Abstract:Objective To identify non-tuberculous Mycobacterium(NTM) rapidly with HAIN molecular assay genotype Mycobacterium kit,and investigate the advantages and disadvantages of this method.Methods Seventy-four clinical NTM isolates were collected from hospitals in Zhejiang and Anhui province.Clinical strains were identified with HAIN molecular assay genotype Mycobacterium kit.16S rRNA gene sequencing was used to estimate and compare with this method.Results The results of kit showed that there were thirty-one M.intracellulare strains,twelve M.chelonae strains,eight M.fortuitum strains,six M.kansasii strains,five M.avium strains,three M.smegmatis strains,two M.phlei strains,two M.scrofulaceum strains and one M.gordon strain.Four strains were identified as Mycobacterium without further identification.Eight M.tuberculosis strains were identified correctly too.Compared with 16S rRNA gene sequencing,except for four strains identified as Mycobacterium,others 70 strains got the same results as 16S rRNA gene sequencing,the coincidence was 94.59％,and it could further identify thirteen Mycobacterium chelonae complex and eight Mycobacterium kansasii complex to subspecies M.abscessus and M.kansasii,respectively.If only to identify strains under the identification range of this kit,the coincidence reach to 100％,Conclusion The method of HAIN molecular assay genotype Mycobacterium kit is simple and accurate,the time is shorter and should widely be applied clinically.

Abstract:Objective To evaluate three molecular methods for rapid identification of nontuberculous Mycobacterium(NTM).Methods Forty-one clinical NTM isolates were collected and 16S rRNA gene sequencing was used as the standard method for NTM identification.Meanwhile,the restriction fragment length polymorphism of hsp65 PCR-RFLP and hsp65 gene sequencing were used to identify NTM strains and compared with 16S rRNA gene sequencing.Results The results of 16S rRNA sequencing showed that there were nine Mycobacterium chelonae complex strains,seven Mycobacteriumfortuitum strains,seven Mycobacterium intracellulare strains,three Mycobacterium avium strains,three Mycobacterium kansasii complex strains, three Mycobacterium smegmatis strains, three Mycobacterium terrae strains, two Mycobacterium phlei strains,two Mycobacterium nonchromogenicum strains,one Mycobacterium scrofulaceum strain and one Mycobacterium arupense strain.Compared with 16S rRNA gene sequencing,hsp65 PCR-RFLP could identify nine Mycobacterium chelonae complexes and three Mycobacterium kansasii complexes to subspecies Mycobacterium abscessus and Mycobacterium kansasii,respectively; One Mycobacterium fortuitum strain and one Mycobacterium nonchromogenicum strain were different from 16S rRNA gene sequencing results ,but other isolates were the same.The coincidence was 95.1%.By hsp65 gene sequencing,only one identification of Mycobacterium hiberniae strain was different from 16S rRNA gene sequencing and the coincidence was 97.6%.And hsp65 gene sequencing could further identify nine Mycobacterium chelonae complexes and three Mycobacterium kansasii complexes to subspecies Mycobacterium abscessus and Mycobacterium kansasii,respectively.Conclusions All three molecular methods can identify NTM strains rapidly.Compared with 16S rRNA gene sequencing,hsp65 gene sequencing and hsp65 PCR-RFLP are easier to identify clinical common NTM strains(such as Mycobacterium kansasii and Mycobacterium abscessus),and can be widely used in clinical practice.
Establishment and Application of Multi-Pcr-sscp Assay for Rapid Detection of Isoniazid- and Rifampin-Resistance Mycobacterium Tuberculosis Isolates

