Abstract:Objective To investigate the clinical characteristics and surgical management of middle ear cholesteato-ma in children. Methods 27 cases of children with middle ear cholesteatoma were enrolled in this study;Clinical symp-toms, signs and surgical experience were reviewed. Of 27 patients, 2 were performed transmeatal atticotomy and tympano-plasty, 25 were performed canal-wall-down modified radical mastoidectomy, and 5 were performed first-stage ossicular chain reconstruction. Results Otorrhea, ear odour and hearing loss were the most common symptoms;Mastoid, sinus tympani and attic were involved in 25 cases;Mesotympanum, hypotympanum and eustachian tube were in-volved in 20 cases;88.9%of patients had ossicular chain destruction;Facial nerve canal was eroded in 7 cases;During the follow-up of one to five years, there was no one with significant complications as facial palsy and intracranial complica-tions, no recurrent cholesteatoma was observed. Conclusions The middle ear cholesteatoma in children show more aggres-sive in complicated condition, appropriate surgical approach should be selected according to the severity of the lesion, opened technique is benificial to complete removal of lesions and prevent recurrence in severe patient.
Scutum reconstruction technique and classification in endoscopic middle ear cholesteatoma surgery

Abstract:Background: Endoscopy has been increasingly used in middle ear cholesteatoma surgery. It can increase the proportion of canal wall up (CWU) surgery and effectively control the recurrence and residual rates. However, there is a lack of systematic description on scutum reconstruction technique in endoscopic middle ear cholesteatoma surgery. Objective: The technique and classification of scutum reconstruction in endoscopic cholesteatoma surgery were proposed, and the outcomes were preliminarily evaluated. Methods: According to the development of mastoid and the way of hearing reconstruction, the reconstruction of scutum was classified into three types. The rates of residual and recurrence and hearing outcomes were observed and analyzed. Results: A total of 57 patients underwent endoscopic cholesteatoma surgery, all of which were CWU surgery. The reconstruction of the scutum was performed using tragal cartilage according to three types. The residual of cholesteatoma was observed in three cases. The recurrence of cholesteatoma and retraction pocket was detected in two cases and five cases, respectively. Postoperative hearing results were generally satisfied in all three types of surgeries. Conclusion: The technique of scutum reconstruction in endoscopic cholesteatoma surgery in this work can effectively control the recurrent disease after CWU surgery.

Abstract:Cholesteatoma is a benign tumor of the middle ear. Tympanoplasty is performed to remove cholesteatoma, prevent recurrence, and avoid complications. Previously, microscopy was used for tympanoplasty; however, endoscopy has become increasingly popular for this procedure. The effectiveness of endoscopy and the appropriate candidacy for endoscopic ear surgery remain controversial. In this retrospective chart review study, we enrolled 107 patients with cholesteatoma who underwent tympanoplasty and compared the microscopic approach (71 patients) and the endoscopic approach (36 patients) at different stages to clarify the benefits of using endoscopy and to determine candidacy for endoscopic ear surgery. Approach selection, complication rate, recurrence rate, and postoperative hearing threshold were compared between microscopic and endoscopic approaches in stages I, II, and III. Endoscopy was used more often than microscopy for early-stage (stage I) cholesteatoma (p = 0.005) and less frequently for advanced-stage (stage II) cholesteatoma (p = 0.02). Endoscopy surgery resulted in chorda tympani injury less often than microscopic surgery (p = 0.049); however, there were no significant differences between these two groups in terms of recurrence rate or postoperative hearing outcome. Endoscopy is particularly useful for early-stage cholesteatoma, and both approaches show no difference in hearing outcome in stage I and II; nevertheless, further research is required to determine an appropriate approach for more advanced stages (stage III).

