Abstract:Listeria monocytogenes remains a significant public health threat, causing invasive listeriosis manifested as septicemia, meningitis, and abortion, with up to 30% of cases having a fatal outcome. Tracking the spread of invasive listeriosis requires an updated knowledge for virulence factors (VFs) and antimicrobial resistance features, which is an essential step toward its clinical diagnosis and treatment. Taking advantage of high-throughput genomic sequencing, we proposed that the differential genes based on the pathogenomic composition could be used to evaluate clinical observations and therapeutic options for listeriosis. Here, we performed the comparative genomic analysis of 60 strains from five continents with a diverse range of sources, representing serotypes 1/2a, 1/2b, 1/2c, and 4b, comprising lineage I and lineage II and including 13 newly contributed Chinese isolates from clinical cases. These strains were associated with globally distributed clonal groups linked with confirmed foodborne listeriosis outbreak and sporadic cases. We found that L. monocytogenes strains from clonal complex (CC) CC8, CC7, CC9, and CC415 carried most of the adherence and invasive genes. Conversely, CC1, CC2, CC4, and CC6 have the least number of adherence and invasive genes. Additionally, Listeria pathogenicity island-1 (LIPI-1), LIPI-2, intracellular survival, surface anchoring, and bile salt resistance genes were detected in all isolates. Importantly, LIPI-3 genes were harbored in CC3, CC224, and ST619 of the Chinese isolates and in CC1, CC4, and CC6 of other worldwide isolates. Notably, Chinese isolates belonging to CC14 carried antibiotic resistance genes (ARGs) against β-lactams (bla TEM-101 , bla TEM-105) and macrolide (ermC-15), whereas CC7 and CC8 isolates harbored ARGs against aminoglycoside (aadA10_2, aadA6_1), which may pose a threat to therapeutic efficacy. Phylogenomic analysis showed that CC8, CC7, and CC5 of Chinese isolates, CC8 (Swiss and Italian isolates), and CC5 and CC7 (Canadian isolates) are closely clustered together and belonged to the same CC. Additionally, CC381 and CC29 of Chinese isolates shared the same genomic pattern as CC26 of Swiss isolate and CC37 of Canadian isolate, respectively, indicating strong phylogenomic relation between these isolates. Collectively, this study highlights considerable clonal diversity with well-recognized virulence and antimicrobial-resistant determinants among Chinese and worldwide isolates that stress to design improved strategies for clinical therapies.

Abstract:We fabricated ultrathin HfO2 gate stacks of very high permittivity by atomic layer deposition (ALD) and novel two-step post-deposition annealing (PDA) technique. First, no-cap PDA degasses residual contaminations in ALD layer, and second, Ti-cap PDA enhances permittivity of HfO2 by generating cubic crystal phase without SiO2 interfacial layer growth. Using these techniques, the dielectric constant of the ALD-HfO2 can be enhanced to ~40, and a 0.3 nm equivalent oxide thickness is obtained.

Abstract:Listeriosis is a severe disease with high mortality rate, especially in immunosuppressed individuals. The causative organism Listeria monocytogenes is primarily transmitted to humans through contaminated foods. To gain an understanding of the prevalence of Listeria in Chinese food products, we reviewed relevant papers from journals published in China from 2000 to 2007. The average recovery rate of Listeria spp. was 3.7% (0.1-7.7%) in all food categories in 13 provinces, with raw meat being the leading source. L. innocua (28.9%, 271/937) and L. monocytogenes (25.3%, 237/937) were more commonly isolated, both at higher proportion in all food types. Subtyping schemes in three laboratories in different provinces revealed that the majority of the L. monocytogenes isolates belonged to lineage II (67.1%), followed by lineage I at 31.6%, including the pathogenic serovars 1/2a, 1/2b, and 4b isolates. Lineage III isolates comprising the low-pathogenic serovar 4a were rare. Knowledge of the prevalence of Listeria in various food products in different regions of China may be useful for developing intervention strategies for control of contaminations along the production chains.

