Work expectations, their fulfillment, and exhaustion among radiologists of all career levels: what can be learned from the example of Germany

Abstract:Objectives: To evaluate work expectations of radiologists at different career levels, their fulfillment, prevalence of exhaustion, and exhaustion-associated factors. Methods: A standardized digital questionnaire was distributed internationally to radiologists of all career levels in the hospital and in ambulatory care via radiological societies and sent manually to 4500 radiologists of the largest German hospitals between December 2020 and April 2021. Statistics were based on age- and gender-adjusted regression analyses of respondents working in Germany (510 out of 594 total respondents). Results: The most frequent expectations were "joy at work" (97%) and a "good working atmosphere" (97%), which were considered fulfilled by at least 78%. The expectation of a "structured residency within the regular time interval" (79%) was more frequently judged fulfilled by senior physicians (83%, odds ratio (OR) 4.31 [95% confidence interval (95% CI) 1.95-9.52]), chief physicians (85%, 6.81 [95% CI 1.91-24.29]), and radiologists outside the hospital (88%, 7.59 [95% CI 2.40-24.03]) than by residents (68%). Exhaustion was most common among residents (physical exhaustion: 38%; emotional exhaustion: 36%), in-hospital specialists (29%; 38%), and senior physicians (30%; 29%). In contrast to paid extra hours, unpaid extra hours were associated with physical exhaustion (5-10 extra hours: OR 2.54 [95% CI 1.54-4.19]). Fewer opportunities to shape the work environment were related to a higher probability of physical (2.03 [95% CI 1.32-3.13]) and emotional (2.15 [95% CI 1.39-3.33]) exhaustion. Conclusions: While most radiologists enjoy their work, residents wish for more training structure. Ensuring payment of extra hours and employee empowerment may help preventing burnout in high-risk groups. Key points: • Most important work expectations of radiologists who work in Germany are "joy at work," a "good working atmosphere," "support for further qualification," and a "structured residency within the regular time interval," with the latter containing potential for improvement according to residents. • Physical and emotional exhaustion are common at all career levels except for chief physicians and for radiologists who work outside the hospital in ambulatory care. • Exhaustion as a major burnout criterion is associated with unpaid extra hours and reduced opportunities to shape the work environment.

Abstract:We report a rare case of a hepatic carcinosarcoma with rabdomyosarcomatous differentiation in its sarcomatous component. A 71-year old Caucasian female patient underwent a liver resection for a 4-cm lesion developed on an underlying HCV-related cirrhosis. Post-operative course was uneventful and the patient was discharged 5 days after surgery. At pathology, the tumor presented the features of hepatocellular carcinoma and rhabdomyosarcoma Three months later the patient experienced a liver recurrence, dying 2 months later for systemic disease. The reported case presents several peculiarities, i.e. the female gender, the HCV-related cirrhotic status, and the European origin of the patient. However, the outcome of our case confirms that this neoplasm pursues a highly aggressive course with poor prognosis.

Abstract:Background: Demographic changes in the society and among doctors, as well as changing attitudes towards and norms of how living should be structured are creating challenges regarding the organization of work environment in the hospital. In addition, organization of medical training is increasingly being influenced by economic considerations as well as a high level of medical specialization. We asked young respiratory physicians how they assessed their current situation with respect to quality of medical training and organization of their work environment. Methods: From September to November 2019, we performed an online survey adressing young respiratory physicians in Germany. Participants were recruited via three emails (baseline and reminders after 2 and 6 weeks) sent by the German Respiratory Society (DGP) and the German Union of Pulmonologists (BdP). The questionnaire consisted of a maximum of 62 questions. Apart from own questions that had been aligned with other questionnaires from similar surveys in other medical specialties, we also assessed the effort-reward ratio (ER ratio) based on the short version of the effort-reward imbalance questionnaire (16 questions). Results: We recuited n = 224 participants (33.8 ± 4.5 years, 5.4 ± 2.9 years of medical training, 54.4 % female , 86.8 % with German nationality). A little under half of the interviewees (n = 103, 46 %) reported to be very or generally satisfied with their working conditions, while n = 60 (27 %) were unsure. The main reasons for not being satisfied were long working hours and high work-load, as well as a lack of streamlining the work environment in the hospital to the specific needs of doctors. Despite the fact that many participants were satisfied, a large majority (n = 166, 88.2 %) depicted an unfavorable effort-reward ratio imbalance (adjusted mean 1.89 ± 2.18). Conclusion: Compared to many other European countries and internationally, the German healthcare system offers high-quality patient care and a well-equipped work environment. Increasing demands in the health care sector, however, are leading to a gratification crisis that not only harms the health and work performance of doctors but is also leading to reduced attractiveness of the job that might possibly lead to the search for new fields of activity or migration. Respiratory medicine is a discipline of growing interest and motivating young doctors to secure the promotion of this discipline is increasingly important. Factors harming the growth of this discipline should be immediately addressed. The results of this survey might help leaders in the field to restructure the work environment and medical education according to the actual needs.

