Abstract:The demand for outpatient services is rapidly growing, resulting in multifaceted challenges due to capacity limitations. This research aims to analyse the patient waiting time in a multi-specialty ophthalmic outpatient clinic using data analysis and discrete event simulation (DES). The patient arrivals, the duration for pre-consultation and post-consultation services are highly uncertain. At first, a linear regression analysis is performed using electronic health record (EHR) log data, and the significant factors that affect patient waiting time are found. Further, a discrete event simulation model of the outpatient clinic is built using FlexSim Healthcare software (5.3) and validated. Improvement scenarios, namely: 1) adding resources; 2) introducing fixed interval appointment scheduling; 3) combining scenarios 1 and 2, are proposed for reducing the patient waiting time and evaluated. From the simulation results, it is inferred that scenario 3 reduces the average waiting time of the patients to 12.45 minutes from 38.37 minutes.

Abstract:Nowadays, the increase in patient demand and the decline in resources are lengthening patient waiting times in many chemotherapy oncology departments. Therefore, enhancing healthcare services is necessary to reduce patient complaints. Reducing the patient waiting times in the oncology departments represents one of the main goals of healthcare managers. Simulation models are considered an effective tool for identifying potential ways to improve patient flow in oncology departments. This paper presents a new agent-based simulation model designed to be configurable and adaptable to the needs of oncology departments which have to interact with an external pharmacy. When external pharmacies are utilised, a courier service is needed to deliver the individual therapies from the pharmacy to the oncology department. An oncology department located in southern Italy was studied through the simulation model and different scenarios were compared with the aim of selecting the department configuration capable of reducing the patient waiting times.

Improving patient waiting times: a simulation study of an obesity care service

Abstract:BACKGROUND: Obesity care services are often faced with the need to adapt their resources to rising levels of demand. The main focus of this study was to help prioritise planned investments in new capacity allowing the service to improve patient experience and meet future anticipated demand.METHODS: We developed computer models of patient flows in an obesity service in an Academic Health Science Centre that provides lifestyle, pharmacotherapy and surgery treatment options for the UK's National Health Service. Using these models we experiment with different scenarios to investigate the likely impact of alternative resource configurations on patient waiting times.RESULTS: Simulation results show that the timing and combination of adding extra resources (eg, surgeons and physicians) to the service are important. For example, increasing the capacity of the pharmacotherapy clinics equivalent to adding one physician reduced the relevant waiting list size and waiting times, but it then led to increased waiting times for surgical patients. Better service levels were achieved when the service operates with the resource capacity of two physicians and three surgeons. The results obtained from this study had an impact on the planning and organisation of the obesity service.CONCLUSIONS: Resource configuration combined with demand management (reduction in referral rates) along the care service can help improve patient waiting time targets for obesity services, such as the 18 week target of UK's National Health Service. The use of simulation models can help stakeholders understand the interconnectedness of the multiple microsystems (eg, clinics) comprising a complex clinical service for the same patient population, therefore, making stakeholders aware of the likely impact of resourcing decisions on the different microsystems.

Abstract:This paper presented a discrete event simulation model for one hospital. This model was used to evaluate the current performance of the hospital, to perform bottleneck analysis of the patient process and to verify, base on simulation outputs, expanding alternatives that the hospital authority proposed. Different expanding alternatives were simulated and the simulation outputs obtained were used to provide the basis for selecting the best alternative. The implementation of the expanding alternative could decrease the waiting time of patients, increase patient throughput, and improve the service level of the hospital.

