Abstract:Increasing prevalence of endemic chronic kidney disease of unknown etiology (CKDu) in Sri Lanka is a major health problem since the 1990s. Despite numerous studies on CKDu, research groups have been unable to develop a comprehensive approach to mitigate the disorder, and thereby to identify research gaps. We conducted a systematic literature review of 119 publications on CKDu in Sri Lanka from Pubmed, Google Scholar, and Scopus, published until end September 2020. A higher CKDu prevalence in the working population of the North Central Province was reported with recent studies indicating patients from non-endemic regions as well. A majority were etiological studies that recorded conflicting and inconclusive evidence on CKDu etiology. Studies on clinico-pathological, diagnostic, biochemical, and molecular biological aspects of CKDu, studies analyzing CKDu symptom burden, anthropological, and behavioral impacts of CKDu, were reviewed as well. A dearth of research exists on nutritional, demographical, immunological, and microbial aspects of CKDu. The overview of the reviewed literature indicated the absence of a comprehensive plan of action to mitigate this situation. Hence, we propose the “One Health” approach with a systems dynamics model as a potential way forward to alleviate the CKDu epidemic in Sri Lanka. This enables the representation of multiple causative agents (and interactions thereof) among environmental, animal, and human systems, in concert with the “exposome” that provides the totality of exposure the individual has undergone since birth.

Abstract:BackgroundThe alarming rise in the prevalence of chronic kidney disease of uncertain etiology (CKDu) among the low socioeconomic farming community in the North Central Province of Sri Lanka has been recognized as an emerging public health issue in the country.MethodsThis study sought to determine the possible factors associated with the progression and mortality of CKDu. The study utilized a single-center cohort registered in 2003 and followed up until 2009 in a regional clinic in the endemic region, and used a Cox proportional hazards model.ResultsWe repeatedly found an association between disease progression and hypertension. Men were at higher risk of CKDu than women. A significant proportion of the patients in this cohort were underweight, which emphasized the need for future studies on the nutritional status of these patients.ConclusionsCompared with findings in western countries and other regions of Asia, we identified hypertension as a major risk factor for progression of CKDu in this cohort.

Abstract:Abstract Background and Aims This study evaluates an integrated care program for chronic kidney disease (CKD), which has demonstrated effectiveness in delaying kidney function decline in the rural population of Thailand. Efficacy of the program was initially established through the ESCORT-1 study, a cluster-randomized controlled trial (doi: 10.1186/s12882-016-0414-4), and further demonstrated the effectiveness by the larger-scale prospective cohort study, ESCORT-2 (doi: 10.1111/nep.13849). The aim of this study is to assess the cost-effectiveness of incorporating this program into Thailand's public health benefit package, with the goal of establishing it as a model for similar rural areas in resource-limited settings. Method Following the Thailand's guideline for health technology assessment, a cost-utility analysis utilizing data from the ESCORT studies, local databases, and literatures. The integrated care program was tailored to primary healthcare system by using preexisting workforces. It involved a collaboration between Multi-Disciplinary Care Teams (MDCT) at district hospitals and Community Care Network (CCN) teams at Sub-District Health Centers (SDHC). The MDCT, comprising general practitioners, CKD nurse managers, pharmacists, a nutritionist, and a physical therapist, provided key patient education on diet, medication adherence, exercise, and avoiding certain medications and herbal remedies. The CCN teams, including community nurses and village health volunteers, conducted home visits for health monitoring and lifestyle guidance. Both MDCT and CCN personnel received training in CKD care based on the NKF-K/DOQI and KDIGO Clinical Practice Guidelines. A decision-analytic Markov model and Monte-Carlo simulation were applied to quantify and compare the lifetime costs and quality-adjusted life years (QALY) outcomes of the intervention and conventional-care from a societal perspective considering both direct and indirect costs. The analysis included extensive sensitivity analyses, incorporating a probabilistic model and scenario-analyses based on patient ages, CKD stages, and varying program efficacy. Additionally, a budget impact analysis from the payer's perspective assessed the financial impact and affordability over a five-year period. Results The integrated care program was found to dominate conventional care. The results indicated an incremental cost-effectiveness of (-)278,000 Thai Baht (∼7,267 EUR) per QALY for the base case analysis. Probabilistic analysis demonstrated consistent outcomes, with the integrated care program being cost-effective at almost 100% of the time during analysis at the 160,000 Thai Baht (∼4,182 EUR) willingness-to-pay threshold (Fig. 1). These results are driven by significant cost savings due to reduced time and overall costs in dialysis before death. The program is most cost-effective when provided early to patients, in terms of both age and CKD stages (Fig. 2). The budget impact analysis showed that introducing the intervention could save the payer 184 billion Thai Baht (4.76 billion EUR) over five years. (The exchange rate; 1 Thai Baht = 38.26 EUR) Conclusion The integrated care program for patients with stage 3-4 CKD is cost-effective and suitable for inclusion in Thailand's health benefit package, mainly due to its significant cost savings and enhanced health outcomes. The program's design, focusing on sustainability and feasibility by utilizing existing workforces, positions it as a viable model for similar economic and healthcare contexts, especially in resource-limited countries.

