Evolution of isolated systolic hypertension with normal central blood pressure in adolescents-prospective study

Abstract:Background: The clinical significance of isolated systolic hypertension with normal central blood pressure known as spurious hypertension (sHT) in adolescents and its evolution over time is not known. Methods: The aim of this study was to analyze changes in office, ambulatory blood pressure (ABPM), central systolic blood pressure (cSBP), hemodynamic parameters, and target organ damage (TOD) over a 1-year follow-up in a group of non-obese children with sHT. Results: Of 294 patients referred for primary hypertension, 138 patients (31 girls; 22%) had hypertension confirmed by ABPM. 48/138 (35%) patients (7 girls; 15%) were diagnosed with sHT (elevated office and ambulatory systolic BP, but normal cSBP); 43 of them (6 girls; 14%) were followed for 12 ± 3 months during non-pharmacological therapy. At baseline 7 (16%) patients had borderline values of cIMT or LVMi indicating mild TOD. After 12 months, 10/43 (3 girls; 23%) patients developed sustained HT (elevated office, ambulatory BP and cSBP), 11/43 (1 girl; 26%) maintained sHT, and 22/43 (2 girls; 51%) evolved to white coat hypertension or normotension. The cSBP values increased in 27 patients (4 girls; 63%), but the group average remained in the normal range. Prevalence of TOD did not change during observation. The multivariate regression analysis showed that the only predictor of cSBP change over time was a change in serum uric acid level. Conclusions: In conclusion, after 1 year of non-pharmacological treatment, 23% of adolescents with sHT developed sustained hypertension, with the main predictor of cSBP change being the change in serum uric acid.
Blood Pressure Increase and Microvascular Dysfunction Accelerate Arterial Stiffening in Children: Modulation by Physical Activity

Abstract:Background Atherosclerotic remodeling starts early in life and can accelerate in the presence of cardiovascular risk (CV) factors. Regular physical activity (PA) can mitigate development of large and small artery disease during lifespan. We aimed to investigate the association of changes in body mass index (BMI), blood pressure (BP), PA behavior and retinal microvascular diameters with large artery pulse wave velocity (PWV) in prepubertal children over 4 years. Methods The school-based prospective cohort study included 262 children initially aged 6–8 years, assessing the above CV risk factors and retinal vessels by standardized procedures at baseline (2014) and follow-up (2018). PWV was assessed by an oscillometric device at follow-up. Results Children with increased systolic BP over 4 years showed higher PWV at follow-up (β [95% CI] 0.006 [0.002 to 0.011] mmHg per unit, P = 0.002). In contrast, increased vigorous PA corresponded to a lower PWV at follow-up (β [95% CI] −0.009 [−0.018 to <0−0.001] 10 min/day per unit, P = 0.047). Progression of retinal arteriolar narrowing and venular widening were linked to a higher PWV after 4 years (β [95% CI] −0.014 [−0.023 to −0.004] 0.01 changes per unit, P = 0.003). Conclusion Increase in systolic BP and progression of microvascular dysfunction were associated with higher PWV after 4 years. Children with increasing levels of vigorous PA were found to have lower PWV at follow-up. Habitual vigorous PA has the potential to decelerate the process of early vascular aging in children and may thus help counteract CV disease development later in life. Evaluation of Arterial Stiffness and Carotid Intima-Media Thickness in Children with Primary and Renal Hypertension

Abstract:Hypertension is an increasing disease in children and the risk of endothelial damage and target organ damage increases in the presence of additional risk factors such as obesity. In our study, the effect of hypertension on early atherosclerotic changes and target organ damage in children was investigated. Twenty four-hour ambulatory pulse wave analysis was performed by oscillometric method in 71 children aged 8-18 years, 17 of whom were diagnosed with primary hypertension without obesity, 18 had both primary hypertension and obesity, and 16 had renal hypertension. Twenty healthy normotensive children were included as the control group. Carotid intima-media thickness (CIMT) and Left Ventricular Mass Index were measured. Central systolic blood pressure (cSBP), central diastolic blood pressure (cDBP), systolic blood pressure (SBP) and diastolic blood pressure (DBP) were higher in the primary hypertension group compared to controls (p = 0.001, p = 0.005, p = 0.001, p = 0.009, respectively), cSBP was higher in the renal hypertension group than the control group (p = 0.018). There was no difference between the groups in terms of pulse wave analysis parameters, CIMT, or left ventricular mass index (p > 0.05). Pulse wave velocity was positively correlated with SBP, DBP, cSBP, cDBP (p < 0.001). Augmentation index was positively correlated with DBP and cDBP (p = 0.01, p = 0.002, respectively). Our findings show that high blood pressure is associated with arterial stiffness and target organ damage beginning in childhood. The detection of early atherosclerotic vascular changes using pulse wave analysis allows to take necessary precautions such as lifestyle changes to prevent target organ damage in hypertensive children.

