Abstract:Notifiable infectious diseases are a major public health concern in China,causing about five million illnesses and twelve thousand deaths every year.Early detection of disease activity,when followed by a rapid response,can reduce both social and medical impact of the disease.We aim to improve early detection by monitoring health-seeking behavior and disease-related news over the Internet.Specifically,we counted unique search queries submitted to the Baidu search engine in 2008 that contained disease-related search terms.Meanwhile we counted the news articles aggregated by Baidu’s robot programs that contained disease-related keywords.We found that the search frequency data and the news count data both have distinct temporal association with disease activity.We adopted a linear model and used searches and news with 1–200-day lead time as explanatory variables to predict the number of infections and deaths attributable to four notifiable infectious diseases,i.e.,scarlet fever,dysentery,AIDS,and tuberculosis.With the search frequency data and news count data,our approach can quantitatively estimate up-to-date epidemic trends 10–40 days ahead of the release of Chinese Centers for Disease Control and Prevention（Chinese CDC）reports.This approach may provide an additional tool for notifiable infectious disease surveillance.
Association Between Cancer Incidence and Mortality in Web-Based Data in China: Infodemiology Study.

Abstract:Background: Cancer poses a serious threat to the health of Chinese people, resulting in a major challenge for public health work. Today, people can obtain relevant information from not only medical workers in hospitals, but also the internet in any place in real-time. Search behaviors can reflect a population's awareness of cancer from a completely new perspective, which could be driven by the underlying cancer epidemiology. However, such Web-retrieved data are not yet well validated or understood. Objective: This study aimed to explore whether a correlation exists between the incidence and mortality of cancers and normalized internet search volumes on the big data platform, Baidu. We also assessed whether the distribution of people who searched for specific types of cancer differed by gender. Finally, we determined whether there were regional disparities among people who searched the Web for cancer-related information. Methods: Standard Boolean operators were used to choose search terms for each type of cancer. Spearman's correlation analysis was used to explore correlations among monthly search index values for each cancer type and their monthly incidence and mortality rates. We conducted cointegration analysis between search index data and incidence rates to examine whether a stable equilibrium existed between them. We also conducted cointegration analysis between search index data and mortality data. The monthly Baidu index was significantly correlated with cancer mortality rates for 24 of 27 cancers. In terms of the whole population, the number of women who searched for cancer-related information has slowly risen over time. People aged 30-39 years were most likely to use search engines to retrieve cancer-related knowledge. East China had the highest Web search volumes for cancer. Conclusions: Search behaviors indeed reflect public awareness of cancer from a different angle. Research on internet search behaviors could present an innovative and timely way to monitor and estimate cancer incidence and mortality rates, especially for cancers not included in national registries.

Low-dose computed tomography (LDCT) for lung cancer screening (LCS) can reduce lung cancer deaths by 20% compared to chest x-rays. Nevertheless, the uptake of LDCT is low for high-risk smokers. In this study we used Twitter data to understand laypeople's emotions towards LCS, find topics on LCS-related tweets, and assess the impact of promotional information on laypeople's discussions.

Abstract:Lung cancer remains the leading cause of cancer mortality. Meanwhile, the Internet remains a remarkably popular source of health information. This study characterizes how patients with lung cancer utilize online resources including social media and examines how this online health information-seeking behaviour (HISB) influences their healthcare journeys. From September 2022 to August 2023, a survey of 38 open- and close-ended questions was distributed to patients with lung cancer at a tertiary cancer centre. Quantitative data was analyzed with descriptive statistics, and qualitative data was evaluated using a grounded-theory approach. A total of 74 surveys were distributed, with 54 responses. Eighty-nine percent of respondents reported online HISB. Of this subgroup, only 29% searched social media for health information, and 77% used a search engine. Eighty-nine percent of the search engine users utilized Google, selecting top hits (57%) and/or websites from sources they considered reputable (57%). Participants who did not use search engines directly accessed websites recommended by trusted sources such as physicians, friends, and family. Treatment was the most frequently sought topic (70%). While most respondents engaging in online HISB considered the Internet to be useful (79%), less than half found it influential for their treatment decisions (45%). Most respondents in this study searched the Internet for lung cancer information. Our data highlights the Internet as complementary to clinical discussions with trusted medical professionals. Physicians should recognize the widespread adoption of online HISB, guide patients to evaluate the credibility of online health information, and contextualize it within the patients' unique healthcare journey.

