Abstract:Introduction: As a relatively new generalist specialty, hospitalists must acquire new competencies that may not have been taught during their training years. Continuing medical education (CME) has traditionally been a mechanism to meet training needs but often fails to apply adult learning principles and fulfill current demands. Methods: We developed an innovative 3-day course called the University of California, San Francisco Hospitalist Mini-College (UHMC) that brings adult learners to the bedside for small-group learning focused on content areas relevant to today's hospitalists. The program was built on a structure of 4 clinical domains and 2 clinical skills labs. Sessions about patient safety and immersion into traditional academic learning vehicles, such as morning report and a morbidity and mortality conference, were also included. Participants completed a precourse survey and a postcourse evaluation. Results: Over 5 years, 152 participants enrolled and completed the program; 91% completed the pre-UHMC survey and 89% completed the postcourse evaluation. Overall, participants rated the quality of the UHMC course highly (4.65; 1-5 scale). Ninety-eight percent of UHMC participants (n = 57) in 2011 to 2012 reported a "high" or "definite" likelihood to change practice, higher than the 78% reported by the 11,447 participants in other UCSF CME courses during the same time period. Discussion: The UHMC successfully brought participants to an academic health center for a participatory, hands-on, and small-group learning experience that was highly rated. A shift of CME from a hotel conference room to the bedside is feasible, valued by participants, and offers a new paradigm for how to maintain and improve hospitalist competencies.
A post-COVID syndrome curriculum for continuing medical education (CME): in-person versus livestream

Abstract:Background: Nearly 30% of patients with COVID-19 infection develop post-COVID Syndrome. Knowledge of post-COVID Syndrome is evolving, creating the need for adaptable curricula. Flipped classrooms (FC) are flexible and dynamic with demonstrated utility in continuing medical education (CME), yet there has been no research on application of FCs, or comparisons between livestream and in-person learning, in post-COVID CME. Methods: We implemented a novel post-COVID curriculum using FCs for in-person and livestream participants at four Mayo Clinic CME conferences. Outcomes were validated measures of knowledge; perceptions of FCs and CME teacher effectiveness; and learner engagement. Pre-conferences surveys were a post-COVID knowledge test and the Flipped Classroom Perception Inventory (FCPI). Post-conference surveys were a post-COVID knowledge test, the FCPI, the CME Teaching Effectiveness Instrument (CMETE), and the Learner Engagement Inventory (LEI). Pre-post knowledge and FCPI scores were analyzed using linear mixed models. CMETE and LEI were compared for in-person versus livestream participants using the Wilcoxon rank-sum test. Results: Overall, 59 participants completed the pre-test, and 72 participants completed the post-test, surveys. Participants were predominantly female (58%), were in nonacademic group practices (65%), and lacked prior experience with flipped classrooms (83%). Following the presentations, participants showed significant improvements in post-COVID knowledge (47% correct precourse to 54% correct postcourse, p-value = 0.004), and a trend toward improved FCPI scores. Teaching effectiveness, learner engagement, and pre-post change in COVID knowledge did not differ significantly between participants of in-person versus livestream sessions. Conclusion: This post-COVID FC curriculum was feasible and associated with improved knowledge scores among a diverse population of physician learners in CME, without any apparent compromise in learner engagement, or in perceptions of teaching effectiveness and FCs, among livestream versus in-person participants.
Evolving radiology continuing medical education: Tapping into the power of online learning

Abstract:Introduction: Global education, particularly Continuing Medical Education (CME) for healthcare professionals, is quickly shifting online. This study assesses the opportunities and challenges of adopting online learning in radiology CME. It explores how radiologists and radiographers have adapted to this digital shift and the changing landscape of radiology education. The study also seeks to envision an innovative future for radiology education. Methods: A descriptive cross-sectional survey was conducted among radiologists and radiographers working in radiology departments in the United Arab Emirates (UAE). The survey collected data on participant demographics, experiences with CME, sources of CME, and perceptions of online learning. Statistical analysis was performed using the Statistical Package for the Social Sciences (SPSS) software. Results: The survey involved 65 radiologists and 215 radiographers. Findings indicated a significant shift from face-to-face to online CME activities, with 76.9% of radiologists and 70.7% of radiographers utilizing online resources for CME. Concerns about time management, technical issues, and expenses have emerged as challenges for online CME. Participants also highlighted the importance of free-of-charge CME and the value of active participation and anonymity in online discussions. Conclusion: Radiology professionals have rapidly adapted to the changing landscape of CME by embracing online learning. While this shift offers greater flexibility and accessibility, technology-related challenges and concerns over time management persist. The study suggests that the future of radiology CME may involve personalized, adaptive, and interactive learning experiences, emphasizing mental well-being and resilience. Implications for practice: Radiology professionals must embrace online CME for continuous skill enhancement, addressing technical challenges, fostering interactive learning environments, and ensuring accessibility to maintain high standards in patient care and medical advancements.