Abstract:Patient satisfaction and operational efficiency are critical in healthcare. Long waiting times negatively affect patient experience and hospital performance. Addressing these issues requires accurate system time predictions and actionable strategies. This paper presents a hybrid framework combining predictive modeling and optimization to reduce system times and enhance satisfaction, focusing on registration, vitals, and doctor consultation. We evaluated three predictive models: multiple linear regression (MLR), log-transformed regression (LTMLR), and artificial neural networks (ANN). The MLR model had the best performance, with an R2 of 0.93, an MAE of 7.29 min, and an RMSE of 9.57 min. MLR was chosen for optimization due to its accuracy and efficiency, making it ideal for implementation. The hybrid framework combines the MLR model with a simulation-based optimization system to reduce waiting and processing times, considering resource constraints like staff and patient load. Simulating various scenarios, the framework identifies key bottlenecks and allocates resources effectively. Reducing registration and doctor consultation wait times were identified as primary areas for improvement. Efficiency factors were applied to optimize waiting and processing times. These factors include increasing staff during peak hours, improving workflows, and automating tasks. As a result, registration wait time decreased by 15%, vitals by 20%, and doctor consultation by 25%. Processing times improved by 10–15%, leading to an average reduction of 22.5 min in total system time. This paper introduces a hybrid decision support system that integrates predictive analytics with operational improvements. By combining the MLR model with simulation, healthcare managers can predict patient times and test strategies in a risk-free, simulated environment. This approach allows real-time decision-making and scenario exploration without disrupting operations. This methodology highlights how reducing waiting times has a direct impact on patient satisfaction and hospital operational efficiency, offering an applicable solution that does not require significant structural changes. The results are practical and implementable in resource-constrained healthcare environments, allowing for optimized staff management and patient flow.

Abstract:The cardiotonic steroid, ouabain, a specific inhibitor of Na+,K+-ATPase, initiates protein-protein interactions that lead to an increase in growth and proliferation in different cell types. We explored the effects of ouabain on glucose metabolism in human skeletal muscle cells (HSMC) and clarified the mechanisms of ouabain signal transduction. In HSMC, ouabain increased glycogen synthesis in a concentration-dependent manner reaching the maximum at 100 nm. The effect of ouabain was additive to the effect of insulin and was independent of phosphatidylinositol 3-kinase inhibitor LY294002 but was abolished in the presence of a MEK1/2 inhibitor (PD98059) or a Src inhibitor (PP2). Ouabain increased Src-dependent tyrosine phosphorylation of α1- and α2-subunits of Na+,K+-ATPase and promoted interaction of α1- and α2-subunits with Src, as assessed by co-immunoprecipitation with Src. Phosphorylation of ERK1/2 and GSK3α/β, as well as p90rsk activity, was increased in response to ouabain in HSMC, and these responses were prevented in the presence of PD98059 and PP2. Incubation of HSMC with 100 nm ouabain increased phosphorylation of theα-subunits of the Na-pump at a MAPK-specific Thr-Pro motif. Ouabain treatment decreased the surface abundance of α2-subunit, whereas abundance of the α1-subunit was unchanged. Marinobufagenin, an endogenous vertebrate bufadienolide cardiotonic steroid, increased glycogen synthesis in HSMC at 10 nm concentration, similarly to 100 nm ouabain. In conclusion, ouabain and marinobufagenin stimulate glycogen synthesis in skeletal muscle. This effect is mediated by activation of a Src-, ERK1/2-, p90rsk-, and GSK3-dependent signaling pathway.

Abstract:The healthcare system is a complex system which exhibits conditions of uncertainty, ambiguity emergence that incurs incoming patient congestion. Discrete event simulation (FlexSim) is considered as a viable decision support tool in analyzing a system for improvement. Using a data-driven discrete event simulation approach, this paper portrays a comprehensive analysis to maximize the number of patients in an on-campus clinic, located at Mississippi State University. The outcome of the analysis of current system exhibits that deploying a few nurse practitioners results in bottlenecks which decreases the systems’ throughput substantially due to the overall longer patients’ waiting time. Access to the laboratory is characterized through multi-server queuing network, arrival process is followed discrete distributions, and batch sizes and arrival times are stochastic in nature. In an effort to plummet inpatient congestion at the outpatient clinic, by using empirically calibrated simulation model, we will figure out the best balance between the number of the lab technician and incoming patient during working hour. An analysis of optimal solutions is demonstrated, which is followed by recommendation and avenues for future research.