Abstract:Objective: Our cross-sectional study showed that arterial hypertension (AH) in children not only leads to macrovascular remodeling but also to alterations in retinal microcirculation which correlates with subclinical injury in common carotid arteries. However, no studies evaluated antihypertensive therapy's effects on microcirculation. The study aimed to prospectively evaluate hypertension-associated changes in retinal microcirculation and their correlation with hypertension-mediated organ damage (HMOD): carotid intima-media thickness (cIMT), pulse wave velocity (PWV) and left ventricular mass index (LVMi). Design and method: : 47/157 children (mean+-SD age = 14.74±3.1 years) enrolled in the cross-sectional study were observed for a mean+-SD period of 1.13±0.9 years while receiving pharmacological/non-pharmacological treatment for AH. All participants underwent ambulatory blood pressure monitoring (ABPM), HMOD assessment, and an ophthalmic examination assessing retinal vessel density (VD), foveal thickness, thickness of retinal nerve fiber layer, ganglion cell complex, and foveal avascular zone using optical coherence tomography at the start of the study and the end of follow-up. Results: At baseline, we observed a lower density of retinal vessels correlated with increased cIMT. 33/47 (70%) patients achieved normotension (24 systolic blood pressure <=95th percentile) at the end of follow-up, in whom the 24h systolic, diastolic, and mean arterial pressure (MAP) Z-scores decreased significantly by a mean of -0.70 to 0.94 Z-scores, significantly different (p<0.05) from 14 patients, in whom the BP increased/did not change during follow-up. Patients who achieved normotension significantly decreased serum cholesterol and LVMi (p<0.05) compared to patients with persistent hypertension. There were, however, no significant differences in other HMOD parameters (PWV, cIMT) and retinal VD parameters between patients achieving normotension and those with persistent hypertension. Similar results were obtained using 24h MAP or 24 diastolic blood pressure and lower BP cutoffs for normotension (90th and 75th percentiles). A trend was observed in the association between a decrease in vascular density and an increase in cIMT-SDS (p=0,088; r=-0,28). No significant correlation with microvascular parameters was found for LVMi and PWV. Conclusions: Our results show that, despite normalization of blood pressure at the end of 1 year of follow-up, there was no significant improvement in retinal microcirculation.

Abstract:Introduction: Primary hypertension (PH) is a multifactorial disease influenced by genetic, epigenetic, and environmental factors. Despite the occurrence of PH-associated cardiovascular events in youth, the molecular mechanism(s) of target organ damage (TOD) are unknown. Objectives: (1) To identify an epigenetic signature and gene expression profiles in adolescents with low blood pressure (BP) and normal left ventricular mass (LVM) compared to those with high BP and high LVM; and (2) to determine novel gene targets and associated signaling pathways for future investigation and intervention. Methods and Results: A total of 397 participants (mean age 15.6 ±1.7 years, 59% male, 63% Caucasian) were enrolled across the distribution of BP. The average daytime ambulatory systolic BP recorded in healthy and hypertensive participants was 112 ±9.71 and 133 ±7.2 mmHg ( p<0.05 ) respectively. Clinical measures revealed higher body mass index (26.8 ±7.02 vs 29.6 ±7.88 Kg/m 2 ; p<0.05 ), and abnormal circulatory HDL (47.4 ±12.1 vs 43.4 ±11.7 mg/dL; p<0.05 ), glucose (87.8 ±7.98 vs 90.8 ±8.17 mg/dL; p<0.05 ), insulin (17.8 ±14.3 vs 23.7 ±19 μIU/dL; p<0.05 ), creatinine (0.718 ±0.13 vs 0.727 ±0.17 mg/dL; p<0.05 ), uric acid (5.4 ±1.63 vs 6.04 ±1.52 mg/dL; p<0.05 ), CRP (1.35 ±1.8 vs 1.92 ±2.14 mg/dL; p<0.05 ), and left ventricular hypertrophy (LVM/ht 2.7 ; 31.4 ±6.74 vs 33.5 ±7.15 g/m 2.7 ; p<0.05 ), and arterial stiffness (Pulse wave velocity; 4.83 ±0.69 vs 5.35 ±0.92 m/sec; p<0.05 ). Using peripheral blood mononuclear cells, mRNA-Seq, miRNA-Seq, and whole-genome DNA methylation analysis revealed master genes, and regulatory pathways related to BP regulation, tissue fibrosis and cardiovascular remodeling. Our study reveals a novel PH-associated TOD mechanism, showing angiogenesis inhibition mediated by VASH1 (Vasohibin-1) upregulation and downtrends in VASH2 (Vasohibin-2), VEGFC (Vascular endothelial growth factor C), HIF1α (Hypoxia-inducible factor 1-alpha), and IGF1 (Insulin Like Growth Factor 1). Moreover, VASH1 targeting miRNA hsa-miR-30e-5p is inversely regulated. Conclusion: Angiogenesis inhibition in the presence of common demographic and clinical intermediate-phenotypes may contribute to the development of TOD in hypertensive youth.

