Abstract:Background: Former American football players have a higher prevalence of cognitive impairment than that of the US general population. It remains unknown what aspects of playing football are associated with neuropsychiatric outcomes. Hypothesis: It was hypothesized that seasons of professional football, playing position, and experience of concussions were associated with cognition-related quality of life (QOL) and indicators of depression and anxiety. Study design: Descriptive epidemiology study. Methods: The authors examined whether seasons of professional football, playing position, and experience of concussions, as measured by self-report of 10 symptoms, were associated with cognition-related QOL and indicators of depression and anxiety in a cross-sectional survey conducted 2015 to 2017. Cognition-related QOL was measured by the short form of the Quality of Life in Neurological Disorders: Applied Cognition-General Concerns. The Patient Health Questionnaire-4 measured depression and anxiety symptoms. Of 13,720 eligible men with apparently valid contact information, 3506 players returned a questionnaire at the time of this analysis (response rate = 25.6%). Results: Seasons of professional play (risk ratio [RR] per 5 seasons = 1.19, 95% CI = 1.06-1.34) and playing position were associated with cognition-related QOL. Each 5 seasons of play was associated with 9% increased risk of indicators of depression at borderline statistical significance (P = .05). When compared with former kickers, punters, and quarterbacks, men who played any other position had a higher risk of poor cognition-related QOL, depression, and anxiety. Concussion symptoms were strongly associated with poor cognition-related QOL (highest concussion quartile, RR = 22.3, P < .001), depression (highest quartile, RR = 6.0, P < .0001), and anxiety (highest quartile, RR = 6.4, P < .0001), even 20 years after last professional play. Conclusion: The data suggest that seasons of play and playing position in the NFL are associated with lasting neuropsychiatric health deficits. Additionally, poor cognition-related QOL, depression, and anxiety appear to be associated with concussion in the long term.
Modifiable Risk Factors for Poor Cognitive Function in Former American-Style Football Players: Findings from the Harvard Football Players Health Study

Abstract:Recent attention to consequences of head trauma among former professional American-style football players has increased the likelihood that former players and their healthcare providers attribute neurocognitive effects to these exposures. In addition to head trauma, however, many potentially modifiable risk factors are associated with cognitive impairment. We examined the association of self-reported risk factors for cognitive impairment (e.g., cardiovascular health, sleep, pain, depression, anxiety, smoking, physical impairment, and physical activity) with cognition-related quality of life, measured by the Quality of Life in Neurological Disorders, Applied Cognition-General Concerns (Neuro-QOL) among 3803 former National Football League (NFL) players. We examined the prevalence of risk factors among men who had experienced a high number of concussion symptoms during playing years, comparing men with good current cognition-related QOL, the "healthy concussed," to men with poor cognition-related QOL, the "unhealthy concussed." Physical functioning, pain, depression, and anxiety were very strongly associated with poor cognitive-related QOL (risk ratio range, 2.21-2.70, p < 0.0001 for all). Short sleep duration and low physical activity were also strongly associated (RR = 1.69 and 1.57, respectively, p < 0.0001 for both). The largest differences between healthy and unhealthy concussed were in chronic pain (72.0% vs. 21.2%), depressive symptoms (50.3% vs. 6.3%), anxiety symptoms (53.4% vs. 11.6%), and physical impairment (52.4% vs. 12.5%). Substantial differences also existed in prevalence of sleep apnea, short sleep duration, high-intensity exercise, weight training, high blood pressure, and body mass index ≥35 kg/m2 (all differences >10 percentage points). We identified cognitive risk factors, including chronic pain, mood problems, sleep problems, obesity, and lack of exercise, that were commonly present in former football players with cognition-related impairment. Better treatment for these factors may reduce cognitive problems in this population.

Abstract:Background: Patterns of cognitive impairment in former American football players are uncertain because objective neuropsychological data are lacking. This study characterized the neuropsychological test performance of former college and professional football players. Methods: One hundred seventy male former football players (n=111 professional, n=59 college; 45-74 years) completed a neuropsychological test battery. Raw scores were converted to T-scores using age, sex, and education-adjusted normative data. A T-score ≤ 35 defined impairment. A domain was impaired if 2+ scores fell in the impaired range except for the language and visuospatial domains due to the limited number of tests. Results: Most football players had subjective cognitive concerns. On testing, rates of impairments were greatest for memory (21.2% two tests impaired), especially for recall of unstructured (44.7%) versus structured verbal stimuli (18.8%); 51.8% had one test impaired. 7.1% evidenced impaired executive functions; however, 20.6% had impaired Trail Making Test B. 12.1% evidenced impairments in the attention, visual scanning, and psychomotor speed domain with frequent impairments on Trail Making Test A (18.8%). Other common impairments were on measures of language (i.e., Multilingual Naming Test [21.2%], Animal Fluency [17.1%]) and working memory (Number Span Backward [14.7%]). Impairments on our tasks of visuospatial functions were infrequent. Conclusions: In this sample of former football players (most of whom had subjective cognitive concerns), there were diffuse impairments on neuropsychological testing with verbal memory being the most frequently impaired domain.
Evaluation of Executive Function and Mental Health in Retired Contact Sport Athletes

