Abstract:Sensory substitution devices (SSDs) such as the 'vOICe' preserve visual information in sound by turning visual height, brightness, and laterality into auditory pitch, volume, and panning/time respectively. However, users have difficulty identifying or tracking multiple simultaneously presented tones - a skill necessary to discriminate the upper and lower edges of object shapes. We explore how these deficits can be addressed by using image-sonifications inspired by auditory scene analysis (ASA). Here, sighted subjects (N=25) of varying musical experience listened to, and then reconstructed, complex shapes consisting of simultaneously presented upper and lower lines. Complex shapes were sonified using the vOICe, with either the upper and lower lines varying only in pitch (i.e. the vOICe's 'unaltered' default settings), or with one line degraded to alter its auditory timbre or volume. Results found that overall performance increased with subjects' years of prior musical experience. ANOVAs revealed that both sonification style and musical experience significantly affected performance, but with no interaction effect between them. Compared to the vOICe's 'unaltered' pitch-height mapping, subjects had significantly better image-reconstruction abilities when the lower line was altered via timbre or volume-modulation. By contrast, altering the upper line only helped users identify the unaltered lower line. In conclusion, adding ASA principles to vision-to-audio SSDs boosts subjects' image-reconstruction abilities, even if this also reduces total task-relevant information. Future SSDs should seek to exploit these findings to enhance both novice user abilities and the use of SSDs as visual rehabilitation tools.
A Wearable Vision-To-Audio Sensory Substitution Device for Blind Assistance and the Correlated Neural Substrates

Abstract:There are a very few people who have the ability to "see" the surroundings by the echoes, which is called echolocation. The study of the brain mechanism of echolocation can not only help to improve the blind assistance device, but also provides a window into the research of brain's plasticity. In this paper, we developed a wearable system to transform the spatial information captured by camera into a voice description and fed it back to blind users which is inspired by echolocation. After our online virtual scene training, users can easily discriminate object location in the camera's view, motion of the objects, even shape of the objects. Compared with natural echolocation, it's easier to learn and be applied in daily life. In addition, the device achieves high spacial resolution. In this study, two trained blind subjects and two non-trained sighted subjects were tested by using functional Magnetic Resonance Imaging (fMRI). We obtain the fMRI images of the subjects' brain activity when they were listening to the sound of the wearable prototype. Intriguingly, we find that after training with the blind assistance system, the blind' visual area of the brain have been activated when they are dealing with the acoustic feedback from the device.
Downward Cross-Modal Plasticity in Single-Sided Deafness

Abstract:The auditory cortex has been shown to participate in visual processing in individuals with complete auditory deprivation. However, it remains unclear whether partial hearing deprivation like single-sided deafness (SSD) leads to similar cross-modal plasticity. To investigate this, we enrolled individuals with long-term SSD, into functional MRI scans under resting-state and a visuo-spatial working memory task. Contrary to previous findings in bilateral deafness, our study revealed decreased activation in the auditory cortex in both left (LSSD) and right (RSSD) single-sided deafness compared to normal hearing controls, with statistical significance in RSSD. The degree of involvement was correlated with residual hearing ability in RSSD. These observations suggest that SSD can lead to a downward cross-modal plasticity: the more hearing ability lost, the fewer brain resources in the auditory cortex can be applied to visual tasks. In addition, the fronto-parietal cortex was observed to be less activated during the visual task in RSSD while the resting-state fMRI revealed increased functional connectivity between the fronto-parietal cortex and the auditory cortex, suggesting fronto-parietal resources may be recruited less by vision but more by hearing. The LSSD showed a similar alteration trend with RSSD, but without statistical significance. Together these findings may indicate that when hearing is partially deprived in SSD, there may be redistribution for brain resources between hearing and vision, and vision tends to allocate less resources. Our findings in this pilot study of unilateral auditory-deprived individuals enrich the understanding of cross-modal plasticity in the brain.
Modulation of alpha5beta1 integrin functions by the phospholipid and cholesterol contents of cell membranes.

