Abstract:Objective To compare the effects of fractional CO2 laser and microablative fractional radiofrequency treatment with promestriene topical estrogen on sexual function and genitourinary syndrome of menopause symptoms. Methods This was a prospective randomized open-label clinical trial conducted with 62 postmenopausal women assigned to three intervention groups: a) topical promestriene for 90 days ( n = 17); b) fractional CO2 laser treatment ( n = 24); and c) microablative fractional radiofrequency treatment ( n = 21). Each of the latter two groups underwent three treatment sessions at 4-week intervals. At baseline and at the end of the study, all participants had a gynecological examination that included vaginal pH measurement, and the completion of the Vaginal Symptom Score, the Vaginal Health Index, and the Female Sexual Function Index. For the energy treatment groups, adverse effects were evaluated after each session. Group homogeneity was assessed at baseline, and results were evaluated over time (from baseline to the end of treatment) and between groups over time. Results All baseline parameters were similar among studied groups. At the end of the study, all 3 treatments had produced similar effects: a reduction of vaginal pH, and an improvement of vulvovaginal symptoms (Vaginal Symptom Score and Vaginal Health Index scores) as well as sexual function (higher total Female Sexual Function Index, and in the desire, arousal, lubrication and pain domain scores), with no differences observed between groups. Side-effects were slight for both energy treatment groups, mainly represented by vaginal discharge. Conclusion The present study suggests that the two energy treatments were efficient along with promestriene at improving postmenopausal genitourinary and sexuality symptoms. Comparative Effectiveness Of Nonsurgical Treatment For Stress Urinary Incontinence In Adult Women: A Systematic Review And Network Meta-Analysis Of Randomized Controlled Trials

Abstract:Objective: This study aimed to explore the most effective nonsurgical therapy to treat stress urinary incontinence (SUI). Methods: Data were pooled and the Bayesian random effects model was utilized to evaluate the inconsistencies between indirect and direct proof. Network sub-analyses, sensitivity and subgroup analysis, and standard pairwise comparisons were utilized in preparation of the main analysis. Results: A total of 17 RCTs (852 participants) were enrolled. With regard to therapy, pelvic floor muscle training (PFMT) was more often recommended than biofeedback (BF). The combination of PFMT and BF was remarkably promoted than PFMT alone or PFMT with vaginal cone (VC) or electrical stimulation (ES). The QL scores of patients who underwent PFMT were noticeably improved compared with those of patients who underwent ES. The combination of BF and PFMT was remarkably promoted compared with BF alone. VC was superior to ES with regard to QL scores. By contrast, ES alone was the least preferred method to decrease urine leakage and to promote QL. Conclusion: The combination of BF and PFMT was proven to be an optimal method to treat SUI which aims to promote urine leakage and LQ compared with either PFMT alone, ES, VC, or circular muscle (CME), while ES was the least preferred method to promote LQ.
The Fractional Co2 Laser For The Treatment Of Genitourinary Syndrome Of Menopause: A Prospective Multicenter Cohort Study

