Abstract:ObjectiveTo provide updated reference standards for cardiorespiratory fitness (CRF) for the United States derived from cardiopulmonary exercise (CPX) testing when using a treadmill or cycle ergometer.Patients and MethodsThirty-four laboratories in the United States contributed data to the Fitness Registry and the Importance of Exercise National Database. Analysis included 22,379 tests (16,278 treadmill and 6101 cycle ergometer) conducted between January 1, 1968, through March 31, 2021, from apparently healthy adults (aged 20 to 89 years). Percentiles of peak oxygen consumption for men and women were determined for each decade from 20 through 89 years of age for treadmill and cycle exercise modes, as well as when defining maximal effort as respiratory exchange ratio (RER) greater than or equal to 1.0 or RER greater than or equal to 1.1.ResultsFor both men and women, the 50th percentile scores for each exercise mode decreased with age and were higher in men across all age groups and higher for treadmill compared with cycle CPX. The average rate of decline per decade over a 6-decade period was 13.5%, 4.0mLO2/kg/min for treadmill CPX and 16.4%, 4.3mLO2/kg/min for cycle CPX. Observationally, the mean peak oxygen consumption was similar whether using an RER criterion of greater than or equal to 1.0 or greater than or equal to 1.1 across the different test modes, ages, and for both sexes. The updated reference standards for treadmill CPX were 1.5 – 4.6 mL O2 × kg-1 × min-1 lower compared with the previous 2015 standards whereas the updated cycling standards were generally comparable to the original 2017 standards.ConclusionThese updated cardiorespiratory fitness reference standards improve the representativeness of the US population compared with the original standards.

Abstract:Introduction: Peak rating of perceived exertion (RPE) is measured during clinical cardiopulmonary exercise testing (CPX) and is commonly used as a subjective indicator of maximal effort. However, no study to date has reported reference standards or the distribution of peak RPE across a large cohort of apparently healthy individuals. Purpose: This study aimed to determine reference standards for peak RPE when using the 6-20 Borg scale for both treadmill and cycle tests. Methods: The analysis included 9551 tests (8821 treadmill, 730 cycle ergometer) from 13 laboratories within the Fitness Registry and Importance of Exercise National Database (FRIEND). Using data from tests conducted January 1, 1980, to January 1, 2021, percentiles of peak RPE for men and women were determined for each decade from 20 to 89 yr of age for treadmill and cycle exercise modes. Two-way ANOVA was used to compare differences in peak RPE values between sexes and across age groups. Results: There were statistically significant differences in RPE between age groups whether the test was performed on a treadmill or cycle ergometer ( P < 0.05). However, the mean and median RPE for each sex, age group, and test mode were between 18 and 19. In addition, 83% of participants met the traditional RPE criteria of ≥18 for indicating sufficient maximal effort. Conclusions: This report provides the first normative reference standards for peak RPE in both male and female individuals performing CPX on a treadmill or cycle ergometer. Furthermore, these reference standards highlight the general consistency of peak RPE responses during CPX.
Recalibrated FRIEND equation for peak oxygen pulse is accurate in endurance athletes: the NOODLE study

Abstract:Peak oxygen pulse (O 2 P peak ) is an important index of cardiorespiratory fitness (CRF). The FRIEND database is a global source of reference values for CRF. However, no reference equation is tailored for endurance athletes (EA) to predict O 2 P peak . Here, we adjusted the well-established FRIEND equation for O 2 P peak to the characteristics of the EA population. 32 (34.0%) female EA and 62 (66.0%) male well-trained EA underwent maximal cardiopulmonary exercise test on a treadmill. V̇O 2max was 4.5 ± 0.5 L min −1 in males and 3.1 ± 0.4 L min −1 in females. O 2 P peak was 23.6 ± 2.8 mL beat −1 and 16.4 ± 2.0 mL beat −1 for males and females, respectively. Firstly, we externally validated the original FRIEND equation. Secondly, using multiple linear regression, we adjusted the FRIEND equation for O 2 P peak to the population of EA. The original FRIEND equation underestimated O 2 P peak for 2.9 ± 2.9 mL beat −1 ( P < .001) in males and 2.2 ± 2.1 mL beat −1 ( P < .001) in females. The updated equation was 1.36 + 1.07 (23.2 · 0.09 · age − 6.6 [ if female ]). The new equation explained 62% of the variance and significantly predicted O 2 P peak (R 2 = 0.62, β = 0.78, P < .001). The error of the EA-adjusted model was 0.1 ± 2.9 mL beat −1 ( P = .82) and 0.2 ± 2.1 mL beat −1 ( P = .65) for males and females respectively. Recalibration of the original FRIEND equation significantly enhances its accuracy among EA. The error of the EA-adjusted model was negligible. A new recalibrated equation should be used to predict O 2 P peak in the population of EA.