Abstract:Several modifications of the alpha5beta1 integrin, which alter its intracellular and extracellular interaction with fibronectin and other proteins, have been reported. However, the significance of the lateral mobility of integrin molecules in the plasma membrane, as a regulator of their distribution and function, is poorly understood. We examined this problem by increasing the cholesterol content of plasma membranes, and consequently modifying the fluidity of membrane phospholipids, in rat fibroblasts. Under these conditions, the clustering of alpha5beta1 integrin molecules in focal adhesions, their adhesion to the cell-binding domain of fibronectin, and their association with the cytoskeletal protein talin were significantly enhanced as compared to control cells. However, the activation of MAP-kinase pathways by the association of fibronectin with alpha5beta1 integrin, and its association with integrin-linked kinase (ilk), were suppressed. The treated cells also showed distinct changes in shape, and their actin stress fiber network was more dense and thick as compared to control cells. The changes in fluidity of phospholipids occurred differentially and fluidity of phosphatidyl-ethanolamine increased, while that of phosphatidyl-choline was reduced. Our results suggest that proteins in focal adhesions could be partitioned in specific lipid domains, which regulate specific aspects of alpha5beta1 integrin functions.
Effect of cochlear implant side on early speech processing in adults with single-sided deafness

Abstract:Objective: In binaurally deaf subjects, speech processing particularly benefits from a cochlear implant (CI) in the right ear, which is contralateral to the commonly left speech-dominant hemisphere. However, it is unclear whether such effects of implantation side also occur in speech processing in patients with single-sided deafness (SSD). Methods: Lateralization of N1 responses was analyzed with a high-density electroencephalogram (EEG) in fourteen adults with postlingually acquired left or right SSD who received a CI in adulthood. During recording, patients performed a speech and a pure-tone discrimination task. Lateralization of N1 responses was assessed by side-specific global field power (GFP) and compared (a) between normal hearing and CI-implanted ears within subjects and (b) between implantation sides across subjects. Results: N1 responses were stronger in the contralateral than in the ipsilateral hemisphere during stimulation of the normal hearing ear (except for left speech stimulation), and was bilateral symmetric with CI stimulation on either side. A significant correlation between speech discrimination performance and left lateralization was found across subjects for the left CI ear. Conclusions: CI stimulation altered auditory processing across hemispheres. Speech discrimination in left CI-implanted SSD patients improved with left lateralization of the N1 response. Significance: Side-specific rehabilitation in SSD patients might improve speech processing across hemispheres.
Immediate improvement of speech-in-noise perception through multisensory stimulation via an auditory to tactile sensory substitution

Abstract:Background: Hearing loss is becoming a real social and health problem. Its prevalence in the elderly is an epidemic. The risk of developing hearing loss is also growing among younger people. If left untreated, hearing loss can perpetuate development of neurodegenerative diseases, including dementia. Despite recent advancements in hearing aid (HA) and cochlear implant (CI) technologies, hearing impaired users still encounter significant practical and social challenges, with or without aids. In particular, they all struggle with understanding speech in challenging acoustic environments, especially in presence of a competing speaker. Objectives: In the current proof-of-concept study we tested whether multisensory stimulation, pairing audition and a minimal-size touch device would improve intelligibility of speech in noise. Methods: To this aim we developed an audio-to-tactile sensory substitution device (SSD) transforming low-frequency speech signals into tactile vibrations delivered on two finger tips. Based on the inverse effectiveness law, i.e., multisensory enhancement is strongest when signal-to-noise ratio is lowest between senses, we embedded non-native language stimuli in speech-like noise and paired it with a low-frequency input conveyed through touch. Results: We found immediate and robust improvement in speech recognition (i.e. in the Signal-To-Noise-ratio) in the multisensory condition without any training, at a group level as well as in every participant. The reported improvement at the group-level of 6 dB was indeed major considering that an increase of 10 dB represents a doubling of the perceived loudness. Conclusions: These results are especially relevant when compared to previous SSD studies showing effects in behavior only after a demanding cognitive training. We discuss the implications of our results for development of SSDs and of specific rehabilitation programs for the hearing impaired either using or not using HAs or CIs. We also discuss the potential application of such a set-up for sense augmentation, such as when learning a new language.