Abstract:Objective: To compare retired professional contact sport athletes with age-matched noncontact sport athletes on measures of executive function and mental health. Setting: The University Concussion Management Clinic. Participants: Twenty-one retired National Football League (NFL) and National Hockey League (NHL) players (mean age 56 years) and 21 age-matched noncontact sport athlete controls. Designs: Case control. Main measure: The self- and informant-reported Behavior Rating Inventory of Executive Function-Adult form (BRIEF-A); Wisconsin Card Sorting Test; Delis-Kaplan Executive Function System; Trail Making Part A and B; Wechsler Adult Intelligence Scale; Neuropsychological Assessment Battery; List Learning; Controlled Oral Word Association Test; Beck Depression Inventory; Beck Anxiety Inventory; and Personality Inventory of the DSM-5. Results: Former NFL and NHL players perceived themselves to have some impairment in 2 of the 9 domains of executive function on the BRIEF-A; however, their informants reported no difference when compared with informants of noncontact athletes. No significant differences were found when comparing contact sport athletes with noncontact athletes on objective neuropsychological testing. Contact sport athletes qualified as clinically anxious and had more "unusual beliefs and experiences," although they remained within with age-based norms. Conclusion: Participation in contact sports at the professional level may not lead to later-life executive dysfunction, as the popular media and some research currently suggest.
Health-promoting behaviours and concussion history are associated with cognitive function, mood-related symptoms and emotional–behavioural dyscontrol in former NFL players: an NFL-LONG Study

Abstract:Objectives To examine the relationships among self-reported sport-related concussion (SRC) history and current health-promoting behaviours (exercise frequency, diet quality and sleep duration) with self-reported measures of brain health (cognitive function, symptoms of depression and anxiety and emotional–behavioural dyscontrol) in former NFL players. Methods In this cross-sectional study, a questionnaire was sent to former NFL players. Respondents reported SRC history (categorical: 0; 1–2; 3–5; 6–9; 10+ concussions), number of moderate-to-vigorous aerobic and resistance exercise sessions per week, diet quality (Rapid Eating Assessment for Participants—Shortened) and average nightly sleep duration. Outcomes were Patient-Reported Outcomes Measurement Information System Cognitive Function, Depression, and Anxiety, and Neuro-QoL Emotional-Behavioral Dyscontrol domain T-scores. Multivariable linear regression models were fit for each outcome with SRC history, exercise frequency, diet quality and sleep duration as explanatory variables alongside select covariates. Results Multivariable regression models (n=1784) explained approximately 33%–38% of the variance in each outcome. For all outcomes, SRC history (0.144≤|β|≤0.217) was associated with poorer functioning, while exercise frequency (0.064≤|β|≤0.088) and diet quality (0.057≤|β|≤0.086) were associated with better functioning. Sleeping under 6 hours per night (0.061≤|β|≤0.093) was associated with worse depressive symptoms, anxiety and emotional–behavioural dyscontrol. Conclusion Several variables appear to be associated with mood and perceived cognitive function in former NFL players. SRC history is non-modifiable in former athletes; however, the effects of increasing postplaying career exercise frequency, making dietary improvements, and obtaining adequate sleep represent important potential opportunities for preventative and therapeutic interventions.

