Occupational diseases in the automotive industry in Czechia - Geographic and medical context

Abstract:Objectives: The automotive industry represents the most important industrial sector in Czechia. The objective of this study has been to analyze the occurrence of occupational diseases (OD) in the automotive industry during the period from 2001 till 2014. Material and methods: Data on OD cases was retrieved from the National Registry of OD. Further, we conducted a survey in automotive companies with focus on occupational health services and positions of the companies in global production networks (GPNs). An analysis of OD distribution in the automotive industry was performed (age, gender, company size and its role in GPNs, regional distribution of studied companies, and regional unemployment rate), and was accompanied by assessment of the quality and range of occupational health services. Results: Employees older than 40 years old have nearly 2.5 times higher probability of OD occurrence as compared with employees younger than 40 years old (odds ratio (OR) = 2.41, 95% confidence interval (CI): 2.05-2.85). Occupational diseases occurrence probability was 3 times higher for women than for men (OR = 3.01, 95% CI: 2.55-3.55). Occupational diseases incidence rates increased with the size of the company (0 OD/10 000 employees in micro enterprises to 57 OD/10 000 employees in large enterprises). A particularly steep rise in OD incidents in the automotive industry was observed in the Plzeň Region between 2001 and 2011. An association between OD incidents and the unemployment rate was not statistically confirmed. Conclusions: A statistically significant increase in OD incidents dependent on company size may be arguably attributed to a higher quality of occupational medical services in bigger companies, which ensures better detection and diagnosis of OD. In the Plzeň Region, the rapid increase in OD incidents was mainly caused by a change in the production process of automobile textiles in one factory due to the introduction of a glue containing isocyanates, which are potent allergising agents. Occupational respiratory disease in Eastern Slovakia between 1990-2021: a shift from agriculture to industrial manufacturing

Abstract:Objectives: Occupational allergic respiratory diseases frequently occur in individuals working in the agricultural and food production sectors, textile manufacturing, and industries involving exposure to isocyanates. The study aimed to describe trends surrounding the prevalence of occupational asthma (OA), occupational rhinitis (OR), and occupational hypersensitivity pneumonitis (OHP) in Eastern Slovakia between 1990-2021. Methods: All cases of OA, OR, and OHP registered in a database at the Louis Pasteur University Hospital in Košice, Slovakia, between 1990 and 2021, were divided into categories based on economic sector (agricultural, food production sectors, textile manufacturing, healthcare, industrial manufacturing, and tertiary sector) and causal agent. Changes in disease prevalence, causal agents, and economic sector association over time were analysed. Results: There were 287 occupational respiratory cases (179 OA, 65 OR, and 43 OHP cases). The annual prevalence of OA declined significantly over the study period (p < 0.05). Overall, there was a significant decrease in cases from the agricultural (p < 0.001) and an increase in the industrial manufacturing (p < 0.01). The number of cases due to farming agents fell markedly over the study period, while metalworking fluids (MWFs) were found to be the most common causes of allergic respiratory diseases since 2018. Conclusions: This study found a decrease in the number of OA cases, as well as changes in economic sectors and causal agents associated with OA and OHP, specifically, in the agricultural sector, with MWFs from the industrial manufacturing sector now being the most common aetiological agent.

Abstract:Attention on work-related musculoskeletal disorders (WMSDs) involves statistical surveys showing an increasing trend in the incidence of WMSDs. Technological development has led to new tools and methods for the assessment of physical load at work. These methods are mostly based on the direct sensing of appropriate parameters, which allows more precise quantification. The aim of this paper is to compare several commonly used methods in Slovakia for the assessment of ergonomic risk reflecting current EU and Slovak legislative regulations. A Captiv wireless sensory system was used at a car headlight quality control assembly workplace for sensing, data acquisition and data processing. During the evaluation of postures and movements at work, we discovered differences in the applicable standards: Decree 542/2007 Coll. (Slovak Legislation), the STN EN 1005-4+A1, and the French standards default in the Captiv system. Standards define the thresholds for hazardous postures with significant differences in several evaluated body segments, which affects the final evaluation of the measurements. Our experience from applying improved risk assessment methodology may have an impact on Slovak industrial workplaces. It was confirmed that there is a need to create uniform standards for the ergonomic risk assessment of body posture, including a detailed description of the threshold values for individual body segments.

