Abstract:Aim Identify and describe nurse‐administered screening tools used in emergency departments (ED) to detect elder abuse. Design A scoping review of literature published between 1999 and 2024 was conducted following the guidance of the Joanna Briggs Institute Manual for Evidence Synthesis and a methodological framework for scoping studies. Methods Two reviewers, an academic faculty member and a senior undergraduate, conducted the screening and data extraction, aiming to identify studies using a nurse‐administered screening tool in the ED to detect elder abuse. Data Sources The final search was conducted on 24 April 2024, using the CINAHL, MEDLINE, PsycINFO, and Cochrane Review databases. Results Ten studies out of 145 met the inclusion criteria, identifying six screening tools that assist healthcare providers, such as nurses, in detecting elder abuse in EDs. The results were summarised and presented according to each screening tool. Implications for the Profession Nurses in EDs are well‐positioned to identify elder abuse due to the significant time spent observing and interacting with patients. The implementation of a screening tool can support nurses in detecting elder abuse and initiating appropriate interventions. Impact Elder abuse is a widespread public health issue projected to increase continuously with the rapidly ageing population. Incorporating nurse‐administered screening tool into EDs has demonstrated practicality and usefulness in identifying elder abuse cases. Various tools exist; however, these instruments are underutilised due to limited reliability and feasibility testing, with no definitive screening tool identified as the "gold standard" for elder abuse detection. Without formal screening, elder abuse is likely to remain undetected, leaving victims vulnerable to harmful consequences. Due to the limited testing and evaluation of a reliable ED screening tool for elder abuse, future research should focus on developing and validating a new screening tool intended specifically for use by nurses in EDs. A Remote and Sensorless Stator Winding Temperature Estimation Method for Thermal Protection for Induction Motor

Abstract:Three-phase induction motors are the "workhorses" of industry; they are the most widely used electrical machine; because of its simple structure and high reliability. This paper proposes a new technique to model the stator winding of the induction motor in Matlab Simulink® software. This simulation of the induction motor would have the thermal behavior of its stator winding; to study the induction motor temperature estimation using motor parameter-based method. The modified model is used to validate a remote and Sensorless stator winding temperature estimation technique; therefore a thermal protection is obtained for soft-starter-connected to induction motors. The soft-starter is used to inject a DC signal in the induction motor terminal voltage and current. The stator winding resistance/temperature is estimated from DC signal injection by changing the gate drive signals of the Thyristor in the soft starter. The level of the injected DC signal is adjusted by the value of the delay angle. The accuracy of stator winding temperature estimation increased with the increase of DC signal level; however the pulsation of the output torque increased also. The thermal behavior is simulated utilizing a thermal resistor block from the Matlab SimscapeTM software. It is used to replace the fixed resistor value of the induction motor model in the Matlab Simulink. The thermal monitoring scheme has been validated from the simulation results of a 7.5 kW induction motor under various loading conditions.

Abstract:This article is based on the lecture for the 2014 American Geriatrics Society Outstanding Scientific Achievement for Clinical Investigation Award. Elder abuse is a global public health and human rights problem. Evidence suggests that elder abuse is prevalent, predictable, costly, and sometimes fatal. This review will highlight the global epidemiology of elder abuse in terms of its prevalence, risk factors, and consequences in community populations. The global literature in PubMed, MEDLINE, PsycINFO, BIOSIS, Science Direct, and Cochrane Central was searched. Search terms included elder abuse, elder mistreatment, elder maltreatment, prevalence, incidence, risk factors, protective factors, outcomes, and consequences. Studies that existed only as abstracts, case series, or case reports or recruited individuals younger than 60; qualitative studies; and non-English publications were excluded. Tables and figures were created to highlight the findings: the most-detailed analyses to date of the prevalence of elder abuse according to continent, risk and protective factors, graphic presentation of odds ratios and confidence intervals for major risk factors, consequences, and practical suggestions for health professionals in addressing elder abuse. Elder abuse is common in community-dwelling older adults, especially minority older adults. This review identifies important knowledge gaps, such as a lack of consistency in definitions of elder abuse; insufficient research with regard to screening; and etiological, intervention, and prevention research. Concerted efforts from researchers, community organizations, healthcare and legal professionals, social service providers, and policy-makers should be promoted to address the global problem of elder abuse.