Predictors and Correlates of Perceived Cognitive Decline in Retired Professional Rugby League Players

Abstract:Objective: Rugby league is an international full-contact sport, with frequent concussive injuries. Participation in other full-contact sports such as American football has been considered to be a risk factor for neuropsychiatric sequelae later-in-life, but little research has addressed the mental and cognitive health of retired professional rugby league players. We examined predictors and correlates of perceived (self-reported) cognitive decline in retired National Rugby League (NRL) players. Methods: Participants were 133 retired male elite level rugby league players in Australia. Participants completed clinical interviews, neuropsychological testing, and self-report measures. The Informant Questionnaire on Cognitive Decline in the Elderly, self-report (IQCODE-Self), measured perceived cognitive decline. Results: The median age of the sample was 55.0 (M = 53.1, SD = 13.9, range = 30-89) and the median years of education completed was 12.0 (M = 11.9, SD = 2.6, range = 7-18). The retired players reported a median of 15.0 total lifetime concussions (M = 28.0, SD = 36.6, range = 0-200). The mean IQCODE-Self score was 3.2 (SD = 0.5; Range = 1.3-5.0); 10/133 (7.5%) and 38/133 (28.6%) scored above conservative and liberal cutoffs for cognitive decline on the IQCODE-Self, respectively. Perceived cognitive decline was positively correlated with current depressive symptoms, negatively correlated with years of professional sport exposure and resilience, and unrelated to objective cognition and number of self-reported concussions. A multiple regression model with perceived cognitive decline regressed on age, concussion history, professional rugby league exposure, depression, resilience, objective cognitive functioning, daytime sleepiness, and pain severity showed depression as the only significant predictor. Conclusion: This is the first large study examining subjectively experienced cognitive decline in retired professional rugby league players. Similar to studies from the general population and specialty clinics, no relationship was found between objective cognitive test performance and perceived cognitive decline. Depressive symptoms emerged as the strongest predictor of perceived cognitive decline, suggesting that subjective reports of worsening cognition in retired elite rugby league players might reflect psychological distress rather than current cognitive impairment.
Medical Conditions in Former Professional American-Style Football Players Are Associated With Self-Reported Clinical Features of Traumatic Encephalopathy Syndrome

Abstract:Consensus criteria for traumatic encephalopathy syndrome (TES) specify that at least one core clinical feature of cognitive impairment (CI; e.g., difficulties with memory, executive function) or neurobehavioral dysregulation (ND; e.g., explosiveness, rage, and mood lability) be present and not fully accounted for by other health disorders. Associations between self-reported symptoms that mirror the core clinical features of TES-and how they may be related to concomitant medical conditions-remain unclear. The purpose of this study was to evaluate the association of medical conditions and football exposures with TES clinical features (CI+/-, ND+/-) in 1741 former professional American-style football (ASF) players (age, 57.7 ± 13.9 years; professional seasons, 6.6 ± 3.9 years). Demographics (age, race/ethnicity, current body mass index, age of first football exposure, use of performance-enhancing drugs, position played, and past concussion symptoms), self-reported medical conditions (anxiety, depression, attention-deficit hyperactivity disorder [ADHD], sleep apnea, headache, stroke, hypertension, heart disease, high cholesterol, erectile dysfunction, and low testosterone) were collected. Of 1741 participants, 7.4% were CI+ and/or ND+ (n = 129). Participants who were CI+ or ND+ were more likely to report one or more coexisting medical conditions than participants who did not report CI or ND (odds ratio [OR] = 2.04; 95% confidence interval: 1.25-3.47; p = 0.003). Separate general linear models for each medical condition that adjusted for demographics and football-related factors identified significant associations between ADHD, diabetes, erectile dysfunction, headaches, sleep apnea, anxiety, and low testosterone and CI+ and/or ND+ (ORs = 1.8-6.0). Chi-square automatic interaction detection (CHAID) multi-variable decision tree models that incorporated medical conditions and football exposures accurately differentiated former players meeting either CI or ND clinical criteria from those meeting none (accuracy = 91.2-96.6%). CHAID identified combinations of depression, headache, sleep apnea, ADHD, and upper quartiles of concussion symptom history as most predictive of CI+ and/or ND+ status. CI+ and/or ND+ players were more likely to report medical conditions known to cause cognitive symptoms. Concussion exposure and medical conditions significantly increased the likelihood that a former ASF player would demonstrate cognitive or neurobehavioral dysfunction. Clinicians engaged with this population should consider whether treatable coexisting condition(s) could account for some portion of the clinical picture associated with TES presentation.
Perceived Chronic Traumatic Encephalopathy and Suicidality in Former Professional Football Players

