Abstract:Introduction Healthcare-based Internet of Things (Healthcare-IoT) is a turning point in the development of health information systems. This emerging trend significantly contributes to enhancing users' awareness of their health, ultimately leading to an extension in life expectancy. Security and privacy are among the greatest challenges for H-IoT systems. To establish complete safety and security in these systems, the implementation of mandatory security requirements is imperative. For this reason, this study identifies the necessary security requirements for H-IoT systems using a Meta-Synthesis approach. Methods Initially, following the Seven-Stage Sandelowski & Barroso approach, the existing literature was searched in the Scopus and Web of Science databases. Among the 844 extracted articles from the period of 2010 to 2020, 78 final articles were reviewed and analyzed, leading to the identification of 51 security requirements. Subsequently, to assess the quality of the identified requirements and their overlap, interviews were conducted with two experts. Results Finally, 14 security requirements, predominantly with technical and quantitative aspects, were identified for designing a Healthcare-IoT system and implementing security mechanisms. Conclusion The findings of this study emphasize that addressing the identified 14 security requirements is crucial for safeguarding Healthcare-IoT systems and ensuring their robustness in the evolving health information landscape.

Abstract:Internet of Things (IoT) applications are switching from general to precise in different industries, e.g., healthcare, automation, military, maritime, smart cities, transportation, logistics, and many more. In the healthcare domain, these applications had demonstrated an incredible improvement in patient assessment, monitoring, and prescription, etc., with ease of access through the Internet. Despite its benefits, this technology also offers several security challenges for the research community and healthcare stakeholders, because of its wireless communication and open-area deployment. To explore, patient wearable devices and other networking entities follows unstructured communication format to share their accumulated data in the network, which makes them susceptible to manifold security threats. Considering the significance of these applications, data acquisition, processing, storage, and assessment on client and remote sides need a high standard of secure communication infrastructure. Therefore, security of these applications is one of the major obstacles that prevent their widespread use in different healthcare domains. To discuss different security constraints, in this paper, we present a comprehensive survey of the theoretical literature from 2015-to-2023 to highlight the unresolved security problems of this emerging technology. Based on the evaluated literature pros and cons, we determine the security requirements and challenges of Healthcare-IoT (HC-IoT) applications. Following this, we demonstrate future research directions that could be useful for the researchers and industry stakeholders working in this domain. To demonstrate the uniqueness of this work and claim its contribution, we compare our work section-wise with previously published papers to answer the question of reviewers, editors, students, and readers, why this review article is required in the presence of already published review articles.

Identifying cloud internet of things requirements in healthcare: a Delphi-based study

Abstract:The Internet of Things (IoT) and cloud computing are emerging technologies whose use has been proven in various fields of medicine and healthcare. The positive impact of these technologies on improving the performance of medical centers is also evident. Therefore, the purpose of the present study is to investigate the technological requirements of the Cloud Internet of Things (CIoT) in the healthcare industry. This was a quantitative study conducted in two rounds using the Delphi method. A non-random, purposeful, and criterion-based method was used for sampling. A minimum of one paper or research project in the IoTs, cloud computing, or CIoT fields should be completed by participants. In addition to having a master's degree or specialized doctorate and at least 5 years of work experience, they should be faculty members. Fifteen participants in the first round of Delphi and 12 participants in the second round who completed all of the questionnaire's questions made up the research sample. A questionnaire was used to gather information. This questionnaire was designed in four main parts with 14 axes and 48 questions. After collecting the questionnaires in the first round of Delphi, each axis was scored on a five-point Likert scale. Finally, the requirements for the implementation of CIoT in healthcare were determined by the collective agreement of experts. Functional requirements of the network layer and cloud computing, platform layer requirements, and components of quality requirements were extracted. The architectures of the application layer, cloud layer, server layer, network layer, and user layer are explored thoroughly. Furthermore, communication technologies, equipment types, analysis and management, wireless networks and sensors, secure data exchange and management, deployment details, and approaches for security management and control are the main infrastructural keys that are extracted during the Delphi rounds. The findings showed that, with a very high level of importance, the use of wireless local networks was approved more than other network layer components. Compliance with security principles emerged as the most crucial component of the platform layer. We also found the use of hybrid clouds and the necessity of software as a service to be important. All the experts emphasized the need to use application development tools, cloud services, development, and deployment on the platform as a service, data analysis, and application programming interface related to the requirements of the platform layer. Scientifically speaking, they found that the components of secure information exchange, establishing secure protocol communication, key exchange mechanisms, and maintaining information confidentiality are crucial in the field of secure data exchange. Based on the findings, it can be concluded that the implementation of this technology can improve the quality of clinical services and increase the speed of responses and patient satisfaction. It is expected that by identifying the technological requirements of CIoT, a suitable model can be designed for healthcare.

