Abstract:The both purposes of our investigation were to determine initially the level of total serum IgE in various parasitic infections and the variations of this level after the initiation of specific treatment. In the study, III sera from black race patients with a clinical and biological diagnosis of helmintic or protozoal infections and without allergic diseases were tested by the commercially available "Radioimmunosorbent test" (Pharmacia Laboratories). In helmintic infections: the IgE concentration was higher than normal level: --in 100% of patients infested with ankylostoma, trichocephalus (mean concentration m: 2,961 UI/ml) or schistosoma (m: 1,578 UI/ml); --in 70% of patients infested with ascaris (m: 1,110 UI/ml). In protozoal infections: the abnormal IGE level was found only in 44% and 14% of cases respectively of malaria (m = 704 UI/ml) and of leishmaniasis (441 UI/ml). The time course of total serum IgE determined after the treatment initiation showed that: a) in patients with intracellular helminthiasis and protozoal infections serum IgE concentrations were gradually increased in a statistically highly significant way; b) in cases with intestinal helminthiasis, high levels disappeared ane returned rapidly to normal values.
Acute respiratory illness among a prospective cohort of pediatric patients using emergency medical services in India: Demographic and prehospital clinical predictors of mortality

Abstract:Background: In India, acute respiratory illnesses, including pneumonia, are the leading cause of early childhood death. Emergency medical services are a critical component of India's public health infrastructure; however, literature on the prehospital care of pediatric patients in low- and middle-income countries is minimal. The aim of this study is to describe the demographic and clinical characteristics associated with 30-day mortality among a cohort of pediatric patients transported via ambulance in India with an acute respiratory complaint. Methods: Pediatric patients less than 18 years of age using ambulance services in one of seven states in India, with a chief complaint of "shortness of breath", or a "fever" with associated "difficulty breathing" or "cough", were enrolled prospectively. Patients were excluded if evidence of choking, trauma or fire-related injury, patient was absent on ambulance arrival, or refused transport. Primary exposures included demographic, environmental, and clinical indicators, including hypoxemia and respiratory distress. The primary outcome was 7 and 30-day mortality. Multivariable logistic regression, stratified by transport type, was constructed to estimate associations between demographic and clinical predictors of mortality. Results: A total of 1443 patients were enrolled during the study period: 981 (68.5%) were transported from the field, and 452 (31.5%) were interfacility transports. Thirty-day response was 83.4% (N = 1222). The median age of all patients was 2 years (IQR: 0.17-10); 93.9% (N = 1347) of patients lived on family incomes below the poverty level; and 54.1% (N = 706) were male. Cumulative mortality at 2, 7, and 30-days was 5.2%, 7.1%, and 7.7%, respectively; with 94 deaths by 30 days. Thirty-day mortality was greatest among those 0-28 days (N = 38,17%); under-5 mortality was 9.8%. In multivariable modeling prehospital oxygen saturation <95% (OR: 3.18 CI: 1.77-5.71) and respiratory distress (OR: 3.72 CI: 2.17-6.36) were the strongest predictors of mortality at 30 days. Conclusions: This is the first study to detail prehospital predictors of death among pediatric patients with shortness of breath in LMICs. The risk of death is particularly high among neonates and those with documented mild hypoxemia, or respiratory distress. Early recognition of critically ill children, targeted prehospital interventions, and diversion to higher level of care may help to mitigate the mortality burden in this population.
Characteristics and health insurance profile of brought-in-dead children in a rural tertiary hospital

Abstract:Background: Despite the regular occurrence of brought-in-dead (BID) phenomenon, little is known about their prevalence, pattern and prehospital contributors in rural settings in Nigeria. In addition, the impact of health insurance in this context has not been described in our locale. Our study aimed at identifying the characteristics and access to health insurance of paediatric BID cases. Design: A cross-sectional, retrospective study in the paediatric emergency department of a rural tertiary hospital. Methods: Data were obtained from the medical records of BID cases from January 2019 to December 2023. Their sociodemographic details, the nature and duration of preceding illnesses/complaints, prior treatment, referral and health insurance types were obtained. Results: Sixty-seven (1.38%) of the 4872 admissions were BID. Their ages ranged between 12 hours and 14 years with a male:female ratio of 1.1:1. Forty-four (69.84%) were under 5s. Acute encephalitis syndrome, gastroenteritis and sepsis/severe malaria were the most prevalent presumed causes of death. Prior to arrival, 30.56% had obtained treatment outside the hospital setting (medicine vendors, herbal treatment, home nurse and religious centres). Almost half of the referrals were informal referrals by family and neighbours. None (0%) presented health insurance records nor was brought in by an ambulance. Conclusion: Infectious diseases were the most common presumed causes of death and under 5s bore the greatest burden of BID. The healthcare choices were poor, but the informal sociocultural support system was strong. There was a dearth of health insurance enrolment, a poor patient referral and transfer system, and poor documentation of BID cases. We identify the need for concerted health education, improved patient referral and transfer, well-coordinated death registration and appropriate health insurance schemes in rural communities.