Abstract:Objective To establish multi-PCR-single strand conformational polymorphism analysis (mPCR-SSCP) for rapid detection of isoniazid (INH) and rifampin (RIF) resistance associated katG,inhA and rpoB genes of Mycobacterium tuberculosis isolates.Methods The INH-and RFP-resistance of 134 isolates was determined by using drug susceptibility test.The primers were designed for detecting INH and RFP resistance-associated katG,inhA and rpoB gene in the isolates by mPCR-SSCP.PCR-DS technique was applied to detect the mutations in katG,inhA and rpoB genes.All the results from different assays were subsequently analyzed as well as compared.Results All of the 134 tested isolates had katG,inhA and rpoB genes.Of the 134 isolates,42 (31.3％) and 45 (33.6％) strains were INH-and RFP-resistant,respectively.The results of mPCR-SSCP and PCR-DS showed that all the 92 INH-susceptible isolates had no mutation in katG and inhA genes with 100％ specificity.In the 89 RFP-susceptible isolates,2 and 1 had mutation in rpoB genes confirmed by mPCR-SSCP and PCR-DS with 97.8％ or 98.9％ specificity,respectively.Among the 42 INH-resistance isolates,33 and 36 strains had the mutations in katG and/or inhA genes due to the results of mPCR-SSCP and PCR-DS with 78.6％ or 85.7％ sensitivity,respectively.The results of mPCR-SSCP and PCR-DS also demonstrated that in the 45 RFP-resistance isolates,41 and 43 strains had the mutations in rpoB gene with 91.1％ or 95.6％ sensitivity,respectively.Conclusion The mPCR-SSCP established in this study can be used to rapidly detect INH and RFP-resistance associated mutations in katG,inhA and rpoB genes of M.tuberculosis with convenience,specificity and sensitivity,which shows a good prospect for application in clinic.

Abstract:Background: Tuberculosis (TB) is a major global public health issue. Prompt and accurate TB diagnosis is crucial for starting appropriate treatments and preventing the disease's spread. Current diagnostic techniques are either slow or expensive. This study aimed to create and evaluate a new, fast, highly reliable, and cost-effective TB detection method using a gene chip and Restriction Fragment Length Polymorphism (RFLP) analysis on Mycobacteria Growth Indicator Tubes (MGIT) specimens. Methods: We assessed the effectiveness of a novel gene chip and RFLP methods targeting the 16S rRNA gene of Mycobacterium tuberculosis in 2000 MGIT culture-positive specimens. RFLP analysis identified the AfeI restriction site within the M. tuberculosis complex (MTBC) genome. Discrepancies were investigated through extensive sequencing and Cobas TaqMan PCR analysis, along with reviewing patient profiles. Results: Both methods showed high efficacy in detecting MTBC in broth cultures, with the gene chip method achieving a sensitivity of 99.27 %, specificity of 98.35 %, and the RFLP method showing a sensitivity of 98.18 %, specificity of 99.31 %. False negatives in two isolates were due to a mutation in the AfeI site. Additionally, five cases showed MTBC presence when nontuberculous Mycobacterium species grew in cultures. Conclusion: Our novel gene chip and RFLP methods are effective for rapid highly-reliable and cost-effective M. tuberculosis detection in MGIT specimens. Both gene chip and RFLP methods are suitable for resource-limited settings, offering an economical advantage. These methods have significant potential to improve clinical TB diagnosis.
[Development and Application of Homologous Recombination Knockout System in Mycobacterium Tuberculosis].

Abstract:OBJECTIVE:We developed a homologous recombination system to make marker-free mutants in Mtb in an easy screening way.METHODS:The plasmid pSL002 harboring recE and recT-like protein gp60 and gp61 was transformed into wt Mtb in order to increase the recombing efficiency. The two homologous arms of target gene were amplified and cloned into pSL001. The purified homologous arms-loxP-gfp-hyg-loxP fragments were further transformed into Mtb (with pSL002 inside) competent cells to obtain the double cross over (DCO) mutants. The plasmid pSL003 was transformed into DCO competent cells to excise the two loxP sites flanked region. The plasmid pSL002 and pSL003 were removed via the counter selection marker sacB.RESULTS:An efficient homologous recombination system was developed. Three different marker free mutants: Rv1364c phosphatase domain, PstP extracellular domain and PstP transmembrane domain were created by the system developed in this study. The efficiency to obtain DCO was varying from 25% to 62.5%, while the efficiency from DCO to KO was close to 100%. The gfp reporter was used to screen SCO, DCO and KO.CONCLUSION:The homologous system shortens the complicated screening process to 3 month in Mtb. This is the fastest allelic exchange system reported so far, providing novel deletion strategy to the repertoire of mycobacterial genetic tools for constructing unmarked mutations.
Using Microfluidic Chip and Allele-Specific PCR to Rapidly Identify Drug Resistance-Associated Mutations of Mycobacterium tuberculosis

