Abstract:Abstract Background Degenerative lumber spondylolisthesis (DLS), a common orthopaedic disease causing low back pain, seriously affects daily life and work. Although sagittal spinopelvic parameters have been studied in single-level DLS patients, investigations into different types of double-level DLS are scarce. This study aims to analyse the demographic and radiological parameters of patients with different types of double-level DLS and compare their differences to provide a reference for guiding surgical treatment and restoring sagittal balance of DLS patients. Methods From January 2014 to January 2020, double-level DLS patients’ records were retrospectively reviewed and divided into three types: anterior, posterior, and combined; the anterior and combined types were studied. Two spin surgeons measured the sagittal spinopelvic parameters: C7 tilt, maximal thoracic kyphosis (TKmax), maximal lumbar lordosis (LLmax), pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS). Following descriptive analysis, demographic and radiographic data were compared between the anterior and combined types. Results Patients in the anterior type group (n = 40) were older (67.68 ± 6.66 vs. 61.72 ± 10.06 years, P = 0.031), and the proportion of females were significantly higher (90% vs. 50%, P = 0.001) than the combined group (n = 18). Both groups had different levels of chronic lower back pain, but the incidence of radiating leg pain and neurogenic claudication was significantly higher in the anterior type. Owestry disability index and visual analogue scale lower back scores were also higher in the anterior type. In the anterior type, C7 tilt (7.14 ± 2.15 vs. 5.41 ± 2.28, P = 0.007), LLmax (50.02 ± 14.76 vs. 36.96 ± 14.56, P = 0.003), PI (68.28 ± 9.16 vs. 55.53 ± 14.19, P < 0.001), PT (28.68 ± 7.31 vs. 19.38 ± 4.70, P < 0.001), and PT/PI (42.45 ± 11.22 vs. 36.04 ± 9.87, P = 0.041) were significantly higher. In the anterior type, PI correlated significantly positively with LLmax ( r = 0.59) and SS ( r = 0.71). LLmax and SS ( r = 0.65) had a positive correlation. PT/PI and SS ( r = -0.77) had a negative correlation. In the combined type, PI correlated positively with LLmax ( r = 0.61) and SS ( r = 0.88), and PT/PI correlated negatively with SS ( r = -0.81). Conclusions In patients with double-level DLS, the sagittal spinopelvic parameters differ between the anterior and combined types. PI and LLmax were significantly higher in the anterior type, resulting in compensation by forward flexion and pelvic retroversion. Spinal surgeons should focus on correcting sagittal deformities, relieving postoperative clinical symptoms, and improving quality of life during fusion surgery, which is proven to guarantee long-term surgical results.

Abstract:BACKGROUND AND OBJECTIVE: The intervertebral disc degeneration changes and paraspinal muscles changes are believed to be risk factors for lumbar degenerative spondylolisthesis (LDS). But there is limited quantitative information about this progression. This study is to reveal their changes the in the progression of LDS. METHODS: Data were gathered from 149 middle-aged degenerative spondylolisthesis patients and same amount of age-and sex-matched control group with both lumber spine MRI and X-ray. Narrowed disc space were measured in percent as anterior inferior disc height (DHIA)/anterior superior disc height (DHSA), inferior disc height (DHI)/superior disc height (DHS), and posterior inferior disc height (DHIP)/posterior superior disc height (DHSP). Signal intensity ratio of multifidus muscle (RM) and erector spinae (RES) to psoas muscle, muscle atrophy ratio of lean CSA (LCSA) to gross CSA (GCSA) of paraspinal muscles were calculated. RESULTS: In the case group the most common slipped vertebra was L4 (75.84%). Disc height (DHIA/DHSA, DHI/DHS) and multifidus muscle atrophy ratio (M-LCSA/M-GCSA) tended to be lower than those in the control group, whereas the disc degeneration degree and T2 signal intensity ratio (RM, RES) of the paraspinal muscles and erector spinae muscle atrophy ratio were higher than control group. The difference between the two groups was statistically significant (P < 0.05). Using multivariate logistic regression analysis, it was confirmed that ES-LCSA/ES-GCSA, especially RM are independent predisposing factors to lumbar spondylolisthesis (OR > 1, P < 0.05) while DHIA/DHSA, M-LCSA/M-GCSA are independent protective factors (OR < 1, P < 0.05). CONCLUSIONS: Decreased anterior disc height and multifidus muscle atrophy are found in the LDS patients and thy could be the cause of LDS. The presence of erector spinae hypertrophy could be a compensatory mechanism to compensate for the instability.

