Abstract:OBJECTIVE:To investigate the biomechanical effects of micro-implant anchorage technique with sliding mechanics on maxillary anterior teeth retraction under different implant insertion heights and different retraction hook heights. METHODS:The three dimensional finite element model of maxillary anterior teeth retraction force system was constructed with CT scanning and MIMICS software and the relationships between brackets, teeth, wire and micro-implant were simulating the clinical factions. Then the initial tooth displacement was calculated when the insertion heights were 4 mm and 8 mm and the retraction hook heights were 1, 4, 7, 10 mm respectively. RESULTS:With retraction hook height added, the anterior teeth movement changed from lingual crown tipping to labial crown tipping and the intrusion movement was more apparent when the micro-implant was inserted in a higher location. CONCLUSION:The ideal teeth movement control could be achieved by different insertion heights of micro-implant and different retraction hook heights in straight wire retraction force system.

Abstract:Objective To analyze the biomechanical distribution of bone around the dental implant with immediate load and delayed load.Methods Three-dimensional finite element models of whole mandible with dental implants for immediate load and delayed load were established by means of CT scanning and USIS(Universal Surgical Integration System)software developed by ourselves.The distribution of stress and strain on the bone around dental implants and displacement of implants were calculated and analyzed with the method of finite element.Results The values of Von Mises stress on the bone around dental implants with immediate load were a bit less than those with delayed load,and both stresses were concentrated on the cortical bone,which were on the neck of dental implants.And the values of Von Mises strain on the bone around dental implants with immediate load were more than those with delayed load apparently,which were concentrated on the cancellous bone,at the bottom of dental implants and in the area of screw.Displacement of dental implants with immediate load was a bit more than those with delayed load.When the implants were loaded with buccolingual force,Von Mises stress,strain and displacement of dental implants with immediate load and delayed load increased respectively.Conclusion Biomechanical characteristics of bone around the dental implant with immediate load were similar to those with delayed load.It is certain that the technique of dental implant with immediate load is available.
Finite element analysis of tooth movement under different retraction force and intrusive force in double-archwire lingual orthodontics system

Abstract:The objectives of this study were to evaluate the tooth movement tendency during space closure in maxillary anterior teeth by various combinations of retraction force and intrusive force in a double-archwire lingual orthodontic system. Mini-implant-double slot lingual orthodontics system models of the bilateral maxillary first premolar extraction cases were constructed. Three-dimensional finite element models of the maxilla were constructed with definite position mini-implants (8 mm) and power arms (6 mm). Different retraction forces(50gf、100gf、150gf)were applied with the help of a nickel-titanium closed coil spring on the plate side. Intrusive forces(0gf、50gf、100gf)were applied with the help of the mini-implant between the two central incisors, and the initial displacements of the maxillary anterior teeth were analyzed. Variable amounts of displacements like controlled tipping, uncontrolled tipping, lingual crown tipping, labial root tipping, extrusion and distal crown tipping were observed in all the models, and these tendencies increased as the magnitude of retraction force increased, and these tendencies decreased as the magnitude of intrusive force increased. When the intrusive force was greater than or equal to the retraction force, the maxillary central incisors showed the trend of lingual crown tipping and labial root tipping, resulting in uncontrolled tipping movement. In terms of horizontal changes, the increasing width of bilateral anterior teeth was observed, with canines showing the least increasing trend. Various combinations of retraction force and intrusive force in a double-archwire lingual orthodontic system provide a new choice for torque control of the anterior teeth. Although anterior mini-implants and elastics can achieve incisor intrusion and lingual root torque, they cannot achieve the expected torque without additional torque control methods.

