Abstract:Amyotrophic Lateral Sclerosis (ALS) is a neurodegenerative disease that leads to progressive muscle weakness and paralysis, ultimately resulting in the loss of ability to communicate and control the environment. EEG-based Brain-Computer Interface (BCI) methods have shown promise in providing communication and control with the aim of rehabilitating ALS patients. In particular, P300-based BCI has been widely studied and used for ALS rehabilitation. Other EEG-based BCI methods, such as Motor Imagery (MI) based BCI and Hybrid BCI, have also shown promise in ALS rehabilitation. Nonetheless, EEG-based BCI methods hold great potential for improvement. This review article introduces and reviews FFT, WPD, CSP, CSSP, CSP, and GC feature extraction methods. The Common Spatial Pattern (CSP) is an efficient and common technique for extracting data properties used in BCI systems. In addition, Linear Discriminant Analysis (LDA), Support Vector Machine (SVM), Neural Networks (NN), and Deep Learning (DL) classification methods were introduced and reviewed. SVM is the most appropriate classifier due to its insensitivity to the curse of dimensionality. Also, DL is used in the design of BCI systems and is a good choice for BCI systems based on motor imagery with big datasets. Despite the progress made in the field, there are still challenges to overcome, such as improving the accuracy and reliability of EEG signal detection and developing more intuitive and user-friendly interfaces By using BCI, disabled patients can communicate with their caregivers and control their environment using various devices, including wheelchairs, and robotic arms.

Abstract:Locomotor malfunction represents a major problem in some neurological disorders like stroke and spinal cord injury. Robot-assisted walking devices have been used during rehabilitation of patients with these ailments for regaining and improving walking ability. Previous studies showed the advantage of brain-computer interface (BCI) based robot-assisted training combined with physical therapy in the rehabilitation of the upper limb after stroke. Therefore, stroke patients with walking disorders might also benefit from using BCI robot-assisted training protocols. In order to develop such BCI, it is necessary to evaluate the feasibility to decode walking intention from cortical patterns during robot-assisted gait training. Spectral patterns in the electroencephalogram (EEG) related to robot-assisted active and passive walking were investigated in 10 healthy volunteers (mean age 32.3±10.8, six female) and in three acute stroke patients (all male, mean age 46.7±16.9, Berg Balance Scale 20±12.8). A logistic regression classifier was used to distinguish walking from baseline in these spectral EEG patterns. Mean classification accuracies of 94.0±5.4% and 93.1±7.9%, respectively, were reached when active and passive walking were compared against baseline. The classification performance between passive and active walking was 83.4±7.4%. A classification accuracy of 89.9±5.7% was achieved in the stroke patients when comparing walking and baseline. Furthermore, in the healthy volunteers modulation of low gamma activity in central midline areas was found to be associated with the gait cycle phases, but not in the stroke patients. Our results demonstrate the feasibility of BCI-based robotic-assisted training devices for gait rehabilitation.

Abstract:Motor disability affects a person's ability to move and maintain balance. To remove this pain from the society, brain computer interface (BCI) system with the assistance of motor imagery (MI) tasks classification plays an important role. BCI translates human intension by brain activity into control signals to communicate with their external environment without direct physical movement. The current BCI system works with massive data through electroencephalogram (EEG) signals. Traditional methods in BCI are limited in efficiency, accuracy, and speed. To overcome these limitations, this study aims to develop an optimized artificial Intelligence-based technique for identifying human intentions of physical movement through EEG data for an advanced BCI system. The proposed technique is designed involving Common Spatial Pattern (CSP) and Medium K Nearest Neighbour (MKNN) technique to achieve higher classifier accuracy, which was tested on a publicly available EEG dataset (IVa) of BCI Competition III. This study produces the highest accuracy score in the case of all subjects above 90%, and the average score is above 97% for our proposed model, which outperforms the existing methods. This study's findings will assist technologists in creating frontier medical science technology and significantly improve BCI systems in Australia and worldwide. This proposed technique will help to identify the intensions of motor disabled people rapidly, assisting patients’ rehabilitation and daily living.

