Abstract:Objective:To investigate the diagnosis and treatment for intra-abdominal fistula in China, and to explore the prognostic factors.Methods:A multi-center cross-sectional study was conducted based on the Registration System of Chinese Gastrointestinal Fistula and Intra-Abdominal Infections to collect the clinical data of patients with intra-abdominal fistula from 18 medical centers from January 1, 2018 to December 31, 2018, including basic information, medical records and prognosis.Results:A total of 106 patients were enrolled in this study, including 57 males and 49 females, with an average age of (48.0±17.8)years. The most common type of intra-abdominal fistula was entero-vesical fistula (34.0%), followed by entero-vaginal fistula (31.1%), entero-enteric fistula (26.4%) and multiple fistula (8.5%). The direct causes of intra-abdominal fistula were mainly surgical operation (66.0%), followed by spontaneous fistula due to Crohn′s disease (18.9%), radiation intestinal injury (11.3%), and 4 cases (3.8%) of unknown reasons. During the whole treatment, 95 patients received nutritional support therapy, mainly EN+ PN (75.8%). Finally, 86 patients (81.1%) received surgical treatment, with a healing rate of 95.3%. After surgery, 8.1% of patients developed surgical site infections (SSI), and 10.5% had a relapse of fistula. 20 patients (18.9%) were treated conservatively, with a self-healing rate of 80.0%. The overall mortality rate was 8.5%, and the highest mortality (15.2%) was found in entero-enteric fistula. Statistical analysis showed that the age ( t=-4.664, P<0.001), leucocyte level ( U=663.000, P=0.010), sepsis ( P=0.002) and multiple organ dysfunction syndrome (MODS) ( P=0.019) were higher in the death group than those in the healing group. Multivariate analysis suggested that advanced age ( OR=1.073, 95% CI: 1.008-1.141, P=0.026) and complications of sepsis ( OR=11.806, 95% CI: 1.064-131.048, P=0.044) were independent risk factors of the death for patients with intra-abdominal fistula. Conclusions:The overall mortality rate of intra-abdominal fistula is still high, and malignant tumor is the most common primary disease. Advanced age and sepsis are independent risk factors for death in patients with intra-abdominal fistula.
Early Active Irrigation-Suction Drainage among Enterocutaneous Fistulas Patients with Chronic Critical Illness: A Retrospective Cohort Study

Abstract:Enterocutaneous fistulas (ECFs) requiring admission to ICU is a serious surgical complication. A growing number of patients survive ECFs but remain chronically critically ill. The aim of our study was to investigate the risk factors of hospital death in patients with chronic critical illness attributed to ECFs. A retrospective single-center study was conducted in 163 ECF patients between 2013 and 2017. Patient-specific baseline characteristics, outcomes, and process of care variables were collected. Risk factors for hospital mortality were determined using univariate and multi-variate analyses. Patients were divided into the following two groups according to the hospital discharge outcome: group survivors (n = 106) and group nonsurvivors (n = 57). Patients who received active irrigation-suction drainage (AISD) within 24 hours after the diagnosis of ECFs had a significantly lower hospital mortality rate than those who received AISD after more than 24 hours (17.9% vs 46.9%, P < 0.001). Multivariate logistic regression analysis demonstrated that delayed AISD (adjusted odds ratio [AOR], 10.24; 95% confidence interval [CI], 3.03234.59; P < 0.001) and no rehabilitation therapy (AOR, 4.77; 95% CI, 1.43215.98; P = 0.011) were independently associated with a greater risk of hospital mortality. The hospital mortality rate in patients with more than or equal to four risk factors was 92.6 per cent (n = 57), compared with a mortality rate of 9.4 per cent (n = 106) in patients who did not have these risk factors ( P < 0.001). The risk of hospital death is exceptionally high among patients with chronic critical illness attributed to ECFs. Efforts aimed at early AISD and rehabilitation therapy are likely to be associated with improved clinical outcomes.
Risk factor and outcome for intra-abdominal bleeding in patients with enterocutaneous fistula.