Abstract:The prevalence of listeriosis in China has been increasing in recent years. Listeriosis primarily spreads through contaminated food. However, the resilient causative organism, Listeria monocytogenes , and its extended incubation period pose challenges in identifying risk factors associated with food consumption and food-handling habits. This study aimed to identify the risk factors associated with food consumption and food-handling habits for listeriosis in China. A matched case–control study (1:1 ratio) was conducted, which enrolled all eligible cases of listeriosis between 1 January 2013 and 31 December 2022 in China. Basic information and possible risk factors associated with food consumption and food-handling habits were collected. Overall, 359 patients were enrolled, including 208 perinatal and 151 non-perinatal cases. Univariate and multivariable logistic analyzes were performed for the perinatal group. For the perinatal and non-perinatal groups, ice cream and Chinese cold dishes were the high-risk foods for listeriosis (odds ratio (OR) 2.09 95% confidence interval (CI): 1.23–3.55; OR 3.17 95% CI: 1.29–7.81), respectively; consumption of leftovers and pet ownership were the high-risk food-handling habits (OR 1.92 95% CI: 1.03–3.59; OR 3.00 95% CI: 1.11–8.11), respectively. In both groups, separation of raw and cooked foods was a protective factor (OR 0.27 95% CI: 0.14–0.51; OR 0.35 95% CI: 0.14–0.89), while refrigerator cleaning reduced the infection risk by 64.94–70.41% only in the perinatal group. The identification of high-risk foods and food-handling habits for listeriosis is important for improving food safety guidelines for vulnerable populations.

Abstract:In the current world of global food distribution and marketing, the outbreak of a particular foodborne disease in a region may eventually occur in another part of the world at the same time. Foodborne diseases are estimated to cause over 600 million illnesses and 420,000 deaths each year. Listeriosis, a zoonotic bacterial disease caused by Listeria spp. has been reported in various parts of the world’s disease outbreak and incidence. The highest number of diseases and outbreaks is caused by Listeria monocytogenes and is highly predominant among elderly persons, immunocompromised individuals, pregnant women, and infants. This bacterium possesses the ability to survive and multiply under refrigeration temperature, a wide range of pH (4.1 - 9.6), and high salt (≤ 10%). The disease can be minimal (non-invasive/febrile gastroenteritis) and the pathogen possesses the ability to cross the epithelium and infect sterile organs (invasive). Outside food contamination, resistance to antibiotics by Listeria monocytogenes poses major public health concerns as this bacterium has developed resistance to various antibiotics in use. In addition to food, Listeria species have been isolated from the soil, water, plants, fruits, and vegetables coupled with the long incubation period (11 - 70 days), which poses a challenge to infection source identification and trace back principle. Controlling Listeria in foods can be achieved by practicing a Hazard Analysis and Critical Control Point (HACCP) strategy in the food industry, improving hygiene measures, and avoiding the consumption of contaminated foods. Also, susceptible individuals should be educated on the risks associated with the consumption of contaminated foods.

Abstract:In this study, we examined Listeria monocytogenes isolates from Chinese food sources in an attempt to gain further insights on the molecular characteristics and virulence potential of this important foodborne pathogen. Of the 88 L. monocytogenes food isolates recovered, 42 (47.7%) were of serovars 1/2a or 3a; 23 (26.1%) of serovars 1/2b or 3b; 15 (17.0%) of 1/2c or 3c; 6 (6.8%) of serovars 4b, 4d or 4e; and 2 (2.2%) of serovars 4a or 4c. In contrast to inlAB locus conserved in all serovars, internalin cluster between ascB and dapE varies with different serovars, with inlC2DE, inlGC2DE and inlGHE predominantly in serovars 1/2b or 4b, serovar 1/2a and serovar 1/2c. While inlF existed in all the inlGHE- and inlGC2DE-containing isolates but 17.4% of those having inlC2DE, lmo2026 existed in all the inlGHE-containing isolates but 20.0% of those bearing inlGC2DE, suggesting that inlF might have co-evolved with inlGC2DE and inlGHE while lmo2026 with inlGHE only. With the exception of serovar 4a isolate, most serovar isolates demonstrated remarkable ability to form plaques on L929 cells and produced significant mouse mortality irrespective of the internalin gene organization and whether an intact actA gene is present or not. These results indicate that majority of these food isolates may have the potential to cause human diseases if ingested via contaminated foods. Given that serovar 4b accounts for nearly half of human clinical listeriosis cases documented, the relative low proportion of serovar 4b food isolates suggests that this serovar is probably more tolerant of the adverse conditions in the host's stomach and/or more efficient in entering host cells than serovars 1/2a, 1/2b and 1/2c.