Abstract:Complete surgical removal of cholesteatoma has been traditionally performed by microscopic approaches. In recent years, use of endoscopes in middle ear surgery is gaining increasing importance. The wide field view offered by the endoscopes in comparison to the narrow field of view of the binocular microscope enables a surgeon to see and dissect around corners, thereby exposing 'hidden recesses' of the middle ear which can be useful during cholesteatoma surgery. To identify remnant cholesteatoma in difficult access sites of the middle ear cleft using endoscopes following a microscope assisted mastoidectomy. A cross sectional study was conducted in patients who were clinically diagnosed with cholesteatoma. Patients were subjected to High Resolution Computed Tomography of the temporal bone and pre-operative audiological tests. Mastoid exploration was performed microscopically followed by endoscopic evaluation and clearance of disease in the same setting. Patients were then followed up post-operative and endoscopically evaluated to look for any remnant disease. A total of 45 patients were included, all of which underwent microscopic assisted mastoidectomy. Remnant cholesteatoma was discovered in 15 out of 45 cases (33%) distributed in the hidden areas; in the region of sinus tympani 9(60%), anterior epitympanum 4(27%), hypotympanum 1(7%) and tip Cells 1(7%). It can be concluded that endoscopes have a definite role in evaluation and complete surgical clearance of cholesteatoma particularly from the hidden areas of middle ear cleft which would have otherwise been missed by the straight line view of a microscope thereby grossly reducing the rate of remnant disease. Abstract:BACKGROUND: Endoscopic ear surgery is a promising technique for removing congenital cholesteatoma in children. It can provide greater visual access to hidden areas of the middle ear and facilitate middle-ear manipulation. This study compares a single-center experience in manag- ing congenital cholesteatoma with an endoscopic approach with that in managing congenital cholesteatoma with a conventional microscopic approach.METHODS: Records of consecutive patients aged under 8 with congenital cholesteatoma confined to the middle ear at our tertiary referral hospital from January 2013 to December 2018 were retrospectively reviewed. Operation time, hospital stay, postoperative complications, and recurrence/residue of congenital cholesteatoma were compared between patients receiving microscopic versus endoscopic surgery.RESULTS: A total of 33 pediatric patients aged from 19 months to 7 years were enrolled; 12 children underwent microscopic surgery, and 21 received an endoscopic approach for removing congenital cholesteatoma. The mean operative time was 1.61 hours for the microscopic group and 1.49 hours for the endoscopic group without statistical difference. No postoperative sensorineural hearing loss and complications were reported. Four cases of recurrence/residue were observed on the follow-up endoscopic exam or computed tomography, and no differences were shown between the 2 groups. Of the total patients, 94.7% (n=11) in the microscopic group and 90.5% (n=19) in the endoscopic group demonstrated an intact tympanic membrane without perforation or retraction after surgery. No audiological differences were reported between the 2 groups.CONCLUSION: Endoscopic ear surgery can effectively and safely remove congenital cholesteatoma in children and is not inferior to conventional microscopic approaches.

Abstract:We report a case of an oncocytic papillary adenocarcinoma of the endometrium in an 89-year-old female with vaginal bleeding. Imaging studies revealed lesions in the uterus, kidneys, pancreas, gluteus, and an enlarged portacaval lymph node. Diagnostic workup included an endometrial biopsy which showed malignant, oncocytic cells in a predominantly papillary pattern. These cells stained positive for epithelial markers (pan-cytokeratin, CK7, epithelial membrane antigen) and weakly for estrogen receptor. The cells were negative for cytokeratin 903, CAM 5.2, progesterone receptor, CD10, RCC Marker, CA-125, c-kit, and vimentin. Consultation with experts in Gynecologic and Genitourinary pathology returned a diagnosis of "adenocarcinoma compatible with metastatic renal cell carcinoma"--an intriguing possibility worthy of further exploration. To our knowledge, there are no reports in the literature of metastatic oncocytic papillary renal cell carcinoma to the endometrium. The clinical and pathologic features of oncocytic papillary endometrial lesions, including primary and metastatic processes, are reviewed.
Analysis of surgical methods and outcome of middle ear cholesteatuma