Abstract:Abstract The Internet of Things (IoT) in health care is an emerging trend which provides a substantial amount of efficient and effective services for patients as well as healthcare professionals for the treatment of various diseases. Although the IoT paradigm offers numerous benefits, there are also inherent security and Privacy challenges in relation to patient data. This chapter aims to investigate such security and privacy-related issues of the IoT Healthcare applications that often operate on Cloud and Fog Computing architectures. In this chapter, 30 peer-reviewed publications from the years 2016 to 2018 are reviewed to construct a structured guide for the identification of related problems. Healthcare software applications, service architectures, patient data, and diverse sensory devices used are some of the factors considered for the collection of data, determination of related security and privacy issues, and implications of the said problems. Based on the factors, a comparison table is developed for analysis. Results show that most of these applications are used for remote patient care and monitoring. Furthermore, the results show that most threats occur due to Unauthorized access to data, Data breaches, and Impersonation. Considering the IoT healthcare applications architecture (consisting of application, communication, device, network, and Transport layer we find that the Device layer is the most affected layer regarding the security issues. Since most of the similar studies are limited to providing solutions regarding a specific aspect of security and privacy without considering the entire process, we have carried out additional research for a holistic approach to solve the bigger problem. We suggest that application developers should pay particular attention to these threats when designing IoT healthcare applications to provide secure and efficient applications to enhance the services within the healthcare sector. This study could be helpful to plan mitigation methodologies to conquer the inherent Vulnerabilities in the IoT healthcare applications.

Abstract:Twelve subjects received single 15 mg oral doses of 14C-ciramadol. Excretion of the dose occurred almost entirely by the renal route (93.5 +/- 11.7 (S.D.)% of the dose), and only 0.7 +/- 0.6% of the dose was recovered in faeces indicating that absorption was essentially complete. More than 90% of the amount recovered in urine was excreted within 24 h after dosing. Unchanged drug accounted for 43.9 +/- 6.5% of the dose, while a phenolic glucuronide conjugate was the only major urinary metabolite accounting for a further 37.9 +/- 7.8%. A second glucuronide that was conjugated with the alicyclic ring was also identified but constituted only 2.3 +/- 0.6% of the dose. Concentrations of radioactivity in plasma reached a peak at 2 h after dosing and declined with a terminal disposition half life of 4.9 h. Only ciramadol and the aryl-O-glucuronide were detected in substantial amounts in plasma. Renal clearance of ciramadol amounted to 298 +/- 54 ml/min suggesting tubular secretion in addition to glomerular filtration.

Abstract:The Internet of Things (IoT) has experienced constant growth in the number of devices deployed and the range of applications in which such devices are used. They vary widely in size, computational power, capacity storage, and energy. The explosive growth and integration of IoT in different domains and areas of our daily lives has created an Internet of Vulnerabilities (IoV). In the rush to build and implement IoT devices, security and privacy have not been adequately addressed. IoT devices, many of which are highly constrained, are vulnerable to cyber attacks, which threaten the security and privacy of users and systems. This survey provides a comprehensive overview of IoT in regard to areas of application, security architecture frameworks, recent security and privacy issues in IoT, as well as a review of recent similar studies on IoT security and privacy. In addition, the paper presents a comprehensive taxonomy of attacks on IoT based on the three-layer architecture model; perception, network, and application layers, as well as a suggestion of the impact of these attacks on CIA objectives in representative devices, are presented. Moreover, the study proposes mitigations and countermeasures, taking a multi-faceted approach rather than a per layer approach. Open research areas are also covered to provide researchers with the most recent research urgent questions in regard to securing IoT ecosystem.