Abstract:In recent years, wide ranging biomedical innovation has provided powerful new approaches for prevention, diagnosis and management of diseases. In order to translate such innovation into effective practice, physicians must frequently update their knowledge base and skills through continuing medical education and training. Medical Professional Societies, run as not-for-profit organizations led by peers, are uniquely placed to deliver balanced, disease oriented and patient centred education. The medical industry has a major role in the development of new, improved technology, devices and medication. In fact, the best innovations have been achieved through collaboration with scientists, clinical academics and practicing physicians. Industry has for many years been committed to ensure the optimal and safe application of its products by providing unrestricted support of medical education developed and delivered by international and national learned societies. Recently adopted Codes of Practice for the Pharmaceutical and Device industry were intended to enhance public trust in the relationship between biomedical industry and physicians. Unexpectedly, changes resulting from adoption of the Codes have limited the opportunity for unconditional industry support of balanced medical education in favour of a more direct involvement of industry in informing physicians about their products. We describe the need for continuing medical education in Cardiovascular Medicine in Europe, interaction between the medical profession and medical industry, and propose measures to safeguard the provision of high quality, balanced medical education.

Abstract:Continuing medical education (CME) and continuing education (CE) provide frameworks for assimilating and disseminating new advancements and are mainstays of clinicians' professional development and accreditation. However, traditional CME/CE approaches may be challenged in providing opportunities for integrated and interprofessional learning and helping clinicians effectively translate innovations into individual practice. This Commentary describes the reflective learning approach, including its integration into CME/CE and how it can support interprofessional education. Also identified are barriers to reflective and interprofessional learning implementation and CME/CE access. The Commentary provides insights based on point-of-care reflection data and outlines considerations in trialing the use of an artificial intelligence (AI)-driven digital platform for reflective learning. Further, the Commentary describes how the AI-driven digital platform may help overcome barriers to reflective learning and interprofessional education and support equitable CME/CE program access.

Abstract:The COVID-19 pandemic has significantly disrupted and transformed continuing education in the health professions to be reliant on digital learning modalities. This retrospective observational study of a large, international health system's continuing education programme compares educational activities offered, participation, and learning outcomes pre- and intra-pandemic to assess the impact of digitisation advanced because of the pandemic. There was a significant increase in internet-based activities that filled the gap of cancelled or postponed live, in-person activities to keep healthcare professionals up to date in their specialities and prepared to handle the clinical and hospital demands of the pandemic. Compared to live, in-person education, virtual activities were offered in shorter increments, reached a much larger amount of participants, and were equally effective in achieving learning outcomes. Questions remain regarding business model implications to generate adequate revenues to cover costs of virtual education. Additionally, there is a general inadequacy of digital learning environments to coalesce groups and meet social needs. Regardless, the efficiencies and effectiveness of digital modalities will be a primary method of teaching healthcare professionals going forward.

Abstract:Background: Consultation-liaison (C-L) psychiatry, similar to other medical specialties, relies on the education of students, residents, fellows, and life-long learners for growth of the field. C-L psychiatry is unique as it exists at the intersection of psychiatry with other medical subspecialties. Traditional teaching methods have been used in C-L psychiatry programs for more than 50 years, while technology has recently advanced as available resources and the learning styles of today's learners have evolved. A growing number of younger trainees are taking advantage of new ways to learn. Objectives: We sought to examine both traditional and novel teaching methodologies and how each of these educational methodologies fits within adult learning theory and in the context of how digital natives learn about C-L psychiatry. Methods: In this narrative review, we drew upon the experiences of the authors as both life-long learners and educators. We then reviewed the literature pertaining to teaching methods that have been used in C-L psychiatry as well as emerging methods that could potentially be used in C-L psychiatry. Results: C-L psychiatry has used traditional teaching methods such as readings, didactic lectures, case-based rounds, and problem-based learning. Novel teaching methodologies such as teaching rotations, simulations, social media, podcasts, movie clubs, and the use of mobile tablet computers have been used in general psychiatry and other medical specialties, while literature specific to C-L psychiatry was sparse. Conclusions: Opportunities abound to make use of new teaching methodologies and technologies to appeal to future generations of C-L psychiatrists.
Localization of a pigment-containing structure near the surface of Xenopus eggs which contracts in response to calcium.

