Abstract:Nasopharyngeal carcinoma (NPC) is a common type of cancer in South East Asia with peculiar epidemiology, pathology, clinical behavior and response to treatment characteristics. To the best of our knowledge, this is the first study to investigate the use of a contrast-enhanced ultrasound (CEUS) as a predictor for the therapeutic response in lymph node metastases of NPC patients treated with radiation-based therapy. Sixty-seven NPC patients with lymph node metastases underwent the lymph nodes CEUS examination twice; pre- and in-treatment (at the 5th fraction radiotherapy), respectively. The CEUS parameters were acquired through Qontrast_4.0 software and mainly included peak intensity (PI) and time to peak (TTP). The response assessment at the lymph nodes revealed a complete response (CR) in 48 patients and partial response (PR) in 19 patients. There was a significant difference in pre-treatment PI (PIpre) between the patients who showed CR or PR, but the predicted sensitivity and specificity of PIpre was low. The mean in-treatment PI (PIin) value of the lymph nodes that achieved a CR was 34.24±3.78%, which was significantly higher than the PIin value for PR, 25.62±2.30% (P<0.001). Furthermore, the PIratio, a PI-quotient, was calculated by dividing the PIin by the corresponding PIpre. The higher PIratio was also observed in CR lymph nodes (0.81±0.01 vs. 0.66±0.01; P=0.001), and the mean change in PI (PIΔ; PIΔ = PIpre-PIin) was smaller in the patients with CR nodes compared to the patients with PR nodes (7.79±3.28 vs. 13.77±1.90%; P=0.000). No difference was observed in TTPpre or TTPin between the CR or PR lymph nodes patients. A receiver operating characteristic curve was constructed to assess the accuracy of the parameters for the prediction of the therapeutic responses. The sensitivity and specificity of PIin in predicting the therapeutic response was 94.3 and 88.2%, and the corresponding figures of the PIratio were 92.5 and 83.8%, respectively. The CEUS parameters during the early course of radiation-based therapy, PIin and PIratio, are associated with the therapeutic response of NPC lymph node metastases, with a high predicted sensitivity and specificity, thus yielding the conceivable predictors with the potential to individualize treatment.

Abstract:BACKGROUND: The authors prospectively evaluated the correlation between technetium-99m methoxyisobutyl isonitrile ((99m)Tc-MB) accumulation in tumors and response to induction chemotherapy in patients with nasopharyngeal carcinoma (NPC). METHODS: Eighty-six patients with locally advanced NPC underwent single-photon emission computed tomography 15 minutes after an intravenous injection of 740 megabecquerels (20 mCi) (99m)Tc-MIBI before chemotherapy. The tumor uptake ratio (TUR) was calculated. Two weeks after the second cycle of combined chemotherapy with 5-fluorouracil (5-FU) and cisplatin (DDP), the tumor response rate was evaluated. The correlation between (99m)Tc-MIBI accumulation in tumors and response to chemotherapy with 5-FU/DDP was examined. RESULTS: Positive accumulation of (99m)Tc-MIBI in tumors was observed in 76 patients (88.4%). The tumor response was a complete response (CR) in 8 patients, a partial response (PR) in 68 patients, stable disease (SD) in 9 patients, and progressive disease (PD) in 1 patient. The response rate (CR and PR) to 5-FU/DDP chemotherapy in patients who had positive (99m)Tc-MIBI accumulation (tumor uptake ratio [TUR]>1.1) was higher than that in patients who had negative (99m)Tc-MIBI accumulation (TUR <= 1.1; 98.7% vs 10%; P<.001). CONCLUSIONS: Patients with negative (99m)Tc-MIBI accumulation were resistant to 5-FU/DDP chemotherapy. (99m)Tc-MIBI imaging in patients with NPC was capable of predicting tumor response to chemotherapy with 5-FU/DDP and can help in the selection of patients for induction chemotherapy. Abstract:Background and purpose: Neoadjuvant chemotherapy with docetaxel was gaining more attention in the treatment for locoregionally advanced nasopharyngeal carcinoma(NPC). The prediction of the sensitivity to neoadjuvant chemotherapy with docetaxel could enable clinicians to individualize treatment protocols for NPC.Recently we found that 99mTc-MIBI imaging in NPC could predict tumor response to chemotherapy with cisplatin plus5-FU. However, there was no study to support similar findings in NPC patients receiving chemotherapy containing docetaxel. This study was to evaluate the value of double-phase 99mTc-MIBI SPECT/CT in predicting response to neoadjuvant chemotherapy with docetaxel-based regimen for nasopharyngeal carcinoma. Methods: Thirty-one nasopharyngeal carcinoma patients participated in this prospective study. Before treatment, early and delayed singlephoton emission computed tomography/compute tomography(SPECT/CT) images were obtained instantly and 2 hours after an intravenous injection of 25-30 mCi 99mTc-MIBI. All patients received neoadjuvant chemotherapy consisting of docetaxel, cisplatin plus 5-FU for two cycles. The relationships between efficacy of neoadjuvant chemotherapy and the early uptake ratio, late uptake ratio and washout rate of99mTc-MIBI were evaluated.Results: According to the MRI, the early uptake of 99mTc-MIBI(2.67±0.83) in the lesions which were sensitive to chemotherapy was significantly higher compared with that(1.69±0.46) in the insensitive lesions(P=0.003). The difference of the late uptake between the sensitive(1.46±0.39) and the insensitive(1.06±0.62) was also statistically significant(P=0.026). However, the washout rate of 99mTc-MIBI was not significantly different between the two groups(P=0.23). Through ROC curve analysis, the AUC for early uptake of 99mTc-MIBI and late uptake were 0.84. The sensitivity, specificity, positive predictive value and negative predictive value were 74.2%, 87.5%, 95.8% and 46.7% for early uptake when the cut off value of 1.97 was used. Conclusion: The uptake of 99mTc-MIBI in both early phase and late phase could predict the efficacy of neoadjuvant chemotherapy with docetaxel-based regimen.
Prediction of the response to docetaxel-based chemotherapy for locoregionally advanced nasopharyngeal carcinoma: the role of double-phase 99m Tc-MIBI SPECT/CT

