Abstract:Abstract The concept of gastric-type mucinous carcinoma of the uterine cervix (GAS) has been accepted worldwide because of its aggressive clinical behaviour and the absence of high-risk human papilloma virus infection. The World Health Organization (WHO) 2020 classification divides cervical tumours into two categories: human papilloma virus-associated and human papilloma virus-independent. Hence, GAS is now classified as an human papilloma virus-independent gastric type. Because clinical studies have reported that GAS is refractory to conventional treatments such as chemotherapy and radiotherapy, especially at an advanced stage, and has aggressive features with widespread dissemination to unusual sites, such as the omentum, peritoneum and distant organs, it is urgent to establish new treatment strategies by comparing the molecular profiles of human papilloma virus-associated adenocarcinomas. A series of genetic mutations characteristic to GAS encourage the development of future treatment strategies such as targeted therapy and immunotherapy.

Abstract:Background We aimed to analyze the clinicopathological features and outcomes of patients with gastric-type of HPV-independent endocervical adenocarcinoma (GAS HPVI ECA), and compare them with non-GAS HPVI ECA cases. Methods Thirty-eight GASs [including 17 minimal deviation adenocarcinoma (MDA), 21 non-MDA GAS] and 17 non-GAS HPVI ECAs were studied. Data of clinical features, pathological characteristics, treatment, and outcomes were evaluated. Results The median age of patients with GAS and non-GAS HPVI ECA was 46 and 48 years, respectively ( p = 0.93). Compared with non-GAS HPVI ECAs, GAS had more common complains of vaginal watery discharge ( p = 0.04). GAS cases were also associated with higher clinical stage ( p = 0.036), more common in deeper cervical stromal invasion ( p = 0.002) and lymphoavascular invasion ( p = 0.044). GAS was associated with worse median progression-free survival (PFS) ( p = 0.02) and median overall survival (OS) ( p = 0.03) over patients with non-GAS HPVI ECAs. MDA had similar clinical and pathological features and prognosis compared with non-MDA GAS. Of note, serum CA19–9 levels were significantly higher in GAS than that in non-GAS HPVI ECA cases. Conclusions GAS cases were more likely to have high risk pathological factors and poorer PFS and OS compared with non-GAS HPVI ECAs. Serum CA19–9 may be helpful for diagnosis and screening in patients with GAS.

Abstract:Gastric-type mucinous endocervical adenocarcinomas (GAS) are new variant types of cervical adenocarcinomas according to the 2014 World Health Organization (WHO) classification. GAS is a unique disease that can be differentiated from typical adenocarcinomas-it is less common and more aggressive and likely to have deep invasion and horizontal diffusion, invasion of the uterus and vagina, early distant metastases, and a lower 5-year survival rate compared to the usual-type cervical cancer. At present, initial treatment and postoperative adjuvant therapy are not conclusive, but early detection and early treatment are a consensus that can improve prognosis. Most of its occurrence has nothing to do with human papillomavirus (HPV) infection. Whether it is only negative for the subtypes that can be detected at present and whether it may be an unknown subtype of infection need to be further explored in the future. The clinical symptoms commonly include aqueous secretion, lower abdominal pain, and elevated serum carbohydrate antigen-19-9 (CA19-9) levels, which may be helpful for diagnosis. MRI and PET-CT can help to describe the characteristics of lesions and judge the state of the systemic metastasis. We believe that early detection and surgical treatment will give patients more benefits. Looking for potential gene and molecular changes and establishing biomarkers to identify molecular targets will be the key to early identification and target therapy.