Abstract:BACKGROUND:Rare diseases have become a major public health concern worldwide. However, detailed epidemiological data are lacking. With the development of the Internet, search queries have played an important role in disease surveillance. In this study, we explored a new method for the epidemiological research on rare diseases, using large-scale online search queries and reported case data. We distilled search logs related to rare diseases nationwide from 2016 to 2019. The case data were obtained from China's national database of rare diseases during the same period.RESULTS:A total of 120 rare diseases were included in this study. From 2016 to 2019, the number of patients with rare diseases estimated using search data and those obtained from the case database showed an increasing trend. Rare diseases can be ranked by the number of search estimated patients and reported patients, and the rankings of each disease in both search and reported case data were generally stable. Furthermore, the disease rankings in the search data were relatively consistent with the reported case data in each year, with more than 50% of rare diseases having a ranking difference of -20 to 20 between the two systems. In addition, the relationship between the disease rankings in the two systems was generally stable over time. Based on the relationship between the disease rankings in the search and reported case data, rare diseases can be classified into two categories.CONCLUSION:Online search queries may provide an important new resource for detecting rare diseases. Rare diseases can be classified into two categories to guide different epidemiological research strategies.
Trends and Prediction in Daily New Cases and Deaths of COVID-19 in the United States: an Internet Search-Interest Based Model.

Abstract:BACKGROUND AND OBJECTIVES:The daily incidence and deaths of coronavirus disease 2019 (COVID-19) in the USA are poorly understood. Internet search interest was found to be correlated with COVID-19 daily incidence in China, but has not yet been applied to the USA. Therefore, we examined the association of internet search-interest with COVID-19 daily incidence and deaths in the USA.METHODS:We extracted COVID-19 daily new cases and deaths in the USA from two population-based datasets, namely 1-point-3-acres.com and the Johns Hopkins COVID-19 data repository. The internet search-interest of COVID-19-related terms was obtained using Google Trends. The Pearson correlation test and general linear model were used to examine correlations and predict trends, respectively.RESULTS:There were 636,282 new cases and,325 deaths of COVID-19 in the USA from March 1 to April 15, 2020, with a crude mortality of 4.45%. The daily new cases peaked at 35,098 cases on April 10, 2020 and the daily deaths peaked at 2,494 on April 15, 2020. The search interest of COVID, "COVID pneumonia" and "COVID heart" were correlated with COVID-19 daily incidence, with 12 or 14 days of delay (Pearson's r = 0.978, 0.978 and 0.979, respectively) and deaths with 19 days of delay (Pearson's r = 0.963, 0.958 and 0.970, respectively). The 7-day follow-up with prospectively collected data showed no significant correlations of the observed data with the predicted daily new cases or daily deaths, using search interest of COVID, COVID heart, and COVID pneumonia.CONCLUSIONS:Search terms related to COVID-19 are highly correlated with the COVID-19 daily new cases and deaths in the USA.
"I Saw it Incidentally but Frequently": Exploring the Effects of Online Health Information Scanning on Lung Cancer Screening Behaviors Among Chinese Smokers

Abstract:In China, lung cancer is the most common cancer with high mortality. While prior research suggests that health information scanning influences cancer screening within the general population, a deeper exploration of the underlying mechanisms is imperative. This study specifically targets smokers, aiming to investigate whether online health information scanning can effectively encourage lung cancer screening and elucidate the mechanisms driving this association. Data were extracted from a Chinese national survey conducted between January and September 2023, exclusively involving current smokers ( N = 992). A moderated mediation model was employed to examine a cognitive-affective sequential chain of mediation through risk perceptions and cancer worry. Results showed that 99.1% of Chinese smokers reported online health information scanning, which possessed significant influence. Online health information scanning was positively associated with effective lung cancer screening among smokers directly ( OR = 1.927, p < .01) and indirectly ( OR = .065, p < .001). The perceived threat of smoking and cancer worry served as the cognitive and affective mediating mechanisms. Furthermore, a moderating effect of eHealth literacy was observed ( OR = 6.292, p < .05). Smokers with higher eHealth literacy are more inclined to undergo effective lung cancer screening. Based on these findings, public health sectors should leverage online platforms to disseminate tailored cancer screening education and implement initiatives to enhance public eHealth literacy.