Retrospective analysis of etiology and target organ damage in children with hypertension

Abstract:Objective To analyze the clinical data of children with primary hypertension and secondary hypertension in order to provide reference for accurate diagnosis and for early detection of target organ damage in case of primary hypertension.Methods All the children who were diagnosed with hypertension and hospitalized between January 1992 and May 2016 were enrolled in this study.Hypertension was classified as primary and secondary based on the etiology of hypertension.Data was analyzed with SPSS 17.0 software.Results (1) The average age of primary hypertension patients was 11.76 years old,and the mean BMI was 26.90 kg/m2,which were higher than those of secondary hypertension patients with statistically significant difference (P =0.000).(2) The composition ratio of secondary hypertension during infancy,preschool period,school age and adolescence was 94.7％、48.3％、17.0％ and 8.6％ respectively.Adolescents were more vulnerable to primary hypertension than children of other ages.(3) 59.42％ of secondary hypertension patients had kidney disease.Blood pressure was higher in secondary hypertension patients.(4) Target organ damage was present during diagnosis in most of the patients with primary hypertension.Elevated levels of serum or urine β2-microglobulin could be an earlier indicator of kidney damage.In the target organ damage group,the mean diastolic blood pressure and insulin level were higher.Conclusions Patients with secondary hypertension were younger than patients with primary hypertension,and blood pressure was higher.The causes of secondary hypertension are mainly kidney and vascular lesions.Target organ damage can be detected during diagnosis in most of the patients with primary hypertension.The higher diastolic blood pressure and insulin levels,the more likely target organ damage can be.
Cardiac Dysfunction Investigation in Prehypertension

Abstract:To study the possible cardiac function alternation in hypertension, 13 cardiac function parameters such as blood pressure (BP), cardiac output (CO), ejection time (ET) and stroke volume (SV), etc. were measured in 898 subjects. The subjects were grouped into 3 groups by blood pressure, namely: normotension, prehypertension and hypertension group. Statistic analyses showed that in prehypertension subjects, peak velocity and cardiac output increased, while the systemic vascular resistance index had no significant increase compared with normotensive. Peak velocity, mean pressure gradient and velocity time integral in prehypertension were significantly correlated with systolic blood pressure, however, no correlation was found in either normotension or hypertension subjects. The results suggest that increasing cardiac output and peak velocity occurred at the phase of prehypertension, which may conduce to the elevation of blood pressure
Performance of home-based and ambulatory blood pressure monitoring in obese children and their correlation with end organ damage

Abstract:Abstract Background The increasing prevalence of childhood obesity has led to a corresponding increase in hypertension among children, necessitating early identification of subclinical target organ damage for accurate cardiovascular risk assessment. However, in the pediatric population, there is a paucity of literature comparing ambulatory and home blood pressure monitoring, and this knowledge gap is exacerbated by limited access to ambulatory blood pressure monitoring (ABPM) facilities, particularly in developing countries, where pediatricians often resort to home blood BP monitoring as the preferred option. Method In this cross-sectional study with 60 obese children (aged 5-18 years) at a tertiary health care in central India, we aimed to comprehensively characterize blood pressure profiles, including office, ambulatory and home and investigated their correlations with indicators of end-organ damage. Result Among 60 children, 26 (43.3%) participants were found to be hypertensive based on 24 Hr ABPM evaluation. Masked hypertension and white coat hypertension (WCH) were observed in 21.6% and 13.3% respectively. Surprisingly, 20% of participants were identified as hypertensive through 7-day home BP monitoring (HBPM). A notable discordance of 36.6% was between HBPM and ABPM results. Moreover, 26.7% of the children had end-organ damage, with higher odds associated with night-time systolic ambulatory hypertension in the adjusted regression model (OR = 1.06, 95% CI: 1.03-1.10, p < 0.001). Conclusion The study highlights 24-hour ABPM’s vital role in classifying hypertensive status, especially in high-risk children. The diagnostic performance of HBPM shows poor sensitivity in detecting MH and lower specificity in identifying WCH compared to ABPM. This limitation translates to missed opportunities for early preventive interventions.