Abstract:Importance: Participation in American-style football (ASF) has been linked to chronic traumatic encephalopathy neuropathological change (CTE-NC), a specific neuropathologic finding that can only be established at autopsy. Despite being a postmortem diagnosis, living former ASF players may perceive themselves to have CTE-NC. At present, the proportion and clinical correlates of living former professional ASF athletes with perceived CTE who report suicidality are unknown. Objective: To determine the proportion, clinical correlates, and suicidality of living former professional ASF players with perceived CTE. Design, setting, and participants: A cross-sectional study within the Football Players Health Study at Harvard University was conducted from 2017 to 2020. Using electronic and paper surveys, this population-based study included former ASF players who contracted with a professional league from 1960 to 2020 and volunteered to fill out a baseline survey. Data for this study were analyzed from June 2023 through March 2024. Exposures: Data included demographics, football-related exposures (eg, position, career duration), and current health problems (anxiety, attention-deficit/hyperactivity disorder, depression, diabetes, emotional and behavioral dyscontrol symptoms, headache, hyperlipidemia, hypertension, low testosterone level, pain, sleep apnea, and subjective cognitive function). Main outcomes and measures: The proportion of participants reporting perceived CTE. Univariable and multivariable models were used to determine clinical and suicidality correlates of perceived CTE. Results: Among 4180 former professional ASF players who volunteered to fill out a baseline survey, 1980 (47.4%) provided follow-up data (mean [SD] age, 57.7 [13.9] years). A total of 681 participants (34.4%) reported perceived CTE. Subjective cognitive difficulties, low testosterone level, headache, concussion signs and symptoms accrued during playing years, depressive/emotional and behavioral dyscontrol symptoms, pain, and younger age were significantly associated with perceived CTE. Suicidality was reported by 171 of 681 participants with perceived CTE (25.4%) and 64 of 1299 without perceived CTE (5.0%). After adjusting for established suicidality predictors (eg, depression), men with perceived CTE remained twice as likely to report suicidality (odds ratio, 2.06; 95% CI, 1.36-3.12; P < .001). Conclusions and relevance: This study found that approximately one-third of living former professional ASF players reported perceived CTE. Men with perceived CTE had an increased prevalence of suicidality and were more likely to have health problems associated with cognitive impairment compared with men without perceived CTE. Perceived CTE represents a novel risk factor for suicidality and, if present, should motivate the diagnostic assessment and treatment of medical and behavioral conditions that may be misattributed to CTE-NC.

Abstract:Abstract Purpose Current understanding of potential long-term negative cognitive and psychiatric outcomes following head-injury exposure is limited, and this study aimed to evaluate these relationships in former collegiate football players. Method Former collegiate football players completed the College Level Aging Athlete Study (CLEAATS) survey including playing career, concussion/medical/psychiatric history, self-reported cognitive functioning, and measures of depression, anxiety symptoms, positive affect/resilience, emotional dysregulation, and objective cognitive function. A head-injury exposure estimate (HIEE, modified from Kerr et al., 2015) was calculated to determine total sports-related head-injury exposure. Partial correlations (controlling for age and education) examined the relationship between HIEE and cognitive functioning and emotional/mood symptoms. Results Participants (n = 216) were 50–87 years old (M = 63.4 [8.5]), 91% White, and well-educated (bachelor’s or graduate degree = 92%). HIEE (M = 763.8 [342.9]) was not correlated with any cognitive or emotional/mood symptom measures (p’s > 0.075). Depression (r = 0.65), anxiety (r = 0.48), and emotional dysregulation indices (r = 0.50) showed moderate positive correlations with subjective cognitive symptoms. Grit (r = −0.36) and positive affect/well-being (r = −0.49) were negatively correlated with cognitive symptom reporting. Conclusions HIEE was not found to be related to cognitive or emotional/mood outcomes, and results suggest that current emotional/mood symptoms have greater associations with subjective cognitive symptoms. Alternatively, positive affect/resilience were found to be negatively associated with cognitive symptoms, and may serve as protective factors against cognitive complaints. These findings are consistent with previous studies that have reported associations between cognitive symptoms, psychological distress, and resilience in former athletes, highlighting the importance of considering potential impact of current emotional/mood symptoms when evaluating retired players with cognitive concerns.

