Abstract:Magnesium phosphate-based bone cements, particularly struvite (MgNH4PO4∙6H2O)-forming cements, have attracted increased scientific interest in recent years because they exhibit similar biocompatibility to hydroxyapatite while degrading much more rapidly in vivo. However, other magnesium-based minerals which might be promising are, to date, little studied. Therefore, in this study, we investigated three magnesium-based bone cements: a magnesium oxychloride cement (Mg3(OH)5Cl∙4H2O), an amorphous magnesium phosphate cement based on Mg3(PO4)2, MgO, and NaH2PO4, and a newberyite cement (MgHPO4·3H2O). Because it is not sufficiently clear from the literature to what extent these cements are suitable for clinical use, all of them were characterized and optimized regarding setting time, setting temperature, compressive strength and passive degradation in phosphate-buffered saline. Because the in vitro properties of the newberyite cement were most promising, it was orthotopically implanted into a partially weight-bearing tibial bone defect in sheep. The cement exhibited excellent biocompatibility and degraded more rapidly compared to a hydroxyapatite reference cement; after 4 months, 18% of the cement was degraded. We conclude that the newberyite cement was the most promising candidate of the investigated cements and has clear advantages over calcium phosphate cements, especially in terms of setting time and degradation behavior.

Abstract:In the search for more ideal bone graft materials for clinical application, the investigation into ceramic bone cements or bone void filler is ongoing. Calcium phosphate-based materials have been widely explored and implemented for medical use in bone defect repair. Such materials are an excellent choice because the implant mimics the natural chemistry of mineralized bone matrix and in injectable cement form, can be implemented with relative ease. However, of the available calcium phosphate cements, none fully meet the ideal standard, displaying low strengths and acidic setting reactions or slow setting times, and are often very slow to resorb in vivo. The study of magnesium phosphates for bone cements is a relatively new field compared to traditional calcium phosphate bone cements. Although reports are more limited, preliminary studies have shown that magnesium phosphate cements (MPC) may be a strong alternative to calcium phosphates for certain applications. The goal of the present publication is to review the history and achievements of magnesium phosphate-based cements or bone void fillers to date, assess how these cements compare with calcium phosphate competitors and to analyze the future directions and outlook for the research, development, and clinical implementation of these cements.
Ready-To-Use and Rapidly Biodegradable Magnesium Phosphate Bone Cement: In Vivo Evaluation in Sheep

Abstract:In clinical practice, hydroxyapatite (HA) cements for bone defect treatment are frequently prepared by mixing a powder component and a liquid component shortly before implantation in the operation theater, which is time-consuming and error-prone. In addition, HA cements are only slightly resorbed, that is, cement residues can still be found in the bone years after implantation. Here, these challenges are addressed by a prefabricated magnesium phosphate cement paste based on glycerol, which is ready-to-use and can be directly applied during surgery. By using a trimodal particle size distribution (PSD), the paste is readily injectable and exhibits a compressive strength of 9-14 MPa after setting. Struvite (MgNH4 PO4 ·6H2 O), dittmarite (MgNH4 PO4 ·H2 O), farringtonite (Mg3 (PO4 )2 ), and newberyite (MgHPO4 ·3H2 O) are the mineral phases present in the set cement. The paste developed here features a promising degradation of 37% after four months in an ovine implantation model, with 25% of the implant area being newly formed bone. It is concluded that the novel prefabricated paste improves application during surgery, has a suitable degradation rate, and supports bone regeneration.
Influence of Monocalcium Phosphate on the Properties of Bioactive Magnesium Phosphate Bone Cement for Bone Regeneration

