Abstract:Background and Aim: Chronic hepatitis C virus (HCV) infection is the leading cause of liver diseases including cirrhosis and hepatocellular carcinoma. In China, it is a major national health problem that demands nationwide coordinated emphasis on prevention and treatment. To inform these initiatives, a nationwide survey was conducted from January to April 2015 to evaluate the knowledge, awareness, and perceived obstacles to HCV care. Methods: A sample of 1000 HCV specialists across mainland China were recruited. Respondents were asked a series of 30 open-ended single or multiple response and Likert-scale questions about their HCV treatment knowledge, experience, assessment of HCV care status in China, and perceptions about treatment barriers. Results: Sixty percent of the respondents answered incorrectly to more than half of the questions on basic HCV treatment principles. Over half of them incorrectly believed that maintenance therapy should be prescribed for non-responders (72%) and longer treatment duration improved sustained viral response rates (62%), regardless of HCV RNA level changes. Sixty-six percent of them believed that HCV treatment would still be interferon-based therapy in the next 5 years in China. Patient-related barriers, in particular lack of disease awareness, were considered to be the most significant barriers to HCV care. Payer and medical-provider barriers included affordability issues, lack of reimbursement coverage for testing and treatments, and lack of referral to HCV specialists. Conclusions: Focused and intense patient and provider education should be carried out to increase awareness. More effective direct-acting antivirals should be made available and affordable in China.
Clinical features of treatment-naive patients with hepatitis B virus infection: A community-based survey from high- and intermediate-hepatitis B endemicity regions in Southeast China

Abstract:The aim of study was to investigate the clinical features of treatment-naive patients in 2 regions with high-and intermediate-hepatitis B endemicity level in Southeast China and provide the baseline data for monitoring health or planning therapy. This study included 8207 cases of treatment-naive patients with hepatitis B virus (HBV) infection from Yuhuan (YH, high-hepatitis B endemicity region) and Shaoxing (SX, intermediate-hepatitis B endemicity region) during 2014-2015. Clinical data were collected from the patients. Blood samples were kept for detecting hepatitis B surface antigen, hepatitis B envelope antigen (HBeAg), hepatitis B envelope antibody, hepatitis B surface antibody, hepatitis B core antibody, liver function, HBV DNA, and alpha-fetoprotein. All persons underwent B ultrasound to exclude liver cirrhosis or cancer. Of all 8207 HBsAg-positive patients, 52.9% patients were in the low-replication (LR) stage and 30.3% in the HBeAg-negative chronic hepatitis B (ENH) stage; 8.8% cases were in the ENH stage with elevated alanine aminotransferase (ALT). More male than female patients were in immune clearance (IC) or ENH stages with elevated ALT (10.4% vs 4.8%, 12.1% vs 5.3%, respectively, P<. 05). The percentage of patients in IC and immune tolerant (IT) stages declined with increasing age, whereas the percentages of ENH with elevated ALT stage were highest in 40 to 60 years. The percentage of patients in IT and IC stages was higher in YH than in SX (9.4% vs 3.8%, 9.9% vs 4.2%, respectively, P<. 05). More patients had HBVDNA >= 10(5)IU/mL in YH than in SX (24.6% vs 16.0%, P<. 05), and more male than female patients had HBVDNA >= 10(5)IU/mL(24.5% vs 17.9%, P<. 05). Clinical features varied in treatment-naive patients with HBV infection between different genders and regions. More attention should be paid to the surveillance and therapy of patients in YH especially male patients for the prevention and prognosis of hepatitis B.

Abstract:Background/purpose: Hepatitis C virus (HCV) is a major public health problem and can cause severe liver disease. The public has little understanding of hepatitis C. Therefore, a survey was performed to investigate the awareness of nonspecialists in China of hepatitis C.Methods: A questionnaire on hepatitis C was publicized on the medical community site DXY. cn from April 15 to July 15, 2009. The questionnaire included ten questions and covered the etiology, epidemiology, diagnosis, treatment, and prevention of hepatitis C.Results: About 1362 physicians from various regions, hospitals, and departments in China took part in the survey. Eleven percent mistakenly believed that the disease was not transmitted from mother to infant, by sharing a needle or syringe, by sharing a razor or toothbrush, or by having sex with a partner with HCV infection. Sixty-two percent did not regard anti-HCV antibody detection as a routine procedure for hospitals. Forty-four percent of respondents indicated that they would not advise a patient who tested positive for anti-HCV antibody to consult a specialist. Seventy-one percent thought that hepatitis C was incurable or had a very low cure rate. Nearly 50% thought that there were no effective treatments for hepatitis C.Conclusion: Chinese nonspecialists have a little understanding of hepatitis C, which may have a negative effect on the prevention and treatment of hepatitis C.
Prevalence of Hepatitis C Virus Infection in a Surgical Population of Southeast China: A Large-Scale Multicenter Study.