Abstract:Investigation of motivation and identification of success factors in radiology research in Germany.Using a German online survey (54 questions, period: 3.5 months), demographic aspects, intrinsic and extrinsic success characteristics, as well as personal and organizational success factors were surveyed based on a career success model. The survey results were reported descriptively. The correlations between success factors and success characteristics were examined using linear, binary-logistic, and multinomial regression models.176 people (164 academically active, 10 not academically active) answered the survey. Most participants (80%, 139/174) worked at a university hospital. 32% had privatdozent or professor as their highest academic title (56/173). The researchers' main motivation was intrinsic interest in research (55%, 89/163), followed by a desire to increase their own career opportunities (25%, 41/163). The following were identified as factors for intrinsic success: i) support from department management (estimate=β=0.26, p<0.001), ii) good work-life balance (β=0.37, p<0.001), and iii) the willingness to pursue science even after reaching the career goal (β=0.16, p<0.016). Relevant factors for extrinsic scientific success were mentoring, protected research time, and activities in professional societies.Researchers in German radiology are mainly intrinsically motivated. Factors known from the literature that determine intrinsic and extrinsic scientific success were confirmed in this study. Abstract:Background: Radiation science is of utmost significance not only due to its growing importance for clinical use, but also in everyday life such as in radiation protection questions. The expected increase in cancer incidence due to an aging population combined with technical advancements further implicates this importance and results in a higher need for sufficient highly educated and motivated personnel. Thus, factors preventing young scientists and medical personnel from entering or remaining in the field need to be identified. Methods: A web-based questionnaire with one general and three occupation-specific questionnaires for physicians, biologists, and medical physicists working in radiation oncology and research was developed and circulated for 6 weeks. Results: While the overall satisfaction of the 218 participants was quite high, there are some points that still need to be addressed in order to ensure a continuing supply of qualified personnel. Among these were economic pressure, work-life balance, work contracts, protected research time, and a demand for an improved curriculum. Conclusion: Mentoring programs, improved education, and strengthening the value of societies in radiation sciences as well as translational approaches and more flexible working arrangements might ensure a high-quality workforce and thus patient care in the future.
The impact of the radiology career on family planning: A survey of practicing radiologists and trainees

Abstract:Purpose: As the field of medicine witnesses evolving attitudes towards work-life balance, barriers to family planning emerge as an important theme. Though these challenges have been investigated in many fields, there has been little work done on this subject within radiology. Here we present the first formal survey of radiologists on topics related to family planning. Materials and methods: In this single-institution prospective study, a 40-question comprehensive survey was developed and distributed via email. Responses from 76 participants were analyzed. Results: Of the 76 respondents, a diverse number of ages, points in the career path, and practice settings were represented. A majority of respondents were male (52/76; 68 %) and married (56/75; 73.7 %). Respondents reported a miscarriage rate of 25 %, which is slightly higher than the reported rate for the general population of 20 %. Significantly more female respondents reported a negative stigma associated with being pregnant as a radiologist as compared to their male colleagues (60.9 % vs. 15.4 %; p < 0.001)). Male respondents reported significantly less parental leave than their female colleagues, most commonly reporting zero weeks of leave as compared to 10 weeks for female respondents (p < 0.001). Numerous respondents cited lack of childcare support as a major issue. Conclusion: We have identified several key areas of concern, including a need for improving parental leave policies, addressing pregnancy stigma, and increasing access to childcare support. Overall, our study lays the groundwork for discussions and policy changes within radiology at both the institutional and national level to ensure the continued interest of trainees and satisfaction of radiologists.

