Abstract:OBJECTIVESpinal osteotomies and anterior column realignment (ACR) are procedures that allow preservation or restoration of spine lordosis. Variations of these techniques enable different degrees of segmental, regional, and global sagittal realignment. The authors propose a comprehensive anatomical classification system for ACR and its variants based on the level of technical complexity and invasiveness. This serves as a common language and platform to standardize clinical and radiographic outcomes for the utilization of ACR.METHODSThe proposed classification is based on 6 anatomical grades of ACR, including anterior longitudinal ligament (ALL) release, with varying degrees of posterior column release or osteotomies. Additionally, a surgical approach (anterior, lateral, or posterior) was added. Reliability of the classification was evaluated by an analysis of 16 clinical cases, rated twice by 14 different spine surgeons, and calculation of Fleiss kappa coefficients.RESULTSThe 6 grades of ACR are as follows: grade A, ALL release with hyperlordotic cage, intact posterior elements; grade 1 (ACR + Schwab grade 1), additional resection of the inferior facet and joint capsule; grade 2 (ACR + Schwab grade 2), additional resection of both superior and inferior facets, interspinous ligament, ligamentum flavum, lamina, and spinous process; grade 3 (ACR + Schwab grade 3), additional adjacent-level 3-column osteotomy including pedicle subtraction osteotomy; grade 4 (ACR + Schwab grade 4), 2-level distal 3-column osteotomy including pedicle subtraction osteotomy and disc space resection; and grade 5 (ACR + Schwab grade 5), complete or partial removal of a vertebral body and both adjacent discs with or without posterior element resection. Intraobserver and interobserver reliability were 97% and 98%, respectively, across the 14-reviewer cohort.CONCLUSIONSThe proposed anatomical realignment classification provides a consistent description of the various posterior and anterior column release/osteotomies. This reliability study confirmed that the classification is consistent and reproducible across a diverse group of spine surgeons.

Abstract:Parts I and II have described a method of automatic pattern grading for both basic block and adapted patterns. Part III, the last of this series, will introduce the structure of the program and the use of the system. Some examples will be given to demonstrate its feasibility. The AutoGrader system has been implemented using CADDS 4X, which is a CAD software program, but the algorithms do not depend on this particular software, and the concept may be applied to any CAD system with the appropriate functionality.

Abstract:PurposeGlaucoma is the main cause of irreversible blindness worldwide. However, the diagnosis and treatment of glaucoma remain difficult because of the lack of an effective glaucoma grading measure. In this study, we aimed to propose an artificial intelligence system to provide adequate assessment of glaucoma patients.MethodsA total of 16,356 visual fields (VFs) measured by Octopus perimeters and Humphrey Field Analyzer (HFA) were collected, from three hospitals in China and the public Harvard database. We developed a fine-grained grading deep learning system, named FGGDL, to evaluate the VF loss, compared to ophthalmologists. Subsequently, we discuss the relationship between structural and functional damage for the comprehensive evaluation of glaucoma level. In addition, we developed an interactive interface and performed a cross-validation study to test its auxiliary ability. The performance was valued by F1 score, overall accuracy and area under the curve (AUC).ResultsThe FGGDL achieved a high accuracy of 85 and 90%, and AUC of 0.93 and 0.90 for HFA and Octopus data, respectively. It was significantly superior (p < 0.01) to that of medical students and nearly equal (p = 0.614) to that of ophthalmic clinicians. For the cross-validation study, the diagnosis accuracy was almost improved (p < 0.05).ConclusionWe proposed a deep learning system to grade VF of glaucoma with a high detection accuracy, for effective and adequate assessment for glaucoma patients. Besides, with the convenient and credible interface, this system can promote telemedicine and be used as a self-assessment tool for patients with long-duration diseases.
A Computer Aided Grading System for Subjective Tests

Abstract:Computer aided tests replace traditional written answers on paper sheets with electronic records. For subjective tests, computers are not able to do all grading jobs, due to limited comprehensive ability of computers. Subjective answers need be reviewed by different graders in order to improve justice. We propose a distributed computational model for grading electronic subjective answers. Answer data are divided into numerous independent data cells. Computational resources are automatically allocated by the system to do grading jobs and independently report grading results to the system. Since the grading results may vary slightly or magnificently, we assign computational factors to data cells and computational resources to minimize the variation. In addition, rules are defined to obtain a final result. We successfully apply the proposed model in our grading system in computer aided spoken English tests.