Abstract:Over the past few years, a microsurgical anterior foraminotomy technique has been developed by the author and used to achieve spinal cord decompression for the treatment of cervical spondylotic myelopathy. A 5 x 8-mm unilateral anterior foraminotomy is accomplished by resecting the uncovertebral joint via an anterior approach. Through the foraminotomy hole, the posterior osteophytes at the spinal cord canal are removed diagonally up to the beginning of the contralateral nerve root. To treat multilevel disease, a tunnel is made among the foraminotomy holes. This technique accomplishes widening of the spinal cord canal in the transverse and longitudinal axes by direct resection of the compressive lesions through the holes of unilateral anterior foraminotomies; however, it does not require bone fusion or postoperative immobilization. Postoperatively patients remain in the hospital overnight, and do not need to wear cervical braces. This new surgical technique has shown excellent clinical outcomes with fast recovery and adequate anatomical decompression in patients with cervical spondylotic myelopathy. The surgical technique is reported and illustrated by two of the author's cases.
Human foodborne listeriosis in England and Wales, 1981 to 2015

Abstract:Almost all cases of human listeriosis are foodborne, however the proportion where specific exposures are identified is small. Between 1981 and 2015, 5252 human listeriosis cases were reported in England and Wales. The purpose of this study was to summarise data where consumption of specific foods was identified with transmission and these comprised 11 sporadic cases and 17 outbreaks. There was a single outbreak in the community of 378 cases (7% of the total) which was associated with pâté consumption and 112 cases (2% of the total) attributed to specific foods in all the other incidents. The proportion of food-attributed cases increased during this study with improvements in typing methods for Listeria monocytogenes. Ten incidents (one sporadic case and nine outbreaks of 2-9 cases over 4 days to 32 months) occurred in hospitals: all were associated with the consumption of pre-prepared sandwiches. The 18 community incidents comprised eight outbreaks (seven of between 3 and 17 cases) and 10 sporadic cases: food of animal origin was implicated in 16 of the incidents (sliced or potted meats, pork pies, pâté, liver, chicken, crab-meat, butter and soft cheese) and food of non-animal origin in the remaining two (olives and vegetable rennet).