Abstract:Background: We aim to analyze the clinicopathological features and outcomes among different histologic subtypes of HPV-independent endocervical adenocarcinomas（HPVI ECAs). Methods: Forty-five HPVI ECAs, including 16 minimal deviation adenocarcinoma (MDA), 17 non-MDA type of gastric adenocarcinoma (GAS), and 12 non-GAS HPVI ECAs were studied. Data of clinical features, pathological characteristics, treatment, and outcomes were evaluated. Results: The median age of patients with GAS was 46 years old (IQR: 41.5, 59.5), with no significant difference compared to patients with non-GAS HPVI ECA (48-year-old, IQR: 40.5, 60.5) ( p =0.92). Compared with non-GAS HPVI ECAs, GAS hade more common complains of vaginal watery discharge ( p =0.047). GAS cases were also associated with higher clinical stage at diagnosis (P=0.016), deeper cervical stromal invasion ( p =0.01), and worse 5-year progression free survival (PFS) ( p =0.032). Compared with non-MDA GAS, MDA had similar clinical and pathological features and prognosis. Of note, cytology results showed a lower positivity rate for HPVI ECAs (65.2% for GAS and 60% for non-GAS HPVI ECA), and MDA had a lower positivity rate than that for non-MDA (40.0% vs 84.6%, p =0.026). Serum CA19-9 levels were significantly higher in MDA than those in non-MDA (184.5 U/ml vs 22.4U/ml, p =0.045) and non-GAS cases (184.5U/ml vs 10.6U/ml, p =0.006). Conclusions: GAS HPVI ECA had different clinical presentation with genital watery discharge compared with non-GAS HPVI ECA cases. Comparison with those of non-GAS HPVI ECAs, GAS cases were more likely to have high risk pathological factors and poorer PFS. Serum CA19-9 may be helpful for diagnosis and screening in patients with GAS, especially those with MDA.

Abstract:Adenocarcinoma of the cervix is less common than squamous cell carcinoma, although its relative prevalence is increasing. Oncogenic (high-risk) human papillomavirus (HPV) infection is implicated in the development of approximately 90% of all cervical adenocarcinomas. Of the remaining non-HPV associated tumours, the most frequent is gastric-type adenocarcinoma (GAS), which is recognised by the World Health Organization as a form of mucinous adenocarcinoma of the cervix. Minimal deviation adenocarcinoma (MDA) of mucinous type (adenoma malignum) is considered an extremely well differentiated variant of GAS and is encompassed within the category of GAS. The concept of gastric-type cervical glandular lesions has emerged over recent decades and our understanding of the full spectrum of benign, premalignant and malignant lesions is still evolving. Established benign lesions, which are rare, include simple gastric metaplasia and lobular endocervical glandular hyperplasia (LEGH) (complex gastric metaplasia). Postulated premalignant lesions comprise atypical LEGH and gastric-type adenocarcinoma in situ (gAIS); these are probably related lesions for which the umbrella term 'gAIS' has been proposed. The term 'gastric-type' derives from the morphological resemblance of the glandular epithelium to that seen in the stomach and pancreaticobiliary tree; intestinal metaplasia with goblet cells and neuroendocrine cells are present in some cases. A gastric immunophenotype has been demonstrated using markers of pyloric gland mucin, MUC6 and HIK1083. Uncommonly, gastric-type cervical glandular lesions involve multiple sites in the female genital tract and it may be problematic to discern whether these represent independent synchronous or metastatic lesions. There is also an association between gastric-type cervical lesions and Peutz-Jeghers syndrome. Awareness of the spectrum of gastric-type lesions is important, as the diagnostic features of both gAIS and GAS, particularly well differentiated examples, may be subtle, and p16 immunohistochemistry is usually negative. GAS has a much poorer prognosis than HPV-associated cervical adenocarcinoma, with propensity for presentation at advanced stage and wide dissemination to unusual sites such as the ovary, omentum and peritoneum. Although uncommon, GAS is likely to increase in relative prevalence with the introduction of HPV vaccination and precursor lesions will not be detected by primary HPV-based screening programs.

