Effects of Spaceflight on Musculoskeletal Health: A Systematic Review and Meta-analysis, Considerations for Interplanetary Travel

Abstract:Background: If interplanetary travel is to be successful over the coming decades, it is essential that countermeasures to minimize deterioration of the musculoskeletal system are as effective as possible, given the increased duration of spaceflight associated with such missions. The aim of this review, therefore, is to determine the magnitude of deconditioning of the musculoskeletal system during prolonged spaceflight and recommend possible methods to enhance the existing countermeasures. Methods: A literature search was conducted using PubMed, Ovid and Scopus databases. 5541 studies were identified prior to the removal of duplicates and the application of the following inclusion criteria: (1) group means and standard deviations for pre- and post-spaceflight for measures of strength, muscle mass or bone density were reported (or provided by the corresponding author when requested via e-mail), (2) exercise-based countermeasures were included, (3) the population of the studies were human, (4) muscle function was assessed and (5) spaceflight rather than simulated spaceflight was used. The methodological quality of the included studies was evaluated using a modified Physiotherapy Evidence Database (PEDro) scale for quality, with publication bias assessed using a failsafe N (Rosenthal method), and consistency of studies analysed using I2 as a test of heterogeneity. Secondary analysis of studies included Hedges' g effect sizes, and between-study differences were estimated using a random-effects model. Results: A total of 11 studies were included in the meta-analyses. Heterogeneity of the completed meta-analyses was conducted revealing homogeneity for bone mineral density (BMD) and spinal muscle size (Tau2 < 0.001; I2 = 0.00%, p > 0.05), although a high level of heterogeneity was noted for lower body force production (Tau2 = 1.546; I2 = 76.03%, p < 0.001) and lower body muscle mass (Tau2 = 1.386; I2 = 74.38%, p < 0.001). The estimated variance (≤ -0.306) for each of the meta-analyses was significant (p ≤ 0.033), for BMD (- 0.48 to - 0.53, p < 0.001), lower body force production (- 1.75, p < 0.001) and lower body muscle size (- 1.98, p < 0.001). Spaceflight results in small reductions in BMD of the femur (Hedges g = - 0.49 [- 0.69 to - 0.28]), trochanter (Hedges g = - 0.53 [- 0.77 to - 0.29]), and lumbo-pelvic region (Hedges g = - 0.48 [- 0.73 to - 0.23]), but large decreases in lower limb force production (Hedges g = - 1.75 [- 2.50 to - 0.99]) and lower limb muscle size (Hedges g = - 1.98 [- 2.72 to - 1.23]). Conclusions: Current exercise countermeasures result in small reductions in BMD during long-duration spaceflight. In contrast, such exercise protocols do not alleviate the reductions in muscle function or muscle size, which may be attributable to the low to moderate loads reported by crewmembers and the interference effect associated with concurrent training. It is recommended that higher-load resistance exercise and the use of high-intensity interval training should be investigated, to determine if such modifications to the reported training practices result in more effective countermeasures to the deleterious effect of long-duration spaceflight on the muscular system.

Abstract:Human spaceflight is associated with a substantial loss of skeletal muscle mass and muscle strength. Neuromuscular electrical stimulation (NMES) evokes involuntary muscle contractions, which have the potential to preserve or restore skeletal muscle mass and neuromuscular function during and/or post spaceflight. This assumption is largely based on evidence from terrestrial disuse/immobilization studies without the use of large exercise equipment that may not be available in spaceflight beyond the International Space Station. In this mini-review we provide an overview of the rationale and evidence for NMES based on the terrestrial state-of-the-art knowledge, compare this to that used in orbit, and in ground-based analogs in order to provide practical recommendations for implementation of NMES in future space missions. Emphasis will be placed on knee extensor and plantar flexor muscles known to be particularly susceptible to deconditioning in space missions.