Abstract:Bone defects occurring for various reasons can lead to deformities and dysfunctions of the human body. Considering the need for clinical applications, it is essential for bone regeneration to exploit a scaffold with bioactive bone cement. In this study, we fabricated bioactive magnesium phosphate bone cement (BMPC) at room temperature; then, it was set at to °C and 100% humidity for 2 h. The process was as follows: Simulating a clinical environment, magnesium oxide (MgO) was formed by calcining basic magnesium carbonate (Mg2(OH)2CO3). MgO, potassium dihydrogen phosphate (KH2PO4) and carboxymethyl chitosan (C20H37N3O14, CMC) were mixed to form magnesium phosphate bone cement (MPC); then, monocalcium phosphate (Ca(H2PO4)2) was added to neutralize the alkaline product after MPC hydration to fabricate bioactive magnesium phosphate bone cement (BMPC). The influence of the doped content of Ca(H2PO4)2 on the properties of bone cement was discussed. The results showed that Ca(H2PO4)2 and CMC can adjust the setting time of bone cement to between 8 and 25 min. The compressive strength increased first and then decreased. After 48 h without additional pressure, the compressive strength reached the maximum value, which was about 38.6 MPa. Ca(H2PO4)2 and CMC can play a synergistic role in regulating the properties of BMPC. The BMPC was degradable in the simulated body fluid (SBF). The results of the cytotoxicity experiment and laser confocal microscopy experiment indicated that BMPC fabricated at room temperature had better biocompatibility and degradability, which was more consistent with clinical operation requirements. BMPC is a promising orthopedic material and is suitable for repairing bone defects.

Abstract:Objective To investigate the effects of magnesium phosphate cement (MPC) on biological fixation intensity and histological change in rabbits. Methods ① Biomechanics:the MPC or calcium phosphate cement (CPC) was implanted into the intercondylar region of bilateral femurs in 36 rabbits, then the push-out tests were made at 1, 2, 3, 4, 6, 8 weeks after the procedure. ② Histology: MPC was implanted into the condylar region of the right femur in 32 rabbits. The non-decalci fication patholigic examinations were undertaken at 24 hours, 1, 2 weeks, 1, 2 , 3 months after the procedure. Results ① Biomechanics: The average biological fixation intensities of MPC were stronger than those of CPC in any different periods, with significant statistical difference(P 0. 05). ②Histology: MPC began to be absorbed by the tissues after 2 weeks of implantation, and was completely absorbed with in two months. The bone fenestra in the condylar region of femur was healed. Conclusions The biological fixation intensity of MPC is stronger than that of CPC. MPC can be absorbed gradually and might potentially be used for fracture adhesive treatment.

Abstract:The poor mechanical strength and bioactivity of magnesium phosphate bone cements (MPCs) are the vital defects for bone reconstruction. Clay minerals have been widely used in biomedical field due to the good reinforcing property and cytocompatibility. Here, laponite, sepiolite or halloysite were incorporated to fabricate MPCs composite, and the composition, microstructure, setting time, compressive strength, thermal stability, degradation performance,in vitrobioactivity and cell viability of MPCs composite were investigated. The results suggested that the MPCs composite possessed appropriate setting time, high mechanical strength and good thermal stability. By contrast, MPCs composite containing 3.0 wt.% of sepiolite presented the highest compressive strength (33.45 ± 2.87 MPa) and the best thermal stability. The degradation ratio of MPCs composite was slightly slower than that of MPCs, and varied in simulated body fluid and phosphate buffer solution. Therefore, the obtained MPCs composite with excellent bioactivity and cell viability was expected to meet the clinical requirements for filling bone defect.

Abstract:Magnesium calcium phosphate biocement (MCPB) with rapid-setting characteristics was fabricated by using the mixed powders of magnesium oxide (MgO) and calcium dihydrogen phosphate (Ca(H 2 PO 4 ) 2 ·H 2 O). The results revealed that the MCPB hardened after mixing the powders with water for about 7 min, and the compressive strength reached 43 MPa after setting for 1 h, indicating that the MCPB had a short setting time and high initial mechanical strength. After the acid–base reaction of MCPB containing MgO and Ca(H 2 PO 4 ) 2 ·H 2 O in a molar ratio of 2 : 1, the final hydrated products were Mg 3 (PO 4 ) 2 and Ca 3 (PO 4 ) 2 . The MCPB was degradable in Tris–HCl solution and the degradation ratio was obviously higher than calcium phosphate biocement (CPB) because of its fast dissolution. The attachment and proliferation of the MG 63 cells on the MCPB were significantly enhanced in comparison with CPB, and the alkaline phosphatase activity of MG 63 cells on the MCPB was significantly higher than on the CPB at 7 and 14 days. The MG 63 cells with normal phenotype spread well on the MCPB surfaces, and were attached in close proximity to the substrate, as seen by scanning electron microscopy (SEM). The results demonstrated that the MCPB had a good ability to support cell attachment, proliferation and differentiation, and exhibited good cytocompatibility.