Abstract:Persons with single-sided deafness (SSD) typically complain about the impaired ability to locate sounds and to understand speech within background noise. However, the findings from previous studies suggest that paying attention to sounds could mitigate the degraded spatial and speech-in-noise perception. In the present study, we characterize the pattern of cortical activation depending on the side of deafness, and attentional modulation of neural responses to determine if it can assist better sound processing in people with SSD. For the active listening condition, adult subjects with SSD performed sound localization tasks. On the other hand, they watched movies without attending to speech stimuli during passive listening. The sensor-level global field power of N1 and source-level N1 activation were computed to compare the active- and passive-listening conditions and left- and right-sided deafness. The results show that attentional modulation differs depending on the side of deafness: active listening increased the cortical activity in individuals with left-sided deafness but not in those with right-sided deafness. At the source level, the attentional gain was more apparent in left-sided deafness in that paying attention enhanced brain activation in both hemispheres. In addition, SSD participants with larger cortical activities in the right primary auditory cortex had shorter durations of deafness. Our results indicate that the side of deafness can change top-down attentional processing in the auditory cortical pathway in SSD patients.

Abstract:The classical view of sensory processing involves independent processing in sensory cortices and multisensory integration in associative areas. This hierarchical structure has been challenged by evidence of multisensory responses in sensory areas, and dynamic weighting of sensory inputs in associative areas, thus far reported independently. Here, we used a visual-to-auditory sensory substitution algorithm (SSA) to manipulate the information conveyed by sensory inputs while keeping the stimuli intact. During scan sessions before and after SSA learning, subjects were presented with visual images and auditory soundscapes. The findings reveal 2 dynamic processes. First, crossmodal attenuation of sensory cortices changed direction after SSA learning from visual attenuations of the auditory cortex to auditory attenuations of the visual cortex. Secondly, associative areas changed their sensory response profile from strongest response for visual to that for auditory. The interaction between these phenomena may play an important role in multisensory processing. Consistent features were also found in the sensory dominance in sensory areas and audiovisual convergence in associative area Middle Temporal Gyrus. These 2 factors allow for both stability and a fast, dynamic tuning of the system when required.

Abstract:The development of computer vision based systems dedicated to help visually impaired people to perceive the environment, to orientate and navigate has been the main research subject of many works in the recent years. A significant ensemble of resources has been employed to support the development of sensory substitution devices (SSDs) and electronic travel aids for the rehabilitation of the visually impaired. The Sound of Vision (SoV) project used a comprehensive approach to develop such an SSD, tackling all the challenging aspects that so far restrained the large scale adoption of such systems by the intended audience: Wearability, real-time operation, pervasiveness, usability, cost. This article is set to present the artificial vision based component of the SoV SSD that performs the scene reconstruction and segmentation in outdoor environments. In contrast with the indoor use case, where the system acquires depth input from a structured light camera, in outdoors SoV relies on stereo vision to detect the elements of interest and provide an audio and/or haptic representation of the environment to the user. Our stereo-based method is designed to work with wearable acquisition devices and still provide a real-time, reliable description of the scene in the context of unreliable depth input from the stereo correspondence and of the complex 6 DOF motion of the head-worn camera. We quantitatively evaluate our approach on a custom benchmarking dataset acquired with SoV cameras and provide the highlights of the usability evaluation with visually impaired users.

Understanding Language, Hearing Status, and Visual-Spatial Skills

Abstract:It is frequently assumed that deaf individuals have superior visual-spatial abilities relative to hearing peers and thus, in educational settings, they are often considered visual learners. There is some empirical evidence to support the former assumption, although it is inconsistent, and apparently none to support the latter. Three experiments examined visual-spatial and related cognitive abilities among deaf individuals who varied in their preferred language modality and use of cochlear implants (CIs) and hearing individuals who varied in their sign language skills. Sign language and spoken language assessments accompanied tasks involving visual-spatial processing, working memory, nonverbal logical reasoning, and executive function. Results were consistent with other recent studies indicating no generalized visual-spatial advantage for deaf individuals and suggested that their performance in that domain may be linked to the strength of their preferred language skills regardless of modality. Hearing individuals performed more strongly than deaf individuals on several visual-spatial and self-reported executive functioning measures, regardless of sign language skills or use of CIs. Findings are inconsistent with assumptions that deaf individuals are visual learners or are superior to hearing individuals across a broad range of visual-spatial tasks. Further, performance of deaf and hearing individuals on the same visual-spatial tasks was associated with differing cognitive abilities, suggesting that different cognitive processes may be involved in visual-spatial processing in these groups.

