Abstract:OBJECTIVE To investigate the clinical epidemiological characteristics of neonatal respiratory failure in 1,108 neonates, and to provide a reference for improvement in clinical treatment and multicenter clinical studies. METHODS The clinical data of 1,108 neonates with respiratory failure were collected with questionnaires, and statistical analysis was performed for the epidemiological indices including primary diseases, clinical therapeutic methods, treatment outcome, and fatality. RESULTS In all the neonates with respiratory failure, the median gestational age was 37 weeks+1 day, the median birth weight was 2,600 g, the median age in days on admission to neonatal intensive care unit was 0.71 days (17 hours), and the boy/girl ratio was 3.1:1. The major primary diseases were respiratory distress syndrome (30.51%), pulmonary infection/sepsis (23.55%), and wet lung (13.18%). Of all the neonates, 48.64% received nasal continuous positive airway pressure (nCPAP), 12.81% received high-frequency oscillatory ventilation, 13.45% received pulmonary surfactant, and 8.66% received nitric oxide inhalation therapy. The fatality was 24.19%. CONCLUSIONS The major primary disease for neonatal respiratory failure is respiratory distress syndrome. Pulmonary surfactant, nCPAP, high-frequency oscillatory ventilation, and nitric oxide inhalation therapy are major therapeutic methods for neonatal respiratory failure, but neonatal respiratory failure still has a high fatality.
Inhaled Nitric Oxide in Preterm and Term Neonates with Hypoxemic Respiratory Failure and Persistent Pulmonary Hypertension

Abstract:AIM: To assess the effects of inhaled nitric oxide (iNO) as a rescue therapy for term and preterm neonates with hypoxemic respiratory failure (HRF). METHODS: Data were collected from retrospective studies at neonatal service of two university children's hospitals. Response to iNO at 5-20 ppm over time and outcome were assessed. RESULTS: Term, (n=40) and preterm (n=35) neonates did not differ in age at entry, initial or maintained iNO dose and duration, baseline blood pH and PaCO2. Oxygenation index (OI) at baseline was higher in the term, and significantly decreased after 30 min, 3 and 24 h after iNO. This effect was seen only at 24 h in the preterm. The decline in time weighted OI (TWOI) at 24 h of the treatment was greater in the term than in the preterm (P<0.01). In the subgroup neonates with baseline pH greater than or equal to 7.25, significant decrease in OI was found at 30 min, 3 and 24 h of iNO, but only at 24 It for the subgroup with pH<7.25 (P<0.05). Intracranial or pulmonary hemorrhage was found mainly in the preterm neonates. The survival was 65 % in all the patients. CONCLUSION: iNO improved oxygenation in both term and preterm neonates with HRF, and its response was in favor of term infants or those with high blood pH values at study entry.
Epidemiology of Respiratory Distress and the Illness Severity in Late Preterm or Term Infants: A Prospective Multi-Center Study