Electroencephalogram based communication system for locked in state person using mentally spelled tasks with optimized network model

Abstract:Due to growth in population, Individual persons with disabilities are increasing daily. To overcome the disability especially in Locked in State (LIS) due to Spinal Cord Injury (SCI), we planned to design four states moving robot from four imagery tasks signals acquired from three electrode systems by placing the electrodes in three positions namely T1, T3 and FP1. At the time of the study we extract the features from Continuous Wavelet Transform (CWT) and trained with Optimized Neural Network model to analyze the features. The proposed network model showed the highest performances with an accuracy of 93.86 % then that of conventional network model. To confirm the performances we conduct offline test. The offline test also proved that new network model recognizing accuracy was higher than the conventional network model with recognizing accuracy of 97.50 %. To verify our result we conducted Information Transfer Rate (ITR), from this analysis we concluded that optimized network model outperforms the other network models like conventional ordinary Feed Forward Neural Network, Time Delay Neural Network and Elman Neural Networks with an accuracy of 21.67 bits per sec. By analyzing classification performances, recognizing accuracy and Information Transformation Rate (ITR), we concluded that CWT features with optimized neural network model performances were comparably greater than that of normal or conventional neural network model and also the study proved that performances of male subjects was appreciated compared to female subjects.
Application of a Brain–Computer Interface System with Visual and Motor Feedback in Limb and Brain Functional Rehabilitation after Stroke: Case Report

Abstract:(1) Objective: To investigate the feasibility, safety, and effectiveness of a brain–computer interface (BCI) system with visual and motor feedback in limb and brain function rehabilitation after stroke. (2) Methods: First, we recruited three hemiplegic stroke patients to perform rehabilitation training using a BCI system with visual and motor feedback for two consecutive days (four sessions) to verify the feasibility and safety of the system. Then, we recruited five other hemiplegic stroke patients for rehabilitation training (6 days a week, lasting for 12–14 days) using the same BCI system to verify the effectiveness. The mean and Cohen’s w were used to compare the changes in limb motor and brain functions before and after training. (3) Results: In the feasibility verification, the continuous motor state switching time (CMSST) of the three patients was 17.8 ± 21.0s, and the motor state percentages (MSPs) in the upper and lower limb training were 52.6 ± 25.7% and 72.4 ± 24.0%, respectively. The effective training revolutions (ETRs) per minute were 25.8 ± 13.0 for upper limb and 24.8 ± 6.4 for lower limb. There were no adverse events during the training process. Compared with the baseline, the motor function indices of the five patients were improved, including sitting balance ability, upper limb Fugel–Meyer assessment (FMA), lower limb FMA, 6 min walking distance, modified Barthel index, and root mean square (RMS) value of triceps surae, which increased by 0.4, 8.0, 5.4, 11.4, 7.0, and 0.9, respectively, and all had large effect sizes (Cohen’s w ≥ 0.5). The brain function indices of the five patients, including the amplitudes of the motor evoked potentials (MEP) on the non-lesion side and lesion side, increased by 3.6 and 3.7, respectively; the latency of MEP on the non-lesion side was shortened by 2.6 ms, and all had large effect sizes (Cohen’s w ≥ 0.5). (4) Conclusions: The BCI system with visual and motor feedback is applicable in active rehabilitation training of stroke patients with hemiplegia, and the pilot results show potential multidimensional benefits after a short course of treatment.

Abstract:Stroke is one of the prime reasons for paralysis throughout the world caused due to impaired nervous system and resulting in disability to move the affected body parts. Rehabilitation is the natural remedy for recovering from paralysis and enhancing the quality of life. Brain Computer Interface (BCI) controlled assistive technology is the new paradigm, providing assistance and rehabilitation for the paralysed. But, most of these devices are error prone and also hard to get continuous control because of the dynamic nature of the brain signals. Moreover, existing devices like exoskeletons brings additional burden on the patient and the caregivers and also results in mental fatigue and frustration. To solve these issues Artificial Muscle Intelligence with Deep Learning (AMIDL) system is proposed in this paper. AMIDL integrates user intentions with artificial muscle movements in an efficient way to improve the performance. Human thoughts captured using Electroencephalogram (EEG) sensors are transformed into body movements, by utilising microcontroller and Transcutaneous Electrical Nerve Stimulation (TENS) device. EEG signals are subjected to pre-processing, feature extraction and classification, before being passed on to the affected body part. The received EEG signal is correlated with the recorded artificial muscle movements. If the captured EEG signal falls below the desired level, the affected body part will be stimulated by the recorded artificial muscle movements. The system also provides a feature for communicating human intentions as alert message to caregivers, in case of emergency situations. This is achieved by offline training of specific gesture and online gesture recognition algorithm. The recognised gesture is transformed into speech, thus enabling the paralysed to express their feelings to the relatives or friends. Experiments were carried out with the aid of healthy and paralysed subjects. The AMIDL system helped to reduce mental fatigue, miss-operation, frustration and provided continuous control. The thrust of lifting the exoskeleton is also reduced by using light weight wireless electrodes. The proposed system will be a great communication aid for paralysed to express their thoughts and feelings with dear and near ones, thereby enhancing the quality of life.

