Abstract:Cardiac remodeling is a common pathological process in various heart diseases, such as cardiac hypertrophy, diabetes-associated cardiomyopathy and ischemic heart diseases. The inhibition of cardiac remodeling has been suggested to be a potential strategy for preventing heart failure. However, the mechanisms involved in cardiac remodeling are quite complicated. Recent studies have reported a close correlation between autophagy and energy homeostasis in cardiac remodeling associated with various heart diseases. In this review, we summarize the roles of autophagy and energy homeostasis in cardiac remodeling and discuss the relationship between these two processes in different conditions to identify potential targets and strategies for treating cardiac remodeling by regulating autophagy.

What problem does this paper attempt to address?
This paper aims to explore the interaction between autophagy and energy homeostasis in cardiac remodeling and its mechanisms. Specifically, the paper attempts to address the following issues: 1. **Background of cardiac remodeling**: Cardiac remodeling is a common pathological process in various heart diseases (such as cardiac hypertrophy, diabetes - related cardiomyopathy, and ischemic heart disease), which is characterized by a series of changes in left ventricular function and structure and may eventually lead to heart failure. Although researchers have focused on the underlying mechanisms of malignant cardiac remodeling and heart failure, the specific mechanisms of cardiac remodeling still need to be further elucidated, which limits the development of new therapies. 2. **Roles of autophagy and energy homeostasis**: The paper summarizes the roles of autophagy and energy homeostasis in cardiac remodeling and discusses the relationship between these two processes. Autophagy is a process of cellular self - phagocytosis, which can degrade damaged organelles, misfolded proteins, and other macromolecular substances to provide energy supply. Energy homeostasis involves how cells maintain energy supply through different metabolic pathways. These two are closely related in the process of cardiac remodeling. Abnormal autophagy will lead to energy metabolism disorders, and energy metabolism disorders will in turn affect autophagy. 3. **Relationship between autophagy and energy homeostasis under different conditions**: The paper explores in detail the interaction between autophagy and energy homeostasis under different conditions, such as their manifestations in myocardial infarction, myocardial ischemia - reperfusion injury, and diabetes - related cardiomyopathy. For example, in myocardial infarction, the activation of autophagy is an adaptive response, which helps protect the heart from damage; while in myocardial ischemia - reperfusion injury, excessive autophagy may lead to the death of cardiomyocytes, thus exacerbating heart damage. 4. **Potential therapeutic targets and strategies**: By understanding the interaction mechanism between autophagy and energy homeostasis, the paper aims to identify potential therapeutic targets and strategies to regulate autophagy, thereby preventing or reversing cardiac remodeling. For example, mTOR (mammalian target of rapamycin) is an important regulator of autophagy and energy homeostasis. By regulating the activity of mTOR, autophagy and energy homeostasis can be optimized simultaneously, providing new ideas for the treatment of cardiac remodeling. In summary, this paper attempts to provide a theoretical basis for the development of new treatment strategies by systematically summarizing and analyzing the roles of autophagy and energy homeostasis in cardiac remodeling and their mutual relationship.
