Abstract:Brucellosis, a significant zoonotic disease, threatens food safety substantially, particularly in developing nations such as the Middle East. This study aimed to comprehensively assess the prevalence of Brucella spp. in Iranian milk and dairy products through a systematic review and meta-analysis. A thorough search of international and domestic databases from January 2008 to October 2023 identified 38 relevant studies encompassing 11,130 samples for meta-analysis. The overall prevalence of Brucella spp. in Iranian dairy products was 22% (95% CI: 16-28%). The highest and lowest overall prevalence of Brucella spp. in milk were found in raw goat milk 27% (95% CI: 11-42%) and raw camel milk 15% (95% CI: -0.42 to 72%), respectively. The overall prevalence of traditional cheese, ice cream, and cream is estimated to be 9% (95% CI: -16 to 35%), 2% (95% CI: -2.78 to 2.82%), and 9% (95% CI: -0.94 to 1.12%). Geographical disparities were evident, with Zanjan province reporting the highest contamination rate, 53%, while Razavi Khorasan province had the lowest, 1%. However, the prevalence of Brucella spp. in Iranian dairy products has fluctuated over time, with a significant association between the study year and sample size. Comprehensive planning, robust policy implementation, and rigorous monitoring are imperative to mitigate and ultimately eliminate Brucella contamination in dairy products effectively. Further research is essential to refine prevalence estimates and develop targeted prevention strategies to safeguard public health.

Simulation results have shown that in heavy traffic, time-controlled (4-D) guidance in the aircraft will be necessary to realize the full potential of fuel conservative procedures. This paper focuses on the analysis of the 4-D guidance problem in the longitudinal plane. Singular Perturbation Theory is applied to reduce the computational burden of solving the resulting optimal control problem.
Serological, cultural, and molecular analysis of Brucella from Buffalo milk in various regions of Iran

Abstract:Brucellosis is a significant infection that causes abortion, decreased milk production, and sterility in livestock, which greatly affects the industry. This study aimed to determine the prevalence of Brucella in buffalo milk samples across various regions of Iran, utilizing serological, molecular, and cultural analyses. A total of 1860 buffalo milk samples were collected from industrial, semi-industrial, and traditional buffalo farms in four major buffalo breeding provinces. The milk ring test agglutination test (MRT) was initially conducted on all milk samples, followed by culture and molecular testing for positive and negative samples in MRT. The study revealed positive results for the presence of Brucella DNA in various provinces of Iran. The MRT had a relatively low sensitivity, with results ranging from 0 to 0.7% in different provinces. However, the AMOS PCR method showed a significantly higher presence of Brucella DNA, ranging from 13 to 46% in these provinces. The highest abundance of Brucella bacterial DNA was found in Ardabil province, while the lowest was in West Azerbaijan province. Brucella abortus was the most commonly detected bacteria, followed by Brucella melitensis. Interestingly, the B. abortus vaccine strain RB51 was detected in 26.3% of positive samples of B. abortus. The culture assay of milk samples further confirmed the presence of B. melitensis biovar 1 in one sample from Khuzestan province. Overall, the study emphasizes that the AMOS PCR method is the most sensitive in detecting Brucella-exposed milk, while the sensitivity of milk sample culture and MRT is relatively lower.
Seroprevalence and risk factor analysis of brucellosis among dairy farmers in Aligarh region, North India: creating awareness of a neglected disease