Abstract:Introduction Elder abuse is a prevalent, though often overlooked and underreported, cause of musculoskeletal injury in the elderly population. The purpose of this review is to provide an updated overview of the prevalence of elder abuse, its association with musculoskeletal injuries, and the available resources to aid orthopaedic surgeons in early detection and intervention. Significance Improved training on this topic is needed throughout the medical education of orthopaedic surgeons to effectively recognize and address elder abuse. Our findings reveal an urgent need for increased awareness, education, and collaboration among healthcare professionals to address this significant public health concern. As the aging population continues to grow, understanding the connection between elder abuse and musculoskeletal injuries is essential for providing comprehensive care to older adults. Results This review offers practical recommendations for identifying individuals at risk of elder abuse and outlines strategies for intervention. Indicators of abuse range from obvious signs like dirty clothes, neglect, and unattended injuries from falls, to more subtle cues requiring careful observation and questioning, such as mental health symptoms and family histories of abuse. Conclusion By shedding light on this often-overlooked issue, this review advocates for a proactive approach to identifying and addressing elder abuse to safeguard the well-being and quality of life of older individuals.

ObjectiveElder Mistreatment (EM) occurs in approximately 10% of older adults and is associated with trauma-related outcomes including depression, anxiety, post-traumatic stress disorder, and early mortality. Low screening and older adult self-reporting rates, especially within Emergency Departments represent missed opportunities for identify and mitigate future EM occurrences and consequences. To date, no studies have obtained EM survivors perspectives of EM screening and response practices in emergency departments.MethodsSemi-structured interviews with 19 EM survivors with Adult Protective Services validated EM were completed in the older adult's home. The Elder Mistreatment Emergency Department Screening and Response Tool (EM-SART) was used to guide the interview questions. All interviews were recorded, transcribed, and analyzed using qualitative thematic analyses.ResultsThe participants were mostly female (63%) and white (58%) with an average age of 74 years. Physical abuse accounted for 67% of the EM incidents. Three themes emerged indicating the survivor preference for (1) building rapport and approaching the older adult with compassion and care, (2) setting the context before asking the EM questions, and (3) allowing mutuality, collaborative work, and shared decision-making during the response.ConclusionsOlder adults are agreeable and willing to self-report and actively engage in the EM screening and response practices in the emergency department if trauma-informed care principles are utilized.

Elder abuse: a call to action

Abstract:An estimated 2 million people a year are victims of elder abuse, which ranges from neglect and mistreatment to physical abuse. By the year 2020, a full 22% of the population will be aged 65 or older. This demographic explosion demands that we identify and protect those at risk. To investigate the incidence of elder abuse or neglect (EAN) and to determine clinician awareness of associated risk factors, we conducted a 1-year retrospective review of thermally injured patients aged 60 or older. Data included age, total body surface area burned, mechanism of injury, length of hospital stay, mortality, abuse or neglect risk factors, and referral to the appropriate social agency. We found that our elderly patients (n = 28) were poorly screened for EAN. While 64% to 96% of patients were screened for cognitive impairment, overall health, and financial resources, none were screened for risk factors of emotional isolation. None of the patient's caregivers, including any spouses, roommates, or guardians, were screened for risk factors of substance abuse, familial violence, dependency needs, or external stresses. With the use of available data, we were able to place 11 patients on the following levels of abuse or neglect: 1) low risk for abuse; 2) self-neglect; 3) neglect; and 4) abuse. By this scale, 7 patients (64%) were victims of self-neglect, 3 patients (27%) were victims of neglect, and 1 patient (9%) was a victim of abuse. Adult Protective Services intervened in 2 cases. Recognizing that all cases of EAN should be preventable, we cannot accept the socioeconomic impact of this entity. The 11 patients identified as victims of neglect, self-neglect, or abuse accounted for 135 hospital days and 8 fatalities. Before we can address EAN, health care personnel must be made aware of the problem and routine screening for risk factors must be implemented. The true incidence of EAN is likely underestimated because health care providers have difficulty recognizing its features. A standard assessment tool to screen for neglect or abuse should be used for each older adult admission.
Secondary analysis of hospital patient experience scores across England’s National Health Service – How much has improved since 2005?