Abstract:Intra-abdominal bleeding is a major and lethal complication in patients with enterocutaneous fistula (ECF) while few studies are involved in this aspect. In the present study, we aimed to investigate the risk factors and assessed the outcome for intra-abdominal bleeding in patients with ECF. A retrospective study was performed from October 2013 to October 2015. Medical records of 67 ECF patients with intra-abdominal bleeding and 134 ECF patients without intra-abdominal bleeding matched as controls were reviewed and analyzed. Logistic regression was performed to evaluate the risk factor of intra-abdominal bleeding in patients with ECF. Outcomes such as mortality, hospital durations, and cost were compared between ECF patients with or without intra-abdominal bleeding. A total of 67 ECF patients suffered intra-abdominal bleeding during hospitalization. In the logistic regression, duodenum fistula (odds ratio [OR]: 3.899, 95% confidence interval [CI]: 1.204-12.634, P=0.023), hemorrhage history (OR: 15.846 95% CI: 5.779-43.448, P<0.001), and acute kidney injury (OR: 6.350 95% CI: 2.033-19.836, P=0.001) were independent risk factors for intra-abdominal bleeding in patients with ECF. In addition, the bleeding patients showed a significantly higher mortality rate, prolonged total parenteral nutrition days, hospital and intensive care unit durations, more complications, and increased cost. We revealed the 3 risk factors for intra-abdominal bleeding among ECF patients. Besides, intra-abdominal hemorrhage was associated with a poor prognosis in patients with ECF.
A population‐based study of incidence, aetiology, treatment and outcome of enterocutaneous fistula

Abstract:Aim The aim of this work was to investigate the incidence of enterocutaneous fistula (ECF), including both small bowel and colonic fistulas, in a defined population of 1.04 million during a 10‐year period and to describe aetiology, treatments, care consumption and outcome. Method A comprehensive search algorithm including diagnostic and procedural codes, enterostomal therapy nurse notes and in‐hospital care for >60 days yielded 1970 search hits. After reviewing medical records, 187 patients with ECF were identified. Results The annual incidence of ECF was 2.3 per 100 000, the incidence of ECF with intestinal failure type II was 0.9 per 100 000. Spontaneous closure of the fistula occurred in 16.0% of patients, while closure was seen in 97.3% of patients who underwent reconstructive surgery with recurrences in 6.7% and 8.3%, respectively. Cumulative ECF‐related in‐hospital care until closure or end of follow‐up was 4 (range 0–61) weeks. Eighty‐eight patients (47%) received home‐based healthcare including parenteral feeding and/or fistula wound care. The estimated overall mortality at 1, 3 and 5 years was 33.7%, 42.1% and 47.6% respectively. Mortality was mainly in patients without spontaneous closure or reconstructive surgery, and the risk of ECF‐related death was 30.2%. Conclusion This study defines the population‐based incidence of ECF and reports a high overall mortality rate. Initial survivors were characterized by either spontaneous closure or eligibility for later reconstructive surgery, but with an eventual mortality rate of approximately 20%. ECF patients are high consumers of care: 55.1% needed ≥4 weeks in hospital and many received home‐based healthcare.

Abstract:Mating elicits two postmating responses in many insect females: the egg laying rate increases and sexual receptivity is reduced. In Drosophila melanogaster, two peptides of the male genital tract, sex-peptide and DUP99B, elicit these postmating responses when injected into virgin females. Here we show that the gene encoding DUP99B is expressed in the male ejaculatory duct and in the cardia of both sexes. The DUP99B that is synthesized in the ejaculatory duct is transferred, during mating, into the female genital tract. Expression of the gene is first seen in a late pupal stage. Males containing an intact ejaculatory duct, but lacking accessory glands, initiate the two postmating responses in their female partners [Xue, L. & Noll, M. (2000) Proc. Natl Acad. Sci. USA97, 3272-3275]. Although such males synthesize DUP99B in wild-type quantities, they elicit only weak postmating responses in their mating partners. Males lacking the Dup99B gene elicit the two postmating responses to the same extent as wild-type males. These results suggest that both sex-peptide and DUP99B can elicit both responses in vivo. However, sex-peptide seems to play the major role in eliciting the postmating responses, while DUP99B may have specialized for other, as yet unknown, functions.