Abstract:Background: The prevalence of hypertension mediated organ damage (HMOD) in patients attending the Emergency Department (ED) with symptomatic blood pressure (BP) rise is unknown, and whether HMOD varies between asymptomatic and symptomatic patients with grade 3 hypertension is unclear. Aim: This study aimed to investigate cardiac and vascular HMOD in hypertensive urgencies (HU) and asymptomatic outpatients with grade 1–3 hypertension. Methods: Patients attending the ED with a symptomatic BP rise ≥180/110 mmHg were prospectively enrolled (HU group), after the exclusion of acute organ damage. HMOD and BP were assessed after 72 h from ED discharge in an office setting. These patients were matched by age and sex to outpatients with grade 3 hypertension (Grade 3 group), and by age, sex, and 72 h office BP values to outpatients with any grade hypertension (Control group). Results: A total of 304 patients were enrolled (76 patients in the HU group, 76 in the Grade 3 group, and 152 in the Control group). Grade 3 patients had increased left ventricular mass (LVMi) compared to patients with HU (106.9 ± 31.5 vs. 96.1 ± 30.7 g/m 2 , p = 0.035). Severe left ventricular hypertrophy (LVH) was more frequent in grade 3 (21.1 vs. 5.3%, p = 0.004), and pulse wave velocity (PWV) was similar in the two groups. There was no difference in LVMi between ED and Control patients (96.1 ± 30.7 vs. 95.2 ± 26.6 g/m 2 , p = 0.807). LVH prevalence was similar (43.4 vs. 35.5%, p = 0.209, respectively), but patients with HU had thicker interventricular septum (11.9 ± 2.2 vs. 11.1 ± 2.2 mm, p = 0.007). PWV was similar between these two groups. Patients with HU needed more antihypertensive drugs than Control patients (2 vs. 1, p < 0.001). Conclusions: Patients with HU had a better cardiac HMOD profile than outpatients with grade 3 hypertension. Their cardiac and vascular HMOD is more comparable to an outpatient with similar in-office BP, although they need more antihypertensive medications.

Abstract:Objective: Guidelines promote screening people with high normal BP for the presence of end-organ damage because their presence is associated with a 2- to 3-fold increase in the risk of CVD. Design and method: 100 consecutive subjects with high normal BP [systolic BP=130-139mmHg and/or diastolic BP=85-89mmHg] were examined for early signs of hypertension-mediated organ damage (HMOD). They underwent an echocardiographic study, a negative for ischemia treadmill exercise test (Bruce protocol) where the index SBP/MET-slope [(peakSBP—restingSBP)/(peakMET-1)] was used. Arterial stiffness was evaluated based on carotid-femoral pulse wave velocity (PWV). The sympathetic drive was assessed by MSNA. Follow-up was scheduled every 6 months for 3 consecutive years, where BP measurements were assessed in the office and with ambulatory BP monitoring (ABPM). All participants offered lifestyle advice to lower their BP. The endpoint was the development of HTN either with Office BP or ABPM. Results: Of 100 subjects (54±8 years, 42 males, baseline office BP: 132/82 mmHg, 24-hour BP: 122/76 mmHg) 40 developed HTN in 3 years. 34 subjects developed Hypertensive Response to Exercise (HRE) (BP >210mmHg in men and >190mmHg in women) and all of them developed HTN. The SBP/MET-slope in Group I was increased in all stages till peak exercise independently of sex type (stage1: 6.7vs4.9 p=0.049, stage2: 8.4vs4.8 p=0.001, peak: 6.7vs4.9, p=0.001). Their exercise capacity was reduced (10vs11.3METs, p=0.002) as well as their maximum exercise heart rate (156vs164, p=<0.0001). Those with HRE had higher 24hSBP (124vs121mmHg, p=0.009), Night SBP (117vs111mmHg, p<0.001), Day SBP (127vs124mmHg, p=0.012) Office PP (51vs47mmHg, p=0.01), while did not differ regarding their metabolic profile at the follow-up. Additionally, they had increased PWV (8.3vs7.5m/sec, p=<0.0001), MSNA levels (36vs28 bursts, p=<0.0001), LVMI(38vs34 gr/m2, p=0.02), deteriorated 2021 CKD-EPI eGFR (90 vs 97 mL/min/1.73 m 2 , p=0.02). Conclusions: Detecting exaggerated blood pressure response is associated with increased systemic vascular resistance and early subclinical HMOD which may upgrade an individual's cardiovascular disease (CVD) risk as they progress to HTN indicating a need for instituting a BP-lowering strategy.