Abstract:Leukotriene B4 (LTB4,5-12-dihydroxy 6,8,10,14-eicosatetraenoic acid), an enzyme-catalyzed oxidation product of arachidonic acid, is a major inflammatory mediator. Human polymorphonuclear leukocytes and rodent hepatic microsomes catabolize LTB4 to 20-OH-LTB4 and 20-COOH-LTB4, which is mediated by a cytochrome P-450 catalyzed reaction termed the LTB4 omega-hydroxylase. In this study we investigated the catabolism of LTB4 in rat, guinea pig, and human epidermis. The incubation of 3H-LTB4 (9 microM) for 60 min in the presence of oxygen, NADPH, and epidermal microsomes prepared from neonatal fat (3.0 mg) or adult guinea pig (2.6 mg) resulted in the formation of 20-OH-LTB4 and 20-COOH-LTB4. Metabolite identification was based on co-chromatography on high pressure liquid chromatography with highly purified reference standards. The formation of 20-OH-LTB4 and 20-COOH-LTB4 was accompanied by the disappearance of LTB4. The rate of formation of 20-OH-LTB4 was 9-12-fold higher than that of 20-COOH-LTB4. Product formation was negligible with boiled microsomes, required NADPH and oxygen, was linear with respect to incubation time and protein, and was maximal at pH 7.4. LTB4-omega-hydroxylase activity was inhibited (greater than 90%) by carbon monoxide or 2-diethylaminoethyl-2,2-diphenylvalerate hydrochloride (SKF-525A) (1 mM), whereas alpha-naphthoflavone produced only moderate (13%) or no effects. Topical application of 3-methylcholanthrene and other conventional inducers of epidermal monooxygenase activities to neonatal rats (100 mg/kg, single treatment) did not result in an increase in epidermal LTB4-omega-hydroxylase activity. The addition of 3H-LTB4 (30 nmoles) to primary human keratinocytes followed by incubation at 37 degrees C resulted in time-dependent disappearance of LTB4 and appearance of 20-OH-LTB4 and 20-COOH-LTB4 in the medium. These results suggest that LTB4 is catabolized by the cytochrome P-450-dependent enzyme system in rodent and human skin and that this may participate in modulating the effects of this proinflammatory lipid in this tissue.