Abstract:Purpose: Cardiorespiratory fitness (CRF) is a strong predictor of cardiorespiratory diseases and varies by race. The purpose of this study was to provide CRF reference standards and a prediction equation for peak oxygen uptake (V ̇O 2peak ) from treadmill-based cardiopulmonary exercise testing (CPX) in Chinese individuals. Methods: Healthy participants (n = 4199) who completed a CPX using a treadmill were studied. The percentiles of V ̇O 2peak were determined for four age groups (decades). A regression prediction model was developed from the derivation cohort (n = 3361), validated in the independent validation cohort (n = 838), and compared with the widely used Wasserman equation and the Fitness Registry and the Importance of Exercise National Database (FRIEND) equation. Results: The mean V ̇O 2peak values of four age groups (20-29, 30-39, 40-49, and 50-59 yr) were 42.6, 41.2, 38.7, and 35.9 mL/kg/min, respectively, for men, and 37.1, 34.7, 32.0, and 30.3 mL/kg/min, respectively, for women. The 50th percentiles of relative V ̇O 2peak decreased with age for both sexes. The prediction equation was: Absolute V ̇O 2peak (mL/min) = 236.68 – (504.64 × sex [male = 0; female = 1]) + (21.23× weight [kg]) – (14.31 × age [yr]) + (9.46 × height [cm]) (standard error of the estimate = 379.59 mL/min, R 2 = 0.66, P < .001). Percentage predicted V ̇O 2peak for the validation sample was 100.2%. The novel equation performed better than the other two equations. Conclusion: This study reports the first CRF reference standards and prediction equation generated from treadmill CPX in China. These reference standards provide a framework for interpreting the CRF of the Chinese population and could be useful information for a global CRF database.

Abstract:Background: Up-to-date normative reference sets for cardiopulmonary exercise testing (CPET) are important to aid in the accurate interpretation of CPET in clinical or research settings. Research question: This study aimed to (1) develop and externally validate a contemporary reference set for peak CPET responses in Canadian adults identified with population-based sampling; and (2) evaluate previously recommended reference equations for predicting peak CPET responses. Study design and methods: Participants were healthy adults who were ≥40 years old from the Canadian Cohort Obstructive Lung Disease who completed an incremental cycle CPET. Prediction models for peak CPET responses were estimated from readily available participant characteristics (age, sex, height, body mass) with the use of quantile regression. External validation was performed with a second convenience sample of healthy adults. Peak CPET parameters that were measured and predicted in the validation cohort were assessed for equivalence (two one-sided tests of equivalence for paired-samples and level of agreement (Bland-Altman analyses). Two one-sided tests of equivalence for paired samples assessed differences between responses in the derivation cohort using previously recommended reference equations. Results: Normative reference ranges (5th-95th percentiles) for 28 peak CPET parameters and prediction models for 8 peak CPET parameters were based on 173 participants (47% male) who were 64 ± 10 years old. In the validation cohort (n = 84), peak CPET responses that were predicted with the newly generated models were equivalent to the measured values. Peak cardiac parameters predicted by the previously recommended reference equations by Jones and colleagues and Hansen and colleagues were significantly higher. Interpretation: This study provides reference ranges and prediction models for peak cardiac, ventilatory, operating lung volume, gas exchange, and symptom responses to incremental CPET and presents the most comprehensive reference set to date in Canadian adults who were ≥40 years old to be identified with population-based sampling.