Abstract:Background: The currently used conventional susceptibility testing for drug-resistant Mycobacterium tuberculosis (M.TB) is limited due to being time-consuming and having low efficiency. Herein, we propose the use of a microfluidic-based method to rapidly detect drug-resistant gene mutations using Kompetitive Allele-Specific PCR (KASP).Methods: A total of 300 clinical samples were collected, and DNA extraction was performed using the "isoChip & REG;" Mycobacterium detection kit. Phenotypic susceptibility testing and Sanger sequencing were performed to sequence the PCR products. Allele-specific primers targeting 37 gene mutation sites were designed, and a microfluidic chip (KASP) was constructed using 112 reaction chambers to simultaneously detect multiple mutations. Chip validation was performed using clinical samples.Results: Phenotypic susceptibility of clinical isolates revealed 38 rifampicin (RIF)-resistant, 64 isoniazid (INH)-resistant, 48 streptomycin (SM)-resistant and 23 ethambutol (EMB)-resistant strains, as well as 33 multi-drug-resistant TB (MDR-TB) strains and 20 strains fully resistant to all four drugs. Optimization of the chip-based detection system for drug resistance detection showed satisfactory specificity and maximum fluorescence at a DNA concentration of 1x101 copies/& mu;L. Further analysis revealed that 76.32% of the RIF-resistant strains harbored rpoB gene mutations (sensitivity, 76.32%; specificity 100%), 60.93% of the INH-resistant strains had katG gene mutations (sensitivity, 60.93%; specificity, 100%), 66.66% of the SM-resistant strains carried drug resistance gene mutations (sensitivity, 66.66%; specificity, 99.2%), and 69.56% of the EMB-resistant strains had embB gene mutations (sensitivity, 69.56%; specificity, 100%). Further, the overall agreement between the microfluidic chip and Sanger sequencing was satisfactory, with a turnaround time of the microfluidic chip was approximately 2 hours, much shorter than the conventional DST method.Conclusion: The proposed microfluidic-based KASP assay provides a cost-effective and convenient method for detecting mutations associated with drug resistance in M. tuberculosis. It represents a promising alternative to the traditional DST method, with satisfactory sensitivity and specificity and a much shorter turnaround time.

Abstract:OBJECTIVE To introduce a new genotyping method, mycobacterial interspersed repetitive units (MIRU) typing, for Mycobacterium tuberculosis and to evaluate its feasibility. METHODS Mycobacterium tuberculosis strains stored at Shanghai Municipal Center for Disease Control and Prevention during 2000 to 2002, were randomly selected by simple digital table and genotyped by MIRU and Spoligotyping. RESULTS By Spoligotyping method, 91 strains were typed to 20 genotypes, of which 89% (81/91) strains belonged to Beijing genotype, while by MIRU method, these strains were divided into 46 genotypes. The MIRU typing showed high discriminatory power, especially for the Beijing genotype strains. The 81 Beijing genotype strains could be subdivided into 39 different MIRU genotypes. In this sample collection, 12 MIRU loci showed different discriminative according to their allelic diversity. Locus 26 showed highly discriminative, while locus 16, 31, and 40 showed moderately discriminative. CONCLUSIONS MIRU genotyping is a simple and fast method. Its numerical result facilitates the comparison among strains of Mycobacterium tuberculosis from different labs.
The Epidemiological Significance And Temporal Stability Of Mycobacterial Interspersed Repetitive Units-Variable Number Of Tandem Repeats-Based Method Applied To Mycobacterium Tuberculosis In China