Abstract:Introduction. Brucellosis, a globally distributed zoonotic disease, is caused by the Gram-negative bacteria known as Brucella. Humans acquire infection through direct contact with the blood, urine and placenta of animals, inhalation of dust or aerosols at infected animal farms, and raw milk and meat intake. This study aimed to assess the prevalence of brucellosis in dairy farmers in and around the Aligarh region of North India, to document various clinical signs and symptoms in Brucella-positive individuals, and to create awareness in dairy farmers concerning brucellosis and ways to prevent it. Methods. This was an observational study that included 125 dairy farmers in and around the Aligarh region. Serum samples were taken from this high-risk group after obtaining informed consent. Further, a pre-designed proforma was used to collect information about their knowledge, attitude and practices (KAP) concerning brucellosis and assess the risk factors for the disease. The Rose Bengal test (RBT), serum agglutination test (SAT) and enzyme-linked immunosorbent assay (ELISA) were performed to detect the seroprevalence of brucellosis. Result.Brucella infection was diagnosed in 64 (51.20 %) cases by indirect ELISA (IgM+IgG), 41 (32.8 %) by RBT and 4 (3.2 %) by SAT. Significant clustering of patients was seen in the 20-55 years age group. The most common symptoms in ELISA IgM-positive patients were joint pain (16.07 %), fatigue (14.28 %), anorexia (12.50 %), weight loss (8.92 %), malaise (5.35 %), undulant fever (3.57 %), night sweats (3.57 %) and headache (1.78 %). The findings of this study indicate that ELISA (IgM+IgG) exhibits great sensitivity as compared to SAT and RBT. KAP was very poor among dairy farmers. Conclusion. In India, Brucella is a frequent but severely underreported illness. ELISA is the most sensitive serological test for diagnosing brucellosis. No potential vaccine has yet been introduced for humans against brucellosis. Thus, it is necessary to impart awareness and sensitize high-risk groups concerning brucellosis.
Risk factors and control strategies for Brucella spp. and RB51 vaccine shedding in buffalo milk: A cross-sectional study

Abstract:Brucellosis is prevalent in water buffaloes, which serve as significant reservoirs. The disease adversely affects meat and dairy products and can be transmitted to humans through contaminated products. This study aimed to determine the risk factors and control strategies for Brucella spp. and RB51 vaccine shedding in raw buffalo milk. A cross-sectional study was conducted across 261 farms where only 182 out of 261 farms received regular vaccinations through PCR tests and a questionnaire survey. The survey addressed potential risk factors and farm management. Univariable and multivariable logistic regression analyses were used to analyze the associated risk factors, represented as odds ratios (OR) with 95 % confidence intervals. The PCR survey revealed a 35.2 % prevalence of Brucella spp at the household level and 30.8 % at the buffalo level. Significant risk factors for Brucella PCR-positivity included the number of buffaloes in a household (OR = 1.512, 95 %CI: 1.107–2.065, p = 0.009) and the presence of resident dogs (OR = 2.157, 95 %CI: 1.053–4.417, p = 0.036). Regular vaccination practices were identified as a preventive factor (OR = 0.304, 95 %CI: 0.172–0.537, p <0.001). No animal-level risk factors were identified, and reduced-dose brucellosis vaccination showed a protective effect. Higher buffalo numbers in a household correlated with increased vaccine shedding in milk (OR= 2.489, p-value <0.001). Shedding was lower in households with primary education farmers than those without education (OR=14.163, p-value <0.001). Borrowing bulls for insemination also increased shedding (OR=13.266, p-value= 0.015). The study highlights the need for targeted control strategies, emphasizing education, vaccination, and hygiene practices to mitigate brucellosis's impact on buffalo and human populations.

Brucella abortus contamination of camel milk in two Iranian regions

Abstract:Brucella spp. commonly infect humans in various regions worldwide. Human brucellosis mainly spreads through the consumption of contaminated raw dairy products and meat from domestic livestock (water buffalo, goats, sheep, cattle, pigs and camels). In this regard, the origin and routes of transmission of this bacterium should be carefully determined in order to control the source of infection. This study aimed to evaluate the rate of Brucella spp. contamination of camel milk samples sent for analysis to the national brucellosis laboratory during 2018 in Iran. For this purpose, 96 milk samples from 96 dairy camel herds were randomly collected from two provinces and investigated for the presence of Brucella spp contaminations by both bacterial culture method and polymerase chain reaction (PCR). No clinical manifestation of brucellosis was reported in camels from which milk samples were collected. Using the culture method, three milk samples (3%) originating from two camels of Isfahan province (4%) and one camel from the Semnan province (2%), were contaminated with Brucella abortus. According to PCR analyses, B. abortus gene was detected in 14 (14.5%) milk samples, including 9 and 5 samples from Isfahan (18%) and Semnan (11%) province, respectively. PCR method revealed significant differences (p = 0.02) in the level of contamination with B. abortus between milk samples collected from two regions. These results represent the first report regarding the isolation of B. abortus from raw camel milk in Iran and highlight the importance to screen apparent healthy camels. Therefore, the consumption of raw camel milk may contribute to the spread of human brucellosis in endemic regions.
Brucellosis in the Middle East: Current situation and a pathway forward