Abstract:Background The severity of respiratory distress was associated with neonatal prognosis. This study aimed to explore the clinical characteristics, therapeutic interventions and short-term outcomes of late preterm or term infants who required respiratory support, and compare the usage of different illness severity assessment tools.Methods Seven neonatal intensive care units in tertiary hospitals were recruited. From November 2008 to October 2009, neonates born at >= 34 weeks' gestational age, admitted at <72 hours of age, requiring continuous positive airway pressure (CPAP) or mechanical ventilation for respiratory support were enrolled. Clinical data including demographic variables, underlying disease, complications, therapeutic interventions and short-term outcomes were collected. All infants were divided into three groups by Acute care of at-risk newborns (ACoRN) Respiratory Score <5, 5-8, and >8.Results During the study period, 503 newborn late preterm or term infants required respiratory support. The mean gestational age was (36.8 +/- 2.2) weeks, mean birth weight was (2734.5 +/- 603.5) g. The majority of the neonates were male (69.4%), late preterm (63.3%), delivered by cesarean section (74.8%), admitted in the first day of life (89.3%) and outborn (born at other hospitals, 76.9%). Of the cesarean section, 51.1% were performed electively. Infants in the severe group were more mature, had the highest rate of elective cesarean section, Apgar score <7 at 5 minutes and resuscitated with intubation, the in-hospital mortality increased significantly. In total, 58.1% of the patients were supported with mechanical ventilation and 17.3% received high frequency oscillation. Adjunctive therapies were commonly needed. Higher rate of infants in severe group needed mechanical ventilation or high frequency oscillation, volume expansion, bicarbonate infusion or vasopressors therapy (P < 0.05). The incidence of complications was also increased significantly in severe group (P < 0.05). The in-hospital mortality in the severe group was significantly higher than other two groups (P < 0.05). ACoRN Respiratory Score was correlated with Score for Neonatal Acute Physiology-Version II (SNAP-II) (P < 0.01). High gestational age, high SNAP-II score and oxygenation index (01), and Apgar score at 5 minutes <5 were independent risks for death.Conclusions Neonatal respiratory distress is still a common cause of hospitalization in China. Illness severity assessment is important for the management. ACoRN Respiratory Score which correlated with SNAP-II score is easy to use and may be helpful in facilitating the caregivers in local hospital to identify the early signs and make the transfer decision promptly. Abstract:The assistance use of pulmonary mechanics measurement can guide the ventilator adjustment and decrease the ventilator-associated lung injury in neonatal RDS. The low PIP, low tidal volume, shorter inspiratory time and adequate PEEP strategies in the treatment of RDS showed no significant effects on the oxygenation in neonates. The mild permissive hypercarbia in newborn did not increase the incidence of IVH.

Abstract:Objective This study evaluated the association of parental interactions and therapies with cerebral oxygenation (rScO 2 ) and rScO 2 variability in infants using near-infrared spectroscopy. Study Design Prospective pilot study in clinically stable infants admitted to the neonatal intensive care unit (NICU). Infants were monitored continuously for 48 hours, and rScO 2 during parental interactions and therapies was compared with periods of no activity. rScO 2 variability was derived using the root mean of successive squared differences of averaged 1-minute rScO 2 values. Wilcoxon matched-pairs signed-rank test was used to compare baseline and activity periods. Results Data analysis included 23 infants. rScO 2 variability increased during periods of parental interaction ( p = 0.04) and during combined parental interaction and therapies ( p = 0.04). Conclusion We observed that routine NICU interventions are associated with increased rScO 2 variability in clinically stable NICU patients. rScO 2 variability may represent a useful biomarker for the early determination of the safety and efficacy of NICU interventions. Abstract:Introduction: Length of hospitalization varies widely in preterm infants and can be affected by multiple maternal and neonatal factors including respiratory instability. Therefore, we aimed to determine the association between postnatal intermittent hypoxemia (IH) and prolonged hospitalization. Methods: This prospective single-center cohort study followed infants born at <31 weeks of gestational age through 2 years corrected age with detailed oxygen saturation data captured from days 7 to 30 of age. Results: 51/164 (31%) of infants were discharged after 40 0/7 weeks of corrected gestational age (CGA). A greater average daily number of IH events (OR per 10 events/day 1.33 [95% CI 1.03–1.72]), duration of events (OR per minute 1.14 [1.07–1.21]), and percent time with oxygen saturation <80% (OR per percent 1.88 [1.25–2.85]) on days 7–30 of age were all significantly associated with prolonged hospitalization past 40 0/7 weeks CGA. In survival analyses, infants with a greater average daily number of IH events (HR per 10 events/day 0.89 [0.81–0.98]), percent time with oxygen saturation <80% (HR per percent 0.79 [0.67–0.94]), and duration of events (HR per minute 0.93 [0.91–0.95]) on days 7–30 of age all had significantly lower probability of earlier discharge. In addition, there was a significant interaction with gestational age; the association between IH and prolonged hospitalization was stronger in more mature infants ( p = 0.024). Conclusions: Physiological instability on days 7–30 of age, as manifested by IH, is significantly associated with prolonged hospitalization. IH likely represents both a marker of initial severity of illness and the beginning of biological cascades, leading to prematurity-associated morbidities.