Abstract:Background. Chronic HCV infection affects 80 million people globally and may progress to advanced liver disease. The present study aims to investigate the present epidemiology of HCV infection in a southeastern Chinese surgical patient cohort. Methods. Blood samples obtained from 78,484 surgical patients from 18 different city and county hospitals were enrolled. The incidence of serum HCV antibody positivity, HCV RNA load, and HCV genotyping, as well as demographics and relevant clinical history, were investigated. Data were stratified using the multistage cluster random sampling method and further analyzed using the SPSS-20 package. Results. HCV antibody positivity was detected in 0.15% of the population (95% confidence interval (CI): 0.12%-0.18%). Genotype 1b (55.74%) was the dominant type. The HCV infection peaked in the age groups of 16-20, 41-50, and 61-65 years, and it was higher in males than in females (0.19% vs. 0.13%, P<0.05). The geographical distribution of infection rates differed: 0.19% (95% CI: 0.14%-0.24%), 0.18% (95% CI: 0.13%-0.23%), and 0.06% (95% CI: 0.03-0.09%) in plain areas, islands, and valley regions, respectively. Patients with transfusion history and urban residence were associated with high HCV RNA levels (adjusted odds ratio = 11.24 and 6.20, P<0.05). Conclusion. The prevalence of HCV infection in this cohort from southeast China was 0.17%, which is lower than the reported 0.43% infection rate in China in 2006. This result can be (partially) explained by the improvement of blood donor screening and the successful campaign for the use of disposable syringes and needles.
Factors associated with specialist assessment and treatment for hepatitis C virus infection in New South Wales, Australia

Abstract:Assessment and treatment for hepatitis C virus (HCV) in the community remains low. We evaluated factors associated with HCV specialist assessment and treatment in a cross-sectional study to evaluate treatment considerations in a sample of 634 participants with self-reported HCV infection in New South Wales, Australia. Participants having received HCV specialist assessment (n = 294, 46%) were more likely to be have been older (vs <35 years; 35-44 OR 1.64, P = 0.117; 45-54 OR 2.00, P = 0.024; ≥55 OR 5.43, P = 0.002), have greater social support (vs low; medium OR 3.07, P = 0.004; high OR 4.31, P < 0.001), HCV-related/attributed symptoms (vs none; 1-10 OR 3.89, P = 0.032; 10-21 OR 5.01, P = 0.010), a diagnosis of cirrhosis (OR 2.40, P = 0.030), have asked for treatment information (OR 1.91, P = 0.020), have greater HCV knowledge (OR 2.49, P = 0.001), have been told by a doctor to go onto treatment (OR 3.00, P < 0.001), and less likely to be receiving opiate substitution therapy (OR 0.10, P < 0.001) and never to have seen a general practitioner (OR 0.24, P < 0.001). Participants having received HCV treatment (n = 154, 24%) were more likely to have greater fibrosis (vs no biopsy; none/minimal OR 3.45, P = 0.001; moderate OR 11.47, P < 0.001; severe, OR 19.51, P < 0.001), greater HCV knowledge (OR 2.57; P = 0.004), know someone who has died from HCV (OR 2.57, P = 0.004), been told by a doctor to go onto treatment (OR 3.49, P < 0.001), were less likely to have been female (OR 0.39, P = 0.002), have recently injected (OR 0.42, P = 0.002) and be receiving opiate substitution therapy (OR 0.22, P < 0.001). These data identify modifiable patient-, provider- and systems-level barriers associated with HCV assessment and treatment in the community that could be addressed by targeted interventions.

Patient Education Improves Patient Knowledge and Acceptance on Antiviral Therapy of Hepatitis C in Rural China.