Abstract:This article presents the results of a nationwide survey addressing the status quo of gender equality and family friendliness within German interventional radiology (IR) with a focus on clinical entry and career development. All members of the German Society for Interventional Radiology and Minimally Invasive Therapy (DeGIR) were invited to participate in an online survey between November 2021 and February 2022. The survey consisted of 39 questions on demographic information, marital and parental status, experiences with entry and promotion in IR, family friendliness, and equality. A descriptive analysis of the responses was conducted. 197 surveys from female (n=76; 39%) and male (n=121; 61%) interventional radiologists at various stages of training and career were analyzed. Fewer female attending physicians (58%) and chiefs of departments (23%) lived with children compared to male attending physicians (76%) and chiefs of departments (55%). Fewer men (4%) than women (41%) were primarily responsible for childcare. More female (55%) than male (6%) attending physicians worked part-time. Women rated entry into IR as more difficult than men. 55% of women felt disadvantaged by their gender (men: 6%); this was due to assumptions that women "perform less than men" (46%), "drop out due to family commitments" (35%) and that "men are promoted preferentially" (19%). 54% believed that it is more difficult for women to combine work and family. The reasons for this are "family commitments" and "lack of flexible work schedules and childcare". Parents with primary responsibility spent less than 50% of their working time on clinical interventions. Fathers from a younger generation were more likely to take parental leave than fathers of a previous generation (52% vs. 17%). Similar numbers of men (51%) and women (55%) planned to work part-time in the future. There are gender-specific differences in the German IR. To ensure sustainable recruitment of young talent, measures such as the normalization of flexible work schedules should be introduced to create equal conditions for women and men, as well as mothers and fathers, and thus take changing (family) structures adequately into account. In diesem Artikel werden die Ergebnisse einer deutschlandweiten Umfrage vorgestellt, die sich mit dem Status Quo der Gleichstellung und Familienfreundlichkeit innerhalb der Interventionellen Radiologie (IR) mit Fokus auf den klinischen Berufseinstieg und die Karriereentwicklung beschäftigt. Alle Mitglieder der Deutschen Gesellschaft für Interventionelle Radiologie und minimal-invasive Therapie (DeGIR) wurden zwischen November 2021 und Februar 2022 zu einer Online-Umfrage eingeladen. Die Umfrage bestand aus 39 Fragen zu demografischen Angaben, Familienstand und Erfahrungen mit dem Einstieg und der Förderung in der IR, Familienfreundlichkeit und Gleichstellung. Es erfolgte eine deskriptive Auswertung der Antworten. 197 Antwortbögen von weiblichen (n=76; 39%) und männlichen (n=121; 61%) Interventionsradiologen aus verschiedenen Ausbildungs- und Berufsphasen wurden analysiert. Gründe hierfür sind "Familienverpflichtungen" sowie "mangelnde flexible Arbeitszeitmodelle und Kinderbetreuung". Primärverantwortliche Eltern verbrachten weniger als 50% der Arbeitszeit mit klinischen Interventionen. Väter unter 45 Lebensjahren nahmen häufiger Elternzeit als Väter älter als 45 Jahre (52% vs. 17%). Ähnlich viele Männer (51%) und Frauen (55%) planten zukünftige Teilzeitarbeit. Es gibt geschlechtsspezifische Unterschiede in der deutschen IR. Um eine nachhaltige Nachwuchsförderung sicherzustellen, sollten Maßnahmen wie die Normalisierung flexibler Arbeitszeitmodelle eingeleitet werden, um gleiche Bedingungen für Frauen und Männer und auch Mütter und Väter zu schaffen, und so sich verändernde (Familien-)Strukturen angemessen zu berücksichtigen.