Abstract:To evaluate the role of double-phase 99m Technetium labeled sestamibi ( 99m Tc-MIBI) single-photon emission computed tomography/compute tomography (SPECT/CT) in predicting the response to neoadjuvant chemotherapy with docetaxel-based regimen in patients with nasopharyngeal carcinoma. Thirty-nine pathologically proven nasopharyngeal carcinoma patients participated in this prospective study. Before treatment, early and delayed SPECT/CT images were obtained 5 min and 2 h after an intravenous injection of 25–30 mCi 99m Tc-MIBI. All patients received neoadjuvant chemotherapy consisting of docetaxel, cisplatin plus 5-fluorouracil for two cycles. The relationships between the efficacy of neoadjuvant chemotherapy and early uptake ratio (EUR), delayed uptake ratio (DUR) and washout rate (WR) of 99m Tc-MIBI were evaluated. The EUR of 99m Tc-MIBI (2.8 ± 0.97) in the lesions which were sensitive to chemotherapy was significantly higher ( p < 0.001) compared with that (1.69 ± 0.46) in the insensitive lesions. The difference of the DUR between the sensitive (1.65 ± 0.58) and the insensitive (1.06 ± 0.62) was also statistically significant ( p = 0.011). However, the WR of 99m Tc-MIBI was not significantly different between the two groups. When a EUR of 1.97 and a DUR of 1.06 were used as the cutoff value, the sensitivity, specificity, positive and negative predictive values were 76.1, 87.5, 97.2 and 38.8 % for EUR, 91.3, 67.5, 90 and 66.6 % for DUR, respectively. These results suggest that the early and delayed uptake ratios of 99m Tc-MIBI calculated with SPECT/CT may have predictive value for identifying the response to neoadjuvant chemotherapy with docetaxel-based regimen in NPC patients.

Abstract:The purpose of this study was to explore the feasibility of 18F-Fluorothymidine (18F-FLT) and 18F-Fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) in predicting treatment response of nasopharyngeal carcinoma (NPC). Patients with NPC of Stage II-IVB were prospectively enrolled, receiving 2 cycles of neoadjuvant chemotherapy (NACT), followed by concurrent chemoradiotherapy. Each patient underwent pretreatment and post-NACT FLT PET/CT and FDG PET/CT. Standard uptake values (SUV) and tumor volume were measured. Tumor response to NACT was evaluated before radiotherapy by MRI (magnetic resonance imaging), and tumor regression at the end of radiotherapy was evaluated at 55 Gy, according to RECIST 1.1 Criteria. Finally, 20 patients were consecutively enrolled. At the end of radiotherapy, 7 patients reached complete regression while others were partial regression. After 2 cycles of NACT both FLT and FDG parameters declined remarkably. Parameters of FDG PET were more strongly correlated to tumor regression than those of FLT PET.70% SUVmax was the best threshold to define contouring margin around the target. Some residual lesions after NACT showed by MRI were negative in PET/CT. Preliminary results showed both 18F-FDG and 18F-FLT PET have the potential to monitor and predict tumor regression.