Abstract:Background and Objectives Genitourinary syndrome of menopause (GSM) is a common condition affecting of most postmenopausal women, which greatly impacks the quality of life,and need to treat. This prospective multicenter cohort study aimed to compare the efficacy and safety of the fractional carbon dioxide (CO2) laser with that of topical estrogen for vaginal treatment and relieving symptoms of genitourinary syndrome of menopause (GSM).Study Design/Materials and Methods This study included 162 postmenopausal patients who received vaginal laser or topical Estriol cream therapy between January 2017 and May 2019 at eight study centers in China. The degree of GSM-related symptoms (vaginal burning, dryness, and dyspareunia) was evaluated using the Vaginal Health Index score (VHIS) and Visual Analog Scale (VAS) at baseline, 1, 3, 6, and 12 months posttreatment. The primary endpoint was the improvement in vaginal burning, dryness, and dyspareunia at 6 months after treatment. Multivariate logistic regression was used to compare the rate of improvement in the two groups.Results At baseline, the laser and control groups showed no significant difference in the mean age, time after menopause, and the VHIS (all P > 0.05). In the laser group, compared with baseline, significant differences were seen in the VHIS after the first or second treatment session and at 1, 3, 6, and 12 months posttreatment (P < 0.01). In the control group, compared with baseline, the VHIS showed significant differences after 1, 3, and 6 months of treatment (P < 0.05). However, there was no significant difference after 3 and 6 months of follow-up between the two groups (P > 0.05). The VHIS scores were significantly higher after 1 month (16.63 +/- 2.79 vs. 15.57 +/- 2.43) and 12 months (15.72 +/- 2.59 vs. 12.12 +/- 4.08) of treatment in both the groups (P < 0.05). At 6 months after treatment, both groups showed improvement in vaginal burning, vaginal dryness, and dyspareunia (P > 0.05). The VAS findings at 6 months posttreatment were significantly different when compared with the pretreatment findings (P < 0.001). There were no significant adverse effects in the two groups.Conclusions Fractional CO2 laser vaginal treatment could be a safe and effective option for treating symptoms of GSM, including vaginal burning, dryness, and dyspareunia. Abstract:Background: Fractional CO2 laser (FCL) has been gradually used in the management of genitourinary syndrome of menopause (GSM) in recent years, but the results remain uncertain. We aimed to estimate the efficacy and safety of FCL as a novel treatment for controlling the GSM, especially sexual dysfunction, compared with intravaginal estrogen. Methods: The following digital databases were searched, including PubMed, Web of Science, Cochrane Library, and Embase. All the studies were limited to randomized controlled trials (RCTs). Included literatures must be in English, without limitation about region and race. The pooled dates were analyzed by Review Manger version 5.4 (Cochrane Collaboration’s Information Management System, London, UK). Results: 3 RCTs were included, which involved 160 patients, 79 of whom received FCL treatment, 81 received intravaginal estrogen treatments. This meta-analysis results showed no statistical significance existed in improving the patient’s vaginal health index (VHI) (mean difference (MD) = 0.59; 95% confidence interval (95% CI): –1.07~2.25; p = 0.49) after FCL treatment. In terms of the female sexual function index (FSFI) total scores, we observed no improvement without statistical significance (MD = –0.13; 95% CI: –3.32~3.06; p = 0.94). Furthermore, all the FSFI domains showed no statistical significance. No serious complications were reported in all included trials. Conclusions: FCL therapy was an interesting and novel treatment for menopausal female sexual dysfunction. However, no statistical evidence existed, which recommended that FCL was superior to intravaginal estrogen. Our results will be confirmed by high-quality and multicentric RCTs in the future.

[Optical Properties of ZnO Films Fabricated by Atomic Layer Deposition].

Abstract:The ZnO films were deposited by atomic layer deposition method using water and diethylzinc as precursors at different temperatures (110 and 190 degrees C). X-ray photoelectron spectroscopy, spectroscopic ellipsometry and photoluminescence spectra (PL) were used to investigate the elemental composition and optical properties of ZnO films. Our results showed that with the increasing of the growth temperature, the amount of -OH groups in the ZnO film decreased, which indicated that the reactions went to completion at high processing temperatures. The PL spectra of the ZnO film deposited at 110 degrees C exhibited two emission bands, one in the UV region and the other in the visible region. When the deposition temperature increased to 190 degrees C, the emission bands in the visible region disappeared, which indicated that the deep level defect in ZnO became less. The carrier mobility improved from 25 to 32 cm2 x (V x S)(-1) with the reduction of the defects in the ZnO film. The refractive index of the ZnO films decreased from 2.33 to 1.9 in the 375-800 nm region. The optical absorption edge (E(g)) values of the ZnO films deposited at different temperature were about 3.27 eV.