Abstract:It is estimated that 130–150 million people are chronically infected with hepatitis C virus (HCV) worldwide. During the late 1980s and early 1990s, illegal commercial plasma or blood donation practices were common in selected rural areas of China, which caused a rapid spread of HCV infection. Cross-sectional studies have showed that the prevalence rates of HCV infection in former paid plasma or blood donors varied from 9.6% to 72.8%. Chronic HCV infection can cause cirrhosis, liver failure, and hepatocellular carcinoma (HCC), and is a major global health problem. Eradicating HCV can prevent decompensation of cirrhosis, HCC, and death.[1] The availability of direct-acting antiviral agents (DAAs) has significantly increased sustained virological response rates and decreased the complexity and adverse effects of HCV treatment. Much of the current debate on HCV treatment focuses on the high cost of these drugs, but even if the cost of HCV drugs is affordable, treatment would only benefit infected persons who have been diagnosed, have access to care, and wish to be treated. Despite the availability of effective treatments for HCV, globally no more than 15% of those infected are aware of their status and no more than 20% of those diagnosed have been treated. To prospectively evaluate the impact of HCV education on patients’ knowledge of HCV and acceptance of antiviral therapy of HCV in rural China, we conducted HCV education program in treatment-naïve patients who joined the Hebei Province Gu’an County Chronic Hepatitis C Program. A total of 151 HCV patients participated in the study including a lecture given by a doctor followed by an interactive discussion and completed both pre- and post-education questionnaires. Prior to the education session, the mean knowledge score was 13.1 ± 5.2 (total score: 28) and the mean percentage of patients with correct answers to each question was 46.8%. Only 41% of the patients knew that there is antiviral therapy for hepatitis C before education. Among those who knew that there is treatment for hepatitis C, only 34% would accept antiviral treatment if recommended. After education, the mean knowledge score increased to 23.0 ± 4.2 (P < 0.001) and the mean percentage of patients with correct answers to each question increased to 82.1% (P < 0.001). The acceptance of antiviral therapy increased from 34% to 66%. Knowledge about HCV is important for those infected, to lower the risk of transmission to others and to help patients make decisions about treatment. Prior studies have shown that HCV-infected persons have limited knowledge of HCV disease, especially in rural areas. A prior study of HCV patients by our group at Ann Arbor, Michigan, US; Beijing, Urban China; and Hebei, rural China showed that HCV knowledge was similar in patients in the US and urban China, while significantly lower among patients in rural China.[2] Lower knowledge in patients in rural China may be a result of many factors including limited education, low health literacy, and lack of access to medical information. This highlights the necessity for a comprehensive HCV education program to reduce the gaps in HCV knowledge in rural China. Despite the low education level of patients and low baseline HCV knowledge, we showed that HCV knowledge can be significantly improved after education. There is no consensus on the most effective method to educate patients. To achieve the best outcome, educational interventions must be culturally sensitive and tailored to the education levels of the population. Large surveys consistently showed that patients considered medical providers to be their most trusted source of medical information. However, a survey showed that the awareness of hepatitis C among nonspecialist physicians was inadequate in China, especially in rural areas.[3] Hence, the government needs to take measures to improve the HCV knowledge of physicians in rural areas first, such as holding regular training courses. Although education from health-care providers appears to be most effective, other sources of information can serve as supplements. Due to the low education levels, HCV patients in rural areas could fairly obtain HCV knowledge through their own reading or online resources. Educational pamphlets prepared in languages at the appropriate reading level or alternatives such as videos can help to overcome the literacy barriers. In addition, education should be tailored to patient's baseline knowledge of HCV. An assessment of education level and prior knowledge would provide valuable guidance for developing appropriate educational plans and for evaluating the effectiveness of the education. The content of HCV education should at least cover topics on HCV transmission, natural history, HCV testing, and treatment progressions. Since the Chinese government implemented strict management on blood stations in 1995 and Blood Donation Law of China in 1998, the transmission through blood transfusion has gradually decreased. However, few patients in rural areas knew that sharing syringes or needles, unprotected sexual intercourse, tattooing, and sharing personal items such as razors could spread HCV. Deficiencies in these areas have important implications for the risk of transmission to others. Since HCV infection is asymptomatic in the initial period, it usually remains neglected for many years until complications have developed. Thus, disseminating the knowledge about the natural history of HCV infection to patients is important for them to make the decision to accept antiviral treatment. HCV testing mainly include testing for hepatitis C antibody, RNA, and genotype. HCV patients in rural areas usually know little about HCV genotype though the information can guide the type and duration of antiviral treatment. Patient knowledge deficiency in HCV genotype may be related to the high cost of the test and physicians may not have recommended the test knowing that treatment is not affordable by most patients. Daclatasvir and asunaprevir were recently approved and many other DAAs will soon be approved in China. Compared to prior standard treatment (pegylated interferon and ribavirin), DAAs have stronger antiviral effect and less adverse events. Helping patients understand hepatitis C can increase the uptake of antiviral treatment. In general, patient-related barriers were viewed as the most significant barrier to HCV treatment, including low health literacy, limited access to medications and experienced providers, fear of side effects, medication expense, and low success rate of treatment.[4] Patient education can improve patients’ health literacy and improvement in the disease knowledge and acceptance of treatment, but the lack of trained specialists in rural areas may limit the effect of patient education. To properly address patient fears, physicians in rural areas must have a thorough understanding of antiviral therapy. Even after education, we still found a substantial proportion of patients in rural areas not willing to accept antiviral therapy. Medication expense remains a major barrier. Improving the management of HCV infection in rural areas needs not only efforts from patient and health-care provider but also efforts from the government. The cost of DAAs will make the treatment less accessible unless the Chinese health department can negotiate special pricing for the rural Chinese. The Chinese health-care system is evolving, and while the government covers partial cost of medical care, the reimbursement ratio is lower in rural areas than that in urban areas. Since 2003, the Chinese government has initiated free antiretroviral therapy programs for the HIV-infected rural plasma donors, but until recently, the free antiviral therapy programs for the HCV-infected rural plasma donors are not widely implemented. In summary, HCV education substantially improved patient knowledge of HCV disease and acceptance of antiviral therapy in rural China. Selecting appropriate education methods and contents for patients in rural areas is especially important to increase the acceptance of antiviral therapy. The Chinese health department also needs to allocate sufficient funds to training physicians and to HCV diagnosis and treatment to improve HCV patient outcomes in rural areas. Abstract:Background and AimIn China, chronic hepatitis C virus (HCV) infection represents a considerable healthcare burden. Although interferon-based therapy has been the standard-of-care for many years, few long-term, real-life studies have assessed interferon-based treatment in China. The objective of CCgenos follow-up study was to analyze long-term treatment patterns and outcomes in a cohort of treatment-naive, Han ethnic, patients with chronic HCV infection. MethodsPatients who had participated in the CCgenos cross-sectional study were invited to enter this 5-year follow up. Clinical information and centralized HCV-RNA measures were collected at scheduled study visits every 6months for untreated patients and every 3months for treated patients. ResultsAmong 512 patients enrolled, 334 (65.2%) received interferon-based treatment and 178 (34.8%) remained untreated over a median of 4.1 (1.2-4.3) years. A total of 82.8% (424/512) of patients had an IL28B CC genotype (GT); 60.7% (311/512) had HCV GT1b infection, including 121 (38.9%) untreated. Most patients with baseline cirrhosis were untreated (26/46, 56.5%). Among patients who completed treatment and 24weeks of post-treatment follow up, the duration of interferon-based therapy was frequently longer than recommended (52.9% [92/174] of GT1b-infected were treated for >1year). Rates of sustained virologic response (SVR24) were 71.1% (226/318) overall; 62.4% (111/178) among patients with HCV GT1b infection; and 42.9% (15/35) among patients with cirrhosis. ConclusionsThere remains a high unmet need for effective HCV treatment in China, evidenced by a high proportion of patients remaining untreated by the current standard-of-care and relatively low SVR24 rates for patients with both GT1b infection and cirrhosis.
2216. Hepatitis C Treatment Wanted Yet Not Received: Barriers to Receiving HCV Treatment Among People Who Inject Drugs