Abstract:Objectives: This study aimed to investigate the differences in treatment outcomes of upper anterior teeth retraction and intrusion with clear aligners and different types of elastics on mini-implants via the 3D finite element method. Methods: Cone-beam computed tomography (CBCT) was conducted to construct a 3D finite element model of maxillary bone, dentition, and clear aligners using Mimics, Geomagic, Solidworks, and Ansys. Four model groups were developed. Group 1 was the control group. Group 2 implanted a mini-implant between the upper second premolar and the upper first molar with a force of 0.98 N applied to the canine cut along the retracting direction for each side. Group 3 was based on Group 2, which implanted an additional mini-implant between the upper central incisors with a force of 0.98 N applied to the upper anterior teeth cuts. Group 4 was also based on Group 2, which implanted 2 additional mini-implants between the upper central and lateral incisors with a force of 0.56 N applied to the upper anterior teeth cuts for both sides. Anterior teeth torque changes, displacement patterns, and stress distributions were analyzed. Results: Among all the experimental groups, the anterior teeth demonstrated intrusion and distal inclination, accompanied with varied degrees of torque loss. In Group 3, the maxillary and lateral incisors had the smallest sagittal coronal displacement. In Group 4, the intrusion values of the maxillary and lateral incisors were the largest among all the experimental groups. The maximum stress concentration in Group 2 was the most significant among all the experimental groups. Conclusions: Implanting the mini-implant between the central incisors for traction is conducive to torque control. By contrast, implanting the mini-implants between the central incisors and lateral incisors combined with vertical traction is conducive to simple intrusion, avoiding the "bowing effect" to a certain extent.

Abstract:Objective To investigate the influences of torque effect for maxillary anterior teeth retraction using micro- implant anchorage system. Methods The three-dimensional finite element model was constructed using MIMICS software and ABAQUS finite element software. Under the situation of no or with torque and for different torque regions, the movement trend of the central incisor and lateral incisor was calculated and analyzed. Results The tendency of both the central and lateral incisor appears a lingual incline movement with no torque. Under the 100 g·cm torque force, with the region of load torque increasing, the labial movement of the central incisor reduced, and the lingual movement of the lateral incisor became more labial. Conclusions Torque was able to change the movement of teeth effectively. If the load torque decreased from the middle of the two central incisors to the distal direction, the results would be better.

Abstract:Objective To construct an 3-D FEM model including e-Brace brackets,dentition,periodontal ligaments,alveolar bone,lingual wires and micro-implants to analyze the biomechanical characteristics of the customized lingual bracket system during retracting maxillary anterior teeth at different micro-implant loading positions.Methods Volume models of the interested parts were first constructed based on CT scan data.Then the finite models of customized lingual system,including eBrace brackets and wires,dentition,periodontal ligaments,alveolar bone,and micro-implants were constructed.Results 1.Six 3-D FEM models were constructed at different micro-implant loading positions.The micro-implants were either between the second premolar and the first molar or the first molar and the second molar,at 2 mm,6 mm or 10 mm levels from the alveolar ridge crest.Every 3-D FEM model includes forty volume models.2.Ten-node tetrahedron analysis of the 3-D FEM model includes 741670 nodes,513925 elements at the micro-implant loading position between the second premolar and the first molar,2mm from the alveolar ridge crest.The other 3-D FEM models include similar nodes and elements at different micro-implant loading positions.Conclusions The originated eBrace customized lingual 3-D FEM model has high geometrical similarity,and is hence reasonable,reliable and practical.It could be used to analyze the biomechanical characteristics of the lingual system during retracting anterior teeth with micro-implants and sliding mechanisms.

Abstract:Objective To investigate the centre of resistance of the maxillary anterior teeth and the accompanied initial displacement of teeth in en masse retraction with micro-implant anchorage.MethodsA dry skull was scanned with high resolution spiral CT,and the obtained data were used to construct the three dimensional finite element model of "the micro-implant-segmented maxillary anterior teeth en masse retraction" force system.A 2 N horizontal retraction force by micro-implant were added to segmented maxillary teeth in every 2-mm interval from the archwire level and the initial displacement were calculated.ResultsThe centre of resistance was located in the position of 5.1 mm above alveolar ridge.The centre of rotation constants were different among the central incisor,lateral incisor and canine.Conclusion The bodily movement or tipping movement can be easily achieved by altering the horizontal height of the retraction force using the "micro-implant-segmented maxillary anterior teeth en masse retraction" force system.