Now days, Ad hoc networks facing some challenges such as; Privacy and Security, Multicast, Energy efficiency, QoS Support, Media access. This paper presents different routing protocols, comparison of different routing protocols, and challenges faced by Ad hoc wireless networks.
High-risk Factors of Respiratory Distress Syndrome in Term Neonates: A Retrospective Case-control Study

Abstract:BACKGROUND: Respiratory distress syndrome (RDS) is a common critical disease in term neonates, but reasons for the occurrence of RDS remains unclear.AIMS: This study aimed to investigate the cause of RDS in full-term neonates by a retrospective case-control study.STUDY DESIGN: Case-control study.METHODS: Among the patients admitted to Bayi Children's Hospital between January 2008 and December 2010, a total of 205 full-term neonates with RDS were assigned to the study group, and 410 full-term neonates without RDS were assigned to the control group. Clinical information, including the presence or absence of premature rupture of membranes (PROM), gender of the neonates, mode of delivery, birth weight, and any conditions suffered by the neonates were recorded.RESULTS: THE RESULTS OF LOGISTIC REGRESSION ANALYSIS SHOWED THAT THE FOLLOWING CAUSES WERE CLOSELY CORRELATED WITH TERM NEONATAL RDS: selective cesarean section (OR: 8.737; 95% CI: 5.232-14.588), severe birth asphyxia (OR: 6.988; 95% CI: 2.990-16.333), small gestational age (OR: 6.222; 95% CI: 2.001-8.993), maternal-fetal infection (OR: 5.337; 95% CI: 1.999-8.233), PROM (OR: 3.380; 95% CI: 1.986-5.754), male sex (OR: 2.641; 95% CI: 1.721-4.053), gestational glucose intolerance or diabetes (OR: 2.415; 95% CI:1.721-4.053), and low birth weight (OR: 2.323; 95% CI: 1.329-4.060).CONCLUSION: Several high-risk factors, such as selective cesarean section, severe birth asphyxia, maternal-fetal infection, PROM, and male sex are closely correlated with full-term neonatal RDS. These could provide a significant reference for the diagnosis and treatment of term neonatal RDS.

Abstract:Objective: The aim of this survey was to explore the relationship between admission volume and mortality of neonates with hypoxemic respiratory failure (NRF) in emerging neonatal intensive care units (NICUs). Methods: NRF from 55 NICUs were retrospectively included with death risk as the major outcome. Perinatal comorbidities, underlying disease severity, respiratory support, facility utilization, and economic burden in the early postnatal period were compared among five NICU admission volume categories defined by NRF incidence, with score for neonatal acute physiology perinatal extension II (SNAPPE-II) also assessed as initial severity. Results: Compared to NICUs with NRF < 50 cases/year, NRF incidence, NRF/NICU, NRF/NICU admissions, and magnitude of ventilator use were several times higher, and mortality rates 20-50% lower, in NICUs of 150-199 and >= 200 cases/year (p < .01), even after adjustment with SNAPPE-II in stratified ranges. Median SNAPPE-II values, which correlated with the death rate of NRF (r = .282, p < .001), were lower in NICUs of 150-199 and >= 200 than in <50, 50-99, and 100-149 categories (13 versus 18, p < .01). NRF mortalities were not correlated with the proportion of very low birth weight patients in each category. Conclusions: Neonates in NICUs with smaller NRF admission volume and decreased magnitude of ventilator use had a higher risk of death as assessed by SNAPPE-II, which should be targeted in the quality improvement of newly established, resource-limited NICUs.