Abstract:Previous studies have reported that the radiosensitivity is associated with apoptosis. Hereby, we aimed to investigate the value of F-18-ML-10 PET/CT, which selectively targeted cells undergoing apoptosis, in predicting radiosensitivity of human nasopharyngeal carcinoma (NPC) xenografts. We used CNE1 (highly differentiated) and CNE2 (poorly differentiated) NPC cell lines to construct tumor models, which had very different radiosensitivities. After irradiation, the volumes of CNE2 tumors decreased significantly while those of CNE1 tumors increased. In F-18-ML-10 imaging, the values of tumor/muscle (T/M) between CNE1 and CNE2 mice were statistically different at both 24 h and 48 h after irradiation. Besides, Delta T/M1-0 and Delta T/M2-0 of CNE2 mice were higher than those of CNE1 mice, demonstrating obvious discrepancy. Furthermore, we observed obvious changes of radioactive distribution in CNE2 group. On the contrary, T/M of F-18-FDG in irradiation group revealed no obvious change in both CNE1 and CNE2 groups. In conclusion, F-18-ML-10 animal PET/CT showed its potential to predict radiosensitivity in NPC.

The role of 18F-FDG PET/CT in predicting the pathological response to neoadjuvant PD-1 blockade in combination with chemotherapy for resectable esophageal squamous cell carcinoma

Abstract:Purpose: Accurate assessment of residual disease of tumor and lymph nodes after neoadjuvant immunochemotherapy is crucial in the active surveillance for patients with pathological complete response (pCR) and the optimal extent of lymphadenectomy for patients with non-pCR. This post hoc analysis aimed to evaluate the performance of 18F-FDG PET/CT to predict the pathological response to neoadjuvant immunochemotherapy for esophageal squamous cell carcinoma (ESCC). Methods: Fifty-eight resectable ESCC patients received two cycles of camrelizumab in combination with chemotherapy and were enrolled in the final analysis. The 18F-FDG PET/CT scans were acquired at baseline (scan-1) and after immunochemotherapy but prior to surgery (scan-2). Maximum standardized uptake value (SUVmax), mean standardized uptake value (SUVmean), tumor-to-blood pool SUVmax ratio (SUVTBR), metabolic tumor volume (MTV), and total lesion glycolysis (TLG) were evaluated for their association with the pathological response to immunochemotherapy. Results: Nineteen patients (32.8%, 19/58) had pCR and thirty-nine patients (67.2%, 39/58) had non-pCR after two doses of camrelizumab and chemotherapy. At scan-2, the SUVmax, SUVmean, SUVTBR, TLG, and MTV were significantly lower in pCR than in non-pCR patients. Decrease in TLG and MTV between scan-2 and scan-1 of the same patient was significantly higher in the pCR than in the non-pCR group. In the receiver operating characteristic curve analysis, SUVmax, SUVmean, SUVTBR, TLG, and MTV in scan-2 showed excellent predictive value for the pCR of primary tumors. Furthermore, SUVmax in scan-2 were higher in positive lymph nodes than in negative ones, suggesting a high negative predictive ability (98.6%) with a cut-off value at 1.4. Conclusion: The parameters of 18F-FDG PET/CT have the excellent performance for predicting pCR after the combined neoadjuvant immunochemotherapy in resectable ESCC. Abstract:A very fast method for calculating line shapes in the presence of an external magnetic field accounting for charge particle dynamics is proposed. It is based on a reformulation of the frequency fluctuation model, which provides an expression of the dynamic line shape as a functional of the static distribution function of frequencies. In the presence of an external magnetic field, the distribution of intensity and polarization of the emission depends on the angle between the observation line and the magnetic field's direction. Comparisons with numerical simulations and experimental results for various plasma conditions show very good agreement. Results on hydrogen lines in the context of magnetic fusion and the Lyman-α line, accounting for fine structure, emitted by argon in the context of inertial fusion, are also presented.