Abstract:To expand hepatitis C (HCV) treatment for people who inject drugs (PWID), programs need to overcome barriers to initiating treatment. We asked HCV-infected PWID about past experiences with HCV care. These data are collected from the first 44 participants enrolled in an ongoing study of HCV care for PWID delivered at a syringe services program in New York City. Eligible participants were HCV RNA positive and had injected drugs in the past 90 days. We used a structured interview to ask about prior linkage to HCV treatment and the reasons for not obtaining treatment. Among the 44 participants, mean age is 40 years; 73% are males; 48% Hispanic, 38% non-Hispanic white; 7% non-Hispanic black and 7% mixed-race or other. Almost all (96%) had health insurance, with 86% having public insurance (Medicaid). Most participants (91%) were aware of their HCV diagnosis before enrollment. All wanted to be treated for HCV, and 88% wanted treatment to prevent infecting others. 48% had previously been referred for HCV treatment, only 21% had been offered treatment, and none had started. When asked about barriers to treatment, 56% participants felt that they would need to stop using drugs to get HCV treatment, 61% felt that HCV treatment had many side effects and 20% reported insurance coverage barriers. Among PWID enrolled at a syringe services program, all wanted HCV treatment; most to prevent infecting others. Despite high rates of insurance coverage and desire to be treated, most participants have neither been referred to HCV care nor offered treatment. Many participants also had misconceptions about HCV treatment eligibility and side effects. Providing HCV care and education in a low-threshold model, such as walk-in visits at community sites, may help alleviate these barriers. S. Abstract:Despite decreasing prevalence, new cases of hepatitis C in China are increasing recently with growing percentage of patients who are with advanced disease, aging, or not eligible for interferon-based treatments. Hepatitis C infection represents a serious public health burden. This review was based on expert's consensus during a medical forum on hepatitis sponsored by the Beijing Wu Jie-Ping Medical Foundation. The literature searches were conducted in PubMed and critical publications in Chinese journals. Data on hepatitis C prevalence, risk factors, viral or host features, and treatment modalities were extracted and reviewed. Recent large-scale surveys reported reducing prevalence of hepatitis C to approximately 0.4% in China, partly because of regulation changes to safer medical practices and illegalizing commercial blood donations. Patient demographics evolved from being dominated by former paid blood donors to include intravenous drug users and others. Although hepatitis C genotype 1 is the most common, other genotypes are emerging in prevalence. The current standard of care is interferon-based without direct acting antivirals. However, many patients failed therapy because of high treatment costs, substantial needs to manage side effects, difficulties with treatment monitoring in the rural areas, and growing populations of elderly and cirrhotic patients. The lack of high efficacy therapies with good safety profile and low disease awareness in China resulted in increasing public burden of advanced hepatitis C disease. Despite significant reduction of hepatitis C prevalence, iatrogenic, nosocomial, and community transmissions are still significant. In addition to promoting disease awareness, interferon-free regimens are needed to reduce the public health burden.