Abstract:Introduction: This study aims to establish normal reference values of peak oxygen uptake (VO2peak) for cardiorespiratory fitness (CRF) in Chinese adults using cardiorespiratory exercise testing (CPET). Methods: A cross-sectional study was done in four communities, two in the North (Beijing) and two in the South (Hezhou, Guangxi) of China from 1 January 2017 to 31 December 2018, with one urban and one rural in each region. Out of 1642 participants screened, 1114 were eligible and completed CPET using a cycle ergometer (Ergosana320F) without abnormal ECG and were included in the analysis. The 2nd and 98th percentiles of V·O2peak were used as the lower and upper limits of the normal reference values. Results: Significant difference in mean V·O2peak was shown between men (27.0 mL·min−1·kg−1) and women (23.7 mL·min−1·kg−1). The mean V·O2peak decreased with age in both sexes, from 35.8 mL·min−1·kg−1 in age 20–29 years to 20.5 mL·min−1·kg−1 in 70–79 years in men and from 29.2 mL·min−1·kg−1 to 17.0 mL·min−1·kg−1 in women. Thus, the age- and sex-specific normal reference values of V·O2peak were presented for each 10-year age group by men and women separately. Conclusions: This first community-based study in China provides age- and sex-specific normal references of V·O2peak as a measure of CRF in Chinese adults, which differed significantly from those established in Western populations. Future studies with national representative samples should be warranted.

Abstract:ABSTRACT Purpose Cardiorespiratory fitness (CRF) measured by peak oxygen consumption (VO 2 peak) declines with aging and correlates with mortality and morbidity. Cardiopulmonary Exercise Testing (CPET) is the criterion method to assess CRF, but its feasibility, validity and reliability in older adults is unclear. Our objective was to design and implement a dependable, safe and reliable CPET protocol in older adults. Methods VO 2 peak was measured by CPET, performed using treadmill exercise in 875 adults ≥70 years in the Study of Muscle, Mobility and Aging (SOMMA). The protocol included a symptom-limited peak (maximal) exercise and two submaximal walking speeds. An adjudication process was in place to review tests for validity if they met any prespecified criteria [VO 2 peak < 12.0 ml/kg/min; maximum heart rate (HR) <100 bpm; respiratory exchange ratio (RER) <1.05 and a rating of perceived exertion <15]. A subset (N = 30) performed a repeat test to assess reproducibility. Results CPET was safe and well tolerated, with 95.8% of participants able to complete the VO 2 peak phase of the protocol. Only 56 (6.4%) participants had a risk alert and only two adverse events occurred: a fall and atrial fibrillation. Mean ± SD VO 2 peak was 20.2 ± 4.8 mL/kg/min, peak HR 142 ± 18 bpm, and peak RER 1.14 ± 0.09. Adjudication was indicated in 47 tests; 20 were evaluated as valid, 27 as invalid (18 data collection errors, 9 did not reach VO 2 peak). Reproducibility of VO 2 peak was high (intraclass correlation coefficient = 0.97). Conclusions CPET was feasible, effective and safe for older adults, including many with multimorbidity or frailty. These data support a broader implementation of CPET to provide insight into the role of CRF and its underlying determinants of aging and age-related conditions.

Relationship Between the Ruffier Test and Maximal Treadmill Testing in Healthy Adults

Abstract:PURPOSE: Estimating aerobic fitness in the busy outpatient clinical setting with gold standard maximal fitness testing is challenging due to time, staffing and equipment access limitations. Short tests that can be performed with minimal equipment and staff may overcome these limitations, and improve clinician ability to provide customized exercise recommendations and track fitness responses over time. This study piloted whether a peak fitness level could be estimated by a short, simple Ruffier exercise readiness test. METHODS: Seventeen adults (32.6 ± 10.8 yrs; 26.0 ± 10.7 kg/m2; 9 F) participated. During one testing session, participants performed a Ruffier fitness readiness test (30 squats in 45 s) and a Balke maximal treadmill fitness test. In the Ruffier test, heart rate (HR) was measured pre-test, immediately post and 1-min post-test. Ruffier scores were calculated and classified from the 3 HR values. During the Balke test, endurance time and peak values for HR, rate of oxygen use (VO2peak), ventilation (VE) and respiratory quotient (RQ) were captured. RESULTS: Mean HR values during the Ruffier test were 68.7 ± 9.9 bpm (pre-test), 129.5 ± 9.7 bpm (immediately-post) and 88.4 ± 19.5 bpm (1-min post), which represented a significant change over time; p<0.0001. The Ruffier score averaged 8.8 ± 3.4 points (from 0-20 points). The Balke VO2peak measure averaged 39.7 ± 11.6 ml/kg*min, with a peak HR of 179.3 ± 14.4 bpm and RQ of 1.2 ± 0.08. Average endurance time was 21.2 ± 4.5 min. Correlation between Ruffier scores and VO2peak value revealed a Pearson correlation coefficient r = 0.540 (two-tailed p-value of p=0.024, at 0.05 level). CONCLUSIONS: Moderate associations were found between the fitness scores of the two tests from this group. The 45 s Ruffier exercise test may be useful for quickly identifying patients with poor aerobic fitness levels in the clinical setting, but may not be long enough to produce physiological responses that could discern fine gradations of fitness.