Abstract:TT‐virus (TTV, patient initials: T.T.), a novel DNA virus, was first isolated in Japan in 1997 from serum of a patient with post‐transfusion hepatitis of unknown aetiology. To date, the contribution of TTV to liver disease remains doubtful. The potential for transmission via blood and blood products makes it essential to establish the prevalence of TTV viraemia in the blood donor population. 413 blood donor serum samples were chosen randomly, the DNA was extracted and TTV‐specific DNA amplified by nested polymerase chain reaction (PCR). TTV infection was present in 13 out of 413 (3.15%) blood donors in the Auckland region of New Zealand using a set of primers targeting open reading frame (ORF) 1. These 13 amplification products (264 bp) were sequenced and TTV genotypes determined. Alignment with published TTV sequences showed that seven (53.8%) of the thirteen positive serum samples belonged to genotype 1, five (38.5%) belonged to genotype 2 and one (7.7%) could not be classified as either genotype 1 or 2. One hundred twenty‐seven blood donor serum samples were retested with a second set of primers targeting the 5′ region of the TTV genome in a single round PCR. Forty‐three samples were positive for TTV DNA with these primers resulting in a prevalence of 37%. The data demonstrate that TTV is present among New Zealand blood donors and support the need for further investigation into the natural history of TTV infection. J. Med. Cetuximab in Combination with Chemoradiation after Induction Chemotherapy of Locoregionally Advanced Nasopharyngeal Carcinoma: Preliminary Results

Abstract:AIM This article aimed to determine treatment compliance, acute toxicities and the short-term curative effects of combining cetuximab with chemoradiation following induction chemotherapy of locoregionally advanced nasopharyngeal carcinoma (NPC). MATERIALS & METHODS A total of 21 patients with locoregionally advanced NPC were scheduled to receive one cycle of induction chemotherapy followed by intensity-modulated radiotherapy, weekly administration of cetuximab and concurrent chemotherapy (at least two cycles with a limit of six). RESULTS All 21 patients completed the planned radiotherapy, 14 patients (67%) without treatment breaks, and 20 patients (95.2%) completed the planned cetuximab therapy. Grade 4 toxicities included leukopenia (seven patients; 33.4%), neutropenia (three patients; 14.3%), thrombocytopenia (one patient; 4.8%) and acneiform rash (one patient; 4.8%). Over a median follow-up period of 13 months (range: 3-23 months), the local, regional and distant control rates were 100, 100 and 95.2%, respectively. CONCLUSION Cetuximab in combination with intensity-modulated radiotherapy and chemoradiation is a feasible strategy against locoregionally advanced NPC. Preliminary survival data are encouraging compared with historic data.
Quantitative parameter analysis of pretreatment dual-energy computed tomography in nasopharyngeal carcinoma cervical lymph node characteristics and prediction of radiotherapy sensitivity

Abstract:Background: Treatment efficacy may differ among patients with nasopharyngeal carcinoma (NPC) at similar tumor-node-metastasis stages. Moreover, end-of-treatment tumor regression is a reliable indicator of treatment sensitivity. This study aimed to investigate whether quantitative dual-energy computed tomography (DECT) parameters could predict sensitivity to neck-lymph node radiotherapy in patients with NPC. Methods: Overall, 388 lymph nodes were collected from 98 patients with NPC who underwent pretreatment DECT. The patients were divided into complete response (CR) and partial response (PR) groups. Clinical characteristics and quantitative DECT parameters were compared between the groups, and the optimal predictive ability of each parameter was determined using receiver operating characteristic (ROC) analysis. A nomogram prediction model was constructed and validated using univariate and binary logistic regression. Results: DECT parameters were higher in the CR group than in the PR group. The iodine concentration (IC), normalized IC, Mix-0.6, spectral Hounsfield unit curve slope, effective atomic number, and virtual monoenergetic images were significantly different between the groups. The area under the ROC curve of the DECT parameters was 0.73-0.77. Based on the binary logistic regression, a column chart was constructed using 10 predictive factors, including age, sex, N stage, maximum lymph node diameter, arterial phase NIC, venous phase NIC, λHU and spectral Hounsfield units at 70 keV. The area under the ROC curve value of the constructed model was 0.813, with a sensitivity and specificity of 85.6% and 81.3%, respectively. Conclusion: Quantitative DECT parameters could effectively predict the sensitivity of NPC to radiotherapy. Therefore, DECT parameters and NPC clinical features can be combined to construct a nomogram with high predictive power and used as a clinical analytical tool.