Abstract:Genitourinary syndrome of menopause (GSM) encompasses a range of distressing symptoms in the vulvovaginal and/or bladder-urethral regions related to menopause changes, negatively influencing woman's quality of life and sexual activity. Fractional micro-ablative CO2 laser therapy has shown the potential to reinstate the vaginal epithelium to a condition akin to the premenopausal state, thereby ameliorating the subjective symptoms associated with GSM. We conducted a prospective, pilot study in 73 sexually active postmenopausal women treated with CO2 laser for their GSM symptoms, while assessing Vaginal Health Index Score (VHIS) and sexual function through the Female Sexual Function Index (FSFI) Questionnaire. The laser treatment resulted in a decrease in VHIS and patient-reported vulvovaginal atrophy (VVA) symptoms, with a significantly lower prevalence of vaginal itching, dryness, and burning (p < 0.001), as well as dyspareunia (p = 0.002). The occurrence of urinary incontinence, urgency, and vaginal heaviness significantly reduced, with an improvement in the staging of cystocele, either to Stage 1 or complete resolution (p < 0.001). FSFI total and domain scores were significantly higher after the treatment, indicating better sexual function, with a post-treatment score median of 25 (p < 0.001). Therefore, using a three-cycle fractional CO2 laser was an effective choice for reducing urogenital discomfort related to GSM in postmenopausal women.
Efficacy of CO2 laser treatment in postmenopausal women with vulvovaginal atrophy: A meta-analysis

Abstract:Objective: To explore the efficacy of CO2 laser treatment in postmenopausal women with vulvovaginal atrophy. Method: PubMed, Embase, Cochrane Library and Web of Science were searched to June 9, 2020. Prospective studies on the efficacy of CO2 laser treatment were included. Two researchers independently reviewed articles and extracted data. Heterogeneity test was conducted for each outcome indicator. Sensitivity analysis was performed in all models. Results: Twelve articles including 459 participants were enrolled. Compared with baseline, vaginal health indeices (VHIs) were significantly higher at the 1-, 3-, 6-, and 12-month follow ups (P < 0.001). For VVA severity, the visual analog scale scores for vaginal dryness at 1-, 3-, 6-, and 12-month follow-ups (P < 0.050), vaginal burning, itching, and dysuria at 1-month follow up (P < 0.001), and dyspareunia at 1-, 3-, 6-, and 12-month follow-ups (P < 0.001) were all significantly lower. For FSFI, total scores at 1-, 3-, 6-, and 12-month follow ups (P < 0.001), and the scores in desire, arousal, lubrication, orgasm, satisfaction, and pain at 1-month follow up (P < 0.050) were all significantly higher. For quality of life, the PCS12 and MCS12 scores were all significantly higher (P < 0.050) at the 1-month follow up. Conclusion: CO2 laser treatment may be effective for postmenopausal women with VVA symptoms in improving quality of life and sexual function.
Carbon dioxide laser therapy for the management of genitourinary syndrome of menopause: A meta‑analysis of randomized controlled trials

Abstract:Genitourinary symptoms of menopause (GSM) affect ~50% of women after menopause. Recently, CO2 laser therapy has been used for managing GSM but without high quality evidence. The present review assessed the effectiveness of CO2 laser therapy in the management of GSM. PubMed, Embase, Web of Science, CENTRAL and Scopus databases were searched for randomized controlled trials (RCTs), published up to June 30, 2023, comparing CO2 laser and sham laser treatments for GSM management. The outcomes of interest included Female Sexual Function Index (FSFI), Vaginal Health Index (VHI) and visual analog scale (VAS) for dyspareunia, dryness, burning, itching and dysuria. A total of seven RCTs were included in the review and meta-analysis, with 6/7 studies using three sessions of laser therapy, 4-8 weeks apart. Meta-analysis demonstrated no statistically significant difference in FSFI [mean difference (MD), -1.48; 95% CI, -5.85, 2.89; I2=45%] and VHI scores (MD, -0.18; 95% CI, -1.66, 1.31; I2 =72%) between laser and control groups. Meta-analysis also demonstrated no statistically significant difference in VAS scores for dyspareunia (MD, -1.63; 95% CI; -4.06, 0.80; I2=91%), dryness (MD, -1.30; 95% CI, -3.14, 0.53; I2=75%), burning (MD, -0.76; 95% CI, -2.03; 0.51 I2=56%), itching (MD, -0.28; 95% CI, -0.95, 0.38; I2=0%) and dysuria (MD, 0.15; 95% CI, -0.37, 0.67; I2=23%) between the groups. The included RCTs had low risk of bias. In conclusion, meta-analyses of high-quality sham-controlled RCTs indicated that CO2 may not have any beneficial effect on GSM. Limited data and high heterogeneity in meta-analyses in this area of research are important limitations that need to be addressed by future RCTs.

