. Out of the 19 patients who underwent surgery, the majority of patients (94.7%, 18/19) had viable ovaries with the exception of 1 patient (5.3%) who required a salpingo-oophorectomy post ovarian detorsion. A total of 13 cases included histopathologic review of specimens out of which 6 (46.2%) had a mature cystic teratoma. The mean time from door to ultrasonography was 1.4 hours and from door to surgery was 11.4 hours. Our study provides valuable insights into the incidence, clinical presentation, physical examination, emergency management, ultrasonographic and intraoperative findings, histopathology reports as well as the time-to-intervention metrics of OT patients presenting to the ED.

The Role of Surgery in Antenatal Ovarian Torsion: Retrospective Evaluation of 28 Cases and Review of the Literature

Abstract:Study objective: Antenatal ovarian torsion (AOT) is rare and requires differentiating from other congenital cystic masses of the abdomen and pelvis in neonates. In this study, it was aimed to evaluate the prenatal characteristics and postnatal outcomes of AOT. DESIGN, SETTING, PARTICIPANTS, INTERVENTIONS, AND MAIN OUTCOME MEASURES: Hospital records of patients (n = 28) with diagnosis of AOT between 2004 and 2020 were reviewed and their prenatal characteristics, postnatal examination, imaging, operative, and histopathological findings were evaluated. Results: There were 28 patients during the study period. In most of the patients (25/28; 89.3%) AOTs were detected prenatally. All were term babies with a mean birth weight of 3010 ± 466.6 g. Mean maternal age was 25.75 ± 3.65 years. Prenatal history was event-free in all and none had additional anomalies. Physical examination revealed mobile intra-abdominal cystic lesions in 16 (57.1%) patients. AOTs mimicked other pathologies as intestinal duplication cyst or mesenteric cyst (n = 7), complex ovarian cyst (n = 3), mature cystic teratoma (n = 3), simple renal cyst (n = 1), and ectopic kidney (n = 1) in 15 (53.6%) patients in postnatal ultrasonography. Elective laparoscopic surgery was performed in 26 (92.8%) patients. The other 2 patients required emergency open surgery because of acute symptoms. Ovaries were autoamputated in 17 (60.7%) patients. Histopathological examination revealed necrosis and dystrophic calcification in all specimens. There was neoplastic involvement in 2 patients (serous cystadenoma and gonadoblastoma). No complication occurred in the early follow-up period (mean: 73.9 ± 46.8 months). Conclusion: AOTs can be detected easily during the antenatal period. Neoplastic involvement with AOTs is rare but possible. Although AOTs might mimic other cystic pathologies, it should be one of the first diagnoses to be considered, in the presence of a palpable intra-abdominal mass and complex cystic lesion in infant girls. It can be efficiently and safely managed using minimally invasive techniques.
Torsion of fallopian tube with gangrenous ovary and early pregnancy: A rare case report

Abstract:Torsion of the fallopian tube during early pregnancy is a very rare phenomenon. Most of them present with acute abdominal pain and are often misdiagnosed with a twisted ovarian cyst, ectopic pregnancy, appendicitis, etc. A 19-year-old female was referred to the medical department for pain abdomen, nausea, and vomiting. Her last menstrual period was 28 days back. The urine pregnancy test was negative. Ultrasonogram (USG) was suggestive of encapsulated exophytic hemorrhagic cyst in the right adnexa arising from the right ovary. A huge right-twisted fimbrial cyst with hemorrhagic contents (14 cm × 12 cm) along with a gangrenous right ovary was evident in emergency laparotomy. Right salpingo-oophorectomy was performed. She was discharged with an uneventful postoperative recovery. She presented again after 2 months with postoperative amenorrhea, nausea, and vomiting. USG suggested 12 weeks of pregnancy, which continued until term. Eventually, a lower segment cesarean section was done for fetal distress. A twisted fallopian tube during pregnancy is a rare phenomenon. A high index of suspicion necessitates prompt diagnosis and prevention of future fertility and obstetric morbidity.

