Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Machine learning (ML) allows the analysis of complex and large data sets and has the potential to improve healthcare. The clinical microbiology laboratory, at the interface of clinical practice and diagnostics, is of special interest for the development of ML systems.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objectives</h3><p>This narrative review aims to explore the current use of ML In clinical microbiology.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Sources</h3><p>References for this review were identified through searches of MEDLINE/PubMed, EMBASE, Google Scholar, biorXiv, arXiV, ACM Digital Library and IEEE Xplore Digital Library up to November 2019.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Content</h3><p>We found 97 ML systems aiming to assist clinical microbiologists. Overall, 82 (85%) ML systems targeted bacterial infections, 11 (11%) parasitic infections, nine (9%) viral infections and three (3%) fungal infections. Forty (41%) ML systems focused on micro-organisms detection, identification and quantification, 36 (37%) evaluated antimicrobial susceptibility and 21 (22%) targeted the diagnosis, disease classification and prediction of clinical outcomes. The ML systems used very diverse data sources: 21 (22%) used genomic data of micro-organisms, 19 (20%) microbiota data obtained by metagenomic sequencing, 19 (20%) analysed microscopic images, 17 (18%) spectroscopy data, 8 (8%) targeted gene sequencing, 6 (6%) volatile organic compounds, 4 (4%) bacterial colonies photographs, 4 (4%) transcriptome data, 3 (3%) protein structure and 3 (3%) clinical data. Most systems used high-income countries data (n=71, 73%) but a significant number used low- and middle-income countries data (n=36, 37%). Performance measures were reported for the 97 ML systems but no article described the use in clinical practice nor reported impact on processes or clinical outcomes.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Implications</h3><p>In clinical microbiology, ML has been used with various data sources and diverse practical applications. The evaluation and implementation processes represent the main gap of existing ML systems, requiring a focus on their interpretability and potential integration into real-world settings.</p>

Abstract:Purpose of review This review examines the current state and future prospects of machine learning (ML) in infection prevention and control (IPC) and antimicrobial stewardship (ASP), highlighting its potential to transform healthcare practices by enhancing the precision, efficiency, and effectiveness of interventions against infections and antimicrobial resistance. Recent findings ML has shown promise in improving surveillance and detection of infections, predicting infection risk, and optimizing antimicrobial use through the development of predictive analytics, natural language processing, and personalized medicine approaches. However, challenges remain, including issues related to data quality, model interpretability, ethical considerations, and integration into clinical workflows. Summary Despite these challenges, the future of ML in IPC and ASP is promising, with interdisciplinary collaboration identified as a key factor in overcoming existing barriers. ML's role in advancing personalized medicine, real-time disease monitoring, and effective IPC and ASP strategies signifies a pivotal shift towards safer, more efficient healthcare environments and improved patient care in the face of global antimicrobial resistance challenges.

Abstract:Background: Surveillance of hospital-acquired infections (HAIs) is the foundation of infection control. Machine learning (ML) has been demonstrated to be a valuable tool for HAI surveillance. We compared manual surveillance with a supervised, semiautomated, ML method, and we explored the types of infection and features of importance depicted by the model. Methods: From July 2021 to December 2021, a semiautomated surveillance method based on the ML random forest algorithm, was implemented in a Brazilian hospital. Inpatient records were independently manually searched by the local team, and a panel of independent experts reviewed the ML semiautomated results for confirmation of HAI. Results: Among 6,296 patients, manual surveillance classified 183 HAI cases (2.9%), and a semiautomated method found 299 HAI cases (4.7%). The semiautomated method added 77 respiratory infections, which comprised 93.9% of the additional HAIs. The ML model considered 447 features for HAI classification. Among them, 148 features (33.1%) were related to infection signs and symptoms; 101 (22.6%) were related to patient severity status, 51 features (11.4%) were related to bacterial laboratory results; 40 features (8.9%) were related to invasive procedures; 34 (7.6%) were related to antibiotic use; and 31 features (6.9%) were related to patient comorbidities. Among these 447 features, 229 (51.2%) were similar to those proposed by NHSN as criteria for HAI classification. Conclusion: The ML algorithm, which included most NHSN criteria and >200 features, augmented the human capacity for HAI classification. Well-documented algorithm performances may facilitate the incorporation of AI tools in clinical or epidemiological practice and overcome the drawbacks of traditional HAI surveillance.