Abstract:AbstractThe purpose of our study is to evaluate sagittal parameters in 2-level lumbar degenerative spondylolisthesis (DS) (TLDS).A total of 15 patients with TLDS, 40 patients with single-level DS (SLDS), and 30 normal volunteers as control were included in our study. All subjects performed on full spine X-ray. Two categorized data were analyzed: patient characteristics—age, sex, body mass index, radiographic parameters-pelvic incidence (PI), pelvic tilt (PT), lumbar lordosis (LL), sacral slope (SS), PI–LL, Cobb between the fifth thoracic vertebral and 12th thoracic vertebral (T5–T12), sagittal vertical axis (SVA) Cobb angle of spondylolisthesis level (CSL), ratio of PT to SS (PT/SS), CSL/LL, variation trend of SS over PI, and LL over PI.The PI (73.1° vs 52.9°), SS (50.8° vs 32.2°), LL (53.1° vs 46.9°), SVA (66.1 vs 22.0 mm), PI–LL (20.0° vs 6.0°), and CSL (23.6° vs 20.0°) in TLDS were significantly larger than these in SLDS. The PI (73.1° vs 40.6°), PT (22.3° vs 17.1°), SS (50.8° vs 23.5°), LL (53.1° vs 32.5°), PI–LL (20.0° vs 8.1°), and SVA (66.1 vs 17.0 mm) in TLDS were significantly larger than those in the normal group (NG). The PI (52.9° vs 40.6°), PT (21.0° vs 17.1°), SS (32.2° vs 23.5°), LL (46.9° vs 32.5°), and SVA (22.0 vs 17.0 mm) in SLDS were significantly higher than those in NG. However, PT/SS (44.0%), LL over PI (y = 0.39x + 24.25), SS over PI (y = 10.79 + 0.55x) were lower in TLDS than these in SLDS (63.8%, y = 0.41x + 25, y = 0.65x − 2.09, respectively), and the similar tend between SLDS and NG (74.0%, y = 0.49x + 13.09, y = 0.67x − 3.9, respectively).Our results showed that 2-level lumbar DS, which was caused by multiple-factors, has a severe sagittal imbalance, but single-level has not any. When we plan for surgical selection for 2-level lumbar DS, global sagittal balance must be considered.
Analyzing lumbar vertebral shape and alignment in female patients with degenerative spondylolisthesis: Comparisons with spinal stenosis and risk factor exploration

