Abstract:Iodine thyroid blocking (ITB) is effective for preventing childhood thyroid cancer when radioactive iodine is released into the environment during a nuclear power plant accident. Japan employs the pre-distribution of stable iodine (PDSI) to residents living near nuclear power plants; however, the number of residents who have actually received stable iodine to date remains limited. The aim of this study was to evaluate the profile of guardians of children living around the Genkai Nuclear Power Plant (GNPP) in Japan. We distributed self-administered questionnaires regarding perception of risks associated with administration of stable iodide to approximated 400 guardians of children aged 0–6 in 10 kindergartens located in four municipalities. We obtained responses from 286 guardians, and after excluding invalid responses, 247 were included in the analysis. Logistic regression analysis revealed that living within 5 km of the GNPP (odds ratio [OR] = 4.48, 95% confidence interval [CI]: 2.43–8.24), awareness of preferential implementation of ITB to children (OR = 3.33, 95%CI: 1.78–6.22), and awareness of the prophylaxis booklet published by the local government (OR = 2.53, 95%CI: 1.37–4.68) were independently associated with PDSI for children. The main reasons for not receiving PDSI were “anxiety about the side effects of stable iodine” (40.2%), “distrust of the effectiveness of SI” (23.5%), “complicated procedures for receiving stable iodine” (15.7%) and “missed the date for receiving stable iodine” (8.8%). In the case of ITB implementation during a nuclear emergency, it is necessary to clarify the risk perceptions of guardians and adapt risk communication accordingly.

Abstract:Splenic T cells from BALB/c mice bearing mammary adenocarcinomas initially demonstrate a primed response to tumor-associated Ags (TAA), which declines to presensitization levels within 4 wk after tumor implantation. Associated with this decline in responses to TAA is the expansion of a subpopulation of Mac-1+ 2+ splenic macrophages (M phi). These Mac-1+ 2+ cells present TAA inefficiently to normal T cells primed to TAA by footpad injection, as compared with the Ag presenting ability of M phi from normal mice. The addition of anti-I-Ed, but not anti-I-Ad, Ab blocked the ability of Ag-pulsed Mac-1+ 2+ cells to present TAA to primed T cells. The converse was observed with macrophages from normal mice. However, presentation of human gamma-globulin or OVA was restricted by I-Ad molecules when APC from normal mice or tumor bearers were used, although less efficiently in the latter. Using cell depletion techniques, it was determined that the I-Ed-restricted presentation preferentially expanded CD8+ T cells, and not CD4+ cells, as was the case for I-Ad-restricted normal macrophages. These CD8+ cells were poor effectors of cytotoxicity against tumor cells; instead they down-regulated the proliferative activity of T cells. Limiting dilution assays indicated that Mac-1+ 2+ macrophages preferentially present TAA to a low frequency inhibitory T cell population that expanded and inhibited further responses to TAA. Thus, alterations of Ag presentation in tumor bearers may help the tumor to subvert potential beneficial host responses and allow the progression of the neoplastic process.
Exploring a model nuclear planning and response program: Evaluating public awareness of written risk and emergency

Abstract:High-reliability organizations (HROs) including commercial airlines, the NASA Space Shuttle engineering team, US Naval aircraft carrier and nuclear submarine crews, and US nuclear power plants are relatively safe. However, these organizations experience system breakdowns often with catastrophic outcomes. This study focuses on risk information management strategies employed by a nuclear energy power plant located within 10 miles of a population center. The evacuation planning zone (EPZ) includes a hospital, several schools, and a public university. The nuclear plant provides written preparedness and evacuation information for all residents within the EPZ in the event of a radiological emergency. Focusing on the campus community within the EPZ, this study investigates individual awareness regarding the potential of a radiological event, the emergency information booklet, and the information provided within the booklet. We use descriptive statistics, frequency distribution, and cross tabulations (contingency tables) to establish awareness levels. Our study determines those participants who read the emergency instructions booklet are prepared to make an informed decision in the event of a radiological incident. We find college affiliation, educational level, university role, and age significantly related to emergency instruction booklet utilization. We also find gender is significantly linked to overall risk perception regarding a radiological event. Findings support previous research regarding women's higher levels of risk aversion and pessimism involving dangerous new risky technologies and activities like nuclear energy plants. These findings support modifying policy to ensure nuclear facilities assess the efficacy of their warning systems in alerting the public. Furthermore, our findings provide guidance regarding the evaluation of the effectiveness of emergency instruction booklet distribution.