Abstract:Objective To analyze the surgical methods and outcome of middle ear cholesteatuma. Methods Retrospective review was performed in 137 patients with middle ear cholesteatuma operated from Jan.2012 to Dec.2014.All the patients were divided into three groups according to different operation method, thus, group A (patients received mastoidectomy), group B (patients re?ceived mastoidectomy and tympanoplasty with Canal Wall Down), and group C (patients received mastoidectomy and tympanoplasty with Canal Wall Up) , and dry ear time and the change of hearing, tinnitus and vertigo were compared before and after surgery. Results A total of 137 patients with middle ear cholesteatoma were studied in this research, including 38 patients of group A, 76 ca?ses of group B and 23 cases of group C. The dry ear time of Group B was shorter than group A, group C was shorter compared with group A, and group C was shorter than group B . The difference between each group was statistically significant. The hearing of group A six months after operation showed no statistically significant difference compared with preoperative, while those of group B and group C were increased, and the difference were statistically significant. Tinnitus disability scale THI ( tinnitus handicap inventory, THI) scores were reduced after operations, and the difference of group B and C group were statistically significant. There were some patients with vertigo symptom in group A and group B before surgery, and DHI scores after surgery ( deziness handicap inventory, DHI ) were dropped after operations, and the difference were statistically significant. Conclusion Compared with the mastoidectomy, mastoidecto?my and tympanoplasty with Canal Wall Down and Canal Wall Up could shorten the dry ear time, improve the postoperative patients with hearing and relieve tinnitus symptoms postoperative in patients with middle ear cholesteatoma.

Abstract:Implementation of endoscopes in cholesteatoma surgery resulted in considerable changes in the management of cholesteatoma in the last two decades. Compared to the microscopic approach with an excellent but straight-line view and limited illumination, the introduction of endoscopes provides a wide-angled panoramic view. Moreover, angled lenses allow the surgeon to visualize the middle ear and its hidden recesses through a transcanal, minimally-invasive approach. The endoscope enables the surgeon to remove limited cholesteatoma of the middle ear and its recesses using an exclusive endoscopic technique by taking advantage of these benefits. This reduces the rate of residual disease and sparing external incisions and excessive temporal bone drilling as in a transmastoid approach. Since transcanal endoscopic access is mainly a one-handed technique, it implies the need for specific procedures and technical refinements. This article describes a step-by-step guide as a surgical manual for endoscopic removal of epitympanic cholesteatoma. Different techniques for cholesteatoma dissection and bone removal for epitympanectomy, including curettage and powered instruments such as drills and ultrasonic devices with their outcomes, are discussed. This may offer ear surgeons insight into technical refinements and the latest technological developments and open the horizon for different techniques.
Outcomes of transcanal endoscopic middle ear surgery for congenital cholesteatoma

Abstract:Background: Transcanal endoscopic surgery offers advantages for congenital cholesteatoma (CC) which is expanding cystic mass located on an intact tympanic membrane. Objectives: To determine the efficacy of transcanal endoscopic ear surgery for the treatment of CC. Material and methods: Eleven patients with CC confined to the middle ear underwent transcanal endoscopic ear surgery by surgeons who were skilled in otoscopic surgery. According to the Potsic classification, four, three, and four patients had lesions in stages 1, 2, and 3, respectively. The mean follow-up duration was 24.5 months. We retrospectively analyzed the recurrence rate, complications, and average hearing thresholds of the patients with CC. Results: All CCs were successfully resected via the transcanal endoscopic approach, without any immediate or serious complications. The only complication was external auditory canal stenosis due to postoperative scarring, which occurred in a patient with a stage-3 cholesteatoma that recurred and required reoperation and second-stage ossicular chain reconstruction. Two more patients underwent one-stage ossicular chain reconstruction. All three patients received a total artificial ossicular replacement prosthesis, and two of them also received cartilage grafts. Conclusions and significance: Transcanal endoscopic ear surgery is an effective method to treat CC belonging to Potsic stages 3 or below.