Abstract:Purpose This study aimed to examine the vertebral body shape characteristics and spondylopelvic alignment in L4 degenerative spondylolisthesis (DS) as well as the risk factors for the development of DS. Methods This cross-sectional study compared vertebral morphology and sagittal spinopelvic alignment in female patients with lumbar DS and lumbar spinal stenosis (LSS). The degree of lumbar lordosis (LL), pelvic incidence (PI), cross-sectional area (CSA), and vertebral body height ratio ( h a /h p ) of the lumbar spine were compared using full-length spine radiographs and computed tomography in 60 females with DS and in 60 women with LSS. Results No significant differences in age or body mass index were observed between the two groups; however, the DS and LSS groups significantly differed in PI (mean, 58.9±10.8 vs. 47.2±11.6, P < 0.001), L4 CSA (mean, 1,166.2 m 2 vs. 1,242.0 m 2 , P = 0.002) and h a /h p (mean, 1.134 vs. 1.007, P < 0.001). The L4 h a /h p was significantly higher in the DS group than in the LSS group. Additionally, LL values were negatively correlated with vertebral L5 CSA in the DS group (r = −0.28, P < 0.05). The LSS and DS groups demonstrated positive correlations between LL and L2, L3, and L4 h a /h p (r = 0.331, 0.267, and 0.317; P < 0.01, < 0.05, and < 0.05, respectively) and between LL and L4 and L5 h a /h p (r = 0.333, 0.331; P < 0.01, respectively). Multivariate regression analyses revealed that PI and h a /h p ratio may be independent predictors of DS development. Conclusion The DS group had significantly larger LL, PI, and L4 h a /h p and smaller L4 CSA than the LSS group. The lumbar vertebral body shape and sagittal spinopelvic alignment in females might be independent predictors of DS development.

Degenerative Spondylolisthesis in the Fifth Lumbar Vertebra and Radiographic Parameters: A Correlation Analysis.

Abstract:Study Design: A retrospective study. Objective: This study aimed to analyze the relationships between degenerative spondylolisthesis in the fifth lumbar vertebra (L5-DS) and radiographic parameters and to further determine the radiographic predictors of the development of L5-DS. Summary of Background Data: Degenerative lumbar spondylolisthesis (DLS) is a common degenerative disease of the spine; however, the correlations between L5-DS and radiographic parameters remain controversial. Patients and Methods: This retrospective case-control study was conducted in our hospital. Between 2011 and 2014, a total of 84 subjects with L5-DS were enrolled in the DLS group, and 56 healthy volunteers were recruited to the control group. A series of radiographic parameters, including the bone mineral density, disk degenerative index, disk height, L5 vertebral size (L5-VS), lumbar lordosis angle (LL), sacral slope angle (SS), pelvic incidence (PI), facet joint angulations (FJA) of the cephalad and caudad portions, and asymmetry of the FJA, were measured in both groups by 3 examiners. Results: The bone mineral density, disk degenerative index, disk height, L5-VS, LL, SS, PI, and FJA exhibited significant differences (P = 0.014-0.045) between the DLS and control groups. Significant changes in the FJA of the cephalad and caudad portions in the L4-L5 and L5-S1 segments were observed between the 2 groups (P = 0.00, 0.00), whereas no significant differences in the asymmetries of FJA were observed in the L4-L5 or L5-S1 segments (P = 0.605-0.972). Among all of the parameters, the L5-VS (P = 0.025), SS (P = 0.020), LL (P = 0.031), PI (P = 0.014), and FJA (P = 0.022) were identified as being associated with the DLS group by multiple logistic regression analysis. Conclusions: In this study, SS, LL, PI, and a more sagittal FJA were proven to be risk factors for L5-DS, whereas L5-VS was found to be a likely protective factor against L5-DS. These parameters should be considered predictors of L5-DS.
Radiographic natural course of lumbar degenerative spondylolisthesis and its risk factors related to the progression and onset in a 15-year community-based cohort study: the Miyama study