Abstract:Background: Globally, the proportion of child deaths that occur in the neonatal period remains a high level of 37-41%. Differences of cause in neonate death exist in different regions as well as in different economic development countries. The specific aim of this study was to investigate the causes, characteristics, and differences of death in neonates during hospitalization in the tertiary Neonatal Intensive Care Unit (NICU) of China. Methods: All the dead neonates admitted to 26 NICUs were included between January 1, 2011, and December 31, 2011. All the data were collected retrospectively from clinical records by a designed questionnaire. Data collected from each NICU were delivered to the leading institution where the results were analyzed. Results: A total of 744 newborns died during the 1-year survey, accounting for 1.2% of all the neonates admitted to 26 NICUs and 37.6% of all the deaths in children under 5 years of age in these hospitals. Preterm neonate death accounted for 59.3% of all the death. The leading causes of death in preterm and term infants were pulmonary disease and infection, respectively. In early neonate period, pulmonary diseases (56.5%) occupied the largest proportion of preterm deaths while infection (27%) and neurologic diseases (22%) were the two main causes of term deaths. In late neonate period, infection was the leading cause of both preterm and term neonate deaths. About two-thirds of neonate death occurred after medical care withdrawal. Of the cases who might survive if receiving continuing treatment, parents' concern about the long-term outcomes was the main reason of medical care withdrawal. Conclusions: Neonate death still accounts for a high proportion of all the deaths in children under 5 years of age. Our study showed the majority of neonate death occurred in preterm infants. Cause of death varied with the age of death and gestational age. Accurate and prompt evaluation of the long-term outcomes should be carried out to guide the critical decision.
Knowledge, Awareness, and Understanding of Pediatric Triage Among Nursing Officers in India: A Multicenter Study

Abstract:Introduction: Triage is crucial in patient screening within emergency departments (EDs) worldwide. It is one of the essential and standard medical practices in many developed countries. However, in India, there is a need for improvement in triage utilization, as it is predominantly performed by resident doctors or medical officers, leading to an uneven distribution of clinical skills among healthcare providers (HCPs). A comprehensive analysis incorporating literature review and data collection revealed that while mandatory screening is conducted in most Indian EDs, the formal implementation of standardized triage protocols remains limited. Like in developed countries, registered nurses or nursing officers (NOs) can be effectively trained and directed to play the role of dedicated triage personnel in EDs of most of the healthcare facilities in India. Method and materials: This study aimed to examine the current state of triage utilization and its impact on the distribution of responsibilities among HCPs in Indian EDs. Through this online survey, the investigators assessed the knowledge and practical understanding of clinical triaging among NOs, working at various hospitals nationwide. Results: The participants included 5,029 NOs working in various parts of India, predominantly nursing graduates (82.52%), the majority being employed in government healthcare settings (84.01%) and most having over five years of cumulative working experience in the ED (70.77%). Nurses showed inadequate knowledge and awareness about the Pediatric Assessment Triangle (PAT) used for quick initial evaluation (62.18% among all participants). Concerning the complete triage process applicable, especially in pediatric ED settings, they had even less satisfactory knowledge and understanding, e.g., identifying primary (28.27%) and secondary (22.69%) survey components via focused history and examination, properly using temperature assessment (23.32%) and instant blood glucose level assessment (22.95%) in triage, and knowing various types of internationally accepted triage systems for ED-based health facilities such as the Emergency Severity Index (ESI), Canadian Triage and Acuity Scale (CTAS), and Australasian Triage Scale (ATS) (15.87%). ANOVA and post hoc analysis revealed that the intergroup performance of the study participants with maximum correct responses to the knowledge-determining specified subset of the questionnaire depicts the significantly higher role of graduate nursing degree over diploma such as General Nursing and Midwifery (GNM)/Auxiliary Nursing and Midwifery (ANM) qualification, working in government hospital versus private setup, and ED working experience of >5 years over that of <5 years. Conclusions: Of the participants in the study, 50% were not evaluated for cognitive or psychomotor domains during their assessment examinations. The research illuminated a significant disparity in knowledge and proficiency levels among Indian nurses concerning pediatric triage, especially with the ability to effectively apply the PAT for initial patient evaluations, discern components of primary and secondary surveys, and comprehend various triage systems. This study underscores the importance of comprehensive reform in the Indian healthcare system and teaching curriculum by emphasizing clinical triage training and interprofessional collaboration, and establishing guidelines and regulations to ensure consistent and standardized triage practices across all EDs.
Association of the distance travelled, and the call to hospital arrival time with early neonatal mortality in neonates born to mothers using emergency medical services at term gestation: a retrospective observational study