Abstract:Market research on services and waiting lends support to the fact that waiting for service is an undesirable phenomenon adversely affecting customer satisfaction and consequently business enterprises. The simulation optimization strategy described in this study addresses both the subjective as well as the objective elements in the patients’ evaluation of healthcare services. The cost function simulates the average sojourn time of the patients in the healthcare system, whereas the fuzzy constraints quantitatively estimate the overall waiting and service experience of the patients. Minimization of the cost function (objective element in the patients’ evaluation of service) subject to the fuzzy constraints (subjective elements in the patients’ evaluation of service) leads to an increased patient satisfaction, increased efficiency of system operation and consequently increased profits. The proposed method makes a topological cum functional model of the healthcare system, simulates the operation via discrete event simulation and optimizes it using the Genetic Algorithm.

Abstract:Emergency Departments (EDs) overcrowding is a well recognized worldwide phenomenon. The consequences range from long waiting times for visits and treatment of patients up to life-threatening health conditions. The international community is devoting greater and greater efforts to analyze this phenomenon aiming at reducing waiting times, improving the quality of the service. Within this framework, we propose a Discrete Event Simulation (DES) model to study the patient flows through a medium-size ED located in a region of Central Italy recently hit by a severe earthquake. In particular, our aim is to simulate unusual ED conditions, corresponding to critical events (like a natural disaster) that cause a sudden spike in the number of patient arrivals. The availability of detailed data concerning the ED processes enabled to build an accurate DES model and to perform extensive scenario analyses. The model provides a valid decision support system for the ED managers also in defining specific emergency plans to be activated in case of mass casualty disasters.

Abstract:Long wait times and crowding are major issues affecting outpatient service delivery, but it is unclear how these affect patients in dual practice settings. This study aims to evaluate the effects of changing consultation start time and patient arrival on wait times and crowding in an outpatient clinic with a dual practice system. A discrete event simulation (DES) model was developed based on real-world data from an Obstetrics and Gynaecology (O&G) clinic in a public hospital. Data on patient flow, resource availability, and time taken for registration and clinic processes for public and private patients were sourced from stakeholder discussion and time-motion study (TMS), while arrival times were sourced from the hospital’s information system database. Probability distributions were used to fit these input data in the model. Scenario analyses involved configurations on consultation start time/staggered patient arrival. The median registration and clinic turnaround times (TT) were significantly different between public and private patients (p < 0.01). Public patients have longer wait times than private patients in this study’s dual practice setting. Scenario analyses showed that early consultation start time that matches patient arrival time and staggered arrival could reduce the overall TT for public and private patients by 40% and 21%, respectively. Similarly, the number of patients waiting at the clinic per hour could be reduced by 10–21% during clinic peak hours. Matching consultation start time with staggered patient arrival can potentially reduce wait times and crowding, especially for public patients, without incurring additional resource needs and help narrow the wait time gap between public and private patients. Healthcare managers and policymakers can consider simulation approaches for the monitoring and improvement of healthcare operational efficiency to meet rising healthcare demand and costs.

Abstract:Abstract Background This study aimed to reduce the total waiting time for high-end health screening processes. Method The subjects of this study were recruited from a health screening center in a tertiary hospital in northern Taiwan from September 2016 to February 2017, where a total of 2342 high-end customers participated. Three policies were adopted for the simulation. Results The first policy presented a predetermined proportion of customer types, in which the total waiting time was increased from 72.29 to 83.04 mins. The second policy was based on increased bottleneck resources, which provided significant improvement, decreasing the total waiting time from 72.29 to 28.39 mins. However, this policy also dramatically increased the cost while lowering the utilization of this health screening center. The third policy was adjusting customer arrival times, which significantly reduced the waiting time—with the total waiting time reduced from 72.29 to 55.02 mins. Although the waiting time of this policy was slightly longer than that of the second policy, the additional cost was much lower. Conclusions Scheduled arrival intervals could help reduce customer waiting time in the health screening department based on the “first in, first out” rule. The simulation model of this study could be utilized, and the parameters could be modified to comply with different health screening centers to improve processes and service quality.

