Abstract:BACKGROUND: The sitting position has become the most common posture in today's workplace. In relation to this position, kinematic analysis of the lumbar spine is helpful in understanding the causes of low back pain and its prevention.METHODS: In this study, we investigated the relationship between sagittal lumbar alignment and pelvic alignment in the standing and sitting positions for 50 healthy adults. Lumbar lordotic angle (LLA), sacral slope (SS), pelvic tilt (PT), and pelvic incidence (PI) were measured on lateral lumbar spine standing and sitting radiographs.RESULTS: Regarding changes from the standing to sitting positions, average LLA, SS, and PT were -16.6° (-49.8 %), -18.7° (-50.3 %), and 18.3° (284.8 %), respectively (P < 0.01). In the sitting position, lumbar lordosis was reduced and pelvic rotation became posterior.CONCLUSIONS: This study showed that LLA decreased by approximately 50 % and PT increased by approximately 25 % in the sitting position compared with the standing position. No significant gender differences were observed for LLA, SS, and PT in the standing position. In the sitting position, however, LLA and SS were markedly larger for women.

Analysis of spino-pelvic sagittal alignment in Chinese young subjects in standing versus sitting positions

Abstract:Objectives:To analyze the spino-pelvic sagittal alignment of Chinese asymptomatic adults in standing versus sitting positions,based on the characteristics of asymptomatic adults' sagittal alignment in standing,the variation in sitting position.Methods:This was a prospective radiological analysis by using fullspine standing and sitting lateral radiographs of Chinese volunteers.145 volunteers (51 males,94 females;mean age,23.1±2.3 years) participated.Pelvic and spinal parameters were measured,including pelvic incidence(PI),pelvic tilt(PT),sacral slope(SS),lumbar lordosis(LL),thoracic kyphosis(TK),thoracic lordotic kyphosis(TLK),lordosis tilt(LT),sagittal vertical axis(SVA) and T1-pelvic angle(T1PA).By using Student's t test,the angular parameters were compared between standing posture and sitting posture.By using Pearson's correlation test according to different position,difference of relationship between spinal and pelvic parameters in standing versus sitting position were discussed.Results:Mean PI was 46.6°±9.1°.When moving from standing to sitting position,the spine lost nearly 24° of LL(49.6°±9.8° vs 25.2°±10.8°,P＜0.001).The SVA also moved more anteriorly by 45mm(-20.7±20.8mm vs 24.5±29.5mm,P＜0.001).Regarding changes from the standing to sitting position,average PT,LT,T1PA were greater(P＜0.05),and TK,SS were decreased(P＜0.05).In sitting position,the correlations of PI-SVA,LL-SVA,PI-T1PA,LL-T1PA were enhanced.Conclusions:In sitting position,the majority of changes occures in the lumbar spine and pelvis alignment.Sitting significantly straightens the spine with decreased TK,LL and SS.Lumbar alignment and SVA move anteriorly.Pelvis rotation and lumbar hypolordosis are the mechanisms of adjusting the trunk sagittal balance in sitting position.This variation in sitting position should be fully considered in terms of long-term effects of the sitting position in patients with lumbar and thoracic fusion.