Abstract:Objective: To investigate the effects of two-step retraction and en-masse retraction on tooth movement pattern of anterior teeth and posterior anchorage with clear aligners using three-dimensional finite element analysis. Methods: A finite element model of maxillary first premolar extraction case undergoing clear aligner treatment was established based on maxillofacial cone-beam CT data of a 24-year-old adult male with individual normal occlusion, who visited Department of Oral Surgery, Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine for impacted mandibular third molar in June, 2022. The initial tooth displacement of five anterior retraction protocols (two-step with canine retraction, two-step with incisor bodily retraction, two-step with incisor retraction-overtreatment, en-masse bodily retraction, and en-masse retraction-overtreatment) were evaluated. Results: Two step with canine retraction caused distal tipping of the canine and labial tipping of the incisors (0.18° for central incisor and 0.13° for lateral incisor). Two step with incisor retraction caused mesial tipping of the canine. In two step with bodily retraction protocol, uncontrolled lingual tipping was found in central incisor (0.29°) and lateral incisor (0.32°). In two-step with incisor retraction-overtreatment protocol, the movement pattern of the incisors didn't change, but the inclinations reduced to 0.21° and 0.18°. En-masse retraction caused distal tipping of the canine. In en-masse bodily retraction protocol, uncontrolled lingual tipping was also found in central incisor (0.19°) and lateral incisor (0.27°). In en-masse retraction-overtreatment protocol, the central incisor showed controlled lingual tipping (0.02°) and the lateral incisor showed palatal root movement (0.03° labial inclination). Posterior teeth exhibited mesial tipping in all five protocols. Conclusion: En-masse retraction with incisor overtreatment was beneficial to incisor torque control in clear aligner treatment.
3D Finite Element Study of the Physiological Anchorage Control Concept on Anchorage Molars in Lingual Orthodontics

Abstract:Objective: To study the effect of the physiological anchorage control concept on anchorage molars in lingual and labial orthodontic techniques. Methods: Three-dimensional finite element models, including the right maxillary first molar, periodontal ligament, alveolar bone, and buccal tube, were established. The models were divided into the McLaughlin-Bennett-Trevisi (MBT™) straight-wire model with 0-degree maxillary first molar axial inclination and the physiologic anchorage Speewire system (PASS) model with -7-degree maxillary first molar axial inclination. Simulated sliding retraction forces (1 N, 1.5 N, and 2 N) were loaded on the buccal side and lingual side, and retraction forces (0.5 N, 0.75 N, and 1 N) were loaded on the buccal and lingual sides simultaneously. The displacements, principal stresses, and von Mises stresses of the periodontal ligament under different conditions were derived. Results: The anchorage molars showed different degrees of rotation, tipping, intrusion, and extrusion. As the force increased, these displacement trends also increased. The mesial displacement of the buccal + lingual force loading was less than that of the other two groups. Under the same force load method, the mesial displacement of the PASS group was less than that of the MBT group. Tilt movement increases the tensile stress of the distal cervical margin and root mesial apical third and the compressive stress of the mesial cervical margin and root distal apical third. The maximum stress of the periodontal ligament was less than that of the other two groups when the lingual force was loaded. Conclusion: The physiological anchorage control concept in lingual orthodontics provides better sagittal anchorage control than in labial orthodontics, but there is no significant difference numerically. Attention should be given to the control of torsion, torque, and arch width. Tilt movement increases the PDL stress of the cervical margin and root apical third. The sliding retraction force should be loaded lingually to maintain the force value of 1∼1.5 N.

Abstract:BackgroundThis study was performed to evaluate the treatment effects of the antero-posterior lingual retractor (APLR), focusing on the 3-dimensional (3D) tooth movement of the maxillary anterior teeth and their alveolar bone levels.MethodsEn masse retraction was performed using either the C-lingual retractor (CLR, C-group, n = 9) or the antero-posterior lingual retractor (APLR, AP-group, n = 8). We evaluated 3D movement of the maxillary anterior teeth and alveolar bone levels, root length of the central incisors, long axes of the maxillary canines, and occlusal plane changes from CBCT images.ResultsAfter retraction, the central incisors were more significantly intruded and their root apex was more retracted in the AP-group. The long axis of the canine was well maintained in the AP-group. There were no differences in the steepness of occlusal plane and the incidence of alveolar bone loss or of root resorption during en masse retraction with the two retractors.ConclusionsThe clockwise bowing effect of the anterior segment was less with the APLR, which prevented unwanted canine movement.