Abstract:Internet of Things (IoT) is a challenging and promising system concept and requires new types of architectures and protocols compared to traditional networks. Security is an extremely critical issue for IoT that needs to be addressed efficiently. Heterogeneity being an inherent characteristic of IoT gives rise to many security issues that need to be addressed from the perspective of new architectures such as software defined networking, cryptographic algorithms, federated cloud and edge computing. The paper analyzes the IoT security from three perspectives: three-layer security architecture, security issues at each layer and security countermeasures. The paper reviews the current state of the art, protocols and technologies used at each layer of security architecture. The paper focuses on various types of attacks that occur at each layer and provides the various approaches used to countermeasure such type of attacks. The data exchanged between the different devices or applications in the IoT environment are quite sensitive; thus, the security aspect plays a key role and needs to be addressed efficiently. This indicates the urgent needs of developing general security policy and standards for IoT products. The efficient security architecture needs to be imposed but not at the cost of efficiency and scalability. The paper provides empirical insights about how the different security threats at each layer can be mitigated. The paper fulfills the need of having an extensive and elaborated survey in the field of IoT security, along with suggesting the countermeasures to mitigate the threats occurring at each level of IoT protocol stack.

Abstract:The fast development of the Internet of Things (IoT) technology in recent years has supported connections of numerous smart things along with sensors and established seamless data exchange between them, so it leads to a stringy requirement for data analysis and data storage platform such as cloud computing and fog computing. Healthcare is one of the application domains in IoT that draws enormous interest from industry, the research community, and the public sector. The development of IoT and cloud computing is improving patient safety, staff satisfaction, and operational efficiency in the medical industry. This survey is conducted to analyze the latest IoT components, applications, and market trends of IoT in healthcare, as well as study current development in IoT and cloud computing-based healthcare applications since 2015. We also consider how promising technologies such as cloud computing, ambient assisted living, big data, and wearables are being applied in the healthcare industry and discover various IoT, e-health regulations and policies worldwide to determine how they assist the sustainable development of IoT and cloud computing in the healthcare industry. Moreover, an in-depth review of IoT privacy and security issues, including potential threats, attack types, and security setups from a healthcare viewpoint is conducted. Finally, this paper analyzes previous well-known security models to deal with security risks and provides trends, highlighted opportunities, and challenges for the IoT-based healthcare future development.