Abstract:There is growing excitement about the clinical use of artificial intelligence and machine learning technologies. Advancements in computing and the accessibility of machine learning frameworks enable researchers to easily train predictive models using electronic health record data. However, there are several practical factors that must be considered when employing machine learning on electronic health record data. We provide a primer on machine learning and approaches commonly taken to address these challenges. To illustrate how these approaches have been applied to address antimicrobial resistance, we review the use of electronic health record data to construct machine learning models for predicting pathogen carriage or infection, optimizing empiric therapy, and aiding antimicrobial stewardship tasks. Machine learning shows promise in promoting the appropriate use of antimicrobials, although clinical deployment is limited. We conclude by describing potential dangers of, and barriers to, implementation of machine learning models in the clinic.

Abstract:Abstract Artificial intelligence (AI) refers to the performance of tasks by machines ordinarily associated with human intelligence. Machine learning (ML) is a subtype of AI; it refers to the ability of computers to draw conclusions (ie, learn) from data without being directly programmed. ML builds from traditional statistical methods and has drawn significant interest in healthcare epidemiology due to its potential for improving disease prediction and patient care. This review provides an overview of ML in healthcare epidemiology and practical examples of ML tools used to support healthcare decision making at 4 stages of hospital-based care: triage, diagnosis, treatment, and discharge. Examples include model-building efforts to assist emergency department triage, predicting time before septic shock onset, detecting community-acquired pneumonia, and classifying COVID-19 disposition risk level. Increasing availability and quality of electronic health record (EHR) data as well as computing power provides opportunities for ML to increase patient safety, improve the efficiency of clinical management, and reduce healthcare costs.

Abstract:Background: Healthcare-associated infections are infections that patients acquire during hospitalization or while receiving healthcare in other facilities. They represent the most frequent negative outcome in healthcare, can be entirely prevented, and pose a burden in terms of financial and human costs. With the development of new AI and ML algorithms, hospitals could develop new and automated surveillance and prevention models for HAIs, leading to improved patient safety. The aim of this review is to systematically retrieve, collect, and summarize all available information on the application and impact of AI in HAI surveillance and/or prevention. Methods: We conducted a systematic review of the literature using PubMed and Scopus to find articles related to the implementation of artificial intelligence in the surveillance and/or prevention of HAIs. Results: We identified a total of 218 articles, of which only 35 were included in the review. Most studies were conducted in the US (n = 10, 28.6%) and China (n = 5; 14.3%) and were published between 2021 and 2023 (26 articles, 74.3%) with an increasing trend over time. Most focused on the development of ML algorithms for the identification/prevention of surgical site infections (n = 18; 51%), followed by HAIs in general (n = 9; 26%), hospital-acquired urinary tract infections (n = 5; 9%), and healthcare-associated pneumonia (n = 3; 9%). Only one study focused on the proper use of personal protective equipment (PPE) and included healthcare workers as the study population. Overall, the trend indicates that several AI/ML models can effectively assist clinicians in everyday decisions, by identifying HAIs early or preventing them through personalized risk factors with good performance. However, only a few studies have reported an actual implementation of these models, which proved highly successful. In one case, manual workload was reduced by nearly 85%, while another study observed a decrease in the local hospital's HAI incidence from 1.31% to 0.58%. Conclusions: AI has significant potential to improve the prevention, diagnosis, and management of healthcare-associated infections, offering benefits such as increased accuracy, reduced workloads, and cost savings. Although some AI applications have already been tested and validated, adoption in healthcare is hindered by barriers such as high implementation costs, technological limitations, and resistance from healthcare workers. Overcoming these challenges could allow AI to be more widely and cost-effectively integrated, ultimately improving patient care and infection management.

Abstract:In recent years, machine learning (ML) has emerged as a powerful tool for solving a wide range of problems, including medical decision-making. The exponential growth of medical data over the past two decades has surpassed the capacity for manual analysis, prompting increased interest in automated data analysis and processing. ML algorithms, capable of learning from data with minimal human intervention, are particularly well-suited for medical data analysis and interpretation. One significant advantage of ML is the reduced cost of collecting labeled training data necessary for supervised learning. While numerous studies have explored the applications of ML in medicine, this survey specifically focuses on the use of ML across various medical research fields. We provide a comprehensive technical overview of existing studies on ML applications in medicine, highlighting the strengths and limitations of these approaches. Additionally, we discuss potential research directions for future exploration. These include the development of more sophisticated reward functions, as the accuracy of the reward function is crucial for ML performance, the integration of ML with other techniques, and the application of ML to new and emerging areas in genomics research. Finally, we summarize our findings and present the current state of the field and the future outlook for ML in medical application.