Abstract:The better knowledge of the mechanisms of nuclear incidents and lessons learned from accidents in the recent past to improve the effectiveness of measures taken following a nuclear accident exposure to fallout of radioactive iodine isotopes. Thus, immediate, passive measures, such as containment, and stopping consumption of contaminated products are paramount. The earliest possible administration of stable iodine as potassium iodide (KI) reduces significantly (up to 90% if taken at the same time of the accident) thyroid radioactive contamination. These tablets should be given in priority to children and pregnant women. The side effects are minor. KI is not recommended for persons aged over 60 years, or for adults suffering from cardiovascular disorders.

Abstract:AIMS: The aims of this paper were to determine the level of knowledge of and attitude to nuclear power among residents around Tianwan Nuclear power plant in Jiangsu of China.DESIGN: A descriptive, cross-sectional design was adopted.PARTICIPANTS: 1,616 eligible participants who lived around the Tianwan nuclear power plant within a radius of 30km and at least 18 years old were recruited into our study and accepted epidemiological survey.METHODS: Data were collected through self-administered questionnaires consisting of a socio-demographic sheet. Inferential statistics, t-test, ANOVA test and multivariate regression analysis were used to compare the differences between each subgroup and correlation analysis was conducted to understand the relationship between different factors and dependent variables.RESULTS: Our investigation found that the level of awareness and acceptance of nuclear power was generally not high. Respondents' gender, age, marital status, residence, educational level, family income and the distance away from the nuclear power plant are important effect factors to the knowledge of and attitude to nuclear power.CONCLUSIONS: The public concerns about nuclear energy's impact are widespread. The level of awareness and acceptance of nuclear power needs to be improved urgently.

Iodine Prophylaxis in the Case of Nuclear Accident

Abstract:Background: On 26th April, 1986 the greatest accident of nuclear plant in Czernobyl occured and isotopes with high percentage of release were erupted: 33-Xe, 131-I, 132-Te, 134-Cs and 137-Cs. The radioactivity of the isotopes was very high - for instance: 33-Xe 6500 PBq, 131-I 1760 PBq. Rest of the 15 isotopes represented similar radioactivity with shorter percentage of release. The most exposed group of people were 237 liquidators, and 11600 people living around had to be evacuated when the limit dose for a person (5mSv) was crossed. Ionizing radiation on the molecular level produces high energy radicals, water radiolysis and ionization of the atoms leading to damage of the enzymes activity centers and receptors, cell membranes DNA, intracellular lysosomes, and especially important for ATP synthesis - mitochondria. These destructions lead to tissue and organs damage. Objective: The aim of this article is the presentation of the protective property of iodine application in the case of nuclear accident. Methods: In Poland at that time, effective iodine prophylaxis did not exist. In the face of such exposition, a special Governement Commission was appointed. When permissioned maximal dose for children and adolescents 50mSv appeared in some areas of the country, the Commission made a decision about obligatory administration of a single pharmacological dose of potassium iodide for all children and adolescents till age 16. No relevant recent patents were available for this WHO report. Results: In this way, the dose of 131-I to thyroid for inhabitants in high, moderated, and low contaminated regions were reduced by about 45%. However, from 1987 to 1997 increase in prevalence of the differentiated thyroid cancer in adults in Polish population especially in women over 40 years old in the southern part of Poland was observed. Conclusion: Actually in European countries work 185 nuclear power plants and to 2045 another 100 is planned. In 1999, WHO issued recommendations on iodine prophylaxis in the case of nuclear accident.