Abstract:Background To develop a clinically applicable equation derived from direct assessment of maximal oxygen uptake (VO 2max ) to predict VO 2max assessed indirectly during cycle ergometry. Design VO 2max was assessed by open-circuit spirometry during a graded maximal exercise test using cycle ergometry. Multivariable linear regression analysis was applied to identify the most relevant variables and construct the best prediction model for VO 2max using a random sample of 70% from each of the following age categories: 70 years; the remaining 30% was used for validation. Work rate (Watts*6.12/kg of body weight) and gender were considered in the final regression model as predictors of measured VO 2max and the resulting equation was compared to the traditional American College of Sports Medicine (ACSM) equation. Methods Participants were part of the Fitness Registry and the Importance of Exercise National Database (FRIEND), a multi-institutional initiative with the primary objective of establishing normative VO 2max values across the adult lifespan. The final cohort consisted of 5100 (3378 men; mean age 35.9 ± 12.1 years and 1722 women; mean age 47.5 ± 14.0 years). Results The following equation was generated: VO 2max in ml O 2 ·kg –1 ·min –1 = 1.74* (Watts*6.12/kg of body weight) + 3.5. The derived FRIEND-ergometry equation predicted VO 2max with an overall relative bias of 0.51% ± 0.11 compared to a 15.46% ± 0.13 associated with the traditional ACSM equations ( P < 0.001). This predictive value was independent of gender, race, cardiac risk factors and cardiac, antihypertensive, metabolic and/or lipid-lowering medication. Conclusion The FRIEND-ergometry equation is considerably more precise than the traditional ACSM equation with an overall error over 30 times lower than that associated with the ACSM equation.

Abstract:BackgroundCardiorespiratory fitness (CRF) is the only major risk factor that is not routinely assessed in the clinical setting, for preventive medicine. A valid and practical CRF test is needed for use in the clinics. The objective of this study is to demonstrate the validity of a 3-minute squat test to assess CRF in primary care.MethodsA cross-sectional study in which the participants performed both the Ruffier squat test and the Balke maximal treadmill test. The study was conducted in a clinical setting from September 2016 to March 2017. We recruited a convenient sample of 40 adults between 18 and 64 years from the general U.S. population. Participants completed 30 squats in 45 seconds, paced by a metronome. Heart rate was measured at rest (P1), immediately after the test (P2), one minute after the test (P3). (V) over dot O(2)max was measured using the Balke maximal treadmill fitness test.ResultsOf the 40 participants, there were 18 men and 22 women. Mean age was 31.2 years (SD = 9.9). We found that the best (V) over dot O-2 max predictors were HR features P1/height and (P2-P3)/age 3. Our best-performing model using these two features predicted individuals' CRF levels with an adjusted R-2 of 0.637, sensitivity of 0.79, and specificity of 0.56.ConclusionsThe study provided strong evidence for the validity of the squat test in the clinical setting. Further, the equation of our model along with (V) over dot O(2)max normative tables provides an efficient and easy way to assess CRF in a primary care setting.
Utility of the oxygen pulse in the diagnosis of obstructive coronary artery disease in physically fit patients