Abstract:Objective To examine trends in patient experience and consistency between hospital trusts and settings. Methods Observational study of publicly available patient experience surveys of three hospital settings (inpatients (IP), accident and emergency (A&E) and outpatients (OP)) of 130 acute NHS hospital trusts in England between 2004/05 and 2014/15. Results Overall patient experience has been good, showing modest improvements over time across the three hospital settings. Individual questions with the biggest improvement across all three settings are cleanliness (IP: +7.1, A&E: +6.5, OP: +4.7) and information about danger signals (IP: +3.8, A&E: +3.9, OP: +4.0). Trust performance has been consistent over time: 71.5% of trusts ranked in the same cluster for more than five years. There is some consistency across settings, especially between outpatients and inpatients. The lowest-scoring questions, regarding information at discharge, are the same in all years and all settings. Conclusions The greatest improvement across all three settings has been for cleanliness, which has seen national policies and targets. Information about danger signals and medication side-effects showed least consistency across settings and scores have remained low over time, despite information about danger signals showing a big increase in score. Patient experience of aspects of access and waiting have declined, as has experience of discharge delay, likely reflecting known increases in pressure on England’s NHS.

Abstract:This systematic review summarises and assesses the evidence for educational interventions that increase the knowledge and recognition of elder abuse. Frontline health workers lack sufficient protocols or experience to identify, handle, and report elder abuse. Developing new curricula guided by the literature continues to be a challenge. Interpretation of the educational programmes described in randomised controlled trials (RCTs) of elder abuse interventions can provide guidance when planning new programmes to increase knowledge and improve case-finding and cooperation. A detailed literature search was performed using five electronic databases related to healthcare: PubMed, PsycInfo, Web of Science, Cochrane Library, and MEDLINE. Only RCTs examining the effectiveness of educational interventions in preventing or reducing elder abuse were included. Framework analysis was used to integrate data from the retrieved documents. Forty-seven papers were read in full, and five were included in the review. Synthesis of the results from these studies resulted in five main themes: 1) Knowledge improvement; 2) Ability to identify; 3) Meta-analysis of recognition; 4) Characteristics of programmes; and 5) Outcome measurement. Increases in the immediate and longer-term outcomes of all interventions in the included studies were also determined. This review provides healthcare practitioners and older adult service providers with insights into educational intervention programmes that can be used for the development of elder abuse training modules.

Abstract:We experimentally demonstrate entanglement distillation of the two-mode squeezed state obtained by parametric down-conversion. Applying the photon annihilation operator to both modes, we raise the fraction of the photon-pair component in the state, resulting in the increase of both squeezing and entanglement by about 50%. Because of the low amount of initial squeezing, the distilled state does not experience significant loss of Gaussian character.