Abstract:This chapter reviews the most recent advances in sensory substitution and the neural correlates of navigation in congenital blindness. Studies have established the superior ability of congenitally blind (CB) participants with the aid of Sensory Substitution Devices (SSDs) to navigate new environments and detect the size and shape of obstacles in order to avoid them. These studies suggest that with training, CB can achieve a representation of space that is equivalent to that of the sighted. From a phenomenological point of view, sensation and perception provided by SSDs have been likened to real vision, but the question remains as to the subjective sensations (qualia) felt by users. We review recent theories on the phenomenological properties of sensory substitution and the recent literature on spatial abilities of participants using SSDs. From these different sources of research, we conclude that training-induced plastic changes enable task-specific brain activations. The recruitment of the primary visual cortex by nonvisual SSD stimulations and, the subsequent activations of associative visual cortices in the congenitally blind, suggest that the sensory information is treated in an amodal fashion; i.e.,: in terms of the task being performed rather than the sensory modality. These anatomical changes enable the embodiment of nonvisual information allowing SSD users to accomplish a multitude of “visual” tasks. We will emphasize here the abilities of CB individuals to navigate in real and virtual environments in spite of a large volumetric reduction in the posterior segment of the hippocampus, a key area involved in navigation. In addition, the superior behavioral performance of CB in a variety of sensory and cognitive tasks, combined with anatomical and functional MRI, underlines the susceptibility of the brain to training-induced plasticity.
How Auditory Experience Differentially Influences the Function of Left and Right Superior Temporal Cortices

Abstract:To investigate how hearing status, sign language experience, and task demands influence functional responses in the human superior temporal cortices (STC) we collected fMRI data from deaf and hearing participants (male and female), who either acquired sign language early or late in life. Our stimuli in all tasks were pictures of objects. We varied the linguistic and visuospatial processing demands in three different tasks that involved decisions about (1) the sublexical (phonological) structure of the British Sign Language (BSL) signs for the objects, (2) the semantic category of the objects, and (3) the physical features of the objects. Neuroimaging data revealed that in participants who were deaf from birth, STC showed increased activation during visual processing tasks. Importantly, this differed across hemispheres. Right STC was consistently activated regardless of the task whereas left STC was sensitive to task demands. Significant activation was detected in the left STC only for the BSL phonological task. This task, we argue, placed greater demands on visuospatial processing than the other two tasks. In hearing signers, enhanced activation was absent in both left and right STC during all three tasks. Lateralization analyses demonstrated that the effect of deafness was more task-dependent in the left than the right STC whereas it was more task-independent in the right than the left STC. These findings indicate how the absence of auditory input from birth leads to dissociable and altered functions of left and right STC in deaf participants. SIGNIFICANCE STATEMENT Those born deaf can offer unique insights into neuroplasticity, in particular in regions of superior temporal cortex (STC) that primarily respond to auditory input in hearing people. Here we demonstrate that in those deaf from birth the left and the right STC have altered and dissociable functions. The right STC was activated regardless of demands on visual processing. In contrast, the left STC was sensitive to the demands of visuospatial processing. Furthermore, hearing signers, with the same sign language experience as the deaf participants, did not activate the STCs. Our data advance current understanding of neural plasticity by determining the differential effects that hearing status and task demands can have on left and right STC function.