Abstract:Cardiopulmonary exercise testing (CPET) guidelines recommend analysis of the oxygen (O2 ) pulse for a late exercise plateau in evaluation for obstructive coronary artery disease (OCAD). However, whether this O2 pulse trajectory is within the range of normal has been debated, and the diagnostic performance of the O2 pulse for OCAD in physically fit individuals, in whom V˙O2 may be more likely to plateau, has not been evaluated. Using prospectively collected data from a sports cardiology program, patients were identified who were free of other cardiac disease and underwent clinically-indicated CPET within 90 days of invasive or computed tomography coronary angiography. The diagnostic performance of quantitative O2 pulse metrics (late exercise slope, proportional change in slope during late exercise) and qualitative assessment for O2 pulse plateau to predict OCAD was assessed. Among 104 patients (age:56 ± 12 years, 30% female, peak V˙O2 119 ± 34% predicted), the diagnostic performance for OCAD (n = 24,23%) was poor for both quantitative and qualitative metrics reflecting an O2 pulse plateau (late exercise slope: AUC = 0.55, sensitivity = 68%, specificity = 41%; proportional change in slope: AUC = 0.55, sensitivity = 91%, specificity = 18%; visual plateau/decline: AUC = 0.51, sensitivity = 33%, specificity = 67%). When O2 pulse parameters were added to the electrocardiogram, the change in AUC was minimal (-0.01 to +0.02, p ≥ 0.05). Those patients without OCAD with a plateau or decline in O2 pulse were fitter than those with linear augmentation (peak V˙O2 133 ± 31% vs. 114 ± 36% predicted, p < 0.05) and had a longer exercise ramp time (9.5 ± 3.2 vs. 8.0 ± 2.5 min, p < 0.05). Overall, a plateau in O2 pulse was not a useful predictor of OCAD in a physically fit population, indicating that the O2 pulse should be integrated with other CPET parameters and may reflect a physiologic limitation of stroke volume and/or O2 extraction during intense exercise.

Abstract:Cardiorespiratory fitness has been shown to be correlated with cardiovascular complications and comparable to other conventional health parameters. Consequently, it can be a useful preventative and a practical first line of information for cardiovascular diseases, such as hypertension. Wearable devices have the capacity to provide a convenient assessment of individuals cardiorespiratory fitness. Despite its clinical relevance measures of cardiorespiratory fitness remain limited to commercial devices for exercise training purposes. This study investigates whether there is potential for cardiorespiratory fitness to be derived from indices measured at rest from a finger photoplethysmogram waveform. Participants (n = 8, 4 females, 39 ± 17 years) first completed two questionnaires to provide two separate estimates of cardiorespiratory fitness. The first questionnaire by Jackson et al. (1990) provides an estimated maximum rate of oxygen (VO2max, gold standard measure of cardiorespiratory fitness) based on height, weight, age, and a subjective physical activity rating score. The second questionnaire by Radim et al. (2005) provides a metabolic equivalent parameter (MET) that has previously been shown to correlate with measured VO2max. Following 10 minutes of seated rest, 30 seconds of finger photoplethysmography was recorded. A 10-second snippet with high quality signal was extracted, and a generalised transfer function was applied to the signal to generate an uncalibrated central aortic waveform. Pulse pressure amplification (PPAmp), ejection duration (ED), subendocardial viability ratio (SEVR), heart rate (HR) and augmentation index corrected to a HR of 75 bpm (AIx75bpm) were then calculated for each cardiac cycle and averaged over 10 seconds. Correlation plots and stepwise linear regression were then performed on the estimated VO2max and MET with the derived parameters. Average baseline estimated VO2max was 44 ± 10 ml/kg/min and estimated MET was 11 ± 2 (1 MET = VO2/3.5). Stepwise linear regression revealed AIx75bpm, SEVR and HR were the strongest predictors of estimated VO2max (model R^2 = 0.929, F = 17.44, p = 0.009). Only AIx75bpm and SEVR were strongly associated with estimated MET (model R^2 = 0.942, F = 40.5, p < 0.001). For both estimated VO2max and estimated MET, individual correlation plots for PPAmp and ED showed poor correlation. A selection of non-exercise derived central aortic parameters from finger photoplethysmography were correlated with cardiorespiratory fitness. This demonstrates that a finger photoplethysmogram signal obtained at rest, such as can be easily measured in wearable devices, may be useful in management and monitoring of cardiorespiratory fitness, and by association health, without the burden of performing rigorous exercise tests.

Abstract:Graded exercise testing is a widely accepted tool for revealing cardiac ischemia and/or arrhythmias in clinical settings. Cardiopulmonary exercise testing (CPET) measures expired gases during a graded exercise test making it a versatile tool that helps reveal underlying physiologic abnormalities that are in many cases only present with exertion. It also characterizes one's health status and clinical trajectory, informs the therapeutic plan, evaluates the efficacy of therapy, and provides submaximal and maximal information that can be used to tailor an exercise intervention. Practitioners can also modify the mode and protocol to allow individuals of all ages, fitness levels, and most disease states to perform a CPET. When used to its full potential, CPET can be a key tool used to optimize care in primary and secondary prevention settings.