Abstract:Objective To compare the spine-pelvic sagittal parameters of the healthy and young Han males in standing, erect sitting and natural sitting. Evaluate the effect of pelvic rotation on sagittal balance. Methods This was a cross-sectional study. Thirty-two volunteers were recruited from the graduate students of Nanjing Drum Tower Hospital from September 2017 to October 2017. All the volunteers were males, aged between 23 to 27 (24.8 ± 1.6) years old, 171 -185 (176.4 ± 5.3) cm in height, 57.7 -88.4 (69.5 ± 8.6) kg in weight, and BMI 21.2-24.7 (22.5 ± 2.3) kg/m2. EOS-X-ray imaging system was applied to all subjects, and vertical scanning method was adopted. Full-length images were scanned once, and standing position, erect sitting and natural sitting position were taken. Parameters were measured included thoracic kyphosis (TK), lumbar lordosis (LL), sacral slope (SS), pelvic incidence (PI), pelvic tilt (PT), sagittal vertical axis (SVA), T1 pelvic angle (TPA). Paired-samples T test was used to compare the changes of parameters in different positions, and Pearson correlation analysis was used to analyze the relationship between spinopelvic parameters. Results For 32 volunteers, in standing position, the average PI was 49.06°± 6.22°, PT was 13.38°± 6.06°, SS was 35.03°± 2.32°, LL was -47.16°± 7.64°, TK was 32.22°± 9.74°, SVA was (3.29 ± 13.64)mm, TPA was 9.47°± 5.26°. In erect sitting position, the average PI was 48.22°± 6.07°, PT was 29.06° ± 10.35°, SS was 18.84° ± 5.90°, LL was -26.38° ± 8.45°, TK was 31.78° ± 9.95°, SVA was (34.73 ± 13.62) mm, TPA was 26.06° ± 8.71°. In natural sitting position, the average PI was 49.22°± 6.20°, PT was 40.50°± 10.42°, SS was 9.84°± 4.87°, LL was -4.03° ± 6.14°, TK was 31.69° ± 9.73°, SVA was (63.37 ± 13.70) mm, TPA was 49.66° ± 11.22°. Compared with the parameters of standing position, PT, SVA and TPA parameters of erect and natural sitting position increased, SS and LL parameters decreased. Compared with the parameters of erect sitting position: PT, SVA and TPA parameters of natural sitting position increased, SS and LL parameters decreased. The differences were statistically significant( all P values<0.05). Pearson correlation analysis results showed that: ( 1 ) In standing position, PI was correlated with PT, LL, TK and TPA. PT was correlated with LL, TK and TPA. LL was correlated with TK and TPA. TK was correlated with TPA.(2) In erect sitting position, PI was correlated with PT, SS, LL, TK and TPA. PT was correlated with SS, LL, TK and TPA. SS was correlated with LL, TK and TPA. LL was correlated with TK and TPA. TK was correlated with TPA.(3) In natural sitting position, PI was correlated with PT, SS, LL, TK and TPA. PT was correlated with SS, LL, TK and TPA. SS was correlated with LL, TK and TPA. LL was correlated with TK and TPA. Differences were statistically significant ( all P values<0.05). Conclusions In the process of from the standing position to the upright sitting position, the upright sitting position to the natural sitting position, Han people the LL of normal Han young men is decreasing and SVA, TPA are increasing. It indicates that SVA moves anteriorly and the pelvic rotates posteriorly. Some correlation between spinal parameters and pelvic parameters are also shown. The changing of pelvis can influence the sagittal alignment and balance of the spine.
Postoperative changes in sagittal spinopelvic alignment in sitting position in adolescents with idiopathic thoracic scoliosis treated with posterior fusion: an initial analysis

Abstract:OBJECTIVE Previous studies have reported spinal straightening and pelvic retroversion when changing from erect to sitting posture in patients with adolescent idiopathic scoliosis (AIS), which were thought to be related to low-back pain after sitting for long periods. However, the sitting sagittal alignment after posterior spinal fusion has not been evaluated. This study aims to assess the influence of posterior fusion surgery upon sitting sagittal spinopelvic alignment in adolescents with idiopathic thoracic curves (thoracic AIS [T-AIS]). METHODS A total of 44 T-AIS patients (30 Lenke I and 14 Lenke II) from the authors' center were included in this study. Preoperative and postoperative long-cassette lateral radiographs of the spine and pelvis were obtained with the patients in standing and sitting positions. Thoracic kyphosis (TK), lumbar lordosis (LL), pelvic incidence (PI), sacral slope (SS), and pelvic tilt (PT) were measured on standing and sitting lateral radiographs. Patients were divided into selective thoracic fusion (STF) and nonselective thoracic fusion (NSTF) groups. RESULTS At baseline, TK, LL, and SS decreased by 27.5%, 42.1%, and 31.1%, respectively, from the standing to the sitting position, while PT increased by 193.6%. After posterior spinal fusion, increased TK, LL, and SS and corresponding decreased PT were observed compared to baseline parameters in the sitting position. Comparison of postoperative sitting and standing values for the whole cohort showed that the mean LS and SS values were significantly lower in the sitting position (decreased by 14.0% and 13.9%, respectively, compared to standing), whereas the mean PT value was significantly greater (increased by 39.0%, compared to standing). Similar changes were also observed in the STF group: postoperatively the mean LL value was 15.6% lower in sitting than in standing, while the mean SS value was 11.5% lower. However, no obvious changes of the postoperative values in sitting were found in the NSTF group. CONCLUSIONS Nonselective thoracic fusion surgery in T-AIS patients diminished spinal straightening and pelvic retroversion during sitting. Reducing distal fusion levels was of special value in not only saving more lumbar mobility, but also preserving the function of pelvic posterior rotation.
Gender differences in spinal mobility during postural changes: a detailed analysis using upright CT