Abstract:Abstract With its recommendation “Iodine prophylaxis following nuclear accidents” (1996) and its reports of 1997 and 2001 the German Commission on Radiological Protection (SSK) followed the recommendations of the WHO “Guidelines for iodine prophylaxis following nuclear accidents” of 1989. The intervention levels were lowered (50 mSv for children/adolescents (up to the age of 18 years) and pregnant women, 250 mSv for adults), the iodine prophylaxis was restricted to persons up to the age of 45 years and the recommended dosage of stable iodine was changed. Due to the lowered reference levels the radius of 25 km around a nuclear power plant that had been the planning radius for the distribution of iodine tablets so far was extended to 100 km. Based on these recommendations the German authorities began to set up new strategies for the provision and distribution of potassium-iodide tablets (iodine tablets). Since 2004, within the radius of 25 km the iodine tablets are pre-distributed to households and/or stored at several points in the municipality for persons up to the age of 45 years. For the new planning radius of 25 – 100 km iodine tablets are stored in 8 central stocks in Germany for children/adolescents (up to the age of 18 years) and pregnant women. A working group with representatives from federal and Länder authorities has developed a distribution strategy for the distribution out of these central stocks in the event of an accident. It describes a possibility of organising and implementing the distribution of the iodine tablets within the radius of 25 – 100 km in a nationwide standardised way.

Abstract:Objectives The number of healthcare workers occupationally exposed to ionizing radiation (IR) is increasing every year. As health effects from exposure to low doses IR have been reported, radiation protection (RP) in the context of occupational activities is a major concern. This study aims to assess the compliance of healthcare workers with RP policies, according to their registered cumulative dose, profession, and perception of radiation self-exposure and associated risk. Methods Every medical worker from one of the participating hospitals in France with at least one dosimetric record for each year 2009, 2014, and 2019 in the SISERI registry was included and invited to complete an online questionnaire including information on the worker's occupational exposure, perception of IR-exposure risk and RP general knowledge. Hp(10) doses were provided by the SISERI system. Multivariate logistic regressions were used. Results Dosimeter wearing and RP practices compliance were strongly associated with "feeling of being IR-exposed" (OR=3.69, CI95% 2.04-6.66; OR=4.60, CI95% 2.28-9.30, respectively). However, none of these factors was associated with RP training courses attendance. The main reason given for non-compliance is unsuitability or insufficient numbers of RP devices. Conclusions This study provided useful information for RP policies. Making exposed workers aware of their own IR-exposure seems to be a key element to address in RP training courses. This type of questionnaire should be introduced into larger epidemiological studies. Advances in knowledge Dosimeter wearing and RP practices compliance are associated to feeling beeing IR-exposed. RP training courses should reinforce workers' awareness of their exposure to IR.

Abstract:Public knowledge and awareness about radiation (both natural and manmade) tend to be low, while perceived risk of radiation tends to be high. High perceptions of risk associated with radiation have been linked to the affect heuristic and general feelings of dread, which are often not proportionate to actual risk. For example, studies routinely show that members of the public rate the risk of radiation from nuclear power plants as significantly higher (and riskier) than radiation from medical X‐rays. This disconnect can have implications for adoption of protective actions during a potential radiation emergency and the perceived efficacy of these actions. This study explores how risk communication efforts influence public risk perceptions, intended protective action, and perceived efficacy of those actions. Using unique data from a survey of New York City adults, we analyze how information provision using different formats—no information, an infographic, an informational video—impact perceptions and response to a hypothetical radiation emergency. We hypothesize that respondents who receive some information, either through the infographic or the video, will have higher perceived efficacy and are more likely to take protective action. Findings suggest that providing information about what to do during a radiation emergency has a statistically significant impact on both perceived efficacy and adoption of protective action. Respondents who saw the informational video were most likely to say that they would take the correct protective actions and had the highest perceived efficacy, followed by those who saw the infographic.

Rationale and Objective. To evaluate the contrast kinetics of gadobutrol for interstitial MR lymphography. Materials and Methods. In 11 rats, 0.5 mL undiluted gadobutrol was injected subcutaneously into the hind paw. Contrast kinetics were measured in lymph nodes, kidney, liver, muscle, and blood of six animals using a time-resolved 2D GRE sequence. Additionally, high-resolution 3D T1-weighted data sets were obtained in five animals. Results. Immediately after injection, a pronounced signal intensity loss was observed in popliteal, inguinal and aorto-iliac lymph nodes, followed by a continuous signal intensity increase. From the data peak concentrations of up to 78 mmol/L were estimated for selected lymph nodes. A contrast enhancement was also observed in kidneys, liver, muscle, and blood. Regional lymphatic vessels, the thoracic duct, as well as popliteal, inguinal, aorto-iliac, and axillary lymph node groups could be visualized with the high-resolution 3D MRI. Conclusion. Gadobutrol is suitable for interstitial MR lymphography, as it rapidly appears in the lymphatic system. Based on estimates of local tissue concentrations future studies have to assess the optimal contrast agent dosage. Furthermore, the investigation of metastatic lymph nodes is required to evaluate the further potential of gadobutrol for interstitial MR lymphography.