Abstract:The Internet of Things (IoT) has been identified in various applications across different domains, such as in the healthcare sector. IoT has also been recognised for its revolution in reshaping modern healthcare with aspiring wide range prospects, including economical, technological and social. This study aims to establish IoT-based smart home security solutions for real-time health monitoring technologies in telemedicine architecture. A multilayer taxonomy is driven and conducted in this study. In the first layer, a comprehensive analysis on telemedicine, which focuses on the client and server sides, shows that other studies associated with IoT-based smart home applications have several limitations that remain unaddressed. Particularly, remote patient monitoring in healthcare applications presents various facilities and benefits by adopting IoT-based smart home technologies without compromising the security requirements and potentially large number of risks. An extensive search is conducted to identify articles that handle these issues, related applications are comprehensively reviewed and a coherent taxonomy for these articles is established. A total number of (n = 3064) are gathered between 2007 and 2017 for most reliable databases, such as ScienceDirect, Web of Science and Institute of Electrical and Electronic Engineer Xplore databases. Then, the articles based on IoT studies that are associated with telemedicine applications are filtered. Nine articles are selected and classified into two categories. The first category, which accounts for 22.22% (n = 2/9), includes surveys on telemedicine articles and their applications. The second category, which accounts for 77.78% (n = 7/9), includes articles on the client and server sides of telemedicine architecture. The collected studies reveal the essential requirement in constructing another taxonomy layer and review IoT-based smart home security studies. Therefore, IoT-based smart home security features are introduced and analysed in the second layer. The security of smart home design based on IoT applications is an aspect that represents a crucial matter for general occupants of smart homes, in which studies are required to provide a better solution with patient security, privacy protection and security of users' entities from being stolen or compromised. Innovative technologies have dispersed limitations related to this matter. The existing gaps and trends in this area should be investigated to provide valuable visions for technical environments and researchers. Thus, 67 articles are obtained in the second layer of our taxonomy and are classified into six categories. In the first category, 25.37% (n = 17/67) of the articles focus on architecture design. In the second category, 17.91% (n = 12/67) includes security analysis articles that investigate the research status in the security area of IoT-based smart home applications. In the third category, 10.44% (n = 7/67) includes articles about security schemes. In the fourth category, 17.91% (n = 12/67) comprises security examination. In the fifth category, 13.43% (n = 9/67) analyses security protocols. In the final category, 14.92% (n = 10/67) analyses the security framework. Then, the identified basic characteristics of this emerging field are presented and provided in the following aspects. Open challenges experienced on the development of IoT-based smart home security are addressed to be adopted fully in telemedicine applications. Then, the requirements are provided to increase researcher's interest in this study area. On this basis, a number of recommendations for different parties are described to provide insights on the next steps that should be considered to enhance the security of smart homes based on IoT. A map matching for both taxonomies is developed in this study to determine the novel risks and benefits of IoT-based smart home security for real-time remote health monitoring within client and server sides in telemedicine applications.

Abstract:The concept of the Internet of Things (IoT) spans decades, and the same can be said for its inclusion in healthcare. The IoT is an attractive target in medicine; it offers considerable potential in expanding care. However, the application of the IoT in healthcare is fraught with an array of challenges, and also, through it, numerous vulnerabilities that translate to wider attack surfaces and deeper degrees of damage possible to both consumers and their confidence within health systems, as a result of patient-specific data being available to access. Further, when IoT health devices (IoTHDs) are developed, a diverse range of attacks are possible. To understand the risks in this new landscape, it is important to understand the architecture of IoTHDs, operations, and the social dynamics that may govern their interactions. This paper aims to document and create a map regarding IoTHDs, lay the groundwork for better understanding security risks in emerging IoTHD modalities through a multi-layer approach, and suggest means for improved governance and interaction. We also discuss technological innovations expected to set the stage for novel exploits leading into the middle and latter parts of the 21st century.

Abstract:The healthcare infrastructure requires robust security procedures, technologies, and policies due to its critical nature. Since the Internet of Things (IoT) with its diverse technologies has become an integral component of future healthcare systems, its security requires a thorough analysis due to its inherent security limitations that arise from resource constraints. Existing communication technologies used for IoT connectivity, such as 5G, provide communications security with the underlying communication infrastructure to a certain level. However, the evolving healthcare paradigm requires adaptive security procedures and technologies that can adapt to the varying resource constraints of IoT devices. This need for adaptive security is particularly pronounced when considering components outside the security sandbox of 5G, such as IoT nodes and M2M connections, which introduce additional security challenges. This article brings forth the unique healthcare monitoring requirements and studies the existing encryption-based security approaches to provide the necessary security. Furthermore, this research introduces a novel approach to optimizing security and performance in IoT in healthcare, particularly in critical use cases such as remote patient monitoring. Finally, the results from the practical implementation demonstrate a marked improvement in the system performance.