Abstract:Brucellosis is a bacterial endemic zoonotic disease of global significance with detrimental impacts on public health and food animal production. It is caused by Brucella spp., an expanding group of pathogens able to infect various host species. Bovines and small ruminants, which excrete the bacteria in milk and in reproductive discharges, are major sources of infection for humans and other animals. Contact with contaminated animals and consumption of unpasteurized dairy products are the main routes for human infection. In spite of the considerable progress of knowledge gained and success achieved in brucellosis control in the developed world, this disease continues to be an important burden in the Middle East (ME). Common risk factors implicated in the difficulty and complexity of brucellosis control within the region include (1) social and political instabilities; (2) insufficient resources and infrastructure for appropriate diagnosis, reporting, and implementation of control measures; (3) variation of livestock husbandry systems and their commingling with other livestock and wildlife; and (4) traditional cultural practices, including consumption of unpasteurized dairy products. Development of core interdisciplinary competencies is required for a true One Health-based endeavor against the disease. National awareness and educational programs addressing all population sectors from consumers to decision-makers seem to be the next logical, sustainable, and economically viable approach toward improving disease status in this region. In the present review, we describe the current situation of brucellosis in the ME, focusing on the major limitations and shortcomings regarding disease control. We propose a regional approach toward public awareness of brucellosis as the first step in mitigating the disease and discuss the potential benefits, and components of such a strategy, which can further be used as a model for other endemic zoonotic diseases.
Characterization of Brucella spp. circulating in industrial dairy cattle farms in Iran: a field study 2016 - 2023

Abstract:Bovine brucellosis, an infectious disease transmitted by Brucella melitensis and Brucella abortus, presents a significant zoonotic risk for agricultural economics and animal health. The primary objective of this study was to present a comprehensive understanding of the prevalence and features of Brucella strains within the industrial dairy farming sector in Iran. Rose Bengal plate test, standard agglutination test, and indirect enzyme linked immunosorbent assay tests were used to confirm all seropositive animals. A total number of 1,311 bovine samples from seropositive animals including were collected from 224 farms in 21 provinces of different regions of Iran and examined. The discovered Brucella isolates were phenotyped and molecularly characterized. The isolates were all B. abortus or B. melitensis. Bacteria analysis revealed that 70.53% of seropositive farms were tested positive for Brucella strains, predominantly B. melitensis biovar 1 (43.42%) and B. abortus biovar 3 (27.11%). Geographical distribution revealed that B. melitensis biovar 1 was the most common in dairy cow farms (16 provinces), followed by B. abortus biovar 3 (six provinces). Also, the prevalence of B. melitensis biovar 2, B. melitensis biovar 3, B. abortus biovar 1, B. abortus biovar 2 and RB51 vaccine were restricted to certain provinces. AMOS (abortus melitensis ovis suis)-polymerase chain reaction and Bruce-ladder PCR confirmed species identification. These results highlighted the complexity of bovine brucellosis in Iran and illustrated that B. melitensis was spread from small ruminants to cattle. This study provided important epidemiological insights for targeting future brucellosis control programs in the Iranian dairy farms.
A scoping review on the epidemiology and public significance of Brucella abortus in Chinese dairy cattle and humans

