Abstract:Although indirect bonding has been advocated for many years as a way to improve speed and outcome of orthodontic treatment, the technique is still only used by a minority of orthodontists. One of the reasons may be that the claims have not been supported by convincing research. However the most common argument is that the laboratory and chairside procedures are overly complex. Digital technologies such as intra-oral scanning and 3D printing are now so far advanced that it may be worthwhile for clinicians to explore the opportunities these developments could bring to indirect bonding. Three different options for the production of indirect bonding trays are presented with the overall emphasis on the orthodontist maintaining complete control of the design and production stages.

Comprehensive clinical evaluation of indirect and direct bonding techniques in orthodontic treatment: a single-centre, open-label, quasi-randomized controlled clinical trial

Abstract:BackgroundFew prospective investigations have compared direct and indirect techniques through comprehensive and detailed clinical evaluations, considering the impact of all factors.ObjectivesThis study aimed to compare and evaluate direct and indirect bonding methods at a single institution and to clarify the selection criteria for the bonding method.Materials and methodsThis single-centre, quasi-randomized controlled clinical trial included 153 patients who required fixed orthodontic treatment. They were randomly divided into indirect and direct binding groups by the project lead (K.K.), who was blinded to all clinical data, and performed the allocation using medical record numbers. The chair time for bracket bonding, discomfort during bracket bonding, oral hygiene after bonding, number of bracket failures, number of intentional bracket reattachments, post-treatment occlusal index, and total treatment time were assessed. Outcomes were compared using a two-sample t -test or Mann–Whitney U test ( P < .05).ResultsFifty-eight patients were included in the indirect bonding group (20 male, 38 female; mean age: 20.63 ± 5.69 years) and 66 (14 male, 52 female; mean age: 23.17 ± 8.83 years) in the direct bonding group. Compared to the direct bonding group, the indirect bonding group had shorter chair time ( P < .001), a shorter total treatment period ( P < .01), and a better final occlusal relationship ( P < .001). The number of bracket detachments was higher ( P < .001) in the indirect bonding group, but the number of intentional reattachments was lower ( P < .001).ConclusionIndirect bonding may improve the efficiency of orthodontic treatment.HarmsNo harm was observed during the study.Trial registration numberThis trial was approved by the Ethics Review Committee of Okayama University (approval number: d10001), UMIN registration number 000022182.

Abstract:Background: To compare the shear bond strength of brackets cemented to dental enamel according to the cementation techniques. Material and methods: Experimental study. We used 90 premolars and placed them in printed polylactic acid (PLA) filament models to simulate the dental arch shape and to then cement brackets using the direct, indirect technique with Transbond™XT and indirect technique with Orthocem®. Then, we carried out a shear bond strength test using a universal testing machine, and we evaluated the enamel surface using the adhesive resin remaining index. Dunn's test was used for the inferential statistical analysis of shear bond strength, and Fisher's exact test was used for the adhesive resin remaining index. Results: The shear bond strength of the brackets recorded mean values of 16.74±4.48Mpa, 15.93±6.49Mpa and 12.09±5.07Mpa in the direct, indirect technique with Transbond™XT and indirect technique with Orthocem® respectively. At an inferential level, a lower statistically significant difference was found in the indirect group with OrthoCem® in contrast to the other two groups. In the evaluation of resin remaining after detachment, the direct technique group registered 46.7% of teeth with more than half of resin remaining and the indirect technique groups with Transbond™XT and Orthocem® registered less than half of resin remaining with an incidence of 53.3% and 43.3% respectively. At an inferential level, a statistically significant difference between groups was evidenced. Conclusions: The indirect cementation technique using Transbond™ XT is more recommended since it presents a higher shear bond strength than using Orthocem®. A comparison of bond strength between direct- and indirect-bonding methods