Abstract:We prospectively investigated the prognostic value of imaging parameters for nasopharyngeal carcinoma (NPC) using dynamic contrast-enhanced MRI (DCE-MRI), diffusion-weighted imaging (DWI), and 2-deoxy-2-[fluorine-18]fluoro-D-glucose positron emission tomography (18F-FDG PET)/computed tomography (CT). Patients with stage III–IVb NPC underwent F-FDG PET/CT, DCE-MRI, and DWI before treatment. Kaplan-Meier and Cox-regression analyses were used to assess associations of PET and MRI imaging biomarkers with overall survival (OS) and recurrence-free survival (RFS). We used independent prognosticators to establish prognostic models; model performance was examined using Harrell’s concordance index (c-index). Sixty-one patients were available for analysis, as 13 patients died and 20 experienced recurrence. Total lesion glycolysis (TLG) (p = 0.002) from PET/CT and the initial area under the curve (iAUC) (p = 0.036) from DCE-MRI were identified as independent prognosticators of OS; Epstein-Barr virus (EBV) DNA (p = 0.027), the extracellular volume fraction (Ve) (p = 0.027) from DCE-MRI, and TLG/iAUC (p = 0.025) were significant predictors of RFS. The c-indices of the prognostic models incorporating TLG + iAUC in predicting OS and incorporating EBV DNA + Ve + TLG/iAUC in predicting RFS were 0.79 and 0.76, respectively. These were significantly higher than the corresponding c-indices of the TNM staging system (p = 0.047 and 0.025, respectively); they were also higher than those of models with only MRI or PET biomarkers. In conclusion, the combination of pretreatment DCE-MRI and 18F-FDG PET/CT imaging biomarkers helps survival prediction in advanced NPC. Integrating MRI perfusion with PET metabolism and plasma EBV information may aid clinicians in planning the optimal personalized management strategy.

Radiomics Analysis of PET and CT Components of PET/CT Imaging Integrated with Clinical Parameters: Application to Prognosis for Nasopharyngeal Carcinoma

Abstract:PurposeTo investigate the prognostic performance of radiomics features, as extracted from positron emission tomography (PET) and X-ray computed tomography (CT) components of baseline 2-deoxy-2-[18F]fluoro-D-glucose ([18F]FDG) PET/CT images and integrated with clinical parameters, in patients with nasopharyngeal carcinoma (NPC).ProceduresOne hundred twenty-eight NPC patients (85 vs. 43 for training vs. validation), containing a subset of 86 patients with local-regional advanced stage, were enrolled. All patients underwent pretreatment PET/CT scans (mean follow-up time 24 ± 14 months). Three thousand two hundred seventy-six radiomics features extracted from PET or CT components and 13 clinical parameters were used to predict progression-free survival (PFS). Univariate analysis with Benjamini–Hochberg false discovery rate (FDR) correction was first used to screen significant features, and redundant features with Spearman’s correlation > 0.8 were further eliminated. Then, seven multivariate models involving PET features and/or CT features and/or clinical parameters (denoted as clinical, PET, CT, clinical + PET, clinical + CT, PET + CT and clinical + PET + CT) were constructed by forward stepwise multivariate Cox regression. Model performance was evaluated by concordance index (C-index).ResultsSixty patients encountered events (28 recurrences, 17 metastases, and 15 deaths). Six clinical parameters, 3 PET features, and 14 CT features in training cohort and 4 clinical parameters, 10 PET features, and 4 CT features in subset of local-regional advanced stage were significantly associated with PFS. Combining PET and/or CT features with clinical parameters showed equal or higher prognostic performance than models with PET or CT or clinical parameters alone (C-index 0.71–0.76 vs. 0.67–0.73 and 0.62–0.75 vs. 0.54–0.75 for training and validation cohorts, respectively), while the prognostic performance was significantly improved in local-regional advanced cohort (C-index 0.67–0.84 vs. 0.64–0.77, p value 0.001–0.059).ConclusionRadiomics features extracted from the PET and CT components of baseline PET/CT images provide complementary prognostic information and improved outcome prediction for NPC patients compared with use of clinical parameters alone.