Abstract:Background Most neonatal deaths in India occur due to a lack of timely and proper care just before and during delivery. Timely availability of an ambulance for institutional delivery has led to some decrease in this. However, the distance travelled and time taken may affect the outcome. Objective The objective of this retrospective observational study was to determine if the distance travelled and the call to hospital arrival time are associated with deaths in the early neonatal period (0–7 days postnatal age), in neonates born to mothers who used a centralised emergency medical services (EMS) agency transportation at term gestation. Design Retrospective observational study. Setting This was a secondary analysis of a de-identified patient dataset from a previous cohort study on EMS usage by pregnant women, in five Indian states in 2014. Participants Neonates born to mothers using EMS at ≥37–42 weeks gestation were included as participants. The groups of pregnant individuals with low distance (≤10 km) and time (≤60 min) to hospital arrival vs longer distance or time to hospital arrival were then compared for neonatal death in the first 7 days of life or early neonatal death (ENND), which was the primary outcome. 2 test and logistic regression were carried out. Additional analysis was also planned to test the higher cut-offs of distance and time, if the above cut-offs were not found to have a statistically significant association with ENND. Results There were a total of 1180 neonates meeting the inclusion criteria. Of these, 272 (23%) were born to mothers who travelled ≤10 km (km) and took time ≤60 min to reach the hospital. There were a total of 57 (4.8%) ENNDs of which 14 occurred in the low distance and time group (≤10 km and ≤60 min). There was no statistically significant difference between the groups for ENND (p value=0.8). On additional univariate analysis for the different cut-offs of distance and time separately, it was found that there were lesser odds of ENND for a cut-off of distance ≤60 km (OR 0.28 (95% CI 0.08 to 0.98), p value=0.046) and for a cut-off time ≤120 min (OR 0.36 (95% CI 0.17 to 0.76), p value=0.007). When adjusted for other variables found to be significant on univariate analysis (caesarean section, <4 antenatal visits, maternal tachycardia and twin gestation) a cut-off distance ≤60 km had a trend towards lower odds of ENND (OR 0.31 (95% CI 0.084 to 1.12), p value=0.075) while a cut-off time of≤120 min had a significantly lower odds of ENND (OR 0.37 (95% CI 0.167 to 0.81), p value=0.013). When both of these cut-offs were combined and analysed in the multivariable logistic regression model, the association with ENND was still significant (OR 0.33 (95% CI 0.16 to 0.72), p value=0.005). Conclusion In neonates born to mothers using EMS at term gestation in India, those with distance travelled by the mother ≤60 km and time taken to reach health facility ≤120 min, had 67% lower odds of ENND compared with those with distance travelled >60 km or time taken >120 min. Further prospective observational studies are required to validate these cut-offs in a larger population.

Abstract:Background Understanding the epidemiological patterns of febrile infants can offer valuable insights for optimising management strategies and developing quality improvement initiatives, aiming to improve healthcare delivery in high-volume, low-resource emergency departments (EDs). Objectives To characterise the epidemiology of febrile infants presenting to the paediatric ED of a tertiary care hospital. Methods A retrospective chart review of medical records was performed for febrile infants ≤1 year old, at paediatric ED, Indus Hospital and Health Network (IHHN), Karachi, Pakistan (1 January 2020–31 December 2020). Results There were a total of 2311 patients in the study, with a male-to-female ratio of 1.4:1. The mean age of presentation was 4.9±2.7 months. Cough (n=1002, 43.2%) was the most frequent presenting symptom. The most common provisional ED diagnosis in ≤1 month of age was sepsis (n=98, 51%), bronchopneumonia (n=138, 28.6%) in 1.1–3 and 3.1–6 months (n=176, 36.45%); and upper respiratory tract illness (n=206, 47.4%) in 6.1–12 months of age. Age was significantly associated with provisional ED diagnosis and outcomes (p<0.001). Of 175 ED admissions (n=47, 26.8%), patients were discharged with a hospital diagnosis of bronchopneumonia and (n=27, 15.4) of sepsis. The infant mortality rate was 3/1000 live births. Conclusions This study is the first of its kind to explore the epidemiology of febrile infants in Pakistan, highlighting the burden and severity of respiratory illnesses and sepsis. It underscores the challenges of resource-limited settings, failing to meet the need for admission of febrile infants presenting to ED, IHHN. Moreover, it has highlighted the necessity to optimise the existing triage systems to effectively allocate resources and manage high patient volumes in low-resource EDs.