Abstract:Calcium transport and ATPase activities were determined in the heavy and mitochondrial fractions isolated from the left and right atria as well as ventricles of dogs. Ultrastructural distribution of these organelles in different areas of the myocardium was also examined. Calcium binding, calcium uptake, and calcium ATPase activities of the atrial microsomes were lower than those of the ventricles. On the other hand, mitochondrial calcium binding and uptake activities in the right atrium were higher than those in other areas. The mitochondrial total ATPase activities in the atria were also higher than those in the ventricles. Mitochondrial as well as microsomal yields from ventricles were significantly higher. Size and number of mitochondria in the ventricles were greater whereas no striking difference in the distribution of sarcoplasmic reticulum was apparent in different areas of the heart. Poorly developed calcium transport functions in the atrial microsomes may be one of the factors responsible for the generation of lower contractile force in this tissue in comparison with the ventricle.

Abstract:<h3 class="Heading">Background</h3><p class="Para">Endocervical gastric-type adenocarcinoma (GAS) is a rare non-human papillomavirus-associated adenocarcinoma (NHPVA) with morphologic and immunohistochemical features of gastric differentiation. This study aimed to evaluate cytologic and clinicopathological features, differential diagnosis of endocervical GAS.</p><h3 class="Heading">Methods</h3><p class="Para">A total of 23 patients diagnosed with endocervical GAS/minimal deviation adenocarcinoma (MDA) at Peking University People's Hospital between 2009 and 2018 were included. Clinical characteristics, cytologic/histopathologic findings, and immunohistochemical results were collected and analyzed.</p><h3 class="Heading">Results</h3><p class="Para">The average age of patients was 51 years old (range from 28 to 73). Cytologically, tall columnar epithelial cells with pale, foamy or vacuolated cytoplasm were mostly common, followed by well-defined cytoplasmic borders. Fourteen endocervical GAS cases demonstrated mild cytologic atypia, and 9 cases showed moderate to marked cytologic atypia. Ovarian and fallopian tube involvement were identified in 5 and 6 cases, respectively. Immunohistochemically, tumor cells were diffusely positive for CK7, MUC6 and CA-IX, but focally positive for CK20 and CDX2. P16 was negative or patchy positive in most cases and p53 mutation was identified in 12 cases (12/21, 57.1%).</p><h3 class="Heading">Conclusions</h3><p class="Para">Endocervical GAS shows different morphologic and immunological features from endocervical usual type adenocarcinoma, but it may be difficult to be differentiated from metastatic mucinous adenocarcinoma to cervix due to similar morphology and overlapping immunohistochemical profile. Therefore, awareness of the morphologic features and immunohistochemical profile of GAS will allow pathologists to recognize and accurately diagnose this rare and aggressive entity.</p>

Histogram-Based Estimation Techniques in Database Systems

Abstract:Many commercial database management systems maintain histograms to summarize the contents of relations in order to perform efficient estimation of query result sizes and access plan costs. The accuracy of these estimates is often of critical importance. But, there has never been a systematic study of all histogram aspects and their effectiveness in providing accurate estimations. In this thesis, we identify (theoretically and experimentally) the most accurate classes of histograms for estimating the sizes and distributions of the results of several important query operators and provide efficient (sampling-based) techniques to construct these histograms. All of these histograms are novel and differ in fundamental ways from traditional histograms. We provide a systematic classification of all classes of histograms based on certain canonical aspects of histograms that determine their effectiveness in a given estimation problem. We also provide techniques to capture dependencies between attributes in a relation and show that these techniques are far more accurate than the traditional attribute independence assumption. Finally, we use histograms to effectively balance load during parallel joins, thus demonstrating their versatility. Our over all conclusion from the accuracy and efficiency of histogram-based techniques is that they can be used both for enhanced accuracy in traditional applications (e.g., query optimizers) as well as in novel applications that can benefit from estimates (e.g., approximate query processors and load balancers).
Mediodorsal thalamic evoked responses in the rat prefrontal cortex: influence of the mesocortical DA system.