Abstract:BackgroundPotential links between a history of sport-related concussions and later-life neurobiological and psychological brain health have been studied in former collision-sport athletes. However, empirical studies of how former athletes perceive the future of their brain health as a result of these injuries are missing.ObjectivesWe aimed to (1) identify the extent to which former National Football League players currently have concerns about their long-term psychological and cognitive functioning as a result of concussions sustained while playing football; (2) examine whether current concerns are different than concerns they had while playing football; (3) examine the relationship between current brain health concerns and self-reported concussion history (SR-CHx); and (4) explore other important factors associated with these concerns.MethodsIn this cross-sectional study, former National Football League players with a SR-CHx of one or more concussions (n = 1514; aged mean [standard deviation] = 52.3 [15.7] years) completed a general health questionnaire. Participants reported their lifetime concussion history, as well as both their current concerns and concerns while playing football (i.e., retrospective concerns) regarding the long-term effects of concussions on their memory, thinking skills, and risk of developing chronic traumatic encephalopathy. Current and retrospective concerns were self-reported on a five-point Likert scale. Four concussion history categories were created based on SR-CHx: 1–2 (n = 309); 3–5 (n = 413); 6–9 (n = 356); and 10 + (n = 436) lifetime concussions. Proportions of participants reporting each level of current and retrospective concerns were examined to identify whether concerns presently exist in these former players, and whether their current concerns are different than retrospective concerns. Next, we explored associations between current concerns and SR-CHx.ResultsMore than one-third of participants reported being currently “extremely concerned” about memory problems (36.9%), thinking skills (37.8%), and developing chronic traumatic encephalopathy (39.5%). In contrast, when asked about concerns while playing, most reported being “not at all concerned” regarding memory = 61.2%, thinking skills = 56.1%, and developing chronic traumatic encephalopathy = 71.2%. Of those who retrospectively endorsed being “not at all” or “slightly” concerned regarding memory (n = 1159/1514), thinking skills (n = 1080/1514), and developing chronic traumatic encephalopathy (n = 1219/1514), approximately half reported being currently “moderately” or “extremely” concerned about those same issues (n = 586/1159; n = 534/1080; n = 619/1219, respectively). Current concerns regarding memory (χ216 = 316.61; p < 0.001; V = 0.264), thinking skills (χ216 = 333.17; p < 0.001; V = 0.271), and developing chronic traumatic encephalopathy (χ216 = 280.85; p < 0.001; V = 0.249) were significantly related to SR-CHx, with more concussions being associated with greater current concerns.ConclusionsFormer National Football League players reported significant concerns regarding the potential effects of their prior concussions on long-term brain health, and these concerns are more prevalent now than when they were playing football. Cognitive and mental health concerns are readily identifiable targets for clinical intervention. Clinicians working with former players may wish to explore the extent to which individual players experience these concerns, the nature and depth of these concerns, and the impact of these concerns on the player’s functioning and well-being.

Abstract:More than 1 million students play high school American football annually, but many health professionals have recently questioned its safety or called for its ban. These concerns have been partially driven by reports of chronic traumatic encephalopathy (CTE), increased risks of neurodegenerative disease, and associations between concussion history and later-life cognitive impairment and depression among retired professional football players. A recent observational study of a cohort of men who graduated from a Wisconsin high school in 1957 found no statistically significant harmful effects of playing high school football on a range of cognitive, psychological, and socio-economic outcomes measured at ages 35, 54, 65, and 72. Unfortunately, these findings may not generalize to younger populations, thanks to changes and improvements in football helmet technology and training techniques. In particular, these changes may have led to increased perceptions of safety but ultimately more dangerous styles of play, characterized by the frequent sub-concussive impacts thought to be associated with later-life neurological decline. In this work, we replicate the methodology of that earlier matched observational study using data from the National Longitudinal Study of Adolescent to Adult Health (Add Health). These include adolescent and family co-morbidities, academic experience, self-reported levels of general health and physical activity, and the score on the Add Health Picture Vocabulary Test. Our primary outcome is the CES-D score measured in 2008 when subjects were aged 24 -- 34 and settling into early adulthood. We also examine several secondary outcomes related to physical and psychological health, including suicidality. Our results can provide insight into the natural history of potential football-related decline and dysfunction.

Abstract:Abstract Purpose Long-term concussion sequelae have been increasingly studied in former professional athletes, and less is known about those who participated in sport at the collegiate level. This study explores the relationship between concussion history and cognition/mood in former collegiate athletes in middle-to-later adulthood. Method 422 former collegiate athletes over age 50 (M = 61.5; 62% male) completed the College Level Aging Athlete Study (CLEAATS) including the Cognitive Function Instrument (CFI), Telephone Interview for Cognitive Status-Modified (TICSm), and self-report questionnaires including concussion history and current mood. Pearson correlations and multiple regression analyses examined the relationship between concussion history, subjective and objective cognitive status, and mood. Results 54% of the sample reported a history of concussion (M = 1.2, SD = 1.7). Number of concussions was marginally associated with subjective cognitive complaints (r = 0.13, p = 0.004) and depression (r = 0.14, p = 0.002), but not objective cognition (p = 0.39) or anxiety (p = 0.33). Regression models including demographic factors and mood were significant and explained 32% and 17% of the variance in subjective and objective cognition, respectively. Concussion history was not a significant predictor in either model, whereas depression (Β = -0.12) was associated with poorer subjective cognition only. Conclusions Concussion history was not associated with subjective or objective cognitive changes in former collegiate athletes. Although a very small relationship between concussion history and depression was found, depression was associated with more subjective cognitive complaints. This suggests mood symptoms have greater impact on subjective cognitive status than concussion history. These findings highlight the importance of considering the effect of current mood symptoms when evaluating former athletes who present with cognitive complaints.