Abstract:During the 5-year-period from July 1983 to July 1988, ERCP was performed within 48 hours after onset of clinical symptoms in 63 patients with suspected gallstone pancreatitis. In 53 patients this etiology was confirmed, and acute endoscopic papillotomy was done in 30 of these patients. ERCP seems to be a safe diagnostic procedure in acute gallstone pancreatitis, and papillotomy gives effective drainage and relief of the acute symptoms.

Abstract:We propose an automated, explainable artificial intelligence (xAI) system for age-related macular degeneration (AMD) diagnosis. Mimicking the physician's perceptions, the proposed xAI system is capable of deriving clinically meaningful features from optical coherence tomography (OCT) B-scan images to differentiate between a normal retina, different grades of AMD (early, intermediate, geographic atrophy (GA), inactive wet or active neovascular disease [exudative or wet AMD]), and non-AMD diseases. Particularly, we extract retinal OCT-based clinical imaging markers that are correlated with the progression of AMD, which include: (i) subretinal tissue, sub-retinal pigment epithelial tissue, intraretinal fluid, subretinal fluid, and choroidal hypertransmission detection using a DeepLabV3+ network; (ii) detection of merged retina layers using a novel convolutional neural network model; (iii) drusen detection based on 2D curvature analysis; (iv) estimation of retinal layers' thickness, and first-order and higher-order reflectivity features. Those clinical features are used to grade a retinal OCT in a hierarchical decision tree process. The first step looks for severe disruption of retinal layers' indicative of advanced AMD. These cases are analyzed further to diagnose GA, inactive wet AMD, active wet AMD, and non-AMD diseases. Less severe cases are analyzed using a different pipeline to identify OCT with AMD-specific pathology, which is graded as intermediate-stage or early-stage AMD. The remainder is classified as either being a normal retina or having other non-AMD pathology. The proposed system in the multi-way classification task, evaluated on 1285 OCT images, achieved 90.82% accuracy. These promising results demonstrated the capability to automatically distinguish between normal eyes and all AMD grades in addition to non-AMD diseases.

Abstract:The skin temperature response of paraplegic skin to application of localized pressure has been measured and compared to the response of nonparalyzed skin. Following pressure release, skin temperature generally increased to a maximum value and then decreased slowly. The peak temperature rise observed was dependent on the magnitude and duration of applied pressure, with higher values of peak temperature rise associated with higher values of both the magnitude and duration of applied pressure. This trend is similar to that observed with nonparalyzed subjects. For both of these groups, the general pattern of temperature response curves are similar, and the magnitudes of peak temperature values are not much different. Skin temperature responses are believed to be due to reactive hyperemia. Pressure magnitudes and pressure application times used were below those needed to cause pressure sore formation.
Grade Like a Human: Rethinking Automated Assessment with Large Language Models

Abstract:While large language models (LLMs) have been used for automated grading, they have not yet achieved the same level of performance as humans, especially when it comes to grading complex questions. Existing research on this topic focuses on a particular step in the grading procedure: grading using predefined rubrics. However, grading is a multifaceted procedure that encompasses other crucial steps, such as grading rubrics design and post-grading review. There has been a lack of systematic research exploring the potential of LLMs to enhance the entire grading~process. In this paper, we propose an LLM-based grading system that addresses the entire grading procedure, including the following key components: 1) Developing grading rubrics that not only consider the questions but also the student answers, which can more accurately reflect students' performance. 2) Under the guidance of grading rubrics, providing accurate and consistent scores for each student, along with customized feedback. 3) Conducting post-grading review to better ensure accuracy and fairness. Additionally, we collected a new dataset named OS from a university operating system course and conducted extensive experiments on both our new dataset and the widely used Mohler dataset. Experiments demonstrate the effectiveness of our proposed approach, providing some new insights for developing automated grading systems based on LLMs.

Abstract:This dissertation describes the design and implementation of a computer vision based varve image analysis system. The primary issues covered are software engineering design, varve image calibrations, varve image enhancement, varve Dynamic Spatial Warping (DSW) profile generation, varve core image registration, varve identification, boundary identification and varve thickness measurement. A varve DSW matching algorithm is described to generate DSW profile and register two core images. Wavelet Multiple Resolution Analysis (MRA) is also used to do the core image registrations. By allowing an analyst to concentrate on other research work while the VARVES software analyzes a sample, much of the tedious varve analysis work is reduced, and potentially increasing the productivity. Additionally, by using new computer vision techniques, VARVES system is able to do some varve analysis which was impossible handled manually.