Abstract:Contractions in surface structures of Xenopus eggs have been induced by application of the calcium ionophore A23187 or calcium ion. Local applications have shown that the contractile structure is present in both animal and vegetal hemispheres. It is, however, much stronger in the animal hemisphere and pigment embedded in it there defines the animal half. The injection of cytochalasin B (CB) into whole cells or the application of the antibiotic to half cells cannot prevent the induced contractions. By experimental means, the contraction of a deeper, pigment-containing structure can be uncoupled from a thin, more superficial and relatively pigment-free layer on the egg surface. By this means it has been possible to establish that the CB-resistant contraction is due, at least partially, to a structurally distinguishable layer subjacent to the outer egg cortex. Scanning and transmission electron microscopy demonstrate a dense grainy matrix near the egg surface in which pigment granules but little yolk are embedded. This structure is much thicker in the pigmented hemisphere. The presence of calcium ions in an isolation medium are shown to cause a loosening or dissolution of the structural connections between the dense, contractile structure near the surface and the cytoskeleton of the endoplasm.

Abstract:There are currently many online resources for medical education during residency and beyond in anesthesiology, intensive care, pain, emergency and palliative medicine. From traditional textbooks and in-person events to learning platforms, apps, podcasts, simulation training and even virtual reality, there are many ways to supplement traditional residency curricula and continuing medical education. The coronavirus disease 2019 (COVID-19) pandemic has been instrumental in making medical education content more accessible and, among other things, accelerate the transfer of knowledge.To include all colleagues in the goal of life-long learning using these modern tools, we recommend the development of a digital media concept that is individually tailored to each department of anesthesiology. First, the goals of the department should be defined, e.g., can existing teaching materials be made more digitally accessible for asynchronous learning? Then, department resources should be compiled, e.g., what learning platforms are already being used and if and how social media should play a role? One or more persons should be named responsible and maintain the new concept. In this context, it is essential to develop quality criteria to properly assess the digital content.With the support of the department, conventional teaching methods can be combined with new digital possibilities in residency education and beyond. In this way, individual shift models, various levels of participation in live teaching events and different types of learners can be taken into account. These diverse digital tools can enrich the training and further education of every team member in an anesthesiology department and will accompany us well into the future.

Fractures of the medial epicondyle must be anatomically reduced, open if necessary. A fibrous union of a minimally displaced fractured medial epicondyle may result in lengthening and functional compromise of the medial collateral ligament. Chronic elbow instability is an unusual lesion. Repair of chronic elbow instability is best performed by restitution of medial collateral ligament function.

Abstract:Research must investigate the three overlapping areas of continuing medical education: it must discover and document what, how, and why physicians learn and change; how better programs of education can be designed to fit into this process; and in what ways the organizations that provide CME can or should differ in their policies, procedures, and resources. In terms of physician learning, for example, the basic assumption must be that physicians are like other adults in the general ways they learn in voluntary settings, yet have specific differences from other professional groups and from their physician colleagues in other specialties. Making the changes necessary for informed medical practice depends on understanding the role of learning in changes--which in turn depends on CME research. But barriers hinder much-needed research: conducting CME research is seldom rewarded in medical centers; CME is usually not a highly valued component of the institution; funding sources are few and support is limited; and persistent debate over the relative merits of qualitative and quantitative methods diverts energy that should be going into productive, cooperative research. The urgency of needed changes in medical practice and organization may give the necessary impetus for interdisciplinary, collaborative CME research projects that will take a longer-term view of the effect of learning on physician change.

Abstract:Background: The Coronavirus Disease-2019 (COVID-19) pandemic accelerated digitalization in medical education. Continuing medical education (CME) as a substantial component of this system was relevantly affected. Here, we present the results of an online survey highlighting the impact on and the role of online CME. Methods: An online survey of 44 questions was completed by users of a German online CME platform receiving an invitation via newsletter. CME habits, requirements, personal perception, and impact of the pandemic were inquired. Standard statistical methods were applied. Results: A total of 2,961 responders took the survey with 2,949 completed surveys included in the final analysis. Most contributions originated from Germany, Austria, and Switzerland. Physicians accounted for 78.3% (57.5% hospital doctors) of responses followed by midwives (7.3%) and paramedics (5.7%). Participating physicians were mainly board-certified specialists (69%; 55.75% hospital specialists, 13.25% specialists in private practice). Frequent online lectures at regular intervals (77.8%) and combined face-to-face and online CME (55.9%) were favored. A duration of 1–2 h was found ideal (57.5%). Technical issues were less a major concern since the pandemic. Conclusion: A shift from face-to-face toward online CME events was expectedly detected since the outbreak. Online CME was accelerated and promoted by the pandemic. According to the perception of users, the CME system appears to have reacted adequately to meet their demand but does not replace human interaction.