Abstract:The purpose of this study was to evaluate and compare the shear bond strength and the sites of bond failure for brackets bonded to teeth, using two indirect-bonding material protocols and a direct-bonding technique. Sixty extracted human premolars were collected and randomly divided into three groups. The direct-bonded group (group 1) used a light-cured adhesive and primer (Transbond XT). One indirect-bonded group (group 2) consisted of a chemical-cured primer (Sondhi Rapid Set) and light-cured adhesive (Transbond XT), whereas the other group (group 3) used a light-cured primer (Orthosolo) and adhesive (Enlight LV). Forty hours after bonding, the samples were debonded. Mean shear bond strengths were 16.27, 13.83, and 14.76 MPa for groups 1, 2, and 3, respectively. A one-way analysis of variance showed no significant difference in mean bond strength between groups (P = .21). Furthermore, a Weibull analysis showed all three groups tested provided over a 90% survival rate at normal masticatory and orthodontic force levels. For each tooth, an Adhesive Remnant Index (ARI) score was determined. Group 2 was found to have a significantly lower ARI score (P < .05) compared with groups 1 and 3. In addition, Pearson correlation coefficients indicated no strong correlation between bond strength and ARI score within or across all groups.

Abstract:This clinical paper provides an in-depth exploration of advanced techniques for bonding orthodontic attachments under special circumstances. Challenges arise when bonding brackets to non-enamel surfaces, such as dental restorations, and in conditions such as amelogenesis imperfecta, which affect enamel integrity. Distinct approaches required for bonding to different restorative materials, including glassy ceramics, zirconia, resin composites and metals, are outlined. Moreover, we describe strategies to manage bonding in conditions including amelogenesis imperfecta, hypodontia and microdontia in a multidisciplinary context.
Bond strength with custom base indirect bonding techniques

Abstract:Different types of adhesives for indirect bonding techniques have been introduced recently. But there is limited information regarding bond strength with these new materials. In this in vitro investigation, stainless steel brackets were bonded to 100 permanent bovine incisors using the Thomas technique, the modified Thomas technique, and light-cured direct bonding for a control group. The following five groups of 20 teeth each were formed: (1) modified Thomas technique with thermally cured base composite (Therma Cure) and chemically cured sealant (Maximum Cure), (2) Thomas technique with thermally cured base composite (Therma Cure) and chemically cured sealant (Custom I Q), (3) Thomas technique with light-cured base composite (Transbond XT) and chemically cured sealant (Sondhi Rapid Set), (4) modified Thomas technique with chemically cured base adhesive (Phase II) and chemically cured sealant (Maximum Cure), and (5) control group directly bonded with light-cured adhesive (Transbond XT). Mean bond strengths in groups 3, 4, and 5 were 14.99 +/- 2.85, 15.41 +/- 3.21, and 13.88 +/- 2.33 MPa, respectively, and these groups were not significantly different from each other. Groups 1 (mean bond strength 7.28 +/- 4.88 MPa) and 2 (mean bond strength 7.07 +/- 4.11 MPa) showed significantly lower bond strengths than groups 3, 4, and 5 and a higher probability of bond failure. Both the original (group 2) and the modified (group 1) Thomas technique were able to achieve bond strengths comparable to the light-cured direct bonded control group.
A comparative assessment of transfer accuracy of two indirect bonding techniques in patients undergoing fixed mechanotherapy: A randomised clinical trial

Abstract:Objectives: To assess the transfer accuracy of three-dimensional (3D) printed transfer trays and compare them with transfer trays made up of polyvinyl siloxane (PVS) for use in indirect bonding. Design: This was a two-arm parallel prospective randomised controlled trial. Setting: The trial was undertaken at the outpatient department of a dental college. Participants: A total of 30 patients (18 men, 12 women) were randomly allocated to two groups. Methods: The inclusion criteria included patients with permanent and fully erupted dentition (age range = 17-24 years), Angles class I malocclusion with crowding <3 mm requiring non-extraction treatment, good oral hygiene and no previous history of orthodontic treatment. Blinding was applicable only for outcome assessment. Indirect bonding was performed by the primary investigator for both the groups. Digital images of the pre-transfer and post-transfer brackets were obtained by means of an intra-oral scanner and compared using software. Superimpositions of pre- and post-transfer images were done to determine the transfer error for linear and angular variables for all tooth types. Results: A total of 600 teeth were bonded, 300 each for both groups. Statistically significant differences were observed in all dimension between the two groups, with 3D-printed trays being more accurate than PVS trays except in the vertical dimension (P < 0.05). The prevalence of clinically unacceptable transfer errors revealed that most of the transfer errors were in the vertical dimensions for 3D-printed trays. Conclusion: 3D-printed trays are more accurate than PVS trays except for transfers in vertical dimension.