Abstract:Background: Hypertension is one of the most prevalent diseases in the United States, affecting an estimated 3.5% of children and adolescents. It can be adversely affect most organ systems but is particularly detrimental to the heart and vascular systems. The repercussions can be gauged through well-established measures of cardiovascular function including left ventricular mass index (LVMI), left ventricular hypertrophy (LVH), carotid intima media thickness (cIMT), and aortic stiffness. Cardiovascular function is also affected by underlying etiologies of hypertension including chronic kidney disease, polycystic kidney disease, coarctation of the aorta, adrenal disorders, renal artery stenosis, obstructive sleep apnea, as well as various drugs and medications (decongestants, stimulants, Non-steroidal Anti-inflammatory Drugs (NSAIDs), and steroids). Methods: An exhaustive literature search was conducted for clinical data regarding pediatric hypertension. Sixty-seven articles were incorporated with data on 189,477 subjects total. The data was then extracted and categorized as relating to hypertension incidence, LVMI, LVH, cIMT, and/or aortic stiffness. Results: The prevalence of pediatric ( < 18 years) hypertension extracted from 47 studies from 1994 to 2018 averaged 4%. The LVMI assessed over 7 studies (n = 661) averaged 39.3 g/ m 2.7 in the hypertensive cohort and 30.1 g/ m 2.7 in the control cohort. The cIMT assessed over 7 studies (n = 580) averaged 0.55 mm in the hypertensive cohort and 0.49 mm in the control cohort. Ambulatory arterial stiffness parameters assessed over 5 studies (n = 573) in the normotensive cohort averaged 99.73 mmHg, 69.81 mmHg, 76.85 mmHg, and 46.90 mmHg, for SBP, DBP, MAP, and PP respectively. Ambulatory arterial stiffness parameters assessed over 5 studies (n = 573) in the hypertensive cohort averaged 129.56 mmHg, 73.69 mmHg, 95.08 mmHg, and 56.80 mmHg, for SBP, DBP, MAP, and PP respectively. Conclusions: The significance of pediatric hypertension is emphasized by evidence of early cardiovascular disease as demonstrated by non-invasive measures including cIMT and arterial stiffness parameters, and target organ damage and including LVH and LVMI factors. Thus, early diagnosis and treatment of high blood pressure is paramount for improving long term cardiovascular health and preventing long term morbidity and mortality.

The objective of this study was to evaluate for the evidence of subclinical target organ damage in two groups of subjects without hypertension. METHODS: This was a prospective cross-sectional study. Subjects seeking care for various clinical conditions in the hospital were invited for the survey. The subjects' were divided in to two groups according to their blood pressure: pre-hypertension (systolic blood pressure 120 to 139 mm Hg or diastolic blood pressure, 80 to 89 mm Hg) and normotension (systolic blood pressure, < 120 mm Hg and diastolic blood pressure, < 80 mm Hg). Urine albumin excretion and other biochemical analyses were performed using standard methods. RESULTS: We recruited a total of 3300 subjects, the prehypertension group included 1100 individuals and the normotension group was composed of 2200 persons. The prevalence of microalbuminuria in subjects in the prehypertension group was 6.8% and in those who are the in the optimal BP group was 3.6% (P < 0.001). Subjects in the prehypertension group had a mean B-type natriuretic peptide (BNP) level of 98 (72) pg/mL compared with 43.6 (20) pg/mL found among subjects in the normotension group (< 0.001). In the logistic regression model, tobacco smoking aOR 2.7 (95% CI.; 1.7-5.8), higher uric acid level aOR 2.2 (1.7-3.2), microalbuminuria 7.6 (4.9-14.7) and a higher BNP level aOR 2.5 (1.8-7.6) were significantly associated with the occurrence of prehypertension. CONCLUSIONS: Microalbuminuria, BNP level and hyperuricemia were significantly associated with prehypertension.