Emergency obstetric referrals in public health facilities: A descriptive study from urban Maharashtra, India

Abstract:Background An effective referral system is key to access timely emergency obstetric care. The criticality of referrals makes it necessary to understand its pattern at the health system level. This study aims to document the patterns and primary reasons of obstetric case referral and the maternal and perinatal outcome of the cases in public health institutions in select areas of urban Maharashtra, India. Methods The study is based on the health records of public health facilities in Mumbai and its adjoining three municipal corporations. The information on pregnant women referred for obstetric emergencies was collected from patient referral forms of municipal maternity homes and peripheral health facilities between 2016 and 2019. Maternal and child outcome data was obtained from “Received-In” peripheral and tertiary health facilities to track whether the referred woman reached the referral facility for delivery. Descriptive statistics were used to analyze demographic details, referral patterns, reasons of referrals, referral communication and documentation, time and mode of transfer and delivery outcomes. Results 14% (28,020) women were referred to higher health facilities. The most common reasons for referral were pregnancy-induced hypertension or eclampsia (17%), previous caesarean section (12%), fetal distress (11%) and Oligohydramnios (11%). 19% of all referrals were entirely due to unavailability of human resources or health infrastructure. Non-availability of emergency Operation Theatre (47%) and Neonatal Intensive Care Unit (45%) were the major non-medical reasons for referrals. Absence of health personnel such as anaesthetist (24%), paediatrician (22%), physician (20%) or obstetrician (12%) was another non-medical reason for referrals. Referring facility had a phone-based communication about the referral with the receiving facility in less than half of the cases (47%). 60% of the referred women could be tracked in higher health facilities. Of the tracked cases, 45% women delivered via caesarean section. Most of the deliveries (96%) resulted in live birth outcomes. 34% of the newborns weighed less than 2,500 grams. Conclusion Improving referral processes are critical to enhance the overall performance of emergency obstetric care. Our findings emphasize the need for a formal communication and feedback system between referring and receiving facilities. Simultaneously, ensuring EmOC at different levels of health facilities by upgradation of health infrastructure is recommended.
Lessons from implementation research on community management of Possible Serious Bacterial Infection (PSBI) in young infants (0-59 days), when the referral is not feasible in Palwal district of Haryana, India

Abstract:Background: Neonatal sepsis is a major cause of death in India, which needs hospital management but many families cannot access hospitals. The World Health Organization and the Government of India developed a guideline to manage possible serious bacterial infection (PSBI) when a referral is not feasible. We implemented this guideline to achieve high coverage of treatment of PSBI with low mortality. Methodology: The implementation research study was conducted in over 50 villages of Palwal district, Haryana during August 2017-March 2019 and covered a population of 199143. Policy dialogue with central, state and district health authorities was held before initiation of the study. A baseline assessment of the barriers in the implementation of the PSBI intervention was conducted. The intervention was implemented in the program setting. The research team collected data throughout and also co-participated in the implementation of the intervention for the first six months to identify bottlenecks in the health system and at the community level. RE-AIM framework was utilized to document implementation strategies of PSBI management guideline. Implementation strategies by the district technical support unit (TSU) included: (i) empower mothers and families through social mobilization to improve care-seeking of sick young infants 0-59 days of age, (ii) build capacity through training and build confidence through technical support of health staff at primary health centers (PHC), community health centers (CHC) and sub-centers to manage young infants with PSBI signs and (iii) improve performance of accredited social health activists (ASHAs). Findings: A total of 370 young infants with signs of PSBI were identified and managed in 5270 live births. Treatment coverage was 70% assuming that 10% of live births would have PSBI within the first two months of life. Mothers identified 87.6% (324/370) of PSBI cases. PHCs and CHCs became functional and managed 150 (40%) sick young infants with PSBI. Twenty four young infants (7-59days) who had only fast breathing were treated with oral amoxicillin without a referral. Referral to a hospital was refused by 126 (84%); 119 had clinical severe infection (CSI), one 0-6 days old had fast breathing and six had critical illness (CI). Of 119 CSI cases managed on outpatient injection gentamicin and oral amoxicillin, 116 (96.7%) recovered, 55 (45.8%) received all seven gentamicin injections and only one died. All 7-59 day old infants with fast breathing recovered, 23 on outpatient oral amoxicillin treatment; and 19 (79%) received all doses. Of 65 infants managed at either district or tertiary hospital, two (3.1%) died, rest recovered. Private providers managed 155 (41.9%) PSBI cases, all except one recovered, but sub-classification and treatment were unknown. Sub-centers could not be activated to manage PSBI. Conclusion: The study demonstrated resolution of implementation bottlenecks with existing resources, activated PHCs and CHCs to manage CSI and fast breathers (7-59 day old) on an outpatient basis with low mortality when a referral was not feasible. TSU was instrumental in these achievements. We established the effectiveness of oral amoxicillin alone in 7-59 days old fast breathers and recommend a review of the current national policy.