Abstract:In the realm of motor rehabilitation, Brain-Computer Interface Neurofeedback Training (BCI-NFT) emerges as a promising strategy. This aims to utilize an individual's brain activity to stimulate or assist movement, thereby strengthening sensorimotor pathways and promoting motor recovery. Employing various methodologies, BCI-NFT has been shown to be effective for enhancing motor function primarily of the upper limb in stroke, with very few studies reported in cerebral palsy (CP). Our main objective was to develop an electroencephalography (EEG)-based BCI-NFT system, employing an associative learning paradigm, to improve selective control of ankle dorsiflexion in CP and potentially other neurological populations. First, in a cohort of eight healthy volunteers, we successfully implemented a BCI-NFT system based on detection of slow movement-related cortical potentials (MRCP) from EEG generated by attempted dorsiflexion to simultaneously activate Neuromuscular Electrical Stimulation which assisted movement and served to enhance sensory feedback to the sensorimotor cortex. Participants also viewed a computer display that provided real-time visual feedback of ankle range of motion with an individualized target region displayed to encourage maximal effort. After evaluating several potential strategies, we employed a Long short-term memory (LSTM) neural network, a deep learning algorithm, to detect the motor intent prior to movement onset. We then evaluated the system in a 10-session ankle dorsiflexion training protocol on a child with CP. By employing transfer learning across sessions, we could significantly reduce the number of calibration trials from 50 to 20 without compromising detection accuracy, which was 80.8% on average. The participant was able to complete the required calibration trials and the 100 training trials per session for all 10 sessions and post-training demonstrated increased ankle dorsiflexion velocity, walking speed and step length. Based on exceptional system performance, feasibility and preliminary effectiveness in a child with CP, we are now pursuing a clinical trial in a larger cohort of children with CP.

Abstract:Nowadays, Electroencephalogram (EEG) devices that do not require invasive procedures get more attraction. Brain-Computer Interface (BCI) systems use EEG analysis to identify users' mental states, cognitive shifts, and stimuli-induced reactions. The Motor Execution (ME) paradigm is a vital control paradigm that holds great significance in this framework. In this manuscript, an Efficient Predefined Time Adaptive Neural Network for EEG-Based Brain-Computer Interaction in Motor Execution Classification (EPTNN-BCI-EEG) is proposed. Initially, the input signals are collected from EEG Dataset. The input signals are preprocessed using Robust M-Type Error-State Kalman Filter (RMESKF) to remove the artifacts and baseline signals. Then, the pre-processed signals are given to Signed Cumulative Distribution Transform (SCDT) for feature extraction. SCDT is used to extract Spatial and temporal features. Afterward, Efficient Predefined Time Adaptive Neural Network (EPTNN) is used to classify the EEG gender and signal, such as Audio/Video and Male/Female. In General, EPTNN does not expose any adoption of optimization systems to determine optimal parameters to classify the EEG gender and signal. Hence, Bitwise Arithmetic Optimization Algorithm (BAOA) is proposed to optimize the EPTNN. The proposed EPTNN-BCI-EEG approach is implemented in MATLAB and the performance metrics, such as Recall, Accuracy, Root Mean Square Error (RMSE), F1-score, Specificity, ROC, Computation time are assessed. The performance of EPTNN-BCI-EEG approach provides 17.82 %, 28.95 %, and 19.2 % higher accuracy, 24.38 %, 34.53 %, and 18.78 % higher recall, 26.7 %, 24.2 %, and 32.7 % higher specificity when analysed with the existing methods.