Abstract:BACKGROUND Enterocutaneous fistula (ECF) is an abnormal connection between the gastrointestinal tract and the skin. ECF can lead to massive body fluid loss, hypercatabolism, and malnutrition. Therefore, nutritional support plays a crucial role in managing ECFs and promoting the healing of fistulas. For nutritional support, enteral nutrition (EN) is the preferred method when gastrointestinal function is recovering. Currently, various EN approaches have been applied for different anatomical positions of the ECF. However, the effectiveness of administering EN support for treating lower ECFs still needs further exploration and improvement. CASE SUMMARY We present the case of a 46-year-old male who underwent gastrointestinal stromal tumour resection. Six days after the surgery, the patient presented with fever, fatigue, severe upper abdominal pain, and septic shock. Subsequently, lower ECFs were diagnosed through laboratory and imaging examinations. In addition to symptomatic treatment for homeostasis, total parenteral nutrition support was administered in the first 72 h due to dysfunction of the intestine. After that, we gradually provided EN support through the intestinal obstruction catheter in consideration of the specific anatomic position of the fistula instead of using the nasal jejunal tube. Ultimately, the patient could receive optimal EN support via the catheter, and no complications were found during the treatment. CONCLUSION Nutritional support is a crucial element in ECF management, and intestinal obstruction catheters could be used for early EN administration.

Management of Enterocutaneous Fistulas: 30-Year Clinical Experience

Abstract:Objectives To explore successful models of management of enterocutaneous fistulas and unresolved problems requiring further study.Method Analysis of therapeutic results of 1168 cases treated in one center from January 1971 to December 2000.Results In this group of patients, the recovery rate was 93% and 37% of fistulas healed spontaneously after non-operative treatment. The mortality rate was 5.5%, most of which occurred due to sepsis. Of 659 cases receiving definitive operations for enteric fistula, 98% recovered. Recovery, mortality and operational success rates (94.2%, 4.4%, 99.7%) of cases treated between January 1985 and December 2000 were, significantly better than those (90.4%, 8.2%, 95.5%) of cases treated earlier (January 1971-December 1984) ( P < 0.05).Conclusions The results from this study were better overall than those reported in previous literatures. The change in therapeutic strategy: improved technique in control of sepsis, rational nutritional support and careful monitoring of vital organs are the key reasons for improvement of managing enteric fistulas. However, increasing spontaneous closure of fistula, improving the therapeutic rate of specific enteric fistula (IBD or radiation enteritis) and performing definitive operations for enteric fistula at early stages are still problematic and require further study.

Abstract:OBJECTIVES:To evaluate the essentials for successful management of entero-cutaneous fistulas and their.METHODS:We analyzed. The therapeutic results of 1 168 cases treated in a single center from January. 1971 to December. 2000.RESULTS:In these patients, the cure rate of fistulas was 93% and 37.1% after non-operative treatment. Most cases died of sepsis (60/65 cases, 93.2%) with a total mortality of 5.5%. In 659 cases who had undergone operation for enteric fistula, 647 (98.2%) recovered. The cure rate, mortality rate and successful rate of operation (94.2%, 4.4%, 99.7%) in the period of January. 1985 - December. 2000 were better than those (90.4%, 8.2%, 95, 5%) in the early period of January 1971 December 1984 (P < 0.05).CONCLUSIONS:The change of therapeutic strategy, improved technique in control of sepsis, rational nutritional support and careful monitoring of vital organs are essential to the management of enteric fistulas. How to increase the spontaneous closure of fistula, the therapeutic rate of specific enteric fistula (e.g. IBD, radiation enteritis) and the operative rate of enteric fistula in the early period requires further study.
Risk factors and prognosis for intra-abdominal bleeding in patients with enterocutaneous fistula