Abstract:Findings from a recent 70 day bedrest investigation suggested intermittent exercise testing in the control group may have served as a partial countermeasure for skeletal muscle size, function, and fiber-type shifts. The purpose of the current study was to investigate the metabolic and skeletal muscle molecular responses to the testing protocols. Eight males (29±2y) completed muscle power (6x4 sec; peak muscle power: 1369±86W) and VO 2 max (13±1min; 3.2±0.2L/min) tests on specially designed supine cycle ergometers during two separate trials. Blood catecholamines and lactate were measured pre, immediately post, and 4h postexercise. Muscle homogenate and muscle fiber-type specific (myosin heavy chain (MHC) I and MHC IIa) mRNA levels of exercise markers ( myostatin, IκBα, myogenin, MuRF-1, ABRA, RRAD, Fn14, PDK4) and MHC I, IIa, and IIx were measured from vastus lateralis muscle biopsies obtained pre and 4h postexercise. The muscle power test altered (p≤0.05) norepinephrine (+124%), epinephrine (+145%), lactate (+300%), and muscle homogenate mRNA ( IκBα, myogenin, MuRF-1, RRAD, Fn14). The VO 2 max test altered (p≤0.05) norepinephrine (+1394%), epinephrine (+1412%), lactate (+736%), and muscle homogenate mRNA ( myostatin, IκBα, myogenin, MuRF-1, ABRA, RRAD, Fn14, PDK4). In general, both tests influenced MHC IIa muscle fibers more than MHC I with respect to the number of genes that responded and the magnitude of response. Both tests also influenced MHC mRNA expression in a muscle fiber-type specific manner. These findings provide unique insights into the adaptive response of skeletal muscle to small doses of exercise and could help shape exercise countermeasures for astronauts and Earth-based populations (e.g., aging individuals).

Abstract:Human spaceflight has been fascinating man for centuries, representing the intangible need to explore the unknown, challenge new frontiers, advance technology and push scientific boundaries further. A key area of importance is cardiovascular deconditioning, that is, the collection of hemodynamic changes - from blood volume shift and reduction to altered cardiac function - induced by sustained presence in microgravity. A thorough grasp of the 0G adjustment point per se is important from a physiological viewpoint and fundamental for astronauts' safety and physical capability on long spaceflights. However, hemodynamic details of cardiovascular deconditioning are incomplete, inconsistent and poorly measured to date; thus a computational approach can be quite valuable. We present a validated 1D-0D multiscale model to study the cardiovascular response to long-term 0G spaceflight in comparison to the 1G supine reference condition. Cardiac work, oxygen consumption and contractility indexes, as well as central mean and pulse pressures were reduced, augmenting the cardiac deconditioning scenario. Exercise tolerance of a spaceflight traveler was found to be comparable to an untrained person with a sedentary lifestyle. At the capillary-venous level significant waveform alterations were observed which can modify the regular perfusion and average nutrient supply at the cellular level. The present study suggests special attention should be paid to future long spaceflights which demand prompt physical capacity at the time of restoration of partial gravity (e.g., Moon/Mars landing). Since spaceflight deconditioning has features similar to accelerated aging understanding deconditioning mechanisms in microgravity are also relevant to the understanding of aging physiology on Earth.

Abstract:Muscle function is compromised by gravitational unloading in space affecting overall musculoskeletal health. Astronauts perform daily exercise programmes to mitigate these effects but knowing which muscles to target would optimise effectiveness. Accurate inflight assessment to inform exercise programmes is critical due to lack of technologies suitable for spaceflight. Changes in mechanical properties indicate muscle health status and can be measured rapidly and non-invasively using novel technology. A hand-held MyotonPRO device enabled monitoring of muscle health for the first time in spaceflight (> 180 days). Greater/maintained stiffness indicated countermeasures were effective. Tissue stiffness was preserved in the majority of muscles (neck, shoulder, back, thigh) but Tibialis Anterior (foot lever muscle) stiffness decreased inflight vs. preflight (p < 0.0001; mean difference 149 N/m) in all 12 crewmembers. The calf muscles showed opposing effects, Gastrocnemius increasing in stiffness Soleus decreasing. Selective stiffness decrements indicate lack of preservation despite daily inflight countermeasures. This calls for more targeted exercises for lower leg muscles with vital roles as ankle joint stabilizers and in gait. Muscle stiffness is a digital biomarker for risk monitoring during future planetary explorations (Moon, Mars), for healthcare management in challenging environments or clinical disorders in people on Earth, to enable effective tailored exercise programmes.