Abstract:BACKGROUND: Persistent pulmonary hypertension of the newborn causes systemic arterial hypoxemia because of increased pulmonary vascular resistance and right-to-left shunting of deoxygenated blood. Inhaled nitric oxide decreases pulmonary vascular resistance in newborns. We studied whether inhaled nitric oxide decreases severe hypoxemia in infants with persistent pulmonary hypertension.METHODS: In a prospective, multicenter study, 58 full-term infants with severe hypoxemia and persistent pulmonary hypertension were randomly assigned to breathe either a control gas (nitrogen) or nitric oxide (80 parts per million), mixed with oxygen from a ventilator. If oxygenation increased after 20 minutes and systemic blood pressure did not decrease, the treatment was considered successful and was continued at lower concentrations. Otherwise, it was discontinued and alternative therapies, including extracorporeal membrane oxygenation, were used.RESULTS: Inhaled nitric oxide successfully doubled systemic oxygenation in 16 of 30 infants (53 percent), whereas conventional therapy without inhaled nitric oxide increased oxygenation in only 2 of 28 infants (7 percent). Long-term therapy with inhaled nitric oxide sustained systemic oxygenation in 75 percent of the infants who had initial improvement. Extracorporeal membrane oxygenation was required in 71 percent of the control group and 40 percent of the nitric oxide group (P=0.02). The number of deaths was similar in the two groups. Inhaled nitric oxide did not cause systemic hypotension or increase methemoglobin levels.CONCLUSIONS: Inhaled nitric oxide improves systemic oxygenation in infants with persistent pulmonary hypertension and may reduce the need for more invasive treatments.

Abstract:Neonatal respiratory distress syndrome (NRDS) is one of the leading causes of neonatal mortality in low-income countries. It is caused by a lack of surface-active substances in the lungs, and the maternal inflammatory response plays an important role in the formation of surface-active substances in the fetal lungs. We aimed to investigate the correlation between maternal prenatal systemic inflammatory indices and respiratory distress syndrome in preterm neonates. This is a retrospective case-control study that collected data from all patients who delivered between 28 and 36 weeks of gestation at Longhua District People's Hospital in Shenzhen City and whose infants were admitted to the neonatal unit, newborns with respiratory distress syndrome were in the experimental group (NRDS group), and newborns without NRDS were in the control group (non-NRDS group). To minimize the effect of confounders on the results, propensity score matching was performed on baseline characteristics. Totally, 524 patients were included (93 in the NRDS group and 431 in the non-NRDS group), and 71 matched pairs (142 patients). The neutrophil-to-lymphocyte ratio (NLR), derived neutrophil-to-lymphocyte ratio (dNLR), systemic immune-inflammation index (SII), systemic inflammation response index (SIRI), aggregate index of systemic inflammation (AISI) and neutrophil lymphocyte to platelet ratio (NLPR) were higher in the NRDS group than in the non-NRDS group (p < 0.05). The ROC curves of NLR, dNLR, SII, SIRI, AISI and NLPR for the diagnosis of NRDS were plotted, and it was found that the combined diagnostic efficacy of these six systemic inflammatory markers was better (AUC: 0.643, P = 0.003). Patients were divided into two groups based on the cut-off values determined from the ROC curves, and analysis using binary regression models revealed that SII ≥ 1199.94 (OR, 2.554; 95% CI 1.245-5.239, P = 0.011) and NLPR ≥ 0.0239 (OR, 2.175; 95% CI 1.061-4.459, P = 0.034) were independent risk factors predicting NRDS. Maternal prenatal SII ≥ 1199.94 and NLPR ≥ 0.0239 are independent risk factors for NRDS, and clinicians may be used to prevent neonatal respiratory distress in advance to reduce the incidence of NRDS.

Abstract:BACKGROUND:Exhaled nitric oxide (eNO) is one of the airway condensate derived markers, reflecting mainly airway inflammation in asthma and other lung diseases. The changes of eNO levels as pathophysiology of neonatal hypoxemic respiratory failure (HRF) in early postnatal life have not been thoroughly studied. The present study was to establish a method for measuring eNO concentrations in neonates with or without HRF. METHODS:Twenty-two newborn infants with HRF and 26 non-NRF controls were included within the first 24 hours of postnatal life. Their eNO levels were detected with a rapid-response chemiluminescence analyzer daily during the first week of their postnatal life, and lung mechanics and gas exchange efficiency were monitored at the same time, such as pulse oxygen saturation (SpO2), inspired fraction of oxygen (FiO2) and other parameters. RESULTS:During the first two days of postnatal life, eNO values of HRF neonates were significantly higher than those of the control neonates (day 1, 7.9±3.2 vs. 5.8±1.8 parts per billion [ppb], P<0.05; day 2, 8.8±3.2 vs. 6.0±2.4 ppb, P<0.05), but there were no significant differences in the following days. With SpO2/FiO2 increasing, difference of eNO values between the HRF and non-HRF neonates became narrowed, but there was still a two-fold difference of eNO/[SpO2/(FiO2×100)] on days 5-7. CONCLUSION:We established a method for measuring eNO and found difference in neonates with or without HRF, which diminished with prolonged postnatal days, reflecting pathophysiological characteristics of HRF.
Current Status of Neonatal Acute Respiratory Disorders: a One-Year Prospective Survey from a Chinese Neonatal Network.