Abstract:Listeria monocytogenes is a food-borne pathogen, typically affecting the elderly, immunocompromised patients and pregnant women. The aim of this study was to determine the population structure of L. monocytogenes clonal complex 1 (CC1) in the UK and describe the genomic epidemiology of this clinically significant CC. We interrogated a working dataset of 4073 sequences of L. monocytogenes isolated between January 2015 and December 2020 from human clinical specimens, food and/or food-production environments. A minimum spanning tree was reconstructed to determine the population structure of L. monocytogenes in the UK. Subsequent analysis focused on L. monocytogenes CC1, as the cause of the highest proportion of invasive listeriosis in humans. Sequencing data was integrated with metadata on food and environmental isolates, and information from patient questionnaires, including age, sex and clinical outcomes. All isolates either belonged to lineage I ( n =1299/4073, 32%) or lineage II ( n =2774/4073, 68%), with clinical isolates from human cases more likely to belong to lineage I ( n =546/928, 59%) and food isolates more likely to belong to lineage II ( n =2352/3067, 77%). Of the four largest CCs, CC1 ( n =237) had the highest proportion of isolates from human cases of disease (CC1 n =160/237, 67.5 %; CC121 n =13/843, 2 %; CC9 n =53/360, 15 %; CC2 n =69/339, 20%). Within CC1, most cases were female ( n =95/160, 59%, P =0.01771) and the highest proportion of cases were in people >60 years old (39/95, 41%, P =1.314×10 −6 ) with a high number of them aged 20–39 years old ( n =35/95, 37%) most linked to pregnancy-related listeriosis ( n =29/35, 83%). Most of the male cases were in men aged over 60 years old (40/65, 62%), and most of the fatal cases in both males and females were identified in this age group (42/55, 76%). Phylogenetic analysis revealed 23 5 SNP single linkage clusters comprising 80/237 (34 %) isolates with cluster sizes ranging from 2 to 19. Five 5 SNP clusters comprised isolates from human cases and an implicated food item. Expanding the analysis to 25 SNP single linkage clusters resolved an additional two clusters linking human cases to a potential food vehicle. Analysis of demographic and clinical outcome data identified CC1 as a clinically significant cause of invasive listeriosis in the elderly population and in women of child-bearing age. Phylogenetic analysis revealed the population structure of CC1 in the UK comprised small, sparsely populated genomic clusters. Only clusters containing isolates from an implicated food vehicle, or food processing or farming environments, were resolved, emphasizing the need for clinical, food and animal-health agencies to share sequencing data in real time, and the importance of a One Health approach to public-health surveillance of listeriosis.

Abstract:Better knowledge regarding the Listeria monocytogenes dose-response (DR) model is needed to refine the assessment of the risk of foodborne listeriosis. In 2018, the European Food Safety Agency (EFSA) derived a lognormal Poisson DR model for 14 different age-sex sub-groups, marginally to strain virulence. In the present study, new sets of parameters are developed by integrating the EFSA model for these sub-groups together with three classes of strain virulence characteristics ("less virulent", "virulent", and "more virulent"). Considering classes of virulence leads to estimated relative risks (RRs) of listeriosis following the ingestion of 1000 bacteria of "less virulent" vs. "more virulent" strains ranging from 21.6 to 24.1, depending on the sub-group. These relatively low RRs when compared with RRs linked to comorbidities described in the literature suggest that the influence of comorbidity on the occurrence of invasive listeriosis for a given exposure is much more important than the influence of the virulence of the strains. The updated model parameters allow better prediction of the risk of invasive listeriosis across a population of interest, provided the necessary data on population demographics and the proportional contribution of strain virulence classes in food products of interest are available. An R package is made available to facilitate the use of these dose-response models.

Abstract:The epidemiology of listeriosis in England and Wales changed during 2001-2008; more patients ≥60 years of age had bacteremia than in previous years. To investigate these changes, we calculated risk for listeriosis by concurrent condition for non-pregnancy-associated listeriosis cases reported to the national surveillance system in England during 1999-2009. Conditions occurring with L. monocytogenes infection were coded according to the International Classification of Diseases, 10th Revision, and compared with appropriate hospital episode statistics inpatient denominator data to calculate incidence rates/million consultations. Malignancies (especially of the blood), kidney disease, liver disease, diabetes, alcoholism, and age ≥60 years were associated with an increased risk for listeriosis. Physicians should consider a diagnosis of listeriosis when treating patients who have concurrent conditions. Providing cancer patients, who accounted for one third of cases, with food safety information might help limit additional cases.