The effects of lingual buttons, precision cuts, and patient-specific attachments during maxillary molar distalization with clear aligners: Comparison of finite element analysis

Abstract:Introduction: This study aimed to analyze the biomechanical effects of the combined use of clear aligners (CA) and auxiliaries (precision cuts, lingual buttons, and patient-specific attachments) on mesial tipping and extrusion of the premolars during maxillary molars distalization. Methods: Three-dimensional finite element method was employed to simulate clinical scenarios of CA with different auxiliaries for molar distalization. As such, 200 g of distal force was applied to the microimplants from the notches, lingual buttons, and hooks. Orthodontic tooth movement and the hydrostatic pressure in the periodontal ligament were compared. Results: Adding auxiliaries can provide the maxillary arch anchorage and promote the distal tipping of premolars and retroclination of maxillary incisors. In contrast, this effect was more pronounced in patient-specific attachment applications than in other types of auxiliaries. The independent application of the CA caused mesial tipping and extrusion of the premolar and also caused the incisor proclination. Conclusions: The anchorage loss caused by the CA alone could be alleviated with the assistance of auxiliaries. Notably, patient-specific attachments further reinforce the anchorage of the anterior arch by incorporating anchor teeth as 1 anchorage unit.
Tooth movement tendency of space closure by sliding mechanics in customized lingual appliance: A 3-dimensional finite element analysis

Abstract:Objective To construct an entire 3D finite element model including dentition,periodontal ligament,alveolar bone and the lingual appliance,according to the design principles of the customized lingual system.The biomechanical properties for sliding mechanics of the space closure stage were analyzed.Methods Using the 3D FE model,the teeth movement tendency in three dimensions were analyzed in different loading conditions(0.75 N,1.00 N,1.50 N ).Results The 3D finite element model including 38 volume models,372 225 nodes and 256 739 elements was constructed.Horizontal and vertical “bowing effect” with some unexpected tooth movement could be found during an increasing loading.Conclusions Both horizontal and vertical “bowing effect” appeared in the dental arch when using a single palatal force sliding mechanics in the customized lingual appliance.Therefore,clinicians should take some corresponding measures to counteract those side effects.
Stress Analysis of Periodontal Tissue in en Masse Retraction With Integration of Maxillary Anterior Teeth: A Three-Dimensional Finite Element Method Study

Abstract:Objective: To simulate the en masse traction technique with the integration (EMTI) of six maxillary anterior teeth using a finite element model (FEM) and explore various protocols for maxillary protrusion. The study aimed to investigate root displacement and stress distribution in the periodontal ligament (PDL) by varying the retraction position and direction of EMTI applied to the maxillary anterior teeth. No actual participants were involved. Materials and methods: The FEM model included six teeth (central and lateral incisors and canines) with a PDL thickness of 0.3 mm. The model encompassing the alveolar bone (ALB) and EMTI had 180,528 elements and 47,836 nodes. The EMTI integrated six anterior teeth via a 0.9-mm-diameter stainless steel lingual wire, equipped with three moment arms extending toward the root apex: one midline (central arm) and two distal to the canines (lateral arms). The position and direction of the traction force applied to the three moment arms of the EMTI were varied to assess crown and apex displacement, as well as PDL stress. Results: Lingual tipping was consistent across all protocols, emphasizing controlled incisor tipping. The application of horizontal traction at 10 mm and traction at 7 mm from the central and lateral arms of the EMTI, respectively, demonstrated the most uniform stress distribution across the PDL of all six anterior teeth. Conclusions and clinical significance: The FEM analysis results suggest that the new EMTI method, which retracts the maxillary anterior teeth as a unit, is effective for tooth movement and PDL stress distribution. The EMTI technique, with its specific traction protocols and emphasis on controlled tipping, appears to be a promising approach for addressing maxillary protrusions.
Three-dimensional finite element analysis of maxillary protraction with labiolingual arches and implants

