Abstract:Introduction: For nearly 50 years, institutional review boards (IRB) and independent ethics committees have featured local oversight as a core function of research ethics reviews. However growing complexity in Alzheimer's clinical research suggests current approaches to research volunteer safety is hampering development of new therapeutics. As a partial response to this challenge, the NIH has mandated that all NIH-funded multi-site studies will use a single Institutional Review Board. The perspective describes a joint program to provide a single IRB of record (sIRB) for phases of multi-site studies. Methods: The approach follows two steps. One, an expert Scientific Review Committee (SRC) of senior researchers in the field will conduct the review principally of scientific merit, significance, feasibility, and the likelihood of meaningful results. The second step will be the IRB's regulatory and ethics review. The IRB will apply appropriate regulatory criteria for approval including minimization of risks to subjects and risks reasonable in relation to anticipated benefits, equitable subject selection, informed consent, protections for vulnerable populations, and application of local context considerations, among others. Results: There is a steady demand for scientific, ethical and regulatory review of planned Alzheimer's studies. As of January 15, 2017, there are nearly 400 open studies, Phase II and III, industry and NIH sponsored trials on disease indications affecting memory, movement and mood in the US. Conclusions: The effort will initially accept protocols for studies of Alzheimer's disease, dementia, and related disorders effecting memory, movement and mood. Future aims will be to provide scientific review and, where applicable, regulatory and ethical review in an international context outside North America with sites possibly in Asia, Europe and Australia.
Effect of Side Feed Pressurization on the Performance of Shimmed Foil Gas Bearings-Part II : Model , Predictions and Comparisons to Rotordynamic Measurements

Abstract:Oil-free micro turbomachinery implementing gas foil bearings (GFBs) has improved mechanical efficiency and reliability. Adequate thermal management for operation in high temperature environments is an issue of importance in applications such as in gas turbines and turbochargers. GFBs often need a cooling gas flow, axially fed through one end of the bearings, to transport the heat conducted from a hot turbine, for example. Side gas pressurization has also a paramount effect on reducing amplitudes of motion, synchronous and subsynchronous. In a comparison 2007 TRC report, shaft motion measurements in a test rotor supported on GFBs show this remarkable effect. A computational gas film model implementing the evolution of gas circumferential velocity as a function of the imposed side pressure is advanced. For tested GFBs, predicted direct stiffnesses and damping coefficients increase as the magnitude of feed pressure raises, while the difference in cross-coupled stiffnesses, directly related to rotor-bearing system stability, decreases. Predictions of threshold speed of instability and whirl frequency ratio are in close agreement with laboratory measurements. A model for GFBs with shims is introduced. The shimmed GFB, whose film clearance resembles a three lobe bearing, generates significantly larger hydrodynamic pressures than those in a conventional GFB. Shimmed GFBs operate with a smaller journal eccentricity and attitude angle than conventional GFB, and with increased direct stiffness and damping coefficients. A linear finite element rotordynamic analysis (XLTRC 2) models the test rotor supported on GFBs and predicts the system rotordynamic stability and synchronous rotor responses, phase angle and amplitude. Predicted synchronous responses are in very good agreement with the test measurements, in particular for small to moderate imbalances. As a final observation, although external pressurization aids to improve GFB rotordynamic performance; a too large supplied gas flow rate for cooling an actual high temperature application may penalize sensibly the efficiency and performance of the turbomachinery.
Enabling responsible public genomics

Abstract:As scientific understandings of genetics advance, researchers require increasingly rich datasets that combine genomic data from large numbers of individuals with medical and other personal information. Linking individuals' genetic data and personal information precludes anonymity and produces medically significant information--a result not contemplated by the established legal and ethical conventions governing human genomic research. To pursue the next generation of human genomic research and commerce in a responsible fashion, scientists, lawyers, and regulators must address substantial new issues, including researchers' duties with respect to clinically significant data, the challenges to privacy presented by genomic data, the boundary between genomic research and commerce, and the practice of medicine. This Article presents a new model for understanding and addressing these new challenges--a "public genomics" premised on the idea that ethically, legally, and socially responsible genomics research requires openness, not privacy, as its organizing principle. Responsible public genomics combines the data contributed by informed and fully consenting information altruists and the research potential of rich datasets in a genomic commons that is freely and globally available. This Article examines the risks and benefits of this public genomics model in the context of an ambitious genetic research project currently under way--the Personal Genome Project. This Article also (i) demonstrates that large-scale genomic projects are desirable, (ii) evaluates the risks and challenges presented by public genomics research, and (iii) determines that the current legal and regulatory regimes restrict beneficial and responsible scientific inquiry while failing to adequately protect participants. The Article concludes by proposing a modified normative and legal framework that embraces and enables a future of responsible public genomics.