Abstract:The disease burden of chronic kidney disease (CKD) and its impact on healthcare systems has been poorly studied in Asia, a socioeconomically diverse region with wide variations in availability, access, and quality of CKD care. The high CKD burden in this region is predominantly driven by an increased prevalence of risk factors including diabetes mellitus, hypertension, obesity, and use of traditional medicines and is further aggravated by challenges associated with effective implementation of population-based screening and surveillance systems in early detection and intervention of CKD. The Asian continent mostly comprised of low- and middle-income countries with resource restraints lacks robust population-based CKD registries resulting in a paucity of data on CKD incidence and prevalence, various treatment modalities, uptake of current guidelines, and the overall impact of implementation of developmental programs. There is an urgent need for a collaborative action plan between the healthcare community and governments in this region to detect CKD in its early stages and prevent its complications including kidney failure, cardiovascular disease, and death. Research-based evidence on the impact of early detection, sustainable treatment options, quality of life, delay or avoidance of dialysis, and related cost analysis is the need of the hour. We highlight successful implementation of strategic and policy-sharing programs adopted in a few countries; also, consolidate available region-specific data, quantify estimates of CKD burden and propose strategies with a multidisciplinary approach involving patients, the healthcare community and governmental bodies to combat CKD and its complications.
Global Economic Burden Associated with Chronic Kidney Disease: A Pragmatic Review of Medical Costs for the Inside CKD Research Programme

Abstract:Introduction: Chronic kidney disease (CKD) is a progressive disease of growing prevalence, posing serious concerns for global public health. While the economic burden of CKD is substantial, data on the cost of CKD is limited, despite growing pressures on healthcare systems. In this review, we summarise the available evidence in 31 countries and regions and compile a library of costing methodology and estimates of CKD management and disease-associated complications across 31 countries/regions within the Inside CKD programme. Methods: We collected country/region-specific CKD costs via a pragmatic rapid literature review of local literature and engagement with local experts. We extracted cost data and definitions from identified sources for CKD stages G3a-5, kidney failure with replacement therapy by modality, covering haemodialysis, peritoneal dialysis, and kidney transplants, and disease-associated complications in local currency, converted to United States dollars (USD) and inflated to 2022. Results: Annual direct costs associated with CKD management rose by an average factor of 4 in each country/region upon progression from stage G3a to G5. Mean annual costs per patient increased considerably more from early stages versus dialysis (stage G3a, mean: $3060 versus haemodialysis, mean: $57,334; peritoneal dialysis, mean: $49,490); with estimates for annual costs of transplant also substantially higher (incident: $75,326; subsequent: $16,672). The mean annual per patient costs of complications were $18,294 for myocardial infarction, $8463 for heart failure, $10,168 for stroke and $5975 for acute kidney injury. Costing definitions varied widely in granularity and/or definition across all countries/regions. Conclusion: Globally, CKD carries a significant economic burden, which increases substantially with increasing disease severity. We identified significant gaps in published costs and inconsistent costing definitions. Cost-effective interventions that target primary prevention and disease progression are essential to reduce CKD burden. Our results can be used to guide cost collection and facilitate better comparisons across countries/regions to inform healthcare policy.

Abstract:CKD is a global problem that causes significant burden to the healthcare system and the economy in addition to its impact on morbidity and mortality of patients. Around the world, in both developing and developed economies, the nephrologists and governments face the challenges of the need to provide a quality and cost-effective kidney replacement therapy for CKD patients when their kidneys fail. In December 2019, the 3rd International Congress of Chinese Nephrologists was held in Nanjing, China, and in the meeting, a symposium and roundtable discussion on how to deal with this CKD burden was held with opinion leaders from countries and regions around the world, including Australia, Canada, China, Hong Kong, Singapore, Taiwan, the UK, and the USA. The participants concluded that an integrated approach with early detection of CKD, prompt treatment to slow down progression, promotion of home-based dialysis therapy like peritoneal dialysis and home HD, together with promotion of kidney transplantation, are possible effective ways to combat this ongoing worldwide challenge.