Abstract:Objective To investigate the risk factors for intra-abdominal bleeding in patients with enterocutaneous fistula and assessed the prognosis of these patients. Methods The clinical data of 201 cases of enterocutaneous fistula patients admitted from October 2013 to October 2015 in Jinling Hospital, Medical School of Nanjing University, were analyzed retrospectively. The patients were classified as bleeding group(67 cases) and non-bleeding group(134 group), risk factors for intra-abdominal bleeding and prognosis of patients were analyzed. Results In the multivariate logistic regression analysis, duodenum fistula (OR 3.348, 95%CI 1.215~9.231, P=0.019), hemorrhage history (OR 10.698,95%CI 4.447~25.737, P<0.001) and acute kidney injury (OR 6.090 95%CI 2.408~15.404, P<0.001) were independent risk factors for intra-abdominal bleeding. In addition, compared with the non-bleeding group, bleeding group showed a significantly higher mortality rate, more prolonged total parenteral nutrition days, hospital and intensive care unit durations and cost, even more complications. Conclusion In duodenum fistula, hemorrhage history and acute kidney injury were independent risk factors for intra-abdominal bleeding. Intra-abdominal bleeding in patients with enterocutaneous fistul is under dangerous condition, the death rate is high, and associated with a poor prognosis.
Early definitive surgery in the management of enterocutaneous fistulas

Abstract:Objective　To study the possibility and effect of early definitive surgery in enterocutaneous fistula (EF) patients with intra-abdominal sepsis. Early definitive surgery was defined as early resection of EF and reanastomosis of the bowel within 14 days after operation or injury caused EF.　Methods　26 patients with EF comprised 18 males (mean age 31.5±16.5 years) and 8 females (mean 38.4±22.4 years). Enterocutaneous fistulas included 2 from the duodenum, 18 from the small bowel, 4 from the colon, and 5 from the ileocolic anastomotic dehiscence. Among them, 2 were complicated by biliary fistula, one patient had pancreatic fistula and two had multiple orifices fistulas. Etiological factors of EF were severe pancreatitis (2 patients), abdominal trauma (11), anastomotic dehiscence of the small bowel (10), and injuries in gynecological procedures (3). All patients underwent resection of EF, anastomosis by using stapling devices and copious peritoneal lavage. Recombinant growth hormone (rhGH, 0.16 U*kg－1*d－1) was given within 3 days after operation, and all the patients had prophylactic antibiotic coverage.　Results　No death was observed in these patients and the morbility reduced significantly after early definitive surgery. Complications included immediate postoperative anastomotic leakage cured spontaneously within three days (3 patients) and acute respiratory insufficiency that needed （3.0±2.4）days of mechanical ventilation (18). The time between definitive surgery and previous operation or trauma induced EF was (8.9±4.6) days. Enteral feeding and total oral feeding were restored after (9.7±3.1) and (18.7±8.1)days accordingly after reoperation. The average length of rhGH treatment was (8.9±2.8)days.　Conclusions　With improved surgical techniques, nutrition support, organ function support, appropriate antibiotics use and rhGH treatment, early resection-anastomosis can be performed safely and effectively for EF patients even with severe intra-abdominal sepsis.

Abstract:BackgroundAbout 75%–85% of enterocutaneous fistulas are postoperative. Such fistulas are still disastrous and frustrating complication for surgeon and the patient and their management remains one of major challenge and dilemma in general surgical practice. Strict adherence to treatment guideline according to SOWATS protocol could results in better outcomes.The objectiveof this study was to assess the management outcomes of 23 patients with postoperative enterocutaneous fistulas.MethodsA total of 23 consecutive patients with postoperative enterocutaneous fistulas during the period of study were included. These fistulas were classified anatomically and physiologically. The predictive factors for spontaneous closure, management outcomes and mortality rate factors for were studied.ResultsOverall Closure of the fistulas was accomplished in 16 patients (69.6%). Spontaneous closure after successful conservative management was seen in 4 patients (17.4%). The average time between fistula development and spontaneous closure was 32 days (range12–66 days). Of 19 patients underwent corrective surgery, closure was achieved in 12 patients (52.2%), thus the surgical success rate was 63.2%. Surgical intervention was performed after an average period of 28 days (range 18–42 days) from diagnosis of the fistula. Five patients with high output (21.7%) died because of sepsis, severe malnutrition and organ failure.Conclusionadherence to standardized protocol and multidisciplinary approach of patients with enterocutaneous fistulas could results in good outcomes. A reasonable period of conservative treatment is always required to optimize patient's general conditions and for spontaneous closure of fistula. Operative correction is usually required for proximal high output and complex fistulas. The complexity of the fistulas, sepsis, high output effluent and comorbidities are the main factors affecting healing rates and fistula related mortality.
Major Risk Factors of Mortality in Adult and Elderly Patients Emergently Admitted for Intestinal Fistulas, Excluding the Rectum and Anus