Abstract:Introduction In 2016, the Royal College of Radiologists (RCR) launched an initiative to address the radiology workforce crisis in the UK, focusing on increasing the number of training places. To address this, the RCR advocated for the introduction of skill mix and expanded imaging training academies. The NHS's 2023 long-term workforce plan proposed increased integration of medical associate professions (MAPs) in radiology, although this has raised concerns regarding regulation and training quality. Methods We conducted an online survey targeting UK radiologists to gather insights on their perceptions of training quality, recent modifications to training, and views on non-radiologist roles. The purpose of the study was to evaluate the ways in which these elements affect their training experiences and general contentment. Results A total of 227 responses were collected, predominantly from residents across all UK training deaneries. 63.9% of trainees expressed satisfaction with their training although this varied, with trainees in new/hybrid academies reporting lower satisfaction. Concerns about training quality were prevalent, attributed to numerous factors such as consultants working from home and increased training numbers. Although opinions on the new curriculum were divided, many commented on the challenges of meeting the new IR competences, and the greater focus on general training was perceived more adversely. Career prospects in radiology were often bleak. Conclusion The survey results highlighted a multifaceted array of perceptions and concerns amongst radiology residents regarding several factors negatively impacting their training, such as changes to the curriculum and growth of non-radiologist roles.

Abstract:Radiology departments with the large diagnostic devices CT and MRI contribute significantly to the overall energy consumption of health facilities. However, there is a lack of systematic knowledge about the opinions of radiological staff on the most relevant aspects of sustainability. For this reason, we conducted a comprehensive survey for radiology employees on sentiment and experiences regarding sustainability in radiology. In collaboration with the Sustainability Network of the German Roentgen Society (DRG), we developed a questionnaire on various dimensions of sustainability in radiology. We conducted a nationwide online survey of radiology employees between July 1 st , 2023 and November 30 th , 2023. The absolute and percentage distributions were then determined. From 109 participants, mainly doctors (67/109; 62%) from university hospitals (48/109; 44.0%), 81 out of 109 rated sustainability in professional environment (74.3%) as important or very important. However, only 38 out of 109 (38%) of the respondents were able to name specific sustainable procedures in their institute. The most important topics for a sustainable radiology were waste management (26/109, 22.6%), energy reduction (19/109, 16.5%), conscious behaviour (15/109, 13%) and reduction of obsolete examinations (14/109, 12.2%). In addition, a lack of qualifications (16%), finances (21%) and compliance (21%) were named as challenges for the implementation of sustainable actions in radiology. The perceived importance of specific, sustainable measures in radiology is generally higher than the amount of already established actions. Radiology has significant, yet untapped, potential for sustainable optimization. There is a need for qualified and sensitized health care workers in radiology who are committed to sustainability in everyday clinical practice. Among other things, in this study the respondents demand a more critical indication for diagnostic workup, including avoiding redundant examinations, and a technological progress towards energy-efficient devices, which requires a dynamic exchange between radiology, industry and health care facilities. Die Radiologie trägt insbesondere mit ihren diagnostischen Großgeräten einen nicht unerheblichen Anteil am energetischen Gesamtverbrauch der Gesundheitseinrichtungen bei. Allerdings fehlt es an systematischen Erkenntnissen über die Meinung radiologischer Mitarbeiter:innen zu den relevantesten Aspekte der Nachhaltigkeit. Aus diesem Grund führten wir eine umfassende Befragung von radiologischem Personal zu den Einstellungen und Erfahrungen bezüglich Nachhaltigkeit in der Radiologie durch. In Zusammenarbeit mit dem Netzwerk Nachhaltigkeit der Deutschen Röntgengesellschaft (DRG) entwickelten wir einen kompakten Fragebogen zu verschiedenen Dimensionen der Nachhaltigkeit in der Radiologie. Zwischen dem 1. Juli 2023 und dem 30. November 2023 führten wir eine nationale Online-Befragung unter radiologischem Personal durch. Die absoluten und prozentualen Verteilungen wurden anschließend ermittelt. Von den 109 Teilnehmenden, mehrheitlich Ärzte:innen (67/109; 62%) aus Universitätskliniken (48/109; 44,0%), bewerteten 81 von 109 Nachhaltigkeit im beruflichen Umfeld (74,3%) als wichtig oder sehr wichtig. Die empfundene Bedeutung spezifischer, nachhaltiger Maßnahmen in der Radiologie ist übergreifend höher als die bisher etablierten Maßnahmen. Die Radiologie besitzt großes, bisher unausgeschöpftes, Potenzial für nachhaltige Optimierung. Es besteht ein Bedarf an qualifiziertem und sensibilisiertem radiologischem Personal, das sich für Nachhaltigkeit im klinischen Alltag engagiert. Unter anderem werden ein technologischer Fortschritt hin zu energieeffizienteren Geräten und eine kritischere Indikationsstellung sowie Vermeidung redundanter Untersuchungen von den Befragten gefordert, was eine Kooperation von Radiologie, Industrie, Ärzteschaft und Praxis-/Krankenhausträgern erfordert.