Abstract:Background: Sepsis, a life-threatening infection-induced inflammatory condition, has significant global health impacts. Timely detection is crucial for improving patient outcomes as sepsis can rapidly progress to severe forms. The application of machine learning (ML) and deep learning (DL) to predict sepsis using electronic health records (EHRs) has gained considerable attention for timely intervention. Methods: PubMed, IEEE Xplore, Google Scholar, and Scopus were searched for relevant studies. All studies that used ML/DL to detect or early-predict the onset of sepsis in the adult population using EHRs were considered. Data were extracted and analyzed from all studies that met the criteria and were also evaluated for their quality. Results: This systematic review examined 1942 articles, selecting 42 studies while adhering to strict criteria. The chosen studies were predominantly retrospective (n = 38) and spanned diverse geographic settings, with a focus on the United States. Different datasets, sepsis definitions, and prevalence rates were employed, necessitating data augmentation. Heterogeneous parameter utilization, diverse model distribution, and varying quality assessments were observed. Longitudinal data enabled early sepsis prediction, and quality criteria fulfillment varied, with inconsistent funding-article quality correlation. Conclusions: This systematic review underscores the significance of ML/DL methods for sepsis detection and early prediction through EHR data.

Abstract:A urinary tract infection, or UTI, is caused when bacteria get into the urinary tract- kidneys, bladder, or urethra. UTIs cause more than 8.1 million visits to healthcare providers each year. About 60% of women and 12% of men get infected with UTI during their lifetime, therefore being more prominent in females. UTIs can be found by analyzing a urine sample. The urine is examined under a microscope for bacteria or white blood cells that show infection. Healthcare providers may also take a urine culture. This test examines urine to detect and identify bacteria and yeast, which may be causing a UTI. Several models have been proposed to predict urine culture positivity based on urinalysis. Applications of machine learning (ML) methods have been used extensively to solve various complex challenges in recent years in various application areas. ML methods are characterized by their ability to examine data and discover exciting relationships, provide interpretation, and identify patterns. ML can help enhance the reliability, performance, predictability, and accuracy of diagnostic systems for many diseases. This survey provides a comprehensive review of the use of ML in the diagnosis of Urinary Tract infection in human beings. To provide a reliable classification of results assistance of 27 algorithms was tested. Algorithms applied included Logistic regression, Decision tree, Random Forest, and Support Vector Machine. Each model was evaluated by F1-score, AUC-ROC, accuracy, sensitivity, and specificity. Baseline epidemiological factors, previous antimicrobial consumption, medical history, and previous culture results were included as features. Machine learning models such as the Artificial neural network have been used as well for the prediction of the presence of urinary infection. Some specific parameters have been selected with the help of the Analysis of variance technique which gave high accuracy. This survey provides a comprehensive review of the use of ML in the medical field highlighting standard technologies and how they affect medical diagnosis. It provides valuable references and guidance for researchers, practitioners, and decision-makers framing future research and development directions. It is found that Machine Learning models can improve the early prediction of urine culture positivity and UTI by combining automated urinalysis with other clinical information. Clinical utilization of the models can reduce the risk of delayed antimicrobial therapy in patients with nonspecific symptoms of UTI and classify patients with UTI who require further treatment and close monitoring. In conclusion, the paper provides a survey on the machine learning models used with the highest accuracy to detect UTI potential patients.
Anthropometric study of male external genitalia of 320 healthy Nigerian adults.

Abstract:Five basic measurements of the male external genitalia were studied in 320 healthy male medical students belonging to the various parts of Nigeria (West Africa). Their ages range from 17 to 23 years. The measurements were as follows: average length of the penis (81.6 +/- 0.94 mm); circumference of the penis (88.3 +/- 0.02 mm); circumference of the scrotum (212.6 +/- 2.48 mm); length of right testis (46.8 +/- 0.54 mm); width of right testis (32.4 +/- 0.37 mm); length of left testis (46.0 +/- 0.53 mm); and width of left testis (31.4 +/- 0.36 mm). The size of the genitalia increases with the increase in age in younger year groups. The adult stages of genitalia development are reached at the age of 21 years in 89.0% of the individuals. The growth of the genitalia is continued with the increasing height and weight, but this growth slows down after attaining a definite height and weight.