Abstract:Introduction: In the United States, intestinal fistulas accounts for $500 million (USD) of healthcare expenditures and 28,000 admissions annually. They are also associated with significant morbidity and mortality. Despite the high prevalence of intestinal fistulas, risk factors of mortality have yet to be fully elucidated. The aim of this study was to identify risk factors of mortality in emergently admitted patients with fistulas of the intestine, excluding the rectum and anus. Materials and methods: Adult and elderly patients emergently admitted with intestinal fistulas, between 2004-2014 were investigated using the National Inpatient Sample Database, ICD-9-CM code 569.81. Clinical outcomes, therapeutic management, demographics, and comorbidities were collected. Associations between mortality and all other variables were established via univariable and multivariable logistic regression models. The final multivariable regression model elucidated the odds ratios (95% confidence interval, p-value) of pertinent mortality risk factors. Results: A total of 7,377 patients were included, of which the average adult and elderly ages were 48.9 and 74.6 years, respectively. Of these patients, 4,241 (57.5%) were female and 3,136 (42.5%) were male. Elderly patients demonstrated a higher mortality rate than adult patients-4.5% and 1.7%, respectively. In the adult group, the odds ratio for mortality was 1.020 for hospital length of stay in days (95% CI: 1.015-1.026, p<0.001), 1.035 for age (95% CI: 1.011-1.060, p=0.004), and 1.033 for days to the first procedure (95% CI: 1.021-1.044, p<0.001), among others. For the elderly group, the odds ratio for mortality was 1.012 for hospital length of stay in days (95% CI: 1.005-1.019, p=0.001), 1.075 for age (95% CI: 1.050-1.101, p<0.001), and 1.026 for days to the first procedure (95% CI: 1.009-1.043, p=0.002), among others. Conclusion: In adult and elderly patients emergently admitted for intestinal fistulas, multiple comorbidities were risk factors for in-hospital mortality. In the elderly cohort, increased age and increased days to operation were additional risk factors for in-hospital mortality.

Abstract:Background: Prevalence and associated risk factors for pressure ulcers (PU) vary in different body areas and diseases. Few studies have focused on PU in patients with enterocutaneous fistula (ECF). The aim of the present study was to investigate the prevalence and risk factors for PU in patients with ECF.Material/Methods: From January 2016 to June 2016, medical records of 140 patients with ECF who were transferred to the Enterocutaneous Fistula Treatment Center, Jinling Hospital, were reviewed and analyzed. The prevalence of PU was investigated. To evaluate the risk factors for PU in patients with ECF, 5 patients with PU before admission were excluded, and the remaining 135 patients were divided into 2 groups: the PU group and the non-PU group. The risk factors for PU were confirmed by multivariate logistic regression analysis of characteristics on admission.Results: There were 42 cases with PU (5 cases with PU before admission, 37 cases with PU in the treatment after ad- mission), and the prevalence of PU in patients with ECF was 30%. In addition, Braden risk score <19 (OR=9.33, CI: 2.80-31.08, p<0.001); underweight (BMI<18.5) (OR=5.21, CI: 1.65-16.39, p=0.005); onset of duodenal fistula (OR=4.86, CI: 1.33-17.78, p=0.017); diabetes (OR=4.95, CI: 1.03-23.85, p=0.046); and APACHE II score (OR=1.34, CI: 1.04-1.72, p=0.019) were associated with PU.Conclusions: The PU prevalence was 30% in patients with ECF. Braden risk score <19, underweight, onset of duodenal fistula, diabetes, and APACHE II score were risk factors for PU in patients with ECF.