Abstract:Background: The natural history and risk factors for lumbar degenerative spondylolisthesis (DS) remain unclear. Because it is important for physicians to take these factors into account to ensure accurate decisions regarding surgical methods, this study aimed to elucidate the natural course and risk factors for the progression of DS. Methods: This is a prospective observation and case control study of 15-year follow-up in a rural mountainous cohort in Wakayama, Japan. In 1990 and 2005, a total of 200 participants (baseline age, range 40-75) were subjected to anteroposterior and lateral radiographs of the lumbar spine, which were acquired with patients in a standing position. The prevalence of DS (slip ≥3 mm) at baseline and the incidence of DS at 15-year follow-up were recorded. Risk factors at baseline for progression of L4 slip (≥3 mm) over the 15-year period were determined by multiple logistic regression analysis. Results: The overall prevalence of DS in 1990 was 10 % (20/200), and by spinal level was as follows: one case at L3, 14 cases at L4, and five at L5. In 2005, the overall prevalence of DS had risen to 22.5 % (45/200). Thus the incidence of de novo DS during the 15-year period was estimated at 14 % (25/180). Progression of the L4 slip (≥3 mm), regardless of baseline condition, was observed in 23 participants after 15 years. In multiple regression analysis, significant risk factors for L4 slip progression were identified as age less than 60 years, female sex, lumbar axis sacral distance, facet sagittalization, and existence of slip at baseline. Conclusions: We successfully elucidated the risk factors for the progression of DS in a general population. Moreover, the results of this study identified preventive factors as well as risk factors for slip progression. This study provides useful information for physicians treating DS.
Different Degeneration Patterns of Paraspinal Muscles Between Double-Level and Single-Level Lumbar Spondylolisthesis: An Magnetic Resonance Imaging Analysis of 140 Patients

Abstract:Objective: To evaluate the degeneration patterns of paraspinal muscles in double-level degenerative lumbar spondylolisthesis (dl-DLS) versus single-level degenerative lumbar spondylolisthesis (sl-DLS). Methods: A total of 67 dl-DLS and 73 sl-DLS patients were included. Multifidus (MF), erector spinae (ES), and psoas major (PM)'s fatty infiltration (FI) and relative cross-sectional area (rCSA) were measured. Sagittal parameters such as lumbar lordosis (LL), sagittal vertical axis (SVA), pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS) were also assessed. Comparisons and correlation analysis were performed between the 2 groups. Results: MF atrophy is worse in dl-DLS patients from L3-4 to L5-S1, with higher FI from L1-2 to L5-S1 compared to sl-DLS patients. ES atrophy and FI are more pronounced in dl-DLS patients from L1-2 to L5-S1. PM atrophy is more significant in dl-DLS patients at L2-3 to L5-S1, with heavier FI from L1-2 to L3-4, though no difference in FI from L4-5 to L5-S1. The rCSA and FI of MF and ES show significant differences between adjacent segments in both groups, except for MF rCSA between L3-4 and L4-5 in dl-DLS. In dl-DLS, PM rCSA negatively correlates with PT from L4-5 to L2-3, while FI of MF and ES in L5-S1 positively correlates with LL. In sl-DLS, PM FI in L4-5 and L5-S1 negatively correlates with LL. Conclusion: Degeneration of MF, ES, and PM is more severe in dl-DLS patients, particularly at the spondylolisthesis level. Severe paraspinal muscle degeneration can lead to spinal force imbalance and progression from sl-DLS to dl-DLS. The degradation of PM and ES correlates negatively with PT and SVA, indicating a link to pelvic decompensation and SVA abnormalities, potentially causing disproportionate degenerative changes in dl-DLS patients.
Correlation analysis between lumbar facets joints and degenerative lumbar spondylolisthesis

Abstract:Objective To study the correlation between the lumbar degenerative spondylolisthesis(LDS) and the mor phologic of the lumbar facet joints through the changes of the lumber arthritis and the sagittal orientation of zy gapophysial joints and the hypozyga-endplate angles.Methods 43 specimens of LDS were collected form October 2009 to November 2011.A group was the 29 specimens of L4/5 withⅠ° LDS and B group was the 14 specimens of L4/5 with Ⅱ° LDS.Computed tomography was used to describe the grading of lumbar facet joints degeneration by Weishaupt method.The facet joints orientation in the horizontal plane of L4/5 and the hypozyga-endplate angles of L4 were surveyed and evaluated through CT.The parameters of the grading of lumbar facet joints degeneration and the sagittal orientation of zygapophysial joint and the hypozyga endplate angle and the symmetry between two groups were compared.Results There was no zero degree between two groups.The specimens of the grading of lumbar facet joints degeneration were 4/11/14 in 1/2/3 degree in A group and the specimens were 1/6/7 in B group.It had not statistic significance between two groups(P > 0.05).The sagittal orientation of zygapophysial joint of L4/5 and its angle subtraction and the hypozyga-endplate angle of L4 were(28.69±8.97)° and(115.91±6.51)° in A group,and(28.23±11.89)° and(117.90±7.23)° in B group.The parameters had statistic significance(P > 0.05).The asymmetry of the sagittal orientation of zygapophysial joint of L4/5 and the hypozyga-endplate angle of L4 were(9.06±4.45)° and(5.21±3.90)° in A group,(12.80±4.95)° and(8.25±3.62)° in B group.The asymmetry had statistic significance in two groups(P < 0.05).Conclusion The asymmetry of the sagittal orientation of zygapophysial joint diminished and the hypozyga-endplate an gle increases the degree of lumbar degenerative spondylolisthesis.
The correlation between bone mineral density and surgical outcomes of lumbar degenerative spondylolisthesis