Abstract:Objective To explore the clinical diagnosis and treatment of ovarian tumors torsion in pregnancy.Methods The clinical data of 34 patients with ovarian tumors torsion in pregnancy were retrospectively analyzed.Results In 34 patients,5 cases were misdiagnosed(2 cases were misdiagnosed as acute appendicitis and 3 cases were misdiagnosed as ovarian tumors rupture).Six cases received conservative treatment,and the other 28 cases were performed operation,in which 9 cases were performed high ligation of the suspensory ligament of ovary followed by reposition plus excision of ovary benign tumors,and the other 19 cases were performed adnexectomy.Twenty-eight healthy neonates were delivered(22 babies of term birth and 6 of premature birth) and miscarriage occurred in 6 cases.Conclusion Ovarian tumors torsion in pregnancy is mobstetrical emergency.To make a prompt diagnosis and perform correct treatment can achieve a good result for mother and infant.
Adnexal Torsion of a 28 cm Paratubal Cyst: A Case Report

Abstract:Objective: To report a rare case of a 28 cm left adnexal torsion requiring surgical intervention including detorsion and salpingectomy Methods: Case report. Results: An 18-year-old nulliparous female with morbid obesity presented to the emergency department complaining of one week of sharp right lower quadrant pain with nausea and vomiting that had acutely worsened. Vital signs were notable for tachycardia, but otherwise within normal ranges. She had a significant leukocytosis. Her pregnancy test was negative. A CT abdomen/pelvis was obtained that demonstrated a large multiloculated complex cystic mass measuring 28.5 x 19.8 x 13.3 cm with possible origin from the ovaries that could represent a serous cystadenoma. Additionally, the scan was remarkable for a heterogeneous and enlarged left ovary measuring 7.6 x 5.9 x 7.2 cm, surrounded by free fluid and mesenteric edema. On examination, she did not demonstrate any surgical abdomen findings. Her pain improved with pain medication. She was admitted, tumor markers obtained, and an ultrasound ordered. The ultrasound demonstrated normal appearing right ovary with present doppler flow and a simple cyst measuring 6.6 x 5.1 x 6.1 cm. The left adnexa was noted to have a complex versus solid mass with unclear boundaries of the ovary. Also noted on the ultrasound were two large simple midline cysts measuring 10.5 x 9.1 x 11.8 cm inferiorly and 18.2 x 15.5 x 17.2 cm superiorly. No free fluid was seen within the right upper quadrant on ultrasound. The view of the left upper quadrant was suboptimal. Due to these findings, Gynecologic Oncology service was also consulted and after discussion, decision made to proceed urgently to diagnostic laparoscopy with robotic assisted cystectomy versus oophorectomy. Upon laparoscopic entry into the abdomen, a large necrotic-appearing left adnexal mass was visualized coming from the left fallopian tube. Both the left fallopian tube and left ovary were necrotic, with the left ovary being torsed numerous times. After resection of the large left fallopian tube and cyst, the left ovary was detorsed, and the decision was made to leave the ovary in situ. It still appeared necrotic, but its blood supply was intact. There was a large simple paratubal cyst on the right fallopian tube that was also drained and excised. Postoperatively, her right lower quadrant pain resolved, and she was discharged on post-operative day 1. She prescribed a two-week course of doxycycline and metronidazole as pelvic inflammatory disease could not be ruled out. At outpatient follow-up, she was asymptomatic and ready to begin working again. Her pathology report confirmed a large left paratubal cyst with associated findings of ischemia due to torsion and a large right paratubal cyst. Conclusions: Adnexal torsion is a rare gynecologic disorder caused by the partial or complete rotation of the ovary and/or the fallopian tube on its vascular support, which can negatively impact fertility. Prompt diagnosis is key, but definitive diagnosis is only by direct visualization. The differential diagnosis for acute onset of severe pelvic pain with nausea and vomiting is broad and includes ectopic pregnancy, pelvic inflammatory disease, endometriosis, ruptured ovarian cyst, urologic issues and gastrointestinal issues. Commonly, ultrasound is used to help evaluate a possible torsion with several classic findings that were not noted in this case, other than a mass. The two best known risk factors for adnexal torsion are a mobile ovarian mass and a history of prior torsion. Although large ovarian masses greater than 5 cm are more likely to cause torsion, some experts hypothesize that very large masses are less likely to torse given the constraints of the pelvis. However, such was not the case here. Additionally, it has been observed that adnexal torsion typically affects the right adnexa due to theoretical protection from the descending colon on the left. Given that the case presented did not have any of the typical imaging findings for adnexal torsion and that her pain was on the right but the torsion on the left, it is important to maintain a high level of suspicion when evaluating patients with acute onset pelvic pain of any location with nausea and vomiting.