Abstract:<h3> Introduction and Objectives</h3><p>Acute respiratory infections (ARI), a major cause of childhood mortality and morbidity, place substantial burden on health services worldwide. Due to scarce data regarding respiratory illnesses requiring Pediatric intensive care unit (PICU) admissions (especially in low income/developing countries), we studied the burden of respiratory illnesses in the PICU and the risk factors contributing to mortality.</p><h3> Methods</h3><p>This prospective observational study was conducted over 18 months. Children (aged 1 month to 12 years) admitted to the PICU for acute respiratory/cardiorespiratory illnesses were enrolled. Demographic and clinical details of the study population were recorded and tabulated. Risk factors contributing to mortality (severity of illness [PRISM III score], diagnosis/etiology, need for mechanical ventilation, immunocompromised status, malnutrition, and length of stay [LOS]) were analyzed (using the <i>χ</i><sup>2</sup> test or Fischer Exact test). </p><h3> Results</h3><p>Two hundred and ninety‐three children were enrolled (median age: 5 months; range, 1‐132 months; male/female: 181/112). Mean LOS in PICU was 5.25 ± 5.48 days and mean length of hospital stay of 14.82 ± 13.35 days. Mechanical ventilation was required in 62.8% cases. Pneumonia contributed to 66.89% of respiratory admissions and 91% of mortality. Ninety patients (30.7%) died; mortality being higher in cardiorespiratory cases (52.7%). PRISM III score and shock (respiratory cases) and age below 1 year (cardiorespiratory group) were significantly associated with higher mortality (<i>P</i> </p><h3> Conclusions</h3><p>Respiratory illnesses are a significant contributor to PICU admissions and are associated with significant mortality risk in presence of high PRISM III score and shock (respiratory group); and age below 1 year (cardiorespiratory group).</p>

Abstract:Introduction: Despite its importance in emergency medicine, the practice of triage in low- and middle-income countries remains rudimentary, due to dearth of human and material resources, among other reasons. Subjects and methods: In the Children Emergency Room of a teaching hospital, some nonclinical staff were intermittently deployed to coordinate patient traffic and provide brief first- contact triage on arrival. For this, they were internally trained in triage. The present study was aimed at assessing the value and accuracy of first-contact triage provided by nonclinical staff in a children emergency department of a tertiary hospital. Results: Data of 45 patients seen during the period were obtained from the records of the triage officers and included their age, gender, presenting complaints and the findings of the paediatric assessment triangle. There was a statistically significant relationship between the triage ratings of the nonCC –BY 4.0 clinical staff and the qSOFA / L- qSOFA scores obtained by the emergency room doctors, the time spent in CHER and the total duration of admission (R2= 0.36 - 0.44; p = 0.003), (R2= 0.23 - 0.28; p = 0.018) and (R2= 0.33 - 0.40; p = 0.007) respectively. Conclusion: This study shows that to a high degree, the nonclinical staff correctly categorized the severity of illness in the Paediatric Emergency Department of a tertiary level hospital, thus limiting undue tertiary delay in treatment and potential harm from such delays. Hence, the study exposes a potential source of human resources for paediatric emergency triage in LMICs where there is a dearth of professional medical personnel.