Material and Methods: Fourteen patients, planned for fixed orthodontic therapy, were randomly divided into two equal groups. For both groups, both dental arches were scanned, to acquire virtual models, brackets were virtually positioned from central incisors to second premolars, and scans for the final bracket positions were performed. In the first group, transfer trays were 3D-printed. In the second group, virtual models were 3D-printed, and vacuum-formed soft sheets were thermoformed on the printed model. Teeth were indirectly bonded and then scanned. Superimposition of the virtual and the final bracket positioning scans was performed to measure linear and angular deviations in brackets positions. Results: The first group showed significantly less occlusogingival and buccolingual linear errors than the second group. No significant differences in angular deviations were found between both groups. The frequencies of clinically acceptable linear errors within 0.5 mm and angular errors within 2° showed no statistically significant difference between both groups (p> 0.05 for all measurements). The transfer errors in both groups showed linear directional biases toward the mesial, gingival and labial directions. There was no statistically significant difference in the rate of immediate debonding between both groups (10.7% and 7.1% for the first and the second groups, respectively, p=0.295). Conclusions: 3D-printed indirect bonding trays were more accurate than vacuum-formed trays, in terms of linear deviations. Both types of trays showed similar angular control.

Abstract:In invisible orthodontics, the role of composite attachments in facilitating complex tooth movements is crucial. This study, which evaluates the efficacy of a novel clinical attachment procedure, holds significant implications for the field. The technique used two templates (one pre-drilled and the other pre-loaded with high-viscosity composites) and was compared with the standard procedure. Fifty attachments were planned for four dental arch prototypes. Dental impressions were taken using digital scans for virtual planning and after tested techniques. The stereolithographic files (STL) obtained were aligned with those of the virtual planning, and a colorimetric map was used to evaluate the composite resin’s maximum excess and defect deviation. The enamel–resin interfaces were observed by scanning electron microscopy (SEM). The Fisher test for the distribution of detachments and morphological defects and the Mann–Whitney test for the maximum values of excess and defect were used. No significant results were found between groups for morphological defects and detachments, and the maximum values of defect and excess were reported. SEM images for the experimental technique showed integrated adhesion. This innovative procedure, which has proven reliable and operationally straightforward, holds promise, instilling confidence in its practicality and potential to advance the field of orthodontics.

Abstract:The adhesive material used to bond orthodontic brackets to teeth should neither fail during the treatment period, resulting in treatment delays, untoward expenses or patient inconvenience nor should it damage the enamel on debonding at the end of the treatment. Although the effectiveness of a bonding system and any unfavorable effects on the enamel may be studied by conducting in-vivo studies, it is nearly impossible to independently analyze different variables that influence a specific bonding system in the oral environment. In-vitro studies, on the other hand, may utilize more standardized protocols for testing different bonding systems and materials available. Thus, the present review focused attention on in-vitro studies and made an attempt to discuss material-related, teeth-related (fluorotic vs non-fluorotic teeth) and other miscellaneous factors that influences the shear bond strength of orthodontic brackets. Within the limitations of this review, using conventional acid-etch technique, ceramic brackets and bonding to non-fluorotic teeth was reported to have a positive influence on the shear bond strength of orthodontic brackets, but higher shear bond strength found on using ceramic brackets can be dangerous for the enamel.
Experimental Bracket Design Performance on Bonding and Polymerization of Orthodontic Composite