Abstract:A systematic review of literature was conducted to evaluate the effectiveness of passive countermeasures in ameliorating the cardiopulmonary and musculoskeletal effects of gravitational unloading on humans during spaceflight. This systematic review is the third of a series being conducted by the European Space Agency to evaluate the effectiveness of countermeasures to physiologic deconditioning during spaceflight. With future long-duration space missions on the horizon, it is critical to understand the effectiveness of existing countermeasures to promote astronaut health and improve the probability of future mission success. An updated search for studies examining passive countermeasures was conducted in 2021 to supplement results from a broader search conducted in 2017 for all countermeasures. Ground-based analogue and spaceflight studies were included in the search. A total of 647 articles were screened following removal of duplicates, of which 16 were included in this review. Data extraction and analysis, quality assessment of studies, and transferability of reviewed studies to actual spaceflight based on their bed-rest protocol were conducted using dedicated tools created by the Aerospace Medicine Systematic Review Group. Of the 180 examined outcomes across the reviewed studies, only 20 were shown to have a significant positive effect in favour of the intervention group. Lower body negative pressure was seen to significantly maintain orthostatic tolerance (OT) closer to baseline as comparted to control groups. It also was seen to have mixed efficacy with regards to maintaining resting heart rate close to pre-bed rest values. Whole body vibration significantly maintained many balance-related outcome measures close to pre-bed rest values as compared to control. Skin surface cooling and centrifugation both showed efficacy in maintaining OT. Centrifugation also was seen to have mixed efficacy with regards to maintaining VO2max close to pre-bed rest values. Overall, standalone passive countermeasures showed no significant effect in maintaining 159 unique outcome measures close to their pre-bed rest values as compared to control groups. Risk of bias was rated high or unclear in all studies due to poorly detailed methodologies, poor control of confounding variables, and other sources of bias (i.e. inequitable recruitment of participants leading to a higher male:female ratios). The bed-rest transferability (BR) score varied from 2–7, with a median score of 5. Generally, most studies had good BR transferability but underreported on factors such as control of sunlight or radiation exposure, diet, level of exercise and sleep-cycles. We conclude that: (1) Lack of standardisation of outcome measurement and methodologies has led to large heterogeneity amongst studies; (2) Scarcity of literature and high risk of bias amongst existing studies limits the statistical power of results; and (3) Passive countermeasures have little or no efficacy as standalone measures against cardiopulmonary and musculoskeletal deconditioning induced by spaceflight related to physiologic deterioration due to gravity un-loading.

Abstract:According to the existing scenarios of interplanetary missions, the Moon is considered as an intermediate base on the way to deep space. In order to prepare for landing and work on the Moon, it is important to study the applicability of countermeasures in such missions. The paper presents the results of a pilot study performed during a short-term spaceflight (12 days). A new experience of using countermeasure impacts of lower body negative pressure (LBNP) at the early stages of adaptation to microgravity conditions has been gained. To assess the effectiveness of LBNP and changes in human physical performance after the spaceflight, we conducted tests on a treadmill, a bicycle ergometer, and testing with model tasks of on-planetary activity "express test". Regulatory mechanisms of the cardiovascular system proved to be quite effective when creating decompression up to -20 mm Hg, which is less than in preparation for returning to Earth. In the treadmill test, a lower speed was achieved after the spaceflight than before (13 km/h and 15 km/h, respectively) and cardiovascular system response to the change in load was slower. At the same time changes in such physiological parameters as oxygen consumption, respiratory rate and pulmonary ventilation were minimal. In the bicycle ergometer test, peak heart rate values were higher after the spaceflight than before, the physiological value of the standard exercise increased. «Express test» showed the positive dynamics from the first day to the third after returning to Earth: the performance of a dual task, the task of controlling the movement of the non-leading hand, and getting up from the supine position improved. Thus, assessment of the state of gravity-dependent physiological systems after short-term flight indicates the decrease of functional reserves of the organism and the necessity to develop appropriate countermeasures. The study was one of the first steps in the development of new medical control operations at the stage of paradigm shift from orbital flights to deep space exploration.

Dynamics of physical performance during long-duration space flight (first results of "Countermeasure" experiment)

Abstract:The efficacy of countermeasure exercise for diminishing disturbances induced by microgravity in motor system and its visceral supply during different stages of long-duration flight was evaluated. The results of both bicycle and locomotor testing indicate that physical fitness of cosmonaut does not become worse in the course of the long-duration flight. On the contrary, the lowest fitness was recorded at the first stage of mission, just after one month of flight. The "dead period" at the beginning of space flight seems to be a manifestation of the acute decrease in physical condition on transition from 1 G to microgravity, when none of the regular countermeasure regimes is sufficiently effective and acute increase of volume and intensity of training is impossible under the conditions of space flight.
NASA SPRINT exercise program efficacy for vastus lateralis and soleus skeletal muscle health during 70 days of simulated microgravity