Abstract:Introduction: Forestry provides a wide range of employment opportunities worldwide and is seen as one of the high-risk industries in terms of occupational accidents. Objectives: The submitted study analyzed the injury rate in the Military Forests and Estates of the Slovak Republic (62.6 thousand ha) between 2013 and 2022. Methods: The data analyses included regression and correlation analyses, χ 2 tests to analyze the relationships between studied variables, and incidence rates. Results: During the observed period, employees suffered 26 occupational accidents, of which 19.2% were light, 57.7% were registered, 23.1% were severe, and 0% were fatal. For every 1 million m3 of harvested timber, 7.7 accidents occurred. The incidence rate during the observed period was 672.1/100,000 employees. The highest proportion of accidents was in the age group 51–60 years and in employees with the lowest length of work experience <5 years. Regarding time, the highest proportion of occupational accidents occurred between 8:01 and 10:00 AM (53.8%) and day-wise on Thursdays (46.2%). The highest proportion of accidents occurred among forest workers (65.3%) during pruning and silviculture activities (42.3%). The most common injury site was forest stands (65.3%). Superficial injuries (34.6%) were the most common, mainly affecting the lower limbs (50%). The most frequent material agents causing the accidents were work and transport areas as sources of worker fall (38.5%), and the most frequent reason for an accident to occur was the lack of personal requirements for proper work performance (92.4%), whereas only (3.8%) of accidents occurred due to the use of forbidden or hazardous working procedures. Conclusion: The presented study identified the most vulnerable worker groups and provided an overview of the overall injury rate at the state forest company in Slovakia. The documentation can be incorporated into the safety strategies of forest enterprises.

Long-term temporal analysis of fatal and severe occupational accidents in Central European forests of the Slovak Republic

Abstract:Introduction: Forestry is one of the most dangerous industries worldwide. Workers handle heavy loads and are exposed to numerous adverse factors of the work environment. This study was focused on analyzing severe and fatal occupational accidents over a long period. Methods: Data were gathered from occupational safety and health databases of the State Forests of the Slovak Republic, which manages 879,113 ha of forests in Slovakia. Between 2007 and 2021, 210 severe and64 fatal accidents were recorded. The data analyses included contingency tables, Pearson χ2 test, and Cramer's V coefficient to analyze the relationships between studied variables. Results: The mean annual number of fatalities was 14, occurring to workers aged 42 on average. Almost 80% of all fatal accidents were suffered by contractors of harvesting operations, who were 5.23 times more likely to suffer a fatality than company employees. On average, 1.28 severe and0.75 fatal accidents occurred per 1 million m3 of harvested timber. The outcomes of the correlation analysis showed a decreasing trend in incidence in the case of severe accidents. Workers ≤ 30 years or between 51 and 60 years of age were the most vulnerable in the case of fatal accidents. In the case of severe accidents, workers between 51 and 60 were the most vulnerable group. Most fatal accidents were suffered by tree fellers (72.2%) during felling (69.8%), who were in contact with falling trees and tree parts (93.7% share for this material agent). Conclusion and Practical Applications: The presented study identified the most vulnerable worker groups as well as the effects of factors on the occurrence of severe and fatal occupational accidents in forestry. The results will serve in development of occupational safety and health strategies in forest enterprises.