Abstract:The risk assessment study of Listeria monocytogenes in ready-to-eat foods conducted by the U.S. Food and Drug Administration is an example of an extensive quantitative microbiological risk assessment that could be used by risk analysts and other scientists to obtain information and by managers and stakeholders to make decisions on food safety management. The present study was conducted to investigate how detailed sensitivity analysis can be used by assessors to extract more information on risk factors and how results can be communicated to managers and stakeholders in an understandable way. The extended sensitivity analysis revealed that the extremes at the right side of the dose distribution (at consumption, 9 to 11.5 log CFU per serving) were responsible for most of the cases of listeriosis simulated. For concentration at retail, values below the detection limit of 0.04 CFU/g and the often used limit for L. monocytogenes of 100 CFU/g (also at retail) were associated with a high number of annual cases of listeriosis (about 29 and 82%, respectively). This association can be explained by growth of L. monocytogenes at both average and extreme values of temperature and time, indicating that a wide distribution can lead to high risk levels. Another finding is the importance of the maximal population density (i.e., the maximum concentration of L. monocytogenes assumed at a certain temperature) for accurately estimating the risk of infection by opportunistic pathogens such as L. monocytogenes. According to the obtained results, mainly concentrations corresponding to the highest maximal population densities caused risk in the simulation. However, sensitivity analysis applied to the uncertainty parameters revealed that prevalence at retail was the most important source of uncertainty in the model.
Vocalization characteristics of North Atlantic right whale surface active groups in the calving habitat, southeastern United States.

Abstract:Passive acoustic surveys were conducted to assess the vocal behavior of North Atlantic right whales (Eubalaena glacialis) in the designated critical calving habitat along the shallow coastal waters of southeastern United States. Underwater vocalizations were recorded using autonomous buoys deployed in close proximity to surface active groups (SAGs). Nine main vocalization types were identified with manual inspection of spectrograms, and standard acoustic descriptors were extracted. Classification trees were used to examine the distinguishing characteristics of calls and quantify their variability within the SAG vocal repertoire. The results show that descriptors of frequency, bandwidth, and spectral disorder are the most important parameters for partitioning the SAG repertoire, contrary to duration-related measures. The reported source levels and vocalization statistics provide sound production data vital to inform regional passive acoustic monitoring and conservation for this endangered species.
Clinical Characteristics and Fatality Risk Factors for Patients with Listeria monocytogenes Infection: A 12-Year Hospital-Based Study in Xi'an, China

Abstract:Introduction: Listeriosis is a severe food-borne disease caused by Listeria monocytogenes infection. The data of listeriosis in Xi'an population are limited. The aim of this study is to evaluate the clinical features and fatality risk factors for listeriosis in three tertiary-care hospitals in Xi'an, China METHODS: The characteristics of demographic data, underlying diseases, clinical manifestations, laboratory indicators, cranial imaging examination, antibiotics therapeutic schemes, and clinical outcomes were collected between 2011 and 2023. Logistic regression analysis was performed. Results: Seventy-one etiologically confirmed listeriosis patients were enrolled, including 12 neonatal and 59 non-neonatal cases. The majority of neonatal listeriosis presented as preterm (50%) and fetal distress (75%). The main clinical manifestations of non-neonatal listeriosis included fever (88%), headache (32%), disorder of consciousness (25%), vomiting (17%), abdominal pain (12%), and convulsions (8%). The fatality rate in neonatal cases was higher than in non-neonatal listeriosis (42 vs. 17%). Although no deaths were reported in maternal listeriosis, only two of 23 patients had an uneventful obstetrical outcome. Five maternal listeriosis delivered culture-positive neonates, three of whom decreased within 1 week post-gestation due to severe complications. Twenty-eight cases were neurolisteriosis and 43 cases were bacteremia. Neurolisteriosis had a higher fatality rate compared with bacteremia listeriosis (36 vs. 12%). The main neuroradiological images were cerebral edema/hydrocephalus, intracranial infection, and cerebral hernia. Listeria monocytogenes showed extremely low resistance to ampicillin (two isolates) and penicillin (one isolate). The fatality risk factors were the involvement of the central nervous system, hyperbilirubinemia, and hyponatremia for all enrolled subjects. Hyperuricemia contributed to the elevation of fatality risk in non-neonatal listeriosis. Conclusions: When the patients suffered with symptoms of fever and central nervous system infection, they should be alert to the possibility of listeriosis. Early administration of ampicillin- or penicillin-based therapy might be beneficial for recovery of listeriosis.