Abstract:Background: Maltreatment of older people (elder abuse) includes psychological, physical, sexual abuse, neglect and financial exploitation. Evidence suggests that 10% of older adults experience some form of abuse, and only a fraction of cases are actually reported or referred to social services agencies. Elder abuse is associated with significant morbidity and premature mortality. Numerous interventions have been implemented to address the issue of elder maltreatment. It is, however, unclear which interventions best serve to prevent or reduce elder abuse. Objectives: The objective of this review was to assess the effectiveness of primary, secondary and tertiary intervention programmes used to reduce or prevent abuse of the elderly in their own home, in organisational or institutional and community settings. The secondary objective was to investigate whether intervention effects are modified by types of abuse, types of participants, setting of intervention, or the cognitive status of older people. Search methods: We searched 19 databases (AgeLine, CINAHL, Psycinfo, MEDLINE, Embase, Proquest Central, Social Services Abstracts‎, ASSIA, Sociological Abstracts, ProQuest Dissertations & Theses Global, Web of Science, LILACS, EPPI, InfoBase, CENTRAL, HMIC, Opengrey and Zetoc) on 12 platforms, including multidisciplinary disciplines covering medical, health, social sciences, social services, legal, finance and education. We also browsed related organisational websites, contacted authors of relevant articles and checked reference lists. Searches of databases were conducted between 30 August 2015 and 16 March 2016 and were not restricted by language. Selection criteria: We included randomised controlled trials (RCTs), cluster-randomised trials, and quasi-RCTs, before-and-after studies, and interrupted time series. Only studies with at least 12 weeks of follow-up investigating the effect of interventions in preventing or reducing abuse of elderly people and those who interact with the elderly were included. Data collection and analysis: Two review authors independently extracted data and assessed the studies' risk of bias. Studies were categorised as: 1) education on elder abuse, 2) programmes to reduce factors influencing elder abuse, 3) specific policies for elder abuse, 4) legislation on elder abuse, 5) programmes to increase detection rate on elder abuse, 6) programmes targeted to victims of elder abuse, and 7) rehabilitation programmes for perpetrators of elder abuse. All studies were assessed for study methodology, intervention type, setting, targeted audience, intervention components and intervention intensity. Main results: The search and selection process produced seven eligible studies which included a total of 1924 elderly participants and 740 other people. Four of the above seven categories of interventions were evaluated by included studies that varied in study design. Eligible studies of rehabilitation programmes, specific policies for elder abuse and legislation on elder abuse were not found. All included studies contained a control group, with five of the seven studies describing the method of allocation as randomised. We used the Cochrane 'Risk of bias' tool and EPOC assessment criteria to assess risk of bias. The results suggest that risk of bias across the included body of research was high, with at least 40% of the included studies judged as being at high risk of bias. Only one study was judged as having no domains at high risk of bias, with two studies having two of 11 domains at high risk. One study was judged as being at high risk of bias across eight of 11 domains.All included studies were set in high-income countries, as determined by the World Bank economic classification (USA four, Taiwan one, UK two). None of the studies provided specific information or analysis on equity considerations, including by socio-economic disadvantage, although one study was described as being set in a housing project. One study performed some form of cost-effectiveness analysis on the implementation of their intervention programmes, although there were few details on the components and analysis of the costing.We are uncertain whether these interventions reduce the occurrence or recurrence of elder abuse due to variation in settings, measures and effects reported in the included studies, some of which were very small and at a high risk of bias (low- and very low-quality evidence).Two studies measured the occurrence of elder abuse. A high risk of bias study found a difference in the post-test scores (P value 0.048 and 0.18). In a low risk of bias study there was no difference found (adjusted odds ratio (OR) =0.48, 95% 0.18 to 1.27) (n = 214). For interventions measuring abuse recurrence, one small study (n = 16) reported no difference in post-test means, whilst another found higher levels of abuse reported for the intervention arms (Cox regression, combined intervention hazard ratio (HR) = 1.78, alpha level = 0.01).It is uncertain whether targeted educational interventions improve the relevant knowledge of health professionals and caregivers (very low-quality evidence), although they may improve detection of resident-to-resident abuse. The concept of measuring improvement in detection or reporting as opposed to measuring the occurrence or recurrence of abuse is complicated. An intervention of public education and support services aimed at victims may also improve rates of reporting, however it is unclear whether this was due to an increase in abuse recurrence or better reporting of abuse.The effectiveness of service planning interventions at improving the assessment and documentation of related domains is uncertain. Unintended outcomes were not reported in the studies. Authors' conclusions: There is inadequate trustworthy evidence to assess the effects of elder abuse interventions on occurrence or recurrence of abuse, although there is some evidence to suggest it may change the combined measure of anxiety and depression of caregivers. There is a need for high-quality trials, including from low- or middle-income countries, with adequate statistical power and appropriate study characteristics to determine whether specific intervention programmes, and which components of these programmes, are effective in preventing or reducing abuse episodes among the elderly. It is uncertain whether the use of educational interventions improves knowledge and attitude of caregivers, and whether such programmes also reduce occurrence of abuse, thus future research is warranted. In addition, all future research should include a component of cost-effectiveness analysis, implementation assessment and equity considerations of the specific interventions under review.

Abstract:Abstract Universal screening of older adults in emergency departments (ED) effectively improves identification of elder mistreatment (EM). To realize improved response, collaboration between ED providers and staff and local Adult Protective Services (APS) is essential. Qualitative data was derived from transcribed and coded key informant interviews with 10 APS workers and 5 ED clinical providers from 6 states. Results indicate each entity approaches EM from a different global perspective which informs key strategies to build effective response. ED providers focus on acute medical needs while APS focuses on long-term psychosocial needs. Assignment of specific individuals in both entities to be liaisons between ED and APS would support improved communication and collaboration. Education should focus on 1) capabilities and limitations in EM response and 2) clarity regarding bilateral sharing of information. Both entities report participation in multidisciplinary teams is valuable to foster collaboration on effective interventions in EM cases.