Abstract:Lumbar spinal alignment is crucial for spine biomechanics and is linked to various spinal pathologies. However, limited research has explored gender-specific differences using CT scans. The objective was to evaluate and compare lumbar spinal alignment between standing and sitting CT in healthy individuals, focusing on gender differences. 24 young and 25 elderly males (M) and females (F) underwent standing and sitting CT scans to assess lumbar spinal alignment. Parameters measured and compared between genders included lumbar lordosis (LL), sacral slope (SS), pelvic tilt (PT), pelvic incidence (PI), lordotic angle (LA), foraminal height (FH), and bony boundary area (BBA). Females showed significantly larger changes in SS and PT when transitioning from standing to sitting ( p = .044, p = .038). A notable gender difference was also observed in the L4-S LA among the elderly, with females showing a significantly larger decrease in lordotic angle compared to males (− 14.1° vs. − 9.2°, p = .039*). Females consistently exhibited larger FH and BBA values, particularly in lower lumbar segments, which was more prominent in the elderly group (M vs. F: L4/5 BBA 80.1 mm 2 [46.3, 97.8] vs. 109.7 mm 2 [74.4, 121.3], p = .019 in sitting). These findings underline distinct gender-related variations in lumbar alignment and flexibility, with a focus on noteworthy changes in BBA and FH in females. Gender differences in lumbar spinal alignment were evident, with females displaying greater pelvic and sacral mobility. Considering gender-specific characteristics is crucial for assessing spinal alignment and understanding spinal pathologies. These findings contribute to our understanding of lumbar spinal alignment and have implications for gender-specific spinal conditions and treatments.

The effect of pelvic motion on spinopelvic parameters

Abstract:Background context: To date, many studies have examined how pelvic position affects the spinal curvature and spinopelvic parameters. However, these studies focus on a static relationship, comparing pelvis and spine in a relaxed or baseline position only. Indeed, the spinopelvic connection is dynamic, as subjects can easily be taught to rotate their pelvis anteriorly or posteriorly on the femoral head, all while maintaining an erect posture. Therefore, for a true understanding of pelvic influence on the spinal column, it is necessary to examine spinopelvic parameters in multiple pelvic positions within the same subject. Purpose: The objective of this study was to examine the dynamic effect of pelvic motion on the spine and associated radiographic parameters. Study design: This is a single-center, cross-sectional study of 50 healthy, asymptomatic volunteers. Patient sample: Subjects were recruited and screened based on the following criteria: between 18 and 79 years of age; no known spinal, pelvic, or lower extremity pain lasting for >48 hours; no history of spinal, pelvic, or lower extremity dysfunction requiring medical care; no radiographic evidence of spinal or pelvic abnormality, scoliosis deformity, or other associated spinal pathologies; not currently pregnant and with no possibility of being pregnant; and a body mass index of <30. 64. The subjects were screened and 14 were excluded for a total of 50 subjects. Outcome measures: The outcome measures included thoracic kyphosis (TK), lumbar lordosis (LL), sagittal vertical axis (SVA), pelvic tilt (PT), sacral slope (SS), and pelvic incidence (PI). Materials and methods: This study was funded by a Small Exploratory Research Grant from the Scoliosis Research Society. Each subject was instructed and observed to stand in three different positions: pelvic resting, anterior pelvic rotation, and posterior pelvic rotation. Lateral standing radiographs were taken in each position and each image was examined by an orthopedic spine surgeon who digitally measured the TK, LL, SVA, PT, SS, and PI. The data were then statistically examined to determine the affect of pelvic position on each parameter. Results: Subjects demonstrated a measurable, statistically significant change in each parameter with pelvic rotation. There was a clear pattern of change for LL, PT, and SS with the anterior and posterior pelvic rotations. A change in LL demonstrated a strong correlation with changes in all measured parameters with pelvic rotation. Conclusions: In asymptomatic subjects, pelvic motion affects the position of the spinal column and resultant spinopelvic parameters. The results of this study demonstrate that one can intentionally change the position of the pelvis and the adjacent spinal column in space. Knowledge of this relationship is important to the understanding of sagittal balance and could influence the treatment of patients with spinal deformity.
Effect of different seat heights on lumbar spine flexion during stand-to-sit motion