Abstract:Objective To observe the correlation between bone mineral density (BMD) and surgical outcomes of posterior lumbar interbody fusion (PLIF) for lumbar degenerative spondylolisthesis (DS).Methods From January 2006 to December 2010,69 patients with DS had undergone PLIF by the same surgical team.According the BMD,the cases were divided into two groups.Normal group (T ≥-1.0) had 33 cases [Male 16 cases,Female 17 cases; mean age,(56.5±9.0) yrs; L,,5 20 cases,L5S1 13 cases].The osteopenia group (T ＜-1.0) had 36 cases [Male 13 cases,Female 23 cases; mean age,(60.5±7.8) yrs; L4.5 21 cases,L5S1 15 cases].Blood loss,surgical duration,intra-and post-operative complications were collected.The clinical improvement was quantified by measurement of pain (visual analogue scale,VAS) and Roland-Morris (RM) Disability Questionnaire.Between two groups,the differences of age,body mass index,blood loss,VAS improvement,and RM improvement were compared.The correlation between BMD and sex,age,segment,screw loose,nonunion,and cage subsidence was analyzed.Results In two groups,the difference between pre-and post-operative RM and VAS was significant respectively.The blood loss was 415.5± 105.8 ml in normal group,significantly less than 528.3±128.7 ml in osteopenia group.There was no significant difference in the duration between normal group (169.7±44.3 min) and osteopenia group (176.4±42.6 min).The improvement of VAS and RM between two groups had no significant difference.There was a negative correlation between the BMD and blood loss (r=-0.407,P=0.001).The other surgical outcomes (surgical duration,VAS improvement,RM improvement,cage subsidence,nonunion,screw loose and etc.) had no correlation with BMD.Conclusion There is a negative correlation between the BMD and blood loss in DS patients managed by PLIF.BMD has no effect on other surgical outcomes.

Abstract:Background: Previous studies have reported the normative values of pelvic sagittal parameters, but no study has analyzed the sagittal spino-pelvic alignment in degenerative lumbar scoliosis (DLS) and its role in the pathogenesis. Methods: Retrospective analysis was applied to 104 patients with DLS, together with 100 cases of asymptomatic young adults as a control group and another control group consisting of 145 cases with cervical spondylosis. The coronal and sagittal parameters were measured on the anteroposterior and lateral radiograph of the whole spine in the DLS group as well as in the two control groups. Results: Statistical analysis showed that the DLS group had a higher pelvic incidence (PI) value (50.5° ± 10.2°), than the normal control group (with PI 47.2° ± 8.8°) and the cervical spondylosis group (46.9° ± 9.1°). In DLS group, there were 38 cases (36.5%) complicated with degenerative lumbar spondylolisthesis, who had higher PI values than patients without it. Besides, the lumbar lordosis (LL) and sacral slope (SS) of DLS group were lower; the scoliosis Cobb’s angle was correlated with pelvic tilt (PT); thoracic kyphosis was correlated with LL, SS, and PT; and LL was correlated with other sagittal parameters. Conclusion: Patients with DLS may have a higher PI, which may impact the pathogenesis of DLS. A high PI value is probably associated with the high prevalence of degenerative lumbar spondylolisthesis among DLS patients. In DLS patients, the lumbar spine maintains the ability of regulating the sagittal balance, and the regulation depends more on thoracic curve.

