Abstract:Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation technique that has been utilized for treating brain disorders and improving cognitive function. In order to achieve targeted tDCS, many optimization methods of montages and electric currents have been proposed. However, these methods have some limitations. Most of them were proposed for single-objective optimization (focality or intensity) and have no constrain with the number of electrodes (Most devices only have less than 8 electrodes currently). In this study, we proposed an operational optimization approach for well-targeted tDCS, which aims to optimize for two objectives of electric field (EF) intensity and focality with constraints on the number of electrodes. Compared with traditional tDCS in our cohort (10 subjects), our method significantly improves the EF focality. When compared to commonly used 4×1 high-definition tDCS (HD-tDCS), our method can achieve higher EF intensity in the target region with less than 8 electrodes. Our method can balance the two objectives of EF and shorten optimization time, which is convenient for practical application.

Abstract:Recently, multifocal transcranial current stimulation (tCS) devices using several relatively small electrodes have been used to achieve more focal stimulation of specific cortical targets. However, it is becoming increasingly recognized that many behavioral manifestations of neurological and psychiatric disease are not solely the result of abnormality in one isolated brain region but represent alterations in brain networks. In this paper we describe a method for optimizing the configuration of multifocal tCS for stimulation of brain networks, represented by spatially extended cortical targets. We show how, based on fMRI, PET, EEG or other data specifying a target map on the cortical surface for excitatory, inhibitory or neutral stimulation and a constraint on the maximal number of electrodes, a solution can be produced with the optimal currents and locations of the electrodes. The method described here relies on a fast calculation of multifocal tCS electric fields (including components normal and tangential to the cortical boundaries) using a five layer finite element model of a realistic head. Based on the hypothesis that the effects of current stimulation are to first order due to the interaction of electric fields with populations of elongated cortical neurons, it is argued that the optimization problem for tCS stimulation can be defined in terms of the component of the electric field normal to the cortical surface. Solutions are found using constrained least squares to optimize current intensities, while electrode number and their locations are selected using a genetic algorithm. For direct current tCS (tDCS) applications, we provide some examples of this technique using an available tCS system providing 8 small Ag/AgCl stimulation electrodes. We demonstrate the approach both for localized and spatially extended targets defined using rs-fcMRI and PET data, with clinical applications in stroke and depression. Finally, we extend these ideas to more general stimulation protocols, such as alternating current tCS (tACS).

Abstract:Transcranial electrical stimulation (TES) is a non-invasive neuromodulation technique with great potential. Electrode optimization methods based on simulation models of individual TES field could provide personalized stimulation parameters according to individual variations in head tissue structure, significantly enhancing the stimulation accuracy of TES. However, the existing electrode optimization methods suffer from prolonged computation times (typically exceeding 1 d) and limitations such as disregarding the restricted number of output channels from the stimulator, further impeding their clinical applicability. Hence, this paper proposes an efficient and practical electrode optimization method. The proposed method simultaneously optimizes both the intensity and focality of TES within the target brain area while constraining the number of electrodes used, and it achieves faster computational speed. Compared to commonly used electrode optimization methods, the proposed method significantly reduces computation time by 85.9% while maintaining optimization effectiveness. Moreover, our method considered the number of available channels for the stimulator to distribute the current across multiple electrodes, further improving the tolerability of TES. The electrode optimization method proposed in this paper has the characteristics of high efficiency and easy operation, potentially providing valuable supporting data and references for the implementation of individualized TES.