Abstract:Female Wistar rats were infused with an isotonic solution (110 mM NaCl + 25 mM NaHCO3) containing 5 mU of Pitressin per milliliter, at a nonexpanding rate of 0.016 ml/min. Per-corten (1.25 mg i.m.) was given 1 h before the experiment. Urine flow rate, sodium and potassium excretion, and creatinine clearance were measured over 20-min periods. After the control values were obtained, bilateral electrolytic lesions were made in the posterior hypothalamus or the lateral hypothalamic area. Another group of rats was sham operated, and in controls an equivalent current was passed through the body. Posterior hypothalamic lesions were immediately followed by significant increases in the urine flow rate, UNa, and UNaV. Creatinine clearance increased insignificantly. However, the significant decrease of the fractional sodium reabsorption indicated that the natriuresis was mainly of tubular origin. Abstract:Background: Increasingly, women have sought alternatives to traditional options (lubricants, estrogen products, and hormone replacement therapy) for unwelcome vaginal changes of menopause.Objectives: This study evaluated whether a series of three monthly fractional CO2 laser treatments significantly improves and maintains vaginal health indices of elasticity, fluid volume, pH level, epithelial integrity, and moisture. Self-reported symptoms of vaginal atrophy were also measured. Biopsy samples after a series of three treatments were evaluated for histological changes to vaginal canal tissue.Methods: Forty postmenopausal women were treated extravaginally and internally with a fractional CO2 laser. Objective measurements of vaginal health index, as well as subjective measurements of symptoms of vulvovaginal atrophy (VVA), urinary incontinence, and sexual function were reported at baseline. Follow-up evaluations were at one, three, six, and 12 months after the third treatment.Results: Vaginal health index improved significantly after the first treatment and was maintained with mean improvement of 9.6 ± 3.3 (P < 0.001) and 9.5 ± 3.3 (P < 0.001) at the 6- and 12-month follow ups, respectively. Vaginal symptoms of dryness, itching, and dyspareunia improved significantly (P < 0.05) at all evaluations. Histological findings showed increased collagen and elastin staining, as well as a thicker epithelium with an increased number of cell layers and a better degree of surface maturation.Conclusions: Fractional CO2 laser treatments were well tolerated and were associated with improvement in vaginal health and amelioration of symptoms of VVA. Histological changes in the epithelium and lamina propria, caused by fractional CO2 laser treatments, correlated with clinical restoration of vaginal hydration and pH to premenopausal levels.Level of Evidence 4:

Abstract:Background: Objective were to evaluate the efficacy of fractional CO2 laser in treating genito-urinary syndrome (GSM). Methods: We did a prospective observational study on 92 post-menopausal women aged 37-84 years during December 2022-December 2023 in a private clinic of district Kanpur. Fractional CO2 laser (Rosch, vaginal 3600 probe) in three sittings were done for women presenting with GSM at the interval of four weeks as a lunch break therapy. The outcome was studied in terms of visual analogue scale (VAS) having 0 to 10 rating at second, third and sixth months. The statistical analysis was performed using MS excel and GraphPad online statistical calculator. Results: Stress urinary incontinence, early prolapse, urinary incontinence was the commonest presenting complaints among females. Significant improvement was observed in VAS score in each follow up. Conclusions: The vaginal fractional CO2 laser can be used as an effective and safe treatment method in GSM. It is necessary to conduct studies with long-term follow-up.

A randomized controlled study of vaginal fractional CO2 laser therapy for female sexual dysfunction