Abstract:It is provided a package easy-to-open for photosensitive material such as an easy-to-open packaged photosensitive material having at least a biaxially stretched plastic film layer and, as an innermost layer, a heat-sealing layer containing not less than 5 wt % of an ethylene copolymer resin and/or a thermoplastic elastomer. At least one layer of the layered film has light shielding performance, and at least one layer other than the biaxially stretched plastic film layer has a ratio of the tear strength in the direction normal to the flow direction to the tear strength in the flow direction of not less than 3. The package has physical strength such as impact resistance, complete hermetic sealing performance, complete light shielding performance, and amenability of discarding and recycling without affecting the quality of the photosensitive material.
Radiographic and Clinical Outcome of Lateral Lumbar Interbody Fusion for Extreme Lumbar Spinal Stenosis of Schizas Grade D: a Retrospective Study

Abstract:Background Extreme lumbar spinal stenosis was thought to be a relative contraindication for lateral lumbar interbody fusion (LLIF) and was excluded in most studies. This is a retrospective study to analyze the radiographic and clinical outcome of LLIF for extreme lumbar spinal stenosis of Schizas grade D. Methods For radiographic analysis, we included 181 segments from 110 patients who underwent LLIF between June 2017 and December 2018. Lumbar spinal stenosis was graded according to Schizas’ classification. Anterior and posterior disc heights, disc angle, foramen height, spinal canal diameter and central canal area were measured on CT and MRI. For clinical analysis, 18 patients with at least one segment of grade D were included. Visual analogue scale (VAS) and Oswestry disability index (ODI) scores were used to evaluate clinical outcome. Continuous variables were compared using Student’s t-test, with P- values < 0.05 considered to indicate statistically significant differences. Results Among the 181 segments included for radiological evaluation, there were 23 grade A segments, 37 grade B segments, 103 grade C segments and 18 grade D segments. Postoperatively, the average change of midsagittal canal diameter of grade D was significantly greater than that of grade A, and not significantly different compared to grades B and C. As to the average change of disc height, bilateral foraminal height, disc angle and central canal area (CCA), grade D was not significantly different from the others. The average postoperative CCA of grade D was significantly smaller than the average preoperative CCA of grade C. Eighteen patients with grade D stenosis were followed up for an average of 19.61 ± 6.32 months. Clinical evaluation revealed an average improvement in the ODI and VAS scores for back and leg pain by 20.77%, 3.67 and 4.15 points, respectively. Sixteen of 18 segments with grade D underwent posterior decompression. Conclusion The radiographic decompression effect of LLIF for Schizas grade D segments was comparable with that of other grades. Posterior decompression was necessary for LLIF to achieve a satisfactory clinical outcome for extreme lumbar spinal stenosis of Schizas grade D.