Single Sided Deaf Cochlear Implant Users in the Difficult Listening Situation: Speech Perception and Subjective Benefit

Abstract:Objectives: Patients with single-sided deafness (SSD) have great difficulties in listening situations which rely on binaural auditory processing. The purpose of this study was to examine to which extent a cochlear implant (CI) can improve speech perception outcomes in various noisy listening environments. Additionally, the ability to use interaural level differences for sound localization and subjective benefit with the CI were assessed. Methods: Ten single-sided deaf patients with CI were tested in different loudspeaker configurations with and without the CI. A multi-source noise field (MSNF) with uncorrelated noise from four different directions was used in addition to a setup with the signal from the CI side and noise from the normal-hearing side (SCINNH, azimuth of ±45 degrees). Ten normal-hearing subjects were used as a control for the setup. Speech understanding was measured by an adaptive sentence test (Oldenburg Sentence Test, OLSA) in stationary speech shaped noise and temporally modulated noise to assess the benefit in each listening situation. Sensitivity to interaural level differences was measured in a lateralization experiment. Furthermore, patients completed the Bern Benefit in Single-Sided Deafness (BBSS) questionnaire to assess subjective benefit with the CI. Results: An overall average benefit in speech reception threshold (SRT) of 1.6 dB (±0.6 dB standard error of the mean [SEM]) was observed in the binaural listening condition (with CI) in all conditions. In the MSNF setup thresholds improved by 0.4 dB (±0.5 dB SEM) and in the SCINNH configuration by 2.7 dB (±0.7 dB SEM). The choice of masking noise effect also had a significant effect on the SRT outcome. The lateralization performance of the SSD users was on a par with the normal hearing group. BBSS scores reflect the overall benefit with the CI apparent in the speech test results. Conclusion: Patients with single-sided deafness do benefit from a CI in difficult listening environments and are able to localize sound based on interaural level differences. Considering these outcomes, cochlear implantation represents a promising treatment option for patients with single-sided deafness.
Hemispheric asymmetries in subcortical visual and auditory relay structures in congenital deafness

Abstract:Neuroplasticity - the capacity of the brain to change as a response to internal and external pressures - has been studied from a number of different perspectives. Perhaps one of the most powerful models is the study of populations that have been congenitally deprived of a sense. It has been shown that the right Auditory Cortex (AC) of congenitally deaf humans is neuroplastically modified in order to represent visual properties of a stimulus. One unresolved question is how this visual information is routed to the AC of congenitally deaf individuals. Here, we performed volumetric analysis of subcortical auditory and visual brains regions - namely the thalamus (along with three thalamic nuclei: the pulvinar, the lateral geniculate nucleus and the medial geniculate nucleus), and the inferior and superior colliculi - in deaf and hearing participants in order to identify which structures may be responsible for relaying visual information toward the altered AC. Because there is a hemispheric asymmetry in the neuroplastic changes observed in the AC of the congenitally deaf, we reasoned that subcortical structures that also showed a similar asymmetry in their total volume could have been enlisted in the effort of relaying visual information to the neuroplastically altered right AC. We show that for deaf, but not for hearing individuals, the right thalamus, right lateral geniculate nucleus and right inferior colliculus are larger than their left counterparts. These results suggest that these subcortical structures may be responsible for rerouting visual information to the AC in congenital deafness.

Audiovisual integration in children with cochlear implants revealed through EEG and fNIRS

Abstract:Sensory deprivation can offset the balance of audio versus visual information in multimodal processing. Such a phenomenon could persist for children born deaf, even after they receive cochlear implants (CIs), and could potentially explain why one modality is given priority over the other. Here, we recorded cortical responses to a single speaker uttering two syllables, presented in audio-only (A), visual-only (V), and audio-visual (AV) modes. Electroencephalography (EEG) and functional near-infrared spectroscopy (fNIRS) were successively recorded in seventy-five school-aged children. Twenty-five were children with normal hearing (NH) and fifty wore CIs, among whom 26 had relatively high language abilities (HL) comparable to those of NH children, while 24 others had low language abilities (LL). In EEG data, visual-evoked potentials were captured in occipital regions, in response to V and AV stimuli, and they were accentuated in the HL group compared to the LL group (the NH group being intermediate). Close to the vertex, auditory-evoked potentials were captured in response to A and AV stimuli and reflected a differential treatment of the two syllables but only in the NH group. None of the EEG metrics revealed any interaction between group and modality. In fNIRS data, each modality induced a corresponding activity in visual or auditory regions, but no group difference was observed in A, V, or AV stimulation. The present study did not reveal any sign of abnormal AV integration in children with CI. An efficient multimodal integrative network (at least for rudimentary speech materials) is clearly not a sufficient condition to exhibit good language and literacy.