Abstract:Prophylaxis by “KI 65 mg, breakable tablet” is a pharmacological countermeasure of radiological protection adapted for the prevention of thyroid cancer after exposure to radioactive iodine. A first modification of the marketing authorization of “KI 65 mg, breakable tablet” for repeated prophylaxis for adults and children above 12 years old was obtained in France in 2020. At this point in time it is recommended to exclude the repeated intake of “KI- 65 mg, breakable tablet” by pregnant and breastfeeding women and children below 12 years old, who should consequently be subjected to a priority evacuation from the contaminated zone. Regulatory toxicology studies are underway in order to support an upcoming proposal of modification of the marketing authorization of “KI- 65 mg, breakable tablet” for repeated prophylaxis for pregnant women and children below 12 years old.

Abstract:To better understand individuals' behaviour in the face of a natural disaster, and to evaluate the preventive information provided by French local authorities, the theatrical performance "Safe and Sound?" ("Sain et Sauf ?"), and its accompanying debriefing session, was tested in six French Alps municipalities in 2019 and 2020 several 2-day sessions in collaboration with local authorities. 200 people took part in this theatrical performance and interviews. The "Safe and Sound?" scheme is based on a 15-minute play, followed by the collection of data from watching the play and debriefing with an interviewer, during about 30 minutes. Two scenarios on flood risks and on seismic risks were narrated by an actor introducing dilemma. Interviews were transcribed and analysed to determine the level and sources of knowledge about the two natural hazards, and the behaviour during a simulation with an actor. Most participants immediately headed to safety only to later put themselves in danger when confronted with an insistent neighbour and the introduction of emotion-provoking characters, i.e. children. When making their choice at each stimulus, participants evoke different sources of reason. A little under half of participants mention common sense or instinct. Although the majority of participants claim they received preventive information from local authorities, this information is rarely applied. The media and own experience were mentioned more often. Lastly, the "Safe and Sound?" campaign was designed to assess behaviour, but findings show that it was also useful to raise awareness among the population.

Abstract:Digital tools are increasingly used for health promotion, but their utility during recovery from a nuclear disaster has yet to be established. This study analysed differences in knowledge, attitude, and practice (KAP) toward digital tools for radiation protection and health promotion, and preferences for specific application functions, among cohorts living within and outside areas affected by the Fukushima Daiichi nuclear power station (FDNPS) accident. A needs assessment was conducted by internet survey, and responses from those affected (N = 86) and not affected (N = 253) were compared and quantified by an adjusted odds ratio (aOR), using logistic regression analyses. KAP toward the radiation-related application in the affected group had an aOR of 1.95 (95% confidence interval (CI) = 1.12–3.38) for knowledge, and 5.71 (CI = 2.55–12.8) for practice. Conversely, toward the health-related application, the aOR of the affected group was 0.50 (CI = 0.29–0.86). The preference in the affected group was significantly lower for two application functions related to radiation measurement and two health-related functions (one about the effects of radiation in general and another about personal health advice in general): aOR range 0.43–0.50. Development of specific applications incorporating the findings from this survey was intended to foster a locally appropriate eHealth environment during recovery from the FDNPS accident.

Abstract:Introduction: The ever-evolving applications of radiology-radiotherapy, apart from the solutions offered in daily diagnostic and therapeutic problems, expose healthcare workers to the risk of irradiation and its consequences.Aim: The aim of the present study was to evaluate nurses’ knowledge on the risk of radiation during the daily work and radiation protection instructions to be followed during their participation in diagnostic and therapeutic procedures involving radiation use.Method and Material: The studied sample consisted of 110 nurses working in Public General Hospital of Athens in clinical departments and units so as in laboratories using mainly radiation such as the CT scanner and the catheterization laboratory. For data collection, a special designed questionnaire with questions that reach key areas of the applied radiation protection policy of this hospital was used. Data analysis was performed with the statistical package Statistical Package for Social Sciences (SPSS).Results: The majority (87.3%) of the participants were women, with an average age of 39.66 years (± 7.33 years). The best part of the nurses who participated in the study (82.7%) has not attended any training program concerning radiation protection. Forty percent (40%) of the participants say they do not know the policy of their hospital on issues of radiation protection, while 45.5% did not know whether their working section met the prescribed radiation safety criteria. Participants working in radiology sector had significantly higher degree of agreement with the proposals concerning the applied radiation protection policy compared with participants who worked either in pathological and surgical field or in the operating room and the ICU (p = 0.002).Conclusion: All health professionals (doctors, nurses, technicians), should be constantly informed of all the rules related to both personal protective equipment and protective measures of the patient and the environment against radiation, and the rules applied for the proper use of the appropriate equipment.
Recall of residential history and dietary habits during pregnancy and lactation in the distant past: reliability of questionnaire-based radiation doses for persons exposed in utero and early life