Abstract:In this case report, a rare case of an adnexal torsion caused by hyperreactio luteinalis (HL) in the third trimester is described, since adnexal torsions are mainly restricted to the first trimester of pregnancy. In an emergency Cesarean section, the patient gave birth to a healthy female baby weighing 3,300 g and we found an enlarged benign multiple luteinized follicular cyst mass in the right adnexum, which led to an adnexal torsion. After detorsion, both ovaries recovered to their normal sizes two months after the intervention.

Chitosan and COA treatments promoted the expression of key phenylpropanoid pathway genes, for synthesis of lignin and flavonoids; only those associated with flavonoid metabolism were up-regulated by BTH. The calcium-dependent protein kinase, endo-β 1,4-glucanase, ascorbate peroxidase, and glutathione-S-transferase genes were up-regulated by BTH. The K+ channel, polygalacturonase, polygalacturonase-inhibiting protein, and β-1,3-glucanase, increased in response to all tested elicitors. The enzyme activities of phenylalanine ammonia lyase, β-1,3-glucanase, Chitinase, and guaiacol peroxidase supported the gene expression results. Similarity of gene expression was >72% between chitosan and COA treatments, while BTH showed lower similarity (38%) with the other elicitors. This study suggests the relationship between the composition of the elicitors and a specific pattern of induced defense genes.
Torsion of a large subserosal fibroid along with omental torsion adherent to it: a rare cause of acute abdomen

Abstract:A woman in her 30s presented to emergency with complaints of acute lower abdominal pain for 3 days, not associated with any menstrual, bowel or urinary symptoms. Examination revealed an abdominopelvic mass corresponding to an 18-week gravid uterus with diffuse tenderness and guarding over her lower abdomen. The patient was a follow-up case of subserosal fibroid uterus, chronic kidney disease stage 4 and rheumatic heart disease on anticoagulants. Fibroid degeneration or torsion was suspected. Ultrasound revealed a large posterior wall subserosal fibroid with free fluid in the pelvis. As findings did not suggest degeneration or pedunculated fibroid, noncontrast CT was done, which showed a similar mass with a pedicle arising from the uterine fundus with free fluid with no other evident cause of acute abdomen. The patient was taken up for emergency laparotomy. Intraoperatively, it was found to be a case of subserosal fibroid with greater omentum adhered to it and twisted around its axis about eight times. This case is being reported to highlight a rare cause of acute abdomen.
Electron microscopic evidence for microglial phagocytic activity and cholinergic cell death after administration of the immunotoxin 192IgG‐saporin in rat

Abstract:192IgG‐saporin represents a novel cholinergic immunotoxin which selectively and specifically destroys cholinergic cells in rat basal forebrain. Activated microglial cells are known to play an important role in phagocytosis in regions of neuronal loss. To study the immunotoxin‐induced phagocytic events in the basal forebrain activated microglial cells were visualized by lectin cytochemistry using Griffonia simplicifolia agglutinin and analyzed by electron microscopy. Three and 7 days following an intracerebro‐ventricular injection of 4 μg 192IgG‐saporin, increased numbers of activated microglial cells were observed at both survival times, but the number was strikingly increased at day 7 postlesion. Three days after immunotoxin application microglial cells displayed features similar to those of resting microglia. Only translucent vacuole‐like hollows were found intracellularly beneath the plasma membrane of microglial cells and in the adjoining extracellular space. Most neurons in the vicinity of microglial cells did not show any signs of degeneration. However, 7 days after injection of the immunotoxin microglial cells revealed different stages of phagocytosis. The majority of microglial cells were localized in perineuronal positions attached by processes to large areas of neuronal soma or dendrites, which in general showed signs of severe degeneration. The present study provides electron microscopic evidence for phagocytic microglial reactions in the rat basal forebrain after cholinergic lesion by 192IgG‐saporin. J. Neurosci. Uremic pericarditis: a prospective echocardiographic and clinical study.