Abstract:Objective: To understand the basic characteristics of previously reported patients with hepatitis C and analyze the related factors affecting their antiviral treatment. Methods: A convenient sampling method was adopted. Patients who had been previously diagnosed with hepatitis C in the Wenshan Prefecture of Yunnan Province and Xuzhou City of Jiangsu Province were contacted by telephone for an interview study. The Andersen health service utilization behavior model and related literature were used to design the research framework for antiviral treatment in previously reported hepatitis C patients. A step-by-step multivariate regression analysis was used in previously reported hepatitis C patients treated with antiviral therapy. Results: A total of 483 hepatitis C patients, aged 51.73 ± 12.06 years, were investigated. The proportion of male, agricultural occupants who were registered permanent residents, farmers and migrant workers was 65.24%, 67.49%, and 58.18%, respectively. Han ethnicity (70.81%), married (77.02%), and junior high school and below educational level (82.61%) were the main ones. Multivariate logistic regression analysis results showed that married patients with hepatitis C (OR = 3.19, 95% CI: 1.93-5.25, compared with unmarried, divorced, and widowed patients) with high school education or above (OR = 2.54, 95% CI: 1.54-4.20, compared with patients with junior high school education or below) were more likely to receive antiviral treatment in the predisposition module. Patients with severe self-perceived hepatitis C in the need factor module (compared with patients with mild self-perceived disease, OR = 3.36, 95% CI: 2.09-5.40) were more likely to receive treatment. In the competency module, the family's per capita monthly income was more than 1,000 yuan (compared with patients with per capita monthly income below 1,000 yuan, OR = 1.59, 95% CI: 1.02-2.47), and the patients had a high level of awareness of hepatitis C knowledge (compared with patients with a low level of knowledge, OR = 1.54, 95% CI: 1.01-2.35), and the family members who knew the patient's infection status (compared with patients with an unknown infection status, OR = 4.59, 95% CI: 2.24-9.39) were more likely to receive antiviral treatment. Conclusion: Different income, educational, and marital statuses are related to antiviral treatment behavior in hepatitis C patients. Family support of hepatitis C patients receiving hepatitis C-related knowledge and their families knowing the infection status is more important in promoting the antiviral treatment of patients, suggesting that in the future, we should further strengthen the hepatitis C knowledge of hepatitis C patients, especially the family support of hepatitis C patients' families in treatment.
Importance of Patient, Provider, and Facility Predictors of Hepatitis C Virus Treatment in Veterans: A National Study

Abstract:OBJECTIVES: Several patient characteristics are known to impact hepatitis C virus (HCV) antiviral treatment rates. However, it is unclear whether, and to what extent, health-care providers or facility characteristics impact HCV treatment rates.METHODS: Using national data obtained from the Department of Veterans Affairs (VA) HCV Clinical Case Registry, we conducted a retrospective cohort study of patients with active HCV viremia, who were diagnosed between 2003 and 2004. We evaluated patient-, provider-, and facility-level predictors of receipt of HCV treatment with hierarchical logistic regression.RESULTS: The overall HCV treatment rate in 29,695 patients was 14.2%. The strongest independent predictor for receipt of treatment was consultation with an HCV specialist (odds ratio=9.34; 8.03-10.87). Patients were less likely to receive HCV treatment if they were Black, older, male, current users of alcohol or drugs, had HCV genotype 1 or 4, had higher creatinine levels, or had severe anxiety/post-traumatic stress disorder or depression. Patients with high hemoglobin levels, cirrhosis, and persistently high liver enzyme levels were more likely to receive treatment. Patient, provider, and facility factors explained 15, 4, and 4%, respectively, of the variation in treatment rates.CONCLUSIONS: Treatment rates for HCV are low in the VA. In addition to several important patient-level characteristics, a specialist consultant has a vital role in determining whether a patient should receive HCV treatment. These findings support the development of patient-level interventions targeted at identifying and managing comorbidities and contraindications and fostering greater involvement of specialists in the care of HCV.

Abstract:We characterized hepatitis C virus (HCV) treatment knowledge, experience and barriers in a cohort of community-based injection drug users (IDUs) in Baltimore, MD. In 2005, a questionnaire on HCV treatment knowledge, experience and barriers was administered to HCV-infected IDUs. Self-reported treatment was confirmed from medical records. Of 597 participants, 71% were male, 95% African-American, 31% HIV co-infected and 94% were infected with HCV genotype 1; 70% were aware that treatment was available, but only 22% understood that HCV could be cured. Of 418 who had heard of treatment, 86 (21%) reported an evaluation by a provider that included a discussion of treatment of whom 30 refused treatment, 20 deferred and 36 reported initiating treatment (6% overall). The most common reasons for refusal were related to treatment-related perceptions and a low perceived need of treatment. Compared to those who had discussed treatment with their provider, those who had not were more likely to be injecting drugs, less likely to have health insurance, and less knowledgeable about treatment. Low HCV treatment effectiveness was observed in this IDU population. Comprehensive integrated care strategies that incorporate education, case-management and peer support are needed to improve care and treatment of HCV-infected IDUs.