Abstract:Magnesium phosphate cement (MPC) is a kind of novel biodegradable bone adhesive for its distinct performance. However, there is few research work concerning on the systemic biocompatibility and genetic toxicological evaluation of MPC. In this study, the investigation on the inherited toxicology of MPC including gene mutation assay (Ames test), chromosome aberration assay (micronucleus test), and DNA damage assay (unscheduled DNA synthesis test) were carried out. Fracture healing and degradation behavior were explored for the evaluation of the biocompatibility of MPC, using macroscopical histological, histomorphometrical, and scanning electron microscopical methods. The results of mutagenicity and potential carcinogenicity of MPC extracts were negative, and the animal implantation illustrated no toxicity and good resorption. The study suggested that bioresorbable MPC was safe for application and might have potential applications for physiological fracture fixation.

Abstract:Magnesium phosphate cement (MPC), a new type of inorganic cementitious material, is favored in engineering and construction because of its fast setting speed and high bonding strength, but is limited in practical application due to its high production cost and excessive release of hydration heat. Relevant research has investigated the application of discarded oyster shell powder (OSP) replacing cement mortar and has reported certain improvements to its performance. Consequently, focusing on discovering more effects of OSP on MPC performance, this study, by using a typical three-point bending test, used 45 cuboid specimens to investigate the influences of OSP mass content on flexural properties of MPC at different curing times. Results illustrated that MPC flexural strength was first increased and then decreased, and 3% is the critical value for OSP mass content. Similarly, the stiffness of all specimens presented a tendency to increase first and then decrease, with a maximum value of 36.18 kN/mm appearing at 3%, i.e., the critical OSP mass content. Finally, scanning electron microscope (SEM) and X-ray diffraction (XRD) were employed to analyze the microstructure and composition of specimens, confirming that the specimens generated not only the hydration product potassium phosphate magnesium (MgKPO4·6H2O, MKP), but also another new reactant (CaHPO4·2H2O).

Novel, Degradable, and Cytoactive Bone Cements Based on Magnesium Polyphosphate and Calcium Citrate

Abstract:Ideal bone-filling materials should be degradable and efficient for fast bone remodeling. In this study, magnesium polyphosphate was applied to bone cement for the first time and a novel degradable bone cement was developed in situ through the interaction of magnesium polyphosphate coacervate and calcium citrate. The weight loss rate when the bone cement was degraded in phosphate buffer solution (PBS) for 56 days was 57.6 ± 1.30%. The bone cement exhibited appropriate setting time (15.3 ± 0.6 min), injectability (46.9 ± 0.5%), good anti-washout ability, and compressive strength (17.0 ± 0.9 MPa) matching the bearing capacity of cancellous bone. In vitro studies showed that this type of bone cement presents cell compatibility and significantly promotes osteoblast differentiation. These results indicate that a composite bone cement based on magnesium polyphosphate and calcium citrate may have the potential for bone tissue repair and regeneration in future clinical applications.