Abstract:The purpose of this study is to evaluate the efficacy and safety of vaginal fractional CO2 laser therapy for female sexual dysfunction (FSD). A total of 84 women at high risk of sexual dysfunction were randomly divided into two groups. Women in the laser group received vaginal fractional CO2 laser therapy. Others in the Kegel group were advised to participate in Kegel exercise training. Sexual distress and sexual function were evaluated by using the Female Sexual Distress Scale-Revised (FSDS-R) and the Chinese version Female Sexual Function Index (CVFSFI), respectively. Adverse events were recorded during the 12-month follow-up. At the end of the 6th and 12th months, the lubrication scores of the CVFSFI in the laser group (4.55±0.05, 4.58±0.09) were significantly higher than those in the Kegel group (4.19±0.15, 4.20±0.14) (P<0.05). The satisfaction scores in the laser group (4.43±0.08) were higher than those in the Kegel group (4.20±0.16) at the end of the 6th month (P<0.05). The self-contrast test in the laser group showed significant improvement in lubrication, pain, satisfaction and total scores after CO2 laser therapy (p<0.05). These improvements were maintained for 1 year. The improvement of FSDS-R in the laser group (10.0±0.2) was more evident than in the Kegel group (11.1±0.4) at the end of the 12th month. There were no major adverse events reported during laser therapy. Vaginal fractional CO2 laser therapy can effectively improve sexual function without any serious adverse events. It might be an effective and relatively safe treatment option for improving vaginal mucosa status in sexually active women with sexual dysfunction.

Abstract:Objective: To compare the effect of noninvasive radiofrequency (RF) with vaginal estrogen (E), and vaginal moisturizer (M) on improving vulvovaginal atrophy (VVA) in women with genitourinary syndrome of menopause. Methods: A total of 32 postmenopausal women who met the inclusion criteria were randomized into three intervention arms to receive one of the following treatments: three sessions of noninvasive RF therapy (RF arm); intravaginal estriol cream 1 mg applied daily for 2 weeks, followed by 1 mg applied two times weekly or 1 mg of estradiol vaginal fast-dissolving film applied daily for 2 weeks, followed by 1 mg applied two times weekly (E arm); and intravaginal moisturizer two times a week (M arm). Assessments at baseline and after 4 months were conducted using Vaginal Health Index score, Vaginal Maturation, visual analog scale for VVA symptoms (dyspareunia, dryness, and burning), and Menopause Rating Scale (MRS) for urogenital symptoms. Vaginal wall biopsies were administered to participants who consented, pretreatment and posttreatment (at baseline and after 4 months of follow-up). Results: After 4 months, the Vaginal Health Index showed an increase of 6.6 points in mean total score in the RF arm, also in the E arm (+7.3 points), with no significant improvement in the M arm (+1.5 points) (interaction effect: RF, E ≠ M, P < 0.001). Regarding vaginal maturation, there was a significant increase in superficial cells in the E arm (+31.3), with no significant changes in the RF (+9.3) and M (-0.5) arms (interaction effect: E ≠ M, P < 0.001). Vaginal pH decreased significantly in the E arm (-1.25), with a similar response in the RF arm (-1.7), with no significant improvement in the M arm (-0.25) (interaction effect: RF, E ≠ M, P < 0.001).There was a significant improvement in the MRS score for VVA symptoms in the three intervention arms, with no predominance of any arm, whereas the improvement in the total MRS score for urogenital symptoms showed a predominance of the RF arm (ΔRF: -7.8; ΔE: -3.5; ΔM: -2.3; RF ≠ E, M). According to histopathologic analysis, there was no statistically significant increase in glycogenation ( P = 0.691) or epithelial cone height ( P = 0.935), despite an increase in the median delta (difference between pretreatment and posttreatment) in the three intervention arms (glycogenation: RF arm Δ = +118.4%; E arm Δ = +130.9%; M arm Δ = +24.9%; epithelial cone height: RF arm Δ = +33.5%; E arm Δ = +18.6%; M arm Δ = +22.3%). Conclusion: The effect of noninvasive RF on the treatment of vulvovaginal symptoms of genitourinary syndrome of menopause was similar to vaginal estrogen, except for hormonal cytology, and superior to vaginal moisturizer, with improvement in some histomorphometric parameters. These findings are promising, especially for the population that cannot or prefers not to use vaginal estrogen therapy.