Abstract:BACKGROUND We conducted a prospective, multicenter investigation of incidence, management and outcome of neonatal acute respiratory disorders (NARD), and evaluated related perinatal risk factors and efficacy of respiratory therapies in neonatal intensive care units (NICUs) in a Chinese neonatal network. METHODS Data were prospectively collected in 2004 - 2005 from infants with NARD defined as presence of respiratory distress and oxygen requirement during the first 3 days of life. RESULTS A total of 2677 NARD was classified (20.5% of NICU admissions). There were 711 (5.44%) with respiratory distress syndrome (RDS), 589 (4.51%) pulmonary infection, 409 (3.13%) meconium aspiration syndrome, 658 (5.03%) aspiration of amniotic fluid and 239 (1.83%) transient tachypnoea. Meconium aspiration syndrome had the highest rate with fetal distress, transient tachypnoea from cesarean section, and RDS with maternal disorders. Assisted mechanical ventilation was applied in 53.4% of NARD, and in above five disorders with 84.7%, 52.3%, 39.8%, 24.5%, and 53.6%, respectively. Corresponding mortality in these disorders was 31.4%, 13.6%, 17.8%, 4.1% and 5.0%, respectively. Surfactant was provided to 33.9% of RDS. In all RDS infants, the survival rate was 78.8% if receiving surfactant, and 63.4% if not (P < 0.001). CONCLUSIONS This study provided NICU admission-based incidence and mortality of NARD, reflecting efficiency of advanced respiratory therapies, which should be a reference for current development of respiratory support in NICU at provincial and sub-provincial levels, justifying efforts in upgrading standard of care in emerging regions through a collaborative manner.

Abstract:Aim: Not all the neonates respond with improvement in oxygenation following inhaled nitric oxide treatment (iNO) treatment. The aim of this study was to assess the independent risk factors associated with non-response to iNO during the 2 weeks of postnatal treatment in neonates diagnosed with persistent pulmonary hypertension (PPHN). Materials and Methods: This retrospective cohort study included all newborns with PPHN who received iNO treatment for more than 24 h. Demographic, obstetric, perinatal data and clinical complications were extracted from the hospitalization records. Subjects were divided into two groups according to their response to iNO inspiration during the first 24 h of iNO treatment. No response was defined as an increase in SpO 2 < 5% or the inability to sustain saturation levels in the first 24 h of iNO treatment. For descriptive statistics, χ 2 and t -test analysis were used to compare categorical and continuous variables between the two groups. To evaluate independent risk factors of non-responsiveness to iNO treatment, binary logistic regression analysis were performed. Results: A total of 75 newborns were included in the study. Sixty-two cases were in the responders group, and 13 cases were in the non-responders group. Univariate analysis showed that asphyxia, neonatal respiratory distress syndrome (NRDS), pulmonary surfactant administration, meconium aspiration syndrome (MAS), the severity of pulmonary hypertension (PH), and high-frequency oscillatory ventilation (HFOV) therapy were the high-risk factors affecting the response to iNO treatment in the newborns with PPHN. The binary logistic regression analysis indicated that asphyxia and NRDS incidence were independent predictors of non-responsiveness to iNO treatment [asphyxia: OR 4.193, 95% CI 1.104–15.927, P = 0.035; NRDS: OR 0.154, 95% CI 0.036–0.647, P = 0.011]. The patients in the non-responders group had shorter iNO inspiration followed by MV duration, supplemental oxygen and hospital stay, and higher mortality. There were no significant differences in IVH, PVL, and BPD between two groups. Conclusion: In the newborns with PPHN, asphyxia and NRDS resulted as the independent risk factors of non-responsiveness to iNO therapy. Asphyxia in the newborns with PPHN is detrimental to the response to iNO treatment, while NRDS is beneficial.