Abstract:[Purpose] This study aimed to investigate the movement of the thorax, lumbar spine, and pelvis when healthy participants sit on a chair, and to identify the kinematic characteristics due to changes in the height of the seat. [Participants and Methods] Twenty healthy participants (14 males, 6 females; mean age, 29 ± 5 years) were recruited for this study. They performed stand-to-sit motion using one seat with a height of 100% that of the lower leg length (standard) and another with a height of 60% that of the lower leg length (lower). A three-dimensional motion analysis system and four force plates were used to analyze each joint angle. [Results] The mean lumbar spine flexion angle was significantly increased in the lower versus the standard seat. As a kinematic characteristic, the pelvis tilted posteriorly while the thorax tilted anteriorly, which increased the lumbar spine flexion angle. The pelvis was tilted posteriorly when the hip joint flexed about 60° regardless of the seat height. [Conclusion] The lumbar spine flexion angle increased in the lower seat stand-to-sit motion, which suggested an increase in the load on the lumbar spine. The lumbar spine flexion angle was influenced by the characteristic movements of the thorax and pelvis.
Variations of Sagittal Alignment in Standing Versus Sitting Positions Under the Roussouly Classification in Asymptomatic Subjects

Abstract:Study Design: Prospective radiological analysis. Objective: To investigate the characteristics of the sagittal spinopelvic alignment in the standing and sitting positions under the Roussouly classification in a healthy population. Methods: This study using standing and sitting lateral radiographs included 143 volunteers whose spinopelvic parameters were measured. The parameters were compared in the standing and sitting positions. The lumbo-pelvic profile was identified according to the Roussouly classification. The differences in the values of the parameters between the standing and sitting positions were analyzed. The different frequencies of the unchanged apex were assessed. Results: All parameter values except the thoracolumbar angle (TLK) were significantly altered. With regard to the classification of the lumbopelvic profile, 15 (10.5%) subjects were excluded because they were evaluated as not belonging to any of the Roussouly types. The remaining 128 subjects were evaluated and classified as follows: 19 (14.8%), type 1; 53 (41.4%), type 2; 9 (7.0%), type 3 + anteverted pelvis (AP); 33 (25.8%), type 3; and 14 (10.9%), type 4. The differences in the pelvic tilt (PT), sacral slope (SS), and thoracic pelvic angle (TPA) values were significantly lower in types 1 and 2 than in other Roussouly types. Corresponding variations in the apical vertebra for the sagittal lumbar curves in the sitting position were observed. Conclusions: Subjects with Roussouly type 1 and type 2 sagittal alignments were found to be less capable of performing changes in alignment when changing to the sitting position. The characteristics of the apical movement were also different.

Abstract:BACKGROUND:The interactions between the spine, pelvis, and lower limbs are dynamic based on the "cone of economy" concept; thus, different global radiographic parameters could be regarded as reflections of different centers of gravity. We conducted this retrospective study to evaluate the offsets of different centers of gravity in asymptomatic populations and to investigate how the global sagittal alignment is supported.METHODS:The following parameters were measured: cervical lordosis, thoracic kyphosis (TK), lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), the ratio between PT and PI (PT/PI), sacral slope, PI minus LL (PI-LL), the sagittal vertical axis (SVA), cranial SVA to ankle center (Cr-A), CrSVA to the femoral head center (Cr-FH), C2SVA to the femoral head center (C2-FH), pelvic translation (P. Shift), and knee angle (KA). Participants were divided into subgroups based on the PT/PI ratio. Mean values were compared using the t-test, and correlations were assessed using Pearson's coefficient.RESULTS:A total of 82 asymptomatic adults were enrolled. The average PT/PI in subgroup 1 was the smallest, showing that individuals in this group may have limited pelvic retroversion. No significant differences in Cr-FH, Cr-A, or C2-FH were found between subgroups (all P>0.1), implying that global alignment was well supported in each group. Specifically, C2-FH showed minor changes between subgroups (P=0.998), showing that C2-FH may be a target for sagittal compensation. There were positive correlations between PT/PI and both P. Shift and SVA (r=0.930 and r=0.606, respectively). However, Cr-FH, Cr-A, and C2-FH were not significantly correlated with P. Shift or PT/PI (all P>0.05). Weak correlations existed between Cr-A, Cr-FH, and age (all P>0.2).CONCLUSIONS:This study revealed that the Cr-FH and C2-FH offsets are stable across the population and could be maintained by regulating only the sagittal spinal curvature when pelvic compensation is limited. Cr-FH is not affected by age in the asymptomatic population. Thus, the stable Cr-FH and C2-FH could provide references for surgeons during the surgical decision-making process in patients with adult spinal deformity with sagittal malalignment.
The Standing and Sitting Spino‐Pelvic Sagittal Alignment in Patients with Instrumented Lumbar Fusion Might Correlate with Adjacent Segment Degeneration