Occupational orthopedic problems and its relation to personal radiation protection in interventional radiology

Abstract:Purpose Several studies report occupational orthopedic problems among interventional cardiologists. These health problems are usually multifactorial. However, the personal protective equipment used should play a major role. An online survey was conducted to determine the frequency of such health problems among interventional radiologists and to correlate them with the use of personal radiation protective clothing. Material and Methods An anonymous online survey that comprised of 17 questions was sent via e-mail to 1427 members of the German Society for Interventional Radiology (DeGIR) in Germany, Austria and Switzerland. The questions were focused on interventional workload, the use of personal radiation protection apparel and orthopedic problems. Given the different scale levels, the associations between the variables were analyzed using different statistical methods. A significance level of p < 0.05 was chosen. Results There were 221 survey responders (15.5 % response rate). About half of responders (47.7 %) suffered from more than five episodes of orthopedic problems during their interventional career. Lumbar spine was involved in 81.7 % of these cases, cervical spine in 32.8 %, shoulder in 28.5 % and knee in 24.7 %. Because of orthopedic problems, 16.1 % of the responders had to reduce and 2.7 % had to stop their interventional practice. The number of affected body regions correlates with the fit of the radiation protection means (p < 0.05, r = 0.135) and the reduction of activity as an interventional radiologist (p < 0.05, r = −0.148). Conclusion Overall, the survey reveals widespread orthopedic problems at several body regions among interventional radiologists, associated with the fit of radiation protection systems, among other factors. A connection between the orthopaedic complaints and the radiation protection system used could not be established.

Abstract:Introduction Radiation oncology is a pivotal modality in the treatment of hematologic malignancies. To enable state-of-the-art patient care, structured education during residency is essential. However, given the lack of detailed data, the scope of educational opportunities available to trainees remains elusive. This prompted our group to perform a national survey amongst radiation oncology residents in Germany assessing the status quo of competences in the treatment of lymphoma and leukemia patients. Furthermore, areas of potential improvement were identified to further the goal of competence-based education for residents. Methods A survey-based analysis was conducted to assess the knowledge and competence of radiation oncology residents in Germany regarding hematological malignancies. A decisive questionnaire covering demographics, self-assessment of competences, and areas for improvement was developed in adaption of a survey by the Association of Residents in Radiation Oncology and distributed amongst 1439 members of the German Society of Radiation Oncology. Responses were collected anonymously via an online survey tool and analyzed using descriptive statistics and chi-square tests. Results A total of 59 complete and 22 partial responses were collected, yielding a 5.6% response rate. Participants' competence varied, with notable experience gaps in pediatric cases, proton therapy, and large-field techniques like total-skin irradiation or pediatric total body irradiation. While participants felt confident in treatment planning and patient counseling, they showed deficiencies in the definition of the planning target volume for modern involved site radiotherapy. Resources for education included national and international guidelines, scientific reviews, and textbooks. Board-certified radiation oncologists and physicians from specialized lymphoma centers demonstrated higher overall competence levels. Conclusion This survey highlights the diversity of resident education regarding hematological malignancies in German radiation oncology programs. Knowledge gaps exist in key areas, including pediatric cases and specialized techniques. Competence-based education, interactive teaching formats, and rotations to specialized centers are potential strategies to address these gaps. The study contributes to the understanding of the federal educational landscape, underscoring the need for standardized and comprehensive training to ensure optimal patient care in hematological malignancies within the context of radiation oncology. Further research and collaborations are warranted to enhance training and expertise in this critical domain.