Abstract:Introduction: The automotive industry is characterized by a complex and combined workers’ exposure to occupational risk factors. The problem of early detection, course and development of prognostic criteria for modern forms of occupational pathology remains relevant. Objective: To assess the main workplace hazards in the automotive industry and the risk of occupational diseases in its workers. Materials and methods: We conducted our own occupational health studies, examined production control data (348 units) and results of special evaluation of working conditions (97 units) at an automobile manufacturing enterprise of the Republic of Bashkortostan in 2020–2023. In total, we took 554 measurements of noise, 554 – of hand-arm vibration, 448 – of chemicals, and 554 – of work heaviness. To assess occupational risk, we used an integral indicator of the frequency and severity of occupational diseases, the index of occupational diseases (IOD), as the inverse value of the categories of risk (Cr) and severity (Cs), a single-digit indicator (IOD), combining Cr and Cs. Calculations were done in Microsoft Excel. Results: In the course of work, employees are exposed to a combination of physical factors, such as noise (Class 3.2) and hand-arm vibration (Class 3.1); chemical factors, including fibrogenic aerosols, metal dusts, toxicants from welding fumes, paints and varnishes (Class 3.1), and physical overload (Class 3.1). The overall working conditions correspond to Hazard Class 3, degrees 1–2. We assessed risks of developing the following occupational diseases (IOD = 0.16, total IOD = 0.32): vibration disease from exposure to hand-arm vibration and sensorineural hearing loss from noise exposure; chronic poisoning with manifestations of disorders of the pulmonary system (toxic bronchitis and chronic obstructive pulmonary disease) related to chemical exposures. Occupational diseases had a mild course with minor functional grade 1 manifestations and no progression of the pathological process. Conclusions: The complex and combined exposure to industrial safety hazards in the automotive industry determines the moderate risk of developing occupational diseases in its workers.
Protecting Occupational Health and Safety in Construction Companies in the Czech Republic

Abstract:Currently it is the third year when the global COVID-19 pandemic is having a negative impact on the lives of individuals, on the activities of economic entities of all sizes and on the economies of countries around the world. Following the partial calming in this area, a crisis linked to the war in Ukraine hit Europe in early 2022. This has a negative impact on economic production and the associated decline in the standard of living. The prices of materials, products and transport are rising, causing a sharp increase in construction prices in the construction sector. Protecting the occupational health of workers and providing a safe environment for their work form an essential part of all construction projects. This article deals with the research into occupational health and safety on construction sites in the Czech Republic. The research described in this article was conducted in several successive steps. In the first step, a research design was drawn up, in the second step data collection was carried out, and in the third step data analysis and compilation of results were carried out. Qualitative approaches to data collection and analysis, namely in-depth interviews and the coding method, were used in the companies under research. Open-ended questions relating to respondents' opinions, experience and overall perceptions of the issues were developed in the preparatory phase of the research. The aim was to obtain credible answers to the questions raised. The research lasted for six months and involved 19 medium and large companies from the Czech Republic. The purpose of the research described in this article was to find out the situation focusing on providing the health and safety of workers during the implementation of construction. The costs of implementing the necessary measures in this field were also investigated.

Abstract:Environmental health is among the priority areas of public health, and the current professional communities are intensively engaged with it. The main objective of the study is to quantify regional disparities of preventable mortality in Slovakia and to study the extent of the influence of selected environmental factors on changes in the development of its values. A cross-sectional linear regression model is used to quantify effects of environmental factors on the preventable mortality. Also, cluster analysis is used to identify regions with similar levels of air pollution. Environmental factors were selected based on the study of the World Health Organization. From the point of view of the influence of environmental factors on preventable mortality in the case of men, statistically significant connection to sewerage, SO2 production, and production of particulate matter was demonstrated. In the case of women, equally important factors showed connection to sewerage and SO2. The results of this study point to significant regional disparities in preventable mortality and a different degree of impact of environmental factors. Preventable mortality is above the Slovak average in most of the least-developed districts. Even in this group, there are significant differences.