Abstract:Objectives Sitting is a common weight‐bearing posture, like standing, but there still lacks enough understanding of sagittal alignment in sitting position for patients after lumbar fusion. This study aimed to investigate the accommodation of fixed spine from standing to sitting position and its influence on unfused segments. Methods Sixty‐two patients after lumbar fusion (test group) and 40 healthy volunteers (control group) were recruited in this research. All subjects underwent lateral radiographs of entire spine in the standing and sitting positions. The spinopelvic parameters including sagittal vertical axis (SVA), T1 pelvic angle (TPA), lumbar lordosis (LL), thoracic kyphosis (TK), and pelvic tilt (PT) were measured. The changes in parameters of patients between two positions were compared with control group, and patients were divided in different groups based on fusion level and their parameters were compared. Results When changing from standing to sitting positions, a forward‐moving SVA and TPA were observed in both patients and control groups, accompanied by the decrease in LL, TK and increase in PT, but the changes of patients were smaller in TPA, LL, and TK (6.5° ± 7.2° vs 9.7° ± 6.0°, 7.7° ± 8.3° vs 13.6° ± 8.5°, 2.2° ± 6.5° vs 5.4° ± 5.1°, respectively, p < 0.05). Increase of PT in the lumbosacral fixation group was lower than that in the control group (4.4° ± 9.1° vs 8.3° ± 7.1°, p < 0.05). Patients who had adjacent segments degeneration (ASD) showed more kyphosis in unfused lumbar segments than the other patients (16.4° ± 10.7° vs −1.0° ± 4.8°, p < 0.05) from standing to sitting. Conclusions The spine straightens in lumbar and thoracic curve, combined with forward‐moving axis and pelvic retroversion when changing to the sitting position. However, these changes are relatively limited in patients after lumbar fusion, so the adjacent unfused lumbar segments compensate to stress during sitting and this may be related to ASD.

Abstract:Abstract Variability in body shape and soft tissue geometry have the potential to affect the body's interaction with automotive safety systems. In this study, we developed a methodology to capture information on body shape, superficial soft tissue geometry, skeletal geometry, and seatbelt fit relative to the skeleton—in automotive postures—using Open Magnetic Resonance Imaging (MRI). Volunteer posture and belt fit were first measured in a vehicle and then reproduced in a custom MRI-safe seat (with an MR-visible seatbelt) placed in an Open MR scanner. Overlapping scans were performed to create registered three-dimensional reconstructions spanning from the thigh to the clavicles. Data were collected with ten volunteers (5 female, 5 male), each in their self-selected driving posture and in a reclined posture. Examination of the MRIs showed that in the males with substantial anterior abdominal adipose tissue, the abdominal adipose tissue tended to overhang the pelvis, narrowing in the region of the Anterior Superior Iliac Spine (ASIS). For the females, the adipose tissue depth around the lower abdomen and pelvis was more uniform, with a more continuous layer superficial to the ASIS. Across the volunteers, the pelvis rotated rearward by an average of 62% of the change in seatback angle during recline. In some cases, the lap belt drew nearer to the pelvis as the volunteer reclined (as the overhanging folds of adipose tissue stretched). In others, the belt-to-pelvis distance increased as the volunteer reclined. These observations highlight the importance of considering both interdemographic and intrademographic variability when developing tools to assess safety system robustness.

Abstract:The effects of vehicle package, seat, and anthropometric variables on posture were studied in a laboratory vehicle mockup. Participants (68 men and women) selected their preferred driving postures in 18 combinations of seat height, fore-aft steering wheel position, and seat cushion angle. Two seats differing in stiffness and seat back contour were used in testing. Driving postures were recorded using a sonic digitizer to measure the 3D locations of body landmarks. All test variables had significant independent effects on driving posture. Drivers were found to adapt to changes in the vehicle geometry primarily by changes in limb posture, whereas torso posture remained relatively constant. Stature accounts for most of the anthropometrically related variability in driving posture, and gender differences appear to be explained by body size variation. Large intersubject differences in torso posture, which are fairly stable across different seat and package conditions, are not closely related to standard anthropometric measures. The findings can be used to predict the effects of changes in vehicle and seat design on driving postures for populations with a wide range of anthropometric characteristics.