Abstract:Brucellosis, caused by Brucella spp., is a re-emerging One Health disease with increased prevalence and incidence in Chinese dairy cattle and humans, severely affecting animal productivity and public health. In dairy cattle, B. abortus is the primary causative agent although infections with other Brucella species occur occasionally. However, the epidemiological and comparative importance of B. abortus in dairy cattle and humans remains inadequately understood throughout China due to the heterogeneity in locations, quality, and study methods. This scoping review aims to describe the changing status of B. abortus infection in dairy cattle and humans, investigate the circulating Brucella species and biovars, and identify factors driving the disease transmission by retrieving publicly accessible literature from four databases. After passing the prespecified inclusion criteria, 60 original articles were included in the final synthesis. Although the reported animal-level and farm-level prevalence of brucellosis in dairy cattle was lower compared to other endemic countries (e.g. Iran and India), it has been reported to increase over the last decade. The incidence of brucellosis in humans displayed seasonal increases. The Rose Bengal Test and Serum Agglutination Test, interpreted in series, were the most used serological test to diagnose Brucella spp. in dairy cattle and humans. B. abortus biovar 3 was the predominant species (81.9%) and biovar (70.3%) in dairy cattle, and B. melitensis biovar 3 was identified as the most commonly detected strain in human brucellosis cases. These strains were mainly clustered in Inner Mongolia and Shannxi Province (75.7%), limiting the generalizability of the results to other provinces. Live cattle movement or trade was identified as the key factor driving brucellosis transmission, but its transmission pattern remains unknown within the Chinese dairy sector. These knowledge gaps require a more effective One Health approach to be bridged. A coordinated and evidence-based research program is essential to inform regional or national control strategies that are both feasible and economical in the Chinese context.

Abstract:Purpose: To evaluate the risk of postimplantation syndrome associated with endovascular aneurysm sealing (EVAS) and endovascular aneurysm repair (EVAR) in patients treated for abdominal aortic aneurysm (AAA). Methods: From December 2013 to May 2015, 41 AAA patients treated with EVAS (mean age 72±9 years; 38 men) and 63 with EVAR (mean age 74±10 years; 55 men) at a single center were retrospectively reviewed. To control for treatment selection bias, propensity score matching was used to compare outcomes by treatment mode. Main outcomes were postimplantation syndrome (defined as temperature >38°C and leukocyte count >12,000/µL), inflammatory response markers [platelets and high-sensitivity C-reactive protein (hs-CRP)], and clinical complications through 30 days. Results: In 39 matched patients per group, the incidences of postimplantation syndrome (p=0.07), mean body temperature (p=0.05), mean leukocyte count (p=0.003), and mean hs-CRP (p<0.001) were proportionally lower with EVAS vs EVAR. Serious adverse events (0% vs 12.8%, p=0.05) and endoleaks (0% vs 10.3%, p=0.13) through 30 days were less frequent with EVAS, but the group differences were not significantly different. The choice of endovascular graft material influenced postoperative and 30-day clinical outcomes, with greater overall risk observed with polyester stent-grafts. Conclusion: Endovascular aneurysm sealing is associated with a blunted systematic inflammatory response compared with EVAR. Polyester stent-grafts induce the greatest periprocedural inflammatory response.

Abstract:Brucella spp. as animal pathogens causes human brucellosis, the most momentous zoonotic diseases all over the world that result in economic pressure, human morbidity, and poverty. Brucellosis is commonly transmitted by infected dairy products. The prevalence of Brucella spp. and high consumption of unpasteurized dairy products in urban areas of Iran imposes further studies to clarify the prevalence of Brucella species. This molecular survey was purposefully designed to determine the prevalence of Brucella in raw milk collected from milk sellers using PCR in Zanjan province, Iran. In this molecular cross-sectional study, 73 raw milk samples with a volume of 40 ml gathered (6 samples from sheep and 67 samples from bovine) from raw milk vendors in Zanjan province. DNA of bacteria was extracted from the raw milk samples using (GeneALL, South Korea) kit according to the manufacturer protocol. Specific primers (B4 and B5) were used to the amplification of BCSP31 gene. Five primers (Ba-sp, Bm-sp, Bo-sp, Bs-sp, and IS711-sp) were used to detect Brucella spp. (Brucella abortus, Brucella melitens, Brucella ovis, and Brucella suis) among the positive samples for bcsp31 gene. From the 73 raw milk samples collected, 38 cases (52%) were positive for Brucella genus, 3 cases (7.8%) belonged to Brucella abortus and 2 cases (5.2%) belonged to Brucella melitensis. There were no positive cases for both Brucella ovis and Brucella suis. Results of this study proved that the prevalence of Brucella in raw milk is still a threat to public health, which should have a deeper look at the control strategies and methods of the prevention of brucellosis in the livestock.
Prevalence, diagnosis, and manifestations of brucellosis: A systematic review and meta-analysis