Abstract:Lung cancer is the most common cause of cancer-related death in Michigan. Most patients are diagnosed at advanced stages of the disease. There is a need to detect clusters of lung cancer incidence over time, to generate new hypotheses about causation and identify high-risk areas for screening and treatment. The Michigan Cancer Surveillance database of individual lung cancer cases, 1985 to 2018 was used for this study. Spatial and spatiotemporal clusters of lung cancer and level of disease (localized, regional and distant) were detected using discrete Poisson spatial scan statistics at the zip code level over the study time period. The approach detected cancer clusters in cities such as Battle Creek, Sterling Heights and St. Clair County that occurred prior to year 2000 but not afterwards. In the northern area of the lower peninsula and the upper peninsula clusters of late-stage lung cancer emerged after year 2000. In Otter Lake Township and southwest Detroit, late-stage lung cancer clusters persisted. Public and patient education about lung cancer screening programs must remain a health priority in order to optimize lung cancer surveillance. Interventions should also involve programs such as telemedicine to reduce advanced stage disease in remote areas. In cities such as Detroit, residents often live near industry that emits air pollutants. Future research should therefore, continue to focus on the geography of lung cancer to uncover place-based risks and in response, the need for screening and health care services.

Understanding Health Communication Through Google Trends and News Coverage for COVID-19: Multinational Study in Eight Countries

Abstract:Background Due to the COVID-19 pandemic, health information related to COVID-19 has spread across news media worldwide. Google is among the most used internet search engines, and the Google Trends tool can reflect how the public seeks COVID-19–related health information during the pandemic. Objective The aim of this study was to understand health communication through Google Trends and news coverage and to explore their relationship with prevention and control of COVID-19 at the early epidemic stage. Methods To achieve the study objectives, we analyzed the public’s information-seeking behaviors on Google and news media coverage on COVID-19. We collected data on COVID-19 news coverage and Google search queries from eight countries (ie, the United States, the United Kingdom, Canada, Singapore, Ireland, Australia, South Africa, and New Zealand) between January 1 and April 29, 2020. We depicted the characteristics of the COVID-19 news coverage trends over time, as well as the search query trends for the topics of COVID-19–related “diseases,” “treatments and medical resources,” “symptoms and signs,” and “public measures.” The search query trends provided the relative search volume (RSV) as an indicator to represent the popularity of a specific search term in a specific geographic area over time. Also, time-lag correlation analysis was used to further explore the relationship between search terms trends and the number of new daily cases, as well as the relationship between search terms trends and news coverage. Results Across all search trends in eight countries, almost all search peaks appeared between March and April 2020, and declined in April 2020. Regarding COVID-19–related “diseases,” in most countries, the RSV of the term “coronavirus” increased earlier than that of “covid-19”; however, around April 2020, the search volume of the term “covid-19” surpassed that of “coronavirus.” Regarding the topic “treatments and medical resources,” the most and least searched terms were “mask” and “ventilator,” respectively. Regarding the topic “symptoms and signs,” “fever” and “cough” were the most searched terms. The RSV for the term “lockdown” was significantly higher than that for “social distancing” under the topic “public health measures.” In addition, when combining search trends with news coverage, there were three main patterns: (1) the pattern for Singapore, (2) the pattern for the United States, and (3) the pattern for the other countries. In the time-lag correlation analysis between the RSV for the topic “treatments and medical resources” and the number of new daily cases, the RSV for all countries except Singapore was positively correlated with new daily cases, with a maximum correlation of 0.8 for the United States. In addition, in the time-lag correlation analysis between the overall RSV for the topic “diseases” and the number of daily news items, the overall RSV was positively correlated with the number of daily news items, the maximum correlation coefficient was more than 0.8, and the search behavior occurred 0 to 17 days earlier than the news coverage. Conclusions Our findings revealed public interest in masks, disease control, and public measures, and revealed the potential value of Google Trends in the face of the emergence of new infectious diseases. Also, Google Trends combined with news media can achieve more efficient health communication. Therefore, both news media and Google Trends can contribute to the early prevention and control of epidemics.

Abstract:Abstract Previous research has demonstrated that various properties of infectious diseases can be inferred from online search behaviour. In this work we use time series of online search query frequencies to gain insights about the prevalence of COVID-19 in multiple countries. We first develop unsupervised modelling techniques based on associated symptom categories identified by the United Kingdom’s National Health Service and Public Health England. We then attempt to minimise an expected bias in these signals caused by public interest—as opposed to infections—using the proportion of news media coverage devoted to COVID-19 as a proxy indicator. Our analysis indicates that models based on online searches precede the reported confirmed cases and deaths by 16.7 (10.2–23.2) and 22.1 (17.4–26.9) days, respectively. We also investigate transfer learning techniques for mapping supervised models from countries where the spread of the disease has progressed extensively to countries that are in earlier phases of their respective epidemic curves. Furthermore, we compare time series of online search activity against confirmed COVID-19 cases or deaths jointly across multiple countries, uncovering interesting querying patterns, including the finding that rarer symptoms are better predictors than common ones. Finally, we show that web searches improve the short-term forecasting accuracy of autoregressive models for COVID-19 deaths. Our work provides evidence that online search data can be used to develop complementary public health surveillance methods to help inform the COVID-19 response in conjunction with more established approaches.