The clinical features of BV were described in 1955 by Gardner & Dukes, who claimed that the condition was caused by infection with a hitherto unclassified bacterium which they named Haemophilus vaginalis, but which has since been re-classified as G. vaginalis (Greenwood & Pickett, 1980). Their conclusion that G. vaginalis was the cause of BV was based on their isolation of the organism from 92 % of patients with BV, but not from women without this condition. The role played by G. vaginalis in the aetiology of BV has become less clear, however, with the subsequent finding that it may be isolated from the normal vaginal flora if sensitive culture techniques are employed (Hill, Eschenbach & Holmes, 1984). Quantitative studies of the vaginal microflora have shown that there is a significant increase in the number of G. vaginalis organisms in the genital tract of women with BV compared to women not suffering from this complaint. However, as there is also a relative increase in the anaerobic vaginal flora and the number of Mycoplasma hominis organisms in women with BV (Hill, Eschenbach & Holmes, 1984), the role of G. vaginalis is still unclear. Chen and colleagues (1979) suggested that a symbiotic relationship existed in the vagina between G. vaginalis and other organisms associated with BV, since in vitro, G. vaginalis produced high concentrations of pyruvic and amino acids which could be metabolized by these organisms. Chen et al. (1979) also identified the various amines present in vaginal fluids from BV patients which probably account for the characteristic malodour and showed that they could be produced in vitro during the growth of mixed vaginal bacteria, but not by G. vaginalis alone. Clearly the signs and symptoms of BV correlate with significant changes in the ecology of the vaginal flora, but the role that G. vaginalis, anaerobic bacteria and M. hominis play in the initiation and development of the disease is still unclear. The finding that smears and biopsies obtained from women with BV show little or no evidence of vaginal inflammation or tissue damage tends to suggest that G. vaginalis and the other organisms associated with this condition are not invasive. Abstract:Listeriosis is a rare but severe foodborne illness which is more common in populations such as pregnant women, and can result in serious complications including miscarriage, prematurity, maternal and neonatal sepsis, and death in the newborn. Population recommendations exist for specific foods and food preparation practices to reduce listeriosis risk during pregnancy. The aim of the present systematic review was to assess the association between listeriosis and these practices during pregnancy to confirm appropriateness of these recommendations. We searched MEDLINE, Embase, CINAHL Plus, Web of Science Core Collection, included articles' references, and contacted clinical experts. All databases were searched until July 2017. Case-control and cohort studies were included which assessed pregnant women or their newborn offspring with known listeriosis status and a nutritional exposure consistent with international population recommendations for minimising listeriosis. Outcomes included listeriosis with or without pregnancy outcomes. Risk of bias was assessed through the Newcastle-Ottawa Scale. Results were described narratively due to clinical heterogeneity in differences in nutritional exposures. Eleven articles comprising case-control or cross-sectional studies met the inclusion criteria. Cases of maternal, fetal or neonate listeriosis were more likely to have consumed high-risk dairy products, meat products or some fruits during pregnancy in comparison with women without listeriosis. Cases of listeriosis were more likely to have consumed foods that are highlighted in population guidelines to avoid to minimise listeriosis in comparison with those without listeriosis during pregnancy. Further research is warranted assessing means of improving the reach, uptake and generalisability of population guidelines for reducing listeriosis during pregnancy.
Comparing Individual and Community-level Characteristics of People with Ground Beef-associated Salmonellosis and Other Ground Beef Eaters: A Case-control Analysis