Abstract:Objectives: Brucellosis is one of the most prevalent zoonotic diseases common between humans and animals. Despite eradication efforts, the burden of the disease is well-known in endemic countries and in countries where brucellosis has not been an important health issue until recently. The aim of this study was to evaluate the prevalence, diagnosis, and manifestations of brucellosis. Methods: In this study, PubMed, Web of Science, Scopus, Embase, and Google scholar databases were systematically searched to find studies published from 2011 to 2021. The search was conducted using text words and Medical Subject Headings (MeSH) Terms on the prevalence of brucellosis. Stata software 14.0 was used for all analyses. Results: Based on the results, the pooled prevalence of brucellosis was 15.27% (95% CI: 9.68–21.86; heterogeneity I 2 index: 97.43; p < 0.001) for man and 15.33% (95% CI: 7.19–25.75; heterogeneity I 2 index: 98.19; p < 0.001) for woman. Age (coefficient: 0.240; p = 0.480), gender (coefficient: −0.017; p = 0.800), and publication year (coefficient: 0.114; p = 0.861) showed no significant effect on heterogeneity among studies. Egger's test indicated a significant publication bias for the prevalence of brucellosis (coefficient 3.894; p < 0.001). Moreover, the trim-and-fill method exhibited that the adjusted prevalence of brucellosis (18.30%, 95% CI: 14.10–22.52) was not significantly different from the original prevalence of brucellosis. Conclusion: The pooled estimate for brucellosis prevalence was estimated as 15.53%. To better understand the epidemiology of brucellosis globally, more extensive studies are needed to be conducted throughout the world, especially in developing and low-income countries.

Abstract:Introduction: Brucellosis is a zoonotic disease with worldwide distribution. It is considered endemic in Eritrea, however, the current prevalence status and related risk factors in animals are unknown. The objective of this study was to determine the prevalence of and risk factors for brucellosis in dairy cattle in Maekel and Debub regions, Eritrea. Methods: A cross sectional study was conducted between August 2021 and February 2022. A total of 2,740 dairy cattle from 214 herds in 10 sub-regions of Eritrea were selected for blood and data collection. Blood samples were tested using Rose Bengal Plate Test (RBPT) and positive samples were confirmed using competitive (c-ELISA). Data on risk factors was collected using questionnaire and analyzed using logistic regression. Results: In total, 34/2740 animals tested positive by RBPT. Of these, 29 were confirmed positive by c-ELISA, giving an apparent and estimated true individual-level prevalence of 1.1% (95% CI: 0.7, 1.5%) and 1.3% (95% CI: 0.9, 1.8%), respectively. Sixteen herds (7.5%) tested positive by RBPT and of these 15 herds (7.0%) were confirmed positive by c-ELISA, giving an estimated true herd-level prevalence of 7.0% (95% CI: 4.0, 10.7). Animal and herd-level apparent prevalence was 1.6 and 9.2% in Maekel, while in Debub it was 0.6 and 5.5%, respectively. Multivariable regression analysis indicated that non-pregnant lactating cows (adjusted odds ratio [aOR] = 3.35; p = 0.042) were more likely to be Brucella sero-positive. History of abortion on the farm (aOR = 5.71; p = 0.026) and larger number of cows in the herd (aOR = 1.14; p < 0.001) were associated with brucellosis sero-positivity in herds. Conclusion: Brucellosis prevalence was low in the study areas. Nonetheless, this low prevalence may increase if the disease is not controlled. Therefore, testing animals before movement, good farming practices, sanitary measures, and an awareness raising program on brucellosis are recommended.