Abstract:As a new medical concept and medical model, precision medicine cannot develop without the collection, storage and sharing of medical clinical data and biomedical big data. These data include personal information such as personal genetic information, electronic medical records, living environment and clinical data. However, information sharing in the context of big data faces many ethical challenges: (1) The issue of ownership of biomedical data, that is, once the biological information (including behavioral information) originally belonging to individuals is mined; (2) the exposure of personal privacy and group privacy and the resulting discrimination and injustice; (3) the reconstruction of the subject of informed consent and the unclear purpose; (4) data conversion for the distribution of benefits after the value, etc. Precision medicine is a scientific and technological activity that benefits both individuals and humankind in general. The development of precision medicine must resolve the contradiction between data sharing and privacy protection. The key to solving this problem is to establish a biomedical big data management mechanism of sharing, joint responsibility, and joint governance, so as to lead the development of precision medicine in the direction of rigorous regulation, scientific order, effective implementation, and proper supervision. On the one hand, it is necessary to strengthen the sense of responsibility and ethics of scientific researchers; on the other hand, it is necessary to strengthen the supervision and governance responsibilities of institutions. On the basis of ensuring the security of personal data, the public is encouraged to donate and contribute personal biomedical data in the spirit of solidarity, benevolence, and mutual aid. To this end, this paper puts forward the following principles: First, respect independent decision-making and encourage data sharing. It must be clear that individuals are the subjects of rights to their biomedical data. At the same time, the public is encouraged to actively participate in precision medicine cohort studies and actively share personal data. Second, protect privacy and balance risks and benefits. While ensuring the sharing of biomedical data, no one shall infringe on personal privacy and endanger the safety of individuals and related groups. To this end, it is necessary to develop and construct privacy and security protection information technology and policies, laws and regulations corresponding to precision medicine from multiple dimensions such as technology, ethics, policy and law. Third, to strengthen shared responsibility and strengthen institutional governance, it is necessary to strengthen moral education and ethical training for practitioners involved in biomedical data, not only to make relevant personnel familiar with and master relevant national laws and regulations, but also to strengthen the privacy protection responsibility of relevant personnel. Those who intentionally leak or even maliciously sell and transfer other people's biomedical data should be held accountable legally and administratively.

Abstract:The practice and development of modern medicine requires large amounts of data, particularly in the domain of cancer. The future of personalized medicine lies neither with “genomic medicine” nor with “precision medicine”, but with “data medicine” (DM) (big data, data mining). The establishment of this DM has required far-reaching changes, to establish four essential elements connecting patients and doctors: biobanks, databases, bioinformatic platforms and genomic platforms. The “transformation” of scientific research areas, such as genetics, bioinformatics and biostatistics, into clinical specialties has generated a new vision of care. Molecular tumor boards (MTB) are one response to these changes and are now providing better access to next-generation sequencing (NGS) and new cancer treatments to patients with inoperable or metastatic cancers, and those for whom the usual treatment has failed. However, MTB face a crucial ethical challenge: maintaining and improving the trust of patients, clinicians, researchers and industry in academic medical centers supported by private or public funding rather than providing genetic data directly to private companies. We believe that, in this era of DM, appropriate modern digital communication networks will be required to maintain this trust and to improve the organization and effectiveness of the system. There is, therefore, a need to reconsider the form and content of informed consent (IC) documents at all academic medical centers and to introduce dynamic and electronic informed consent (e-IC).

Abstract:<section><section><p>Medicine has long sought to match diagnostic and treatment approaches to the particular needs and risks of individual patients. The decreasing cost and increasing ease of genetic sequencing have propelled the rise of precision medicine. Precision medicine aims to use genetic and other information to provide care tailored to the individual patient, with the goal of improving clinical outcomes and minimizing unnecessary diagnostic and therapeutic interventions. Although developments in genetic sequencing have the potential to transform clinical care, there are important limitations, including uncertainty in the clinical interpretation of many genetic variants and concerns about privacy, discrimination, and cost.</p><p>To help clinicians understand the basics of genetic sequencing and how to apply it in clinical practice, is launching a new "Precision Medicine" series. This introduction provides a general overview of clinical sequencing, with a focus on germline variation. Subsequent articles will use a case-based format to provide concise summaries of specific clinical precision medicine scenarios that are relevant to the practice of internal medicine. These cases will highlight specific clinical indications; interpretation of genetic test results; and ethical, legal, cost, and privacy issues related to genetic testing. The goal is to provide practical information on the appropriate application and interpretation of genomics in routine clinical practice.</p></section></section>