Abstract:The prefrontal cortex (PFC) receives a dopaminergic (DA) innervation from the ventral tegmental area (VTA) and is reciprocally connected with the mediodorsal thalamic nucleus (MD). The present study was performed in anaesthetized rats to determine the influence of the mesocortical DA system on excitatory responses evoked in the PFC by stimulation of MD: (1) short latency (< 4 ms) responses resulting from activation of the MD-PFC pathway; (2) long latency responses (> 10 ms) resulting from activation of recurrent collaterals of PFC neurons projecting to MD. Local DA application and VTA stimulation did not affect short latency responses but blocked long latency responses. These results suggest that the mesocortical DA system inhibits excitations by recurrent collaterals of the PFC-MD neurones but not the excitatory MD inputs to the PFC.
Perception of Body and Hand Animations for Realistic Virtual Characters

Abstract:Researchers and artists bring astonishing cartoon style characters and fantasy creatures to life. However, despite great improvements in recent years, creating a persuasive and highly realistic virtual human is still a big challenge. The motions of a virtual character in a movie or in a game have a large impact on the observer’s impression of it. People are remarkably good in recognizing human motion and very subtle details can be distinguished and interpreted differently. Every person moves and walks in her own way, but all these different styles are perceived as human motions. However, minor changes can lead observers to classify the motion as unnatural. Correct motions are therefore crucial in order to achieve compelling characters. Motion capture technology is commonly used to bring highly realistic characters to life. Even though this technology generates impressive results it is not without flaws and can produce motion artefacts. The impact of such errors is not well understood so far. Conventional perceptual experiments with short snippets of animation are valuable for measuring straightforward perceptibility thresholds such as just noticeable difference. However, this approach does not capture the higher level cognitive responses of complex scenarios, such as those found in movies or games. We therefore introduce and apply a method to study the effect of virtual characters on the viewer that evaluates changes in the emotional response, attention to details, interpretation, and perceived quality of an animation due to degradations in the motion. As the results of our experiments highlighted the importance of correct finger motions, we then focus on the perception, understanding, and enhancement of finger animations. To generate highly realistic animations, we use optical motion capture technology. The accurate capturing of finger motions is still challenging and error-prone due to the small scale and dexterity of fingers. Thus, we improve existing techniques to reach the required accuracy for the creation of our stimuli. Our perceptual approach uses vignettes, i.e. longer animations that tell a story. Within three experiments, where we investigate people’s reactions to synchronization errors and degradations in human motion with questionnaires and by tracking their gaze, we steadily revise and improve this method. To get further insight into finger motions, we perform two perceptual studies, which analyse the noticeability of synchronization errors and the perceived quality of four types of hand animations. Additionally, we examine and reduce the dimensionality of finger motion by identifying barely noticeable movements and rotations that can be accurately computed as a linear relation of each other. Finally, we present a pipeline to add finger motions to a crowd animation system as a platform for further experiments.

Comprehensive genomic profiling and prognostic analysis of cervical gastric-type mucinous adenocarcinoma

Abstract:Gastric-type mucinous adenocarcinoma (GAS) is an uncommon cervical adenocarcinoma, which is not associated with human papillomavirus (HPV) infection. Compared with HPV-associated cervical adenocarcinoma, GAS has characteristics of larger volume, deep invasion, and easy to metastasize to distant sites. Also, GAS is typically resistant to chemo/radiotherapy. Few studies have reported the molecular genetic characteristics of GAS. In order to investigate the molecular genetic characteristics of GAS and reveal its possible pathogenesis, 15 GAS patients were enrolled from Peking University People's Hospital (2009-2019) and examined with next-generation sequencing (NGS). Based on the clinicopathologic feature analysis, we found that the presence of lymph node metastasis and extensive lymphovascular invasion were associated with poor survival outcomes of GAS (p = 0.0042 and p = 0.005, respectively). Based on the NGS testing, our results showed that the most frequently mutated gene was TP53 (8/15, 53.3%), followed by STK11, CDKN2A, and ARID1A. STK11 mutations were more frequent in well-differentiated GAS (33.3% vs. 0.0%, p = 0.026) and patients with extensive lymphovascular invasion (33.3% vs. 0.0%, p = 0.044). Survival analysis revealed that STK11 mutations were significantly associated with the poor prognosis of GAS (p = 0.01). Our results also showed that mutations in the target drug were detected in 53.3% of GAS patients. Patients with ERBB2 amplification (13.3%) presented the highest level of evidence according to OncoKB recommendations. These results indicate that the genomic alterations of GAS mainly involved the cell cycle and PI3K/AKT signaling pathways, and some therapeutic candidates were identified in GAS patients.
Independent Validation of Distinct Clinicopathological Features and Prognosis among Usual-Type, Mucinous-Type and Gastric-Type Endocervical Adenocarcinoma Categorised by New WHO Classification (2020).