Design reviews and decision-making using collaborative virtual reality prototypes : a case study of the large scale mk3 project

Abstract:LKAB, a large mining company in Sweden, is investing 290 million Euros in a new pelletising plant in Malmberget, Sweden (MK3). The complexity of the project, the number of actors involved and the desire to involve end users such as industrial workers responsible for the future plant operations in the design makes VR an excellent enriched source of communication in the design review process. This paper describes a practical approach to facilitate decision-making, coordination and to communicate client requirements in the design review process using a number of collaborative VR (Virtual Reality) prototypes of the plant including the construction and installations. The model based working methods that are used in the case study is discussed in the context of lean construction. The case study shows that the use of VR has increased the value for the client and the reliability in the design process. VR mock-ups have also minimised the waste in the production phase by eliminating collisions between the different designs. Even though the Partnering concept facilitates the cooperation between the different stakeholders the main cause for the intense information flow and willingness to share the information has been the time pressure forcing the different design teams to act concurrently.

Abstract:The diagnosis and management of hypertrophic cardiomyopathy (HCM) is challenging. HCM is often missed in patients, leading to potentially devastating consequences. Until recently, treatment options could only address disease symptomatology. However, the therapeutic landscape has recently evolved with the approval of a novel drug class, cardiac myosin inhibitors, that can target the underlying disease pathophysiology. Given the emergence of this novel treatment approach, there is a renewed need to improve clinicians’ understanding of how to optimally diagnose HCM and how to translate the latest data on novel therapies into practice. We developed a continued medical education (CME) program aimed at improving clinicians’ knowledge and competence related to understanding guideline-recommended methods for diagnosing HCM, describing clinical impact of cardiac myosin inhibitors, and identifying strategies for evidence-based care. On Nov 5, 2022, a 60-minute CME-certified, interactive symposium titled “Integrating Novel Cardiac Myosin Inhibitors into the Management of Hypertrophic Cardiomyopathy: Practical Strategies for Diagnosis and Treatment”, presented by two cardiologists with expertise in HCM, took place at the American Heart Association's Scientific Sessions. Learners could participate in three different ways- 1) in person, 2) by attending virtually, or 3) by watching the full audiovisual replay following the live event. Changes in knowledge and competence were measured using pre- and post-test learner analysis. Assessments were scored in aggregate and at the individual level to compare between specialty audiences. Practice change and patient impact were also measured. As of Nov 28, 2022, a total of 388 learners participated, with the leading learner specialties being internal medicine specialists (52%) and cardiologists (36%). Across specialties, baseline knowledge was lowest regarding risk assessment of patients with HCM and patient eligibility criteria for novel cardiac myosin inhibitors. A subset of learners (n=108) completed pre- and post-test assessments. Of these learners, post-education, 34% improved knowledge related to guideline-recommended diagnosis of HCM, 21% improved knowledge on the clinical profiles of cardiac myosin inhibitors and 33% showed improved competence on incorporating these novel therapies into practice. Of the learners, 81% expressed their intention to make changes to their practice based on the activity. Based on learners’ self-report of number of patients seen per week, this program potentially impacted care of 822 patients. More than 50% of the learners also expressed interest in learning about adverse event management associated with cardiac myosin inhibitors and receiving further education via case examples related to HCM management. Our findings showed that hybrid CME was effective in improving knowledge and competence among clinicians related to the diagnosis and treatment of HCM. Our results also imply that there is an ongoing need and interest among clinicians to learn more about these topics.

Abstract:The mission statement of the European Society of Cardiology (ESC) is "to reduce the burden of cardiovascular disease". The ESC is the leading scientific society for cardiovascular health care professionals across Europe and increasingly the world. Recognising the need for democratisation of education in cardiology, the ESC has for many years embraced the digital world within its education programme. As in all areas of medicine, the COVID-19 pandemic required an agile response to be able to continue to provide not only a digital congress but also education, training and assessment in an almost totally digital world. In this paper we will describe the digital learning activities of the ESC, the successes and the challenges of the transformation that has taken place in the last 18 months as well as an overview of the vision for education, training and assessment in the post-COVID digital era. We understand the need to provide a portfolio of educational styles to suit a diverse range of learners. It is clear that digital CME provides opportunities but it is likely that it will not entirely replace in-person learning. Abstract:Healthcare professionals need to maintain their knowledge and skills to deliver the best possible care to patients. Medical societies play an important role as providers of continuing medical education (CME) and have actively continued this role during the COVID-19 pandemic adapting the delivery of education to virtual meetings and courses. The Biomedical Alliance in Europe CME Experts Committee conducted two surveys to collect information on the delivery of CME, generally, and during the COVID-19 pandemic from the member medical societies. In this article, we will present the most relevant data collected and share some reflections based on this analysis.