Abstract:INTRODUCTION: Hypertension is a leading cardiovascular risk factor. Accurate blood pressure measurement is pivotal in hypertension diagnosis and management. Conventional office measurements (OBP) are error-prone, exacerbated by the white coat effect. Unattended automated office blood pressure measurement (UAOBP) is emerging as an alternative, mitigating the white coat effect. However, its ability to predict hypertension-mediated organ damage (HMOD) remains disputable.OBJECTIVES: This study compares UAOBP with OBP in terms of their association with various HMODs, including left ventricular hypertrophy, left atrial enlargement, left ventricular systolic and diastolic dysfunction, intima-media complex thickening, microalbuminuria and abnormal pulse wave velocity.PATIENTS AND METHODS: A total of 219 hypertensive patients were recruited, interviewed, and examined. Subsequently, blood pressure measurements were conducted in a randomized manner: 1) UAOBP: after 5 minutes of solitary rest in the examination room, the patient's blood pressure was automatically measured three times at 1-minute intervals. 2) OBP: after 5 minutes of rest, a physician performed three consecutive blood pressure measurements at 1-minute intervals. Subsequent evaluations aimed to detect HMODs, including echocardiography, carotid artery ultrasound, pulse wave velocity assessment, and laboratory tests.RESULTS: The blood pressure values in UAOBP were significantly lower than those in OBP (mean (SD) 124.7 (14.4) vs. 128.2 (14.2) mmHg, P <0.001 for systolic, and 73.3 (10.2) vs. 75.2 (10.6) mmHg, P <0.001 for diastolic). Correlation and ROC curve analyses revealed no method's superiority in predicting HMOD.CONCLUSIONS: The UAOBP did not prove superior in predicting HMOD compared to OBP, Further research is warranted to determine the role of UAOBP in clinical practice.

High and low affinity binding of folate to proteins in serum of pregnancy women.

Abstract:Binding of [3H]folate to proteins in serum of pregnant women was studied in equilibrium dialysis experiments (pH 7.4, 37 degrees C). A Scatchard analysis revealed the presence of high-affinity (Kass = 10(10) M(-1), N = 0.4 nM folate) and low-affinity sites. The high-affinity folate binding protein (Mr approximately 30 000--35 000) appeared in front effluent after application of serum to a DEAE-Sepharose CL-6B column equilibrated with 0.05 M imidazole buffer (pH 6.3)/30 mM NaCl. Low-affinity binding protein eluted from the column after a rise in NaCl concentration to 1 M was mainly similar to albumin. A minor part was, however, associated with a large molecular size (Mr > 200 000) protein, probably alpha 2-macroglobulin. High-affinity binding which displayed positive cooperativity was saturated at folate concentrations above 10(-10) M. Folate dissociation was a complex process consisting of an initial rapid phase (terminated within 48 h) followed by a slow release. At pH 3.5 dissociation became rapid and complete. Purified methotrexate had no effect on high-affinity binding, whereas N10-methylfolate (an impurity in the methotrexate preparation) acted as a potent inhibitor. Low-affinity binding was proportional to the folate concentration within the range 10(-10)--10(-7) M. Dissociation of folate was rapid.

Abstract:INTRODUCTION: Adhesion molecules: E-selectin and intercellular adhesion molecule 1 (ICAM-1) are well-established markers of endothelial injury. The aim of the study was to assess the relation between E-selectin and ICAM-1 and clinical and biochemical parameters in children and adolescents with primary hypertension (PH).MATERIAL AND METHODS: In 77 patients with PH (15.04 ±2.62 years, 50 boys, 27 girls) we evaluated serum E-selectin, ICAM-1, and selected clinical and biochemical parameters including inflammatory indicators and ambulatory blood pressure monitoring (ABPM).RESULTS: The E-selectin concentration was 55.63 ±26.49 ng/ml and the ICAM-1 concentration was 302.17 ±67.14 ng/ml. E-selectin and ICAM-1 correlated (p < 0.05) with BMI Z-score (r = 0.24, r = 0.29), ICAM-1 also with uric acid (r = 0.35), HDL-cholesterol (r = -0.28), platelet-to-lymphocyte ratio (r = 0.26), and systolic and mean blood pressure variability (r = 0.24, r = 0.24); in boys ICAM-1 correlated with mean platelet volume (r = 0.29). In multivariate analysis the only significant predictor of E-selectin was mean arterial pressure during 24 hours (β = 0.329, 95% CI: 0.012-0.646) and of ICAM-1 - uric acid (β = 0.430, 95% CI: 0.040-0.819). In 27 children with newly diagnosed PH E-selectin correlated negatively with diastolic blood pressure dipping (r = -0.54, p = 0.004) and positively with ambulatory arterial stiffness index (r = 0.51, p = 0.012).CONCLUSIONS: Elevated mean arterial pressure and hyperuricemia are risk factors of endothelial damage in paediatric patients with primary hypertension. In children with untreated primary hypertension there may be a relation between endothelial damage and disturbed circadian blood pressure profile.