Abstract:The design and development process for the Internet of Things (IoT) applications is more complicated than that for desktop, mobile, or web applications. First, IoT applications require both software and hardware to work together across different nodes with different capabilities under different conditions. Secondly, IoT application development involves different software engineers such as desktop, web, embedded and mobile to cooperate. In addition, the development process required different software\hardware stacks to integrated together. Due to above complexities, more often non-functional requirements (such as security and privacy) tend to get ignored in IoT application development process. In this paper, we have reviewed techniques, methods and tools that are being developed to support incorporating security and privacy requirements into traditional application designs. By doing so, we aim to explore how those techniques could be applicable to the IoT domain. In this paper, we primarily focused on two different aspects: (1) design notations, models, and languages that facilitate capturing non-functional requirements (i.e., security and privacy), and (2) proactive and reactive interaction techniques that can be used to support and augment the IoT application design process. Our goal is not only to analyse past research work but also to discuss their applicability towards the IoT.

Abstract:The rapid development and the expansion of Internet of Things (IoT)-powered technologies have strengthened the way we live and the quality of our lives in many ways by combining Internet and communication technologies through its ubiquitous nature. As a novel technological paradigm, this IoT is being served in many application domains including healthcare, surveillance, manufacturing, industrial automation, smart homes, the military, etc. Medical Internet of Things (MIoT), or the use of IoT in healthcare, is becoming a booming trend towards improving the health and wellbeing of billions of people by offering smooth and seamless medical facilities and by enhancing the services provided by medical practitioners, nurses, pharmaceutical companies, and other related government and non-government organizations. In recent times, this MIoT has gained higher attention for its potential to alleviate the massive burden on global healthcare, which has been caused by the rise of chronic diseases, the aging population, and emergency situations such as the recent COVID-19 global pandemic, where many government and non-government medical resources were challenged, owing to the rising demand for medical resources. It is evident that with this recent growing demand for MIoT, the associated technologies and its interconnected, heterogeneous nature adds new concerns as it becomes accessible to confidential patient data, often without patient or the medical staff consciousness, as the security and privacy of MIoT devices and technologies are often overlooked and undermined by relevant stakeholders. Hence, the growing security breaches that target the MIoT in healthcare are making the security and privacy of Medical IoT a crucial topic that is worth scrutinizing. In this study, we examined the current state of security and privacy of the MIoT, which has become of utmost concern among many security experts and researchers due to its rapid demand in recent times. Nevertheless, pertaining to the current state of security and privacy, we also examine and discuss a number of attack use cases, countermeasures and solutions, recent challenges, and anticipated future directions where further attention is required through this study.

Furthermore, the paper thoroughly analyzes the specific IoT security and privacy features required for smart healthcare, including security requirements, privacy and security, attack taxonomies, and threat models from a healthcare perspective. Thanks to this technology-based treatment approach, there is an unprecedented opportunity to better the quality and productivity of treatments and better the patient's well-being and better government funding.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>: In this paper, we provide a comprehensive overview of the primary uses of IoT in healthcare. We used the Systematic Literature Review (SLR) method to analyze and comparison articles published in this field between 2015 and March 2020.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>: A comprehensive taxonomy is presented based on the contents of the articles under study. In this article, a brief overview of selected articles based on research questions is given and highlights the most critical challenges and case studies for the future use of IoT in healthcare.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>: According to a detailed study of the 89 articles and a glimpse into about 208 articles, challenges and future trends in healthcare have been identified.</p>

Abstract:The Internet of Medical Things (IoMT), an application of the Internet of Things (IoT) in the medical domain, allows data to be transmitted across communication networks. In particular, IoMT can help improve the quality of life of citizens and older people by monitoring and managing the body's vital signs, including blood pressure, temperature, heart rate, and others. Since IoMT has become the main platform for information exchange and making high-level decisions, it is necessary to guarantee its reliability and security. The growth of IoMT in recent decades has attracted the interest of many experts. This study provides an in-depth analysis of IoT and IoMT by focusing on security concerns from different points of view, making this comprehensive survey unique compared to other existing studies. A total of 187 articles from 2010 to 2022 are collected and categorized according to the type of applications, year of publications, variety of applications, and other novel perspectives. We compare the current studies based on the above criteria and provide a comprehensive analysis to pave the way for researchers working in this area. In addition, we highlight the trends and future work. We have found that blockchain, as a key technology, has solved many problems of security, authentication, and maintenance of IoT systems due to the decentralized nature of the blockchain. In the current study, this technology is examined from the application fields' points of view, especially in the health sector, due to its additional importance compared to other fields.