Abstract:Technology in Cancer Research &Treatment, Volume 22, Issue , January-December 2023. PurposeTo investigate the capability of an Magnetic resonance imaging (MRI) radiomics model based on pretreatment texture features in predicting the short-term efficacy of recombinant human endostatin (RHES) plus concurrent chemoradiotherapy (CCRT) for nasopharyngeal carcinoma (NPC).MethodsWe retrospectively enrolled 65 patients newly diagnosed as having NPC and treated with RHES + CCRT. A total of 144 texture features were extracted from the MRI before RHES + CCRT treatment of all the NPC patients. The maximum relevance minimum redundancy (mRMR) method was used to remove redundant, irrelevant texture features, and calculate the Rad score of the primary tumor. Multivariable logistic regression was used to select the most predictive features subset, and prediction models were constructed. The performance of the 3 models in predicting the early response of RHES + CCRT for NPC was explored.ResultsThe diagnostic efficiency of combined model and radiomics model in distinguishing between the effective and the ineffective groups of patients was found to be moderate. The area under the ROC curve (AUC) of the combined model and radiomics model was 0.74 (95% confidence interval [CI]: 0.62-0.86) and 0.71 (95% CI: 0.58-0.84), respectively, with both being higher than the AUC of the clinics model (0.63, 95% CI: 0.49-0.78). Compared with the radiomics model, the combined model showed marginally improved diagnostic performance in predicting RHES + CCRT treatment response. The accuracy of combined model and radiomics model for RHES + CCRT response assessment in NPC were higher than those of the clinics model (0.723, 0.723 vs 0.677).ConclusionThe pretreatment MRI-based radiomics may be a noninvasive and effective method for the prediction of RHES + CCRT early response in patients with NPC.

Abstract:Purpose To prospectively evaluate the feasibility of diffusion-weighted magnetic resonance imaging (DWI) for monitoring early treatment response to chemoradiotherapy (CRT) of nasopharyngeal carcinoma (NPC). Materials and methods Thirty-one patients with stage III and IV NPC were enrolled in this study from February 2012 to November 2012．T2-weighted and DWI sequences with diffusion factor of 0 and 800mm²/s were performed using a 3.0 T Philips Achieva TX scanner at baseline and 3 days, 20 days (after the first cycle of chemotherapy), 50 days (6 days after radiotherapy initiation) after neoadjuvant chemotherapy (NAC) initiation. The diameter of each primary lesion and target metastatic lymph node before and after the first cycle of NAC was measured and classified into stable disease (SD), partial response (PR) or completed response (CR) based on RECIST 1.1. The apparent diffusion coefficient (ADC) values and changes compared to baseline at each time point were compared between responders (CR and PR) and non-responders (SD). The rates of residual at the end of CRT were compared between these two groups. Results A significant increase in ADC was observed at each stage of therapy (P=.001) in lesions of primary and metastatic. The ADC values (ADC), ADC changes (ΔADC) and percentage ADC changes (Δ%ADC) of day 20 in responders were significantly higher than in non-responders for both primary lesions (p=.005, p=.006, p=.008, respectively) and metastatic lymph nodes (p=.002, p=.002, p=.003). Non-responders showed a higher rate of residual for both primary lesions (p=.008) and metastatic lymph nodes (p=.024) than responders. Conclusions DW MR imaging allows for detecting early treatment response of NPC. Patients with high ADC values and large ADC increase early after NAC initiation tended to respond better to CRT. Thus, accessing the curative effect of NAC in advanced NPC provides the opportunity to adjust following CRT regimen.