Abstract:Introduction: In this study, we aimed to evaluate the effects of maxillary protraction using traditional labiolingual arches and implant-type protraction devices before orthopedic treatment of patients with skeletal Class III malocclusion. Methods: A 3-dimensional finite element model of the maxillofacial bones with high biologic similarity and including the sutures was constructed. Through stress and displacement calculations, a biomechanical study was performed for the maxillofacial bones, mandible, and sutures. Results: We quantified detailed changes in the sutures with 2 protraction methods to analyze their effects on the growth of the maxillofacial bones. Conclusions: (1) The labiolingual arch is suitable for skeletal Class III patients with crossbite and deep overbite. The frontomaxillary and zygomaticomaxillary sutures played major roles in the forward displacement and counterclockwise rotation of the maxilla. The temporozygomatic and pterygopalatine sutures did not change significantly. (2) The implant type of protraction device is suitable for skeletal Class III patients with crossbite and open bite. Both the frontomaxillary and zygomaticomaxillary sutures played decisive roles in the forward displacement and clockwise rotation of maxilla. The temporozygomatic and pterygopalatine sutures showed small changes. (3) The labiolingual arch caused less stimulatory growth on the maxilla, whereas the implant caused greater stimulatory growth on the maxilla. Protraction with the labiolingual arch is more suitable for early skeletal Class III patients at a younger age; protraction with an implant is applicable to skeletal Class III patients in the late mixed dentition or early permanent dentition.
The displacement of teeth and stress distribution on periodontal ligament under different upper incisors proclination with clear aligner in cases of extraction: a finite element study

Abstract:Abstract Objectives To investigate the displacement of dentition and stress distribution on periodontal ligament (PDL) during retraction and intrusion of anterior teeth under different proclination of incisors using clear aligner (CA) in cases involving extraction of the first premolars. Methods Models were constructed, consisting of the maxilla, PDLs, CA and maxillary dentition without first premolars. These models were then imported to finite element analysis (FEA) software. The incisor proclination determined the division of the models into three groups: Small torque (ST) with U1-SN = 100°, Middle torque (MT) with U1-SN = 110°, and High torque (HT) with U1-SN = 120°. Following space closure, a 200 g intrusion force was applied at angles of 60°, 70°, 80°, and 90° to the occlusal plane, respectively. Results CA therapy caused lingual tipping and extrusion of incisors, mesial tipping and intrusion of canines, and mesial tipping of posterior teeth in each group. As the proclination of incisors increased, the incisors presented more extrusion and minor retraction, and the teeth from the canine to the second molar displayed an increased tendency of intrusion. The peak Von Mises equivalent stress (VMES) value successively decreased from the central incisor to the canine and from the second premolar to the second molar, and the VMES of the second molar was the lowest among the three groups. When the angle between the intrusion force and occlusal plane got larger, the incisors exhibited greater intrusion but minor retraction. Conclusions The "roller coaster effect" usually occurred in cases involving premolar extraction with CA, especially in patients with protruded incisors. The force closer to the vertical direction were more effective in achieving incisor intrusion. The stress on PDLs mainly concentrated on the cervix and apex of incisors during the retraction process, indicating a possibility of root resorption.

Abstract:Background: Overtreatment design of clear aligner treatment (CAT) in extraction cases is currently primarily based on the clinical experience of orthodontists and is not supported by robust evidence on the underlying biomechanics. This study aimed to investigate the biomechanical effects of overtreatment strategies involving different maxillary anterior teeth intrusion patterns during anterior teeth retraction by CAT in extraction cases. Materials and methods: A finite element model of the maxillary dentition with the first premolar extracted was constructed. A loading method of clear aligners (CAs) based on the initial state field was proposed. The iterative method was used to simulate the long-term orthodontic tooth movement under the mechanical load exerted by the CAs. Three groups of CAs were utilized for anterior teeth retraction (G0: control group; G1: incisors intrusion group; G2: anterior teeth intrusion group). Tooth displacement and occlusal plane rotation tendency were analyzed. Results: In G0, CAT caused lingual tipping and extrusion of the incisors, distal tipping and extrusion of the canines, mesial tipping, and intrusion of the posterior teeth. In G1, the incisors showed minimal extrusion, whereas the canines showed increased extrusion and distal tipping tendency. G2 showed the smallest degree of posterior occlusal plane angle rotation, while the inclination tendency of the canines and second premolars decreased. Conclusion: 1. In CAT, tooth displacement tendency may change with increased wear time. 2. During anterior teeth retraction, the incisor intrusion pattern can provide effective vertical control for the lateral incisors but has little effect on the central incisors. Anterior teeth intrusion patterns can alleviate the inclination of canines and second premolars, resulting in partial relief of the roller-coaster effect.