Abstract:Over the last decade, the study of genomes has rapidly advanced to the point that genomic research now serves as the basis for many medical decisions and public health initiatives. Genetic variation is likely to contribute substantially to the variation in drug response observed across human populations. Genomic tools such as sequence variation and, more specifically, personal genome sequencing incorporating genome-wide association studies and next-generation sequences enable the precise prediction and treatment of disease. At present, DNA-based risk assessment for common complex diseases, application of molecular signatures and dose selection of therapeutic drugs are the important issues in personalized medicine. In order to make personalized medicine effective, these genomic techniques must be standardized and integrated into health systems and clinical workflow for prediction of disease incidence, genetic and environmental information to optimize the medical care and outcomes for each patient. Technology continues to lead the field of personalized medicine since the interpretation of the human genome is progressing as the cost and duration of genomic sequencing continue to decrease sharply. This review aimed at understanding the technologies that reveal how the changes that occur within the genome can alter their functions and the genomic variations that constitute individual susceptibility to diseases and responses to therapy.

Abstract:Machine learning can have major societal impact in computational biology applications. In particular, it plays a central role in the development of precision medicine, whereby treatment is tailored to the clinical or genetic features of the patient. However, these advances require collecting and sharing among researchers large amounts of genomic data, which generates much concern about privacy. Researchers, study participants and governing bodies should be aware of the ways in which the privacy of participants might be compromised, as well as of the large body of research on technical solutions to these issues. We review how breaches in patient privacy can occur, present recent developments in computational data protection, and discuss how they can be combined with legal and ethical perspectives to provide secure frameworks for genomic data sharing.

Patients' perspectives related to ethical issues and risks in precision medicine: a systematic review

Abstract:Background: Precision medicine is growing due to technological advancements including next generation sequencing techniques and artificial intelligence. However, with the application of precision medicine many ethical and potential risks may emerge. Although, its benefits and potential harms are relevantly known to professional societies and practitioners, patients' attitudes toward these potential ethical risks are not well-known. The aim of this systematic review was to focus on patients' perspective on ethics and risks that may rise with the application of precision medicine. Methods: A systematic search was conducted on 4/1/2023 in the database of PubMed, for the period 1/1/2012 to 4/1/2023 identifying 914 articles. After initial screening, only 50 articles were found to be relevant. From these 50 articles, 24 articles were included in this systematic review, 2 articles were excluded as not in English language, 1 was a review, and 23 articles did not include enough relevant qualitative data regarding our research question to be included. All full texts were evaluated following PRISMA guidelines for reporting systematic reviews following the Joanna Briggs Institute criteria. Results: There were eight main themes emerging from the point of view of the patients regarding ethical concerns and risks of precision medicine: privacy and security of patient data, economic impact on the patients, possible harms of precision medicine including psychosocial harms, risk for discrimination of certain groups, risks in the process of acquiring informed consent, mistrust in the provider and in medical research, issues with the diagnostic accuracy of precision medicine and changes in the doctor-patient relationship. Conclusion: Ethical issues and potential risks are important for patients in relation to the applications of precision medicine and need to be addressed with patient education, dedicated research and official policies. Further research is needed for validation of the results and awareness of these findings can guide clinicians to understand and address patients concerns in clinical praxis.

Abstract:INTRODUCTION: Next-generation sequencing (NGS) is expected to revolutionize health care. NGS allows for sequencing of the whole genome more cheaply and quickly than previous techniques. NGS offers opportunities to advance medical diagnostics and treatments, but also raises complicated ethical questions that need to be addressed.AREAS CONSIDERED: This article draws from the literature on research and clinical ethics, as well as next-generation sequencing, in order to provide an overview of the ethical challenges involved in next-generation sequencing. This article includes a discussion of the ethics of NGS in research and clinical contexts.EXPERT OPINION: The use of NGS in clinical and research contexts has features that pose challenges for traditional ethical frameworks for protecting research participants and patients. NGS generates massive amounts of data and results that vary in terms of known clinical relevance. It is important to determine appropriate processes for protecting, managing and communicating the data. The use of machine learning for sequencing and interpretation of genomic data also raises concerns in terms of the potential for bias and potential implications for fiduciary obligations. NGS poses particular challenges in three main ethical areas: privacy, informed consent, and return of results.