Elder Abuse and Dementia: A Review of the Research and Health Policy

Abstract:Older adults with dementia may be at high risk for abuse, but the topic has not been well studied. We conducted a literature review to examine the relationships between elder abuse and dementia. We found that psychological abuse was the most common form of abuse among older adults, with estimates of its prevalence ranging from 27.9 percent to 62.3 percent. Physical abuse was estimated to affect 3.5-23.1 percent of older adults with dementia. We also found that many older adults experienced multiple forms of abuse simultaneously, and the risk of mortality from abuse and self-neglect may be higher in older adults with greater levels of cognitive impairment. We summarize programs and policies related to the abuse of older adults with dementia, including adult protective services, mandatory elder abuse reporting, and the Long-Term Care Ombudsman Program. We also summarize aspects of the National Alzheimer's Project Act, the Older Americans Act, and the Elder Justice Act. In spite of a recent increase in research and policy developments on elder abuse, challenges such as insufficient funding, limited knowledge about elder abuse, a lack of funding for the implementation of federal and state programs relevant to elder abuse and dementia, and a lack of dementia-specific training for front-line health care staff persist. Stronger programs targeting the well-being of older adults with dementia are needed.

Towards the development of a screening tool to enhance the detection of elder abuse and neglect by emergency medical technicians (EMTs): a qualitative study

Abstract:BackgroundTo develop a screening tool to enhance elder abuse and neglect detection and reporting rates among emergency medical technicians (EMTs). Our primary aim was to identify the most salient indicators of elder abuse and neglect for potential inclusion on a screening tool. We also sought to identify practical elements of the tool that would optimize EMT uptake and use in the field, such as format, length and number of items, and types of response options available.MethodsQualitative data were collected from 23 EMTs and Adult Protective Services (APS) caseworkers that participated in one of five semi-structured focus groups. Focus group data were iteratively coded by two coders using inductive thematic identification and data reduction. Findings were subject to interpretation by the research team.ResultsEMTs and APS caseworks identified eight domains of items that might be included on a screening tool: (1) exterior home condition; (2) interior living conditions; (3) social support; (4) medical history; (5) caregiving quality; (6) physical condition of the older adult; (7) older adult’s behavior; and, (8) EMTs instincts. The screening tool should be based on observable cues in the physical or social environment, be very brief, easily integrated into electronic charting systems, and provide a decision rule for reporting guidance to optimize utility for EMTs in the field.ConclusionsWe described characteristics of a screening tool for EMTs to enhance detection and reporting of elder abuse and neglect to APS. Future research should narrow identified items and evaluate how these domains positively predict confirmed cases of elder abuse and neglect.

Abstract:Objectives Elder mistreatment (EM), encompassing abuse and neglect, is a significant public health issue, affecting up to 10% of community-dwelling older adults annually. Elder mistreatment is a growing concern with a higher prevalence in institutional settings and substantial associated healthcare costs. Prehospital clinicians (PHCs) such as emergency medical technicians and paramedics are uniquely positioned to detect and report EM during their interactions with older adults in their homes. The objective of the study is to describe the rate and characteristics of EM documented by PHCs using the National Emergency Medical Services Information System (NEMSIS) database.

Interventions targeting the elderly population to reduce emergency department utilisation: a literature review

Abstract:Objective: High utilisation of emergency department (ED) among the elderly is of worldwide concern. This study aims to review the effectiveness of interventions targeting the elderly population in reducing ED utilisation. Methods: Major biomedical databases were searched for relevant studies. Qualitative approach was applied to derive common themes in the myriad interventions and to critically assess the variations influencing interventions' effectiveness. Quality of studies was appraised using the Effective Public Health Practice Project (EPPHP) tool. Results: 36 studies were included. Nine of 16 community-based interventions reported significant reductions in ED utilisation. Five of 20 hospital-based interventions proved effective while another four demonstrated failure. Seven key elements were identified. Ten of 14 interventions associated with significant reduction on ED use integrated at least three of the seven elements. All four interventions with significant negative results lacked five or more of the seven elements. Some key elements including multidisciplinary team, integrated primary care and social care often existed in effective interventions, while were absent in all significantly ineffective ones. Conclusions: The investigated interventions have mixed effectiveness. Our findings suggest the hospital-based interventions have relatively poorer effects, and should be better connected to the community-based strategies. Interventions seem to achieve the most success with integration of multi-layered elements, especially when incorporating key elements such as a nurse-led multidisciplinary team, integrated social care, and strong linkages to the longer-term primary and community care. Notwithstanding limitations in generalising the findings, this review builds on the growing body of evidence in this particular area.
DISPARITIES BETWEEN THREE DIFFERENT ELDER ABUSE ASSESSMENT METHODS

