Abstract:Objective To research changes of symptoms and bone structure imagings of temporomandibular joint(TMJ) after occlusal reconstruction.Methods Choose 20 patients who needed occlusal reconstruction in Department of Prosthodontics from January to December in 2011.Check the clinical symptoms and bone structure of TMJ by CBCT before and 3 months after occlusal reconstruction.Measure the anterior,middle and posterior joint space according to reconstructing image perpendicular to the horizontal axis of condyle.The results were analysed through chi-square test,paired t test and linear correlation analysis.Results 3 months after occlusal reconstruction,masticatory muscle and joint pain was significantly reduced,but joint clicking wasn' t ameliorated obviously.The results of CBCT imaging showed the anterior joint space significantly decreased and the posterior space enlarged.There was significant correlation between posterior joint space variation and occlusal increased distance according to linear correlation analysis and regression analysis.Conclusions There is reference value to measure TMJ space for deciding the amount of occlusal increased distance.
Study of changes of condylar morphology and position by MRI with fixed inclined bite-plate therapy in mandibule-retrusion patients

Abstract:Objective To study the changes of condylar morphology and position in the mandibule-retrusion patients during fixed inclined bite-plate treatment.Methods The subjects were 13 consecutive Class II osseous malocclusions with mandibular retrusion treated with fixed inclined-bite.From each subject,four MRI images of TMJ were obtained and evaluated:before treatment,at the beginning of treatment,during treatment and after treatment.Results MRI-signs of condylar remodeling were observed at the posterior-superior border in all 13 subjects treated with fixed inclined-bite.Condylar-fossa relationship changes were unconspicuous before and after treatment.Conclusion Condylar remodeling seems to contribute significantly to the increase in mandibular growth resulting from fixed inclined bite-plate treatment,while condylar-fossa relationship has no remarkable changes.

Advanced Strategies for NetZero Energy Building : Focused on the Early Phase and Usage Phase of a Building ’ s Life Cycle

Abstract:To cope with ‘Post-2020’, each country set its national greenhouse gas (GHG) emissions reduction target (e.g., South Korea: 37%) below its business-as-usual level by 2030. Toward this end, it is necessary to implement the net-zero energy building (nZEB) in the building sector, which accounts for more than 25% of the national GHG emissions and has a great potential to reduce GHG emissions. In this context, this study conducted a state-of-the-art review of nZEB implementation strategies in terms of passive strategies (i.e., passive sustainable design and energy-saving technique) and active strategies (i.e., renewable energy (RE) and back-up system for RE). Additionally, this study proposed the following advanced strategies for nZEB implementation according to a building’s life cycle: (i) integration and optimization of the passive and active strategies in the early phase of a building’s life cycle; (ii) real-time monitoring of the energy performance during the usage phase of a building’s life cycle. It is expected that this study can help researchers, practitioners, and policymakers understand the overall implementation strategies for realizing nZEB.
Correlation Between Mandibular Condylar Process Fracture and Temporomandibular Joint

Abstract:This review evaluates the literature on the relationship between mandibular condylar process fracture and temporomadibular joint (TMJ). The topic of condylar fracture generated more discussion and controversy than any other field of maxillofacial trauma associated with TMJ. Disturbance of occlusal function, deviation of mandible, internal derangements of TMJ, and ankylosis of the joint with resultant inability to move the jaw are sequelae of condylar process fracture. Thus it is necessary to understand how the masticatory system adapts to the structural alterations that accompany fractures of the mandibular condyle. Treatment of condylar process fracture include two methods ; closed treatment and open treatment. If one chooses to treat closed, one must understand that adaptations in the musculature, skeleton, and dentition will be necessary. Open treatment of condylar process fractures probably requires fewer adaptations within the masticatory system to provide a favorable functional outcome. However, one must weigh the risk of open surgery against the possible improvement in outcome. The risks are not just surgical risk, but biological risk as well, such as disruption of the blood supply to the condyle. This review presents relevant aspects of change of TMJ associated with condylar process fracture.

Stress Distribution in the Mandible with Unilateral Condylar Fracture.