Abstract:Machine learning (ML), artificial intelligence (AI), and other modern statistical methods are providing new opportunities to operationalise previously untapped and rapidly growing sources of data for patient benefit. Abstract:Nosocomial infections and Antimicrobial Resistance (AMR) stand as formidable healthcare challenges on a global scale. To address these issues, various infection control protocols and personalized treatment strategies, guided by laboratory tests, aim to detect bloodstream infections (BSI) and assess the potential for AMR. In this study, we introduce a machine learning (ML) approach based on Multi-Objective Symbolic Regression (MOSR), an evolutionary approach to create ML models in the form of readable mathematical equations in a multi-objective way to overcome the limitation of standard single-objective approaches. This method leverages readily available clinical data collected upon admission to intensive care units, with the goal of predicting the presence of BSI and AMR. We further assess its performance by comparing it to established ML algorithms using both naturally imbalanced real-world data and data that has been balanced through oversampling techniques. Our findings reveal that traditional ML models exhibit subpar performance across all training scenarios. In contrast, MOSR, specifically configured to minimize false negatives by optimizing also for the F1-Score, outperforms other ML algorithms and consistently delivers reliable results, irrespective of the training set balance with F1-Score.22 and.28 higher than any other alternative. This research signifies a promising path forward in enhancing Antimicrobial Stewardship (AMS) strategies. Notably, the MOSR approach can be readily implemented on a large scale, offering a new ML tool to find solutions to these critical healthcare issues affected by limited data availability.

Abstract:Abstract Background Artificial intelligence (AI) and machine learning (ML) are poised to transform infectious disease testing. Uniquely, infectious disease testing is technologically diverse spaces in laboratory medicine, where multiple platforms and approaches may be required to support clinical decision-making. Despite advances in laboratory informatics, the vast array of infectious disease data is constrained by human analytical limitations. Machine learning can exploit multiple data streams, including but not limited to laboratory information and overcome human limitations to provide physicians with predictive and actionable results. As a quickly evolving area of computer science, laboratory professionals should become aware of AI/ML applications for infectious disease testing as more platforms are become commercially available. Content In this review we: (a) define both AI/ML, (b) provide an overview of common ML approaches used in laboratory medicine, (c) describe the current AI/ML landscape as it relates infectious disease testing, and (d) discuss the future evolution AI/ML for infectious disease testing in both laboratory and point-of-care applications. Summary The review provides an important educational overview of AI/ML technique in the context of infectious disease testing. This includes supervised ML approaches, which are frequently used in laboratory medicine applications including infectious diseases, such as COVID-19, sepsis, hepatitis, malaria, meningitis, Lyme disease, and tuberculosis. We also apply the concept of “data fusion” describing the future of laboratory testing where multiple data streams are integrated by AI/ML to provide actionable clinical knowledge.

Abstract:Hospital-acquired infections (HAIs) contribute to increased mortality rates and extended hospital stays. Patients with complex neurological impairments, secondary to conditions such as acquired brain injury or progressive degenerative conditions are particularly prone to HAIs and often have the worst resulting clinical outcomes and highest associated cost of care. Research indicates that the prompt identification of such infections can significantly mitigate mortality rates and reduce hospitalisation duration. The current standard of care for timely detection of HAIs for inpatient acute and post-acute care settings in the UK is the National Early Warning Score v02 (NEWS2). NEWS2, despite its strengths, has been shown to have poor prognostic accuracy for specific indications, such as infections. This study developed a machine learning (ML) based risk stratification tool, utilising routinely collected patient electronic health record (EHR) data, encompassing over 800+ patients and 400k+ observations collected across 4-years, aimed at predicting the likelihood of infection in patients within an inpatient care setting for patients with complex acquired neurological conditions. Built with a combination of historical patient data, clinical coding, observations, clinician reported outcomes, and textual data, we evaluated our framework to identify individuals with an elevated risk of infection within a 7-day time-frame, retrospectively over a 1-year "silent-mode" evaluation. We investigated several time-to-event model configurations, including manual feature-based and data-driven deep generative techniques, to jointly estimate the timing and risk of infection onset. We observed strong performance of the models developed in this study, achieving high prognostic accuracy and robust calibration from 72-6 hours prior to clinical suspicion of infection, with AUROC values ranging from 0.776-0.889 and well-calibrated risk estimates exhibited across those time intervals (IBS<0.178). Furthermore, by assigning model-generated risk scores into distinct categories (low, moderate, high, severe), we effectively stratified patients with a higher susceptibility to infections from those with lower risk profiles. Post-hoc explainability analysis provided valuable insights into key risk factors, such as vital signs, recent infection history, and patient age, which aligned well with prior clinical knowledge. Our findings highlight our framework's potential for accurate and explainable insights, facilitating clinician trust and supporting integration into real-world patient care workflows. Given the heterogeneous and complex patient population, and our under-utilisation of the data recorded in routine clinical notes and lab reports, there are considerable opportunities for performance improvement in future research by expanding our model's multimodal capabilities, generalisability, and additional model personalisation steps.
Machine learning in infectious diseases: potential applications and limitations