Patient demographics, preoperative optimization, surgical management, postoperative complications, and follow-up information were collected from a prospectively maintained database.ResultsA total of 74 eligible patients were identified. The median interval between CD diagnosis and EUF diagnosis was 2 (0.08–6.29) years. Patients with EUF presented with pneumaturia (75.68%), urinary tract infections (72.97%), fecaluria (66.22%), and hematuria (6.76%). Fistulae originated most commonly from the ileum (63.51%), followed by the colon (14.86%), the rectum (9.46%), the cecum (2.70%), and multiple sites (9.46%). The EUF symptoms, weight, nutritional status, laboratory results were significantly improved after preoperative optimization. The absence of EUF symptoms was observed in 42 patients after the optimization and only 9 of which required bladder repair. However, 19 of 32 patients whose symptoms did not resolve required bladder repair (P = 0.001). Only 1 patient developed a bladder leakage in the early postoperative period and 3 patients experienced recurrent bladder fistula.ConclusionsSurgical management of EUF complicating CD is effective and safe, with a low rate of postoperative complication and EUF recurrence. Preoperative optimization, which is associated with the resolution of urinary symptoms and improved surgical outcomes, should be recommended.

Abstract:Background/objectives: Exclusive enteral nutrition (EEN) has been shown to be effective in the management of Crohn’s disease (CD). However, few experiences have been reported regarding its role in postoperative intra-abdominal septic complications (IASCs) after bowel resections for enterocutaneous fistulas (ECFs). Our aim was to investigate the influence of preoperative 3-month EEN on the incidence of IASCs and to clarify the risk factors of IASCs in fistulizing CD. Subjects/methods: A retrospective study on 123 CD patients suffering from ECFs was conducted from February 2001 to April 2011. Fifty-five patients (44.7%) received preoperative 3-month EEN. The changes in serum albumin and C-reactive protein (CRP) were compared. Perioperative data were analyzed using logistic regression to identify the independent risk factors affecting the incidence of postoperative IASCs. Results: Patients were similar in gender, age, fistula conditions and perioperative medications in the EEN and non-EEN groups. The EEN group had a significantly higher serum albumin level and lower CRP at operation, and suffered a lower risk of IASCs (3.6% vs 17.6%, P <0.05). Two years after operation when follow-up ended, the two groups had comparable cumulative risk of IASCs ( P =0.109). A logistic regression analysis identified age at operation and preoperative EEN as independent risk factors of postoperative IASCs. Conclusions: Preoperative EEN reduced the risk of postoperative IASCs after operation for ECFs in CD. In addition, age at operation may be another factor of influence.

Abstract:Crohn disease (CD) is a chronic nonspecific inflammatory disease of the bowel. Despite being a major treatment modality, surgery for CD is associated with more numerous and severe postoperative complications (anastomotic fistula, intestinal fistula, multisite infection, bleeding, and short bowel syndrome) than that for other gastrointestinal diseases. These complications affect the quality of life of patients, prolong the duration of hospitalization, and lead to reoperation. [ 1 ] Among them, intestinal fistula is a serious complication and a key factor leading to surgical failure that can be life-threatening if not treated properly. Thus, we retrospectively analyzed clinical data from patients with CD admitted for surgical treatment to investigate which preoperative clinical characteristics of patients were predictive of postoperative intestinal fistula. In addition, the risk factors for postoperative intestinal fistula in patients with CD were analyzed by univariate and multivariate logistic regression. A nomogram prediction model was also constructed to facilitate aggressive intervention against high-risk factors to reduce the incidence of postoperative intestinal fistula in patients with CD. This can enable patients to obtain greater benefits from treatment and provide a reference for predicting postoperative intestinal fistula.