Abstract:This study aimed to validate the epidemiological significance and temporal stability of Mycobacterial Interspersed Repetitive Units -Variable Number of Tandem Repeats (MIRU-VNTR) typing in a genetically and geographically diverse set of clinical isolates from patients diagnosed with pulmonary tuberculosis in China. Between 2010 and 2013, a total of 982 Mycobacterium tuberculosis isolates were collected from four population -based investigations in China. Apart from the currently applied 24 -locus MIRU-VNTR, six additional hypervariable loci were analyzed in order to validate the MIRU-VNTR combinations in terms of their epidemiological links, clustering time span, and paired geographic distance. In vitro temporal stability was analyzed for both individual MIRU-VNTR loci, and for several combinations of loci. In the present study, four MIRU-VNTR combinations, including the hypervariable loci 3820, 3232, 2163a, and 4120, were evaluated. All of these combinations obtained a Hunter -Gaston discriminatory index (HGDI) value over 0.9900 with a reduced clustering proportion (from 32.0% to 25.6%). By comparing epidemiological links, clustering time span, and paired geographic distance, we found that the performances of the four MIRU-VNTR combinations were comparable to the insertion sequence 6110 restriction fragment length polymorphism (IS6110-RFLP), and significantly better than that of 24 -locus MIRU-VNTR genotyping alone. The proportion of temporally stable loci ranged from 90.5% to 92.5% within the combined MIRU-VNTR genotyping, which is higher than IS6110-RFLP (85.4%). By adding four hypervariable loci to the standard 24 -locus MIRU-VNTR genotyping, we obtained a high discriminatory power, stability and epidemiological significance. This algorithm could therefore be used to improve tuberculosis transmission surveillance and outbreak investigation in China.

Abstract:The standard 15- and 24-locus variable-number tandem repeat (VNTR) genotyping methods have demonstrated adequate discriminatory power and a small homoplasy effect for tracing tuberculosis (TB) transmission and predicting Mycobacterium tuberculosis lineages in European and North American countries. However, its validity for the definition of transmission in homogenous M. tuberculosis populations in settings with high TB burdens has been questioned. Here, we genotyped a population-based collection of 191 Beijing strains based on standard 15-locus VNTR (VNTR-15) and 8 single nucleotide polymorphisms (SNPs) in Shanghai, China. Limited discriminatory power and high rates of VNTR homoplasy were observed in the homogenous population of evolutionarily "modern" Beijing strains. Additional typing of three hypervariable loci (VNTR3820, VNTR4120, and VNTR3232) was performed for VNTR-15-based clusters. High variations of hypervariable alleles were observed in clusters with inconsistent SNP sublineages. We concluded that SNPs and hypervariable VNTR loci are helpful to enhance the discriminatory power and decrease the VNTR homoplasy effect for defining clusters. We recommend the combination of standard VNTR-15 and SNPs as first-line typing methods and the hypervariable loci for second-line typing of clustered strains for molecular epidemiology studies of homogenous M. tuberculosis populations.

Abstract:A fully automated bacteria whole genome sequencing (WGS) assay was evaluated to characterize Mycobacterium tuberculosis (MTB) and non-tuberculosis Mycobacterium (NTM) clinical isolates. The results generated were highly reproducible, with 100% concordance in species and sub-lineage classification and 92% concordance between antimicrobial resistance (AMR) genotypic and phenotypic profiles. Using extracted deoxyribonucleic acid (DNA) from MTB clinical isolates as starting material, these findings demonstrate that a fully automated WGS assay, with a short turnaround time of 24.5 hours, provides timely and valuable insights into MTB outbreak investigation while providing reliable genotypic AMR profiling consistent with traditional antimicrobial susceptibility tests (AST). This study establishes a favorable proposition for the adoption of end-to-end fully automated WGS solutions for decentralized MTB diagnostics, thereby aiding in World Health Organization's (WHO) vision of tuberculosis eradication.