Abstract:Introduction: Risk assessment and management in companies plays a significant role in the prevention section of any field. In the field of Occupational Health and Safety (OHS), its inconsistent or incorrect application has a direct impact on the life and health of employees. In some companies, even today, it is not properly implemented and adequate procedures and methods are not used. The article discusses the development of a step-by-step procedure for risk assessment in industrial environments in the area of OHS. Methods: Main parts of the model and its steps present the partial results of a survey conducted on a sample of 500 small and micro enterprises in the field of risk assessment and the systematic procedure developed following the main survey results. The survey covered only enterprises located in the construction, manufacturing, transport and storage and agriculture, forestry and fishing sectors, which is also a significant statistical feature. Within the structure of respondents, statistical features such as: size of enterprise, sector, region by work are identified. Only enterprises with size by number of employees - micro enterprises from 1 to 9 employees and small enterprises from 10 to 49 employees - were included for the survey. Results: New elements of the methods were integrated into the developed systematic procedure, which was subsequently validated in 7 plants of the one company on the same position. The application of the developed model was verified by an expert group consisting of 7 members, an odd number, and the developed checklists and risk register were applied. On the basis of the verification, the model, checklist and risk register were corrected. In addition, the scoring method and the risk matrix were also used, but they did not contain new elements. Discusion: The procedure is still in use today and employees have been trained to use it. On the basis of the developed methodology and the Checklist, the procedure has been transposed into the European OiRA tool and can be used by companies throughout the European Union.

Abstract:For the individual and for society, human life and health are great values, which are often particularly difficult or impossible to measure, though they are irreplaceable values. Even in the oldest historical stages of the development and life of society, many important doctors and scientists were interested in the social aspects of health care, e.g. Hippocrates, Aristotle, Avicenna. Paracelsus († 1541, Swiss philosopher and physician) and Georgius Agricola († 1555, German chemist and mineralogist). Based on their own experience gained from daily contact with workers and their working environment, they examined disorders caused by the harmful factors of the working environment and working processes. The ever-changing world of work increasingly brings new risks into working processes, which are directly related to the work performed by employees. As part of the EU-OSHA (European Agency for Safety and Health at Work) strategy to increase health and safety at work, the European Risk Observatory called on EU Member States to improve the detection of these risks in the working environment. Considering the above, the authors of this article have set themselves the goal of mapping the current situation in the area of the creation, protection and quality management of the working environment. They describe general requirements for protecting occupational health and safety, mainly in Slovakia. They are convinced that current developments present the need to know as much as possible about the living and working environments, to have a thorough knowledge of working conditions and all the factors that affect the health of employees, to know and choose assessment procedures to reduce the negative effects of these factors to a minimum. It is important that we keep in mind that healthy working conditions are among the most valuable assets of individuals, communities and nations.
Health Conditions and Long Working Hours in Europe: A Retrospective Study

Abstract:Studies have shown that working conditions and employee health are related; therefore, this study aimed to determine the relationships between working long hours per week with health outcomes in different European countries. We analyzed data derived from the Survey of Health, Ageing, and Retirement in Europe. The sample included 12.099 participants aged ≥ 50 from 16 European countries. We analyzed self-reported working hours, health outcomes of blood cholesterol, heart attack, diabetes, and hypertension, as well as a longitudinal analysis of health outcomes among healthcare workers and workers in 13 other industries. Statistical analyses identified that men are working longer hours per week compared with women in all included countries, and different levels of increase in health conditions in different industries. We also observed a slower increase in the prevalence of health conditions for healthcare workers compared with workers in other industries, especially diabetes and hypertension. The largest increase in prevalence of observed health conditions was reported for cholesterol, which increased for 17.14% among healthcare workers and for 21.70% in other industries over the observed nine-year period. Although the data point to a potentially high level of awareness in the field of preventive health among healthcare workers, more preventive health activities should be included in workplaces to strengthen employees’ health.