Abstract:OBJECTIVE:The purpose of this study was to investigate the influence of unilateral condylar neck fracture upon the stress distribution in the mandible and the temporomandibular joint (TMJ) to realize primarily the biomechanical mechanism of the temporomandibular joint disorders (TMD) caused by TMJ injury.METHODS:The character of stress distribution in the mandible of a normal human and a patient with TMD induced by unilateral condylar neck fracture followed by malpositioned healing was analyzed quantitatively and compared during centric occlusion by combining spiral computed tomography scanning technology with the three-dimensional finite element method.RESULTS:The patient with unilateral condylar neck fracture followed by malpositioned healing had unbalanced stress distribution in the mandible, such as differing nature and value of stress. The stress in the fractured side was higher than in the nonfractured side, in which the condyle was more evident. The maximum principal stress and minimum principal stress of each region in the fractured mandible were much higher than those in a normal mandible. Stress quality also differed.CONCLUSIONS:The abnormal stress changes after unilateral condylar neck fracture with malpositioned healing may be related to the effect of condylar shape change on masticatory muscle function and occlusion. TMJ injury not only damaged the condylar structure, but also compromised its biomechanic environment. It is preliminarily thought that unbalanced stress distribution, evidently increased stress, and varied stress character are biomechanic mechanisms of TMD. Condylar dislocated fracture should be treated promptly and properly to maintain the proper prognostic stress distribution.
[Radiographic and masticatory physiologic evaluation after conservative treatment of condylar fractures in children and adolescents].

Abstract:OBJECTIVE:To evaluate the developmental and functional outcome of condylar fractures in children and adolescents after conservative treatment. METHODS:Eight children and adolescents with unilateral condylar fracture, aged 5-13 were included. A removable occlusal splint, the thickness of which was determined according to the age, the developmental stage of the dentition, the level of the fracture and the degree of dislocation, worn for 1-3 months, and the patients were asked to perform functional exercises. The patients were followed up by clinical observation, panoramic radiograph, temporomandibular joint(TMJ ) cone beam computed tomography(CBCT), and surface electromyography(sEMG) of masticatory muscles (superficial masseter, anterior temporalis, and anterior digastric muscles). Ramus height and body length of mandible were measured on panoramic radiograph. The patients were asked to return for follow-up visits at 1, 3, and 6 months after treatment, and then once a year. The patients underwent clinical examination at each follow-up visit, and radiological examinations at 6 months and then annually. RESULTS:All the patients showed clinically satisfactory results. CBCT showed smooth and continuous cortex. Panoramic X-ray revealed that the ramus height was shorter in the fractured side than in the contralateral side, while body length was longer. The mean asymmetry index(AI,x±s) for ramus height and body length were (3.29±2.68)% and (4.01 ± 2.54)%. sEMG showed either hypertension or hypotension in the masticatory muscles of the fractured side and asymmetries were obvious. The mean AI for sEMG activity of the anterior temporalis, masseter, and anterior digastric muscle were masseter: (15.0 ± 16.9)%; anterior temporalis: (21.5 ± 15.9)%; anterior digastric muscles: (11.9 ± 10.7)%. CONCLUSIONS:Conservative treatment of condylar fracture in children and adolescents had clinically satisfactory results, while mandibular development was slightly interrupted. Asymmetries of EMG activities of masticatory muscles were obvious. EMG could objectively reveal the functional recovery of condylar fracture in children.

Abstract:By the surgical orthodontic treatment for the patient who has a malocclusion, scaffold reverse occlusion, it is considered that remarkable change of maxillofacial form in the oro-facial region affects the stomatognathic functions. The purpose of this study was to analyze stomatognathic functions during border-movements and masticatory movements before and after surgical orthodontic treatment. The lateral border movements were analyzed regarding the symmetry of both sides and the change of angle of balancing condylar path. The masticatory movements were analyzed during opening and closing phases. We also analyzed cross patterns. In consequence, the lateral border movements were extended after the surgical orthodontic treatment, and the symmetries of the border-movement were improved. The angle of balancing condylar path did not show a large change in the sagittal plane by a remarkable change of the anterior guidances during lateral movements. However, the balancing condylar path in the horizontal and frontal plane had a tendency to change. The cross patterns of the masticatory movements tended to decrease after the surgical orthodontic treatment, and each of the opening phase and the closing phase tended to separated independently, i.e. each movement became stable. It was also suggested that the occlusal relationships of the upper and lower jaw including occlusal contacts had particular effect on the stomatognathic functions.
Changes in Condylar Position after Mandibular Reconstruction with Condylar Head Preservation by Computed Tomography.