Abstract:Surgery for cholesteatoma should be tailored to individual patients, considering demographic and disease factors, to obtain a dry, safe, and functional ear. The EAONO/JOS classification and staging system provide a valuable framework for data collection and outcome assessment. Canal wall-up and canal wall-down surgical approaches each have their advantages and disadvantages, though it is not definitive that one approach is clearly more advantageous than the other. Mastoid obliteration techniques show promise in reducing recidivistic disease rates but require further research and standardization. Endoscopic ear surgery further augments our surgical capabilities to visualize and eradicate cholesteatoma.

Abstract:This article reviews the history and current status of endoscopic ear surgery,and describes in detail as-sociated technical difficulties. Based on otological techniques used and equipment requirements, various approaches to categorize endoscopic ear surgery are proposed, including categorization based on utilization of otoendoscopic devices as a tool or based on the location, approach and technical requirement of the surgery. Finally, two evaluation indexes of minimally invasive capability for endoscopic ear surgery are proposed. The objective of this article is to, through illus-trating classification of endoscopic ear surgery, provide assistance or guidance to institutions or individuals involved in utilization of the endoscopic technique.

Abstract:Introduction: Surgical removal is widely employed in children with congenital middle ear cholesteatoma (CMEC). Here, we report the surgical outcomes of CMEC removal via endoscopic ear surgery (EES) and microscopic ear surgery (MES) in children. Methods: Children with CMEC who underwent preoperative medical history inquiry, hearing test, endoscopic evaluation, and radiology imaging before receiving EES or MES were included. Postoperative audiological outcomes and recurrence rates were collected. Results: Seventeen children (20 ears) with stage II-IV CMEC were included. Of those, 11 ears (55.0%) underwent EES, and 9 ears (45.0%) underwent MES. The follow-up time was 35 ± 13.5 months. One child in the EES group with stage III CMEC had a recurrence during the follow-up period. In the EES group, the average minimum diameter of the external auditory canal on the affected side was 5.8 mm (4.3–8.0 mm). No linear association was found between age and the minimum diameter of the external auditory canal. Discussion: EES is a promising treatment option for children with early-stage CMEC because of its low recurrence rate and minimally invasive nature. The minimum diameter of the external auditory canal on the affected side should be meticulously examined when performing EES in children.

Abstract:Evaluation of 19 patients with a history of renal trauma revealed three angiographic features helper in the diagnosis of posttraumatic renal scarring: (1) a cup-shaped renal contour defect characterized by smooth margins, infolding of the renal cortex, and extension of the calyces to the edge of the defect; (2) occlusion of an extra- or intrarenal artery associated with a distal avascular zone of infarction; and (3) a grossly heterogeneous nephrogram characterized by scattered islands of renal tissue separated by broad or narrow avascular scars. Limited pathologic correlation suggests that the cup-shaped contour defect reflects subcapsular hemorrhage and that the mottled, heterogeneous nephrogram reflects a shattered kidney.

Clinical Characteristics of Pars Tensa Cholesteatoma: A Comparative Study of Area-Based Classification Systems Proposed by the Japanese Otological Society and the European Academy of Otology - Neuro-Otology

Abstract:Objectives: To assess the clinical characteristics of extent patterns in pars tensa cholesteatoma. Materials and methods: This was a retrospective chart review. Forty-four patients with pars tensa cholesteatoma who underwent primary surgery at a tertiary academic medical center were included. The main outcomes measured were sex, age, clinical background, and stage classification of pars tensa cholesteatoma (including the extent of cholesteatoma and involvement of the sinus tympani) according to two staging classifications: criteria advocated by the Japanese Otological Society (JOS) and those advocated by the European Academy of Otology and Neuro-Otology (EAONO)/JOS joint consensus statements. Results: The mean patient age ± standard deviation was 38.4±19.6 years. The patients comprised 19 men and 25 women. According to the JOS classification, 18 ears (40.9%) were classified as stage I, 22 (50.0%) as stage II, and 4 (9.1%) as stage III. According to the EAONO/JOS joint consensus statements, 14 ears (31.8%) were classified as stage I, 26 (59.1%) as stage II, and 4 (9.1%) as stage III. Fourteen ears (31.8%) demonstrated involvement of the sinus tympani. Four ears (9.1%) that were originally categorized as stage I cholesteatoma by the JOS criteria showed sinus tympani invasion and were subsequently categorized as stage II according to the EAONO/JOS criteria. Conclusion: We determined the clinical characteristics of pars tensa cholesteatoma based on the novel and well-defined classification criteria. Further studies including long-term outcomes are necessary to demonstrate the clinical relevance of the discrepancy between the two criteria with respect to involvement of the sinus tympani.