Abstract:Objective. To evaluate the enamel bonding ability and orthodontic adhesive resin degree of conversion using the experimental bracket design. Material and Methods. Thirteen bovine teeth were used in the study. The experimental bracket was modified with a translucent region in the center of its body. After enamel etching, Orthocem orthodontic adhesive (FGM, Joinville, Brazil) was applied on the bracket base for bonding. The groups were divided as follows (n = 10 per group): (1) control (CB) with standard brackets and (2) spot bracket (SB) with experimental brackets featuring a 0.8 mm translucent region at the center using carbide bur. Shear bond strength (SBS) was evaluated after 24 hours in a universal testing machine and adhesive remnant index (ARI). The degree of conversion (DC) was analyzed using Raman spectroscopy (n = 3 per group). Data were then analyzed using Student's t‐test and Mann–Whitney statistical methods. Results. The SB group exhibited a higher mean SBS (10.33 MPa) compared to the CB Group (8.77 MPa). However, there was no statistical difference between the groups (p = 0.376). Both SB and CB groups had a mean ARI score of 1. Raman analysis revealed a higher degree of conversion in the SB group (49.3%) compared to the CB group (25.9%). Conclusions. The experimental support showed a higher degree of adhesive conversion, although there was no significant increase in bond strength.

Abstract:Introduction: In orthodontics, shear bond strength plays an important role because it provides a good bond between the brackets and tooth surface; it avoids fracture of the tooth surface and prevents debonding of brackets from the tooth surface. All of these allow sufficient treatment time. Many factors, including the adhesive, its thickness, its strength, the bonding procedure, the clinician's ability, the base design, the geometry of the bracket, the material, and the kind of bracket all contribute to the shear bond strength. Brackets joined using conventional adhesive and adhesive pre-coated (APC) flash-free glue were the subjects of this comparison and evaluation research, which aimed to measure shear bond strength, enamel microfracture, and adhesive residual index. Method: 60 recently removed premolars from humans were used in this investigation. Before mounting on the acrylic block, the teeth were meticulously cleaned and preserved in artificial saliva. Two groups were formed from the collected premolars the control group and the experimental group. For the control group, we used American Orthodontics (AO) Master/Mini Master series brackets glued with resin composite kits. To make sure the adhesive was uniformly thick, we flashed extra adhesive around the brackets. In the meantime, samples were bonded using 3M Unitek APC flash-free technology in the experimental group. Results: The research indicated that there was a statistically significant difference between the two groups to the adhesive remnant index (ARI) and mean shear bond strength. The shear bond strength of the experimental group averaged 10.96 megapascals (MPa), whereas the control group's was 5.70 MPa. The control group's ARI score was 2.97, whereas the experimental group's score was 2.4. There was no statistically significant change seen in enamel microfracture. Conclusion: A more robust shear bond may be possible using APC flash-free brackets. Compared to conventional bonding techniques and brackets, APC flash-free brackets have a lower adhesive residual index. The APC flash-free bracket technology also causes more enamel microfracture than conventional bonding and bracketing methods.

Abstract:For the widely studied high-carrier-mobility species, pentacene, it is found that perfluorination turned it into an n-type organic field-effect transistor (OFET), but doubled the internal reorganization energy (lambda) of the electron/hole hopping. Here, Cl- and N-functionalized pentacenes are designed to test two strategies that may reduce the impact of functionalization on lambda. Calculation results show that the strategies are feasible. Moreover, combined with crystal-structure data of Cl- and N-functionalized aromatic compounds, it is concluded that compared to fluorination, the presence of Cl and N atoms in pentacene not only reduces the influence of functionalization on lambda and affords a larger window for tuning HOMO (highest occupied molecular orbital) and LUMO (lowest unoccupied molecular orbital) energies, but also provides the opportunity to promote pi-stacked structures through ClCl and C--HN interactions.
Development of New Method to Reposition Orthodontic Brackets Without Debonding by Using an Ultrasonic Device: An In Vitro Study