Abstract:To determine the role of hepatitis C virus (HCV) genotype 1 b genetic variation in the open reading frame for treatment outcomes of the pegylated-interferon/ribavirin (peg-IFN/RBV) combination therapy by examining patients from Henan Province with chronic hepatitis C (CHC).Thirty-seven treatment naive patients infected with HCV genotype 1b were included in the study. Prior to initiation of a 48-week course of peg-IFN/RBV therapy, peripheral blood was drawn for sequencing of the viral ORF 5'-half. Patients were assessed at the end of the 48 weeks of treatment and at a 6-month follow-up appointment. The patient data was stratified according to the status of sustained viral response (SVR) group and non-response (NR) and statistical analysis was performed to determine the correlation between detected genetic variations and treatment response status.Genetic variability in the ORF 5'-half was significantly higher among the individuals in the SVR group than among those of the NR group. Significant differences were found in the gene regions encoding p7, NS2 and NS3. For p7, the NR group had actual and expected frequencies of special mutation of 9.0 and 17.4 and total mutation of 77.0 and 68.6, while the SVR group had actual and expected frequencies of special mutation of 42.0 and 33.6 and total mutation of 124.0 and 132.4 (x2 =7.725, P =0.05). For NS2, the NR group had actual and expected frequencies of special mutation of 36.0 and 54.3 and total mutation of 270.0 and 251.7, while the SVR group had actual and expected frequencies of special mutation of 106.0 and 87.7 and total mutation of 388.0 and 406.3 (x2 = 12.16, P less than 0.01). For NS3, the NR group had actual and expected frequencies of special mutation of 49.0 and 53.4 and total mutation of 241.0 and 236.6, while the SVR group had actual and expected frequencies of special mutation of 81.0 and 76.6 and total mutation of 335.0 and 339.4 (x2 =6.745, P =0.043). The inter-patient genetic variations in the NS3 gene were concentrated in the protease domain. Furthermore, there was a strong correlation between HCV diversity in p7 and treatment outcome.The genetic data reported here provides strong support for the role of NS2, NS3 and p7 in antagonizing the peg-IFN/RBV response during the treatment of HCV infections. We conclude that higher inter-patient viral genetic diversity correlates with successful treatment and may modulate the efficacy of antiviral therapy in CHC patients of Henan.

Abstract:Background & aims: Oral antiviral therapy with nucleos(t)ide analogues (NAs) for chronic hepatitis B (CHB) is well-tolerated and lifesaving, but real-world data on utilization are limited. We examined rates of evaluation and treatment in patients from the REAL-B consortium. Methods: This was a cross-sectional study nested within our retrospective multinational clinical consortium (2000-2021). We determined the proportions of patients receiving adequate evaluation, meeting AASLD treatment criteria, and initiating treatment at any time during the study period. We also identified factors associated with receiving adequate evaluation and treatment using multivariable logistic regression analyses. Results: We analyzed 12,566 adult treatment-naïve patients with CHB from 25 centers in 9 countries (mean age 47.1 years, 41.7% female, 96.1% Asian, 49.6% Western region, 8.7% cirrhosis). Overall, 73.3% (9,206 patients) received adequate evaluation. Among the adequately evaluated, 32.6% (3,001 patients) were treatment eligible by AASLD criteria, 83.3% (2,500 patients) of whom were initiated on NAs, with consistent findings in analyses using EASL criteria. On multivariable logistic regression adjusting for age, sex, cirrhosis, and ethnicity plus region, female sex was associated with adequate evaluation (adjusted odds ratio [aOR] 1.13, p = 0.004), but female treatment-eligible patients were about 50% less likely to initiate NAs (aOR 0.54, p <0.001). Additionally, the lowest evaluation and treatment rates were among Asian patients from the West, but no difference was observed between non-Asian patients and Asian patients from the East. Asian patients from the West (vs. East) were about 40-50% less likely to undergo adequate evaluation (aOR 0.60) and initiate NAs (aOR 0.54) (both p <0.001). Conclusions: Evaluation and treatment rates were suboptimal for patients with CHB in both the East and West, with significant sex and ethnic disparities. Improved linkage to care with linguistically competent and culturally sensitive approaches is needed. Impact and implications: Significant sex and ethnic disparities exist in hepatitis B evaluation and treatment, with female treatment-eligible patients about 50% less likely to receive antiviral treatment and Asian patients from Western regions also about 50% less likely to receive adequate evaluation or treatment compared to Asians from the East (there was no significant difference between Asian patients from the East and non-Asian patients). Improved linkage to care with linguistically competent and culturally sensitive approaches is needed.
Cross-sectional retrospective analysis of clinical characteristics of chronic hepatitis B patients with oral antiviral treatment in eastern China