Abstract:In this paper, a simulation model of an emergency department (ED) at a large community hospital, Central Baptist Hospital in Lexington, KY, is developed. Using such a model, we can accurately emulate the patient flow in the ED and carry out sensitivity analysis to determine the most critical process for improvement in quality of care (in terms of patient length of stay). In addition, a what-if analysis is performed to investigate the potential change in operation policies and its impact. Floating nurse, combining registration with triage, mandatory requirement of physician's visit within 30 min, and simultaneous reduction of operation times of some most sensitive procedures can all result in substantial improvement. These recommendations have been submitted to the hospital leadership, and implementations are in progress.

Abstract:With increasing competition, many healthcare organizations have undergone tremendous reform in the last decade aiming to increase efficiency, decrease waste, and reshape the way that care is delivered. This study focuses on the operational efficiency improvement of hospital’s registration process. The operational efficiency related factors including the service process, queue strategy, and queue parameters were explored systematically and illustrated with a case study. Guided by the principle of business process reengineering (BPR), a simulation approach was employed for process redesign and performance optimization. As a result, the queue strategy is changed from multiple queues and multiple servers to single queue and multiple servers with a prepare queue. Furthermore, through a series of simulation experiments, the length of the prepare queue and the corresponding registration process efficiency was quantitatively evaluated and optimized.

Abstract:BACKGROUND: Hospitals are complex adaptive systems within which multiple components such as patients, practitioners, facilities, and technology interact. A careful approach to optimization of this complex system is needed because any change can result in unexpected deleterious effects. One such approach is discrete event simulation, in which what-if scenarios allow researchers to predict the impact of a proposed change on the system. However, studies illustrating the application of simulation in optimization of general internal medicine (GIM) team inpatient operations are lacking.METHODS: Administrative data about admissions and discharges, data from a time-motion study, and expert opinion on workflow were used to construct the simulation model. Then, the impact of four changes: aligning medical teams with nursing units, adding a hospitalist team, adding a nursing unit, and adding both a nursing unit and hospitalist team with higher admission volume were modeled on key hospital operational metrics.RESULTS: Aligning medical teams with nursing units improved team metrics for aligned teams but shifted patients to unaligned teams. Adding a hospitalist team had little benefit, but adding a nursing unit improved system metrics. Both adding a hospitalist team and a nursing unit would be required to maintain operational metrics with increased patient volume.CONCLUSION: Using simulation modeling, we provided data on the implications of four possible strategic changes on GIM inpatient units, providers, and patient throughput. Such analyses may be a worthwhile investment to study strategic decisions and make better choices with fewer unintended consequences.

Abstract:The emergency department is important as a fundamental element of the healthcare system. Patient flow processes in emergency departments are a critical factor that determines effective and rapid medical intervention as well as the overall performance of the healthcare system. Aim: This study aimed to analyze patient waiting times and hospital stay times by developing a different approach to the current patient flow processes in the green area of a hospital emergency department. Methods: Numerical and statistical results were obtained using a discrete event simulation model for the current and new situation of patient flow processes. Results: The waiting time decreased from 12.91 minutes in the current simulation model to 12.81 minutes in the proposed simulation model. The length of stay time decreased from 54.72 minutes in the current simulation model to 53.90 minutes in the proposed simulation model. In this case, it is seen that the proposed model provides a reduction in the length of stay and patient waiting time. Conclusion: The result shows that the proposed model provides some improvement in both waiting and length of stay time. This study offers a different approach to patient flow, allowing emergency department personnel to use their resources effectively and minimizing the time spent on patients' treatment or waiting periods. Thus, this study concluded that the effective management of recommended patient flow processes increases the capacity of emergency departments to deal with emergencies and accelerates access to emergency medical services.
A discrete‐event simulation model for assessing operating room efficiency of thoracic, gastrointestinal, and orthopedic surgeries