The Combined Application of Multiple Genotyping Methods in Identifying Genotypes of Mycobacterium Tuberculosis Strain Circulating in Rural China

Abstract:OBJECTIVE:To evaluate the performance of MIRUs (mycobacterial interspersed repetitive units) genotyping alone, IS6110-RFLP (IS6110 restriction fragment length polymorphism) genotyping alone and their combination applied in the molecular epidemiological study of Mycobacterium tuberculosis (MTB) isolates circulating in rural China.METHODS:A cross-sectional study was designed to collect MTB isolates from the TB patients registered in local TB dispensaries of Deqing county and Guanyun county from 2004 to 2005. The proportion method was used to determine drug susceptibility of MTB isolates to the first line anti-TB drugs (isoniazid, rifampin, ethambutol and streptomycin). The Beijing family of MTB was identified by Spoligotyping. All isolates were genotyped by MIRUs alone, IS6110-RFLP alone and their combination.RESULTS:Of the 351 studied MTB isolates, 243 (69.2%) had the genotypes that belonged to the Beijing family, and 223 (63.5%) were resistant to at least 1 anti-TB drug, including 53 (15.1%) resistant to isoniazid and rifampin simultaneously or multidrug resistant (MDR). The heterogeneity of 12 MIRUs loci differed from 0.76 in MIRU26 to 0.003 in MIRU2. MIRUs alone identified 235 genotypes (HGI = 0.9317), including 46 "clusters" containing 162 isolates and 189 "unique" pattern/isolates. Thirty-eight isolates comprised the largest MIRUs defined cluster and presented MIRUs type 2233 2517 3533. MIRUs based clusters were further identified by IS6110-RFLP into 28 subgroups containing 80 isolates (HGI = 0.9989). In comparison, IS6110-RFLP determined 267 genotypes from all isolates, including 46 "clusters" containing 130 isolates (HGI = 0.9684) and 221 "unique" pattern/isolates. All the 46 IS6110-RFLP defined clusters could also be further identified by MIRUs into 31 subgroups (HGI = 0.9992). The performance of MIRUs prior to IS6110-RFLP combination was comparable to that of IS6110-RFLP prior to MIRUs combination, especially in Beijing genotype strain (HGI: 0.9930 vs 0.9933) and MDR-TB strains (HGI: 0.9965 vs 0.9963).CONCLUSION:For feasibility, cost and discriminatory power, MIRUs prior to IS6110-RFLP combination is more suitable for the massive epidemiological investigation of MTB in rural China.
Rapid Detection of Rpsl and Rrs Mutations in Mycobacterium Tuberculosis by DNA Chip

Abstract:[Objective] To develop a new method, DNA chip, which can be used for rapid detection of rpoB mutations in Mycobacterium tuberculosis. [Method] According to the rpoB gene sequence of Mycobacterium tuberculosis, the oligonucleotide probes were designed and synthesized, and then DNA chips were prepared. The DNA fragment which contains hot mutation sites of rpoB gene was amplified with TAMRA-lablled primers by PCR. Then it was hybridized with gene chip. PCR-SSCP technique and DNA sequencing were used as the control. [Results] Of 240 M. tuberculosis clinical isolates, the results of DNA chips showed that the rpoB genes from 55 of drug-sensitive strains and 66 rifampicin-sensitive strains (but resistance to other anti-TB drug) were all wild types, similar to that of PCR-SSCP. Of 119 M. tuberculosis rifampicin-resistant isolates, 117 strains had rpoB gene mutations, 111 strains were single location mutations. 78 strains had a base mutations at codon 531, which were TCG→TTG (Ser→Leu), TCG→TGG (Ser→Trp) and TCG→TAC (Ser→Tyr). 24 strains had mutations of CAC→TAC (His→Tyr), CAC→GAC (His→Asp), CAC→CCC (His→Pro), CAC→CTC (His→Leu)and CAC→CGC (His→Arg) at codon 526. 4 strains had GAC→GTC (Asp→Val) and GAC→GGC (Asp→Gly) mutations at codon 516. 3 strains had CTG→CCG (Leu→Pro) mutation at codon 533. 1 strain had CTG→CCG (Leu→Pro) mutation at codon 511. 1 strain had CAA→AAA (Gln→Lys) mutation at codon 513. 6 strains had a base mutation at two different codons, respectively. Of them, 3 strains had base mutations at codon 511 and 516, 2 strains at codon 533 and 515, 1 strain at codon 517 and 518. The results of DNA chips were consistent with that of DNA sequencing. 1 strain that had GAC-TAC (Asp→Tyr) mutation at codon 516 and 1 strain that had GAC-TAC (Asp→Tyr) mutations at codon 516 and AAC-CAC (Asn→His) at codon 518 showed wild type by DNA chips, which were because there were no relevant specific probes on DNA chips. [Conclusion] DNA chip might be a rapid and specific method for the detection of rpoB gene mutation in the majority of rifampin-resistant Mycobacterium tuberculosis.