Abstract:The efficacy of the NASA SPRINT exercise countermeasures program for quadriceps (vastus lateralis) and triceps surae (soleus) skeletal muscle health was investigated during 70 days of simulated microgravity. Individuals completed 6° head-down-tilt bedrest (BR, n=9), bedrest with resistance and aerobic exercise (BRE, n=9), or bedrest with resistance and aerobic exercise and low-dose testosterone (BRE+T, n=8). All groups were periodically tested for muscle (n=9 times) and aerobic (n=4 times) power during bedrest. In BR, surprisingly, the typical bedrest-induced decrements in vastus lateralis myofiber size and power were either blunted (MHC I) or eliminated (MHC IIa), along with no change (P>0.05) in %MHC distribution and blunted quadriceps atrophy. In BRE, MHC I (vastus lateralis and soleus) and IIa (vastus lateralis) contractile performance was maintained (P>0.05) or increased (P<0.05). Vastus lateralis hybrid fiber percentage was reduced (P 0.05), while not all of these positive responses were observed in the soleus. Exercise offset 100% of quadriceps and ~ 2⁄3 of soleus whole muscle mass loss. Testosterone (BRE+T) did not provide any benefit over exercise alone for either muscle, and for some myocellular parameters appeared detrimental. In summary, the periodic testing likely provided a partial exercise countermeasure for the quadriceps in the bedrest group, which is a novel finding given the extremely low exercise dose. The SPRINT exercise program appears to be viable for the quadriceps; however, refinement is needed to completely protect triceps surae myocellular and whole muscle health for astronauts on long-duration spaceflights.

Abstract:Muscle atrophy occurs with extended exposure to microgravity. This study quantified the overall muscle size, lean muscle area and fat infiltration changes pre- to post-flight that occur in the thoracic and lumbar spine with long-duration spaceflight. Pre- and post-flight magnetic resonance imaging (MRI) scans were obtained from 9 crewmembers on long-duration (≥6 months) International Space Station (ISS) missions. Muscle size was measured by the cross-sectional area (CSA) and lean muscle tissue by the functional cross-sectional area (FCSA). Muscle-fat infiltration (MFI) was measured by the mean pixel intensities of the MRI in fat and water phases. A mixed model with random subject effect was used to analyze pre- to post-flight changes. Significant decreases were seen in the quadratus lumborum muscle size (-1.8 ± 0.6% per month, p = 0.002) and lean muscle tissue content in the paraspinal muscles (-0.7 ± 0.2% per month, p ≤ 0.001). Fat infiltration increased significantly in the transversospinalis (+4.1 ± 1.0% per month, p ≤ 0.01) muscle. Treadmill exercise had a tendency to reduce fat content in the paraspinal and quadratus lumborum muscles, while counteracting muscle build-up only in the paraspinal muscles. Cycle ergometer exercise suggested benefits for the psoas muscle. Resistance training appeared to benefit lean muscle mass of most thoracolumbar muscles. Our findings highlight the need for countermeasures to prevent muscle atrophy and detrimental effects in muscle composition during long-duration spaceflight.

Abstract:Space colonization represents the most insidious challenge for mankind, as numerous obstacles affect the success of space missions. Specifically, the absence of gravitational forces leads to systemic physiological alterations, with particular emphasis on the musculoskeletal system. Indeed, astronauts exposed to spaceflight are known to report a significant impairment of bone microarchitecture and muscle mass, conditions clinically defined as osteoporosis and sarcopenia. In this context, space medicine assumes a crucial position, as the development of strategies to prevent and/or counteract weightlessness-induced alterations appears to be necessary. Furthermore, the opportunity to study the biological effects induced by weightlessness could provide valuable information regarding adaptations to spaceflight and suggest potential treatments that can preserve musculoskeletal health under microgravity conditions. Noteworthy, improving knowledge about the latest scientific findings in this field of research is crucial, as is thoroughly investigating the mechanisms underlying biological adaptations to microgravity and searching for innovative solutions to counter spaceflight-induced damage. Therefore, this narrative study review, performed using the MEDLINE and Google Scholar databases, aims to summarize the most recent evidence regarding the effects of real and simulated microgravity on the musculoskeletal system and to discuss the effectiveness of the main defence strategies used in both real and experimental settings.