Abstract:Summary This report describes a cell culture system with nasal epithelial (NE) cells in which the interferon (IF) response of these cells was compared with the IF response of human foreskin fibroblast (HFF) cells. The antiviral state that developed in NE cells after exposure to human leukocyte IF was less than the antiviral state that developed in HFF cells. In addition, the slope of the dose-response curve for HFF cells was greater than that for NE cells. It could be demonstrated that the antiviral state developed rapidly in NE cells following a brief (15-min) exposure to IF at 34°C provided a high IF concentration was used. Two respiratory viruses were tested for sensitivity to the IF-induced antiviral state that develops in NE cells. Both coxsackievirus A type 21 and parainfluenza virus type 3 were slightly less sensitive to the IF-induced antiviral state in NE cells compared to vesicular stomatitis virus. Abstract:Abstract Introduction The medical complexity of patients referred to our pediatric laboratory has been increasing. These children are at higher risk of severe sleep disordered breathing and/or risk of emergent medical issues during the study. As a result, patients may need to be referred directly to emergency room or inpatient admission from the ambulatory sleep lab. In this single site pediatric sleep lab quality improvement project, we identify the frequency of ED/inpatient referrals from attended, in-lab sleep studies, reasons for referrals, and contributing factors for referral. Methods This is a cohort study of patients who had in-lab ambulatory polysomnogram at Boston Children’s Hospital between March 2017 to May 2022 identified through chart review search terms (ED or inpatient diagnostic codes within 24 hours of polysomnogram billing code). We performed chart review for demographics, past medical history, sleep study variables, medications, and disposition. We used chi-square tests to compare age and medical complexity categories. Results Of 6724 PSG studies conducted, 95 (1.4%) patients were referred to ED or inpatient admission within 24 hours from the sleep study. Demographics: Patients ranged in age from 1 month to 20 years [mean age 5.5(6) years] and the cohort was 38% female, 53% Caucasian, 12% Black, 2% Asian, 13% Hispanic, 6% other, 14% race/ethnicity not reported. Of the 95 patients referred to ED/inpatient hospitalization, 64% needed continued medical assistance in the morning due to various problems including persistent hypoxia, hypercarbia, altered mental status, difficulty tolerating oral intake, or need for assistance ambulating. Features such as age and patient triage level assigned prior to sleep study did not predict whether patient returned to baseline upon wakening. The reason for referral for ED or admission included severe sleep disordered breathing (e.g. obstructive sleep apnea or central sleep apnea; 33.8%), persistent hypoxia/hypercarbia (23.1%), cardiac (e.g. arrhythmias, worsening heart disease; 10.8%), vomiting/diarrhea (10.8%), neurologic (e.g. seizures, paroxysmal events, altered mental status; 7.7%), infection (e.g. fever/infection concern; 3.1%), allergy/anaphylaxis (1.5%), behavioral/psychiatric (1.5%), and metabolic derangements (e.g. hypoglycemia; 1%). Conclusion This quality improvement project informs sleep lab policies and procedures regarding staffing, training and management of urgent medical issues that arise during pediatric sleep studies. Abstract:Knight (1936) has stated that the nutritional requirements of the coliform bacteria are apparently quite simple. As reported by Koser and Rettger (1919), all strains of Escherichia coli, Escherichia acidi-lactici, Aerobacter aerogenes, and Aerobacter cloacae tested were able to grow in continued subculture in a medium of salts with ammonia or a single amino acid as source of nitrogen, glucose or glycerol being supplied as carbon source. Amino acids which were suitable nitrogen sources for these organisms were glycine, lysine, histidine, aspartic acid, tryptophane and tyrosine. Braun and Cahn-Bronner (1921) grew E. coli aerobically with ammonium lactate as nitrogen-carbonenergy source. Koser (1918) reported that A. aerogenes was able to grow well in a synthetic medium containing uric acid as the sole source of nitrogen, whereas E. coli was unable to do so. He suggested that the differentiation was due to the inability of Escherichia strains to break down the purine ring and thus obtain nitrogen for growth from the uric acid. Several other investigators have studied utilization of purine derivatives by coliform bacteria. Mitchell (1937), in a comprehensive study of 360 strains of Escherichia, Aerobacter and Citrobacter, pointed out some interesting relationships. She determined the ability of these bacteria to grow in synthetic media containing various purine derivatives as sole source of nitrogen. Abstract:Objectives The aim of this study was to describe the incidence of brief resolved unexplained events (BRUEs) and compare the impact of a national clinical practice guideline (CPG) on admission and diagnostic testing practices between general and pediatric emergency departments (EDs). Methods Using the Nationwide Emergency Department Sample for 2012–2019, we conducted a cross‐sectional study of children <1 year of age with an International Classification of Diseases diagnostic code for BRUE. Population incidence rate was estimated using Centers for Disease Control and Prevention birth data. ED incidence rate was estimated for all ED encounters. We used interrupted time series to evaluate the associated impact of the CPG publication on the outcomes of ED disposition (discharge, admission, and transfer) and electrocardiogram (ECG) use. Results Of 133,972 encounters for BRUE, 80.0% occurred in general EDs. BRUE population incidence was 4.28 per 1000 live births and the annual incidence remained stable (p = 0.19). BRUE ED incidence was 5.06 per 1000 infant ED encounters (p = 0.14). The impact of the BRUE CPG on admission rates was limited to pediatric EDs (level shift −23.3%, p = 0.002). Transfers from general EDs did not change with the CPG (level shift 2.2%, p = 0.17). After the CPG was published, ECGs increased by 13.7% in pediatric EDs (p = 0.005) but did not change in general EDs (level shift −0.2%, p = 0.82). Conclusions BRUEs remain a common pediatric problem at a population level and in EDs. Although a disproportionate number of infants present to general EDs, there is differential uptake of the CPG recommendations between pediatric and general EDs. These findings may support quality improvement opportunities aimed at improving care for these infants and decreasing unnecessary hospital admissions or transfers.