Abstract:There is a growing awareness that molecular diagnostics for detect-to-treat applications will soon need a highly multiplexed mutation detection and identification capability. In this study, we converted an open-amplicon microarray hybridization test for multidrug-resistant (MDR) Mycobacterium tuberculosis into an entirely closed-amplicon consumable (an amplification microarray) and evaluated its performance with matched sputum and sediment extracts. Reproducible genotyping (the limit of detection) was achieved with ∼25 M. tuberculosis genomes (100 fg of M. tuberculosis DNA) per reaction; the estimated shelf life of the test was at least 18 months when it was stored at 4°C. The test detected M. tuberculosis in 99.1% of sputum extracts and 100% of sediment extracts and showed 100% concordance with the results of real-time PCR. The levels of concordance between M. tuberculosis and resistance-associated gene detection were 99.1% and 98.4% for sputum and sediment extracts, respectively. Genotyping results were 100% concordant between sputum and sediment extracts. Relative to the results of culture-based drug susceptibility testing, the test was 97.1% specific and 75.0% sensitive for the detection of rifampin resistance in both sputum and sediment extracts. The specificity for the detection of isoniazid (INH) resistance was 98.4% and 96.8% for sputum and sediment extracts, respectively, and the sensitivity for the detection of INH resistance was 63.6%. The amplification microarray reported the correct genotype for all discordant phenotype/genotype results. On the basis of these data, primary sputum may be considered a preferred specimen for the test. The amplification microarray design, shelf life, and analytical performance metrics are well aligned with consensus product profiles for next-generation drug-resistant M. tuberculosis diagnostics and represent a significant ease-of-use advantage over other hybridization-based tests for diagnosing MDR tuberculosis.
The Relationship of Self-Esteem, Impulsivity and Temperament in Bipolar Patients: Is it Differentially Related to Gender?

Abstract:Objective: The aim of this study was to investigate the relation between self-esteem, impulsivity and temperament, and to investigate whether it is differentially related to gender. Methods: One hundred consecutive cases who referred to our outpatient clinic for routine control and gave informed consent and who were diagnosed with Bipolar Disorder according to DSM-IV and were in remission period for at least eight week were evaluated. Impulsivity was evaluated with the Barrat Impulsivity Scale, self-esteem was evaluated with the Rosenberg self-esteem scale and affective temperament was evaluated with the Temperament Evaluation of the Memphis, Pisa, Paris, and San Diego Autoquestionnaire. Results: A moderate inverse relation was found between impulsivity and self-esteem. This relation was stronger in women than men. Impulsivity was found to be moderately related with depressive and anxious temparements and strongly related with cyclothymic and irritable temparements. There was no relation between impulsivity and hyperthymic temperament. A moderate level relation was found between self-esteem and hyperthymic temperament. This relation was stronger in men than women. Conclusion: Self-esteem, impulsivity and temperament are differentially related to gender in bipolar patients.
Rapid identification of Mycobacterium species by DNA chip