Abstract:Precision medicine is making an impact on patients, health care delivery systems, and research participants in ways that were only imagined fifteen years ago when the human genome was first sequenced. Discovery of disease-causing and drug-response genetic variants has accelerated, while adoption into clinical medicine has lagged. We define precision medicine and the stakeholder community required to enable its integration into research and health care. We explore the intersection of data science, analytics, and precision medicine in the formation of health systems that carry out research in the context of clinical care and that optimize the tools and information used to deliver improved patient outcomes. We provide examples of real-world impact and conclude with a policy and economic agenda necessary for the adoption of this new paradigm of health care both in the United States and globally.

Abstract:Precision medicine is being developed today within a complex landscape of public policy, science, economics, law, and regulation. Important features of this landscape include health care financing, patent protection for new innovations, regulatory approvals of tests and therapies, patient consent and access, and protection of privacy. In these and other policy areas, the goal of developing individually-tailored therapies poses novel challenges for health care research, delivery and policy. In December 2015, the University of Utah School of Medicine, Huntsman Cancer Institute and S.J. Quinney College of Law convened a two-day symposium at which national experts in genetics, medicine, bioinformatics, intellectual property, regulatory science, health economics and bioethics discussed and debated many of the pressing questions raised by precision medicine. The key arguments, conclusions and unresolved issues that emerged from the symposium are summarized in these proceedings.

Abstract:Developments in genetics, pharmacology, biomarker identification, imaging, and interventional biotechnology are enabling medicine to become increasingly more precise in "personalized" approaches to assessing and treating individual patients. Here we describe current scientific and technological developments in precision medicine and elucidate the dual-use risks of employing these tools and capabilities to exert disruptive influence upon human health, economics, social structure, military capabilities, and global dimensions of power. Abstract:Tools and methods of precision medicine are developing rapidly, through both iterative discoveries enabled by innovations in biomedical research (e.g., genome editing, synthetic biology, bioengineered devices). These are strengthened by advancements in information technology and the increasing body of data—as assimilated, analyzed, and made accessible—and affectable—through current and emerging cyber—and systems- technologies. Taken together, these approaches afford ever greater volume and availability of individual and collective human data. Machine learning and/or artificial intelligence approaches are broadening this dual use risk; and in the aftermath of COVID-19, there is growing incentive and impetus to gather more biological data from individuals and their environments on a routine basis. By engaging these data—and the interventions that are based upon them, precision medicine offer promise of highly individualized treatments for disease and injury, optimization of structure and function, and concomitantly, the potential for (mis) using data to incur harm. This double-edged blade of benefit and risk obligates the need to safeguard human data from purloinment, through systems, guidelines and policies of a novel discipline, cyberbiosecurity, which, as coupled to ethical precepts, aims to protect human privacy, agency, and safety in ways that remain apace with scientific and technological advances in biomedicine. Herein, current capabilities and trajectories precision medicine are described as relevant to their dual use potential, and approaches to biodata security ( viz. - cyberbiosecurity) are proposed and discussed.

Improving Expanded Access in the United States: The Role of the Institutional Review Board

Abstract:BACKGROUND: The FDA allows patients with a serious or immediately life-threatening illness to use investigational medical products outside of clinical trials through its "expanded access" program. In response to criticism that the process to apply for expanded access is too onerous, numerous changes have been made over the last few years. These have been largely focused on the FDA and the pharmaceutical industry, while institutional review boards (IRBs)-which must approve expanded access protocols, except in emergencies when there is not time to do so-have remained relatively unstudied. We conducted a pilot study to review a sample of publicly available IRB policies from the United States to investigate how these entities handle expanded access.METHODS: We performed an online search to find publicly available policies for IRBs operating in the United States, utilizing a convenience sampling strategy, selecting the first 100 eligible policies we identified.RESULTS: Of the 95 policies reviewed, the majority (92.6%, n = 88) contained language referencing nonemergency expanded access and/or expanded access for emergency requests for a single patient. Of these 88 policies, 11.4% (n = 19) did not explicitly specify detailed procedures for handling nonemergency single-patient expanded access requests. Of the 88 policies that mentioned expanded access in nonemergency situations, 11.5% did not explicitly specify whether full IRB review was required, as was the rule at that time. There was considerable variation in other aspects of these policies, including charging patients for use of investigational products and the use of data from expanded access.CONCLUSIONS: Based on the findings of our pilot, IRB policies on expanded access vary considerably. It is often difficult to find, interpret, and understand IRB policies on expanded access.