Abstract:Background Almost one in five newly born babies in Nairobi County, Kenya, may need some form of inpatient neonatal care. Prior work showed many facilities are inadequately prepared to provide sick newborn care, however, only 16.6% of babies from facilities were referred to a better-equipped facility. We sought to examine referrals to and from three busy first-level referral public hospitals in Nairobi and what infrastructure and systems are available to support neonatal transport from these first-referral level hospitals to the main tertiary care center. Methods Patient-level data of newborns over 13 months were retrospectively abstracted from routinely collected patient data and examined to characterize those referred into and out of three newborn units in the study hospitals. Structural assessments using a checklist completed during hospital visits were used to describe hospitals’ readiness to support newborn referral and transport. Results Five percent (398/7720) of the cohort studied were referrals either out or to the hospitals. 68% (272/398) were referred to these hospitals. A majority (397/398) and two-thirds (268/398) of patients had their sex and gestation age documented respectively. Among both referrals in and out, 63% (251/397) were male and 44% (118/268) were preterm infants. Among those referred in, 26% (69/272) died and 2.6% (7/272) were further referred to a tertiary-care newborn unit in Nairobi County. Equipment, drugs, and medical supplies needed to provide safe in-transit care in the ambulance, including oxygen and infant warmers were unavailable. Conclusion Neonatal transport from first-referral hospitals to the tertiary center in Nairobi County is poor and likely to be unsafe for newborn transfer services. This jeopardizes their well-being and survival. There is a need to develop, equip and maintain a high-quality referral and newborn transport system which can support the continuum of newborn care across referral care pathways into and from first-referral level hospitals.

Abstract:Objectives: To describe the quality of pediatric resuscitative care in general emergency departments (GEDs) and to determine hospital-level factors associated with higher quality. Methods: Prospective observational study of resuscitative care provided to 3 in situ simulated patients (infant seizure, infant sepsis, and child cardiac arrest) by interprofessional GED teams. A composite quality score (CQS) was measured and the association of this score with modifiable and nonmodifiable hospital-level factors was explored. Results: A median CQS of 62.8 of 100 (interquartile range 50.5-71.1) was noted for 287 resuscitation teams from 175 emergency departments. In the unadjusted analyses, a higher score was associated with the modifiable factor of an affiliation with a pediatric academic medical center (PAMC) and the nonmodifiable factors of higher pediatric volume and location in the Northeast and Midwest. In the adjusted analyses, a higher CQS was associated with modifiable factors of an affiliation with a PAMC and the designation of both a nurse and physician pediatric emergency care coordinator, and nonmodifiable factors of higher pediatric volume and location in the Northeast and Midwest. A weak correlation was noted between quality and pediatric readiness scores. Conclusions: A low quality of pediatric resuscitative care, measured using simulation, was noted across a cohort of GEDs. Hospital factors associated with higher quality included: an affiliation with a PAMC, designation of a pediatric emergency care coordinator, higher pediatric volume, and geographic location. A weak correlation was noted between quality and pediatric readiness scores.
Bedside critical care training: A quasi-experimental study in the paediatric emergency department of a referral hospital in Nigeria