Abstract:The management of enterocutaneous fistulas (ECF) can be challenging because of massive fluid loss, which can lead to electrolyte imbalance, severe dehydration, malnutrition and sepsis. Nutritional support plays a key role in the management and successful closure of ECF. The principle of nutritional support for patients with ECF should be giving enteral nutrition (EN) priority, supplemented by parenteral nutrition if necessary. Although total parenteral nutrition (TPN) may be indicated, use of enteral feeding should be advocated as early as possible if patients are tolerant to it, which can protect gut mucosal barrier and prevent bacterial translocation. A variety of methods of enteral nutrition have been developed such as fistuloclysis and relay perfusion. ECF can also be occluded by special devices and then EN can be implemented, including fibrin glue application, Over-The-Scope Clip placement and three-dimensional (3D)-printed patient-personalized fistula stent implantation. However, those above should not be conducted in acute fistulas, because tissues are edematous and perforation could easily occur.

Abstract:Background: Necrotizing soft tissue infections (NSTIs) is severe surgical infections which can occur following trauma or abdominal surgery. NSTIs secondary to gastrointestinal (GI) fistula is a rare but severe complication. Methods: A retrospective cohort study was performed on all subjects presenting with GI fistulas associated NSTIs were included. Clinical characteristics, microbiological profile, operations performed, and outcomes of patients were analyzed. Results: Between 2014 and 2017, 39 patients were finally enrolled. The mean age were 46.9 years and male were the dominant. For the etiology of fistula, 25 (64.1%) of the patients was due to trauma. Overall, in-hospital death occurred in 15 (38.5%) patients. Microbiologic findings were obtained from 31 patients and Klebsiella pneumoniae was the most common species (41.0%). Eight patients were treated with an open abdomen; negative pressure wound therapy was used in 33 patients and only 2 patients received hyperbaric oxygen therapy. Younger age and delayed abdominal wall reconstruction repair were more common in trauma than in non-trauma. Non-survivors had higher APACHE II score, less source control< 48 h and lower platelet count on admission than survivors. Multiple organ dysfunction syndrome, multidrug-resistant organisms and source control failure were the main cause of in-hospital mortality. Conclusions: Trauma is the main cause of GI fistulas associated NSTIs. Sepsis continues to be the most important factor related to mortality. Our data may assist providing enlightenment for quality improvement in these special populations.
Predictors of Response to Enteral Nutrition in Abdominal Enterocutaneous Fistula Patients with Crohn's Disease

Abstract:Background/Objectives: Enterocutaneous fistula (ECF) is a serious complication of Crohn’s disease (CD). Enteral nutrition (EN) is believed as one of therapeutic strategies of CD. This study is dedicated to identify predictors of response to EN in CD, which may lead to a better selection of fistula patients for this therapy. Subjects/Methods: Forty-eight CD patients with ECF treated with short-peptide-based EN for 3 months were included in this study. All patients were followed up for at least 6 months. Logistic regression was performed to investigate the potential predictors of response to EN in these patients. Results: In total, 30 out of 48 patients were confirmed with a successful closure of fistula after 3 months’ EN therapy. The average closure time was 32.4±8.85 days. Inflammatory parameters (erythrocyte sedimentation rate, C-reactive protein (CRP) and platelet count) improved significantly after EN therapy in all enrolled patients. Specifically, the improvement of CRP after therapy in closed group was more important compared with that in unclosed group ( P =0.035). Nutrition status (body weight, body mass index (BMI), hemoglobin, serum albumin (ALB), serum prealbumin and total protein (TP)) improved as well ( P <0.05). Similarly, after treatment, the improvement of serum albumin ( P =0.046) and prealbumin ( P =0.006) in closed group was much more important than those in unclosed group. Logistic regression analysis discovered that a decreased CRP level and an elevated BMI level would be beneficial to the response to EN in CD patients with ECF. Conclusions: In CD patients with ECF, lower CRP and higher BMI are associated with higher possibility of closure after EN treatment. EN therapy can lead to a closure of ECF in a certain proportion of patients. EN therapy could also ameliorate inflammatory condition and improve nutrition status.