Abstract:To investigate the characteristics of standing and sitting spinopelvic sagittal alignment among Chinese healthy population with different age groups. This cross-sectional, prospective study included a total of 235 volunteers aged 19 to 71 years. Volunteers were divided into two groups: group A (age ≤ 40 years; n = 140) and group B (age > 40 years, n = 95). Student’s t test was performed to compare the sagittal parameters including sagittal vertical axis (SVA), T1 pelvic angle (TPA), cervical lordosis (CL), thoracic kyphosis (TK), lumbar lordosis (LL) and pelvic tilt (PT) between standing and sitting positions of two groups. Multiple regression was performed to explore the influence factors of differences between two positions. In the standing position, group B had larger SVA, TK, PT and TPA than group A. When moving from standing to sitting position, increased SVA and PT were found in both groups, accompanied by decreased LL and TK. However, despite similar change in SVA, group B presented with lesser changes in LL, PT and TPA than group A in sitting position. Age and gender independently influenced the difference in PT and LL. In the standing position, the older volunteers showed larger SVA, TPA, TK, CL and PT than young population. Both groups showed similar changes when moving from standing to sitting, but the differences between the positions were smaller in older population. These characteristics in the standing and sitting positions of different age groups should be considered when planning surgical reconstruction of sagittal alignment. Effect of office chair design features on lumbar spine posture, muscle activity and perceived pain during prolonged sitting

Abstract:Chair design features are typically compared using multiple seats, which can lead to confounding effects. Using a single chair, configurable to four designs (control, lumbar support, seat pan tilt and scapular relief), we investigated the effect of chair design on spine posture and movement, muscle activity and perceived pain in a sample of 31 asymptomatic adults. A total of 39% of the population were classified as pain developers, having significantly higher peak pain levels across most body regions. The lumbar support and seat pan tilt condition resulted in more neutral spine and pelvic postures. Greater muscle activity was found in the seat pan condition and non-pain developers displayed lower spine muscle activation levels overall. Despite some improvements in spine posture, sitting-induced pain was present in the study sample at similar proportions to those reported previously. Future studies may consider investigating interventions targeted to sitting-induced pain developers as opposed to the general population. Practitioner summary: Four office chair configurations were tested. The lumbar support and seat pan tilt conditions resulted in the most neutral back posture but did not mitigate the clinically significant levels of sitting-induced pain experienced by a large portion of the tested sample. Future work should target interventions to these individuals.

Abstract:Background This study aimed to investigate whether the distance between the C2 vertical line and the femoral heads (C2-FH) is quasi-invariant for the sitting position. Methods A cross-sectional, prospective study was conducted. A cohort of 59 asymptomatic volunteers was prospectively recruited between February 1, 2020, and February 31, 2020, at Nanjing Drum Tower Hospital of Nanjing University Medical School. The following radiographical parameters were evaluated: T4–T12 thoracic kyphosis (TK), cervical lordosis (CL), lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS), sagittal vertical axis (SVA), T1-pelvic angle (TPA), and the C2-FH. The inclusion criteria were as follows: age between 18 and 35 years and no symptoms related to neck or back pain. The exclusion criteria were as follows: a history of any prior lower extremity or spine surgery; and a history of inflammatory arthritis, neuromuscular disorders, or congenital anomalies. Individuals were divided into the following 3 groups according to the percentile of ΔPT (PT difference between standing position and sitting position): group A (1–25% ΔPT, n=14), group B (25–75% ΔPT, n=30), and group C (75–100% ΔPT, n=15). Summary statistics calculated using analysis of variance (ANOVA) were used to provide 95% confidence intervals for measurement errors. The significance level of all statistical analyses was set as P < 0.05. Results There was no significant difference in any of the parameters among the 3 groups in the standing position. In the sitting position, for different degrees of pelvic retroversion, there were significant differences between the 3 groups in the other parameters except for C2-FH (P=0.80; 95% CI: −20.486 to −3.893), CL (P=0.47; 95% CI: −5.645 to 4.964), TK (P=0.54; 95% CI: 17.058–25.1), and PI (P=0.44; 95% CI: 40.747–49.087). Surprisingly, C2-FH changed slightly among the 3 groups in both the standing (P=0.87) and sitting (P=0.80) positions. As ΔPT gradually increased, ΔSS, ΔLL, ΔSVA, ΔL1-L4, ΔL4-S1, ΔTPA, ΔPT/PI, and ΔPI-LL showed corresponding gradual changes (all P values <0.05). However, there was no significant difference in PI (P=0.39), CL (P=0.46), C2-FH (P=0.51), or TK (P=0.51). Conclusion C2-FH is a quasi-invariant parameter in both the standing and sitting positions for asymptomatic adults. The mean value of C2-FH was −11.95 mm in the standing position and −1.01 mm in the sitting position. Stable C2-FH could serve as a reference during the surgical decision-making process in adult patients with spinal deformity and sagittal malalignment.
The Effects of Backward Adjustable Thoracic Support in Wheelchair on Spinal Curvature and Back Muscle Activation for Elderly People