Intrauterine Inflammation, Excessive Fetal Growth and Respiratory Morbidities in Moderate-To-Late Preterm Neonates

Abstract:Introduction: Respiratory morbidities in neonates are often progressive and life-threatening, and its early prediction is crucial. Intrauterine inflammation is one of the key control variables of respiratory morbidities in both very preterm and term neonates; however, little is known about its effects in the remaining group of moderate-to-late preterm neonates born between 32+0 and 36+6 weeks of gestation. This study aimed to confirm whether intrauterine inflammation is associated with respiratory morbidities in moderate-to-late preterm neonates. Methods: A single-center retrospective observational study was conducted in neonates born between 32+0 and 34+6 weeks of gestation between April 2013 and March 2018. The correlation between respiratory morbidities (defined as a requirement for invasive mechanical ventilation longer than the median duration of 3 days) and intrauterine inflammation was assessed using multivariable logistic regression analysis. Results: The study population comprised 242 neonates born at 33.7 ± 0.8 weeks of gestation and weighing 1,936 ± 381 g. The multivariable model to predict the outcome comprised respiratory distress syndrome (odds ratio [OR]: 9.1; 95% confidence interval [CI]: 3.7-22.5; p < 0.001), lower gestational age (per week; OR: 0.5; 95% CI: 0.3-0.8; p < 0.005), higher birth-weight z-score (OR: 1.6; 95% CI: 1.2-2.2; p < 0.005), lower cord blood pH (per 0.10; OR: 0.5; 95% CI: 0.3-0.7; p < 0.005), and chorioamnionitis (OR: 2.8; 95% CI: 1.1-7.2; p < 0.05). Conclusion: Together with the incidence of respiratory distress syndrome and gestational age, chorioamnionitis and high birth-weight z-scores were associated with an increased incidence of respiratory morbidities in moderate-to-late preterm neonates. The deleterious impact of intrauterine inflammation on the lungs may be common in neonates of virtually all gestational ages. Traditional admission policy of neonatal intensive care units based on a threshold birth-weight, may leave a group of neonates without close observation despite their increased risks for respiratory morbidities.
Meta-analysis of Inhaled Nitric Oxide for Hypoxaemic Respiratory Failure in Preterm Infants