Abstract:OBJECTIVE:To investigate the relationship between radiographic parameters and the 4th lumbar(L4) degenerative spondylolisthesis.METHODS:From April 2010 to April 2012, 60 patients with the L 4 degenerative spondylolisthesis (DLS) were enrolled in DLS group, 56 healthy volunteers were recruited in control group. A series of radiographic parameters were measured in the two groups, including disc height (DH), disc degeneration index(DDI), L4 vertebral inclination angle(L4-VA), pelvic incidence (PI), L4 vertebral size (L4-VS), lumbar lordosis angle (LLA), facet joint angulation (FJA) of cephalad and caudad portions, delta FJA of cephlad and caudad portions, asymmetry variation of FJA, bone mineral density(BMD). Student's test was used to compare difference of parameters between two groups. Multivariate logistic regression analysis was used to reveal risk factors of the development of DLS.RESULTS:Fifty-three cases of L4 spondylolisthesis in DLS group were classified into grade I, 7 cases of L4 spondylolisthesis were classified into grade II. The average Boxall index was 0.17 ± 0.05. There were significant difference of DH, DDI, L4-VS, L4-VA, LLA, PI, FJA, BMD between DLS group and control group (t = 2.28-9.33, P = 0.021-0.043) . There were significant differences of delta FJA of cephlad and caudad portions in L3-4, L4-5 between DLS group and control group (t = 3.398 and 28.122, P = 0.000 and 0.039). There was no significant difference of asymmetry variation of FJA in L3-4, L4-5 between DLS group and control group (t = 0.209-0.465, P = 0.295-0.858). Multivariate logistic regression analysis showed that LDS was more frequent among patients with smaller L4-VS(OR = 1.01, 95%CI = 1.000-1.024, P = 0.048), larger L4-VA (OR = 1.88, 95%CI = 14.000-14.600, P = 0.037), larger LLA (OR = 1.90, 95%CI = 1.600-15.800, P = 0.040), larger PI (OR = 2.58, 95%CI = 18.000-19.600, P = 0.029) and the more sagittal FJA (OR = 2.46, 95%CI = 1.400-16.400, P = 0.035) than those in control group.CONCLUSIONS:DLS is signifantly correlated with L4-VS, L4-VA, LLA, PI, FJA . They may be risk factors of the development of DLS.

Abstract:[Objective]To study the pathogenesis of lumbar degenerative spondylolisthesis(LDS)through the degeneration degree of lumbar facet joints and the changes of the joint sagittal angle of little zygapophysial joints and the endplate hypozygal angle in patients with LDS and lumbar disc herniation (LDH).[Methods]The image data of 79 patients with lumbar degenerative diseases were collected from October 2009 to November 2011.There were 42 patients with L4-5 LDS(experimental group)and 37 patients in same age with L4-5 LDH(control group).The L4-5 facet joints were graded by Weishaupt ect.lumbar facet joint osteoarthritis imaging grading method.The joint sagittal angle of L4-5 little zygapophysial joints and the L4endplate hypozygal angle were detected by CT.The L4-5 facet joint osteoarthritis grading,joint sagittal angle of lumbar little zygapophysial joints,L4 endplate hypozygal angle and bilateral symmetry between two groups were compared.[Results]There was no grade 0 of facet joint osteoarthritis in both two groups.The numbers of grade 1,2,3 in experimental group were 4,20,18 cases,respectively,while the numbers in control group were 21,14,2 cases.There was significant difference between the numbers of two groups(P ＜ 0.05).The joint sagittal angle of L4-5 little zygapophysial joint,deviation angle of sagittal angle asymmetry and L endphtehypozygal angle in experimental group(26.62°± 1.67°,8.79°± 4.28°,116.92±6.86°)had statistical significance compared with those in control group(45.07° ± 7.32°,5.14° ± 4.28°,104.95° ± 4.97°)(P ＜0.01).The deviation angle of endplate hypozygal angle asymmetry in experimental group had no statistical significance compared with that in control group(P ＞ 0.05).[Conclusions] The sagittal orientation of lumbar facet joint angle and the endplate hypozygal angle increasing have important significance in LDS.