Abstract:Background: There is a need for critical care services outside intensive care units (ICUs), especially in emergency departments (EDs). However, there is a paucity of skilled manpower for ED critical care or emergency critical care (ECC) in resource-limited settings. Objectives: To evaluate the impact of bedside training on emergency critical care practices of clinical staff. Methods: This was a quasi-experimental study using a pretest - post-test design in a paediatric ED. The intervention was a six-week structured bedside training on bubble continuous positive airway pressure (CPAP) high-flow nasal cannula (HFNC) and mechanical ventilation. Participants' actual ECC practices pre and post training were evaluated, including their perceived proficiency on an uncalibrated 100 mm visual analogue scale (VAS). Descriptive and inferential analyses were done; p<0.05 was considered significant. Results: A total of 35 clinical staff participated in the training, comprising 9 (24.3%) paediatric registrars, 12 (32.5%) senior registrars, 10 (27.0%) nurses, 4 (10.8%) house officers and 2 (5.4%) paediatric consultants. The male:female ratio of the participants was 1:1.6, and their mean (SD) age was 33.24 (6.30 years. Participants' understanding of testing the CPAP circuit, connecting the patient and weaning significantly improved following training (p=0.004). Their capacity to select appropriate HFNC parameters improved (p=0.013). They performed more endotracheal intubations in the post-training period (p=0.001). Their pretest-post-test proficiency in mechanical ventilation increased on VAS (mean scores 45.26+31.99 v. 63.26+22.26; p=0.038). Also, there was a significant increase in their perceived proficiency in paediatric analgesia/sedation (30.83+29.86 v. 49.83+23.90; p=0.029). Conclusion: Short-term bedside critical care training enhanced the self-reported competency of paediatric ED staff. There is a need for on-the-job ECC training and retraining of clinical staff in our setting. Contribution of the study: Paediatric emergency department clinical staff desire hands-on critical care training in developing settings. Bedside training improves the knowledge and skills of nursing staff in non-invasive ventilation. Bedside training also builds the capacity of medical staff in invasive ventilation in the emergency department. Overall, short-term bedside training enhances the clinical knowledge and self-reported critical care practice of clinical staff in Children's Emergency Room.

Abstract:Background: The establishment of a resuscitation room management for nontraumatic critically ill children appears to make sense. This study collected data of pediatric patients suffering from nontraumatic critically ill conditions treated in a resuscitation room. Methods: The retrospective OBSERvE-DUS-PED study (November 2019-October 2022) recorded pediatric patients (age < 18 years) who were admitted to the emergency department (ED) for resuscitation room care. The routinely documented data on treatment were taken from the hospital information system MEDICO® and the patient data management system COPRA® in accordance with the OBSERvE dataset. The study was approved by the Ethics Committee of the Medical Faculty of the Heinrich Heine University (2023-2377). Results: The study included 52 pediatric resuscitation room patients. Adolescents aged 14-17 years were the most frequent in the cohort representing 37% of the total and neonates/infants (0-1 year) were lowest at 8%. The most common symptoms categorized according to ABCDE problems were disturbance of consciousness (D) at 61%, cardiovascular failure (C) at 25%, respiratory insufficiency (B) at 6%, airway obstruction (A) and exposure/environment (E) problems each at 4%. The out-of-hospital and in-hospital emergency procedures were performed with the following frequencies: venous (58% vs. 65%), intraosseous (14% vs. 2%) and central venous access (0% vs. 12%), invasive airway management (35% vs. 8%), cardiopulmonary resuscitation (21% vs. 10%), vasopressors (15% vs. 17%), and intra-arterial pressure measurement (0% vs. 17%). The mean duration of resuscitation room management was 70 ± 43 min. The 30-day mortality was 17%. Conclusion: The OBSERvE-DUS-PED study demonstrates the major challenges in the care of critically ill nontraumatic pediatric patients, both in out-of-hospital and in-hospital management. The variety and complexity of the referral diagnoses as well as the immediate vital threat to the patients make it appear sensible to treat such patients primarily in a resuscitation room of the ED due to the available material, infrastructural and personnel resources.

Abstract:Most renal masses exhibit an expansile growth pattern characterized by radial tumor enlargement that displaces normal renal parenchyma and forms spherical, often exophytic, lesions. These expansile masses have pushing margins that impress adjacent normal renal parenchyma but do not infiltrate it; this behavior results in a well-defined, encapsulated appearance at both radiologic and gross pathologic examination. In contrast, certain disease processes involve the kidney in an infiltrative fashion by using the normal renal architecture as scaffolding for interstitial growth. These infiltrative renal lesions lack a sharp border of demarcation with the normal parenchyma and therefore demonstrate ill-defined zones of transition between the lesion and normal parenchyma. Although infiltrative lesions frequently enlarge the kidney, its reniform shape is usually maintained. Cross-sectional imaging can often help distinguish infiltrative from expansile growth patterns through analysis of the parenchymal interface between the process and the kidney, the effect of the lesion on the collecting system and renal sinus, and the overall renal morphology. A wide variety of neoplastic and inflammatory conditions characteristically involve the kidney by cellular infiltration. Although considerable overlap of the imaging features exists among the various infiltrative processes, the correct diagnosis may be suspected when the clinical data and associated radiologic findings are considered together.