Abstract:Objective To utilize the high-scale and high efficiency of DNA chip to develop a rapid,simple , and specific method for identification of Mycobacterium species, to provide a basis for the correct diagnosis of mycobacterium diseases. Methods DNA sequencing was used as the control, the target DNA fragments of 28 Mycobacterium reference strains,9 non-Mycobacterium strains and 465 Mycobacterium isolates were analyzed by PCR-SSCP and DNA chip. Results The reference strains of 28 Mycobacteria and 9 non-Mycobacteria were analyzed by DNA chips, the results showed that the DNA chip was specific. Of 465 mycobacterium Clinical isolates, 256 strains were identified as Mycobacterium tuberculosis complex(MTBC)with PCR-SSCP,and were positive hybridization with Mycobacterium genus probe M and MTBC-specific probe a on DNA chip.209 strains were identified as non-tuberculosis mycobacteria with PCR-SSCP,and their species were identified by DNA chips as follows: 68 were M. chelonae , 46 were M. intracellulare , 34 were M. kansasii, M. gastri, M. scrofulaceum , M. simiae complex,31 were M. fortuitum, 16 were M. gordonae,3 were M. avium,2 were M. marinum and M. abscess complex , 1 was M. terrae , 1 was M. diernhoferi, and 1 was M, phlei. 6 strains that were only positive hybridization with probe M were sequenced and showed that 5 were M. intracellulare, which had different DNA sequences with the reference strain, and 1 was M. neoaurum. There is not specific probe for M. neoaurum on DNA chip. Conclusion It might be a simple, rapid, sensitive and specific method for identification of most Mycobacterium species byDNA chip, and it could raise correct diagnosis rate of Mycobacterium diseases, and direct the physicians toperform rational chemotherapy.

Abstract:Drug-resistant tuberculosis (TB), especially multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB), is one of the urgent clinical problems and public health challenges. Culture-based phenotypic drug susceptibility testing (pDST) is time-consuming, and PCR-based assays are limited to hotspot mutations. In this study, we developed and validated a convenient and efficient approach based on high-throughput nanopore sequencing technology combined with multiplex PCR, namely nanopore targeted sequencing (NTS), to simultaneously sequence 18 genes associated with antibiotic resistance in Mycobacterium tuberculosis (MTB). The analytical performance of NTS was evaluated, and 99 clinical samples were collected to assess its clinical performance. The NTS results showed that MTB and its drug resistance were successfully identified in approximately 7.5 h. Furthermore, compared to the pDST and Xpert MTB/RIF assays, NTS provided much more drug resistance information, covering 14 anti-TB drugs, and it identified 20 clinical cases of drug-resistant MTB. The mutations underlying these drug-resistant cases were all verified using Sanger sequencing. Our approach for this TB drug resistance assay offers several advantages, including being culture-free, efficient, high-throughput, and highly accurate, which would be very helpful for clinical patient management and TB infection control.

A Novel Method of Identifying Mycobacterium Tuberculosis Beijing Strains by Detecting SNPs in Rv0444c and Rv2629

Abstract:A particular genotype of tuberculosis, named Beijing strain, is strongly associated with drug resistance and high virulence. Therefore, rapid prospective identification of Mycobacterium tuberculosis Beijing strains is very important for identifying and controlling tuberculosis of Beijing genotype. In the present study, we found that the co-mutation, A191C in Rv2629 and G243C in Rv0444c, is closely related to Beijing genotype. Gene Rv2629 and Rv0444c of 139 clinical isolates of M. tuberculosis were analyzed by PCR amplification and sequencing. Among 99 Beijing strains, 86 % (n = 85) isolates had the mutation G243C in Rv0444c and 92.93 % (n = 92) isolates had the mutation A191C in Rv2629. Among 40 non-Beijing isolates, only six isolates carried the mutation G243C in Rv0444c and eight isolates carried the mutation A191C in Rv2629. The co-mutation existed in 84.85 % (n = 84) of 99 clinical genome samples of W-Beijing strains and in only 12.5 % (n = 5) of the 40 non-Beijing strains, and the positive predictive value of 94.38 %, obtained in our experiment with a designed ratio of Beijing isolates, is similar to that in China at present. This result suggested that the detection method of the co-mutation, A191C in Rv2629 and G243C in Rv0444c, proposed in this study was a rapid, reliable, and sensitive one for identifying tuberculosis with Beijing genotype.
Pan-lineage Mycobacterium tuberculosis reference genome for enhanced molecular diagnosis