Abstract:Background: In China, more than 20 million patients with chronic hepatitis B need antiviral treatment. Side effects of antiviral treatment such as renal complications can be problematic, particularly in an aging population. Methods: The data were retrospectively extracted from the hospital medical charts of five centers in eastern China from January 1 to December 31, 2018. Results: A total of 8309 patients with CHB was enrolled in this study. The median age of the patients was 46 years. The prevalence of diabetes mellitus, hypertension, and hepatic cirrhosis was respectively 3.49%, 4.42%, and 23.72%. The prevalence of these comorbidities increased with age (P < 0.001). Of the patients with CHB, 5332 had complete renal function results. Among them, patients with an estimated glomerular filtration rate of < 60 mL/min/1.73m2 accounted for 4.14%, and those with proteinuria for 8.33%. According to the definition of chronic kidney disease, the proportion of patients with chronic kidney disease was 11.37%. The prevalence of chronic kidney disease increased with age (P < 0.001). In a multivariate analysis, age group [odds ratio (OR) = 2.387], diabetes mellitus (OR = 1.486), hypertension (OR = 2.557), hepatic cirrhosis (OR = 1.295), and a history of exposure to adefovir dipivoxil (OR = 1.644) were significantly associated with CKD (P < 0.05). Among patients with CKD, 17.66% (107/606) had a history of lamivudine exposure, and 34.65% (210/606) had a history of nucleotide analogue exposure CONCLUSION: The management of Chinese patients with CHB should take into consideration age, previous medication history, and renal impairment.

Abstract:Hepatitis C virus (HCV) represents a formidable menace to human health, necessitating urgent attention. The objective of this study was to assess the efficacy and safety of HCV health management in the city of Guigang which consists of five districts, employing a comprehensive multi‐modal approach. The study systematically carried out HCV screening in Guigang city which consists of five districts, such as Gangbei District, Gangnan District, Guiping District, Qintang District, and Pingnan District from 1 January 2016 to 30 December 2022. The target population consisted of individuals residing in these aforementioned districts, falling within the age range of 30–75 years. A multidisciplinary HCV management team was established to deliver anti‐HCV screening, diagnosis, and direct‐acting antiviral (DAA) therapy. The primary outcome of interest was the achievement of sustained virologic response (SVR). A total of 2489 individuals were included as the target population, with 1694 individuals residing in Gangbei District, 202 in Gangnan District, 111 in Qintang District, 167 in Pingnan District, and 315 in Guiping District. Out of these individuals, 2478 were subjected to anti‐HCV screening. The screening rates varied across the districts, ranging from a peak of 99.55% in Guigang City to a nadir of 98.41% in Guiping District. Remarkably, within Guigang City, a noteworthy enhancement was observed in the HCV‐RNA diagnosis rate from 23.4% prior to program implementation to a remarkable 100% following 7 years of intervention and management. Furthermore, the diagnosis and treatment coordination rate experienced a substantial improvement, rising from 26.8% before program inception to 80%. Importantly, a total of 1180 individuals affected by hepatitis C were successfully cured, equating to a 100% cure rate. Logistic regression analysis revealed a significant association between serological status and factors such as Aging, bilirubin, and glutamic oxalacetic transaminase. The findings from our investigation unveil a pioneering HCV management model, exemplified by the Guigang model, which has contributed crucially to HCV microclearance efforts and serves as an invaluable reference for future initiatives.