Abstract:Summary Administration of methyldopa (L-a-methyldopa) to reserpine-treated dogs and rabbits results in restoration of pressor responses to tyramine. The amine metabolites of methyldopa, α-methyldopamine and α-methylnorepinephrine, cause similar but more prompt enhancement of tyramine responses with the latter compound showing maximum potency and more prompt onset of effect. That the effects of methyldopa are due to its amine metabolites is also indicated by the fact that D-α-methyldopa, which is not decarboxylated, is inactive. Present evidence favors α-methylnorepinephrine as the mediator responsible for the blood pressure lowering produced by methyldopa in man, as well as the preservation and restoration of tyramine responses following administration of the parent drug.
Calcium silicate cements endowing bioactivity and sustaining mechanical strength of low-heat-releasing and fast-curing magnesium phosphate cements

Abstract:It is known that magnesium phosphate cements (MPCs) show appreciable mechanical strength and biocompatibility, but the hydration reaction processes often lead to intense heat release while the hydration products present weak resistance to mechanical decay and low bioactivity. Herein we developed an MPC-based system, which was low-heat-releasing and fast-curing in this study, by compounding with self-curing calcium silicate cements (CSCs). The MPC composed of magnesium oxide (MgO), potassium dihydrogen phosphate (KH2PO4), disodium hydrogen phosphate (Na2HPO4), magnesium hydrogen phosphate trihydrate (MgHPO4·3H2O) and chitosan were weakly basic, which would be more stable in vivo. The physicochemical properties indicated that the addition of CSCs could increase the final setting time while decrease the heat release. Meanwhile, the CSCs could endow MPC substrate with apatite re-mineralization reactivity, especially, which add 25 wt.% CSCs showed the most significant apatite deposition. What's more, the mechanical evolution in buffer demonstrated CSCs could enhance and sustain the mechanical strength during degradation, and the internal constructs of cement implants could still be reconstructed by μCT analysis in rabbit femoral bone defect model in vivo. Particularly, appropriate CSCs adjusted the biodegradation and promoted new bone tissue regeneration in vivo. Totally, the MPC/CSCs composite system endows bioactivity and sustains mechanical strength of the MPC, which may be promising for expending the clinical applications of MPC-based bone cements.
An injectable porous bioactive magnesium phosphate bone cement foamed with calcium carbonate and citric acid for periodontal bone regeneration

Abstract:Magnesium phosphate cement (MPC) has been evaluated as a novel bone substitute owing to its favorable biocompatibility, plasticity, and osteogenic potential. However, the low porosity of MPC prevents growth factors and osteoblasts from fully growing into the material, thereby limiting its clinical use. In this study, different concentrations (0-5%) of calcium carbonate and citric acid (CA) were used as foaming agents to prepare porous MPC. The MPC containing 3% CaCO3/CA exhibited the best physicochemical properties, including greater porosity, improved injectability, extended setting time, and decreased hydration temperature. The proliferation and adhesion of cells on 3%CaCO3/CA-MPC were higher than those on MPC alone. To explore its osteogenesis in vivo, 3% CaCO3/CA-MPC and Bio-Oss® bone powder were implanted into periodontal bone defects in rats for 4 weeks and 12 weeks, respectively. Micro-CT and histological analysis demonstrated the improved bone regeneration of 3%CaCO3/CA-MPC compared to the blank group (P < 0.05); it had slightly lower bone regeneration than the Bio-Oss® group but no statistical difference. The results indicated that porous MPC foamed with calcium carbonate and CA improved its physicochemical properties and enhanced its biocompatibility, making it a promising material for bone regeneration.
Injectability and mechanical properties of magnesium phosphate cements

Abstract:Up to now magnesium phosphate cements are mainly being utilized in wastewater treatment due to their adsorptive properties. Recently they also have been shown to have a high potential as degradable biocements for application as replacement materials for bone defects. In comparison to degradable calcium phosphate cements they have the advantage of setting at neutral pH, which is favorable in biological environment. In this study two parameters of the cement composition, namely powder-to-liquid ratio (PLR) and citrate content, were varied in order to optimize the injectability properties of the cement paste and the mechanical properties of the reaction product. These properties were determined by means of testing setting time and temperature, paste viscosity, and injectability as well as phase composition and compressive strength of the set cements. Best results were obtained, when the cements were prepared with a PLR of 2.5 and a binder liquid consisting of an aqueous solution of 3 mol/l diammonium hydrogen phosphate and 0.5 mol/l diammonium citrate.