Abstract:SIR–I read with interest the report of a suspected case of malignant hyperthermia in a child with laminin alpha 2 (merosin) deficiency in the absence of a triggering agent (1). I have two concerns about the report. First I believe that the authors did not substantially support their diagnosis of malignant hyperthermia. Their scoring of the episode is on the high side (2), nor is their explanation for the lack of a very elevated CPK very strong, since despite the use of dantrolene, CPK continued to rise in the postoperative period. As their patient exhibited a primary muscle response, did the authors look for myoglobinuria? My second concern is the linking of the disease laminin alpha 2 (merosin) deficiency to malignant hyperthermia. This is a common mistake to causally link two very rare diseases just because they happened simultaneously. As this particular child had previous general anesthetics with no evidence of malignant hyperthermia, it is not reasonable to label all children with this disease susceptible to MH. Torque Control of the Maxillary Incisors in Lingual and Labial Orthodontics: A 3-Dimensional Finite Element Analysis

Abstract:Introduction: Lingual orthodontics has developed rapidly in recent years; however, research on torque control variance of the maxillary incisors in both lingual and labial orthodontics is still limited, especially studies with 3-dimensional finite element methods. Thorough understanding of the biomechanical differences of incisor torque control during lingual and labial orthodontic treatment is critical for the best results. Methods: A 3-dimensional finite element model of the maxilla and the maxillary incisors was made with 98,106 nodes, 71,944 10-node solid elements, and 5236 triangle shell units. Horizontal retraction force, vertical intrusive force, and lingual root torque were applied to simulate labial and lingual orthodontic treatment. Then the distribution of the stress-strain (maximum and minimum principal stresses; maximum and minimum principal strains) in the periodontal ligament, the total displacement, and the vector graph of displacement of the nodes of the maxillary central incisor were analyzed and compared between labial and lingual orthodontics. Results: Loads of the same magnitude produced translation of the maxillary incisor in labial orthodontics but lingual crown tipping of the same tooth in lingual orthodontics. This suggests that loss of torque control of the maxillary incisors during retraction in extraction patients is more likely in lingual orthodontic treatment. Conclusions: Lingual orthodontics should not simply follow the clinical experience of the labial techniques but should increase lingual root torque, increase vertical intrusive force, and decrease horizontal retraction force properly to achieve the best orthodontic results. Evaluation of stress and displacement of maxillary canine during the single canine retraction in the maxillary first premolar extraction cases- A finite element study

Abstract:Objectives: This finite element study aimed to simulate maxillary canine movement during anterior teeth retraction. Materials and methods: Three methods of maxillary canine movement including miniscrew sliding with high hooks (MSH), miniscrew sliding with low hooks (MSL), and the traditional sliding method (TS) without using miniscrews were simulated using three-dimensional finite element analysis. The initial displacement of the maxillary canine, the maximum principal stress of the periodontal ligament and the Von Mises stress were calculated. Results: The distolingual tipping movements of the canine were shown in three movement modes. MSH showed a small tendency to lingual tipping movement and a extrusion movement while MSL had the largest lingual inclination. TS demonstrated a tendency toward distolingual torsion displacement. Compressive stress values were mainly concentrated in the range - 0.003 to -0.006 MPa. For tensile stress, the distribution of MSH and MSL was concentrated in the range 0.005 to 0.009 MPa, TS was mainly distributed about 0.003 MPa. Von Mises equivalent stress distribution showed no significant difference. Conclusions: The loss of tooth torque was inevitable, irrespective of which method was used to close the extraction space. However, miniscrew application and higher hooks reduced the loss of torque and avoided lingual rotation. Clinical relevance: This study shows that miniscrew implants with different hooks can better control the movement of the maxillary canines. The non-invasive nature of the finite element analysis and its good simulation of dental stress and instantaneous motion trend have a clinical advantage in the analysis of tooth movement.