Methods: A total of 63 adolescents diagnosed with obesity were evaluated between January 2019 and December 2019. Office blood pressure was measured for all children, and all of them underwent ABPM. Patients with and without hypertension in ABPM were compared in terms of clinical and laboratory findings. Results: The mean age was 14.0 ± 1.7 years, females composed 49.2% of the study population. Office blood pressure measurement revealed hypertension in 4 (6.3%) patients and prehypertension in 15 (23.8%) patients. Thirteen patients (20.9%) were diagnosed with masked hypertension, white coat hypertension was diagnosed in 3 (4.7%) patients. Abnormal ABPM patterns were found to be significantly more frequent in patients with severely obesity (with obesity: 26.4% vs severe obesity: 55.6%, p = 0.03) and patients with a higher waist circumference and waist circumference/height ratio. Conclusion: The prevalence of masked hypertension in obese adolescents has been found to be quite high. Therefore, we recommend ABPM in adolescents with high waist circumference/height ratio and severe obesity, even if their office blood pressure measurements are normal. It has been shown that there is a relationship between obesity and dyslipidaemia, hypertension and insulin resistance. 1 Obese children are at increased risk for diabetes, coronary artery disease and hypertension not only in childhood but also in adulthood. For this reason, it is very important to be alert in terms of preventing obesity and its accompanying morbidities. 2,3 Hypertension is one of the most important preventable causes of chronic kidney disease and heart disease. 4 Although its frequency in children is reported to be 3–5%, it gradually increases with the rise of obesity. Studies have found that the possibility of developing hypertension in obese children is three times higher than that in non-obese children. 5 Early recognition and treatment of hypertension is critical in preventing end organ damage. 4 For this reason, obese children should be evaluated with a blood pressure measurement at each visit. 6 However, with office blood pressure (OBP) measurement, hypertension may be overlooked, and the distinction cannot be made between white coat hypertension (WCH) and masked hypertension (MH). Therefore, the gold standard in the diagnosis of hypertension is ambulatory blood pressure monitoring (ABPM). 7 Although it is not reflected in the OBP measurement in obese children, ambulatory BP changes are known to occur. Abstract:Hypertension, Ahead of Print. BACKGROUND:We studied whether increased systolic blood pressure (SBP), as determined by auscultatory SBP, ambulatory SBP, and the number of cardiovascular health risk indicators, are associated with neurocognition in adolescents.METHODS:This cross-sectional study included 365 adolescents (mean age, 15.5 years) from 6 academic medical centers in the United States. The sample was 59.5% male, 52.6% White, with 23.9% of the caregivers having less than or equal to a high school degree. Primary exposures included the following: auscultatory SBP, ambulatory SBP, and the number of cardiovascular risk factors. Neurocognitive outcomes comprised nonverbal IQ, attention, and parent ratings of executive functions.RESULTS:After examining the models for the effects of targeted covariates (eg, maternal education), higher auscultatory SBP was associated with lower nonverbal IQ (β=−1.39;P<0.001) and verbal attention (β=−2.39;P<0.05); higher ambulatory 24 hours. SBP (β=−21.39;P<0.05) and wake SBP (β=−21.62;P<0.05) were related to verbal attention; and all 3 ambulatory blood pressure measures were related to sustained attention accounting for small to medium amounts of variance (adjusted R2=0.08–0.09). Higher ambulatory blood pressure sleep SBP also was significantly associated with parent ratings of behavior regulation (β=12.61;P<0.05). These associations remained stable after a sensitivity analysis removed cases with hypertension. Number of cardiovascular risk factors performed similarly, with more risk factors being associated with lower nonverbal IQ (β=−1.35;P<0.01), verbal attention (β=−1.23;P<0.01), and all parent ratings of executive functions.CONCLUSIONS:Elevated SBP, even below the hypertension range, and general cardiovascular health are associated with neurocognitive outcomes in adolescents. How these findings might guide clinical care is worthy of additional study.