Abstract:Jayasinghe et al. [Science of the Total Environment, 705 (2020) 135766] propose a 'dynamical' model of Chronic Kidney Disease of Unknown etiology (CKDu) wherein CKDu arises as an emergent property of a complex system where they claim that weak multiple environmental factors contribute. They criticize the usual approaches as being "reductionist". We use their model as a basis of a discussion on the possibility of treating CKDu as an emergent property resulting from the interaction of multiple weak environmental factors with the organism. The model does not reveal anything beyond what is already known from simple considerations of well-known feed-back loops, but has the merit of re-stating those issues in a different format. If a proper weighting of the possible environmental factors is included, most proposed environmental factors drop out and what Jayasinghe et al. call the "reductionist" approach naturally arises due to the weight of evidence. The theory that the consumption of water containing fluoride and magnesium ions as found in water from regolith aquifers drawn via house-hold wells is found to clearly hold within this model when proper weighting is included. However, we show by examples that such models can be easily misused, leading to completely misleading conclusions. A response formalism useful in the theory of complex systems and emergent modes is presented in the context of the current problem. In addition to there being a lack of adequate data to fully implement such a theory, it is seen that such elaborations are unnecessary and misleading in the present context.

Abstract:Kidney disease of unknown aetiology (CKDu) has been identified in many countries extending from MesoAmerica and Egypt, to South-east Asia and China. Although CKDu has been linked by various authors to farming, it is an artifact of treating multi-modal disease distributions as unimodal. There is NO correlation of CKDu with agriculture since affected farming villages are often surrounded by other farming villages free of CKDu. Initial studies looked for a correlation of CKDu with toxic heavy metal residues of arsenic, cadmium etc., or herbicides like glyphosate that may be present in the environment, as the causative factors. There is now considerable consensus that their concentrations are below danger thresholds, be it in Mesoamerica or south-east Asia. The conceptual basis of a search for etiology within a systems approach is discussed, and attempts to name the disease to bias the identification of its etiology are reviewed. Current research has narrowed down the etiology to geochemical electrolytic contaminants like fluorides and ionic components in hard water, nanosilica (found in water as well as in the air), as well as renal toxins similar to indoxyl sulphates that may arise from interactions of ions with humic acids contained in aqueous organic matter. However, while agrochemical toxins are increasingly considered less relevant to the etiology of CKDu, it has become a firm public belief. In Sri Lanka this has spawned ideology-based agricultural policies for partial and complete banning of agrochemicals (2014-2021), followed by some back-tracking, disrupting the economy and the food supply. A farmer's uprising in 2022 was spawned by poor harvests. It triggered a larger popular uprising that led to the collapse of a government wedded to romanticized eco-extremist agricultural policies in a country already facing difficulties in the wake of Covid and Ukraine.

Abstract:Chronic kidney disease of unknown origin (CKDu) has emerged as a serious health risk for farming communities in remote semi-arid lowland regions of Sri Lanka, with geoenvironmental factors widely believed to be associated with the etiology. Although numerous case–control studies have been carried out to determine the causes of CKDu, none have been conducted in at-risk communities. The main objective of this study is to identify the potential geo-environmental risk factors associated with the at-risk community in the Wilgamuwa region, a CKDu endemic area in Sri Lanka. The study collected and analyzed 92 water sources currently or previously used in the study area for major cations, anions, and trace elements, mainly As, Cd, Pd, Mn, Zn, Ba and Sr. The major cations and anions in CKDu-susceptible groundwater varied Si 4+ > Ca 2+ > Na + > Mg 2+ > K + and HCO 3 − > Cl − > SO 4 2− > NO 3 − > F − > PO 4 3− > Br − . The higher average values of Si 4+ , F − and total hardness (TH) were reported in groundwater consumed by communities at risk of CKDu, with average values (in mg/L) of Si 4+ —46, F − —0.62, and TH—178. Considering the Water Quality Index (WQI) values, 73% of groundwater sources are classified as poor water quality, and the groundwater quality mainly depends on the fluoride distribution in the region. According to the Hazard Quotient (HQ), the risk of adverse health effects from exposure to F − and SiO 2 increased in magnitude in the order of male < female < children. Health hazards due to As, Cd and Pb exposure fell between the 'no-risk' and 'low-risk' categories. As outlined in this study, continuous monitoring of vulnerable communities for environmental exposures to key groundwater constituents is important in the dry zone of Sri Lanka.