Abstract:PURPOSE:Condylar position can change after mandibular reconstruction using the free fibula flap. The present study evaluated changes in condylar position using computed tomography (CT) after mandibular reconstruction with condylar head preservation.MATERIALS AND METHODS:This retrospective study consisted of 16 patients. CT data of 32 temporomandibular joints (TMJs) were recorded before surgery (T0), 7 to 10 days after surgery (T1), and 16.8 ± 7.4 months after surgery (T2). The anteroposterior condylar position was evaluated using the method of Pullinger and Hollender (Oral Surg Oral Med Oral Pathol 62:719, 1986). Repeated-measures analysis of variance (P = .05) was performed.RESULTS:Data of 16 patients were obtained for statistical analysis. Condylar position changed over time after mandibular reconstruction. The ipsilateral condyles moved anteroinferiorly after surgery (T0 to T1) and tended to move anterosuperiorly during follow-up (T1 to T2). No major changes were noted in the contralateral condyles.CONCLUSION:Condylar positions showed obvious changes over time after mandibular reconstruction with condylar preservation. Nevertheless, additional studies are warranted to further evaluate the relation between condylar position and TMJ function.
Cone-beam Computed Tomographic Evaluation of the Anatomical Changes of Temporomandibular Joint Use of Pre-post Dentures: A Time-control Study

Abstract:Aim and objective: The growth of the temporomandibular joint (TMJ) gets affected by multiple factors like aging, occlusion state, and by the movement of the jaw while masticating and swallowing. Radiographic imaging is often utilized as a vital diagnostic adjunct in the evaluation of certain examinations of the TMJ. Materials and methods: In this in vivo study, 30 male participants with mean age 55 years, having edentulous maxillary and mandibular arches from the Outpatient Department of Prosthodontics, were randomly selected. Group I (n = 30) patients who were edentulous for the last 4-5 years but without wearing dentures. Whereas group II (n = 30) patients who were edentulous for the last 4-5 years but were wearing dentures for this period. Maxillary and mandibular dentures were fabricated and delivered to subjects. Subjects were subjected to the TMJ analysis with the help of CBCT. Radiological images of dentomaxillofacial structures were analyzed by a specialist with a dual monitor inside a darkened silent room. On the monitor, three times measurements were recorded followed by calculation of mean value. The recordings were taken on both sides and thus, 210 sites were analyzed altogether, followed by the statistical analysis using SPSS software version 15.0. Results: The mean ages of group I and II were 59.00 ± 6.74 and 58.27 ± 6.75 years, respectively. The intra- and intergroup comparisons were done using a one-sample t-test. Differences in mean intercondylar width in groups I and II were not found to be statistically significant. The difference in mean length of glenoid fossa was not statistically significant at any of the above observation periods. A continuous decline in mean length of glenoid fossa was observed with time in both groups. The range of change in articular eminence length was found to be statistically significant for both the groups (p < 0.05). Conclusion: This study shows that the articular eminence flattening is correlated with age; on the other hand, the rate of deformation was found significantly more in total edentulous subjects as compared to subjects having normally maintained occlusion. The anatomical changes inside the TMJ have been much greater expressed within the completely edentulous subjects in whom the angle of sagittal condyle path declines and so does the articular eminence height. Clinical significance: It is essential to provide the edentulous patient with early prosthetic and occlusal rehabilitation after extractions to prevent the anatomical changes in TMJ.