Abstract:Transcranial electric stimulation (TES) can modulate intrinsic neural activity in the brain by injecting weak currents through electrodes attached to the scalp. TES has been widely used as a neuroscience tool to investigate how behavioural and physiological variables of brain function are modulated by electric stimulation of specific brain regions. For an unambiguous interpretation of TES experiments, it is important that the electric fields can be steered towards one or several brain regions-of-interest. However, the conductive proprieties of the human head impose inherent physical limitations on how focal the electric fields in the brain produced by multi-electrode TES can be. As a rule of thumb, it is not feasible to selectively target deep brain areas with TES, although focusing the field in some specific deeper locations might be possible due to favourable conductive properties in the surrounding tissue. In the present study, we first propose a computationally efficient method for the automatic determination of electrode placements and stimulation intensities to optimally affect a given target position. We provide a robust implementation of the optimization procedure that is able to adhere to safety constraints, while explicitly controlling both the number of active electrodes and the angular deviation of the field in the target area relative to the desired field direction. Leveraging the high computational efficiency of our method, we systematically assess the achievable focality of multi-electrode TES for all cortex positions, thereby investigating the dependence on the chosen constraints. Our results provide comprehensive insight into the limitations regarding the achievable TES dose and focality that are imposed by the biophysical constraints and the safety considerations of TES.

Abstract:Transcranial electrical stimulation (tES) is a promising non-invasive brain modulation technique in clinical application. Optimization methods have been used to provide optimal stimulus patterns to improve focality and intensity at target based on accurate individual head model, but most of tES optimization approaches were implemented using the same small size electrode. There is a lack of relevant research that investigate the effect of electrode size and maximum current limitation on tES optimization. The purpose of this study is to evaluate the optimization performance using multi-size electrode and different maximum current limitations at each electrode. By quantitatively comparing distribution changes of electric field and optimal current, it was found that electrode size has no significant effect on stimulation optimization within certain range. More focal fields and fewer active electrodes were observed when the maximum per electrode current limitation increases. It suggests that tolerance and operability of tES could improve when the maximum current limitation for per electrode and electrode size increases synchronously.

Abstract:Transcranial direct current stimulation (tDCS) delivers weak current into the brain to modulate neural activities. Many methods have been proposed to determine electrode positions and stimulation intensities. Due to the trade-off between intensity and focality, it is actually a multi-objective optimization problem that has a set of optimal solutions. However, traditional methods can produce only one solution at each time, and many parameters need to be determined by experience. In this study, we proposed the nondominated sorting genetic algorithm II (NSGA-II) to solve the current optimization problem of multi-electrode tDCS. We also compared the representative solutions with LCMV solutions. The result shows that a group of solutions close to the optimal front can be obtained just in only one run without any prior knowledge.

Abstract:Transcranial electrical stimulation (TES) is a promising non-invasive neuromodulation technique. How to increase the current intensity entering the skull and reduce scalp shunting has become a key factor significantly influencing regulatory efficacy. In this study, we introduce a novel approach for optimizing TES by adjusting local scalp temperature to modulate scalp conductivity. We have developed simulation models for TES-induced electric fields and for temperature-induced alterations in scalp conductivity. Two common types of stimulation montage (M1-SO and 4×1 montage) were adopted for the evaluation of effectiveness. We observed that the modulation of scalp temperature has a significant impact on the distribution of the electric field within the brain during TES. As local scalp temperature decreases, there is an increase in the maximum electric field intensity within the target area, with the maximum change reaching 18.3%, when compared to the electric field distribution observed under normal scalp temperature conditions. Our study provide insights into the practical implementation challenges and future directions for this innovative methodology.

Abstract:Background: Transcranial electrical stimulation (tES) generates an electric field (or current density) in the brain through surface electrodes attached to the scalp. Clinical significance has been demonstrated, although with moderate and heterogeneous results partly due to a lack of control of the delivered electric currents. In the last decade, computational electric field analysis has allowed the estimation and optimization of the electric field using accurate anatomical head models. This review examines recent tES computational studies, providing a comprehensive background on the technical aspects of adopting computational electric field analysis as a standardized procedure in medical applications. Methods: Specific search strategies were designed to retrieve papers from the Web of Science database. The papers were initially screened based on the soundness of the title and abstract and then on their full contents, resulting in a total of 57 studies. Results: Recent trends were identified in individual- and population-level analysis of the electric field, including head models from non-neurotypical individuals. Advanced optimization techniques that allow a high degree of control with the required focality and direction of the electric field were also summarized. There is also growing evidence of a correlation between the computationally estimated electric field and the observed responses in real experiments. Conclusion: Computational pipelines and optimization algorithms have reached a degree of maturity that provides a rationale to improve tES experimental design and a posteriori analysis of the responses for supporting clinical studies.