Abstract:Background: The anticipatory proportion (AP), the ratio between perseverative and anticipatory speech errors, is reduced in patients with brain injury. However, it is unknown whether the AP is also reduced in elderly speakers with cognitive impairment. Methods: 20 elderly speakers with a Mini Mental State Examination (MMSE) score of 25-27 and 20 elderly speakers with an MMSE score of 28-30 were assessed using a tongue-twister-based speech test, the Regensburg Word Fluency Test (RWT) and an object naming test. Results: The AP in the group of speakers with an MMSE score of 25-27 was significantly lower. Accordingly, the AP and scores in the RWT and the object naming test were higher in persons with an MMSE score of 28-30. Conclusion: Language alterations in mild cognitive dysfunction are detectable with the AP. Further longitudinal studies are needed to evaluate the predictive value of the AP.

Abstract:Transforming human intentions into patterns to direct the devices connected externally without any body movements is called Brain-Computer Interface (BCI). It is specially designed for rehabilitation patients to overcome their disabilities. Electroencephalogram (EEG) signal is one of the famous tools to operate such devices. In this study, we planned to conduct our research with twenty subjects from different age groups from 20 to 28 and 29 to 40 using three-electrode systems to analyze the performance for developing a mobile robot for navigation using band power features and neural network architecture trained with a bioinspired algorithm. From the experiment, we recognized that the maximum classification performance was 94.66% for the young group and the minimum classification performance was 94.18% for the adult group. We conducted a recognizing accuracy test for the two contrasting age groups to interpret the individual performances. The study proved that the recognition accuracy was maximum for the young group and minimum for the adult group. Through the graphical user interface, we conducted an online test for the young and adult groups. From the online test, the same young-aged people performed highly and actively with an average accuracy of 94.00% compared with the adult people whose performance was 92.00%. From this experiment, we concluded that, due to the age factor, the signal generated by the subjects decreased slightly.

Abstract:A smart wheelchair provides mobility assistance to persons with motor disabilities by processing sensory inputs from the person. This involves accurately collecting inputs from the user during various movement activities and using them to determine their intended motion. These smart wheelchairs work by collecting brain signals in the form of electroencephalography (EEG) signals and by processing them into a quantized format to provide movement assistance to people. Such systems can be referred to as brain–computer interface (BCI) systems that work with EEG signals. Acquiring data from human beings in the form of brain signals through EEG, along with processing of those signals and ensuring the correctness of actions instigated by those brain signals involve a huge amount of data. In this work, we carried out an experiment by taking 100 human subjects and recording their brain signals using a NeuroMax device. Typical wheelchairs are constrained by design as the motion of those is limited either by manual operation or controlled by haptic sensors and actuators. The main objective in this work was to design a wheelchair with better usability and control using machine learning-based knowledge, which is typically a data-driven approach. However, the proposed approach was designed to take inputs from human gestures and brain sensory activities to provide better usability to the wheelchair. The attention meditation cost–benefit analysis (AMCBA) proposed in this paper aims to reduce the risk of inappropriate results and improve performance by considering various cost–benefit parameters. The said classifier aims to improve the quality of emotion recognition by filtering features from EEG signals using methods of feature selection. The operation of the proposed method is described in two steps: in the first step, we assign weights to different channels for the extraction of spatial and temporal information from human behavior. The second step presents the cost–benefit model to improve the accuracy to help in decision-making. Moreover, we tried to assess the performance of the wheelchair for various assumptions and technical specifications. Finally, this study achieves improved performance in the most difficult circumstances to provide a better experience to persons with immobility.

Abstract:The present study aimed to use the proposed system to measure and analyze brain waves of users to allow intelligent upper limb rehabilitation and to optimize the system using a genetic algorithm. The study used EPOC Neuroheadset for Emotiv with EEG electrodes attached as a non-invasive method for measuring brain waves. The brain waves were measured according to the EEG 10-20 standard electrode layout, which allows measurement of signals from each spot where electrodes are attached based on EEG characteristics. The measured data were added in a database. In the intelligent neuro-fuzzy model, wave transform was used for extracting brain wave characteristics according to user intentions and to eliminate noise from the signals in an effort to increase reliability. Moreover, to construct the option rules of the neuro-fuzzy system, FCM technique and optimal cluster evaluation method were used. Furthermore, the asymmetric Gaussian membership function was used to improve performance, whereas SD and WF divided into left and right sides were used to express the chromosomes. Optimal EEG electrode locations were found, and comparative analysis was performed on the differences based on membership function, number of clusters, and number of learning generations, learning algorithm, and wavelet settings. The performance evaluation results showed that the optimal EEG electrode locations were F7, F8, FC5, and FC6, whereas the accuracy of learning and test data of user-intention recognition was found to be 94.2% and 92.3%, respectively, which suggests that the proposed system can be used to recognize user intention for specific behavior. The system proposed in the present study can allow continued rehabilitation exercise in everyday living according to user intentions, which is expected to help improve the user's willingness to participate in rehabilitation and his or her quality of life.