Abstract:During a 14 month period 59 male patients with chronic renal failure who were candidates for chronic hemodialysis (HD) were evaluated clinically and echocardiographically for pericarditis. All were evaluated prospectively prior to or at the initiation of HD. Definite pericarditis was present in 8, all of whom were severely uremic and required initiation of HD on a semiemergent basis rather than electively (i.e., preselected level of renal function). In 6 of these 8, pericardial effusion responded to dialysis alone, one required pericardiectomy because of hypotension complicating dialysis, and one expired during a right atriogram. Patients dialyzed on an elective basis were all free of pericarditis at the initiation of HD. Pericarditis arising some months after the initiation of HD was a less frequent problem. It is concluded that (a) the incidence of pericarditis in the uremic state is decreased by early initiation of HD before advanced uremic symptoms have developed; (b) pericarditis present at the initiation of HD usually but not always is resolved with the initiation of HD; (c) echocardiography is an important clinical and epidemiological toole to investigate pericarditis in uremic patient populations.

Abstract:Abstract The safety of adnexal detorsion in pregnant women is debatable as the hypercoagulable state during pregnancy intensifies the risk of thromboembolism. The diagnosis and treatment is challenging because its rarity and complexity due to the foetal health. Here we presented a rare case of venous thromboembolism (VTE) after ovarian detorsion during early pregnancy and conducted a review of the literature. VTE after ovarian torsion-detorsion in pregnancy is an infrequent event and is more likely to occur in the first and second trimester. The diagnosis depends mainly on the clinical manifestation, D-dimer level and imaging tests such as compression ultrasonography and computed tomography pulmonary angiogram. Low-molecular-weight heparin remains the drug of choice for both prophylactic measurement and therapeutic strategy. With prompt diagnosis and immediate treatment, VTE after ovarian torsion-detorsion is usually uneventful.
Omental torsion mimicking acute appendicitis: A case report

Abstract:Introduction: Omental torsion is a rare cause of acute abdominal pain caused by twisting of the omentum along its long axis, thus compromising its vascularity. Its presentation is non-specific and can mimic other common pathologies, making its pre-operative diagnosis challenging. Presentation of case: A 44-year-old female presented for periumbilical abdominal pain. Her laboratory results showed no leukocytosis and CRP was within normal range. CT scan of the abdomen and pelvis with oral and IV contrast showed a well demarcated pericecal mass at the right side, mostly suggestive of transmesenteric internal herniation with strangulation. The patient eventually required laparoscopic surgical intervention. Discussion: The acute abdominal manifestations in patients with omental torsion are due to the development of edema and necrotic tissue distal of the torsion after the arterial supply and venous drainage have been obstructed. Rotation around the right gastroepiploic artery is considered to be the most common cause of omental torsion. Primary torsion is considered to be idiopathic, while secondary torsion occurs due to an identifiable predisposing pathology such as omental cysts, hernias, adhesions, or intra-abdominal tumors. Since symptoms of omental torsion are non-specific, it is crucial to consider the differential diagnosis and rule out other causes of acute abdomen. Surgical intervention is the mainstay treatment when there is uncertainty in the diagnosis, or when the patient's clinical, radiological, and laboratory findings worsen with conservative treatment. Conclusion: Early surgical intervention in cases of omental torsion reduces the incidence of formation of abscesses, adhesions, and omental necrosis. In cases of non-operative candidates, conservative treatment is the best option; therefore, the choice of treatment of omental torsion should be considered on a case-by-case basis.