Abstract:Background and Aims: Little is known about whether Helicobacter pylori infection alters fatty acid metabolism in gastric mucosal cells. By using cultured rat gastric mucosal cells (RGM-1), we investigated the effect of H. pylori broth culture filtrates on this point. Furthermore, our study aimed to find out whether n-6 long chain polyunsaturated fatty acids from linoleic acid are formed in RGM-1 cells. Methods: Rat gastric mucosal cells were incubated with 10, 20 and 40 mg/mL of linoleic acid or medium alone. Phosphatidylcholine content extracted from whole RGM-1 cells was quantitated by using a densitometer, and its fatty acid composition was analyzed by using gas chromatography. Prostaglandin E2 concentration in the culture medium was measured by using radioimmunoassay. The expression of cyclooxygenase (COX)-1 and COX-2 was examined by using reverse transcription–polymerase chain reaction. In addition, after incubation with [1-C] linoleic acid, radioactivities of both linoleic acid and arachidonic acid components of the PC fraction were counted.The effects of H. pylori broth culture filtrates on PC content, its fatty acid composition and prostaglandin (PG)E2 synthesis were also assessed. Results: Linoleic acid addition caused an increase in the composition of arachidonic acid, as well as linoleic acid, and also in PGE2 concentration. Cyclo-oxygenase-2 expression was induced in RGM-1 cells by the addition of linoleic acid. In addition, [1-C] linoleic acid added to the culture medium was converted to [1-C] arachidonic acid in RGM-1 cells. Helicobacter pylori broth culture filtrates decreased linoleic acid composition and increased arachidonic acid composition. Moreover, after incubation with H. pylori broth culture filtrates, PGE2 concentrations were higher than that of the controls. Conclusions: These findings suggest the presence of fatty acid elongase and Dand D-desaturases synthesize arachidonic acid from linoleic acid in RGM-1 cells. Thus, H. pylori infection may enhance PGE2 synthesis and accelerate n-6 fatty acid metabolism in gastric mucosal cells, which could make the gastric mucosal barrier more fragile. © 2001 Blackwell Science Asia Pty Ltd

Abstract:Objective: Neurobehavioral dysregulation (NBD), a core clinical feature of traumatic encephalopathy syndrome, encompasses neuropsychiatric symptoms reported among individuals with a history of repetitive head impact exposure, including contact sport athletes. The objective of this study was to examine the construct and subconstructs of NBD through a series of factor and cluster analyses. Methods: Six clinician-scientists selected self-report questionnaire items relevant to NBD from seven available neuropsychiatric scales through a blinded voting process. These items were subjected to confirmatory factor analyses in a sample of 178 former college and professional American football players and 60 asymptomatic individuals without a history of repetitive head impact exposure. All participants were enrolled in the Diagnostics, Imaging, and Genetics Network for the Objective Study and Evaluation of Chronic Traumatic Encephalopathy Research Project. Factor scores were generated on the basis of the optimal expert-informed model for NBD. Construct validity was assessed with neuropsychiatric scales not included in generation of the factor scores. Cluster analyses with NBD factor scores were used to examine symptom profiles. Results: Factor analyses confirmed that NBD was composed of four subconstructs: explosivity, emotional dyscontrol, impulsivity, and affective lability. Cluster analyses indicated four distinct symptom profiles of NBD in this group of former football players: asymptomatic (N=80, 45%), short fuse (N=33, 19%), high affective lability (N=34, 19%), and high NBD (N=31, 17%). Conclusions: These findings characterize NBD as a multifaceted clinical construct with a heterogeneous presentation, providing a foundation for empirical work on the diagnostic criteria for traumatic encephalopathy syndrome and research on the neurobiological underpinnings of NBD.