Abstract:To the Editor: We applaud the Journal of the American Geriatrics Society's attention to the global health issue of elder abuse and read with great enthusiasm the article by Cohen et al.1 The authors found in the cross-sectional analyses of hospitalized patients in Israel that there are disparities between self-reported elder abuse, professional identification for evident signs of abuse, and expanded indicator of abuse (E-IOA) assessment of high risk of abuse. The authors underscored significant disparities in detection methods and called for multiple assessment methods to simultaneously detect elder abuse. However, we have some thoughts and questions that might suggest the possibility of overestimation of elder abuse cases from the professional identification of evident signs of abuse and the E-IOA assessment of high risk for abuse. Elder self-neglect is the most common type of elder abuse reported to social services agencies in the United States, with studies suggesting that 45% to 77% of reports are of elder self-neglect cases.2 Although the study by Cohen et al. did not include self-neglect, we caution that the study design may have inadvertently captured self-neglect cases and thus potentially included self-neglect cases as elder abuse cases. The integral question is the definition of caregiver and caregiver responsibilities that might constitute neglect. The authors stated that information from the patient and family member determined caregiver status during the hospital stay, but the authors did not document how many patients were living with the caregiver or the amount of exposure the family member needed to have with the patient before determining caregiver status. Prior study has defined caregiver status by the total hours of caregiving exposure per week for the older person.3 In the study by Cohen et al., does the family member who lives with the patient have similar caregiving responsibility as a family member who does not live with the patient and visits only occasionally? Does the patient who lives alone and refuses self-care and adopts dangerous lifestyle choices resulting in self-neglect without the awareness of the family member experience caregiver neglect? We do not believe that the professional identification of evident signs of abuse or the E-IOA assessment of risk for abuse took these factors into consideration before the determination of neglect or its risks. Specifically, the authors stated that 52.1% of caregivers shared a household, but it is unclear whether that is shared with the patient. It appears that a significant proportion of caregivers live alone. Furthermore, it is unclear how many patients lived alone, especially because the authors stated that almost 48% of patients were not married. For patients who live alone, how did the study distinguish what is self-neglect from caregiver neglect when using the professional identification of evident signs of abuse or the E-IOA methods of assessing high risk for abuse. In the professional assessment of evident signs of abuse, the criteria for neglect included assessment using 12 items, such as under- or overmedication, hypothermia, poor hygiene, appropriate clothing, and signs of dehydration. These symptoms and signs can also be the presenting symptoms and signs of elder self-neglect.4 The same question applies to the E-IOA methods, where the indicators are about social isolation, and lack of support. These indicators are frequently found in elder self-neglect5 and could have increased the E-IOA risk scores of patients. Although the E-IOA indicators of risk for abuse are collected regarding caregivers, it is unclear whether these “caregivers” live with the patient and whether the caregivers have caregiving responsibilities. In cases in which patients live alone, without a clear definition of a responsible caregiver, there lies a great possibility for detecting self-neglect, rather than caregiver neglect, especially considering these two assessment methods. Given the authors' findings that caregiver neglect was the most frequently identified abuse category by professionals' assessment of evident signs of abuse (67.3%), it is possible that symptoms and signs of self-neglect could have unintentionally significantly driven up the number of cases by professional assessment of evident signs of elder abuse as well as higher high-risk scores of abuse. We believe that these above issues not only may widen the perceived disparities of three different elder abuse assessment methods, but also lead to other methodological issues of studying elder abuse.6 We agree that older people are more exposed to abuse than they are willing to admit, but we also feel that, until those issues are further clarified, we should proceed with caution before deciding on the degree of disparities in assessment methods and that three assessment tools are needed for optimal identification of abuse.