Abstract:The new World Health Organization (WHO) classification of tumours of the female genital tract (2020) divides endocervical adenocarcinoma (EAC) into human papilloma virus (HPV)-related adenocarcinoma (HPVA) and HPV-independent adenocarcinoma (HPVI) to underscore the morphological and pathogenetic correlation. It may be potentially prognostic. In this study, we appraised the new WHO classification in an independent, single institution-based EAC cohort from China to assess the clinicopathological features and prognostic value among tumour types. Our study cohort contained 402 consecutive, surgically excised EACs consisting of 298 (74.1%) HPVA, 88 (21.9%) HPVI and 16 (4%) adenocarcinomas not otherwise specified (NOS). Usual-type (55.7%) and gastric-type adenocarcinoma (GAC) (18.2%) was the most common type in HPVA and HPVI, respectively. Block p16 staining (94.7% vs 24.4%) and HPV mRNA signal (89.4% vs 0) were more common in HPVA than in HPVI (p<0.001). HPVI or GAC were more frequently associated with prognostically adverse variables including old age, large tumour size, deep invasion of the cervical wall, high tumour stage, spread of the upper genital tract, lymphovascular invasion, and mutant-type p53 expression, compared to HPVA or mucinous/usual-type HPVA, respectively (all p<0.001). In univariate survival analysis, HPVI had a worse overall survival and higher tumour recurrence compared to HPVA (p<0.05). Mucinous-type HPVA showed a worse prognosis than usual-type HPVA, but better than GAC (p<0.001). Multivariate survival analysis demonstrated that HPVI was independently associated with a worse overall survival and tumour recurrence (p<0.05) while GAC was an adverse prognostic factor independently of FIGO stage (p<0.05). Our findings validate the value of the new WHO classification in prognostic stratification and pathogenetic correlation in EAC and its subtypes.
Whole-exome Sequence Analysis of Gastric-type Adenocarcinoma of the Uterine Cervix and Adjacent Lobular Endocervical Glandular Hyperplasia in the Same Case

Abstract:Lobular endocervical glandular hyperplasia (LEGH) may be a precursor lesion of gastric-type adenocarcinoma of the uterine cervix (GAS). However, the genetic mechanisms underlying its carcinogenesis remain unclear. To elucidate the oncogenic process from LEGH to GAS, we compared gene mutations in early-stage GAS and adjacent LEGH in the same case. Fresh-frozen tissue sections were obtained from a patient with Stage IB3 GAS and adjacent LEGH who had undergone hysterectomy. Using laser microdissection, we harvested the LEGH and GAS portions separately from these sections and extracted the genomic DNA. Somatic variant analysis using whole-exome sequencing used DNA from the normal myometrium as a reference sequence. Somatic variants involving amino acid substitutions were detected in 61 and 125 locations in LEGH and GAS, respectively. Seven variants were common in both lesions, of which the pathogenic variant was GNAS only (c.2531G>A, p.R844H), a mutation frequently reported in pancreatic and colorectal cancers. LEGH had no other pathogenic variants; another pathogenic variant in GAS was found only at the same amino acid site as GNAS (c.2530C>T, p.R844C). In the present case, LEGH and GAS shared the same pathogenic variant of GNAS, indicating that both lesions had a common origin. Furthermore, the current results showed that the second GNAS variant is associated with the progression of LEGH to GAS. Further studies are required to elucidate GAS's pathogenesis and biological characteristics.