Abstract:Stroke is the second largest cause of death worldwide and one of the most common causes of disability. However, several approaches have been proposed to deal with stroke patient rehabilitation like robotic devices and virtual reality systems, researchers have found that the brain-computer interfaces (BCI) approaches can provide better results. In this study, the electroencephalography (EEG) dataset from post-stroke patients were investigated to identify the effects of the motor imagery (MI)-based BCI therapy by investigating sensorimotor areas using frequency and time-domain features and to select particular methods that help in enhancing the MI-based BCI systems for stroke patients using EEG signal processing. Therefore, to detect the imagined movements that are typically required within conventional rehabilitation therapy with good identification accuracies, the conventional filters and wavelet transform (WT) denoising technique was used in the first stage. Next, attributes from frequency and entropy domains were computed. Finally, support vector machine (SVM) classification techniques were utilized to test the motor imagery (MI)-based BCI rehabilitation. The results demonstrate the capability of the WT denoising technique together with the used features and SVM classifier to discriminate the tested classes of the left hand, right hand and foot MI-based BCI rehabilitation. This study will help medical doctors, clinicians, physicians and technicians to introduce a good rehabilitation program for post-stroke patients.

Abstract:Reliance on wheelchairs after spinal cord injury (SCI) leads to many medical co-morbidities. Treatment of these conditions contributes to the majority of SCI health care costs. Restoring able-body-like ambulation after SCI may reduce the incidence of these conditions, and increase independence and quality of life. However, no biomedical solution exists that can reverse this lost neurological function, and hence novel methods are needed. Brain-computer interface (BCI) controlled lower extremity prosthesis may constitute one such novel approach. One subject with able-body and one with paraplegia due to SCI underwent electroencephalogram (EEG) recording while engaged in alternating epochs of idling and walking kinesthetic motor imagery (KMI). These data were analyzed to generate an EEG prediction model for online BCI operation. A commercial robotic gait orthosis (RoGO) system (treadmill suspended), was interfaced with the BCI computer. In an online test, the subjects were tasked to ambulate using the BCI-RoGO system when prompted by computerized cues. The performance of this system was assessed with cross-correlation analysis, and omission and false alarm rates. The offline accuracy of the EEG prediction model averaged 86.3%. The cross-correlation between instructional cues and BCI-RoGO walking epochs averaged 0.812 +/- 0.048 (p-value<10^-4). There were on average 0.8 false alarms per session and no omissions. This is the first time a person with parapegia due to SCI regained basic brain-controlled ambulation, thereby indicating that restoring brain-controlled ambulation is feasible. Future work will test this system in a population of individuals with SCI. If successful, this may justify future development of invasive BCI-controlled lower extremity prostheses. This system may also be applied to incomplete SCI to improve neurological outcomes beyond those of standard physiotherapy.

Abstract:Brain–computer interface (BCI) is a novel communication method between brain and machine. It enables signals from the human brain to influence or control external devices. Currently, much research interest is focused on the BCI-based neural rehabilitation of patients with motor and cognitive diseases. Over the decades, BCI has become an alternative treatment for motor and cognitive rehabilitation. Previous studies demonstrated the usefulness of BCI intervention in restoring motor function and recovery of the damaged brain. Electroencephalogram (EEG)-based BCI intervention could cast light on the mechanisms underlying neuroplasticity during upper limb recovery by providing feedback to the damaged brain. BCI could act as a useful tool to aid patients with daily communication and basic movement in severe motor loss cases like amyotrophic lateral sclerosis (ALS). Furthermore, recent findings have reported the therapeutic efficacy of BCI in people suffering from other diseases with different levels of motor impairment such as spastic cerebral palsy, neuropathic pain, etc. Besides motor functional recovery, BCI also plays its role in improving the behavior of patients with cognitive diseases like attention-deficit/hyperactivity disorder (ADHD). The BCI-based neurofeedback training is focused on either reducing the ratio of theta and beta rhythm, or enabling the patients to regulate their own slow cortical potentials, and both have made progress in increasing attention and alertness. With summary of several clinical studies with strong evidence, we present cutting edge results from the clinical application of BCI in motor and cognitive diseases, including stroke, spinal cord injury, ALS, and ADHD.