Abstract:Considering thermal environment aspects have a major impact not only on occupational health and safety (OH&S) performance but also on the productivity and satisfaction of the workers, the aim of the case study was to assess the thermal comfort of a group of 33 workers in an automotive industry company, starting with collecting data about the thermal environment from different workplaces, continuing with the analytical determination and interpretation of thermal comfort using the calculation of the Predicted Mean Vote (PMV) and Predicted Percentage of Dissatisfied (PPD) indices, according to provisions of the standard ISO 7730:2005, and comparing the results with the subjective perception of the workers revealed by applying individual questionnaires. The results of the study represent an important input element for establishing the preventive and protective measures for the analysed workplaces in correlation with the measures addressing other specific risks and, also, could serve as a model for extending and applying to other similar workplaces in future studies. Moreover, the mathematical model and the software instrument used for this study case could be used in further similar studies on larger groups of workers and in any industrial domain.

Occupational diseases in Murmansk Oblast: 1980-2010

Abstract:Background: Official statistics tend to underestimate the incidence of occupational disease (OD) nationally and regionally in Russia. Objectives: The general aim was to obtain an accurate estimate of ODs in Murmansk Oblast in 1980-2010 and to determine the rate of specific types of ODs among cohorts of workers who had been exposed to the hazardous factors causing the disease. Materials and methods: Data were retrieved from the Murmansk Oblast ODs database for the oblast and 2 enterprises--Apatite JSC and Kolskaya MSC--which contributed to more than half of the ODs in the oblast in 1980-2010. The total number of ODs and 5 specific categories (musculoskeletal, respiratory, nervous diseases, hearing loss and vibration disease) were analysed. Results: THE TOTAL RATE OF ODS AMONG WORKERS OF MAIN SHOPS IN BOTH ENTERPRISES WHO WERE ACTUALLY EXPOSED TO HARMFUL FACTORS WERE EXTREMELY HIGH: the rate for Apatite JSC was 25 times higher than in Russia and 15 times higher than in Murmansk Oblast, while the rate for Kolskaya MSC was about 30 and 20 times greater than in Russia and in Murmansk Oblast, respectively; in the 2000s the difference reached 100-150 times. The rise in reported ODs in both enterprises corresponded to the time when intensive medical examinations were conducted by the Kola Research Laboratory for Occupational Health (KRLOH) in Kirovsk. A similar pattern was also observed for the sub-categories of musculoskeletal, respiratory, nervous diseases, hearing loss and vibration disease. It is likely that the true burden of OD is even higher due to misdiagnosis, reluctance of workers concerned about job security to present for care and the lack of reliable information on working conditions needed to establish a causal link between disease and occupational exposure. Conclusions: As with many other regions across Russia, ODs in Murmansk Oblast are grossly underestimated. Serious problems exist in the Russian occupational health care system and the collection of occupational health statistics that require urgent, fundamental reform.

Abstract:Background: Occupational skin disease (OSD) is a common occupational disease. Although primary prevention strategies are known, OSDs remain prevalent in a variety of work environments including the service sector (restaurant/food services, retail/wholesale, tourism/hospitality and vehicle sales and service). Aims: To obtain information about awareness and prevention of OSD in the service sector. Methods: Focus groups and a survey were conducted with two groups. The first consisted of staff of the provincial health and safety association for the service sector and the second group comprised representatives from sector employers. Focus groups highlighted key issues to inform the survey that obtained information about perceptions of awareness and prevention of OSD and barriers to awareness and prevention. Results: Both provincial health and safety association staff and sector employer representatives highlighted low awareness and a low level of knowledge of OSD in the sector. Barriers to awareness and prevention included a low reported incidence of OSD, low priority, lack of training materials, lack of time and cost of training, lack of management support and workplace culture. Conclusions: A starting point for improving prevention of OSD in the service sector is increased awareness. Identification of the barriers to awareness and prevention will help to shape an awareness campaign and prevention strategies. Building on existing experience in Europe will be important.
The Impact of the COVID-19 Pandemic on the Quality of Examination in Eye Clinics in the Czech Republic - Questionnaire Study