Percent Predicted Peak Exercise Oxygen Pulse Provides Insights Into Ventricular-Vascular Response and Prognosticates HFpEF

Abstract:Background: Peak oxygen consumption and oxygen pulse along with their respective percent predicted measures are gold standards of exercise capacity. To date, no studies have investigated the relationship between percent predicted peak oxygen pulse (%PredO2P) and ventricular-vascular response (VVR) and the association of %PredO2P with all-cause mortality in heart failure with preserved ejection fraction (HFpEF) patients. Objectives: The authors investigated the association between: 1) CPET measures of %PredO2P and VVR; and 2) %PredO2P and all-cause mortality in HFpEF patients. Methods: Our cohort of 154 HFpEF patients underwent invasive CPET and were grouped into %PredO2P tertiles. The association between percent predicted Fick components and markers of VVR (ie, proportionate pulse pressure, effective arterial elastance) was determined with correlation analysis. The Cox proportional hazards model was used to identify predictors of mortality. Results: The participants' mean age was 57 ± 15 years. Higher %PredO2P correlated with higher exercise capacity. In terms of VVR, higher %PredO2P correlated with a lower pressure for a given preload (effective arterial elastance r = -0.45, P < 0.001 and proportionate pulse pressure r = -0.22, P = 0.008). %PredO2P distinguished normal and abnormal percent predicted peak stroke volume and correlated positively with %PredVO2 (r = 0.61, P < 0.001). Participants had a median follow-up time of 5.6 years and 15% death. Adjusted for age and body mass index, there was a 5% relative reduction in mortality (HR: 0.95, 95% CI: 0.92-0.98, P = 0.003) for every percent increase in %PredO2P. Conclusions: In HFpEF, %PredO2P is a VVR marker that can stratify invasive parameters such as percent predicted peak stroke volume. %PredO2P is an independent prognostic marker for all-cause mortality and those with higher %PredO2P exhibited longer survival.

Abstract:To explore the correlation between the maximum percentage heart rate and the maximum percentage oxygen intake, provide an evaluation basis for heart rate for the assessment of exercise load intensity. Four boys and 4 girls were randomly selected, aged 26.25 ± 2.12 years old with good health, good cardiopulmonary function, no other medical history, and irregular physical training history. The subject measured the first 30 min of rest at 25°C at room temperature, kept awake and static, and the heart rate was measured as the quiet heart rate in the state. Prepredicted maximum heart rate and health index were determined according to the Polar s810 heart rate table instructions. Prepredicted maximum heart rate and health index were measured three consecutive times and reliability analysis was performed on three measurements. The regression equations were established by a stepwise method with data represented that all metrics were tested for normality for fitness index and maximum oxygen intake compared using a paired t test with a significance level of P < 0.05 . The results showed that the highest value of VO2max motor cardiopulmonary test was 47.83 ml/(kg·min), the lowest was 35.06 ml/(kg·min), the two-step test was 44.50 ml/(kg·min), and the lowest was 32.89 ml/(kg min).With a positive correlation between the postexercise heart rate and the maximum oxygen intake, the maximum oxygen intake value can be indirectly inferred using the heart rate after the exercise and the work completed by the exercise. The results measured by two-step test have some accuracy and can be used to speculate the maximum oxygen intake in the ordinary young population. The polar heart rate meter allows subjects to indirectly measure the maximum oxygen intake in silence, requiring less equipment and being easy to operate. The indirect measurement of the maximum oxygen intake can be used for the monitoring of competitive sports and national fitness.

Abstract:Cardiorespiratory fitness (CRF) is a well-established biomarker that has applications to all adults across the health and disease spectrum. Despite overwhelming evidence supporting the prognostic utility of CRF, it remains vastly underutilized. CRF is optimally measured via cardiopulmonary exercise testing which may not be feasible to implement on a large scale. Therefore, it is prudent to develop ways to accurately estimate CRF that can be applied in clinical and community settings. As such, several prediction equations incorporating non-exercise information that is readily available from routine clinical encounters have been developed that provide an adequate reflection of CRF that could be implemented to raise awareness of the importance of CRF. Further, technological advances in smartphone apps and consumer-grade wearables have demonstrated promise to provide reasonable estimates of CRF that are widely available, which could enhance the utilization of CRF in both clinical and community settings.