Abstract:Objective Inhaled nitric oxide(iNO) has been used in preterm infants with hypoxic respiratory failure (HRF). However, there is controversy about its therapeutic effect. In this article, the studies of iNO for HRF were summarized and the effect of iNO for HRF was assessed in preterm infants using meta-analysis method. Methods Medical electronic databases and other sources were searched without language restriction. According to inclusion and exclusion cretria, articles were evaluated (Shekelle PG, et al). RCTs were assessed according to the Juni assessment. Software Rev Man 4.2 was used to assess control trials, and other studies were reviewed and descrbied. Results Nine RCTs with 6 blind trials and 3 non-blind trials were selected from 883 articles. Publication bias was not found through funnel polt analysis. There were 3 RCTs that compared the oxygen use and the duration of ventilation, the length of hospitalization, including 259 patients in iNO and 259 in the control group. Compared to the control group, iNO could reduce the oxygen use (WMD=-10.24,95%CI:-19.62,-0.86,P0.05); There was no difference in the duration of ventilation between iNO and the control group (WMD=-4.87,95%CI:-10.68,0.94,P0.05); Compared to the control group, iNO could reduce the length of hospitalization (WMD=-8.31,95%CI:-16.23,-0.38,P0.05). There were 6 RCTs that compared the death before discharge, including 1084 patients in iNO and 1096 in the control group. There was no difference in the deaths before discharge between iNO and the control group (OR=0.95,95%CI:0.77,1.17,P0.05). There were 5 RCTs that compared the incidence of BPD among survivors at 36 weeks, including 618 patients in iNO and 621 in the control group. Compared to the control group, iNO could reduce the incidence of BPD (OR=0.75,95%CI:0.58,0.95,P0.05). There was no difference in the incidence of intraventricular hemorrhage (all grades) between iNO and the control group (OR=0.79,95%CI:0.62,1.00,P=0.05). There were 4 RCTs that compared the incidence of intraventricular hemorrhage (grade 3 or 4), including 767 patients in iNO and 784 in the control group. Compared to the control group, iNO could reduce the incidence of intraventricular hemorrhage (grade 3 or 4) (OR=0.84,95%CI:0.61,1.16,P0.05). There were 5 RCTs that compared the intraventricular hemorrhage (grade 3 or 4) or periventicular leukomacia, including 1021 patients in iNO and 1012 in the control. There was no difference in the incidence of intraventricular hemorrhage (grade 3 or 4) or periventicular leukomacia between iNO and the control group (OR=0.95,95%CI:0.68,1.22,P0.05). There were 2 RCTs that compared the incidence of threshold ROP, including 608 patients in iNO and 605 in the control group. There was no difference in the incidence of threshold ROP between iNO and the control group (OR=0.98,95% CI:0.72,1.34,P0.05). There were 4 RCTs that compared the incidence of ROP, including 474 patients in iNO and 463 in the control group. There was no difference in the incidence of ROP between iNO and the control group (OR=1.08,95%CI:0.75,1.57,P0.05). There was 1 RCT that compared the incidence of ROP requiring surgery, including 294 patients in iNO and 288 in the control group. There was no differencein the incidence of ROP requiring surgery between iNO and the control group(OR=1.05,95%CI:0.72,1.53, P0.05). There were 3 RCTs that compared the incidence of cerebral palsy, including 168 patients in iNO and 184 in the control group. There was no difference in the incidence of cerebral palsy between iNO and the control group (OR=1.08,95%CI:0.75,1.57,P0.05). There were 2 RCTs that compared the incidence of Bayley MDI2SD, including 156 patients in iNO and 110 in the control group. There was no difference in the incidence of Bayley MDI2SD between iNO and the control group (OR=0.84,95%CI:0.50,1.42,P0.05). There were 2 RCTs that compared the incidence of Bayley PDI2SD, including 155 patients in iNO and 167 in the control group. There was no difference in the incidence of Bayley PDI2SD between iNO and the routine therapy group (OR=0.94,95%CI:0.57,1.58,P0.05). Conclusions The currently published evidence from randomised trials does not support the routine use of iNO in preterm infants with HRF. Further multicentre and large-scale RCTs are still needed to be performed to evaluate the potential benefit of iNO as routine therapy in preterm infants with HRF, and these trials need to be designed to evaluate not only the short term effects, but should be powered to evaluate neurondevelopmental outcome at a minimum of two years of age.
Nitric oxide in neonatal hypoxemic respiratory failure

Abstract:Nitric oxide (NO) is a cellular signaling molecule and a powerful vasodilator. NO modulates basal pulmonary vascular tone and it is important to reduce blood pressure and to treat hypoxemic respiratory failure, such as persistent pulmonary hypertension (PPHN) in newborns. PPHN is defined as a failure of normal pulmonary vascular adaptation at or soon after birth, resulting in a persisting high pulmonary vascular resistance. iNO therapy decreases the need of extracorporeal membrane oxygenation (ECMO) although it did not reduce mortality of these patients. Severe meconial aspiration syndrome is associated with PPHN, resulting in severe hypoxemia; iNO administration combined with HFV results in ameliorate oxygenation. The cause of hypoxemic respiratory failure in patients with congenital diaphragmatic hernia (CDH) is complex. CDH patients experienced oxygenation improvement after iNO therapy, but they can be often considered iNO poor responders. In some cases iNO therapy can reduce the need of ECMO in presurgical stabilization. The pathophysiology of respiratory failure and the potential risks differ substantially in preterm infants. Pulmonary hypertension can complicate respiratory failure in preterm babies. Current evidence does not support use of iNO in early routine, early rescue or layer rescue regimens in the care of preterm infants.