Abstract:BACKGROUND: Adult cervical deformity (ACD) classifications have not been implemented in a prospective ACD population and in conjunction with adult spinal deformity (ASD) classifications.OBJECTIVE: To characterize cervical deformity type and malalignment with 2 classifications (Ames-ACD and Schwab-ASD).METHODS: Retrospective review of a prospective multicenter ACD database. Inclusion: patients ≥18 yr with pre- and postoperative radiographs. Patients were classified with Ames-ACD and Schwab-ASD schemes. Ames-ACD descriptors (C = cervical, CT = cervicothoracic, T = thoracic, S = coronal, CVJ = craniovertebral) and alignment modifiers (cervical sagittal vertical axis [cSVA], T1 slope minus cervical lordosis [TS-CL], modified Japanese Ortphopaedic Association [mJOA] score, horizontal gaze) were assigned. Schwab-ASD curve type stratification and modifier grades were also designated. Deformity and alignment group distributions were compared with Pearson χ2/ANOVA.RESULTS: Ames-ACD descriptors in 84 patients: C = 49 (58.3%), CT = 20 (23.8%), T = 9 (10.7%), S = 6 (7.1%). cSVA modifier grades differed in C, CT, and T deformities (P < .019). In C, TS-CL grade prevalence differed (P = .031). Among Ames-ACD modifiers, high (1+2) cSVA grades differed across deformities (C = 47.7%, CT = 89.5%, T = 77.8%, S = 50.0%, P = .013). Schwab-ASD curve type and presence (n = 74, T = 2, L = 6, D = 2) differed significantly in S deformities (P < .001). Higher Schwab-ASD pelvic incidence minus lumbar lordosis grades were less likely in Ames-ACD CT deformities (P = .027). Higher pelvic tilt grades were greater in high (1+2) cSVA (71.4% vs 36.0%, P = .015) and high (2+3) mJOA (24.0% vs 38.1%, P = .021) scores. Postoperatively, C and CT deformities had a trend toward lower cSVA grades, but only C deformities differed in TS-CL grade prevalence (0 = 31.3%, 1 = 12.2%, 2 = 56.1%, P = .007).CONCLUSION: Cervical deformities displayed higher TS-CL grades and different cSVA grade distributions. Preoperative associations with global alignment modifiers and Ames-ACD descriptors were observed, though only cervical modifiers showed postoperative differences.

Abstract:Grading of examination papers is a hectic, time-labor intensive task and is often subjected to inefficiency and bias in checking. This research project is a primitive experiment in the automation of grading of theoretical answers written in exams by students in technical courses which yet had continued to be human graded. In this paper, we show how the algorithmic approach in machine learning can be used to automatically examine and grade theoretical content in exam answer papers. Bag of words, their vectors & centroids, and a few semantic and lexical text features have been used overall. Machine learning models have been implemented on datasets manually built from exams given by graduating students enrolled in technical courses. These models have been compared to show the effectiveness of each model.
Assessment of Radiological and Morphological Grading Systems of Intervertebral Disc Degeneration

Abstract:Introduction: Intervertebral disc (IVD) degeneration is considered to be one of the main pathophysiological causes of low back pain. Several grading systems have been developed for both morphological and radiological assessment. The aim of this study was to assess the morphological and radiological characteristics of IVD degeneration and validate popular radiological Pfirrmann scale against morphological Thompson grading system. Methodology: Full spinal columns (vertebrae L1-S1 and IVD between them) were harvested from cadavers through an anterior dissection. MRI scans of all samples were conducted. Then, all vertebral columns were cut in the midsagittal plane and assessed morphologically. Result: A total of 100 lumbar spine columns (446 IVDs) were included in the analysis of the degeneration grade. Morphologic Thompson scale graded the majority of discs as grade 2 and 3 (44.2% and 32.1%, respectively), followed by grade 4 (16.8%), grade 1 (5.8%), and grade 5 (1.1%). The Radiologic Pfirrmann grading system classified 44.2% of discs as grade 2, 32.1% as grade 3, 16.8% as grade 4, 5.8% as grade 1, and 1.1% as grade 5. The analysis on the effect of age on degeneration revealed significant, although moderate, positive correlation with both scales. Analysis of the agreement between scales showed weighted Cohen’s kappa equal to 0.61 (p < 0.001). Most of the disagreement occurred due to a 1-grade difference (91.5%), whereas only 8.5% due to a 2-grade difference. Conclusion: With the increase in the prevalence of IVD disease in the population, reliable grading systems of IVD degeneration are crucial for spine surgeons in their clinical assessment. While overall there is an agreement between both grading systems, clinicians should remain careful when using Pfirmann scale as the grades tend to deviate from the morphological assessment.
Simplified Universal Grading of Lumbar Spine MRI Degenerative Findings: Inter-Reader Agreement of Non-Radiologist Spine Experts