Abstract:Background: Spinal cord injury (SCI) can leave the affected individuals unable to ambulate. Since there are no restorative treatments for SCI, novel approaches such as brain-controlled prostheses have been sought. Our recent studies show that a brain-computer interface (BCI) can be used to control ambulation within a virtual reality environment (VRE), suggesting that a BCI-controlled lower extremity prosthesis for ambulation may be feasible. However, the operability of our BCI has not been tested in a SCI population. Methods: Five subjects with paraplegia or tetraplegia due to SCI underwent a 10-min training session in which they alternated between kinesthetic motor imagery (KMI) of idling and walking while their electroencephalogram (EEG) were recorded. Subjects then performed a goal-oriented online task, where they utilized KMI to control the linear ambulation of an avatar and make 10 sequential stops at designated points within the VRE. Multiple online trials were performed over 5 experimental days. Results: Classification accuracy of idling and walking was estimated offline and ranged from 60.5% (p=0.0176) to 92.3% (p=1.36*10^-20) across subjects and days. In the online task, all subjects achieved purposeful control with an average performance of 7.4 +/- 2.3 successful stops in 273 +/- 51 sec (p<0.01). All subjects maintained purposeful control throughout the study, and their online performances improved over time. Conclusions: The results demonstrate that SCI subjects can purposefully operate a self-paced BCI walking simulator to complete a goal-oriented ambulation task. The operation of this BCI system requires short training, is intuitive, and robust against subject-to-subject and day-to-day neurophysiological variations. These findings indicate that BCI-controlled lower extremity prostheses for gait rehabilitation or restoration after SCI may be feasible in the future.

Abstract:Objective: Amyotrophic lateral sclerosis (ALS) is a rare disease, but is also one of the most common motor neuron diseases, and people of all races and ethnic backgrounds are affected. There is currently no cure. Brain computer interfaces (BCIs) can establish a communication channel directly between the brain and an external device by recognizing brain activities that reflect user intent. Therefore, this technology could help ALS patients in promoting functional independence through BCI-based speller systems and motor assistive devices. Methods: In this paper, two kinds of ERP-based speller systems were tested on 18 ALS patients to: (1) assess performance when they spelled 42 characters online continuously, without a break; and (2) to compare performance between a matrix-based speller paradigm (MS-P, mean visual angle 6 degree) and a new speller paradigm that used a larger visual angle called the large visual angle speller paradigm (LS-P, mean visual angle 8 degree). Results: Although results showed that there were no significant differences between the two paradigms in accuracy trend over continuous use (p>0.05), the fatigue during the LS-P condition was significantly lower than that of MS-P (p<0.05). Results also showed that continuous use slightly reduced the performance of this ERP-based BCI. Conclusion: 15 subjects obtained higher than 80% feedback accuracy (online output accuracy) and 9 subjects obtained higher than 90% feedback accuracy in one of the two paradigms, thus validating the BCI approaches in this study. Significance: Most ALS subjects in this study could spell effectively after continuous use of an ERP-based BCI. The new LS-P display may be easier for subjects to use, resulting in lower fatigue.

Abstract:Motor imagery (MI) based brain computer interface (BCI) has been extensively studied to improve motor recovery for stroke patients by inducing neuroplasticity. However, due to the lower spatial resolution and signal-to-noise ratio (SNR) of electroencephalograph (EEG), MI based BCI system that involves decoding hand movements within the same limb remains lower classification accuracy and poorer practicality. To overcome the limitations, an adaptive hybrid BCI system combining MI and steady-state visually evoked potential (SSVEP) is developed to improve decoding accuracy while enhancing neural engagement. On the one hand, the SSVEP evoked by visual stimuli based on action-state flickering coding approach significantly improves the recognition accuracy compared to the pure MI based BCI. On the other hand, to reduce the impact of SSVEP on MI due to the dual-task interference effect, the event-related desynchronization (ERD) based neural engagement is monitored and employed for feedback in real-time to ensure the effective execution of MI tasks. Eight healthy subjects and six post-stroke patients were recruited to verify the effectiveness of the system. The results showed that the four-class gesture recognition accuracies of healthy individuals and patients could be improved to % and %, respectively. Moreover, the designed hybrid BCI could maintain the same degree of neural engagement as observed when subjects solely performed MI tasks. These phenomena demonstrated the interactivity and clinical utility of the developed system for the rehabilitation of hand function in stroke patients.