Abstract:OBJECTIVES: To investigate the effects of backward adjustable thoracic support on spinal curvature and back muscle activation during wheelchair sitting.METHODS: Twenty elderly people were recruited for this study. The backward adjustable thoracic support sitting posture was compared with the slumped, normal, and lumbar support sitting postures. Spinal curvatures (pelvic, lumbar, and thoracic angles) and muscle activations of 4 back muscles on both sides (maximal voluntary isometric contraction of the lumbar multifidus, lumbar erector spinae, iliocostalis lumborum pars thoracis, and thoracic erector spinae at T9) were measured and compared between the different sitting postures using one-way analysis of variance with repeated measures.RESULTS: The backward adjustable thoracic support sitting posture showed a relatively neutral pelvic tilt (-0.32±4.80°) when compared with the slumped (22.84±5.27°) and lumbar support (-8.97±3.31°) sitting postures (P<0.001), and showed relatively higher lumbar lordosis (-23.38±6.50°) when compared with the slumped (14.77±7.83°), normal (0.44±7.47°), and lumbar support (-16.76±4.77°) sitting postures (P<0.05). It also showed relatively lower back muscle activity when compared with the normal and lumbar support sitting postures (P<0.05).CONCLUSIONS: The backward adjustable thoracic support sitting concept was suggested because it maintains a more neutral pelvic tilt, higher lumbar lordosis, and lower back muscle activation, which may help maintain a better sitting posture and reduce the risk of back pain.

Sagittal Plane Analysis of the Spine and Pelvis in Adult Idiopathic Scoliosis.

Abstract:BACKGROUND:There has been an increasing recognition of the importance of sagittal spinopelvic alignment in patients with scoliosis as it relates to clinical outcomes. However, the changes seen in sagittal spinopelvic alignment in adult idiopathic scoliosis patients is poorly defined. This study was conducted to evaluate the sagittal alignment of pelvis and spine in adult idiopathic scoliosis patients.METHODS:The sagittal parameters of the spine and pelvis were analyzed in lateral standing radiographs of 124 patients (mean age 47.4 years) with adult idiopathic scoliosis, including thoracic kyphosis (TK), thoracolumbar junction kyphosis (TLJ), lumbar lordosis (LL), pelvic incidence (PI), sacrum slope (SS), pelvic tilt (PT) and C7 plumb line (C7PL). The patients were divided into three groups according to the age: 20 - 40 years, 41 - 64 years, and ≥ 65 years. The parameters were compared with those in normal adults and adolescent idiopathic scoliosis (AIS) patients. The relationship between all parameters as well as age and sagittal parameters were analyzed.RESULTS:The PI in patients with adult idiopathic scoliosis was 58.1° ± 13.0°, which was significantly higher than that in normal adults. The PT (19.9° ± 10.6°) was also higher than that in both normal adults and AIS patients, while the SS (38.1° ± 12.0°) was similar or smaller. As age increased, C7PL, PT and TJL increased while LL decreased. There was no relationship between age and both PI and TK. PT had the strongest statistical association with the C7PL.CONCLUSIONS:PI is higher in adult idiopathic scoliosis than normal subjects. The PT is the most relevant pelvic parameter to the global sagittal alignment of the spine. Age significantly influences sagittal parameters of the spine and pelvis except the PI and TK.
Cervical Sagittal Alignment and Balance Associated with Aging Chinese Adults: A Radiographic Analysis