Abstract:Electrode montage optimization for transcranial electric stimulation (tES) is a challenging topic for targeting a specific brain region. Targeting the deep brain region is difficult due to tissue inhomogeneity, resulting in complex current flow. In this study, a simplified protocol for montage optimization is proposed for multichannel tES (mc-tES). The purpose of this study was to reduce the computational cost for mc-tES optimization and to evaluate the mc-tES for deep brain regions. Optimization was performed using a simplified protocol for montages under safety constraints with 20 anatomical head models. The optimization procedure is simplified using the surface EF of the deep brain target region, considering its small volume and non-concentric distribution of the electrodes. Our proposal demonstrated that the computational cost was reduced by >90%. A total of six–ten electrodes were necessary for robust EF in the target region. The optimization with surface EF is comparable to or marginally better than using conventional volumetric EF for deep brain tissues. An electrode montage with a mean injection current amplitude derived from individual analysis was demonstrated to be useful for targeting the deep region at the group level. The optimized montage and injection current were derived at the group level. Our proposal at individual and group levels showed great potential for clinical application.

Abstract:BACKGROUND: Transcranial magnetic stimulation (TMS) is widely used in brain research and treatment of various brain dysfunctions. However, the optimal way to target stimulation and administer TMS therapies, for example, where and in which electric field direction the stimuli should be given, is yet to be determined.OBJECTIVE: To develop an automated closed-loop system for adjusting TMS parameters (in this work, the stimulus orientation) online based on TMS-evoked brain activity measured with electroencephalography (EEG).METHODS: We developed an automated closed-loop TMS-EEG set-up. In this set-up, the stimulus parameters are electronically adjusted with multi-locus TMS. As a proof of concept, we developed an algorithm that automatically optimizes the stimulation orientation based on single-trial EEG responses. We applied the algorithm to determine the electric field orientation that maximizes the amplitude of the TMS-EEG responses. The validation of the algorithm was performed with six healthy volunteers, repeating the search twenty times for each subject.RESULTS: The validation demonstrated that the closed-loop control worked as desired despite the large variation in the single-trial EEG responses. We were often able to get close to the orientation that maximizes the EEG amplitude with only a few tens of pulses.CONCLUSION: Optimizing stimulation with EEG feedback in a closed-loop manner is feasible and enables effective coupling to brain activity.

A computational study on the optimization of transcranial temporal interfering stimulation with high-definition electrodes using unsupervised neural networks

Abstract:Transcranial temporal interfering stimulation (tTIS) can focally stimulate deep parts of the brain related to specific functions using beats at two high frequencies that do not individually affect the human brain. However, the complexity and nonlinearity of the simulation limit it in terms of calculation time and optimization precision. We propose a method to quickly optimize the interfering current value of high-definition electrodes, which can finely stimulate the deep part of the brain, using an unsupervised neural network (USNN) for tTIS. We linked a network that generates the values of electrode currents to another network, which is constructed to compute the interference exposure, for optimization by comparing the generated stimulus with the target stimulus. Further, a computational study was conducted using 16 realistic head models. We also compared tTIS with transcranial alternating current stimulation (tACS), in terms of performance and characteristics. The proposed method generated the strongest stimulation at the target, even when targeting deep areas or performing multi-target stimulation. The high-definition tTISl was less affected than tACS by target depth, and mis-stimulation was reduced compared with the case of using two-pair inferential stimulation in deep region. The optimization of the electrode currents for the target stimulus could be performed in 3 min. Using the proposed USNN for tTIS, we demonstrated that the electrode currents of tTIS can be optimized quickly and accurately. Moreover, we confirmed the possibility of precisely stimulating the deep parts of the brain via transcranial electrical stimulation.
DeepFocus: A Transnasal Approach for Optimized Deep Brain Stimulation of Reward Circuit Nodes