Abstract:Abstract Objective 1) To describe a simplified multidisciplinary grading system for the most clinically relevant lumbar spine degenerative changes. 2) To measure the inter-reader variability among non-radiologist spine experts in their use of the classification system for interpretation of a consecutive series of lumbar spine magnetic resonance imaging (MRI) examinations. Methods ATS multidisciplinary and collaborative standardized grading of spinal stenosis, foraminal stenosis, lateral recess stenosis, and facet arthropathy was developed. Our institution’s picture archiving and communication system was searched for 50 consecutive patients who underwent non-contrast MRI of the lumbar spine for chronic back pain, radiculopathy, or symptoms of spinal stenosis. Three fellowship-trained spine subspecialists from neurosurgery, orthopedic surgery, and physiatry interpreted the 50 exams using the classification at the L4–L5 and L5–S1 levels. Inter-reader agreement was assessed with Cohen’s kappa coefficient. Results For spinal stenosis, the readers demonstrated substantial agreement (κ = 0.702). For foraminal stenosis and facet arthropathy, the three readers demonstrated moderate agreement (κ = 0.544, and 0.557, respectively). For lateral recess stenosis, there was fair agreement (κ = 0.323). Conclusions A simplified universal grading system of lumbar spine MRI degenerative findings is newly described. Use of this multidisciplinary grading system in the assessment of clinically relevant degenerative changes revealed moderate to substantial agreement among non-radiologist spine physicians. This standardized grading system could serve as a foundation for interdisciplinary communication.

Abstract:Many instructors choose to assess their students using open-ended written exam items that require students to show their understanding of physics by solving a problem and/or explaining a concept. Grading these items is fairly time consuming, and in large courses time constraints prohibit providing significant individualized feedback on students' exams. Instructors typically cross out areas of the response that are incorrect and write the total points awarded or subtracted. Sometimes, instructors will also write a word or two to indicate the error. This paper describes a grading method that provides greater individualized feedback, clearly communicates to students expected performance levels, takes no more time than traditional grading methods for open-ended responses, and seems to encourage more students to take advantage of the feedback provided.

Almost fine gradings on algebras and classification of gradings up to isomorphism

Abstract:We consider the problem of classifying gradings by groups on a finite-dimensional algebra $A$ (with any number of multilinear operations) over an algebraically closed field. We introduce a class of gradings, which we call almost fine, such that every $G$-grading on $A$ is obtained from an almost fine grading on $A$ in an essentially unique way, which is not the case with fine gradings. For abelian groups, we give a method of obtaining all almost fine gradings if fine gradings are known. We apply these ideas to the case of semisimple Lie algebras in characteristic $0$: to any abelian group grading with nonzero identity component, we attach a (possibly nonreduced) root system and, in the simple case, construct an adapted grading by this root system.

Abstract:The Genant's semi-quantitative (GSQ) criteria is currently the most used approach in epidemiology studies and clinical trials for osteoporotic vertebral deformity (OVD) evaluation with radiograph. The qualitative diagnosis with radiological knowledge helps to minimize false positive readings. However, unless there is a face-to-face training with experienced readers, it can be difficult to apply GSQ criteria by only reading the text description of Genant <i>et al.</i> (in 1993), even for a musculoskeletal radiologist. We propose an expanded semi-quantitative (eSQ) OVD classification with the following features: (I) GSQ grade-0.5 is noted as minimal grade (eSQ grade-1) for OVDs with height loss <20%; (II) GSQ mild grade (grade-1) is the same as eSQ mild grade (grade-2); (III) GSQ moderate grade (grade-2) is subdivided into eSQ grade-3 (moderate, >25%-1/3 height loss) and eSQ grade-4 (moderately-severe, >1/3-40% height loss); (IV) GSQ severe grade is subdivided into eSQ grade-5 (severe, >40%-2/3 height loss) and eSQ grade-6 (collapsed, with >2/3 height loss). We advocate to estimate vertebral height loss with adjacent vertebral heights as the reference (rather than using individual vertebra's posterior height as the reference). This article presents radiographs of 36 cases with OVD, together with gradings using GSQ criteria and eSQ criteria. The examples in this article can serve as teaching material or calibration database for readers who will use GSQ criteria or eSQ criteria. Our approach for quantitative measurement is explained graphically.