In Japan, postoperative adjuvant therapy for cervical cancer includes not only radiation therapy (RT) or concurrent chemoradiotherapy (CCRT) but also chemotherapy in many cases. However, no previous studies have analyzed adjuvant therapy for GAS. In the present study, we investigated the efficacy of adjuvant therapy for GAS. Methods: This was a preplanned secondary analysis of a dataset from previous nationwide, retrospective, observational study. The study population comprised women with stage I-II GAS who underwent surgery. Progression-free survival (PFS) and overall survival (OS) were compared among patients who did and did not receive adjuvant therapy using the Kaplan-Meier method. Results: Data were analyzed for a total of 102 enrolled patients, who were classified as low- (17 patients), intermediate- (37 patients), or high risk (48 patients) based on the risk of postoperative cervical cancer recurrence. In the intermediate-risk group, median survival could not be assessed due to a lack of sufficient events, but the no-adjuvant and RT groups tended to exhibit better prognoses. In contrast, within the high-risk group, patients in the RT subgroup exhibited a trend towards better PFS and OS than those in the CCRT and chemotherapy groups. Conclusions: The prognosis of GAS was confirmed to be poor, even in cases of early-stage cancer and following surgical resection. Chemotherapy strategies, including CCRT as postoperative adjuvant therapy, tend to have a poor prognosis.
Gastric-type Mucinous Carcinoma with an Abnormal Increase of CA199: A Case Report and Literature Review

Abstract:Objective Gastric-type mucinous carcinoma (GAS), as a rare subtype of mucinous adenocarcinoma, accounts for approximately 1%–3% of cervical adenocarcinoma. It was considered as a new type of cervical mucinous adenocarcinoma by the World Health Organization (WHO) in 2014. GAS represents more aggressive disease than does usual type endocervical adenocarcinoma (UEA). Case report A case of cervical adenocarcinoma with an abnormal increase of CA199 in a 50-year-old Chinese woman was reported. Our patient presented with abnormal vaginal discharge and combined with elevated Ca199 at the value of 2,729 U/mL. Imaging examinations showed no abnormalities. Diagnostic conical resection suggested cervical adenocarcinoma in situ . Post-operative pathology confirmed mucinous cervical adenocarcinoma (considering gastric type), infiltrating cervical interstitial >2/3, involving the deep myometrium, accompanied by vascular carcinoma infiltration and lymph node metastasis. The patients received an extensive hysterectomy and post-operative adjuvant chemoradiotherapy. The chemotherapy regimen was paclitaxel, combined with platinum. After 20 months of follow-up, the patient showed no signs of recurrence. Conclusion Preoperative diagnosis of cervical adenocarcinoma is insidious and can be easily misdiagnosed. For patients with high preoperative Ca199, the possibility of GAS should be kept open.

Effects of dietary inulin on serum lipids.

Abstract:Inulin is a carbohydrate belonging to a class of compounds known as fructans. Because inulin is resistant to digestion in the upper gastrointestional tract it reaches the large intestine essentially intact, where it is fermented by indigenous bacteria. Thus, it may be classified as a soluble dietary fiber. Soluble fibers have been shown to modulate serum lipids. A recent study examined the effect of consuming three servings per day of inulin-containing foods, compared with the effect of similar foods without inulin, on serum lipid profiles among hypercholesterolemic men and women. In addition, the practicality of including 18 g/d of inulin in a low fat diet was investigated. The recent study randomized, double-blind, crossover trial with two 6-wk treatment periods, separated by a 6-wk washout. Men and women (n = 21) with baseline LDL increased significantly (7.4 and 12.3%, respectively) during the control phase. There were small, nonsignificant declines in total (1.3%) and LDL-C (2.1%) during the inulin phase. Thus, differences in response between periods (inulin - control) were significant (P < 0.05) for LDL-C (-14.4%) and total cholesterol (-8.7%). Mild gastrointestinal discomfort was more common during the inulin than the control food phase; however, the gastrointestinal side-effect profile of inulin was similar to that of other soluble fibers. Although it was not possible to draw firm conclusions, inulin may have blunted the hypercholesterolemic effects observed during consumption of control foods. Additional research will be required to confirm the possible lipid-modulating properties of dietary inulin in humans.