Abstract:Objective: The purpose of this study was to develop a new method to change a bracket position without debonding the metal bracket that was bonded with a polymethyl methacrylate (PMMA) orthodontic adhesive resin by using an ultrasonic device. Materials and Methods: An originally designed scaler tip was developed for a piezoelectric ultrasonic scaler. The adhesive turned to melting state by thermal heat that was generated by ultrasonic frictional heat. When the adhesive was in melting state, the bracket was rotated approximately 20° and held in this position until the adhesive hardened again by air cooling. Shear bond strengths were measured on each sample in the control and the repositioning groups. To evaluate the thermal effects of the increased temperature on the enamel surface and pulpal cavity, extracted human lower premolars were monitored during the repositioning time. Results: The time required for the repositioning procedures was approximately 7 s. The temperature of the enamel surface increased by 53°C and the pulpal wall increased by 0.5°C during the repositioning procedures. The bonding strength showed no significant difference between the control and repositioning groups. Conclusion: This method for bracket repositioning without debonding using an ultrasonic device proved to be a new method for bracket repositioning.
Performance of indirect technique in peri-implant soft tissue contour duplication in the anterior maxilla: A clinical pilot study

Abstract:Objective: To evaluate the performance of the indirect technique in peri-implant soft tissue contour duplication after the delivery procedure in the anterior maxilla. Materials and methods: Patients with single implant-supported fixed restorations in the anterior maxilla were recruited. For the impression procedure, an intraoral scan was acquired by both the direct and the indirect techniques. For the delivery procedure, implants were randomly allocated into one of the two groups according to the approaches of digital impression preceding definite crown fabrication (A-direct technique; B-indirect technique) and were scanned again after the definite crown delivery. The stereolithography files were superimposed to analyze changes in peri-implant soft tissue contour after the delivery procedure. The main outcomes were dimensional deviations of peri-implant mucosa, and the secondary outcome was differences in the pink esthetic score (PES). Results: A total of 20 implants that underwent the complete workflow were included. After the delivery procedure, significant deviations in palatal tissue thickness between the provisional and definite crowns were observed in Group A but these were absent in Group B. Additionally, deviations in labial thickness (0.27 ± 0.12 mm vs. 0.08 ± 0.09 mm) and palatal thickness (0.17 ± 0.15 mm vs. 0.03 ± 0.08 mm), and labial volume of soft tissue (1.87 ± 0.94 mm3 vs. 0.75 ± 0.74 mm3 ) in Group A were significantly higher than those in Group B. No significant differences in PES were found. Conclusion: The indirect technique of scanning the provisional crown can more accurately duplicate the peri-implant soft tissue contour than the direct technique, resulting in a smaller deviation of the soft tissue in the delivery procedure.

Abstract:Aim: To evaluate in vitro and in vivo the accuracy of 3D-printed customized transfer devices during indirect bonding technique (IBT). Methods: A search for articles published in the English language until April 2022 was carried out using PubMed, Web of Science, Scopus, and Google Scholar databases and by applying a specific search strategy for each database to identify all potentially relevant in vivo or in vitro studies. After the removal of duplicate articles and data extraction according to the participants-intervention-comparison-outcome-study design schema scheme, the methodological quality of the included studies was assessed using the Swedish Council on Technology Assessment in Health Care Criteria for Grading Assessed Studies. Results: The initial search identified 126 articles, 43 of which were selected by title and abstract. After full-text reading, 15 papers were selected for the qualitative analysis and seven studies for the quantitative analysis. The evidence quality for the selected studies was moderate. Conclusions: Except for the bucco-lingual direction, the 3D-printed customized devices have a transfer accuracy within the clinically acceptable limits established by the American Board of Orthodontics. Therefore, 3D-printed transfer devices may be considered an accurate method for bonding position during IBT, both in vitro and in vivo. Additional randomized clinical studies in vivo should be suggested.