Abstract:A woman aged 23 years is described who had watery diarrhoea and colicky abdominal pain with normal results on sigmoidoscopy and barium enema. Rectal biopsy showed a thick layer of collagen in the lamina propria below the free epithelial surface. This lesion extended throughout the length of the descending colon. The only change revealed by electron microscopy was an increase in the thickness of the sub-epithelial collagen layer which is normally present in the colon. The maturation of the fibroblasts was normal. The lesion in this patient may be considered as a small change from the normal situation. This lesion is typical of ‘collagenous colitis’ of which only three other cases have as yet been published.
Characteristics and treatment of mandibular condyle sagittal fracture

Abstract:Objective To investigate the clinical characteristics and the treatments of the mandibular condylesagittal fracture.Methods 315 patients with fracture of mandible were investigated in the present study.Among them there were 21 cases(23 TMJs)diagnosed as sagittal fracture of the mandibular condyle.Diagnosed radiograghy included the panoramic radiograph,2D-CT or 3D-CT scans.Treatments included surgery,maxilla-mandibular fixation(MMF)and functional therapy.Follow-up mandibular movements were recorded by digital calipers.Results The main clinical signs and symptoms of SFMC were pain in the TMJ area(21/21),limited mouth-opening(18/21),open-bite of front teeth(13/21),deflective occlusal contact(12/21)and so on.The images of 2D-CT and 3D-CT were characteristic and very valuable in diagnosis of SFMC.All patients had good protrusive,lateral protrusive or opening movements after treatment.Conclusions The choice of therapies for SFMC varies with the dislocation of the fracture fragments and patients' age.After effective treatment,the TMJs with SFMC will have good functions.
Effect of Molar Distalization on Condyle-Glenoid Fossa Relationship

Abstract:Objective: It is essential to be aware of the potential effects of orthodontic treatment on tissues and anatomical structures associated with the masticatory system, especially the temporomandibular joint (TMJ). Little information is available about the consequences of molar distalization on the TMJ. Therefore, this study is aimed at investigating the changes of the condyle-fossa relationship after molar distalization using the distal jet appliance. Materials and methods: The sample consisted of twenty-five patients (mean age 20.4 ± 2.6) who underwent molar distalization by the distal jet appliance. CBCT scans were taken before (T0) and after (T1) the completion of the molar distalization. Joint spaces (anterior, superior, and posterior) and cephalometric vertical angles (SN.GOME and Björk sum) were measured and compared at T0 and T1. Results: Superior and posterior joint spaces increased significantly after molar distalization (PS 0.29 mm, P < 0.001, SS 0.06 mm, P < 0.5). Vertical cephalometric angles also increased after molar distalization by the distal jet appliance (SN.GOME 0.92°, Björk 1.11°). Conclusion: There was a statistically significant increase in the superior and posterior joint spaces after molar distalization. However, this increase may not be of clinical importance. The vertical dimension has also increased.

Abstract:Intraoral splints are a commonly used dental treatment for a variety of conditions. Because such splints alter the condyle-disc-fossa relationship, they probably change the loading status of the temporomandibular joint (TMJ), including the TMJ disc. Collagen, a major constituent of the disc, acts to resist tensile loading, and it is presumed that the fiber orientations of the individual disc bands reflect their functional loading. Therefore, the purpose of this study was to examine effects of intraoral splint wear on TMJ morphology in general, and collagen orientation of the intra-articular disc in particular. Young adult, female miniature pigs were divided into three groups: open-bite splint, protrusive-bite splint, and unsplinted control. Splints were worn for 2 months, after which the TMJ discs were harvested for histological examination and stereological analysis, and the skulls were cleaned. Although the splints had no effect on skull dimensions, changes were seen in the TMJs. The discs of the protrusively-splinted group showed an increased thickness of the posterior band (P < 0.015) and minor changes in collagen orientation of the anterior band. The most striking change was the presence of a degenerative osseous defect on the medial side of the mandibular condyle in half of the splinted animals. These results indicate that prolonged splint wear can induce remodeling and even injury of TMJ tissues.

Abstract:Background/Objectives: Degenerative joint disease (DJD) of the TMJ can impact patients' quality of life and complicate orthodontic treatment. Stabilizing splints are a common conservative treatment in managing TMDs symptoms, although their long-term effects on condylar morphology are poorly studied. This study aimed to assess the impact of stabilizing splints on condyle morphology using cone-beam computed tomography (CBCT) in patients with various stages of DJD. Forty-two condyles with pre- (T1) and post- (T2) splint therapy scans were analyzed. Methods: CBCT scans were sectioned into sagittal and coronal slices for condyle classification and measurement. T1 and T2 CBCTs were superimposed before linear and area measurements at different poles. Results: Our results indicate that condyles in the normal group remain unchanged after splint therapy. The majority of subjects in the degenerative groups remained in the same classification group: six out of fourteen degenerative-active patients became degenerative-repair, while three out of twenty-two degenerative-repair patients progressed to degenerative-active. There is no significant remodeling of condylar width pre- and post-splint therapy. On average, there is more bone deposition than reduction in condylar height after splint therapy, although individual variation exists. Conclusions: Stabilizing splints offer a low-risk intervention for managing DJD and may contribute to favorable adaptive changes in the condyles.