Abstract:Background and purpose:The early efficacy of definitive radiotherapy for patients with nasopharyngeal carcinoma(NPC) is conventionally evaluated by CT scan after treatment.We investigated the role of post-treatment CT scans in assessing response to definitive radiotherapy.Methods:Between March 1999 and October 2003,a total of 132 consecutive patients with newly diagnosed NPC were studied in this IRB approved protocol.Sixty-one patients with early stage disease(AJCC stage Ⅰ/Ⅱ) were treated with radiation only;71 patients with locally advanced disease(stage Ⅲ/Ⅳ) but no evidence of distant metastasis(DM) were treated with concurrent chemoradiotherapy.All patients received CT scans of the head and neck,nasopharyngoscopy,and biopsies of primary sites at 4-6 months after completion of radiotherapy.Clinical response of the primary tumor as determined by comparison of pre-and post-treatment CT scans was correlated to pathology results.Results:The median follow-up time for all patients was 25 months(range 9 to 40 months).Radiological progression was seen in 5 patients,stable disease in 18 patients,and radiographic partial(rPR) and complete responses(rCR) were seen in 67 and 42 patients respectively at 4-6 months of follow-up.Biopsies of the nasopharynx were positive in 6 patients.For patients with rCR,2 patients(4.8%) had positive biopsies.Four patients with residual disease(rPR,stable,or progressive disease) following treatment had positive biopsies.The positive and negative predictive values,sensitivity,and specificity of CT scans in evaluating the NPC response to radiotherapy were 4.4%,95.2%,66.7%,and 31.8% respectively.Conclusions:Pathologic CR for nasopharyngeal carcinoma is usually evident at 4-6 months after definitive radiotherapy;however,there is no correlation between pathologic and radiographic response.Although longer follow-up is required to define the relationship between radiographic and pathologic responses with respect to disease control,we find CT scan at 4-6 months after radiation treatment to be neither sensitive nor specific in predicting the response of primary NPC to

Abstract:We highlight the benefits of a tailored system-level modeling approach for the enhanced design of MEMS devices and systems demonstrated for an industrial capacitive silicon microphone. The performance of such microphones is determined by distributed effects like viscous damping and inhomogeneous capacitance variation across the membrane as well as by system-level phenomena like package-induced acoustical effects and the impact of the electronic circuitry for biasing and read-out. In order to meet these antipodal targets, a so-called mixed-level simulation approach is applied, which combines distributed and lumped element models on system-level. This provides maximum insight into the device and system operation while keeping the computational expense low. The presented model enables the investigation of the most relevant figures of merit such as the frequency response, the total harmonic distortion, and the signal-to-noise ratio as well as the analysis of their respective dependence on design parameters. The accuracy of the derived model and its potential for predictive simulation is assured by extensive calibration of each submodel and demonstrated through comparison to measured data.
[99Mtc]tc-Duramycin, a Potential Molecular Probe for Early Prediction of Tumor Response after Chemotherapy.

Abstract:OBJECTIVE:Apoptosis plays a crucial role in many biological processes, especially in cancer. However, real-time monitoring of apoptosis is challenging. [99mTc]duramycin can selectively target an apoptosis biomarker: phosphatidylethanolamine (PE), which is normally located on the intracellular cell-membrane surface but redistributes onto the outer cell-membrane upon apoptosis. Therefore, 99mTc-duramycin is a potential probe for non-invasive detection of apoptosis in real-time. The aim of this study was to evaluate the value of [99mTc]duramycin for detecting early apoptotic response in tumors after chemotherapy, thus providing a tool for early prediction of curative effects in tumors.METHODS:Human breast cancer MDA-MB-468 model mice, randomly divided into two groups, were injected with cisplatin or vehicle once per day. [99mTc]duramycin imaging was performed for group 1 before treatment and 24 h after the third day of treatment to evaluate treatment response through animal single-photon emission computed tomography (SPECT/CT). Mice in group 2 were treated for 10 days consecutively, to observe treatment response by tumor volume changes. Treatment response was further demonstrated through TdT-mediated dUTP nick-end labeling (TUNEL) and cleaved caspase-3 (CC3).RESULTS:[99mTc]duramycin uptake in MDA-MB-468 tumors was significantly higher in the treatment group than the control group after as few as 3 days of cisplatin treatment (p = 0.0001), and it also increased after treatment as comparison with that before treatment (p = 0.0001). Moreover, [99mTc]duramycin uptake in tumors clearly correlated with immunohistochemistry results (TUNEL: r = 0.892, p = 0.0001, and CC3: r = 0.89, p = 0.0001). Additionally, tumor size reduction, indicating effective treatment, was not observed until the eighth day after treatment, far later than the time when diagnosis could be made through [99mTc]duramycin imaging.CONCLUSIONS:[99mTc]duramycin SPECT/CT provides a non-invasive molecular imaging strategy for early detection of tumor apoptosis after chemotherapy and thus may have great potential value in the clinic.