Abstract:Acute calcific longus colli tendinitis (ACLCT), a very rare cause of severe neck pain, dysphagia and odynophagia, is often mistaken for other common causes of neck pain. However, prompt recognition of this uncommon presentation is important to prevent unnecessary medical and surgical intervention. A 46-year-old Caucasian man presented with a 1-day history of severe neck pain, headache and odynophagia. The patient was afebrile with stable vital signs, however, the laboratory data showed mildly elevated C reactive protein and erythrocyte sedimentation rate. The physical examination was remarkable for markedly reduced cervical range of motion. MRI revealed the pathognomonic findings of paravertebral oedema and calcification. The definitive diagnosis of ACLCT was made and the patient was successfully managed with a short course of oral steroid, benzodiazepine and aural acupuncture, with complete resolution of the condition within a week.

Abstract:The water buffalo (Bubalus bubalis) in Iran represent an important source of meat and milk products with high biological value. Given the importance of water buffalo in Iran and the prevalence of brucellosis as one of the most important zoonotic diseases in this ruminant species, this study summarized available data on history, epidemiology, diagnosis, and control of brucellosis in water buffalo from previous studies that have been carried out in Iran. According to the documented data, there are three main groups of Iranian buffalo, including the Khuzestan ecotype (Khuzestan province); the Azary ecotype (Western/ Eastern Azarbaijan and Ardabil provinces); and the North ecotype (Gylan and Mazandaran provinces). Preliminary studies conducted on Iranian buffaloes either by serological or molecular tools reported that buffaloes' infection occurred due to natural exposure to a wild strain of Brucella abortus and Brucella melitensis. Previous studies dealing with brucellosis in Iranian buffalo are next to none. This review notifies the importance of reliable and detailed epidemiological investigations of Iranian buffaloes through continuous monitoring systems of the health status of buffalo populations. Continuous test and slaughter strategy, vaccination, and re-planning of veterinary activities are required to mitigate buffalo's role in disseminating and maintaining brucellosis in Iran.

Abstract:Brucellosis is a zoonotic disease of great animal welfare and economic implications worldwide known since ancient times. The emergence of brucellosis in new areas as well as transmission of brucellosis from wild and domestic animals is of great significance in terms of new epidemiological dimensions. Brucellosis poses a major public health threat by the consumption of non-pasteurized milk and milk products produced by unhygienic dairy farms in endemic areas. Regular and meticulous surveillance is essentially required to determine the true picture of brucellosis especially in areas with continuous high prevalence. Additionally, international migration of humans, animals and trade of animal products has created a challenge for disease spread and diagnosis in non-endemic areas. Isolation and identification remain the gold standard test, which requires expertise. The advancement in diagnostic strategies coupled with screening of newly introduced animals is warranted to control the disease. Of note, the diagnostic value of miRNAs for appropriate detection of B. abortus infection has been shown. The most widely used vaccine strains to protect against Brucella infection and related abortions in cattle are strain 19 and RB51. Moreover, it is very important to note that no vaccine, which is highly protective, safe and effective is available either for bovines or human beings. Research results encourage the use of bacteriophage lysates in treatment of bovine brucellosis. One Health approach can aid in control of this disease, both in animals and man.

Abstract:Brucellosis continues to be a serious infection to human and animal populations in developing countries with detrimental impacts on public health and food animal production. This work aimed to estimate the prevalence of brucellosis in sheep and goats’ raw milk samples at Kirkuk Governorate, Iraq by identifying anti-Brucella antibodies and isolation of Brucella species. A total of 430 raw milk samples (210 sheep milk and 220 goats’ milk) were randomly collected from dairy females during the period from July to December 2019. The results showed an overall prevalence of Brucella antibodies in 12.3% and 10.7% of animals according to MRT and indirect ELISA, respectively. The overall isolation of Brucella species from raw milk samples was 10.0%. The isolated species of Brucella were B. abortus (37.2%) and B. melitensis (62.8%). An observable increase in occurrence during autumn (September to November) was detected, while autumn progress was associated with declining in brucellosis. In conclusion, brucellosis is still a significant public health hazard in Kirkuk Governorate. Based on test performance, the study recommends MRT as a rapid screening test for detecting brucellosis in milk in farms, centres, and dairy factories. Consumers are also recommended to sufficiently pasteurize the milk in order to kill this milk-borne pathogen before consumption.