Abstract:Objective: Transcranial electrical stimulation (TES) is an effective technique to modulate brain activity and treat diseases. However, TES is primarily used to stimulate superficial brain regions and is unable to reach deeper targets. The spread of injected currents in the head is affected by volume conduction and the additional spreading of currents as they move through head layers with different conductivities, as is discussed in [1]. In this paper, we introduce DeepFocus, a technique aimed at stimulating deep brain structures in the brain's "reward circuit" (e.g. the orbitofrontal cortex, Brodmann area 25, amygdala, etc.). Approach: To accomplish this, DeepFocus utilizes transnasal electrode placement (under the cribriform plate and within the sphenoid sinus) in addition to electrodes placed on the scalp, and optimizes current injection patterns across these electrodes. To quantify the benefit of DeepFocus, we develop the DeepROAST simulation and optimization platform. DeepROAST simulates the effect of complex skull-base bones' geometries on the electric fields generated by DeepFocus configurations using realistic head models. It also uses optimization methods to search for focal and efficient current injection patterns, which we use in our simulation and cadaver studies. Main Results. In simulations, optimized DeepFocus patterns created larger and more focal fields in several regions of interest than scalp-only electrodes. In cadaver studies, DeepFocus patterns created large fields at the medial orbitofrontal cortex (OFC) with magnitudes comparable to stimulation studies, and, in conjunction with established cortical stimulation thresholds, suggest that the field intensity is sufficient to create neural response, e.g. at the OFC. Significance. This minimally invasive stimulation technique can enable more efficient and less risky targeting of deep brain structures to treat multiple neural conditions.

Abstract:Neuromodulation is based on the principle that brain stimulation produces plastic changes in cerebral circuitry. Given the intersubject structural and functional variability, neuromodulation has a personalized effect in the brain. Moreover, because of cerebral dominance and interhemispheric functional and structural differences in the same individual, the characterization of specific brain circuitries involved is currently not feasible. This notion is extremely important for neuromodulation treatments applied in neuropsychiatry. Specifically, the efficacy of the neuromodulation treatments is critically dependent on the anatomical precision of the brain target and the circuitry which has been affected. However, a complete understanding of how the brain behaves under stimulation needs a combined characterization of its neurophysiological response. This can be achieved by TMS-EEG guided by current multimodal neuroimaging techniques in real time. Herein, we present novel data based on dMRI tractography-guided TMS-EEG on one healthy young adult volunteer.

Abstract:The ability to stimulate deep brain regions in a focal manner brings new opportunities for treating brain disorders. Temporal interference (TI) stimulation has been suggested as a method to achieve focused stimulation in deep brain targets. Individual-level knowledge of the interferential currents has permitted personalizing TI montage via subject-specific digital human head models, facilitating the estimation of interferential electric currents in the brain. While this individual approach offers a high degree of personalization, the significant intra-and inter-individual variability among specific head models poses challenges when comparing electric-field doses. Furthermore, MRI acquisition to develop a personalized head model, followed by precise methods for placing the optimized electrode positions, is complex and not always available in various clinical settings. Instead, the registration of individual electric fields into brain templates has offered insights into population-level effects and enabled montage optimization using common scalp landmarks. However, population-level knowledge of the interferential currents remains scarce. This work aimed to investigate the effectiveness of targeting deep brain areas using TI in different populations. The results showed a trade-off between deep stimulation and unwanted cortical neuromodulation, which is target-dependent at the group level. A consistent modulated electric field appeared in the deep brain target when the same montage was applied in different populations. However, the performance in terms of focality and variability varied when the same montage was used among populations. Also, group-level TI exhibited greater focality than tACS, reducing unwanted neuromodulation volume in the cortical part by at least 1.5 times, albeit with higher variability. These results provide valuable population-level insights when considering TI montage selection.