Abstract:This study evaluates the reliability of information obtained by standardized questionnaires used in by personal interviews for estimation of radiation thyroid doses of 1065 individuals in the Belarusian cohort of individuals who were exposed in utero and early life following the Chernobyl accident in April 1986. Data from two interviews conducted in 2012-2017 and in 2018-2022 with mothers, who were pregnant or gave birth shortly after the Chernobyl accident, were analysed. The most reliable answers dealt with various attributes related to residential history. In contrast, the reliability of answers regarding consumption rates of milk from privately owned cows or trade network was moderate, while the agreement in responses for consumption of milk products and leafy vegetables was fair. Information from the two interviews was used to calculate thyroid doses received by the cohort members. Specifically, 'model-based' thyroid doses due to 131I were estimated using input data on individual residential history and food consumption reported during the personal interviews and ecological data (131I ground deposition in the corresponding settlements). In addition, for a subset of cohort subjects (n = 205) whose mothers were measured for 131I thyroid activity, 'measurement-based' thyroid doses were calculated by adjusting the model-based dose using a scaling factor that is defined as the ratio of measured 131I thyroid activity to model-based 131I thyroid activity calculated for the date of measurement. A moderate agreement was observed for total (prenatal and postnatal) model-based thyroid doses due to 131I intake, the arithmetic mean ± standard deviation for the Jaccard similarity coefficient ([Formula: see text]) was 0.45 ± 0.34 (median = 0.39), while measurement-based doses showed a much better agreement with a [Formula: see text] of 0.78 ± 0.29 (median = 0.93). For model-based thyroid doses from external irradiation and from ingestion of 134Cs and 137Cs, [Formula: see text] was 0.82 ± 0.23 (median = 0.90) and 0.84 ± 0.24 (median = 0.96), respectively. Measurement-based doses due to ingestion of radiocaesium isotopes resulted in an almost perfect agreement, [Formula: see text] was 0.91 ± 0.19 (median = 1.0). The present findings suggest that long-term memory recall can be reliable, if a person is asked about unique or important life events, such as pregnancy and childbirth occurring around the time of a nuclear reactor accident. However, the substantial difference (more than 10 times) observed for model-bases doses calculated using the two questionnaires represents an important source of human factor uncertainties that needs to be considered in any dose response analyses. Other lessons learned from this study are that (i) individual measurements of radionuclides in the human body are the most valuable source of information for estimating radiation doses, and (ii) whenever a radiation accident occurs, a sample of affected people should be asked to keep a diary, if at all possible.
The Analysis with Quantitative Indexes for Public's Awareness of Radiation Knowledge in Taiwan

Abstract:(1) Background: The purpose of this study was to evaluate the radiation awareness level of the public in Taiwan. (2) Methods: This study designed an online survey form to investigate the radiation awareness level with six topics: basic knowledge of radiation, environmental radiation, medical radiation, radiation protection, and university/corporate social responsibility. The score of respondents were converted into knowledge and responsibility indexes for the quantitative evaluation. Logistic regression was used to assess the correlation between the knowledge index and individual factors. Paired t-test was used to assess the significant difference in knowledge index between pre-training and post-training. (3) Results: The knowledge index of each job category reflected the proportion of radiation awareness of the job. The logistic regression result indicated that radiation-related people could get higher knowledge index. The paired t-test indicated that the knowledge index before and after class had significant differences in all question topics. (4) Conclusions: The public's awareness of medical radiation was the topic that needed to be strengthened the most-the responses with high knowledge index significantly correlated with their experience in radiation education training or radiation-related jobs. It significantly increased the knowledge index of radiation if the public received radiation education training.