Abstract:Objective: Controversial definition of arterial hypertension (AH) by the ACC/AHA 2017 Guidelines renewed interest in the earlier stages of blood pressure (BP) elevation. The aim: to investigate the features of the heart rate variability (HRV), large vessel remodelling (VR) and metabolic parameters in subjects with stage 1 AH according to the new American guidelines (130–139 / or 80–89 mmHg). Design and method: We investigated 148 untreated subjects with the following BP levels (mmHg): < 120 / and < 80 (group 1; n = 33), 120–129 / and < 80 (group 2; n = 27), 130–139 / or 80–89 (group 3; n = 60), > = 140 / or > = 90 mmHg (group 4; n = 28). HRV indices were assessed by 24-hour Holter monitoring data, large vessel remodelling parameters - on the basis of applanation tonometry (Sphygmocor) and carotid echo-tracking (Artlab) data. The results are presented as mean ± standard deviation. Results: In comparison to group 1, group 2 had lower levels the high-density lipoproteins levels (61.7 ± 15.6 and 51.7 ± 11.6 mg/dl, respectively, p = 0.009), increased diameter of carotid artery (6.8 ± 0.4 and 7.5 ± 0.5 mm, respectively, p = 0.034) and similar pattern of HRV parameters. Group 3, compared to groups 1 and 2, had significantly higher glucose and uric acid levels (p = 0.045 and p = 0.017, respectively), and also increased carotid-femoral pulse wave velocity, compared to groups 1 (7.8 ± 1.2 and 7.2 ± 0.9 m/s, respectively, p = 0.025). These alterations were associated with an increase in the high-frequency component of HRV (HF, p = 0.013 and r-MSSD, p = 0.022) and greater distensibility of the carotid vascular wall (p = 0.004), which were not observed in the group 4. Conclusions: Subjects with blood pressure of 130–139 / or 80–89 mmHg are characterized by distinct metabolic abnormalities, initial signs of vascular remodelling and alterations of HRV, which might predispose to further progression of BP elevation and development of target organ damage.

Abstract:Purpose of ReviewPediatric hypertension is increasing in incidence with concomitant increases in childhood obesity. Target organ damage from primary or secondary pediatric hypertension is a growing concern, as the pathogenesis of vascular disease, typically observed later in life, may begin at a young age. While hypertensive complications are described extensively in adults, much less is known about end organ damage from elevated blood pressure in children and adolescents.Recent FindingsThis review highlights the recent advances describing the target organ damage in the macro- and microvasculature resulting from hypertension. Persistently elevated blood pressure in childhood is associated with vascular changes affecting the heart, brain, kidneys, and retina. Emerging research shows that elevated blood pressure in children has effect on neurocognitive function in specific domains such as executive functioning, attention, and memory. Microalbuminuria is an early marker of kidney disease and a sign of glomerular injury and endothelial dysfunction that increases the risk of kidney damage and is associated with future kidney and cardiovascular events. Screening with ambulatory blood pressure monitoring to improve blood pressure control and echocardiogram may aid in early prevention of end organ damage.SummaryIn this review, we summarize and critically evaluate recent findings, describe ongoing studies, and address future research questions. Implications for diagnosis, prognosis, and need for better control of hypertension are also discussed.

Abstract:Background: Hemodynamic alterations have been described in hypertensive disease. However, we still do not know if the identification of an altered hemodynamic pattern in patients with controlled hypertension could predict cardiovascular risk or evolution of hypertension. Objective: to identify the hemodynamic pattern of hypertensive patients with BP values in goals. Methods: Controlled hypertensive subjects were recruited (n = 107) and included a control group of normotensive subjects (n: 35.9). A non-invasive hemodynamic study (Z Logic, lying down / standing) was performed, and the cardiac index (CI) and vascular resistance index (VRI) data were recorded for the determination of the hemodynamic pattern. Hypodynamic pattern was considered: CI 2500 dynas.seg.cm-5.m2; Hyperdynamic pattern IC> 4,4 L / min / m2 and VRI <2500 dynas.seg.cm-5.m2 Results: The SBP / DBP of the HTA group was 133.09 +/- 6.01 / 82.4 +/-7.2 mm Hg; and of the NT group was SBP/ DBP 123.6 +/- 9.7 / 75.5 +/- 8.9. The CI and VRI of the HTA group obtained was 2.81 l / m2 / min and 2842.31 dynas that, without corresponding to a hypodynamic pattern, showed a predominance of the VRI increase. In the control group of NT subjects, the values were 3.23 and 2289.23, respectively, corresponding to a normal pattern. The values obtained were different between both groups (p <.05). Conclusions: Controlled hypertensive patients with hypertension goals have a hemodynamics pattern with an increase in the VRI different from that of normotensive subjects. These results coincide with those reported by Ferrario and col. The relevance of the imbalance found could only be analyzed in the context of long-term follow-up to identify whether it is a predictor of CV events and / or complications.