Abstract:Current antiviral medications are effective in suppressing hepatitis B virus (HBV) replication and improving patient prognosis. 1 -4 However, international HBV guidelines propose different criteria for antiviral treatment. 1 -4 Only 3% of patients received treatment globally in 2022. 4 To improve the uptake of antiviral therapy, simplifying and expanding the eligibility for treatment is the future direction of HBV management. 4 Recently, the World Health Organization (WHO) updated HBV guidelines, with expanded treatment eligibility being a key priority area. 4 ,5 However, there is limited information on the differences in treatment eligibility rates among international hepatitis B guidelines. We compared treatment-eligible rates across Asian Pacific Association for the Study of the Liver (APASL) 2015, American Association for the Study of Liver Diseases (AASLD) 2018, European Association for the Study of the Liver (EASL) 2017, and WHO 2024 guidelines. This retrospective cross-sectional study included 12 217 treatment-naive patients with chronic HBV from 2015 to 2023 across 3 medical centers. Treatment criteria of international guidelines used in this study are showed in the eTable in Supplement 1. Significant liver fibrosis was identified by aspartate aminotransferase-to-platelet ratio index (≥1.5), fibrosis index based on 4 factors (≥3.25), liver stiffness measurement (≥8.0 kPa), or liver biopsy (liver histology ≥stage 2 fibrosis). This study was approved by the Institutional Review Board of Nanjing Drum Tower Hospital. Due to the study's retrospective design, informed consent was waived by the ethics committee of Nanjing Drum Tower Hospital. This study adhered to the ethical guidelines of the Declaration of Helsinki and followed the STROBE reporting guideline for cross-sectional studies. Due to insufficient clinical data, 2175 patients were excluded from the analysis. Among 10 042 patients included (median [IQR] age, 37.0 [30.0-48.0] years; 61.5% male), median (IQR) levels of alanine aminotransferase and HBV DNA were 32.1 (20.3-64.5) U/L and 3.2 (2.7-6.2) log 10 IU/mL, respectively, and 30.1% of patients were hepatitis B e antigen (HBeAg) positive (Table). A total of 2318 patients (23.1%), 2773 patients (27.6%), and 2841 patients (28.3%) were treatment eligible according to APASL 2015, AASLD 2018, and EASL 2017 guidelines, respectively, while 6338 patients (63.1%) met the treatment criteria of WHO 2024 guidelines, which was significantly higher than for the other guidelines ( P < .001) (Figure, A). More patients who were HBeAg positive than negative were treatment eligible in different guidelines, with rates as high as 88.7% for WHO 2024, followed by 65.1% for EASL 2017, 48.0% for AASLD 2018, and 46.9% for APASL 2015 guidelines ( P < .001) compared with proportions of 52.1%, 12.5%, 17.1%, and 12.9%, respectively, in patients who were HBeAg negative ( P < .001) (Figure, B). The proportion of treatment-eligible patients for WHO 2024 guidelines was 67.0% in males, which was significantly higher than that for EASL 2017 (31.1%), AASLD 2018 (29.8%), and APASL 2015 (26.4%) guidelines ( P < .001). A similar trend was observed in females (Figure, C). Older patients had a higher treatment-eligible rate (Figure, D). The numbers of patients who met option 1, 2, and 3 of WHO 2024 guidelines were 2831 patients (28.2%), 3996 patients (39.8%), and 2631 patients (26.2%), respectively (Figure, E). In this cross-sectional study, we found that the WHO 2024 guidelines had the highest treatment-eligible rate compared with other international guidelines. Although the treatment-eligible rate increased significantly according to the WHO 2024 guidelines, efforts to increase treatment uptake and linkage to care are equally important. A study limitation was that all patients were retrospectively included from liver centers or hospitals in China; therefore, further prospective studies involving racially and ethnically diverse populations and broader regions of the general HBV-infected population are needed to validate our findings. Abstract:About 10 million people in China are infected with hepatitis C virus (HCV), with the seroprevalence of anti-HCV in the general population estimated at 0.6%. Delaying effective treatment of chronic hepatitis C (CHC) is associated with liver disease progression, cirrhosis, hepatocellular carcinoma, and liver-related mortality. The extrahepatic manifestations of CHC further add to the disease burden of patients. Managing CHC-related advanced liver diseases and systemic manifestations are costly for both the healthcare system and society. Loss of work productivity due to reduced well-being and quality of life in CHC patients further compounds the economic burden of the disease. Traditionally, pegylated-interferon plus ribavirin (PR) was the standard of care. However, a substantial number of patients are ineligible for PR treatment, and only 40%-75% achieved sustained virologic response. Furthermore, PR is associated with impairment of patient-reported outcomes (PROs), high rates of adverse events, and poor adherence. With the advent of direct acting antivirals (DAAs), the treatment of CHC patients has been revolutionized. DAAs have broader eligible patient populations, higher efficacy, better PRO profiles, fewer adverse events, and better adherence rates, thereby making it possible to cure a large proportion of all CHC patients. This article aims to provide a comprehensive evaluation on the value of effective, curative hepatitis C treatment from the clinical, economic, societal, and patient experience perspectives, with a focus on recent data from China, supplemented with other Asian and international experiences where China data are not available.

Abstract:Infection with the hepatitis C virus (HCV) is a major cause of liver disease and hepatocellular carcinoma in China. Rapid economic development has had an enormous impact on the epidemiology and treatment of hepatitis C. The prevalence of anti-HCV antibodies in Mainland China is approximately 0.91%, and use of injected drugs has become the main route of HCV transmission in China. Reimbursement for 3 direct-acting antivirals (DAAs) has been approved by the National Medical Insurance scheme in China, which ensures the accessibility of treatment for an HCV infection. To improve the awareness of treatments for hepatitis C among medical personnel and the rate of in-hospital screening for HCV, the Chinese Medical Association has formulated guidelines for the diagnosis and treatment of hepatitis C and a process of in-hospital screening for hepatitis C in China. These efforts have standardized the screening, diagnosis, treatment, and management of hepatitis C. Based on the international strategy for micro-elimination of hepatitis C, China has also screened and treated groups at risk of hepatitis C infection, and this has reduced the number of the infected.