Abstract:Tens of millions of people live blind, and their number is ever increasing. Visual-to-auditory sensory substitution (SS) encompasses a family of cheap, generic solutions to assist the visually impaired by conveying visual information through sound. The required SS training is lengthy: months of effort is necessary to reach a practical level of adaptation. There are two reasons for the tedious training process: the elongated substituting audio signal, and the disregard for the compressive characteristics of the human hearing system. To overcome these obstacles, we developed a novel class of SS methods, by training deep recurrent autoencoders for image-to-sound conversion. We successfully trained deep learning models on different datasets to execute visual-to-auditory stimulus conversion. By constraining the visual space, we demonstrated the viability of shortened substituting audio signals, while proposing mechanisms, such as the integration of computational hearing models, to optimally convey visual features in the substituting stimulus as perceptually discernible auditory components. We tested our approach in two separate cases. In the first experiment, the author went blindfolded for 5 days, while performing SS training on hand posture discrimination. The second experiment assessed the accuracy of reaching movements towards objects on a table. In both test cases, above-chance-level accuracy was attained after a few hours of training. Our novel SS architecture broadens the horizon of rehabilitation methods engineered for the visually impaired. Further improvements on the proposed model shall yield hastened rehabilitation of the blind and a wider adaptation of SS devices as a consequence.
Localization and Speech-in-Noise Performance in a Virtual Reality Test Environment: A Pilot Study of Adults With Single-Sided Deafness Using a Cochlear Implant

Abstract:Purpose: The purpose of this exploratory study was to (a) construct a virtual reality (VR) test environment to measure speech recognition in noise (SIN) and localization, and (b) use the VR test environment to establish degree of binaural hearing benefit among a small number of adults with single-sided deafness (SSD) using a cochlear implant (CI). Method: This pilot study included five adults implanted for SSD. The test environment was composed of an eight-speaker array that delivered restaurant noise and Institute of Electrical and Electronics Engineers sentences. VR head-mounted display goggles delivered a video recording of a busy restaurant. Participants completed SIN and localization in two conditions: (a) normal-hearing ear and a CI on the contralateral SSD side (CI-ON) and (b) normal-hearing ear and unaided on the contralateral SSD side (CI-OFF). Results: Overall, CI benefits for SIN and localization within the VR test environment were improved for some participants, although not all. CI benefit for SIN and localization was dependent on speaker location. Conclusions: VR test environments present new opportunities for studying SIN and localization abilities in participants with CIs. This pilot study shows that, within a VR test environment, degree of CI benefit among SSD participants for SIN and localization varies across speaker location and across participants.

Abstract:Spatial hearing, which largely relies on binaural time/level cues, is a challenge for patients with asymmetric hearing. The degree of the deficit is largely variable, and better sound localization performance is frequently reported. Studies on the compensatory mechanism revealed that monaural level cues and monoaural spectral cues contribute to variable behavior in those patients who lack binaural spatial cues. However, changes in the monaural level cues have not yet been separately investigated. In this study, the use of the level cue in sound localization was measured using stimuli of 1 kHz at a fixed level in patients with single-sided deafness (SSD), the most severe form of asymmetric hearing. The mean absolute error (MAE) was calculated and related to the duration/age onset of SSD. To elucidate the biological correlate of this variable behavior, sound localization ability was compared with the cortical volume of the parcellated auditory cortex. In both SSD patients ( n = 26) and normal controls with one ear acutely plugged ( n = 23), localization performance was best on the intact ear side; otherwise, there was wide interindividual variability. In the SSD group, the MAE on the intact ear side was worse than that of the acutely plugged controls, and it deteriorated with longer duration/younger age at SSD onset. On the impaired ear side, MAE improved with longer duration/younger age at SSD onset. Performance asymmetry across lateral hemifields decreased in the SSD group, and the maximum decrease was observed with the most extended duration/youngest age at SSD onset. The decreased functional asymmetry in patients with right SSD was related to greater cortical volumes in the right posterior superior temporal gyrus and the left planum temporale, which are typically involved in auditory spatial processing. The study results suggest that structural plasticity in the auditory cortex is related to behavioral changes in sound localization when utilizing monaural level cues in patients with SSD.