Abstract:Abstract Purpose This study assessed whether age of first exposure (AFE) to football was associated with later-life psychiatric and neurobehavioral symptoms, cognitive difficulties, and general health problems in a cohort of former amateur football players. Method A cross-sectional survey study of men with a history of amateur football exposure was conducted using the ResearchMatch online platform. Independent variables included demographics, number of lifetime concussions, AFE to football (dichotomized as AFE < 12 years vs. AFE ≥ 12 years), and total years of football participation. Main outcomes included current depressive (Patient Health Questionnaire-9), anxiety (Generalized Anxiety Disorders-7), cognitive (British Columbia Cognitive Complaints Inventory), and neurobehavioral symptoms (Neurobehavioral Symptom Inventory). T-tests (AFE < 12 vs. AFE ≥ 12) and multivariable regressions assessed for associations between AFE and outcome variables, adjusting for age, number of prior concussions, and years of football exposure. Results A total of 107 male participants (mean age = 60.6 ± 15.1 years) reported an average of 4.2 ± 2.7 years of exposure to amateur football, with 41 participants reporting AFE < 12 (38.3%). In multivariable analyses, AFE < 12 was not a significant predictor of depressive (B = 0.51, SE = 1.25, p = 0.682), anxiety (B = 0.09, SE = 0.95, p = 0.926), cognitive (B = -0.65, SE = 0.77, p = 0.403), or neurobehavioral symptom scores (B = -0.56, SE = 2.93, p = 0.850). However, having more prior concussions were associated with worse depressive (B = 0.44, SE = 0.10, p < 0.001), anxiety (B = 0.33, SE = 0.07, p < 0.001), cognitive (B = 0.26, SE = 0.06, p < 0.001), and neurobehavioral symptoms (B = 1.04, SE = 0.23, p < 0.001). Conclusions AFE to football was not associated with adverse psychiatric, cognitive, or neurobehavioral difficulties. However, these outcomes were associated with a greater number of lifetime concussions.

Abstract:A potential causal relationship between head injuries sustained by NFL players and later-life neurological decline may have broad implications for participants in youth and high school football programs. However, brain trauma risk at the professional level may be different than that at the youth and high school levels and the long-term effects of participation at these levels is as-yet unclear. To investigate the effect of playing high school football on later life depression and cognitive functioning, we propose a retrospective observational study using data from the Wisconsin Longitudinal Study (WLS) of graduates from Wisconsin high schools in 1957. We compare 1,153 high school males who played varsity football to 2,751 male students who did not. 1,951 of the control subjects did not play any sport and the remaining 800 controls played a non-contact sport. We focus on two primary outcomes measured at age 65: a composite cognitive outcome measuring verbal fluency and memory and the modified CES-D depression score. To control for potential confounders we adjust for pre-exposure covariates such as IQ with matching and model-based covariate adjustment. We will conduct an ordered testing procedure that uses all 2,751 controls while controlling for possible unmeasured differences between students who played sports and those who did not. We will quantitatively assess the sensitivity of the results to potential unmeasured confounding. The study will also consider several secondary outcomes of clinical interest such as aggression and heavy drinking. The rich set of pre-exposure variables, relatively unbiased sampling, and longitudinal nature of the WLS dataset make the proposed analysis unique among related studies that rely primarily on convenience samples of football players with reported neurological symptoms.

Abstract:Importance: Players of American football may be at increased risk of long-term neurological conditions, particularly chronic traumatic encephalopathy (CTE).Objective: To determine the neuropathological and clinical features of deceased football players with CTE.Design, Setting, and Participants: Case series of 202 football players whose brains were donated for research. Neuropathological evaluations and retrospective telephone clinical assessments (including head trauma history) with informants were performed blinded. Online questionnaires ascertained athletic and military history.Exposures: Participation in American football at any level of play.Main Outcomes and Measures: Neuropathological diagnoses of neurodegenerative diseases, including CTE, based on defined diagnostic criteria; CTE neuropathological severity (stages I to IV or dichotomized into mild [stages I and II] and severe [stages III and IV]); informant-reported athletic history and, for players who died in 2014 or later, clinical presentation, including behavior, mood, and cognitive symptoms and dementia.Results: Among 202 deceased former football players (median age at death, 66 years [interquartile range, 47-76 years]), CTE was neuropathologically diagnosed in 177 players (87%; median age at death, 67 years [interquartile range, 52-77 years]; mean years of football participation, 15.1 [SD, 5.2]), including 0 of 2 pre-high school, 3 of 14 high school (21%), 48 of 53 college (91%), 9 of 14 semiprofessional (64%), 7 of 8 Canadian Football League (88%), and 110 of 111 National Football League (99%) players. Neuropathological severity of CTE was distributed across the highest level of play, with all 3 former high school players having mild pathology and the majority of former college (27 [56%]), semiprofessional (5 [56%]), and professional (101 [86%]) players having severe pathology. Among 27 participants with mild CTE pathology, 26 (96%) had behavioral or mood symptoms or both, 23 (85%) had cognitive symptoms, and 9 (33%) had signs of dementia. Among 84 participants with severe CTE pathology, 75 (89%) had behavioral or mood symptoms or both, 80 (95%) had cognitive symptoms, and 71 (85%) had signs of dementia.Conclusions and Relevance: In a convenience sample of deceased football players who donated their brains for research, a high proportion had neuropathological evidence of CTE, suggesting that CTE may be related to prior participation in football.