Abstract:Objective: We aimed to assess the clinical characteristics of extent patterns in congenital cholesteatoma, based on the Japan Otological Society (JOS) staging system. Methods: This was a retrospective chart review that included 80 ears of 80 patients with congenital cholesteatoma who underwent primary surgery at a tertiary academic medical center. The main characteristics and outcomes reviewed were sex, age, clinical background, surgical method, and stage classification according to two staging classifications: the criteria advocated by JOS and Potsic staging system. Results: The age at the time of surgery ranged from 1 to 35 years (average 8.4 years), and there were 54 men and 26 women. According to the JOS staging system, 12 ears were Stage Ia (15%), 7 ears were Stage Ib (9%), 1 ear was Stage Ic (1%), 59 ears were Stage II (74%), and 1 ear was Stage III (1%). In the study of postoperative residual recurrence, there were 4 cases after the primary operation and 3 cases after the staged operation. All 3 ears with residual disease after planned surgery were cholesteatomas that extended to all the tympanomastoid space. Conclusion: We consider the JOS staging system to be more suitable, in terms of anatomical classification and surgical procedure selection for comparison between Europe, the United States, and Asia. Specifically, it was advantageous that the PTAM classification and the S classification are associated with surgical procedure selection and postoperative course.

[The Selection of Surgical Technique for Cholesteatoma in Pediatric Patients].

Abstract:OBJECTIVE:To discuss the surgical treatments and results for cholesteatoma in pediatric patients.METHOD:A retrospective analysis of 25 cases of cholesteatoma in pediatric patients. Out of all 25 patients, 18 underwent the tympanoplasty with open technique, 1 underwent the tympanoplasty with close technique, and 6 underwent the radial mastoidectomy.RESULT:During the follow-up, 24 patients were cured in first-stage surgery, no recurrent cholesteatoma was observed, no observed complications occurred after surgery. Compared with the hearing thresholds of 7 patients were improved, 7 patients' hearing thresholds had no change, and 11 patients' were decreased.CONCLUSION:The cholesteatoma in children are more widespread and erosive. The surgery should completely remove the diseased tissues and then preserve the hearing. The surgery should clear away all the lesions thoroughly, and then to conserve hearing ability maximally. Surgical techniques should be chosen depending on the lesions' extension, generally the tympanoplasty with open technique was more suitable. The postoperative hearing decrease based on the diseased tissue and disease extension.

Abstract:OBJECTIVES: We aimed to compare clinical outcomes including hearing improvement and cholesteatoma recurrence between endoscopic and conventional microscopic surgeries in patients with attic cholesteatoma.METHODS: We collected data from patients with attic cholesteatoma who were treated using endoscopic (10 patients) and microscopic (10 patients) approaches by a single surgeon. The data were retrospectively reviewed for patient characteristics, intraoperative findings, hearing levels, and follow-up clinical status. Recurrence of the cholesteatoma, improvement of hearing, and operation time were evaluated.RESULTS: Ossiculoplasty was performed in four patients in the endoscopic group and two patients in the microscopic group. Lempert endaural incision II was used in all the patients in the microscopic group, whereas Lempert I incision was used in all the patients in the endoscopic approach group. There were no significant differences between the two groups regarding hearing improvement and operating time. And, there were no recurrences during the follow-up period in both groups.CONCLUSION: The endoscopic approach for the management of attic cholesteatoma is as useful as the microscopic approach.