Grading of Aortic Valve Calcification Severity and Risk Stratification in Aortic Stenosis

Abstract:Background: Thresholds of aortic valve calcification (AVC) to define hemodynamically moderate aortic stenosis (AS) from mild are lacking. We aimed to establish a novel grading classification of AVC as quantified by computed tomography and determine its prognostic value. Methods and results: This study included 915 patients with at least mild AS (mean age 70±12 years, 30% women) from a multicenter prospective registry. All patients underwent Doppler-echocardiography and noncontrast computed tomography within 3 months. Primary end point was the occurrence of all-cause death. Receiver operating characteristic curves analyses were used to determine the sensitivity and specificity of sex-specific thresholds of AVC to identify hemodynamically moderate AS. Optimal thresholds (ie, with best sensitivity/specificity) of AVC to distinguish moderate (aortic valve area 1.0-1.5 cm2 and mean gradient 20-39 mm Hg) from mild AS (aortic valve area >1.5 cm2 and mean gradient <20 mm Hg) were AVC ≥360 arbitrary units in women and ≥1037 arbitrary units in men. Based on the guidelines' thresholds for severe AS and the new thresholds in our study for moderate AS, 312 (34%) patients had mild, 253 (28%) moderate, and 350 (38%) severe AVC. During a mean follow-up of 5.6±3.9 years, 183 (27%) deaths occurred. In Cox multivariable models, AVC remained associated with an increased risk of death (adjusted hazard ratio per grade increase, 1.94 [95% CI, 1.53-2.56]; P<0.001). Conclusions: A novel grading classification of anatomic AS severity based on sex-specific thresholds of AVC provides significant prognostic value for predicting mortality. These findings support the complementarity of computed tomography-calcium scoring to Doppler-echocardiography to corroborate AS severity and enhance risk stratification in patients with AS.

Abstract:This technical report presents SpineNetV2, an automated tool which: (i) detects and labels vertebral bodies in clinical spinal magnetic resonance (MR) scans across a range of commonly used sequences; and (ii) performs radiological grading of lumbar intervertebral discs in T2-weighted scans for a range of common degenerative changes. SpineNetV2 improves over the original SpineNet software in two ways: (1) The vertebral body detection stage is significantly faster, more accurate and works across a range of fields-of-view (as opposed to just lumbar scans). (2) Radiological grading adopts a more powerful architecture, adding several new grading schemes without loss in performance. AVRA: Automatic Visual Ratings of Atrophy from MRI images using Recurrent Convolutional Neural Networks

Abstract:Quantifying the degree of atrophy is done clinically by neuroradiologists following established visual rating scales. For these assessments to be reliable the rater requires substantial training and experience, and even then the rating agreement between two radiologists is not perfect. We have developed a model we call AVRA (Automatic Visual Ratings of Atrophy) based on machine learning methods and trained on 2350 visual ratings made by an experienced neuroradiologist. It provides fast and automatic ratings for Scheltens' scale of medial temporal atrophy (MTA), the frontal subscale of Pasquier's Global Cortical Atrophy (GCA-F) scale, and Koedam's scale of Posterior Atrophy (PA). We demonstrate substantial inter-rater agreement between AVRA's and a neuroradiologist ratings with Cohen's weighted kappa values of $\kappa_w$ = 0.74/0.72 (MTA left/right), $\kappa_w$ = 0.62 (GCA-F) and $\kappa_w$ = 0.74 (PA), with an inherent intra-rater agreement of $\kappa_w$ = 1. We conclude that automatic visual ratings of atrophy can potentially have great clinical and scientific value, and aim to present AVRA as a freely available toolbox.

Dental Cavity Grading: Comparing Algorithm Reliability and Agreement with Expert Evaluation

Abstract:Aim: The current study introduces a novel, algorithm-based software developed to objectively evaluate dental cavity preparations. The software aims to provide an alternative or complement to traditional, subjective assessment methods used in operative dentistry education. Materials and methods: The software was tested on cavity preparations carried out by 70 participants on artificial molar teeth. These cavities were also independently assessed by an experienced academic panel. The software, using 3D imaging, calculated cavity dimensions and assigned an error score based on deviation from ideal measurements. Statistical analyses included sensitivity, specificity, positive predictive value, negative predictive value, Cohen's kappa, the intraclass correlation coefficient (ICC3k), Spearman's rho, Kendall's tau correlation coefficients, and a confusion matrix. Result: The software demonstrated a high degree of accuracy and agreement with the panel assessments. The average software and panel scores were 64.1 and 60.91, respectively. Sensitivity (0.98) was high, specificity (0.55) was moderate, and the ICC3k value (0.857) indicated a strong agreement between the software and the panel. Further, Spearman's rho (0.73) and Kendall's tau (0.56) suggested a strong correlation between the two grading methods. Conclusion: The results support the algorithm-based software as a valid and reliable tool for dental cavity preparation assessments. The software's potential use in dental education is promising, though future research is necessary to validate and optimize this technology for wider application.