Abstract:When considering disaster preparedness, one challenge is mitigating the health impacts of evacuations. Nuclear disaster preparedness has evolved based on past experiences from numerous disasters, including the Fukushima Daiichi Nuclear Power Plant (FDNPP) accident. However, there is a lack of comprehensive reporting on the awareness of administrative staff, medical personnel, and residents in the areas surrounding nuclear power plants (NPPs). This study reports on a survey aimed at gaining insights into the understanding and current state of disaster preparedness and elucidating the differences in perceptions of nuclear disaster preparedness among the relevant stakeholders surrounding NPPs. Interview surveys were conducted from 14 to 16 September 2022 in the area surrounding Kyushu Electric Power's Genkai NPP in Saga Prefecture and from 11 to 13 January 2023 in the area around Shikoku Electric Power's Ikata NPP. The surveys targeted administrative, medical, and nursing care facilities and residents. Responses from 57 participants indicated a lack of awareness of natural and nuclear disasters, challenges in evacuation planning, and a gap between nuclear disaster training and residents' understanding of evacuation protocols. This study highlights inadequacies in nuclear disaster preparedness and the need for a better understanding among residents regarding evacuation procedures. This study identified three key issues: (1) a lack of awareness about disasters, including nuclear disasters; (2) concerns about complex disasters and the difficulties in creating evacuation plans; and (3) a discrepancy between nuclear disaster training and residents' understanding of evacuation procedures. To bridge this gap, it is important to deepen residents' understanding of nuclear disasters, continuously convey the lessons learned from the FDNPP accident, and regularly reassess and update nuclear disaster preparedness strategies.

Abstract:Background: Intake of potassium iodide (KI) reduces the accumulation of radioactive iodine in the thyroid gland in the event of possible contamination by radioactive iodine released from a nuclear facility. The WHO has stated the need for research for optimal timing, appropriate dosing regimen and safety for repetitive iodine thyroid blocking (ITB). The French PRIODAC project, addressed all these issues, involving prolonged or repeated releases of radioactive iodine. Preclinical studies established an effective dose through pharmacokinetic modeling, demonstrating the safety of repetitive KI treatment without toxicity. Summary: Recent preclinical studies have determined an optimal effective dose for repetitive administration, associated with pharmacokinetic modelling. The results show the safety and absence of toxicity of repetitive treatment with KI. Good laboratory practice level preclinical studies corresponding to individuals > 12 years have shown a safety margin established between animal doses without toxic effect. After approval from the French health authorities, the market authorization of the 2 tablets of KI-65mg/day was defined with a new dosing scheme of a daily repetitive intake of the treatment up to 7 days unless otherwise instructed by the competent authorities for all categories of population except pregnant women, and children under the age of 12 years. Conclusions: This new marketed authorization resulting from scientific-based evidence obtained as part of the PRIODAC project may serve as an example to further harmonize the application of KI for repetitive ITB in situations of prolonged radioactive release at the European and International levels, under the umbrella of the WHO.

Returning Home after Decontamination? Applying the Protective Action Decision Model to a Nuclear Accident Scenario

Abstract:Studies of the aftermath of nuclear power plant accidents show that affected citizens assess higher risks and adopt more risk-avoidant behaviors than authorities expect. This results in differences between the planned recovery and actual outcomes. Based on this knowledge, this study examined the factors that affect citizens' preference to continue living in a decontaminated area. Testing the key aspects of the protective action decision model (PADM), this study analyzed Swedish survey data (N = 2291) regarding such an accident scenario. Several aspects of the PADM, from the layperson's view of threats and protective actions, to stakeholders and situational factors, were strongly supported. The most influential variables affecting settlement choices are perceptions of radiation risk, perceptions of decontamination effectiveness, government information, living with certain restrictions, and attachment to an area because of one's work. A novel contribution of this study is that it ranked the significance of such effects on behavioral intentions in an emergency scenario. Regarding the policy recommendations, this study concluded that a recovery program must facilitate most aspects of people's lives and provide trustworthy information on decontamination efficiency. As some people will avoid potential health risks and leave a decontaminated area, planning to implement one solution for everyone would likely not be optimal.