Abstract:Infectious diseases are a major threat for human and animal health worldwide. Artificial Intelligence (AI) combined algorithms including Machine Learning and Big Data analytics have emerged as a potential solution to analyse diverse datasets and face challenges posed by infectious diseases. In this commentary we explore the potential applications and limitations of ML to management of infectious disease. It explores challenges in key areas such as outbreak prediction, pathogen identification, drug discovery, and personalized medicine. We propose potential solutions to mitigate these hurdles and applications of ML to identify biomolecules for effective treatment and prevention of infectious diseases. In addition to use of ML for management of infectious diseases, potential applications are based on catastrophic evolution events for the identification of biomolecular targets to reduce risks for infectious diseases and vaccinomics for discovery and characterization of vaccine protective antigens using intelligent Big Data analytics techniques. These considerations set a foundation for developing effective strategies for managing infectious diseases in the future.

Machine Learning-based Prediction Models for C difficile Infection: A Systematic Review

Abstract:Background: Despite research efforts, predicting Clostridioides difficile incidence and its outcomes remains challenging. This systematic review aimed to evaluate the performance of machine-learning (ML) models in predicting CDI incidence and complications using clinical data from electronic health records. Methods: We conducted a comprehensive search of databases (OVID, Embase, MEDLINE ALL, Web of Science, and Scopus) from inception up to September 2023. Studies employing ML techniques for predicting CDI or its complications were included. The primary outcome was the type and performance of ML models assessed using the area under the receiver operating characteristic curve (AUROC). Results: Twelve retrospective studies that evaluated CDI incidence and/or outcomes were included. The most common used ML models were random forest and Gradient Boosting. The AUROC ranged from 0.60 to 0.81 for predicting CDI incidence, 0.59 to 0.80 for recurrence, and 0.64 to 0.88 for predicting complications. Advanced ML models demonstrated similar performance to traditional logistic regression. However, there was notable heterogeneity in defining CDI and the different outcomes, including incidence, recurrence, and complications, and a lack of external validation in most studies. Conclusion: ML models show promise in predicting CDI incidence and outcomes. However, the observed heterogeneity in CDI definitions and the lack of real-world validation highlight challenges in clinical implementation. Future research should focus on external validation and the use of standardized definitions across studies.

Identifying the need for infection-related consultations in intensive care patients using machine learning models

Abstract:Infection-related consultations on intensive care units (ICU) have a positive impact on quality of care and clinical outcome. However, timing of these consultations is essential and to date they are typically event-triggered and reactive. Here, we investigate a proactive approach to identify patients in need for infection-related consultations by machine learning models using routine electronic health records. Data was retrieved from a mixed ICU at a large academic tertiary care hospital including 9684 admissions. Infection-related consultations were predicted using logistic regression, random forest, gradient boosting machines, and long short-term memory neural networks (LSTM). Overall, 7.8% of admitted patients received an infection-related consultation. Time-sensitive modelling approaches performed better than static approaches. Using LSTM resulted in the prediction of infection-related consultations in the next clinical shift (up to eight hours in advance) with an area under the receiver operating curve (AUROC) of 0.921 and an area under the precision recall curve (AUPRC) of 0.541. The successful prediction of infection-related consultations for ICU patients was done without the use of classical triggers, such as (interim) microbiology reports. Predicting this key event can potentially streamline ICU and consultant workflows and improve care as well as outcome for critically ill patients with (suspected) infections.