Abstract:Chronic kidney disease (CKD) is a global public health concern and a key determinant of poor health outcomes. While the burden of CKD is reasonably well defined in developed countries, increasing evidence indicates that the CKD burden may be even greater in developing countries. Diabetes, hypertension, and obesity are major contributors to the global burden of the disease and are important traditional CKD risk factors; however, nontraditional CKD risk factors such as nephrotoxin exposure, kidney stones, fetal and maternal factors, infections, environmental factors, and acute kidney injury are also increasingly being recognized as major threats to global kidney health. A broad approach to CKD prevention begins with the identification of CKD risk factors in the population, followed by the development of appropriate mitigation strategies. Effective prevention policies rely on an accurate understanding of the incidence and prevalence of CKD in a given setting, as well as the distribution and burden of risk factors. Populations or individuals at CKD risk must be screened and treated early to prevent the onset of and delay the progression of the kidney disease. Systematically collected data should be analyzed at country, province, and district levels to identify regional disparities and CKD hotspots and develop targeted prevention strategies. Race-ethnicity, genetics, sex, socioeconomic status, and geography are likely modifiers of CKD risk. A comprehensive, informed approach to prevention that takes into account all of these factors is therefore required to successfully tackle the global CKD epidemic.

Abstract:Chronic kidney disease with uncertain etiology (CKDu) in Sri Lanka has attracted much attention as a global health issue. However, how environmental factors in local drinking water induce kidney damage in organisms is still elusive. We investigated multiple environmental factors including water hardness and fluoride (HF), heavy metals (HM), microcystin-LR (MC-LR), and their combined exposure (HFMM) to elucidate their toxic effects on CKDu risk in zebrafish. Acute exposure affected renal development and inhibited the fluorescence of Na, K-ATPase alpha1A4:GFP zebrafish kidney. Chronic exposure influenced the body weight of both genders of adult fish and induced kidney damage by histopathological analyses. Furthermore, the exposure significantly disturbed differential expression genes (DEGs), diversity and richness of gut microbiota, and critical metabolites related to renal functions. The transcriptomic analysis revealed that kidney-related DEGs were linked with renal cell carcinoma, proximal tubule bicarbonate reclamation, calcium signaling pathway, and HIF-1 signaling pathway. The significantly disrupted intestinal microbiota was closely related to the environmental factors and H&E score, which demonstrated the mechanisms of kidney risks. Notably, the Spearman correlation analysis indicated that the changed bacteria such as Pseudomonas, Paracoccus, and ZOR0006, etc were significantly connected to the DEGs and metabolites. Therefore, the assessment of multiple environmental factors provided new insights on "bio-markers" as potential therapies of the target signaling pathways, metabolites, and gut bacteria to monitor or protect residents from CKDu.

Abstract:Chronic kidney disease of unknown aetiology (CKDu) occurs in tropical countries, including Latin America, Mexico, Sri Lanka, and India. The disease occurs mainly in men who work in the agricultural sector. Although many studies have been conducted in CKDu areas, hardly any have examined emerging organic contaminants (EOCs) in drinking water. Therefore, this study examined EOCs, including pesticide residues, pharmaceuticals, and personal care products, in 29 shallow groundwater samples collected from wells commonly used by CKDu patients. These patients were identified through renal biopsy examinations from the Girandurukotte and Wilgamuwa regions in the dry zone of Sri Lanka. Of 110 EOCs measured, 40 were found in at least one or more samples. Among the detected EOCs, all groundwater samples examined showed the highest concentrations of caffeine (max: 950 ng/L) and DEET (max: 647 ng/L). In particular, the insect repellents commonly used in the region, such as coils, mats, vaporizers, spray and creams, would be the source of DEET in the environment. Pharmaceuticals and personal care products were the most common EOCs in all groundwater samples, with concentrations ranging from 129 (propylparaben) to 1.26 ng/L (carbamazepine) and 1.25 ng/L (hydrochlorothiazide), respectively, followed by herbicides and fungicides. Sources of such compounds would be insect repellents, antifungal and bactericidal preservatives, and agricultural pesticides. Notably, none of the identified EOCs reached critical values and were often below the values in European drinking water standards. Although primary evidence of EOCs as a cause of the disease does not emerge from the results of this study, regular monitoring of EOCs in groundwater, particularly in the dry zone of Sri Lanka, is essential as the presence of these compounds highlights their vulnerability in shallow groundwater wells.