Abstract:As the COVID-19 ravaging through the globe, accurate forecasts of the disease spread are crucial for situational awareness, resource allocation, and public health decision-making. Alternative to the traditional disease surveillance data collected by the United States (US) Centers for Disease Control and Prevention (CDC), big data from Internet such as online search volumes also contain valuable information for tracking infectious disease dynamics such as influenza epidemic. In this study, we develop a statistical model using Internet search volume of relevant queries to track and predict COVID-19 pandemic in the United States. Inspired by the strong association between COVID-19 death trend and symptom-related search queries such as "loss of taste", we combine search volume information with COVID-19 time series information for US national level forecasts, while leveraging the cross-state cross-resolution spatial temporal framework, pooling information from search volume and COVID-19 reports across regions for state level predictions. Lastly, we aggregate the state-level frameworks in an ensemble fashion to produce the final state-level 4-week forecasts. Our method outperforms the baseline time-series model, while performing reasonably against other publicly available benchmark models for both national and state level forecast.
Estimating Rare Disease Incidences with Large-scale Internet Search Data: Development and Evaluation of a Two-step Machine Learning Method

Abstract:Background: As rare diseases (RDs) receive increasing attention, obtaining accurate RD incidence estimates has become an essential concern in public health. Since RDs are difficult to diagnose, include diverse types, and have scarce cases, traditional epidemiological methods are costly in RD registries. With the development of the internet, users have become accustomed to searching for disease-related information through search engines before seeking medical treatment. Therefore, online search data provide a new source for estimating RD incidences. Objective: The aim of this study was to estimate the incidences of multiple RDs in distinct regions of China with online search data. Methods: Our research scale included 15 RDs in China from 2016 to 2019. The online search data were obtained from Sogou, one of the top 3 commercial search engines in China. By matching to multilevel keywords related to 15 RDs during the 4 years, we retrieved keyword-matched RD-related queries. The queries used before and after the keyword-matched queries formed the basis of the RD-related search sessions. A two-step method was developed to estimate RD incidences with users' intents conveyed by the sessions. In the first step, a combination of long short-term memory and multilayer perceptron algorithms was used to predict whether the intents of search sessions were RD-concerned, news-concerned, or others. The second step utilized a linear regression (LR) model to estimate the incidences of multiple RDs in distinct regions based on the RD-and news-concerned session numbers. For evaluation, the estimated incidences were compared with RD incidences collected from China's national multicenter clinical database of RDs. The root mean square error (RMSE) and relative error rate (RER) were used as the evaluation metrics. Results: The RD-related online data included 2,749,257 queries and 1,769,986 sessions from 1,380,186 users from 2016 to 2019. The best LR model with sessions as the input estimated the RD incidences with an RMSE of 0.017 (95% CI 0.016-0.017) and an RER of 0.365 (95% CI 0.341-0.388). The best LR model with queries as input had an RMSE of 0.023 (95% CI 0.017-0.029) and an RER of 0.511 (95% CI 0.377-0.645). Compared with queries, using session intents achieved an error decrease of 28.57% in terms of the RER (P=.01). Analysis of different RDs and regions showed that session input was more suitable for estimating the incidences of most diseases (14 of 15 RDs). Moreover, examples focusing on two RDs showed that news-concerned session intents reflected news of an outbreak and helped correct the overestimation of incidences. Experiments on RD types further indicated that type had no significant influence on the RD estimation task. Conclusions: This work sheds light on a novel method for rapid estimation of RD incidences in the internet era, and demonstrates that search session intents were especially helpful for the estimation. The proposed two-step estimation method could be a valuable supplement to the traditional registry for understanding RDs, planning policies, and allocating medical resources. The utilization of search sessions in disease detection and estimation could be transferred to infoveillance of large-scale epidemics or chronic diseases.
Estimating disease burden using Internet data