Abstract:Abstract Introduction exertional dyspnea is a symptom present in several diseases, identifying the origin is of fundamental diagnostic and therapeutic importance. Cardiorespiratory exercise testing (CPET) is a valuable tool not only to assess functional capacity but also for diagnostic and prognostic purposes. Clinical case 55–year–old patient with a history of episode of acute pulmonary edema. Echocardiogram showed FE 35% with diffuse left ventricular (LV) hypokinesis and mild–to–moderate mitral insufficiency (MI). Diagnostic coronarography showed lesion–free coronary tree. An interrupted CPET at 140W showed peak O2 consumption of 23.6 mL/kg/min that excluded indication for cardiac transplantation. An echocardiogram from us confirmed dilated cardiomyopathy (CMPD) with FE=32%, diffuse VS hypokinesia and mild–to–moderate functional MI. CPET was repeated, maximal for respiratory quotient (1.42) interrupted due to muscle exhaustion, indicative of moderate reduction in functional capacity with peak VO2 at 64% of predicted, preserved anaerobic threshold at 51% of VO2 max and reduced VO2/W and Pulse Oxygen Ratio (VO2/HR) values from cardiogenic limitation. Also evident drop in VO2/HR during the final stages of the test and associated change in slope of the VO2/Work relationship a sign of decline in stroke volume and cardiogenic efficiency. Absent signs of ventilatory limitation, pulmonary vascular (Ve/VCO2=20.8 in normal range), or exercise desaturation. In the final phase of the test increase in VCO2 and Ve/VCO2 and Ve/VO2 equivalents. In order to find the explanation to the data collected by CPET, exercise echocardiogram was performed, which revealed the development of severe mitral valvular insufficiency and subsequent pulmonary hypertension justifying the behavior of the parameters collected by CPET. Discussion The oxygen pulse represents a metabolic surrogate for stroke volume. Normally during the active phases of exertion there is a progressive increase followed by a plateau. The evident flattening of the VO2/Work curve during the final phases of exercise and the concomitant decrease in oxygen pulse denote inadequate cardiac performance relative to the increased energy demands during exercise. Conclusions CPET proves to be a valuable clinical tool to guide the diagnostic–therapeutic pathway, particularly in the presence of complex heart disease (CMPD or IM).

Abstract:The maximal oxygen uptake (V02 max) is a well-validated measure of cardiorespiratory function that is calculated during a maximal cardiopulmonary exercise test. V02 max enables physicians to objectively assess cardiopulmonary function to aid in decision-making for patients with CHD. A significant proportion of these patients however are unable to achieve a maximal exercise test, and as such, there is a need for reliable submaximal predictors of cardiorespiratory reserve. The oxygen uptake efficiency slope represents a measure of how effectively oxygen is extracted from the lungs and taken into the body and can be calculated from a submaximal exercise test. Its reliability as a predictor of cardiorespiratory reserve has been validated in various patient populations, but there is limited evidence for its validity in adult patients with CHD. Retrospective analysis of cardiopulmonary exercise test data in 238 consecutive patients with CHD who completed a maximal cardiopulmonary exercise test at our tertiary cardiology centre demonstrated a strong correlation between peak V0 2 and the oxygen uptake efficiency slope (0.936). A strong correlation with peak V02 was also demonstrated when oxygen uptake efficiency slope was calculated at ventilatory anaerobic threshold (OUES VAT ), 75% ( OUES75 ), and 90% ( OUES90 ) of the test (0.833, 0.905, 0.927 respectively). In adult patients with CHD who are unable to complete a maximal cardiopulmonary exercise test, the oxygen uptake efficiency slope is a reliable indicator of cardiopulmonary fitness which correlates strongly with peak V02 at or beyond the ventilatory anaerobic threshold. Further research is required to validate the findings in patients with less common anatomies and to assess the relationship between the oxygen uptake efficiency slope and mortality.