Abstract:Introduction: This study focuses on broadening the applicability of the metaheuristic L1-norm fitted and penalized (L1L1) optimization method in finding a current pattern for multichannel transcranial electrical stimulation (tES). The metaheuristic L1L1 optimization framework defines the tES montage via linear programming by maximizing or minimizing an objective function with respect to a pair of hyperparameters. Methods: In this study, we explore the computational performance and reliability of different optimization packages, algorithms, and search methods in combination with the L1L1 method. The solvers from Matlab R2020b, MOSEK 9.0, Gurobi Optimizer, CVX's SeDuMi 1.3.5, and SDPT3 4.0 were employed to produce feasible results through different linear programming techniques, including Interior-Point (IP), Primal-Simplex (PS), and Dual-Simplex (DS) methods. To solve the metaheuristic optimization task of L1L1, we implement an exhaustive and recursive search along with a well-known heuristic direct search as a reference algorithm. Results: Based on our results, and the given optimization task, Gurobi's IP was, overall, the preferable choice among Interior-Point while MOSEK's PS and DS packages were in the case of Simplex methods. These methods provided substantial computational time efficiency for solving the L1L1 method regardless of the applied search method. Discussion: While the best-performing solvers show that the L1L1 method is suitable for maximizing either focality and intensity, a few of these solvers could not find a bipolar configuration. Part of the discrepancies between these methods can be explained by a different sensitivity with respect to parameter variation or the resolution of the lattice provided.

Abstract:Transcranial Direct Current Stimulation (tDCS) has benefits for motor rehabilitation in stroke patients, but its clinical application is limited due to inter-individual heterogeneous effects. Recently, optimized tDCS that considers individual brain structure has been proposed, but the utility thereof has not been studied in detail. We explored whether optimized tDCS provides unique electrode positions for each patient and creates a higher target electric field than the conventional approach. A comparative within-subject simulation study was conducted using data collected for a randomized controlled study evaluating the effect of optimized tDCS on upper extremity function in stroke patients. Using Neurophet tES LAB 3.0 software, individual brain models were created based on magnetic resonance images and tDCS simulations were performed for each of the conventional and optimized configurations. A comparison of electrode positions between conventional tDCS and optimized tDCS was quantified by calculation of Euclidean distances. A total of 21 stroke patients were studied. Optimized tDCS produced a higher electric field in the hand motor region than conventional tDCS, with an average improvement of 20% and a maximum of 52%. The electrode montage for optimized tDCS was unique to each patient and exhibited various configurations that differed from electrode placement of conventional tDCS. Optimized tDCS afforded a higher electric field in the target of a stroke patient compared to conventional tDCS, which was made possible by appropriately positioning the electrodes. Our findings may encourage further trials on optimized tDCS for motor rehabilitation after stroke.

The Pursuit of DLPFC: Non-neuronavigated Methods to Target the Left Dorsolateral Pre-frontal Cortex With Symmetric Bicephalic Transcranial Direct Current Stimulation (tDCS)

Abstract:Background: The dose of transcranial direct current stimulation (tDCS) is defined by electrode montage and current, while the resulting brain current flow is more complex and varies across individuals. The left dorsolateral pre-frontal cortex (lDLPFC) is a common target in neuropsychology and neuropsychiatry applications, with varied approaches used to experimentally position electrodes on subjects. Objective: To predict brain current flow intensity and distribution using conventional symmetrical bicephalic frontal 1 × 1 electrode montages to nominally target lDLPFC in forward modeling studies. Methods: Six high-resolution Finite Element Method (FEM) models were created from five subjects of varied head size and an MNI standard. Seven electrode positioning methods, nominally targeting lDLPFC, were investigated on each head model: the EEG 10-10 including F3-F4, F5-F6, F7-8, F9-F10, the Beam F3-System, the 5-5 cm-Rule and the developed OLE-System were evaluated as electrode positioning methods for 5 × 5 cm(2) rectangular sponge-pad electrodes. Results: Each positioning approach resulted in distinct electrode positions on the scalp and variations in brain current flow. Variability was significant, but trends across montages and between subjects were identified. Factors enhancing electric field intensity and relative targeting in lDLPFC include increased inter-electrode distance and proximity to thinner skull structures. Conclusion: Brain current flow can be shaped, but not focused, across frontal cortex by tDCS montages, including intensity at lDLPFC. The OLE-system balances lDLPFC targeting and reduced electric field variability, along with clinical ease-of-use.