Abstract:Background: Chronic kidney disease of unknown etiology (CKDu) is a tubulointerstitial disease that disproportionately affects young, primarily male, agricultural workers in Mesoamerica and South Asia who lack traditional risk factors for kidney disease. Extensive research in adult populations suggests the etiology is complex and indicates that early childhood exposures could have an integral role. Objectives: To identify, summarize, and compare research in children living in CKDu endemic areas or with relevant CKDu-related exposures. Methods: A systematic literature search was conducted in six databases for studies that report kidney health outcomes of pediatric populations living in proximity to CKDu-affected areas with no limitations on geography or study design. Studies were independently screened for inclusion and underwent quality assessment using the Appraisal Tool for Cross-Sectional Studies (AXIS) or the JBI Critical Appraisal Tool for Cohort Studies based on the study design by at least two authors. Data are compared narratively and graphically. Results: We included twenty peer-reviewed publications and two meeting abstracts from eight different countries. The most common study design involved cross-sectional analysis of biological specimens from children in an established CKDu endemic area. Marked decreases in estimated glomerular filtration rate were generally not identified and prevalence of albuminuria differed widely between countries. Novel urinary biomarkers frequently demonstrated subclinical kidney damage, although the specific biomarker(s) varied between studies. Epidemiologic factors associated with evidence of subclinical kidney damage in children included proximity to agriculture or agrichemicals. Despite heterogenous study outcomes, all studies concluded that there were signs of kidney injury in children living in CKDu endemic areas or with a CKDu-relevant environmental exposure. Conclusion: This systematic review suggests that the pathophysiologic process leading to CKDu may begin prior to adulthood. Future longitudinal research aimed at elucidating the multifaceted factors and exposures impacting entire communities, including children, is imperative for disease prevention strategies.

Abstract:Cholera has been a global pandemic in past centuries, and its persistent emergence and spread pose a significant public health challenge globally. Despite efforts to contain the disease, recurrent cholera outbreaks in sub-Saharan Africa remain a major health threat. This has attracted substantial research interest, raising questions about the effectiveness of prevention and control methods of cholera spread in sub-Saharan Africa. Addressing this health challenge by adopting a sustainable, convenient, and cost-effective intervention will improve the health, well-being, and productivity of vulnerable populations in sub-Saharan Africa. Household-level solutions, which are characterized by relatively low-cost and independence from potentially insufficient public water supply infrastructure were examined to determine their effectiveness in reducing the incidence of cholera if widely adopted across the continent. We perform a mixed-methods retrospective analysis on the Cholera epidemic data obtained from 2010 to 2016 in sub-Saharan Africa. Using an empirical epidemiological model, we estimate the performance efficacy of a suite of household water treatment (HWT) technologies. We also develop economic estimations to perform benefit–cost analyses to determine the cost effectiveness, convenience of use and durability of these products. We find that—if universally adopted—the HWT technologies evaluated here offer comparable and effective microbiological potential for eradicating cholera disease in sub-Saharan Africa but are potentially not affordable for low-income households that reside in cholera hotspots. As such, household subsidies are necessary in lowering barriers to economic access to these products. This finding provides substantial insights on the efficacy and affordability of these household water treatment technologies—insights which can inform stakeholder decisions on the applicability of this intervention in eradicating cholera.