Abstract:IntroductionRadiation protection is becoming more important with an ongoing increase in radiation exposure due to the use of X-rays in minimally invasive procedures in orthopaedic and trauma surgeries. However, sufficient education in medical physics and radiation protection can often be improved.Materials and methodsA questionnaire consisting of four questions about personal data and ten questions about radiation protection was distributed to lead consultants, consultants, residents, medical students, and medical technical assistants at two institutions, a level 1 trauma center and a children’s hospital.ResultsThis study consisted of 83 participants. The compliance with radiation protection, i.e., usage of a dosimetry, an apron, and a thyroid shield on a regular basis was only seen in 54 %. Participants from the trauma center wore a dosimeter and thyroid shield significantly more often. The regular use of a thyroid shield differed significantly between job positions. It was observed in 80 % of students, but only 15 % of technical assistants. Only 65 % of all knowledge questions were answered correctly. There was a discrepancy between incorrectly answered knowledge questions (35 %) and those marked as uncertain (20 %). Different job positions did not have an impact on the answers to the questions in most instances.ConclusionsThe compliance with and the knowledge about radiation protection seems to be unnecessarily low in trauma physicians and technical assistants. The discrepancy in falsely answered questions and those marked as uncertain may suggest that participants may overestimate their knowledge about radiation protection, which is potentially harmful due to the increased radiation exposure. Therefore, we advocate a quick and valuable training of trauma surgeons and medical staff addressing the important preventive measures, some of which are illustrated in the present study. These consist of wearing dosimetry and protection devices, reduction in X-ray duration, preferably antero-posterior C-arm positioning with the image intensifier close to the patient and the surgeon, maximal distance, collimation, and increased voltage. Furthermore, the use of visual feedback on complex and potentially hazardous radiation facts may be useful for training purposes.Study designCross-sectional study with a questionnaire.

Current status and developments of German curriculum-based residency training programmes in radiation oncology

Abstract:Abstract Purpose The current status of German residency training in the field of radiation oncology is provided and compared to programmes in other countries. In particular, we present the DEGRO-Academy within the international context. Methods Certified courses from 2018 and 2019 were systematically assigned to the DEGRO-Curriculum, retrospectively for 2018 and prospectively for 2019. In addition, questionnaires of course evaluations were provided, answered by course participants and collected centrally. Results Our data reveal a clear increase in curriculum coverage by certified courses from 57.6% in 2018 to 77.5% in 2019. The analyses enable potential improvements in German curriculum-based education. Specific topics of the DEGRO-Curriculum are still underrepresented, while others decreased in representation between 2018 and 2019. It was found that several topics in the DEGRO-Curriculum require more attention because of a low DEGRO-curriculum coverage. Evaluation results of certified courses improved significantly with a median grade of 1.62 in 2018 to 1.47 in 2019 ( p = 0.0319). Conclusion The increase of curriculum coverage and the simultaneous improvement of course evaluations are promising with respect to educational standards in Germany. Additionally, the early integration of radiation oncology into medical education is a prerequisite for resident training because of rising demands on quality control and increasing patient numbers. This intensified focus is a requirement for continued high standards and quality of curriculum-based education in radiation oncology both in Germany and other countries.

Abstract:Background: The Competence Centre for Residency Training in Family Medicine Bavaria (CCRTB) was established to improve the quality of postgraduate medical training by offering additional seminars and mentoring programmes as well as regular 'train-the-trainer' courses for educating physicians. In addition, residents have the opportunity to participate in a regional training network. Objective: The aim was to assess the burden of burnout and the importance of the learning environment in the clinical training phase. Methods: We conducted a cross-sectional study. Burnout was assessed using the Maslach Burnout Inventory (MBI), which comprises the scales "Emotional Exhaustion", "Depersonalisation" and "Personal Accomplishment". The quality of the learning environment was recorded using the German version of the Dutch Residency Educational Climate Test (D-RECT German). In addition, multivariable linear regressions were performed to estimate the impact of learning environment, year of training and participation in a regional network on the level of burnout. Results: 129 clinical residents enrolled in the CCRTB were invited to participate in the study, 78 (61%) of whom submitted a response. 76 (59%) of these residents were included in the analyses. The present study discloses an increased burden of burnout among residents in the clinical training phase, with approx. 40% reaching a critical burnout score. A higher quality of the learning environment was associated with significantly milder burnout symptoms on the majority of the D-RECT scales. Conclusion: Family medicine residents in the clinical training phase show a high burden of burnout. Therefore, increasing the quality of the learning environment appears to be an effective key element in achieving a reduction of burnout in clinical training. This might contribute to an increase in professional satisfaction, which finally may also prevent migration from the medical profession.