Abstract:Salmonella is estimated to be the leading bacterial cause of U.S. domestically acquired foodborne illness. Large outbreaks of Salmonella attributed to ground beef have been reported in recent years. The demographic and sociodemographic characteristics of infected individuals linked to these outbreaks are poorly understood. We employed a retrospective case-control design; case-patients were people with laboratory-confirmed Salmonella infections linked to ground beef-associated outbreaks between 2012 and 2019, and controls were respondents to the 2018-2019 FoodNet Population Survey who reported eating ground beef and denied recent gastrointestinal illness. We used county-level CDC/ATSDR Social Vulnerability Index (SVI) to compare case-patient and controls. Case-patient status was regressed on county-level social vulnerability and individual-level demographic characteristics. We identified 376 case-patients and 1,321 controls in the FoodNet sites. Being a case-patient was associated with increased overall county-level social vulnerability (OR: 1.21 [95% CI: 1.07-1.36]) and socioeconomic vulnerability (OR: 1.24 [1.05-1.47]) when adjusted for individual-level demographics. Case-patient status was not strongly associated with the other SVI themes of household composition and disability, minority status and language, and housing type and transportation. Data on individual-level factors such as income, poverty, unemployment, and education could facilitate further analyses to understand this relationship.
Epidemiology of Human Listeriosis in China During 2008-2017

Abstract:Listeria monocytogenes is a facultative, intracellular foodborne pathogen that causes listeriosis and is prevalent worldwide. However, our knowledge of this bacterium and the listeriosis it causes is still extremely limited until now. Therefore, this retrospective study of patients in mainland China over 10 years (2008-2017) was performed to better understand the demographic trends and clinical features of listeriosis in China. Both electronic and manual retrieval systems were used to collect the relevant literature on listeriosis in mainland China. A total of 759 cases were reported from 22 provinces. Among the clinical cases, septicemia was the most common presentation (49%), followed by central nervous system infection (25%). The overall case fatality rate was 18%, with a higher rate among neonatal patients (73%). In recent years, listeriosis has been reported annually and even peaked in 2014. The median age of nonperinatal cases was 36 years (range, 0-102), with a predominance of male cases (52%). Sporadic cases were frequent from March to May. Efforts to prevent and control the spread of listeriosis are required through further research and collaborative efforts to improve the capacities of clinical diagnosis and treatment.
Intralesional Autologous Blood Injection Compared to Corticosteroid Injection for Treatment of Chronic Plantar Fasciitis. A Prospective, Randomized, Controlled Trial

Abstract:Background: The response of chronic plantar fasciitis to any treatment is unpredictable. Autologous blood might provide cellular and humoral mediators to induce healing in areas of degeneration, the underlying pathology in plantar fasciitis. This study compared the efficacy of intralesional autologous blood with corticosteroid injection for plantar fasciitis present for more than 6 weeks. Methods: A prospective, randomized, controlled, observer-blinded study was done over a period of 6 months. Sixty-four patients were randomly allocated to either the autologous blood or corticosteroid treatment group. All patients were assessed for the worst pain daily on visual analogue scale (VAS) and tenderness threshold (TT) at the plantar fascia origin using a pressure algometer before treatment, and at 6 weeks, 3 months, and 6 months after treatment. A p value of 0.05 was considered significant. Results: Data were complete for 61 patients. The reduction in VAS and increase in TT for both groups was significant over time (p < 0.0001). At 6 weeks and 3 months, the corticosteroid group had significantly lower VAS than the autologous blood group (p < 0.011 and p < 0.005, respectively), but the difference was not significant at 6 months. The corticosteroid group had significantly higher TT than the autologous blood group at 6 weeks, 3 months and 6 months (p < 0.003, p < 0.003, p < 0.008, respectively). Although the trends were different, repeated-measures F test of both VAS and TT showed no significant difference in improvement between the groups over time. Conclusions: Intralesional autologous blood injection is efficacious in lowering pain and tenderness in chronic plantar fasciitis, but corticosteroid is more superior in terms of speed and probably extent of improvement.