Abstract:Data on disease burden are often used for assessing population health, evaluating the effectiveness of interventions, formulating health policies, and planning future resource allocation. We investigated whether Internet usage and social media data, specifically the search volume on Google, page view count on Wikipedia, and disease mentioning frequency on Twitter, correlated with the disease burden, measured by prevalence and treatment cost, for 1633 diseases over an 11-year period. We also applied least absolute shrinkage and selection operator to predict the burden of diseases. We found that Google search volume is relatively strongly correlated with the burdens for 39 of 1633 diseases, including viral hepatitis, diabetes mellitus, multiple sclerosis, and hemorrhoids. Wikipedia and Twitter data strongly correlated with the burdens of 15 and 7 diseases, respectively. However, an accurate analysis must consider each condition’s characteristics, including acute/chronic nature, severity, familiarity to the public, and the presence of stigma.

Abstract:BACKGROUND China has accumulated large amount of cancer-related data from multiple sources. Studies have thoroughly reviewed Chinese healthcare systems and emphasized the importance to promote data utilization while there is no study illustrating the utilization status of cancer-related databases. OBJECTIVE To understand data utilization of major cancer-related databases in China and promote further researches in healthcare data systems. METHODS A bibliometric analysis was applied on peer-reviewed publications using 15 representative cancer-related databases classified as cancer registry, medical insurance database, hospital-based database, and publicly accessible database according to source of data collection in China. Peer-reviewed articles written in English or Chinese, indexed in PubMed, Web of Science, Chinese National Knowledge Infrastructure (CNKI) and Wanfang Data (in Chinese), and published since database inception to December 2021 were included. Data was analysed in November 2022. Bibliometric parameters were extracted to describe the numbers, quality, and research contents of published articles. The quality of the publications was described by total citation, journal impact factors (2020 edition) and Journal Citation Reports quartile (JCR, 2020 edition). Research contents were demonstrated by cancer sites and research topics. Between-group comparisons were made using Kruskal-Wallis H test or Fisher-Freeman-Halton exact test, as appropriate. RESULTS Of 1037 articles included, 637 was from cancer registries, 313 from hospital-based databases, 65 from medical insurance databases and 22 from public databases. An upward trend in annul number of publications was observed. Articles published in Chinese were nearly two-fold of those in English. Papers based on cancer registry, publicly accessible databases and the joint exploration of medical insurance databases were mainly published in journals of JCR Q2. The distribution of JCR quartiles and total citations were significantly different for the four data sources (P<0.0001). Lung, stomach, colorectal were most studied cancer sites while cancer incidence, mortality and prognosis were most focused research interests. CONCLUSIONS The upward trend in publication number was mainly contributed by articles from registries. The limited number of publications in English might weaken the impact of sharing research findings generated from Chinese data, in the international community. It highlights the importance of promoting data accessibility and utilization from various sources in China. CLINICALTRIAL None
Assessing Smoking-Related Behaviours Using Massive Online Search Query Data

Abstract:Online search queries have been used in various behaviour studies. As smoking has become a global health issue, studies that assess smoking-related behaviours using online search queries have faced limitations in data utilization and study design. This study conducts comparative analyses to investigate changes in smoking-related behaviours represented by online search volumes. Baidu search queries from 2013 to 2017 were used to examine the search volume containing four groups of smoking-related search keywords. A validation process was used to validate the proposed method by comparing changes in search queries with the observed tobacco consumption. The results show changes in smoking-related behaviours assessed by online search queries at the city level in China. The validation experiments illustrate the consistency between changes in search volumes and tobacco consumption. Thus, online search queries were verified to be an effective instrument for assessing smoking-related behaviours, and this study sheds light on broader behaviour studies and policy assessments.
Using Baidu Search Index to Monitor and Predict Newly Diagnosed Cases of HIV/AIDS, Syphilis and Gonorrhea in China: Estimates from a Vector Autoregressive (VAR) Model.