Abstract:Background and aim A plateau in oxygen uptake ( V ̇ O 2 ) during an incremental cardiopulmonary exercise test (CPET) to volitional exhaustion appears less likely to occur in special and clinical populations. Secondary maximal oxygen uptake ( V ̇ O 2 max ) criteria have been shown to commonly underestimate the actual V ̇ O 2 max . The verification phase protocol might determine the occurrence of ‘true’ V ̇ O 2 max in these populations. The primary aim of the current study was to systematically review and provide a meta-analysis on the suitability of the verification phase for confirming ‘true’ V ̇ O 2 max in special and clinical groups. Secondary aims were to explore the applicability of the verification phase according to specific participant characteristics and investigate which test protocols and procedures minimise the differences between the highest V ̇ O 2 values attained in the CPET and verification phase. Methods Electronic databases (PubMed, Web of Science, SPORTDiscus, Scopus, and EMBASE) were searched using specific search strategies and relevant data were extracted from primary studies. Studies meeting inclusion criteria were systematically reviewed. Meta-analysis techniques were applied to quantify weighted mean differences (standard deviations) in peak V ̇ O 2 from a CPET and a verification phase within study groups using random-effects models. Subgroup analyses investigated the differences in V ̇ O 2 max according to individual characteristics and test protocols. The methodological quality of the included primary studies was assessed using a modified Downs and Black checklist to obtain a level of evidence. Participant-level V ̇ O 2 data were analysed according to the threshold criteria reported by the studies or the inherent measurement error of the metabolic analysers and displayed as Bland-Altman plots. Results Forty-three studies were included in the systematic review, whilst 30 presented quantitative information for meta-analysis. Within the 30 studies, the highest mean V ̇ O 2 values attained in the CPET and verification phase protocols were similar (mean difference = -0.00 [95% confidence intervals, CI = -0.03 to 0.03] L·min -1 , p = 0.87; level of evidence, LoE: strong). The specific clinical groups with sufficient primary studies to be meta-analysed showed a similar V ̇ O 2 max between the CPET and verification phase ( p > 0.05, LoE: limited to strong). Across all 30 studies, V ̇ O 2 max was not affected by differences in test protocols ( p > 0.05; LoE: moderate to strong). Only 23 (53.5%) of the 43 reviewed studies reported how many participants achieved a lower, equal, or higher V ̇ O 2 value in the verification phase versus the CPET or reported or supplied participant-level V ̇ O 2 data for this information to be obtained. The percentage of participants that achieved a lower, equal, or higher V ̇ O 2 value in the verification phase was highly variable across studies (e.g. the percentage that achieved a higher V ̇ O 2 in the verification phase ranged from 0% to 88.9%). Conclusion Group-level verification phase data appear useful for confirming a specific CPET protocol likely elicited V ̇ O 2 max , or a reproducible V ̇ O 2 p e a k , for a given special or clinical group. Participant-level data might be useful for confirming whether specific participants have likely elicited V ̇ O 2 max , or a reproducible V ̇ O 2 p e a k , however, more research reporting participant-level data is required before evidence-based guidelines can be given. Abstract:Abstract Background Patients with chronic obstructive pulmonary disease (COPD) are more likely to develop cardiovascular comorbidities, such as pulmonary hypertension or heart failure. COPD patients are frequently adversely affected by compromised cardiovascular function. Oxygen pulse (O2P) serves as a proxy for stroke volume. However, studies concerning O2P, health-related quality of life (HRQL), and exercise capacity in COPD patients are lacking. Our objective was to verify the association between O2P, exercise capacity, and severe COPD exacerbation. Materials and methods Fifty COPD patients were evaluated using spirometry, echocardiography, and a cardiopulmonary exercise test (CPET) for this study. Hospitalizations and emergency department visits due to COPD, as well as cardiovascular co-morbidities, were tracked. Patients with normal peak O2P and those with impaired peak O2P were compared for these measures. Peak oxygen consumption (O2P) was correlated with cardiopulmonary exercise testing (CPET) and lung function by simple linear regression. Results Higher exercise capacity (peak oxygen uptake and work rate) and fewer hospitalizations due to COPD were observed in patients with normal peak O2P. Forced expiratory volume in one second (FEV1) was found to have a statistically significant correlation with arterial oxygen pressure (O2P) in a linear regression model. Conclusion Hospitalization due to COPD and exercise ability are both significantly affected by peak O2P. Peak O2P is strongly correlated with FEV1. The severity of COPD can be measured in part by the patient's peak O2P.