Abstract:Abstract Background Few reports to date have evaluated the effects of different pedicle screw insertion depths on sagittal balance and prognosis after posterior lumbar interbody and fusion (PLIF) in patients with lumbar degenerative spondylolisthesis (LDS). Methods A total of 88 patients with single-level PLIF for LDS from January 2018 to December 2019 were enrolled. Long screw group (Group L): 52 patients underwent long pedicle screw fixation (the leading edge of the screw exceeded 80% of the anteroposterior diameter of vertebral body). Short screw group (Group S): 36 patients underwent short pedicle screw fixation (the leading edge of the screw was less than 60% of the anteroposterior diameter of vertebral body). Local deformity parameters of spondylolisthesis including slip degree (SD) and segment lordosis (SL), spino-pelvic sagittal plane parameters including pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS) and lumbar lordosis (LL), Oswestry Disability Index (ODI), and Visual Analog Scale (VAS) for back pain of both groups were compared. Postoperative complications, including vertebral fusion rate and screw loosening rate, were recorded. Results Except that PI in Group S at the final follow-up was not statistically different from the preoperative value ( P > 0.05), other parameters were significantly improved compared with preoperative values one month after surgery and at the final follow-up ( P 0.05). At the final follow-up, SD, SL, LL, PT and PI-LL differed significantly between the two groups ( P 0.05). Conclusions PLIF can significantly improve the prognosis of patients with LDS. In terms of outcomes with an average follow-up time of 2 years, the deeper the screw depth is within the safe range, the better the spino-pelvic sagittal balance may be restored and the better the quality of life may be.

Radiological variables analyzed were slip percentage, translational and angular motion, facet diastasis/cyst/orientation, laterolisthesis, disc height, scoliosis, main and fractional curve Cobb angles, and spinopelvic parameters. Results. 233 patients (37% decompression, 63% fusion) were included. At < 3 months, high pelvic tilt (PT) (OR 0.92, P 0.02) and depression (OR 0.28, P 0.02) were predictors of MCID non-achievement and GRC non-betterment, respectively. Neither retained significance at > 6 months and hence, were identified as predictors of slower improvement. At > 6 months, low preoperative VAS leg (OR 1.26, P 0.01) and high facet orientation (OR 0.95, P 0.03) were predictors of MCID non-achievement, high L4-5 slip percentage (OR 0.86, P 0.03) and L5-S1 angular motion (OR 0.78, P 0.01) were predictors of GRC non-betterment, and high preoperative ODI (OR 0.96, P 0.04) was a predictor of PASS non-achievement. Conclusions. High PT and depression were predictors of slower improvement and low preoperative leg pain, high disability, high facet orientation, high slip percentage, and L5-S1 angular motion were predictors of non-improvement. However, these are preliminary findings and further studies with homogeneous cohorts are required to establish these findings.

Abstract:Objective: To investigate the segmental instability of degenerative lumbar spondylolisthesis (DLS) with a kyphotic configuration at the involved segment, and to determine the most useful diagnostic modalities in the evaluation of instability. Patients and methods: This study reviewed a consecutive series of patients with L-4/5 DLS between July 2010 and May 2016. The enrolled patients were divided into two groups based on preoperative neutral radiographs: the kyphotic group (Group K) and non-kyphotic group (Group NK). Translational and angular motion was determined by comparing upright lateral radiograph (U) with a supine sagittal MR image(S) (Combined, US) or flexion/extension radiographs (FE). Results: There were 26 and 201 patients in Groups K and NK, respectively. In comparison to Group NK, Group K demonstrated significantly higher translational motion (12.4% vs. 7.0%, P < 0.001) on US analysis, but significantly lower translational motion (4.2% vs. 6.4%, P < 0.001) on FE analysis. Angular motion was detected to be significantly lower in US versus FE in Group NK (1.2 degrees vs. 7.8 degrees, P < 0.001), while of no difference in Group K (P > 0.05). In Group K, "instability" was recognized in 84.6% of patients using US versus 11.5% patients using FE (P < 0.001); While in Group NK, no significant difference was observed in the incidence of "instability" between FE and US (31.3% vs. 27.8%, P = 0.444). Overall, Group K had a significantly higher incidence of instability than Group NK (84.6% vs. 31.3%, P < 0.001). Conclusion: DLS with a kyphotic configuration is a distinct subgroup associated with segmental instability. The modality of US is shown to be superior to traditional FE in measuring translational motion and identifying instability for DLS patients with a kyphotic configuration.