Abstract:Background: Menopause involves a range of bodily changes, with impacts on physical and psychological well-being. Around half of the postmenopausal women experience genitourinary syndrome of menopause (GSM). Fractional CO2 laser treatment can promote tissue regeneration in the vaginal wall to potentially assist with managing GSM. However, the results from clinical trials of this treatment have been mixed, and the personal perceptions and experiences of women receiving this treatment have been largely unexplored. Objectives: To qualitatively explore the motivations and outcomes of women who have undergone vaginal fractional CO2 laser treatment. Methods: Fourteen postmenopausal women were involved in the study. These women had undergone vaginal fractional CO2 laser treatment between 2 and 48 months earlier (M = 32.1, standard deviation = 14.9 months). Telephone interviews were conducted to explore women's motivations and their experiences after treatment. Interviews were recorded and transcribed verbatim. Deductive and inductive thematic analysis was conducted to analyze interviews. Results: Analyses produced 4 major themes. First, motivations mostly revolved around participants seeking relief from menopausal symptoms. Second, some participants noted positive sexual outcomes, including improved sexual pleasure after treatment. Third, participants noted positive physical and psychological effects, including improvements in incontinence and overall confidence. Lastly, a subset of participants reported no discernible changes. Conclusions: This novel qualitative exploration of women's motivations and outcomes of fractional CO2 laser therapy demonstrates the multifaceted impact of treatment. These findings highlight the importance of considering the holistic effects of fractional CO2 laser therapy on women's health during midlife, particularly amid menopausal changes.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Genitourinary syndrome of menopause (GSM) negatively affects sexual function and quality of life. Abstract:Aim To characterize the effects of CO2 laser treatment and estrogen treatment on vaginal microbiota in patients with genitourinary syndrome of menopause (GSM). Methods Sixty‐four patients with genitourinary syndrome were divided into the estrogen group, the CO2 laser group, and the control group. The control group did not receive any treatment. Vaginal mucosa was collected after 3 and 12 months of treatment. The former was used for 16S rRNA sequencing, and the latter was used for pathological evaluation. Vaginal health and voiding function were assessed using the vaginal health index (VHI) scale and the UDI‐6 scale at 3 and 12 months after treatment. Results The results showed that both treatments reduced alpha diversity in the vaginal flora. Additionally, the abundance of 65 genera differed significantly between the treatment and control groups, with an increase in potentially beneficial bacteria such as Lactobacillus, IheB3_7, Mycoplasma urealyticum, and Streptococcus. In addition, the VHI and UDI‐6 scores improved in both treatment groups compared to the control group after 3 months. Whereas VHI and UDI‐6 scores were close to baseline in the estrogen group, and remained significantly improved in the CO2 laser group after 12 months. Pathological results showed that both methods improved the vaginal health status of patients with GSM after 12 months of treatment. However, the CO2 group exhibited a more significant increase in type III collagen. Conclusions Both CO2 laser and estrogen therapies can regulate the vaginal flora imbalance of GSM and improve the corresponding symptoms. However, the long‐term efficacy of CO2 laser therapy is superior compared to estrogen therapy.

Abstract:IntroductionMenopause is the condition when a woman no longer manifests menstrual periods. GSM (Genitourinary Syndrome) is the acronym to describe possible symptoms of this condition. Vaginal dryness can be a particular problem for women entering menopause, it can make the intercourse painful and may cause the increase in the need to urinate. Aim of this study is to evaluate the efficacy of combined fractional CO2 (10.500 nm) ablative and 1540 nm non‐ablative lasers on these symptoms. Materials and Methods40 postmenopausal non‐estrogenized women with moderate to severe vaginal atrophy referring symptoms related to GSM were recruited. A treatment cycle included 3 laser applications (every 4 weeks), was performed.Time points of the study were baseline (T1), week 4 (T2), week 8 (T3) and week 12 (T4).. Results evaluation was done with a modified FSFI (Female Sexual Function Index) and a modified Stress Urinary Incontinence scale. ResultsA significant improvement both of FSFI total score and each individual FSFI domain score was observed. No undesired side effects or short‐term or long‐term complications were shown. ConclusionThis study shows that the "two wavelengths endovaginal laser treatment" is a safe and effective technique to treat signs and symptoms related to GSM syndrome. For the treatment of women with severe urogenital symptoms who do not benefit from lifestyle changes, vaginal moisturizers or local estrogen therapy, clinicians should consider the use of vaginal fractional laser treatment.This article is protected by copyright.