Abstract:Background In hospital management, pinpointing steps that most enhance operating room (OR) throughput is challenging. While prior literature has utilized discrete event simulation (DES) to study specific strategies such as scheduling and resource allocation, our study examines an earlier planning phase, assessing all workflow stages to determine the most impactful steps for subsequent strategy development. Methods DES models real‐world systems by simulating sequential events. We constructed a DES model for thoracic, gastrointestinal, and orthopedic surgeries summarized from a tertiary Chinese hospital. The model covers preoperative preparations, OR occupation, and OR preparation. Parameters were sourced from patient data and staff experience. Model outcome is OR throughput. Post‐validation, scenario analyses were conducted for each department, including: (1) improving preoperative patient preparation time; (2) increasing PACU beds; (3) improving OR preparation time; (4) use of new equipment to reduce the operative time of a selected surgery type; three levels of improvement (slight, moderate, large) were investigated. Results The first three improvement scenarios resulted in a 1%–5% increase in OR throughput across the three departments. Large reductions in operative time of the selected surgery types led to approximately 12%, 33%, and 38% increases in gastrointestinal, thoracic, and orthopedic surgery throughput, respectively. Moderate reductions resulted in 6%–17% increases in throughput and slight reductions of 1%–7%. Conclusions The model could reliably reflect OR workflows of the three departments. Among the options investigated, model simulations suggest that improving OR preparation time and operative time are the most effective.

Nutrient intake and consumption of supplementary nutrition by pregnant mothers in ICDS and non-ICDS areas--a pilot study.

Abstract:This pilot study examined the dietary intake among pregnant women and their use of supplementary nutrition provided by the ICDS program in India. Data were obtained from 174 pregnant women registered in an ICDS project in the National Capital Territory of Delhi and from 51 pregnant women who lived in a neighboring Community Development Block without nutrition supplementation as controls. Data were obtained about dietary intake using the 24-hour recall method. 52.9% were of low socioeconomic status. Findings indicate that the energy intake was 1406 kcal in the 1st trimester 1330 kcal in the 2nd trimester and 1333 kcal in the 3rd trimester in the ICDS area. For the 3 pregnancy stages energy intakes in the non-ICDS area were 1241 kcal 1049 kcal and 1240 kcal respectively. Only 24% of mothers registered for supplementary nutrition and actually collected their food. Only 11% consumed 75-100% of food received. 36% consumed under 50% of food received. All mothers shared the nutritional supplements with family members. It is recommended that evaluation research monitor consumption of nutritional supplements in public programs. The costs of nutritional supplements are high and better targeted to vulnerable areas with chronic food shortages.

Abstract:In spite of continuous medical and technological advances, there are still treatments today, e.g., infertility treatments, that have not been addressed using remote monitoring due to the absence of reliable devices and smart health monitoring systems (SHMSs). A recent European Union report highlights the need for scientific investment in remote monitoring efficiency. To expedite cost-effective real-data experiments, the scientific community emphasizes complementing SHMSs with modeling and simulation techniques, allowing the projection of comprehensive treatment scenarios and informed decision-making based on synthetic data. In this paper, we conducted simulations of patient flow in two assisted reproduction clinics, with real data, applying discrete event simulation techniques. Four simulation scenarios were run for the Alicante clinic: (i) current system organization, (ii) using SHMS with the same resources, (iii) SHMS with reduced clinical staff, and (iv) SHMS with modified staff allocation. In a large-scale clinic in Madrid, two scenarios were simulated: (i) current system organization and (ii) SHMS with the same resources. The simulation of different scenarios enabled (i) the identification of a bottleneck in an ultrasound scanning area in the current system's organization; (ii) modelling of the relocation of the clinical staff within the clinic; and (iii) a proposal of the most optimal scenario with SHMS. Comparing the obtained results, we showed a reduction in the workload of the receptionists, in the ultrasound and blood collection areas and finally reduced the workload of physicians with first half and the second half of the working day schedules in both clinics.

Abstract:The emergency department (ED) serves as a primary patient entry point into hospitals, and bottlenecks in the ED cause patient flow problems and increase costs throughout the entire hospital. EDs increasingly employ discrete event simulation as an analytical decision making tool to alleviate these problems. Simulation models quantify complicated emergency care delivery systems and help improve emergency department efficiency, simultaneously raising hospital revenues and providing better service to patients. Successfully modeling ED operations requires a solid understanding of how data from the hospital fits into the simulation model. However, little guidance exists regarding the impact of data validation and input analysis on the outcomes of hospital simulations. This paper describes means by which hospital administrators and simulation professionals can better understand how the choices made during input analysis influence the results of simulation models.