Abstract:Internet of Things (IoT) is predicted as one of the biggest emerging environments in the future that creates simultaneous smart communication between machines or a variety of digital devices. Besides improving how data can be controlled, monitored and collected, IoT also allows us to generate revenue through the identifying of new business opportunities and deployment of advanced analytics processes for decision-making purposes. IoT enables us to accelerate changes in the healthcare environment in improving patient engagement and outcome, as well as transforming healthcare from reactive to proactive accessibility. Nevertheless, IoT’s expansion brings new vulnerabilities, risk and security challenges for healthcare practitioners and their patients. However, there is still a lack of study focusing on IoT risk management in healthcare. Existing researches tend to focus only on the implementation of IoT peripherals in a healthcare environment and tend to embed the secured applications solution with it. Since healthcare information and data are highly confidential, it is important to ensure that a secured health IoT application is in place. Thus, the aim of this study is to investigate the IoT risk management aspects in a healthcare environment with particular attention to proposing a step-by-step process of an IoT risk management model. The proposed IoT risk management model was developed leveraging on DEMATEL IoT Risk Assessment Procedure. This study was conducted based on a case study of one hospital in Sudan which has recently invested in IoT technologies in their operation. Interviews were conducted with selected respondents from the hospital and findings suggested that the selected case study does not have an established IoT risk management mechanism due to the ad-hoc IoT implementation approach. The case study also lacks of protection for health data and information with several unclear work process. As a solution, this study proposes an enhanced IoT risk management model for healthcare with consideration of three risk categories; 1) Secured Technology, 2) Human Privacy and 3) Trustable Process and Data. The proposed model was then evaluated by three IoT experts and two IT healthcare practitioners based on System Usability Score (SUS) and received Good Usability score which means the model is usable as a healthcare IoT risk management model.

Abstract:The Internet of Things (IoT) has not been around for very long. However, since the notion of IoT introduced, most of IoT studies focused on a strategic level such as planning, architectures, standardization, and latest technologies, however, studies of risk management plan of IoT are still lacking. IoT has been widely used to link existing medical resources and provide reliable, effective and smart healthcare services to elderly and patients with chronic illnesses. However, a systematic process is missing when managing and anticipating the risk of IoT usage in healthcare. For this purpose, this paper extensively explores various IoT technologies used in health care services and its security challenges. As a result, IoT Security Risk Model for Healthcare is introduced to cater a complete process of risk management based on ISO/IEC 27005:2018 standard. It is believed that by having this model, it will emphasize on iterative IoT risk management process as it may increase the depth and detail of the assessment at each iteration.

Abstract:The internet of things (IoT) refers to a framework of interrelated, web associated objects that can gather and move information over a remote network without human interference. With a quick development in the arrangement of IoT gadgets and expanding want to make medical care more financially savvy, customized, and proactive, IoT is ready to assume a solid function in all perspectives of the healthcare industry. In this context, IoT-based healthcare provides several benefits such as instant and reliable treatment, cost reduction, improved communication, etc. by using different new technologies. Wireless Body Area Networks (WBAN) technologies can enhance the quality of data gathering and data transferring in smart systems. Machine Learning(ML) are put to use at every level of smart healthcare systems. Fog computing reduces communication cost and provides low latency. Software-Defined Networking (SDN) and Network Function Virtualization (NFV) technologies provide less complex and more flexible network structures. Blockchain technology offers a better way of protection of users' sensitive information. This paper aims to provide comprehensive descriptions of ongoing research projects and the utilization of the above-mentioned technologies in smart healthcare systems. In this paper, the latest studies, proposed techniques, and the current solutions of smart healthcare systems are elaborated in the context of emerging technologies, applications and challenges of these systems to provide a better understanding of what IoT means in the healthcare industry now and what it will mean in the future.