Abstract:Purpose: The aim of the study was to map the behavior of ophthalmologists regarding protective equipment during the COVID-19 pandemic (coronavirus disease 2019), both during the time of the mandatory restrictive measures and after their relaxation. Another aim was to evaluate the awareness of ophthalmologists in the Czech Republic about the possible impact of nose and mouth protective measures (masks, respirators) on the quality of eye examinations, especially on the results of standard automated perimetry (SAP) and intraocular pressure (IOP) measurement. Materials and methods: As part of two professional ophthalmological events in the Czech Republic, which took place in 2022, we obtained and evaluated data from the ophthalmologists in attendance using a questionnaire. We evaluated demographic parameters, frequency of use and type of nose and mouth protective equipment and their influence on the quality of ophthalmological examination as well as the awareness of ophthalmologists about their possible influence on the outcome of SAP and IOP measurements. Results: We obtained data from a total of 212 respondents (148 women, 44 men, in 20 cases gender was not stated). In 91.5% of cases, ophthalmologists agreed that the use of respirators and masks makes ophthalmological examination more difficult. The most common problems were eyepiece fogging (85.8%), examination lens fogging (85.8%), and lens fogging when spectacles correction was prescribed (79.2%). The respondents most often combated these problems either by completely removing the respirator (24.1%) or at least by pulling it under the nose (39.2%). At the time when the measures were relaxed, significantly more men did not use any nose and mouth protection at all during ophthalmological examinations (15.8% of men vs. 4.2% of women; p = 0.032). An alarming finding was the fact that 35.6% of respondents did not know whatsoever whether the nurse was performing a perimetry examination on a patient with a respirator/mask or without protective equipment, i.e. they were not aware whatsoever of the possible formation of artifacts. Only 21.2% of respondents were aware of the possible difficulties of measuring IOP while wearing a respirator, while 59.9% of respondents were not aware of this risk (39.6% had never considered this problem, 20.3% of respondents were convinced that a respirator could not have an effect on the measurement of IOP). Conclusion: The use of nose and mouth protective equipment clearly affects the ophthalmological examination and makes it more difficult. Although ophthalmologists belong to a group at high risk for the possible transmission of infection in the performance of their profession, they often removed nose and mouth protection in an effort to eliminate fogging of eyepieces and examination lenses. The awareness of ophthalmologists regarding the possible influence on the results of SAP and IOP measurement by wearing a respirator was low in our questionnaire survey. It is therefore advisable to discuss this issue more widely and warn doctors about these risks.
The Influence of Occupational Categories on Overall and Domain-Specific Physical Activity and the Association with Chronic Diseases. An Analysis Using the Austrian Health Interview Survey

Abstract:Background: The performance of physical activity (PA) in different domains varies between different occupational groups and they contribute differently to the prevention and management of chronic diseases. This study aimed to give a fuller picture of the potential influence occupational categories have on the different domains of PA among the Austrian population of working age. Methods: A total of 8251 gainfully employed persons in 9 major and 39 sub-major occupational groups from the Austrian Health Interview Survey 2014 were analyzed. PA was measured with the Physical Activity Questionnaire of the European Health Interview Survey (EHIS-PAQ) and the prevalence of 17 chronic diseases was obtained. Results: A total of 48.2% were mostly active when working, 18.4% reported transport-related PA in the upper quintile, 50.4% performed at least 150 min per week of moderate PA or cycling, 32.7% performed muscle-strengthening PA at least twice a week, and 76.3% were either mostly physically active when working or complied with the aerobic PA guidelines. As a general rule, people in physically active occupational groups tended to perform less PA in their leisure time and vice versa. Occupational groups with especially low amount of PA were Information Technology workers, directors, and secretarial staff. People with a chronic disease tended to perform less PA, but there was an interaction between occupation and chronic disease on PA. Conclusions: Domain-specific programs to promote PA should be developed for various occupational categories.