Abstract:Chronic kidney disease of uncertain etiology (CKD-u) is an important public health issue in Sri Lanka and around the world, but published evidence of the progression of this disease is scanty. Our aim is to analyze the progression patterns and the associated risk factors of definite and probable CKD-u cases. This observational study was based on records of CKD-u cohort from 2005-14 at Girandurukotte, an endemic area for CKD-u in Uva Province, Sri Lanka. Data (rate of progression, survival, and risk factors) were analyzed using R statistical software. CKD-u cases (379) were included in analyses based on the adequacy of variables. Mean age was 53 years, male-to-female ratio of 2.5:1, and smoking were significant risk factors (P <0.10) for CKD-u progression. Males had 2.3 times hazard for CKD-u survival than females, and males who smoked had 2.57 times risk of CKD-u progression than nonsmoking males. Faster eGFR decline rate of >5 mL/min/1.73 m2/year have been identified in 25% of the sample (n = 100); this group is significantly younger than the slower progression group (mean age 46 years) and was at an early stage at the time of presentation (mean eGFR 76.02). CKD-u progression was not equal in all patients but faster in young people who presented at earlier stages. Continuous exposure to environmental risk factors may influence the rate of progression. Females have higher CKD-u survival rates than males. Tobacco smoking was associated with a lower survival of CKD-u but could be a proxy indicative of other exposures.

Abstract:Vlahos et al., Ref. 1, NPJ Clean water. 4, 50 (2021) have reported the presence of pesticide contamination above safe levels in a "single time-point analysis" of well water in a region in Sri Lanka where chronic kidney disease of unknown etiology (CKDu) is endemic. They conclude "that agrochemical use in paddy and other agricultural practices ... of the Green Revolution in Sri Lanka may now be contributing to ill health, rapid progression of disease, and mortality". The authors also propose "reducing ... agrochemical contaminants in Sri Lanka and other tropical countries to reduce ... CKDu". These conclusions, based on what they call a "single time-point analysis", tantamount to an identification of the etiology of CKDu are unsupported by the evidence presented by Vlahos et al. They do not satisfy, say, even the simplest of Bradford-Hill criteria for causation. In particular, (i) similar but non-persistent pesticide excesses have been detected sporadically in most parts of the country including where there is no CKDu; (ii) the pesticides reported in (1) cause both hepatotoxicity and nephrotoxicity; the latter with glomerular damage while CKDu is associated with tubulo-interstitial damage where no hepatotoxic symptoms have been reported; (iii) the pesticides detected have short half-lives and are used over short periods during farming; so the one time-point analysis is inadequate and misleading; (iv) farming communities that use pesticides in the same way but remain essentially without CKDu are found to exist adjacent to communities with CKDu; (v) the CKDu prevalence seems to correlate with local geomorphology but without correlation to agriculture which is practiced in most parts of the country. .

Abstract:Background: Chronic kidney disease (CKD) is a global concern that presents significant challenges for disease management. Several factors drive CKD prevalence, including primary risk factors, such as type 2 diabetes and hypertension, and an ageing population. Inside CKD is an international initiative that aims to raise awareness of the substantial burden incurred by CKD. Methods: Using a peer-reviewed microsimulation method, the clinical burden of CKD was estimated from 2022 to 2027. Demographic data from the Americas, Europe, and Asia-Pacific/Middle East were used to generate virtual populations and to project the prevalence of CKD, kidney replacement therapy, associated cardiovascular complications, comorbid conditions, and all-cause mortality in the CKD population over the modelled time frame. Findings: Across the 31 participating countries/regions, the total prevalence of CKD was projected to rise to 436.6 million cases by 2027 (an increase of 5.8% from 2022), with most cases (∼80%) undiagnosed. Inside CKD projected a mean of 8859 cases of heart failure, 10,244 of myocardial infarction, and 7797 of stroke per 100,000 patients with CKD by 2027. Interpretation: The clinical impact of CKD is substantial and likely to increase; the high prevalence of undiagnosed cases and associated complications may benefit from the implementation of health policy interventions that promote screening, earlier diagnosis, and interventions to improve outcomes.