Abstract:In Mycobacterium tuberculosis (MTB) control, whole genome sequencing-based molecular drug susceptibility testing (molDST-WGS) has emerged as a pivotal tool. However, the current reliance on a single-strain reference limits molDST-WGS's true potential. To address this, we introduce a new pan-lineage reference genome, 'MtbRf'. We assembled 'unmapped' reads from 3,614 MTB genomes (751 L1; 881 L2; 1,700 L3; and 282 L4) into 35 shared, annotated contigs (54 coding sequences [CDSs]). We constructed MtbRf through: (1) searching for contig homologues among genome database that precipitate results uniquely within Mycobacteria genus; (2) comparing genomes with H37Rv ('lift-over') to define 18 insertions; and (3) filling gaps in H37Rv with insertions. MtbRf adds 1.18% sequences to H37rv, salvaging >60% of previously unmapped reads. Transcriptomics confirmed gene expression of new CDSs. The new variants provided a moderate DST predictive value (AUROC 0.60-0.75). MtbRf thus unveils previously hidden genomic information and lays the foundation for lineage-specific molDST-WGS.

Abstract:In this study, we evaluated the diagnostic accuracy of multiple cross displacement amplification (MCDA) combined with real-time PCR platform in pulmonary tuberculosis (PTB) patients. Total 228 PTB patients and 141 non-TB cases were enrolled. Based on the analysis of the first available sample of all participants, MCDA assay showed a higher overall sensitivity (64.0%), with a difference of more than 10% compared with Xpert MTB/RIF (Xpert) assay (51.8%, P < 0.05) and combined liquid and solid culture (47.8%, P < 0.001) for PTB diagnosis. In particular, MCDA assay detected 31 probable TB patients, which notably increased the percentage of confirmed TB from 57.9% (132/228) to 71.5% (163/228). The specificities of microscopy, culture, Xpert and MCDA assay were 100% (141/141), 100% (141/141), 100% (141/141), and 98.6% (139/141), respectively. Among the patients with multiple samples, per patient sensitivity of MCDA assay was 60.5% (52/86) when only the first available sputum sample was taken into account, and the sensitivity increased to 75.6% (65/86) when all samples tested by MCDA assay were included into the analysis. Therefore, MCDA assay established in this study is rapid, accurate and affordable, which has the potential in assisting the accurate and rapid diagnosis of PTB and speed up initiation of TB treatment in settings equipped with real-time PCR platform.

Abstract:OBJECTIVE:To identify a variable number of tandem repeats (VNTR) typing method that is suitable for molecular epidemiological study of tuberculosis in China.METHODS:We systematically evaluated the commonly used VNTR typing methods, including 4 methods (MIRU-12, VNTR-15/VNTR-24 and VNTR "24+4") proposed by foreign colleagues and 2 methods (VNTR-L15 and VNTR"9+3") developed by domestic researchers using population-based collection of 891 clinical isolates from 5 provinces across the country.RESULTS:The order (from high to low) of discriminatory power for the 6 VNTR typing methods was VNTR"24+4", VNTR"9+3", VNTR-24, VNTR-15, VNTR-L15 and MIRU-12. The discriminatory power of VNTR"9+3" was comparable with VNTR"24+4" and higher than that of VNTR-15/24. The concordance for defining clustered and unique genotypes between VNTR"9+3" and VNTR"24+4" was 96.59%.CONCLUSIONS:Our results suggest that VNTR"9+3" is a suitable method for molecular typing of M. tuberculosis in China by considering its high discriminatory power, high consistency with VNTR"24+4" and relative small number of VNTR locus.