Abstract:BACKGROUND: Bone defects arising from diverse causes, such as traffic accidents, contemporary weapon usage, and bone-related disorders, present significant challenges in clinical treatment. Prolonged treatment cycles for bone defects can result in complications, impacting patients' overall quality of life. Efficient and timely repair of bone defects is thus a critical concern in clinical practice. OBJECTIVE: This study aims to assess the scientific progress and achievements of magnesium phosphate bone cement (MPC) as an artificial bone substitute material. Additionally, the research seeks to explore the future development path and clinical potential of MPC bone cement in addressing challenges associated with bone defects. METHODS: The study comprehensively reviews MPC's performance, encompassing e.g. mechanical properties, biocompatibility, porosity, adhesion and injectability. Various modifiers are also considered to broaden MPC's applications in bone tissue engineering, emphasizing drug-loading performance and antibacterial capabilities, which meet clinical diversification requirements. RESULTS: In comparison to alternatives such as autogenous bone transplantation, allograft, polymethyl methacrylate (PMMA), and calcium phosphate cement (CPC), MPC emerges as a promising solution for bone defects. It addresses limitations associated with these alternatives, such as immunological rejection and long-term harm to patients. MPC can control heat release during the curing process, exhibits superior mechanical strength, and has the capacity to stimulate new bone growth. CONCLUSION: MPC stands out as an artificial bone substitute with appropriate mechanical strength, rapid degradation, non-toxicity, and good biocompatibility, facilitating bone repair and regeneration. Modification agents can enhance its clinical versatility. Future research should delve into its mechanical properties and formulations, expanding clinical applications to create higher-performing and more medically valuable alternatives in bone defect repair.

Construction of macroporous magnesium phosphate-based bone cement with sustained drug release

Abstract:<p>Magnesium phosphate-based bone cements (MPBCs) have been widely applied in orthopedic and dental fields owing to their excellent self-setting ability and high strength. However, their lack of macroporosity and poor drug release properties restrict their use. In this study, we incorporated various degrees of cross-linking of gelatine microspheres (GM) into MPBC and improved the physicochemical and biocompatible properties, biodegradation and drug release behaviour of the composites. Diclofenac sodium (DS) was utilized as a model drug. Through experiments and observations, we found that the GM induced an adjustable setting time (12 min–16 min), high compression strength (23 MPa–58 MPa), abundant macropores (30.2%–37.8%) and sustained DS release with the double exponential biphasic kinetic model (more than 2 months) into the MPBC composites. Moreover, the sustained release of magnesium and calcium ions had a synergistic effect with GM on the proliferation, osteogenesis differentiation, mineralization ability and gene expression (COL I, OPN, and RUNX2) of MC3T3-E1 cells. Subcutaneous implantation and histological analyses indicated that the MPBC-GM composites activated angiogenesis without inducing severe inflammatory reactions. In brief, we prepared biocompatible MPBC composites with adjustable physicochemical properties, formation of macropores, degradation and drug release behaviour.</p>

Property Assessment of Magnesium Phosphate Cement

Abstract:Magnesium phosphate cement (MPC) paste is prepared by mixing dead burned magnesia powder and potassium di-hydrogen phosphate with a proper proportion of water. The superior properties such as fast setting, high early strength, low shrinkage and good bonding makes MPC suitable for applications in rapid repair of infrastructures. However, the setting time of MPC is sometimes too fast, only taking a few minutes, which causes difficulties in manipulation. How to retard and control the setting time of MPC becomes a challenging topic in both scientific and engineering fields.It was found that the setting of MPC was greatly influenced by the reactivity of magnesia, which could be determined by a quick, accurate and convenient test method. Two kinds of magnesia, namely, high-reactivity magnesia and low-reactivity magnesia were blended in order to control the setting time of MPC. Effects of magnesia to phosphate molar ratio, blending ratio and borax addition on setting time, workability and mechanical properties of MPC cement paste were investigated. Finally, the suitable ranges of the different constitutes of MPC based repair materials with satisfied workability and strength development were proposed for the practical engineering applications.