Abstract:OBJECTIVE:To study the X-ray and clinical characteristics in sagittal fracture of the mandibular condyle (SFMC).METHODS:14 cases (15 sides) of SFMC in 48 cases who had condylar fractures were studied in the research, every patient was examined by the conventional x-ray, 2D-CT and 3D-CT, and followed up for 3-39 months.RESULTS:The fracture line mainly passed through lateral 1/3 and middle 1/3 of anterior condyle and the middle 1/3 of posterior condyle. Following-up (3-39 months) showed that the patients' occlusions were normal in all cases.CONCLUSIONS:The fracture line mainly located in the lateral pterygoid muscle fossa. Good results can be gotten by effective treatment. The 2D-CT and 3D-CT are valuable to the diagnosis.
Bilateral temporomandibular joint ankylosis as a sequelae of bilateral fracture of the mandibular condyle - A case report

Abstract:Temporomandibular Joint ankylosis is an extremely disabling affliction that causes problems in mastication, digestion, speech, appearance, and hygiene. Trauma and infection are the leading causes of ankylosis. However, in a young patient, a joint injury may not be noticed immediately. Ankylosis might have been developed as a sequel of the mandibular fractures. Early diagnosis and treatment are crucial if the worst sequelae of this condition are to be avoided.

Working condylar movement and its effects on posterior occlusal morphology

Abstract:The movement of the mesiolingual cusp of the maxillary left first molar was studied in the frontal and horizontal planes. The movement was studied while varying the top wall, rear wall, and incisal guidance on the articulator. It can be concluded that the top wall inclination may significantly influence movement of the mesisolingual cusp of the maxillary first molar during working mandibular movement, the rear wall inclination has less influence than top wall inclination on cusp movement during a working mandibular movement, and the working side condylar movement should be considered when evaluating or restoring the dentition.

Abstract:Bilateral sagittal split ramus osteotomy (BSSO) may change condylar position, which can be one of the factors contributing to skeletal relapse. This study evaluated short- and long-term changes in condylar position using cone-beam computed tomography (CBCT) and investigated changes in temporomandibular joint (TMJ) signs after BSSO for mandibular advancement in combination with Le Fort I osteotomy.Thirty-one patients were included, and CBCT data of 62 TMJs were collected before surgery (T0), immediately after surgery (T1), 3 months after surgery (T2), and at the last follow-up at 12.1 ± 3.0 months after surgery (T3). The relation of the condyle to the fossa was evaluated by the method of Pullinger and Hollender (Oral Surg Oral Med Oral Pathol 62:719, 1986). Clinical examination, with a special focus on signs of temporomandibular disorder (TMD), was documented at T0, T2, and T3. Repeated-measures analysis of variance (P = .05) and χ2 test (P = .05) were performed.Data of 27 patients were used for statistical analysis. Values from the formula of Pullinger and Hollender changed significantly with time, but there was no significant difference between the right and left condyles. Condyles moved inferoposteriorly immediately after surgery (T0 to T1) followed by anterosuperior movement 3 months after surgery (T1 to T2). The superimposed effect showed posterosuperior movement compared with the initial position before surgery (T0 to T2) and this position remained stable at 1-year follow-up (T2 to T3). A decrease of TMD signs over time, from 22.6% (T0) to 12.9% (T2) and 9.7% (T3), was observed, which showed no statistical significance.There were obvious changes in condylar position after BSSO in combination with Le Fort I osteotomy. Condyles tended to be located in a concentric position in relation to the glenoid fossa 3 months after surgery and remained stable during the 1-year follow-up. These changes did not cause an increase of TMD signs.