Abstract:Study Design This was a retrospective cohort radiographic study. Objective To determine the age- and gender-related normative values and correlation of cervical sagittal parameters in asymptomatic Chinese adults, and to explore the changes and compensating mechanisms across different age groups. Methods The asymptomatic subjects were divided into 6 groups according to age and then one-way analysis of variance was used to compare the multiple sets of cervical sagittal parameters among the different age groups. Independent t-tests were performed to compare the sagittal parameters among different gender and different cervical spine alignments. Relationships between each parameter were tested by Pearson’s correlation. Linear regression analysis based on T1 slope (T1S) and C2 slope (C2S) was used to provide an equation to predict normal cervical alignment. Results Mean values of each cervical sagittal parameter were presented based on age and gender. There were positive correlations between age and cervical lordosis (CL) (r = -.278, P < .001), T1S (r = .271, P < .001), cervical sagittal vertical axis (cSVA) (r = .218, P < .001), C2-C4 Cobb angle (r = -.283, P < .001), horacic inlet angle (TIA) (r = .443, P < .001), and neck tilt (NT) (r = .354, P < .001). Older groups (aged >50 years) had greater T1 Slope, C2S, and TIA. The C2-C4 Cobb angle maintained a steadily increasing trend and significantly increased in the older adult groups ( P < .05), while the C5-C7 Cobb angle was relatively constant. Mean values of parameters were larger in males ( P > .05). Linear regression analysis indicated a strong association between T1S and CL (R2 = .551, standard error 1.16°), T1S and C5-7 (R2 = .372; P < .001), and C2S and C2-4 (R2 = .309; P < .001). Conclusions Normative values of cervical sagittal parameters vary by age and sex. The CL, cSVA, and T1S, C2-4 Cobb angle changed with increasing age, and it can influence the recruitment of compensation mechanism. Normative CL of Chinese adults was predicted by the equation CL = T1S-14.7° ± 1.2°, which could serve as a reference when planning for cervical surgery.

Abstract:To establish the age- and sex-related normative values of sagittal alignment in asymptomatic Chinese adults, and to investigate the changes and possible associated compensation mechanisms across age groups. 584 asymptomatic Chinese adults aged 20-89 years were recruited. Subjects were grouped according to age and gender. Whole-body standing radiographs were acquired for evaluating sagittal alignment from spine to lower limb. Sagittal parameters between gender in different age groups were compared via independent t test. Pearson correlation analysis was used to demonstrate relationships between parameters. Thoracic kyphosis (TK) increased steadily while lumbar lordosis decreased gradually in both genders. Pelvic tilt (PT) in male is greater than in female across all age groups with age related gradual increase. There were significant differences between male and female from 20s to 60s in terms of knee flexion angle (KA) and ankle dorsiflexion angle (AA), but the differences were not significant after 60s. T1 pelvic angle (TPA) was significantly correlated with spinal, pelvic and lower-limb alignment. The older group (≥50 years) had a stronger correlation of TPA with PT and KA, whereas the younger (<50 years) had stronger correlation with TK. This study comprehensively presented the normative sagittal alignment based on a large asymptomatic population, which could serve as an age- and gender-specific reference value for spine surgeons when planning for correction surgery. Age can influence the recruitment of compensation mechanism that involve more pelvic and lower limb mechanisms for elderly people.

Abstract:Recent studies have suggested that a relationship exists between crash injury risk and occupant posture, particularly in postures different from those used with anthropomorphic test devices (ATDs) in crash testing. The objective of this study was to increase scientific understanding of typical front-seat passenger postures through a naturalistic study.Video cameras were installed in the passenger cabins of the vehicles of 75 drivers. Reflective targets were attached to the seats and the seat position and seat back angle was moved through their available ranges during instrumentation. The video data, along with vehicle acceleration and location data, were downloaded after the vehicles were operated as usual by their owners for two weeks. Video frames were manually coded to identify characteristics of front-seat passenger posture and position. Seat position and seat back angle were estimated using the calibration data obtained during vehicle instrumentation.Video frames from a total of 2733 trips were coded for 306 unique front-seat passengers. For these trips, a total of 13638 frames were coded; each frame represents about four minutes of travel time. The head was rotated left or right in 33% of frames, and the torso was rotated left or right about 10% of the time and pitched forward in almost 10% of frames. No seat position or seat back angle change was noted in 40 (53%) of vehicles and the distributions of seat position and seat back angle on arrival were essentially unchanged during travel. The seat was positioned full-rear on the seat track about 23% of the time and rearward of the mid-track position in 92% of frames. The mean seat back angle was 25.4 degrees (standard deviation 6.4 degrees); seat back angle was greater than 30 degrees in 15% of frames and greater than 35 degrees in less than 1% of frames.This study is the first to report distributions of postures, seat positions, and seat back angles for front-seat passengers. Seat positions rearward of the middle of the seat adjustment range are common, but highly reclined postures are infrequent. Non-nominal torso and head postures also are nontrivial.