Abstract:OBJECTIVES:Internet search engine data have been widely used to monitor and predict infectious diseases. Existing studies have found correlations between search data and HIV/AIDS epidemics. We aimed to extend the literature through exploring the feasibility of using search data to monitor and predict the number of newly diagnosed cases of HIV/AIDS, syphilis and gonorrhoea in China.METHODS:This paper used vector autoregressive model to combine the number of newly diagnosed cases with Baidu search index to predict monthly newly diagnosed cases of HIV/AIDS, syphilis and gonorrhoea in China. The procedures included: (1) keywords selection and filtering; (2) construction of composite search index; (3) modelling with training data from January 2011 to October 2016 and calculating the prediction performance with validation data from November 2016 to October 2017.RESULTS:The analysis showed that there was a close correlation between the monthly number of newly diagnosed cases and the composite search index (the Spearman's rank correlation coefficients were 0.777 for HIV/AIDS, 0.590 for syphilis and 0.633 for gonorrhoea, p<0.05 for all). The R2 were all more than 85% and the mean absolute percentage errors were less than 11%, showing the good fitting effect and prediction performance of vector autoregressive model in this field.CONCLUSIONS:Our study indicated the potential feasibility of using Baidu search data to monitor and predict the number of newly diagnosed cases of HIV/AIDS, syphilis and gonorrhoea in China.

Lung cancer mortality trends and disparities: A cross-sectional analysis 1999-2020

Abstract:Background: Lung cancer remains a leading cause of morbidity and mortality in the United States. Given the importance of epidemiological insight on lung cancer outcomes as the foundation for targeted interventions, we aimed to examine lung cancer death trends in the United States in the recent 22-year period, exploring demographic disparities and yearly mortality shifts. Methods: Mortality information was obtained from the CDC Wide-ranging Online Data for Epidemiologic Research database from the years 1999-2020. Demographic information included age, sex, race or ethnicity, and area of residence. We performed log-linear regression models to assess temporal mortality shifts and calculated average annual percentage change (AAPC) and compared age-adjusted mortality rates (AAMR) across demographic subpopulations. Results: A total of 3,380,830 lung cancer deaths were identified. The AAMR decreased from 55.4 in 1999-31.8 in 2020 (p<0.001). Males (AAMR 57.6) and non-Hispanic (NH) (AAMR 47.5) populations were disproportionately impacted compared to females (AAMR 36.0) and Hispanic (AAMR 19.1) populations, respectively. NH Black populations had the highest AAMR (48.5) despite an overall reduction in lung cancer deaths (AAPC -3.3 %) over the study period. Although non-metropolitan regions were affected by higher mortality rates, the annual decrease in mortality among metropolitan regions (AAPC -2.8 %, p<0.001) was greater compared to non-metropolitan regions (AAPC -1.7 %, p<0.001). Individuals living in the Western US (AAPC -3.4 %, p<0.001) experienced the greatest decline in lung cancer mortality compared to other US census regions. Conclusions: Our findings revealed lung cancer mortality inequalities in the US. By contextualizing these mortality shifts, we provide a larger framework of data-driven initiatives for societal and health policy changes for improving access to care, minimizing healthcare inequalities, and improving outcomes.

Abstract:In this study, we developed a method for modeling the progression and detection of lung cancer based on the smoking behavior at an individual level. The model allows obtaining the characteristics of lung cancer in a population at the time of diagnosis. Lung cancer data from Surveillance, Epidemiology and End Results (SEER) database collected between 2004 and 2008 were used to fit the lung cancer progression and detection model. The fitted model combined with a smoking based carcinogenesis model was used to predict the distribution of age, gender, tumor size, disease stage and smoking status at diagnosis and the results were validated against independent data from the SEER database collected from 1988 to 1999. The model accurately predicted the gender distribution and median age of LC patients of diagnosis, and reasonably predicted the joint tumor size and disease stage distribution.

Abstract:Awareness is vital for cancer prevention. US studies show a strong link between web searches and cancer incidence. In Europe, the relationship remains unclear. This study characterizes regional and temporal relationships between cancer incidence and web searches and investigates the content of searches related to breast, cervical, colorectal, lung, prostate, and testicular cancer, brain tumors, and melanoma in Germany (July 2018–December 2019). Aggregate data from Google Ads Keyword Planner and national cancer registry data were analyzed. Spearman's correlation coefficient ( r S ) examined associations between cancer incidence and web search, repeated measures correlation ( r rm ) assessed time trends and searches were qualitatively categorized. The frequency of malignancy-related web searches correlated with cancer incidence ( r S = 0.88, P = 0.007), e.g., breast cancer had more queries than the lower-incidence cervical cancer. Seasonally, incidence and searches followed similar patterns, peaking in spring and fall, except for melanoma. Correlations between entity incidence and searches (0.037 ≤ r rm ≤ 0.208) varied regionally. Keywords mainly focused on diagnosis , symptoms, and general information , with variations between entities. In Germany, web searches correlated with regional and seasonal incidence, revealing differences between North/East and South/West. These insights may help improve prevention strategies by identifying regional needs and assessing impact of awareness campaigns.