Abstract:Astronauts are exposed to microgravity-induced health problems in spaceflight missions. Countermeasures and physical exercises have received increasing attention and its current research trends and landscapes warranted investigation. We conducted a comprehensive bibliometric analysis on astronaut training/countermeasures using the available data from the Web of Science Core Collection database from 1992 to 2022 to summarize the research trends and identify future directions. A total of 1,520 relevant articles were identified. Annual publications of the field have been increased over the years with the emergence of new and effective countermeasures. 'Microgravity' was the centered hotspot surrounded by the topics included 'spaceflight', 'hind leg hanging', 'simulated microgravity', and 'simulated weightlessness'. The top countries that produced the most publications included United States (726 articles), Germany (129 articles), and France (84 articles). The United States played a dominant role in the collaboration network with other countries. Meanwhile, NASA from the United States led the global collaborations and dominated the literature. Future research trend might lie on the design of physical training exercises to tackle the potential health problems on osteoporosis, muscle atrophy, and abnormality on the nervous and cardiovascular system; and artificial/simulated gravity with interdisciplinary sports countermeasure research on physiology, brain science, biomechanics, and aerospace medicine.

Acute cardiovascular and muscular response to rowing ergometer exercise in artificial gravity – a pilot trial

Abstract:Prolonged immobilization and spaceflight cause cardiovascular and musculoskeletal deconditioning. Combining artificial gravity through short-arm centrifugation with rowing exercise may serve as a countermeasure. We aimed to compare the tolerability, muscle force production, cardiovascular response, and power output of rowing on a short-arm centrifuge and under terrestrial gravity. Twelve rowing athletes (4 women, aged 27.2 ± 7.4 years, height 179 ± 0.1 cm, mass 73.7 ± 9.4 kg) participated in two rowing sessions, spaced at least six weeks apart. One session used a short-arm centrifuge with +0.5 Gz, while the other inclined the rowing ergometer by 26.6° to mimic centrifugal loading. Participants started self-paced rowing at 30 W, increasing by 15 W every three minutes until exhaustion. We measured rowing performance, heart rate, blood pressure, ground reaction forces, leg muscle activation, and blood lactate concentration. Rowing on the centrifuge was well-tolerated without adverse events. No significant differences in heart rate, blood pressure, or blood lactate concentration were observed between conditions. Inclined rowing under artificial gravity resulted in lower power output (−33%, p < 0.001) compared to natural gravity, but produced higher mean and peak ground reaction forces ( p < 0.0001) and increased leg muscle activation. Muscle activation and ground reaction forces varied with rotational direction. Rowing in artificial gravity shows promise as a strategy against cardiovascular and muscular deconditioning during long-term spaceflight, but further investigation is required to understand its long-term effects.

Abstract:Thoracoscopic enucleation of benign distal esophageal leiomyoma is a minimally-invasive procedure that is comprised of four basic steps: (1) port placement and exposure, (2) dissection, (3) reconstruction, and (4) drain placement and closure. The procedure can be performed with minimal perioperative pain, excellent morbidity and mortality, and a high-degree of patient satisfaction. Some patients may experience mild dysphagia or dyspepsia in the postoperative interval, which is managed medically with proton pump inhibitors or surgically with antireflux repair.

Abstract:With National Aeronautics and Space Administration's plans for longer distance, longer duration spaceflights such as missions to Mars and the surge in popularity of space tourism, the need to better understand the effects of spaceflight on the musculoskeletal system has never been more present. However, there is a paucity of information on how spaceflight affects orthopaedic health. This review surveys existing literature and discusses the effect of spaceflight on each aspect of the musculoskeletal system. Spaceflight reduces bone mineral density at rapid rates because of multiple mechanisms. While this seems to be recoverable upon re-exposure to gravity, concern for fracture in spaceflight remains as microgravity impairs bone strength and fracture healing. Muscles, tendons, and entheses similarly undergo microgravity adaptation. These changes result in decreased muscle mass, increased tendon laxity, and decreased enthesis stiffness, thus decreasing the strength of the muscle-tendon-enthesis unit with variable recovery upon gravity re-exposure. Spaceflight also affects joint health; unloading of the joints facilitates changes that thin and atrophy cartilage similar to arthritic phenotypes. These changes are likely recoverable upon return to gravity with exercise. Multiple questions remain regarding effects of longer duration flights on health and implications of these findings on terrestrial medicine, which should be the target of future research.

Abstract:We report the first case of ciliated gastric metaplasia found in a Caucasian American patient. Cilia were found in cystically dilated glands underneath a gastric adenoma. The rarity of ciliated metaplasia among Caucasians and its high frequency among the Japanese suggests a difference in the gastric mucosa in the two populations.