Abstract:Background: Traditional therapist-based rehabilitation training for patients with movement impairment is laborious and expensive. In order to reduce the cost and improve the treatment effect of rehabilitation, many methods based on human-computer interaction (HCI) technology have been proposed, such as robot-assisted therapy and functional electrical stimulation (FES). However, due to the lack of active participation of brain, these methods have limited effects on the promotion of damaged nerve remodeling. New method: Based on the neurofeedback training provided by the combination of brain-computer interface (BCI) and exoskeleton, this paper proposes a multimodal brain-controlled active rehabilitation system to help improve limb function. The joint control mode of steady-state visual evoked potential (SSVEP) and motor imagery (MI) is adopted to achieve self-paced control and thus maximize the degree of brain involvement, and a requirement selection function based on SSVEP design is added to facilitate communication with aphasia patients. Comparison with existing methods: In addition, the Transformer is introduced as the MI decoder in the asynchronous online BCI to improve the global perception of electroencephalogram (EEG) signals and maintain the sensitivity and efficiency of the system. Results: In two multi-task online experiments for left hand, right hand, foot and idle states, subject achieves 91.25% and 92.50% best accuracy, respectively. Conclusion: Compared with previous studies, this paper aims to establish a high-performance and low-latency brain-controlled rehabilitation system, and provide an independent and autonomous control mode of the brain, so as to improve the effect of neural remodeling. The performance of the proposed method is evaluated through offline and online experiments.

Abstract:Purpose: Brain–computer interface (BCI)-controlled assistive robotic systems have been developed with increasing success with the aim to rehabilitation of patients after brain injury to increase independence and quality of life. While such systems may use surgically implanted invasive sensors, non-invasive alternatives can be better suited due to the ease of use, reduced cost, improvements in accuracy and reliability with the advancement of the technology and practicality of use. The consumer-grade BCI devices are often capable of integrating multiple types of signals, including Electroencephalogram (EEG) and Electromyogram (EMG) signals.Materials and Methods: This paper summarizes the development of a portable and cost-efficient BCI-controlled assistive technology using a non-invasive BCI headset "OpenBCI" and an open source robotic arm, U-Arm, to accomplish tasks related to rehabilitation, such as access to resources, adaptability or home use. The resulting system used a combination of EEG and EMG sensor readings to control the arm. To avoid risks of injury while the device is being used in clinical settings, appropriate measures were incorporated into the software control of the arm. A short survey was used following the system usability scale (SUS), to measure the usability of the technology to be trialed in clinical settings.Results: From the experimental results, it was found that EMG is a very reliable method for assistive technology control, provided that the user specific EMG calibration is done. With the EEG, even though the results were promising, due to insufficient detection of the signal, the controller was not adequate to be used within a neurorehabilitation environment. The survey indicated that the usability of the system is not a barrier for moving the system into clinical trials.For the rehabilitation of patients suffering from neurological disabilities (particularly those suffering from varying degrees of paralysis), it is necessary to develop technology that bypasses the limitations of their condition. For example, if a patient is unable to walk due to the unresponsiveness in their motor neurons, technology can be developed that used an alternate input to move an exoskeleton, which enables the patient to walk again with the assistance of the exoskeleton.This research focuses on neuro-rehabilitation within the framework of the NHS at the Kent and Canterbury Hospital in UK. The hospital currently does not have any system in place for self-driven rehabilitation and instead relies on traditional rehabilitation methods through assistance from physicians and exercise regimens to maintain muscle movement.This paper summarises the development of a portable and cost-efficient BCI controlled assistive technology using a non-invasive BCI headset "OpenBCI" and an open source robotic arm, U-Arm, to accomplish tasks related to rehabilitation, such as access to resources, adaptability or home use.