Abstract:Background: Mental health disorders are linked to prolonged concussion symptoms. However, the association of premorbid anxiety/depression symptoms with postconcussion return-to-play timelines and total symptom burden is unclear. Objective: To examine the association of self-reported premorbid anxiety/depression symptoms in collegiate student-athletes with (1) recovery times until asymptomatic, (2) return-to-play, and (3) postconcussion symptom burden. Study design: Athletes in the Concussion Assessment, Research and Education Consortium completed baseline concussion assessments (Sport Concussion Assessment Tool [SCAT3] and Brief Symptom Inventory-18 [BSI-18]). Athletes were tested postinjury at <6 hours, 24 to 48 hours, time of asymptomatic and start of return-to-play protocol, unrestricted return-to-play, and 6 months after injury. Injured athletes were categorized into 4 groups based on BSI-18 scores: (1) B-ANX, elevated anxiety symptoms only; (2) B-DEP, elevated depression symptoms only; (3) B-ANX&DEP, elevated anxiety and depression symptoms; and (4) B-NEITHER, no elevated anxiety or depression symptoms. Relationship between age, sex, BSI-18 group, SCAT3 total symptom and severity scores, and time to asymptomatic status and return-to-play was assessed with Pearson's chi-squared test and robust analysis of variance. Level of evidence: Level 3. Results: Among 1329 athletes with 1352 concussions, no respondents had a self-reported premorbid diagnosis of anxiety/depression. There was no difference in time until asymptomatic or time until return-to-play between BSI-18 groups (P = 0.15 and P = 0.11, respectively). B-ANX, B-DEP, and B-ANX&DEP groups did not have higher total symptom or severity scores postinjury compared with the B-NEITHER group. Conclusion: Baseline anxiety/depression symptoms in collegiate student-athletes without a mental health diagnosis are not associated with longer recovery times until asymptomatic, longer time to return-to-play, or higher postconcussion total symptom and severity scores compared with athletes without baseline symptoms. Clinical relevance: Anxiety and depression symptoms without a clear mental health diagnosis should be considered differently from other comorbidities when discussing prolonged recovery in collegiate student-athletes.

Abstract:The results of thirty-one Manchester knee arthroplasties performed on twenty-eight patients are reviewed. There were sixteen patients with rheumatoid arthritis all of whom were satisfactory at the time of follow-up. Of the fifteen patients with osteoarthritis over half the arthroplasties failed after between twelve and thirty-six months. We suggest that resurfacing arthroplasty of the knee using the Manchester prosthesis should be used only for rheumatoid patients.
Career duration and later-life health conditions among former professional American-style football players

Abstract:Objectives: Career duration is often used as a metric of neurotrauma exposure in studies of elite athletes. However, as a proxy metric, career length may not accurately represent causal factors, and associations with health outcomes may be susceptible to selection effects. To date, relationships between professional American-style football (ASF) career length and postcareer health remain incompletely characterised. Methods: We conducted a survey-based cross-sectional cohort study of former professional ASF players. Flexible regression methods measured associations between self-reported career duration and four self-reported health conditions: pain, arthritis, mood and cognitive symptoms. We also measured associations between career duration and four self-reported ASF exposures: prior concussion signs and symptoms (CSS), performance enhancing drugs, intracareer surgeries and average snaps per game. Models were adjusted for age and race. Results: Among 4189 former players (52±14 years of age, 39% black, 34% lineman position), the average career length was 6.7±3.9 professional seasons (range=1-20+). We observed inverted U-shaped relationships between career duration and outcomes (all p<0.001), indicating that adverse health effects were more common among men with intermediate career durations than those with shorter or longer careers. Similar findings were observed for play-related exposures (eg, CSS and snaps). Conclusions: Relationships between ASF career duration and subsequent health status are non-linear. Attenuation of the associations among longer career players may reflect selection effects and suggest career length may serve as a poor proxy for true causal factors. Findings highlight the need for cautious use of career duration as a proxy exposure metric in studies of former athletes.
